Vancomycin‐resistant enterococci (VRE) have emerged to become a significant nosocomial pathogen. However, detection may be challenging and treatment possibilities are limited. Reports of resistance to linezolide, daptomycin and tigecycline underline the need for reliable susceptibility testing with respect to these compounds. We evaluated the in vitro activity of vancomycin, linezolid, daptomycin and tigecycline against a panel of VRE and vancomycin‐susceptible enterococci by broth microdilution (BMD). Etest for determination of minimum inhibitory concentration of these four antibiotics and two disc diffusion assays for detecting VRE and for susceptibility testing against tigecycline and linezolid were evaluated. Before susceptibility testing, all isolates were classified by polymerase chain reaction as vanA or vanB gene positive or vanA/B gene negative. Linezolid, daptomycin and tigecycline had excellent in vitro activity towards all isolates. For daptomycin and tigecycline, the overall agreement between BMD and Etest was suboptimal. For both disc diffusion assays, use of current break points was inadequate to detect vancomycin resistance for isolates carrying the vanB gene. Inspection of the inhibition zone for a diffuse edge, as recommended, accurately predicted presence of the vanB gene. 相似文献

Pathogenic variants in the cyclin‐dependent kinase‐like 5 (CDKL5) gene cause the neurodevelopmental disorder, the CDKL5 deficiency disorder. Reports of individuals with pathogenic variants in CDKL5 without seizures are exceedingly rare, and in‐depth analyses of their variants have been lacking. Whole‐genome sequencing was performed on a 29‐year‐old female with mild intellectual disability who, in the absence of overt seizures, presented with multiple episodes of altered mental status over a 24‐year period. Clinical history was supplemented by a parent completed questionnaire from the International CDKL5 Disorder Database. We identified a de novo heterozygous variant in CDKL5 (NM_003159.2:c.645T>A;p.Ser215Arg). In‐depth computational analysis performed to predict the impact of the variant on protein structure and function demonstrated that the variant was likely pathogenic. In this light, cell‐based studies showed that the S215R substitution causes a marked reduction in CDKL5 kinase activity. Similarities between our case and one previously reported case are striking. These cases, both without seizures but with apparent behavioral symptomatology, together question whether seizures are mandatory in this neurodevelopmental disorder. 相似文献

The measure of success for a scientific journal, which publishes original research, is its visibility in the international scientific community. The most comprehensive bibliographical database of the Institute for Scientific Information (ISI, now Thomson ISI), Web of Science, indexes 13 out of 176 journals that received financial support from the Ministry of Science in 2003, and 7 of them are in the 9 divisions of the most selective ISI bibliographic database, Current Contents. Taking into account the specificities of the small scientific community, or the so-called scientific "periphery" to which Croatia belongs, we proposed a new system of financial support of scientific journals in Croatia. Such system would support all scientific journals fulfilling basic requirements of scholarly publishing, such as structure of the editorial board and office, editorial procedures and records. Indexing in professional and general bibliographic databases would be encouraged by additional financial support and continual control of the editorial work in the journals. 相似文献
