目的 探讨以持续性眩晕为表现的良性阵发性位置性眩晕(benign paroxysmal positional vertigo,BPPV)患者的诊治。方法 回顾性分析2例以持续性眩晕为表现的BPPV患者的临床资料。结果 此2例患者在坐位及平躺位见方向向患侧的水平自发持续性眼震,甩头试验健侧阳性,平卧侧头试验双侧均诱发出水平离地性眼震,平躺后头部向患侧连续转360°时分别出现2个眼震消失点和2个眼震最强点,给予手法复位后患者眩晕症状缓解。结论 水平半规管BPPV患者偶可表现为持续性眩晕发作,其病因为壶腹嵴帽耳石症,临床表现与耳石重力因素和水平半规管空间位置相关。  相似文献

目的探讨良性阵发性位置性眩晕(Benign Paroxysmal Positional Vertigo,BPPV)患者Dix-Hallpike诱发试验双侧阳性的眼震特点和诊治策略。方法回顾性分析2018年1月至2019年12月就诊于安徽医科大学第二附属医院耳鼻喉科的BPPV患者的诊治过程。结果 Dix-Hallpike诱发试验,阳性患者150例,其中双侧阳性患者14例,2例双侧可见扭转、向上、向地性眼震,进一步行平躺试验为垂直向上性眼震,分次给予双侧改良Epley法复位后双侧眼震均消失;12例患侧为扭转、向上、向地性眼震,健侧为扭转、向上、离地性眼震者,给予患侧改良Epley法复位后双侧眼震均消失。结论除了双侧后半规管BPPV,部分单侧后半规管BPPV患者行Dix-Hallpike诱发试验也可表现为双侧阳性结果,临床诊治应注意眼震方向形式的差别。  相似文献

目的探讨甲磺酸倍他司汀对良性阵发性位置性眩晕(BPPV)位置试验眼震的影响。方法2022年1~6月就诊的头晕患者并且予BPPV位置试验检查,其中61例检查前24h内仅口服过甲磺酸倍他司汀治疗,199例检查前24h内口服其他药物或者多种药物联合治疗。观察其停药前、后BPPV位置试验眼震变化情况:检查前24h内有用药史者,先予第1次BPPV位置试验,记录眼震情况;若第1次位置试验为阴性,嘱其停药24h后再予第2次BPPV位置试验,记录眼震情况;若第1次位置试验为阳性,予手法复位治疗,1h后复查,若复位效果欠佳,嘱其停药24h后再予第2次BPPV位置试验,记录眼震情况。位置试验眼震均通过视频眼罩观察。结果检查前24h内仅口服过甲磺酸倍他司汀的患者中,第1次BPPV位置试验眼震阳性者占16.39%(10/61),停药24h后第2次BPPV位置试验眼震阳性者占49.02%(25/51),停药前、后BPPV位置试验眼震阳性率有显著统计学差异(x2=8.89,P=0.003)。第1次位置试验眼震阳性者中,有5例(50%)在停药24h后第2次位置试验中眼震强弱程度发生变化,导致定位患侧半规管发生变化:2例左水平半规管BPPV修正为右水平半规管BPPV;1例左水平半规管帽型BPPV修正为右水平半规管帽型BPPV;2例水平半规管帽型BPPV,从第1次位置试验无法根据眼震强弱程度辨别患侧,到第2次位置试验可以根据眼震强弱程度明确患侧。结论甲磺酸倍他司汀对BPPV位置试验的精确性有一定影响,主要表现为抑制患者发病时原有的眼震,建议停药24h后再做位置试验,有助于BPPV精准诊疗。  相似文献

目的探讨水平半规管良性阵发性位置性眩晕(BPPV)的诊断。方法自2003年1月至2006年9月,本眩晕中心共确诊为水平半规管BPPV(HSC BPPV)患者46例。诊断依据为典型的病史及用红外视频眼震电图仪记录患者在Dix—Hallpike试验和滚转试验中的眼震结果,并排除其他疾病。研究HSC BPPV的眼震特点。结果441例诊断为BPPV患者中,46例(10.43%)为水平半规管BPPV。其中38例(82.61%)为单侧病变,5例(10.87%)患者合并同侧后半规管病变,3例(6.52%)为双侧病变。35例患者通过两种试验诱发出水平眼震,11例患者仅通过滚转试验诱发出水平眼震。25例患者眼震方向向地,13例患者眼震方向背地,3例患者眼震方向不固定,5例患者各种手法诱发出同一方向的眼震。29例患者在双侧手法中出现眼震,17例患者在一侧手法中出现眼震。结论HSC BPPV眼震为完全水平性且多为快相向地。通常受累耳在双侧手法中均可出现眼震,以向患侧为重。一侧水平半规管和后半规管可同时受累。滚转试验在HSC BPPV检查中比Dix—Hallpike试验更加敏感。Dix—Hallpike试验结合滚转试验可使更多的HSCBPPV患者得到确诊.  相似文献

目的:探讨视频眼震图(VNG)在良性阵发性位置性眩晕(BPPV)中的应用价值。方法:回顾126例BPPV患者的VNG资料,分析总结各型BPPV在Dix-Hallpike和滚转试验中VNG上的眼震特点。结果:126例BPPV患者中,后半规管BPPV(PSC-BPPV)98例(77.8%),水平半规管BPPV(HSC-BPPV)17例(13.5%),前半规管BPPV(ASC-BPPV)5例(3.9%),混合型BPPV6例(4.8%);28例PSC-BPPV记录到反转相眼震。VNG上显示PSC和ASC管石症Dix-Hallpike悬头位垂直相眼震分别向上、向下,水平相眼震均向对侧,回到坐位时眼震反向。HSC-BPPV滚转试验向两侧转头均可诱发出眼震,眼震与转头方向相同时,可判断为HSC管石症,以能够诱发较强眼震的转头侧为患侧;眼震与转头方向相反时,则为HSC嵴顶结石症,以能够诱发较弱眼震的转头侧为患侧。结论:VNG能够客观地记录BPPV患者的眼震情况,准确判断耳石所在的半规管,并且保存了眼震数据资料,可以进一步指导临床实践,值得推广。  相似文献

目的 探讨水平半规管良性阵发性位置性眩晕(benign paroxysmal positional vertigo,BPPV)的临床特点.方法 回顾性分析56例水平半规管BPPV患者的临床表现及眼震电图特征,通过体位试验确诊BPPV后,水平向地性眼震者以Barbecue翻滚法复位治疗,水平背地性眼震者经左右侧头训练后再同法复位,并限制体位.结果 56例患者中,平卧侧头试验见水平向地性眼震者41例,潜伏期为2.5±1.3s,其中5例患者无明显潜伏期,眼震持续时间22.9±16.4s,2例持续时间≥60 s;诱发出水平背地性眼震者15例,潜伏期为3.7±3.1s,眼震持续时间67.2±17.7s,其中2例持续时间≤60 s.41例水平向地性眼震者中39例1次复位成功,首次复位治疗成功率为95.12%(39/41),其中2例眼震持续时间>60 s的患者均一次复位成功;有2例复位不成功.15例水平背地性眼震患者中,2例在首次左右侧头训练时即见眼震转化为水平向地,3例在2~4次随访时转化为水平向地;眼震转化后再行平卧侧头试验表现为,转向起初眼震为水平背地较弱侧时,其水平向地性眼震更明显,且眼震速度明显增快、潜伏期明显缩短.结论 在水平半规管BPPV患者的复位治疗过程中,翻滚复位法结合左右侧头训练及体位限制有助于复位成功.  相似文献

目的 探讨水平半规管良性阵发性位置性眩晕(BPPV)眼震的方向、强度、时间等参数的客观特征及机制.方法 水平半规管BPPV患者233例,其中水平半规管管石症(horizontal semicircular canalithasis,HSC-Can) 179例,水平半规管壶腹嵴顶结石症(horizontal semicircular canal cupulolithiasis,HSC-Cup)54例.应用视频眼震图仪分别记录滚转试验诱发眼震,比较各个试验头位的眼震方向、强度、时间等参数特点.采用SPSS17.0统计软件进行数据处理.结果 水平半规管BPPV患者滚转试验在左侧、右侧转头位均可诱发出水平性眼震.HSC-Can诱发眼震方向与转头方向相同,向患侧和健侧转头位诱发眼震的潜伏期(-x±s,下同)为(1.922±1.501)s和(1.447±0.855)s,持续时间为(25.620±10.409)s和(22.110±10.931)s,强度为(56.441±33.168)°/s和(24.239±13.892)°/s,向患侧转头诱发眼震的潜伏期、持续时间及强度均大于健侧,差异均具有统计学意义(t值分别为3.715、15.219和4.070,P值均<0.01),其中眼震强度之比约为2∶1.HSC-Cup诱发眼震方向与转头方向相反,向健侧转头诱发眼震的强度明显大于向患侧转头,其强度之比约为2∶1,差异具有统计学意义(t=-7.634,P<0.01);而向健侧和向患侧转头诱发眼震的潜伏期,差异则无统计学意义(t=1.670,P=0.101).HSC-Can滚转试验中向患侧转头诱发眼震的潜伏期、强度均大于HSC-Cup向健侧转头,但差异无统计学意义(t值分别为1.554和0.305,P值均>0.05).结论 水平半规管BPPV患者滚转试验向左右侧转头诱发眼震的强度均遵循Ewald定律,强弱之比约为2∶1;潜伏期、持续时间及强度等眼震图参数可作为BPPV的客观诊断指标.  相似文献

目的 探讨水平半规管良性阵发性位置性眩晕(benign paroxysmal positional vertigo,BPPV)的临床特点.方法 回顾性分析2003年8月至2010年12月诊治的239例水平半规管BPPV患者的临床表现.结果 水平半规管BPPV占同期全部BPPV患者的25.7%(239/931).平卧侧头试验见水平向地性眼震者197例,眼震的平均潜伏期为(0.88±0.72)s,持续时间(26.36±19.71)s;水平离地性眼震者42例,平均潜伏期(2.69±1.83)s,持续时间(53.48±43.12)s;其中39例眼震表现为水平略带扭转向上而非纯水平,占16.3%(39/239).离地组眼震潜伏期明显长于向地组(t=-6.33,P<0.001),眼震持续时间亦明显长于向地组(t=-3.99,P<0.001).水平向地性眼震者予以Barbecue翻滚法治疗,192例经(1.6±0.8)个循环复位成功;水平离地性眼震者经左右侧头训练后,40例眼震转化为向地性,经(1.9±0.8)个循环后复位成功.结论 水平半规管BPPV占同期BPPV的比例较预期高.水平向地性眼震患者可直接给予Barbecue翻滚法治疗,水平离地性眼震患者应先行左右侧头训练,再给予Barbecue翻滚法复位.  相似文献

良性阵发性位置性眩晕(BPPV)是最常见的一种位置性眩晕,可能因后半规管(PSC)壶腹顶耳石或半规管耳石脱落引起。典型的PSC-BPPV眼震电图特点是巾一大的上跳性垂直眼震和一小的向上的水平性服震构成的非共轭性眼震。潜伏期短呈易疲劳性。阵发性位置性水平眼震同时伴有与BPPV相同的症状,可在有位置性眩晕的小部分病人中观察到。这种眼晨可能是外半规管BPPV的一种变型。该作者评定了9例在仰卧、头转向左或右均出现向上性眼震的BPPV患者。病人均经采集病史和详细的耳神经学检查。位置试验按Dix和HallPike法进行,病人均进行临…  相似文献

目的 探讨上半规管良性阵发性位置性眩晕(BPPV)诊断和治疗的最佳方案。方法 回顾性分析41例上半规管BPPV患者的病历资料,并对所有患者的诊断及复位进行评估分析。结果 SRM-Ⅳ模拟Dix-Hallpike试验诱发出典型垂直向下眼震28例(68.3%),其中眼震伴有向地扭转的成分19例,不伴有扭转成分9例;SRM-Ⅳ上半规管BPPV诱发试验诱发出典型垂直向下眼震35例(84.6%),其中眼震伴有向地扭转的成分30例,不伴有扭转成分5例。在所有41例患者中有38例能通过两种诱发试验对受累侧别做出判断,其中因诱发眼震伴有扭转成分而判断侧别30例,单侧诱发诱发出垂直向下眼震且不伴有扭转成分3例,双侧诱发出垂直向下眼震5例,且不伴有扭转成分,但因眩晕及眼震的强度有明显差别而判断出侧别,患者对受累侧别不能判断3例。在能判断出侧别的38例患者中有左侧上半规管受累25例,右侧上半规管受累13例。对41例采用SRM-Ⅳ上半规管BPPV复位法进行治疗,通过一次治疗痊愈28例,有效12例,无效1例。结论 上半规管BPPV 临床上被越来越重视,应用SRM-ⅣBPPV诊疗系统对上半规管BPPV进行诊治效果好,应该在临床得到推广。  相似文献

BACKGROUND: Horizontal canal benign paroxysmal positional vertigo is characterized by horizontal direction-changing nystagmus induced by lateral head turning in supine position. According to Ewald's second law, the direction of head turning that creates a stronger response represents the affected side in geotropic nystagmus and the healthy side in apogeotropic nystagmus. However, it may not always be possible to lateralize the involved ear only by comparing the intensity of the nystagmus. We studied the values of nystagmus induced by position change from sitting to supine in the lateralization of horizontal canal benign paroxysmal positional vertigo. METHODS: A retrospective study of 54 patients who had been diagnosed as having horizontal canal benign paroxysmal positional vertigo at the Dizziness Clinic of Seoul National University Bundang Hospital from May 2003 to February 2004 was performed. The directions of the nystagmus induced by lying down were compared with those determined by Ewald's second law. RESULTS: Of the 54 patients, 32 (20 apogeotropic and 12 geotropic) showed horizontal nystagmus induced by lying down. The nystagmus tended to be ipsilesional in apogeotropic patients (80%) and contralesional in their geotropic counterparts (75%). CONCLUSION: In horizontal canal benign paroxysmal positional vertigo, lying-down nystagmus mostly beats toward the involved ear in the apogeotropic type and directs to the healthy ear in the geotropic type. The direction of lying-down nystagmus may help lateralizing the involved ear in horizontal canal benign paroxysmal positional vertigo.  相似文献

IntroductionMulti-canal benign paroxysmal positional vertigo is considered to be a rare and controversial type in the new diagnostic guidelines of Bárány because the nystagmus is more complicated or atypical, which is worthy of further study.ObjectiveBased on the diagnostic criteria for multi-canal benign paroxysmal positional vertigo proposed by International Bárány Society, the study aimed to investigate the clinical characteristics, diagnosis and treatment of multi-canal benign paroxysmal positional vertigo.MethodsA total of 41 patients with multi-canal benign paroxysmal positional vertigo were included and diagnosed by Roll, Dix-Hallpike and straight head hanging tests. Manual reduction was performed according to the involvement of semicircular canals.ResultsAmong the 41 cases, 19 (46.3%) patients showed vertical up-beating nystagmus with or without torsional component and geotropic, apogeotropic horizontal nystagmus, and were diagnosed with posterior-horizontal canal. 11 (26.8%) patients showed vertical up-beating nystagmus with torsional component on one side and vertical down-beating nystagmus with or without torsional component on the other side during Dix-Hallpike test or straight head hanging test and were diagnosed with posterior-anterior canal benign paroxysmal positional vertigo 9 (26.8%) patients showed vertical down-beating nystagmus with or without torsional component and geotropic, apogeotropic horizontal nystagmus, and were diagnosed with anterior-horizontal canal 2 (4.9%) patients showed vertical geotropic torsional up-beating nystagmus on both sides and were diagnosed with bilateral posterior canal benign paroxysmal positional vertigo. High correlation between the sides with reduced vestibular function or hearing loss and the side affected by Multi-canal benign paroxysmal positional vertigo was revealed (contingency coefficient = 0.602, p = 0.010). During one-week follow up, nystagmus/vertigo has been significantly alleviated or disappeared in 87.8% (36/41) patients.ConclusionPosterior-horizontal canal benign paroxysmal positional vertigo was the most common type. Multi-canal benign paroxysmal positional vertigo involving anterior canal was also not uncommon. Caloric tests and pure tone audiometry may help in the determination of the affected side. Manual reduction was effective in most of Multi-canal benign paroxysmal positional vertigo patients.  相似文献

Over a period of 4 years we observed 15 cases of benign positional vertigo (BPV) probably caused by deposition of otoliths in the horizontal semicircular canal. Rapid rolling onto one side in recumbent position provokes a paroxysmal, purely horizontal and geotropic nystagmus which nearly always spontaneously inverts direction. Rolling the patient onto the other side provokes a left intense geotropic nystagmus. BPV is more violent but resolves more rapidly than that of the better known posterior canal positional vertigo. Three days after the initial fitting of the cochlear device a 40-year-old woman complained of severe rotational vertigo following head movements associated with neurovegetative symptoms. Otoneurological evaluation revealed a horizontal paroxysmal positional nystagmus beating towards the lowermost ear, induced by rolling the patient's head from supine both to the right or to the left lateral position suggesting the diagnosis of benign paroxysmal positional vertigo of the left horizontal semicircular canal. The nystagmus characteristics were the same whether the cochlear device was switched on or off. The hypothesis of an otolith dislodging due to the electrical stimulation during the initial fitting is discussed.  相似文献

ObjectiveAssesment of 8 new cases of benign paroxysmal positional vertigo of the horizontal semicircular canal (BPPV-HC), since this variant of benign positional vertigo occurs rarely. An oculographic study of features in the positional response was carried out. Electrooculograpy (EOG) allows an exhaustive study of positional nystagmus.Material and methodsWe have studied retrospectively 8 patients with BPPV-HC and positive head rotation manoeuvre registered by EOG techniques. Possible alterations in other EOG tests in the group of patients were analyzed.ResultsMost of the patients (7/8) expressed bilateral horizontal geotropic changing-direction nystagmus. One patient exhibited apogeotropic horizontal nystagmus. 5/8 cases showed caloric hypofunction.ConclusionsWe have proven that BPPVHC is an uncommon disorder. In most of the cases, provocative manoeuver generates bilateral horizontal geotropic changing-direction nystagmus that probably is due to canalitiasis of the horizontal semicircular canal. The rest, a few cases, exhibit apogeotropic horizontal response that can be secondary to cupulolitiasis or location particles in the anterior portion of the horizontal canal. A caloric test showed abnormal in many and can help to locatize the affected ear.  相似文献

目的 探讨继发性良性阵发性位置性眩晕(BPPV)的诊断和治疗。方法研究继发性后半规管BPPV的内耳疾病6例病历资料,诊断依据为病史及Dix-Hallpike试验诱导出现的眼震结果。结果 6例内耳疾病(分别为突发性聋3例, 梅尼埃病2例, 前庭神经元炎1例)伴有后半规管良性阵发性位置性眩晕被确诊,通过Dix-Hallpike试验诱发出垂直扭转型眼震。结论 继发性BPPV临床较少见,常为后半规管受累,通过Dix-Hallpike试验和Epley手法复位可以确诊和治愈。  相似文献

The benign paroxysmal positional vertigo of the horizontal semicircular canal is manifested with either geotropic or apogeotropic horizontal nystagmus. A 61-year-old male patient who experienced repeated episodes of positional vertigo is presented in this study. The vertigo was reported to be more severe while rotating his head to the left and then to the right. The initial examination revealed a geotropic purely horizontal nystagmus at the lateral positions of the head compatible with canalolithiasis of the left horizontal semicircular canal. In this case, the otoconia debris migrates from the vestibule into the horizontal semicircular canal through its nonampullary end, where they float freely (canalolithiasis). Five days later, the geotropic nystagmus transformed to apogeotropic. Thus, it may be assumed that the otoconia debris adhered to the cupula and converted the canalolithiasis to cupulolithiasis of the horizontal semicircular canal on the same side. With rotation of the head to the left while the patient was in the supine position, gravity causes the weighted cupula to deflect ampullofugally, resulting in apogeotropic nystagmus; the opposite was noticed when the head was rotated to the right. The so-called barbecue maneuver was initially effective curing the geotropic form of the condition and consequently the modified Semont maneuver for the apogeotropic form.  相似文献

When diagnosing benign paroxysmal positional vertigo, extraocular muscle contraction and the nystagmus it causes, though recognized as an important indicator, is less commonly seen as a principal method of diagnosis. However, through determining the direction of resulting nystagmus during diagnostic tests such as the supine roll test and the Dix-Hallpike test, which semicircular canals are involved in individual cases of benign paroxysmal positional vertigo can be ascertained, in both cases involving only one semicircular canal or cases of multi-canal benign paroxysmal positional vertigo.  相似文献

Some new modificutiont of existing diagnostic and therapeutic manoeuvres (repositioning of particles) have been proposed, basing an the applications of the principles of hydrodynamics, inertial and gravitational forces in the semicircular canals. The above has been tried successfully on patients with benign paroxysmal positional vertigo (BPPV) and horizontal eanal paroxysmal positional vertigo (HCPPV) which can be executed by the patients themselves at home without the kelp of a therapist. Benign paroxysmal positional vertigo (BPPV) is a well-established entity whereas Horizontal Canal Paroxysmal Positional Vertigo (HCPPV) is a recently proposed one. In positional vertigo any canal may be involved. It is quite possible that in the past this diagnosis (HCPPV) was missed in many of the cases of positional vertigo. Kapfschuttel nystagmus (head-shaking nystanmas) is induced by side to side (to and fro) movements of the head around a vertical axis in the plane of the horizontal semicircular canal which is thought to be sensitise the labyrinth, Any manoeuvres that in induce nystagmus with or without vertino is hound to he a manifestation of an organic lesion and not a non-organic one. In a number of instances the postionul test Jor BPPV may yield negative results; but doing this test after kopfschuttel test a dormant positional nystagmus may appear on the surface i.e. uncovered in a number of cases which may have remained undetected if Kopfschutlel manoeuvre were not done. This implies- that by Kopfsi huttel test both the horizontal and vertical canals are sensitised. Moreover, Kopfschuttel nystaagmus is likely to be a manifestation of the horizontal canal stimulation, at leastin some cases, since the movements are executed in the plane of the horizontal canal (vide infra) when displaced otoconia impinges on the cupula /. crista bringing about its stimulation leading to vertigo and or nystagmus.  相似文献

The incidence of benign paroxysmal positional vertigo (BPPV) of the horizontal and superior semicircular canals is much less than that of BPPV due to affection of the posterior semicircular canal. Their diagnosis is however much more difficult and still prone to controversies. The provocative manoeuvre of the BPPV of the horizontal canal (BPPV-HSC) is the manoeuvre of rotation of the head in dorsal position. A horizontal positional nystagmus is obtained. There are two forms of BPPV-HSC: the geotropic form and the ageotropic form. In the geotropic form, the liberatory manoeuvre is a "barbecue" rotation of 180 with 360 degrees towards the healthy side. In the ageotropic form, there is no universal liberatory manoeuvre. Moreover as some cases of neurological aetiology have been recognized, it is not appropriate to apply ineffective manoeuvres. The BPPV of the superior canal (BPV-SSC) is very rare. The provocative manoeuvre is the Dix and Hallpike's manoeuvre. It causes positional torsional and vertical nystagmus with an opposite direction to that obtained for a BPPV of the contra-lateral posterior canal. The liberatory manoeuvre is a Semont manoeuvre, which is identical to that we would make for a contra-lateral BPPV of the posterior canal.  相似文献
