Preoperative F-18 FDG PET/CT study in this 57-year-old woman showed an FDG avid lesion in the left upper lung without evidence of lymphadenopathy or distant metastasis. She underwent a left upper lobectomy in June 2005 revealing moderately poorly differentiated adenocarcinoma (pT3N0M0) and subsequent chemotherapy completed December 2005. Nine months later, a left parietal lobe metastatic lesion was surgically resected. A true whole body FDG PET/CT study in November 2006 for restaging demonstrated new FDG avid spinal cord foci, which were highly suspicious for spinal cord metastases; these lesions were confirmed by MRI. Clinically, the patient recently developed back pain without evidence of neurologic deficits.  相似文献

It is well-known that thymoma can be associated with diverse extrathymic malignancies. The authors present a case of a 72-year-old man with myasthenia gravis in which F-18 FDG PET/CT demonstrated high FDG uptake in both an anterior mediastinal mass and a lung nodule. The FDG uptake in the lung nodule was significantly higher than that of the thymic lesion, which might suggest the possibility of a second development of lung cancer in this thymoma patient. Surgical resection was done for the 2 tumors, and histologic examination revealed anterior mediastinal thymoma and primary adenocarcinoma of the lung. This case demonstrates the potential usefulness of PET/CT in detecting and assessing the second extrathymic malignancy in a patient with thymoma.  相似文献

Uterine epithelioid angiosarcoma can have conventional imaging characteristics similar to those of other uterine tumors, such as leiomyoma, leiomyosarcomas or hemangioendothelioma. Uterine epithelioid angiosarcoma exhibiting increased fluorine-18 fluorodeoxyglucose (F-18 FDG) activity can be misdiagnosed. A 61-year-old woman who was diagnosed with uterine epithelioid angiosarcoma underwent F-18 FDG positron emission tomography/computed tomography (PET/CT) as a part of the pretreatment work up for surgery. F-18 FDG PET/CT showed an intense F-18 FDG uptake in the uterus in addition to increased F-18 FDG uptake at the paraaortic and aortocaval lymph nodes. To our knowledge, this is the first case report of intense F-18 FDG uptake in uterine epithelioid angiosarcoma in Korea.  相似文献

A 29-year-old woman presented with a new right supraclavicular mass on a background of resection of a malignant melanoma from her right shoulder before 4 years. F-18 fluorodeoxyglucose positron emission tomography/computed tomography (FDG PET/CT) imaging revealed uptake in a right supraclavicular node and the inferior pole of the left lobe of the thyroid gland. Subsequent resection and histology of the right supraclavicular node and the left lobe of the thyroid gland demonstrated metastatic melanoma and palpation thyroiditis, respectively. A number of malignant and benign pathologies of the thyroid demonstrate FDG uptake, with 75% being benign. To our knowledge, the findings of palpation thyroiditis on FDG PET/CT have never been previously described.  相似文献

We report a case of relapsing polychondritis for which fluorodeoxyglucose (FDG) positron-emission tomography/computed tomography (PET/CT) showed increased FDG accumulation in all rib cartilages, as well as in the larynx, trachea, and major bronchi. Contrast-enhanced CT during PET/CT showed smooth tracheal and bronchial wall thickening with calcification and airway narrowing. After steroid therapy, clinical symptoms and laboratory data were improved and cartilaginous FDG accumulation had completely disappeared. FDG PET/CT is considered to be a powerful radiological tool to assess the disease activity of relapsing polychondritis.  相似文献

We report F-18 FDG PET/CT images of biopsy-proven intramuscular metastasis from primary uterine cervix cancer. A 70-year-old woman was admitted because of a mass in the left axilla. She was diagnosed with uterine cervix cancer and underwent total abdominal hysterectomy, bilateral salpingo-oophorectomy, and pelvic node dissection and received adjuvant chemotherapy and radiation therapy 9 years ago. Magnetic resonance imaging showed a large soft tissue mass in the intramuscular region of the left axilla. The mass showed intense F-18 FDG uptake with a central necrotic portion on F-18 FDG PET/CT. Hypermetabolic lymph nodes were also noted in the left axilla, interpectoral, and left supraclavicular areas. Additionally, several hypermetabolic nodules were noted in both lungs. The lesion in the left axilla was excised and the pathologic results were consistent with metastasis from uterine cervix cancer.  相似文献

PURPOSE: There are approximately 32,000 new cases of thyroid carcinoma annually in the United States. F-18 FDG PET/CT has an established role in cancer management, including thyroid cancer, usually in patients who are thyroglobulin (Tg) positive/iodine negative. We reviewed our experience with F-18 FDG PET/CT in thyroid cancer, with an emphasis on correlation with Tg, and maximum standardized uptake values (SUV). We also analyzed the role of thyroid stimulating hormone (TSH) on PET/CT results. MATERIALS AND METHODS: This is a retrospective study (January 2003 to December 2006) of 76 patients with differentiated thyroid cancer, who had F-18 FDG PET/CT scans. There were 44 women and 32 men, with age range of 20 to 81 years (average, 51.1 +/- 18.1). The administered doses of F-18 FDG ranged from 396 to 717 MBq (15.8-19.4 mCi) (average, 566 +/- 74.8) (15.3 +/- 2). Reinterpretation of the imaging studies for accuracy and data analysis from medical records were performed. RESULTS: A total of 98 PET/CT scans were analyzed (59 patients had 1 scan, 12 patients had 2, and 5 patients had 3). PET/CT was 88.6% sensitive (95% CI: 78.-94.3) and 89.3% specific (95% CI: 71.9-97.1). Mean Tg level was 1203 ng/mL (range, 0.5-28,357) in patients with positive PET/CT and 9.72 ng/mL (range, 0.5-123.0) in patients with negative PET/CT scans (P = 0.0389). Mean SUV max was 10.8 (range, 2.5-32) in the thyroid bed recurrence/residual disease and 7.53 (range, 2.5-26.2) in metastatic lesions (P = 0.0114). Mean SUV max in recurrent/residual disease in patients with TSH 30 mIU/L was 8.1 (range, 2.6-32) (P = 0.2994). CONCLUSION: F-18 FDG PET/CT had excellent sensitivity (88.6%) and specificity (89.3%) in this patient population. Metastatic lesions were reliably identified, but were less F-18 FDG avid than recurrence/residual disease in the thyroid bed. TSH levels at the time of PET/CT did not appear to impact the FDG uptake in the lesions or the ability to detect disease. In the setting of high or rising levels of Tg, our study confirms that it is indicated to include PET/CT in the management of patients with differentiated thyroid cancer.  相似文献

Cardiac hemangiomas are extremely rare, benign tumors, which can occur anywhere in the heart. Symptoms are variable according to the size, extension and tumor location, but most cases are asymptomatic and are detected incidentally. They may grow, remain stable and regress; therefore, the natural course of the tumors is unpredictable. Diagnosis mainly depends upon echocardiography, CT, MRI and angiography. Reports of detection by F-18 FDG PET/CT are very limited. We report a case of cardiac hemangioma attached to the right ventricle, compressing the ventricle. It was revealed incidentally on F-18 FDG PET/CT for routine evaluation of thyroid cancer. During two serial F-18 FDG PET/CTs, it grew from 2.8 cm to 4.0 cm with mild FDG uptake. After surgery, the patient remained stable without any complications.  相似文献

Peritoneal lymphomatosis is uncommon, but when encountered is associated with aggressive histological subtypes of high-grade lymphoma, such as small-cell, large-cell, mixed large and small cell, non-cleaved, lymphoblastic Burkitt-like, and diffuse large B cell lymphomas. The CT findings of peritoneal lymphomatosis are linear or nodular peritoneal thickening, retroperitoneal lymphadenopathy, omental and mesenteric involvement with streak-like infiltrations or a bulky mass, bowel wall thickening, hepatosplenomegaly, and ascites. The authors report the first FDG PET/CT images of diffuse large B-cell lymphoma of small bowel origin associated with peritoneal lymphomatosis in a 69-year-old man. The lesions demonstrated intense FDG uptake in PET/CT images.  相似文献
