Background: In recent years, success rate of replantion of severed finger is increased every year with developing of fiber surgery. But postoperative local vascular spasm and hypercoagulability often leads to the incidence of thrombosis and circulatory crisis that were an important reason leading to operation failure.

MATERIALS AND METHODS Materials 46 cases of auuputated finger patients were received from August1989 to July 1997, with 26 males and 20 females, age ranging from18 to 46 years, weighing 45 -70 kg. Epidural duct was input in in-termuscular, replantation of amputated finger were operated bybrachial plexus block for 5- 16 h, and all the branchial plexusblock were successful.

PREPARATION AND CHARACTERIZATION OF TWO MONOCLONAL ANTIBODIES AGAINST A_α CHAIN'S C TERMINUS OF FIBRINOGEN

Two monoclonal antibodies (McAbs) against Aα chain's C terminus offibrinogen (Fg) have been prepared and designated SZ-78 and SZ-79. Both theantigens in binding assay and immunoblot analysis showed that the two McAbs recognized the epitopes located in residues 549-560 of the Aαchain. The two McAbs couldaccelerate rate of fibrin polymer assembly both in the purified system and in the humanplasma. From the pictures of transmission electronmicroscope, the average diametersof the fibers increase significantly to an average diameters of 375 nm after incubationwith the McAbs, while it was only 75nm without addition of the McAbs. There were al-so more branchings of fibers with addition of McAbs. These observations demonstratethat the amino acid sequences ofα 549-560 in the COOH terminus of the Aα chain mayplay an important role in the assembly of a fibrin clot, presumably being involved in lat-eral aggregation of protofibrils. The preparation of the McAbs supplies a usuful probe for the investigation of the fun     

人性化管理在妇产科手术室护理管理中的应用观察研究

Objective To discuss the value of humanized management application in obstetrics and gynecology operating room nursing management. Methods 200 cases of patients admitted to the department of obstetrics and Gynecology of our hospital in the operating room as the research objective, divided into observation group, reference group by parity method, humanized management application in the observation group, routine operating room nursing management application in the reference group, compare the nursing data of the two groups. Results The rate of nurse-patient dispute in the operation room of the observation group was lower than that of the reference group, and the total incidence of complications was also lower than that of the reference group, which was significant (P＜0.05). Compared with the reference group, all nursing quality scores in the observation group were significantly higher (P＜0.05). The total nursing satisfaction in the observation group was significantly higher than that in the reference group (P＜0.05). Conclusion The application of humanized management in obstetrics and gynecology operating room nursing management, help to improve patient nursing satisfaction, and reduce the incidence of complications, reduce the incidence of nurse-patient disputes, improve the quality of nursing, improve the quality of life of patients, has clinical application value.

局部氧疗促进烧伤患者残余难愈创面修复的有效性研究

Objective In order to know the effective of local oxygen therapy to promoting renovation of residual surface of bums wound.Methods Divided 60 patients with burns into the experimental group and the control group,there were 30 cases in the each group.Routine nursing cares were used in the control group,while the local oxygen therapy was used in the experimental group.Compare the condition of secretion of wound between the two groups.Results The wound healing time of experimental grou was(5.87±3.29)d,which significant shorten than that of in the control group,the kinds of bacteria in the control group was more than in the experimental group.Conclusions The local oxygen therapy can effective promote the healing of residual surface of bums wound,which can reduce the infection rate of bacteria.     

丹奥对脑梗死神经功能恢复的近期疗效观察

Backgroud:The cerebral infarction is one of the major reasons which cause deathes or disabilities in the aged.The initiaion factors of cerebral infarction were increased aggregation of the platelets and released thromboxane synthase(TXA2).Tehenzyme caused the aggregation of the platelets and the constration of blood vessels and so caused the formation of thrombus.Dan'ao(Ozagren sodium)may inhibit the synthesis of thrombus synthase,inhibit synthesis of platease and facilitate production of Prostaglandin synthase.     

经皮关节腔内臭氧注射治疗动物骨关节炎的组织与超微结构观察研究

Objective To investigate the pathologic effect of intra-articular injection of ozone on the ar-ticular cartilage, and to explore the feasibility of direct intra-articular injection of ozone in the treatment of arthri-tis. Methods Animals with induced-arthritis were given three different concentrations of medical ozone (high and low concentrations)by intra-articular injection, to observe pathological changes of cartilage. Results The score result of cartilage degradation was based on Mankin's grading scale,and the higher the score,the severer the symptom was. The score of normal articular cartilage was 0.0±0.0, and the score of model group was sig-nificantly higher than that of the normal group(P<0.01), while obviously lower than that of the treatment group (P<0.01). Moreover,the score of high-dose treatment group was significantly higher than that of low-dose group(P<0.01). Conclusion Long-term intra-articular injection of ozone had a bad effect on articular carti-lage, and damage-level was increased with the enhancement of ozone concentrations.     

断指再植术后早期康复介入及随访效果

BACKGROUND: Survive rate after replantation of servered finger has been improved, but functional recovery isn' t yet satisfying. So, rehabilitation treatment has an important significance to recovery of function which can eliminate or reduce disability of limbs function due to injury and restore function of affected limbs at a maximal degree.     

产科新生儿不同部位经皮测黄报警预值的可靠性观察

目的对比观察产科新生儿不同部位经皮胆红素（TCB）报警预值的可靠性。方法132例产科新生儿采取随机数字分组法分为正常产组和剖宫产组各66例,新生儿均于产后第4天同一时间点应用KJ8000经皮测黄仪分别测量额、胸、腹、额胸、额胸腹TCB值,TCB〉12．9mg／dl者,取得亲属同意抽取静脉血检测血清胆红素（SB）,对比分析不同部位TCB及其与sB值的差异。结果两组分别有17例或21例达到TCB报警预值。两组TCB或sB相同方法及相同部位比较,差异无统计学意义（P〉0．05）;两组TCB不同部位对比,额部值最低、胸部值最高,且与其他部位同组对比差异均有统计学意义（P〈0．01）;两组sB值对比差异无统计学意义（t=1．53,P〉0．05）,与不同部位TCB对比均以胸部数值差异无统计学意义（P〉0．05）,而与其他部位TCB两组差异均有统计学意义（P〈0．01）。结论正常产与剖宫产新生儿术后sB对比差异无意义;TCB动态监测以胸部结果更接近SB。     

血液透析患者家庭护理提供者的生活质量调查

目的:了解江汉油田血液透析(血液透析)患者家庭护理提供者(护理者)的生活质量。方法:对60例血液透析患者的家庭护理提供者进行一般情况和生活质量综合评定问卷(QOLI-74)调查,并进行相关性和多因素回归分析。结果:家庭护理提供者各维度的主观生活满意度与其客观指标相关,但也与其需求、年龄、文化程度、与患者的关系有关。结论:客观状态是影响主观生活满意度的重要因素,同时应考虑护理者的需求、年龄、文化程度、与患者的关系对护理者主观生活满意度的影响。     

韶关市农村留守儿童孤独感状况调查

目的了解广东省韶关市农村地区留守儿童孤独感现状及其影响因素。方法对韶关市某地区两所农村小学3～6年级学生中的489名留守儿童采用儿童孤独量表和自编调查表进行问卷调查。结果17．6％留守儿童存在孤独感，不同性别孤独感发生率无差异性，不同年龄及不同年级间孤独感发生率差异均有极显著性（P〈0.01）；随年级增加，孤独感发生率呈下降趋势（X^2趋势=5．970，P〈0.05）。留守儿童孤独感与健康状况、学习成绩、学习困难程度、父母教育方式、父母间关系和老师教育方式等因素显著相关（P〈0.01～0．05）。结论农村地区留守儿童中存在一定程度的孤独感问题，老师和家长应以正确的态度和方法对待留守儿童，以减少其孤独感的发生。     

5例自发性脾破裂误诊分析

目的：探讨自发性脾破裂误诊的原因,方法：回顾分析1986－1998年我院收治的8例自发性脾破裂的临床资料,结果误诊5例,误诊率62．5％,结论凡不明原因上腹部疼痛,伴有失血征象而无法解释、无外伤史,积极行B超、腹穿以明确。若有血吸虫、肝炎等肝脾病变或脾亢更应警惕本病,发病前有饱食或重体力劳动等腹压增高原因有助诊断。     

胰腺癌治疗的Cochrane系统评价证据

目的评价胰腺癌治疗的Cochrane系统评价证据,以及纳入系统评价的临床随机对照试验（RCT）的方法学质量。方法检索Cochrane Library数据库（2009年第4期）中有关胰腺癌治疗的系统评价,并运用RewMan5.0.21对所纳入研究的偏倚进行评估。结果共检索到胆道支架置入术姑息治疗梗阻型胰腺癌的系统评价、放化疗治疗不能手术的进展期胰腺癌的系统评价共2篇系统评价,共纳入79个RCT。依照Cochrane协作网推荐的质量评价方法,对所纳入RCT的偏倚进行评估,表明均存在不同程度的偏倚,方法学质量普遍较低。结论 Cochrane系统评价是公认的最高质量的研究证据,但目前缺少足够强度的证据来支持胆道支架置入术姑息治疗梗阻型胰腺癌的疗效。其他治疗手段的疗效如胰腺癌围手术期的营养支持治疗等还需要通过进一步的完成系统评价来评估。建议推行临床试验透明化,实施临床试验注册制度以及按照CONSORT声明严格规范RCT的报告,以便于总结胰腺癌治疗的临床证据。     

尿幽门螺杆菌抗体测定在幽门螺杆菌感染诊断中的价值

目的：探讨幽门螺杆菌（Hp)抗体阴性用于排除近期Hp感染的临床价值。方法：Hp感染诊断采用^13C尿素呼气试验、快速尿素酶试验和W－S银染色，Hp抗体测定采用Hp抗体测定层析板。结果：31例Hp感染患10分钟阳性率87．1％（27／31），30分钟阳性率96．7％（30／31），56例无Hp感染患假阳性率19．7％（11／56）。对Hp感染诊断敏感性96．78％，特异性80．3％。对Hp感染阳性组以Hp抗体，^13C尿素呼气试验阳性率最高，Hp感染阴性组以Hp抗体阳性率最高。46例阳性病例中仅1例^13C尿素呼气试验阳性，W－S银染色、快速尿素酶试验均阴性。结论：Hp抗体阴性基本可排除近期有Hp感染，可避免病人作进一步的无益检查。     

Evaluation of the aesthetics of physical methods of euthanasia of anesthetized rats

Dissection of living brain tissue for in vitro experiments requires the use of a rapid euthanasia method. However, the method must not subject animals to unnecessary pain and must be aesthetically acceptable to experimenters. The purposes of the current study were to assess the aesthetics of 6 euthanasia methods, measure the procedure duration, and evaluate brain for pathology after each procedure. We digitally recorded euthanasia of isoflurane-anesthetized rats by 6 physical methods: anesthetic overdose, cardiac exsanguination, decapitation, closed intrathoracic transection of the great vessels and heart, thoracic percussion, and thoracotomy with rupture of great vessels. Volunteer researchers and animal caretakers watched the video and completed an associated questionnaire. Anesthetic overdose and cardiac exsanguinations were rated most aesthetically pleasing, although these procedures took the longest to complete. In contrast, decapitation and thoracic percussion were the least aesthetically pleasing, but these methods were the quickest. No demographic factor was identified that could predict whether a given euthanasia procedure would be favored for aesthetic reasons, and participants provided a wide variety of rationales for the aesthetic ratings they assigned. Although all of these euthanasia methods meet the criteria of approved methods of euthanasia of anesthetized rats as defined by the AVMA, aesthetic features and the scientific need for rapid euthanasia are both considerations in selecting a method.     

Analysis of the Concept of Quality of Life

ABSTRACT. Nurses provide the constant human care that can make the difference between a life that is valuable to the last and a life that ends in humiliation, deprivation and suffering. Nurses, therefore, more than other health professionals, need a deep and reflective view of patients' value and quality of life. In this paper the concept of quality of life is analysed from a philosophical and scientific perspective. Quality of life in connection with treatment of a disease has its roots in the philosophical conception of the value of human life in general. Characteristics such as happiness, physical activity and interpersonal relations are what give life value and meaning. From the scientific viewpoint, it seems difficult to define the concept, given its multidimensional characteristics. However, the analysis indicates a growing interest among health-professionals to develop scientific intersubjectivity of the concept in terms of measurable parameters.     

主动脉窦瘤的超声心动图诊断

对３７例经超声心动回检查（包括两维超声、频谱多普勒、彩色多普勒、声学造影及经食道超声心动图）诊断为主动脉窦瘤患者进行手术对照研究。结果表明：超声心动图能准确显示主动脉窦瘤的形态、起源、突入腔室及破裂状况，并能基本正确评估其合并的右室流出道狭窄、主动脉瓣脱垂及返流、心包积液等病损。文中尚对窦瘤是否破裂、是否并发室间隔缺损等鉴别诊断进行了讨论。     

Radiographic presentation of avascular necrosis of the proximal femur after treatment of developmental dislocation of the hip - review of classifications of severity of changes

The fate of the hip with avascular necrosis occuring as a consequence of treatament of developmental hip dislocation depends on the severity of changes, mainly on the disturbance of growth within growth cartilages of the proximal femur. The most commonly used classifications of avascular necrosis are presented in this paper with special emphasis on its influence on the final result after cessation of growth.
