Purpose

This study was undertaken to prospectively evaluate the diagnostic performance of colour Doppler ultrasonography (CDUS), first-pass (FP) and steady-state (SS) contrast-enhanced magnetic resonance angiography (MRA) and computed tomography angiography (CTA) of the carotid arteries using digital subtraction angiography (DSA) as the reference standard.

Materials and methods

A total of 170 patients with previous cerebrovascular events and suspected carotid artery stenoses underwent CDUS, blood-pool MRA, CTA and DSA. Accuracy, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were calculated for CDUS, FP MRA, SS MRA and CTA. The McNemar and Wilcoxon tests and receiver operating characteristic (ROC) curve analysis were used to determine significant differences (p<0.05) between the diagnostic performances of the four modalities, and the degree of stenosis was compared using linear regression.

Results

A total of 336 carotid bifurcations were studied. The area under the curve (AUC) for degree of stenosis was: CDUS 0.85±0.02, FP MRA 0.982±0.005, SS MRA 0.994±0.002 and CTA 0.997±0.001. AUC analysis showed no statistically significant difference between CTA and MRA (p=0.0174) and a statistically significant difference between CDUS and the other techniques (p<0.001). Plaque morphology analysis showed no significant difference between CTA and SS MRA; a significant difference was seen between CTA and SS MRA versus FP MRA (p=0.04) and CDUS (p=0.038). Plaque ulceration analysis showed a statistically significant difference between MRA and CTA (0.04< p<0.046) versus CDUS (p=0.019).

Conclusions

CTA is the most accurate technique for evaluating carotid stenoses, with a slightly better performance than MRA (97% vs. 95% for SS MRA and 92% for FP MRA) and a greater accuracy than CDUS (97% vs. 76%). Blood-pool contrast-enhanced SS sequences offer improved evaluation of degree of stenosis and plaque morphology with accuracy substantially identical to CTA.  相似文献

BACKGROUND AND PURPOSE:Dual-source CTA and black-blood MRA are recently developed techniques for evaluating carotid stenosis. The purpose of this study was to compare dual-source CTA with black-blood MRA and conventional TOF MRA in both detecting carotid stenosis by using DSA as a reference standard and demonstrating plaque morphology.MATERIALS AND METHODS:Thirty patients with suspected carotid artery stenosis underwent unenhanced MRA by using black-blood and TOF MRA and dual-source CTA. Source images from unenhanced MRAs and dual-source CTA were reconstructed with MIP or curved planar reconstruction. The degree of carotid artery stenosis was measured, and plaque surface morphology at the stenosis was analyzed and compared among different techniques.RESULTS:Good correlation was observed for measuring the degree of carotid stenosis among dual-source CTA, black-blood MRA, TOF MRA, and DSA. Sensitivity and specificity for detecting severe stenosis were 100% and 97% with dual-source CTA, 100% and 95% with black-blood MRA, and 79% and 95% with TOF MRA. None of the 3 technologies resulted in stenosis of <50% being overestimated. Plaque surface irregularity or ulceration was more frequently detected with dual-source CTA and black-blood MRA than with TOF MRA and DSA.CONCLUSIONS:This preliminary study shows that black-blood MRA is a promising technique, comparable with dual-source CTA and DSA, but better than TOF MRA, in the evaluation of carotid stenosis. Unlike dual-source CTA, black-blood MRA requires no intravenous contrast or radiation.

Carotid artery atherosclerosis is a major cause of ischemic cerebrovascular disease.1,2 Measurement of carotid stenosis and demonstration of plaque morphology are critical for the management of patients with carotid atherosclerosis. DSA is the current reference standard for evaluating carotid artery stenosis. The diagnostic role of DSA has largely been replaced, however, by noninvasive techniques such as sonography, CTA, and MRA.Sonography has been the most commonly performed technique but may be restricted by its operator dependence and limited coverage. CTA is another widely used technique for the evaluation of carotid artery stenosis with high accuracy.3 Dual-source CTA (DSCTA) uses 2 x-ray sources and 2 detectors at the same time. With this technique, 2 images can be simultaneously acquired with different tube voltages; this feature has been shown to be an advantage for the evaluation of densely calcified carotid stenosis.4,5 Contrast-enhanced MRA has been established as an alternative for carotid imaging with a diagnostic accuracy similar to that of CTA.6,7 Both CTA and contrast-enhanced MRA use contrast media and are restricted in patients with impaired renal function, and CTA also requires ionizing radiation. As a result, unenhanced MRA without gadolinium is a desirable alternative, especially in patients with renal failure. Conventional TOF MRA has been widely used in clinical practice for carotid visualization, but it is limited by local reduction of signal intensity related to slow and turbulent flow and also prolonged imaging time.8 T2-weighted black-blood MRA (BB MRA) is a newly developed technique showing potential in the evaluation of both the lumen and the wall of the carotid artery after optimal suppression of the signal from flowing blood.9,10 Few studies, to our knowledge however, have compared DSCTA with BB MRA and conventional TOF MRA in evaluating carotid stenosis.The aim of this study was to prospectively and intraindividually compare these 2 unenhanced MRA methods with DSCTA in detecting carotid artery stenosis by using DSA as the standard of reference and in demonstrating plaque morphology.  相似文献

Objectives

To assess the capability of the three-dimensional (3D) Fast Imaging Employing Steady-State Acquisition (FIESTA) sequence in evaluating renal artery stenosis (RAS).

Methods

We retrospectively analysed 79 patients referred for suspected RAS, examined by 3D FIESTA and contrast-enhanced magnetic resonance angiography (CE-MRA), using a 1.5T whole-body scanner. Image quality was assessed as well as the presence and grade of RAS. Patients with RAS?≥?50 % were evaluated for possible digital subtraction angiography (DSA). Sensitivity, specificity, negative predictive value (NPV), positive predictive value (PPV), and accuracy of 3D FIESTA were calculated with ROC analysis using CE-MRA and DSA as the standard of reference.

Results

A total of 186 renal arteries were assessed; 36 had RAS?≥?50 % demonstrated by CE-MRA. Ten patients underwent DSA, for a total evaluation of 22 arteries. Sensitivity, specificity, NPV, PPV, and accuracy of 3D FIESTA were 91.7 %, 100 %, 98 %, 100 %, and 98 %, respectively, as compared to CE-MRA, and 88.2 %, 100 %, 71.4 %, 100 %, and 91 %, respectively, as compared to DSA. The area under the ROC curve (AUC) of 3D FIESTA as compared to CE-MRA and DSA was 0.958 and 0.941, respectively.

Conclusions

Our study demonstrated the capability of the 3D FIESTA sequence in evaluating RAS, with high-quality images and good diagnostic accuracy.

目的 探讨磁共振血管造影(MRA)、CT血管造影(CTA)在诊断颈动脉狭窄及内膜切除术后随访中的价值。方法 11例22支颈动脉行MRA、CTA、CT仿真内窥镜(CTVE)、多普勒超声(DUS)检查。12支结果与DSA对照。3支颈动脉重度狭窄者行内膜切除术,MRA采用二维、三维时间飞越法。CTA经最大强度投影(MIP)及遮盖表现显示法(SSD)三维重建。应用导航功能行CTVE成像。结果 颈动脉轻度狭窄8支,中度狭窄3支,重度狭窄5支,闭塞1支,5支正常。12支颈动脉与DSA对照:CTA评估血管狭窄与DSA相符者11支,MRA与DSA相符者9支,DUS与DSA相符者7支。CTA、CTVE显示斑块、壁血栓6支,3支手术证实。术后MRA示狭窄解除,CTA示斑块消失。结论 MRA、DUS可用于颈动脉狭窄的筛选及术后随访。CTA评估血管狭窄程度与DSA、手术比较有较好的一致性,并能直接显示钙化斑块。  相似文献

Objective

To evaluate the clinical value of unenhanced magnetic resonance angiography (MRA) at 3.0?T for the diagnosis and therapeutic planning of patients with subarachnoid haemorrhage (SAH).

Methods

A total of 165 patients with SAH were referred for three-dimensional time-of-flight MRA (3D-TOF-MRA) before digital subtraction angiography (DSA). For each aneurysm, 3D-TOF-MRA was used to determine whether the aneurysm was suitable for coil placement with or without balloon/stent-assisted coiling, surgical clipping or conservative treatment. Treatment planning with 3D-TOF-MRA was compared with actual treatment decisions or treatment that had been carried out in each aneurysm decided using DSA.

Results

The aneurysm-based evaluation yielded accuracy of 96.9%, sensitivity of 97.6%, specificity of 93.1%, positive predictive value (PPV) of 98.8% and negative predictive value (NPV) of 87.1%, in the detection of intracranial aneurysms. Treatment planning could be correctly made on the basis of aneurysm anatomy and working view by volume rendering (VR) 3D-TOF-MRA with accuracy, sensitivity, specificity, PPV and NPV of 94.9%, 94.0%, 100%, 100% and 74.4%, respectively, on a per aneurysm-based evaluation.

Conclusions

VR 3D-TOF-MRA offers high diagnostic accuracy in the detection of ruptured intracranial aneurysms, and appears to be an effective treatment planning tool for most patients with SAH.

Introduction

Ostium of vertebral artery (VA) is a common site of pseudostenosis on contrast-enhanced MR angiography (CE-MRA). The purpose of this study was to determine the diagnostic accuracy of CE-MRA at 3 T in the evaluation of ostial stenosis of VA and to find associated coincidental stenoses using logistic regression analysis.

Methods

One hundred and thirty-five VA ostial regions from 72 patients who received CE-MRA of neck vessels, intracranial time of flight (TOF) MRA, and digital subtraction angiography (DSA) were retrospectively reviewed. The sensitivity and specificity of the CE-MRA in detection of ostial stenosis were calculated with reference standard of DSA. Ostial stenosis on MRA was correlated with coincidental lesions in intracranial and cervical arteries by logistic regression analysis.

Results

The sensitivity and specificity of the CE-MRA were 100% and 80.4% for detection of significant stenosis. In case of significant stenoses, CE-MRA showed a tendency of overestimation with a false-positive rate of 52.5%. Logistic regression analysis showed that the stenoses of middle cerebral artery (MCA) on TOF MRA was associated with significant stenoses of VA ostia (OR?=?5.84, 95% confidence intervals 1.41–24.17).

Conclusion

CE-MRA is sensitive in detection of VA ostial stenosis although it has high false-positive rate. True positive ostial stenosis should be considered in cases of coincidental stenoses of MCA on TOF MRA.  相似文献

Objectives

To compare the use of an unenhanced high-resolution time-of-flight MR angiography sequence (Hr-TOF MRA) with fat-suppressed axial/coronal T1-weighted images and contrast-enhanced angiography (standard MRI) for the diagnosis of cervical artery dissection (cDISS).

Methods

Twenty consecutive patients (9 women, 11 men, aged 24–66 years) with proven cDISS on standard MRI underwent Hr-TOF MRA at 3.0 T using dedicated surface coils. Sensitivity (SE), specificity (SP), positive and negative predictive values (PPV, NPV), Cohen’s kappa (к) and accuracy of Hr-TOF MRA were calculated using the standard protocol as the gold standard. Image quality and diagnostic confidence were assessed on a four-point scale.

Results

Image quality was rated better for standard MRI (P?=?0.02), whereas diagnostic confidence did not differ significantly (P?=?0.27). There was good agreement between Hr-TOF images and the standard protocol for the presence/absence of cDISS, with к?=?0.95 for reader 1 and к?=?0.89 for reader 2 (P?<?0.001). This resulted in SE, SP, PPV, NPV and accuracy of 97 %, 98 %, 97 %, 98 % and 97 % for reader 1 and 93 %, 96 %, 93 %, 96 % and 95 % for reader 2.

Conclusions

Hr-TOF MRA can be used to diagnose cDISS with excellent agreement compared with the standard protocol. This might be useful in patients with renal insufficiency or if contrast-enhanced MR angiography is of insufficient image quality.

Purpose

The authors investigated the effectiveness of high-resolution magnetic resonance angiography (MRA) using a single-artery highlighting technique for the accurate diagnosis of intracranial aneurysms at 3.0 T in a large cohort of patients with suspected intracranial aneurysms against the current gold standard, i.e. cerebral angiography.

Methods

A total of 307 patients with suspected aneurysms were referred for three-dimensional time-of-flight MRA (3D-TOF-MRA) prior to digital subtraction angiography (DSA). We summarised patient-based, aneurysm-based and vessel-based diagnostic performance parameters, namely, accuracy, sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for detecting intracranial aneurysms with 3D-TOF-MRA. Interobserver agreement was calculated with the kappa (κ) statistic.

Results

Patient-based accuracy was 98.1%, sensitivity 99.5%, specificity 95.3%, PPV 97.6% and NPV 99% according to observer A; and 97.7%, 99.5%, 94.3%, 97.1% and 99%, respectively, according to observer B. Aneurysmbased accuracy was 98.3%, sensitivity 99.6%, specificity 95.2%, PPV 98.1% and NPV 99% according to observer A, and 98.0%, 99.6%, 94.3%, 97.7% and 99%, respectively, according to observer B. Vessel-based accuracy was 99.0%, sensitivity 99.6%, specificity 98.7%, PPV 97.8%, and NPV 99.7% according to observer A, and 98.9%, 99.6%, 98.5%, 97.3%, and 99.7%, respectively, according to observer B. Interobserver reading differences were not significant. The interobserver agreement was good, with a kappa (κ) value of 0.931 for patient-based evaluation, 0.934 for aneurysmbased evaluation and 0.953 for vessel-based evaluation.

Conclusions

Our results demonstrate the effectiveness of high-resolution MRA using a single-artery-highlighting technique for accurate diagnosis of intracranial aneurysms in a large cohort of patients with suspected aneurysms.  相似文献

Introduction

Novel postprocessing techniques have enabled accurate quantification of intracranial carotid atherosclerotic disease on CT Angiography (CTA). Our purpose was to estimate the prevalence of intracranial carotid artery disease, i.e., stenosis and calcium, on CTA in patients with recent neurological symptoms.

Methods

The degree of stenosis and calcium volume of 162 extracranial and intracranial internal carotid arteries (ICAs) was quantitatively measured on CTA images of 88 consecutive patients with recent neurological symptoms and extracranial ICA stenosis as screened by ultrasound. The prevalence of intracranial ICA stenosis and presence of calcium was estimated and correlated with extracranial ICA stenosis.

Results

Intracranial ICA stenosis was observed in 83 % (95 %CI: 77–89 %) and 39 % (95 %CI: 31–47 %) for a stenosis of ≥30 % and ≥50 %, respectively. Only on the symptomatic side, a statistical significant correlation between intracranial and extracranial stenoses was observed (Pearson's r 0.32, P?=?0.006). In the 37 arteries with an extracranial ICA stenosis of ≥70 %, 89 % (95 %CI: 79–99 %) and 46 % (95 %CI: 30–62 %) of the intracranial ICA showed a stenosis of ≥30 % and ≥50 %, respectively.

Conclusion

In our population of patients with recent neurological symptoms and extracranial stenosis as screened by ultrasound, CTA imaging resulted in a substantially higher prevalence of intracranial ICA disease than previously reported. This remarkably high prevalence of intracranial ICA disease on CTA may have important future implications for acute and preventive treatment strategies.  相似文献

Objective

To evaluate the diagnostic performance of computed tomography angiography (CTA) and contrast-enhanced magnetic resonance angiography (CE-MRA) in detecting haemodynamically significant arterial stenosis or occlusion in patients with critical limb ischaemia (CLI) or intermittent claudication (IC).

Methods

Medline and Embase were searched for studies comparing CTA or CE-MRA with digital subtraction angiography as a reference standard, including patients with CLI or IC. Outcome measures were aortotibial arterial stenosis of more than 50 % or occlusion. Methodological quality of studies was assessed using QUADAS.

Results

Out of 5,693 articles, 12 CTA and 30 CE-MRA studies were included, respectively evaluating 673 and 1,404 participants. Summary estimates of sensitivity and specificity were respectively 96 % (95 % CI, 93–98 %) and 95 % (95 % CI, 92–97 %) for CTA, and 93 % (95 % CI, 91–95 %) and 94 % (95 % CI, 93–96 %) for CE-MRA. Regression analysis showed that the prevalence of CLI in individual studies was not an independent predictor of sensitivity and specificity for either technique. Methodological quality of studies was moderate to good.

Conclusion

CTA and CE-MRA are accurate techniques for evaluating disease severity of aortotibial arteries in patients with CLI or IC. No significant differences in the diagnostic performance of the two techniques between patients with CLI and IC were found.

PURPOSE: This study was undertaken to evaluate the accuracy of contrast-enhanced magnetic resonance angiography (CE-MRA) in detecting renal artery stenosis using intra-arterial digital subtraction angiography (DSA) as the gold standard. MATERIALS AND METHODS: Thirty-five consecutive patients with possible renovascular hypertension were prospectively studied; 26 of them underwent both MRA and DSA. In these 26 cases, two readers assessed the number of renal arteries, the presence of stenoses and their degree. Results were compared with DSA, and sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and diagnostic accuracy of MRA were determined. Interobserver variability was also calculated. RESULTS: DSA showed 51 main renal arteries (one patient had a single kidney) and six accessory arteries (total number of arteries 57) in the 26 patients considered. Both MRA readers detected all of the 51 main renal arteries and only one accessory vessel. When the presence of stenosis was considered, the readers' results, respectively, were as follows: sensitivity 77% and 72%, specificity 69% and 69%, PPV 86% and 85%, NPV 55% and 50% and diagnostic accuracy 75% and 71%. When the detection of significant stenosis was considered, the results, respectively, were: sensitivity 83% and 83%, specificity 73% and 78%, PPV 60% and 65%, NPV 90% and 91%, and diagnostic accuracy 76% and 80%. Interobserver variation was good when considering stenosis detection (kappa=0.69) and excellent when considering detection of significant stenosis (kappa=0.85). CONCLUSIONS: MRA results do not appear as positive as in the majority of papers in the literature. Multiple reasons can probably be invoked to explain this difference. The mean age of our patients, higher than in many other studies, should be noted and may have accounted for their possible poor cooperation. Moreover, all of the missed significant stenoses were distally located, and therefore, the failure to detect them might be related to the suboptimal spatial resolution of MRA. Nevertheless, MRA showed a high NPV for detecting significant stenoses, a finding of considerable clinical relevance in that it allows patients with normal MRA findings to be spared additional more invasive procedures.  相似文献

We evaluated quantification of calcified carotid stenosis by dual-energy (DE) CTA and dual-energy head bone and hard plaque removal (DE hard plaque removal) and compared the results to those of digital subtraction angiography (DSA). Eighteen vessels (13 patients) with densely calcified carotid stenosis were examined by dual-source CT in the dual-energy mode (tube voltages 140 kV and 80 kV). Head bone and hard plaques were removed from the dual-energy images by using commercial software. Carotid stenosis was quantified according to NASCET criteria on MIP images and DSA images at the same plane. Correlation between DE CTA and DSA was determined by cross tabulation. Accuracies for stenosis detection and grading were calculated. Stenosis could be evaluated in all vessels by DE CTA after applying DE hard plaque removal. In contrast, conventional CTA failed to show stenosis in 13 out of 18 vessels due to overlapping hard plaque. Good correlation between DE plaque removal images and DSA images was observed (r 2 = 0.9504) for stenosis grading. Sensitivity and specificity to detect hemodynamically relevant (>70%) stenosis was 100% and 92%, respectively. Dual-energy head bone and hard plaque removal is a promising tool for the evaluation of densely calcified carotid stenosis.  相似文献

目的:探讨3D TRICKS MRA对颈动脉粥样硬化狭窄的诊断价值。方法:对43例临床疑诊颈动脉粥样硬化狭窄的患者行2D TOF MRA和3D TRICKS MRA检查,其中31例同期行DSA检查,以颈总动脉法(CC法)计算狭窄率。结果:颈动脉闭塞2支,重度狭窄10支,中度狭窄31支,轻度狭窄25支,正常18支。2D TOF MRA、3D TRICKS MRA与DSA对照,显示狭窄部位均与DSA所示相符。2D TOF MRA高估狭窄程度6支,3D TRICKS MRA高估狭窄程度1支。以DSA为标准,2D TOF MRA诊断颈动脉狭窄的敏感度、特异度、诊断符合率分别为91.6%,85.7%,90.3%(K=0.737);3D TRICKS MRA诊断颈动脉狭窄的敏感度、特异度、诊断符合率分别为97.9%,100%,98.4%(K=0.955)。两种方法对诊断颈动脉狭窄的敏感度、特异度和诊断符合率差异均具有统计学意义(P〈0.05)。结论:3D TRICKS MRA与DSA对评估颈动脉粥样硬化狭窄具有极好的一致性(K=0.955),明显优于2D TOF MRA(K=0.737,P〈0.05),是对颈动脉粥样硬化狭窄筛查、术前评估、术后随访的最佳检查方法,能基本替代DSA检查。  相似文献

Objective

To quantitatively estimate lumen narrowing and to assess the volume and composition of atherosclerotic plaque with 256-slice computed tomography angiography (CTA), using conventional quantitative coronary angiography (QCA) as the gold standard.

Methods

Twenty-seven consecutive patients with suspected coronary artery disease (CAD) underwent 256-slice CTA and subsequent coronary angiography within 4 weeks. Quantification of lumen narrowing was performed on curved multiplanar reformatted CTA images, in identical projections to those used for QCA. Atherosclerotic plaque volume and composition were assessed by using commercially available software.

Results

The overall correlation between the stenosis severity by QCA compared with CTA was high (r 2?=?0.79, p?<?0.001). For the detection of ≥50% and ≥75% diameter lesions, CTA yielded high sensitivity, specificity and accuracy (86%, 95% and 90%; and 89%, 100% and 96%, respectively), using QCA as the standard reference. Furthermore, assessment of atherosclerotic plaque yielded highly reproducible results (inter-observer and intra-variability of 13% and 9%, respectively, for the assessment of plaque volume, and high agreement between observers (κ?=?0.86) for the differentiation between non-calcified, mixed and calcified plaque).

Conclusions

Clinically indicated 256-slice CT angiography in symptomatic patients can aid both quantification of lumen narrowing and evaluation of atherosclerotic plaque, with high reproducibility.  相似文献

目的:探讨双源CT双能量去骨技术在颈动脉钙化性狭窄病变中的应用价值。方法:回顾性分析18例颈动脉钙化性狭窄患者的病例资料,根据北美症状性颈动脉内膜切除试验(NASCET)标准评价血管的狭窄程度,分析颈动脉双能量去骨技术去除钙化斑块后的图像与DSA图像在量化分析颈动脉狭窄程度上的相关性。以DSA图像为对照,评价双能量去骨技术诊断钙化性颈动脉狭窄的敏感性、特异性、准确性、阳性和阴性预测值。结果:64段有明显致密钙化斑块的颈动脉血管,双能量去骨技术检出54个血管节段狭窄,DSA检出50个血管节段狭窄,两者对于颈动脉钙化性狭窄病变的检出差异无统计学意义(P>0.05),两者对于狭窄程度的分级相关性好(R2=0.913)。以DSA为对照,双能量去骨技术检出颈动脉狭窄的敏感度为98.0%,特异度为100.0%,符合率为90.6%,阳性预测值为90.7%,阴性预测值为90.0%。结论:采用双能量去骨技术去除钙化斑块可以很好地评价颈动脉钙化性狭窄病变。  相似文献

Purpose

This study was performed to validate a highresolution whole-body magnetic resonance angiography (MRA) protocol with parallel imaging and biphasic administration of a single bolus of contrast agent in the preliminary assessment of systemic atherosclerotic burden in patients referred for endovascular procedures.

Materials and methods

Forty patients referred for endovascular treatment of atherosclerotic disease of the carotid arteries (n=23), peripheral vessels (n=14) or aorta (n=3) on the basis of previous clinical and diagnostic examinations underwent high-resolution whole-body MRA at 1.5 T with 3D spoiled gradient recalled echo (GRE) sequences, featuring parallel imaging acquisition technique with ×2 acceleration factor. Sixty-eight surface coil elements and a four-station imaging protocol were employed. Biphasic intravenous administration of a paramagnetic contrast agent [gadolinium benzyloxyproprionic-tetraacetic acid (Gd-BOPTA)] was performed with the following protocol: 10 ml at a speed of 1 ml/s followed by further 10 ml at a speed of 0.5 ml/s. For image analysis, the arterial system was divided into 42 segments for evaluation. The presence or absence of atherosclerotic lesions was evaluated by two observers in consensus; segments were classified as having clinically significant disease (≥50% stenosis or an aneurysmal dilatation) or no significant disease (<50% stenosis). The presence of stenoocclusive disease, determined at all segments, was compared with findings on digital subtraction angiography (DSA), which were interpreted by a third independent reader. Sensitivity, specificity and concordance of whole-body MRA findings with DSA were calculated, and receiver operating characteristic (ROC) analysis was performed for all vascular territories.

Results

A total of 1,680 arterial segments was evaluated; 138 (8.3%) were affected by atherosclerotic alterations. Carotid lesions were confirmed in 23 patients (34 segments), involvement of peripheral vessels in 14 (57 segments) and abdominal aneurysms in three. Sensitivity and specificity of whole-body MRA were, respectively, 95%–97% for head and neck vessels, 100%-100% for thoracoabdominal vessels, 98%-97% for thigh vessels and 84%–88% for calf vessels; concordance with the DSA findings was significant (p<0.05). Subclinical atherosclerotic lesions were evidenced in 25 patients, involving carotid arteries (12 segments), peripheral vessels (21 segments) and abdominal aorta (one segment). All these lesions were confirmed by a second modality, and ten of these patients required further care.

Conclusions

High-resolution whole-body MRA with Gd-BOPTA may be considered a reliable modality for imaging systemic atherosclerosis in candidates for endovascular procedures. The subclinical detection of the total atherosclerotic burden has potential implications for secondary care in this population.  相似文献

Objectives

Multi-station contrast-enhanced magnetic resonance angiography (MRA) is considered as the imaging investigation of first choice in patients suffering from peripheral arterial occlusive disease. In order to overcome venous overlay and to gain dynamic flow information as provided by digital subtraction angiography (DSA), we developed a triple injection protocol for high-resolution MRA of the entire peripheral vascular system, applying time-resolved (TR) four-dimensional (4D) MRA sequences.

Methods

Ten patients underwent three-station TR-MRA of the pelvis and lower extremities with DSA as reference standard. Both investigations were compared concerning stenosis on a segment-by-segment basis. Furthermore, 28 consecutive patients underwent the same MR-only imaging protocol. All images were evaluated concerning image quality (1 = non-diagnostic, 4 = excellent), venous overlay (from none up to substantial) and time to venous enhancement (very early/early/normal/late).

Results

Three-station TR-MRA proved feasible and was comparable with DSA in 282 vessel segments, with underestimation grade of stenosis in four segments and overestimation in four segments, respectively. In 32/38 patients no venous overlay was noted; in six patients there was mild venous overlay. Image quality was rated excellent or good in most cases.

Conclusions

TR-MRA provides morphological and functional information without any timing issues due to optimal arterial enhancement at high spatial resolution without venous overlay.

Objectives

Carotid siphon calcification is often visible on unenhanced head CT (UCT), but the relation to proximal carotid artery stenosis (CAS) is unclear. We investigated the association of carotid siphon calcification with the presence of CAS.

Methods

This IRB-waived retrospective study included 160 consecutive patients suspected of stroke (age 64?±?14 years, 63 female) who underwent head UCT and CTA of the head and neck. CAS was rated on CTA as not present or present with non-significant (<50 %), moderate (50–69 %) or significant (≥70 %) stenosis. Presence, shape (on UCT) and volume (on CTA) of carotid siphon calcifications were related to CAS.

Results

Carotid siphon calcification was absent in 41 % of patients and bilateral in 94 % of those with calcifications. Presence, shape and volume of calcification resulted in odds ratios for having significant CAS of 10.1, 3.9 and 8.4, with 95 % CIs of 1.3–79.6, 1.1–14.1 and 2.6–26.8, respectively. Corresponding NPVs were 0.98, 0.98 and 0.96, while PPVs were 0.14, 0.07 and 0.29, respectively.

Conclusion

Absence of calcification in the carotid artery siphon on UCT has high negative predictive value for carotid artery stenosis in patients with suspected stroke. However, siphon calcification is not a reliable indicator of significant carotid artery stenosis.

Purpose

The purpose of the study was to compare the diagnostic value of color Doppler ultrasonography (CDUS) and multidetector computed tomography (MDCT) angiography against that of digital subtraction angiography (DSA) or surgery in the evaluation of failing hemodialysis arteriovenous fistulas (AVFs).

Materials and methods

CDUS and MDCT angiography were performed with 41 patients (24 men, 17 women; mean age 55.8) with dysfunctional hemodialysis fistulas. The presence of stenosis, thrombosis, aneurysm, pseudoaneurysm and seroma were recorded. The sensitivity, specificity, positive and negative predictive values (PPV and NPV) and accuracy of CDUS and MDCT angiography were calculated both individually and in combination for the detection of vascular segments with significant stenosis, thrombosis, aneurysms, pseudoaneurysms, perivascular complications and stenosis subgroups.

Results

Sixty-four segmental lesions were diagnosed by DSA or surgery. Sensitivity, specificity, PPV, NPV and accuracy of CDUS for all vascular tree lesions were 85.9%, 99.2%, 96.4%, 96.7% and 94.5%, respectively. For MDCT angiography the figures were 96.8%, 99.6%, 98.4%, 99.2% and 98.5%, respectively. When both tests were used in combination, sensitivity, specificity, PPV, NPV and accuracy for all vascular tree lesions rose to 100%.

Conclusion

Combined use of MDCT and CDUS for diagnosis of AVF dysfunctions is of equivalent value to surgery or DSA, a gold standard technique.  相似文献

Objectives

To evaluate the ability of ultrasound non-invasive vascular elastography (NIVE) strain analysis to characterise carotid plaque composition and vulnerability as determined by high-resolution magnetic resonance imaging (MRI).

Methods

Thirty-one subjects with 50 % or greater carotid stenosis underwent NIVE and high-resolution MRI of internal carotid arteries. Time-varying strain images (elastograms) of segmented plaques were generated from ultrasonic raw radiofrequency sequences. On MRI, corresponding plaques and components were segmented and quantified. Associations between strain parameters, plaque composition and symptomatology were estimated with curve-fitting regressions and Mann–Whitney tests.

Results

Mean stenosis and age were 72.7 % and 69.3 years, respectively. Of 31 plaques, 9 were symptomatic, 17 contained lipid and 7 were vulnerable on MRI. Strains were significantly lower in plaques containing a lipid core compared with those without lipid, with 77–100 % sensitivity and 57–79 % specificity (P?<?0.032). A statistically significant quadratic fit was found between strain and lipid content (P?<?0.03). Strains did not discriminate symptomatic patients or vulnerable plaques.

Conclusions

Ultrasound NIVE is feasible in patients with significant carotid stenosis and can detect the presence of a lipid core with high sensitivity and moderate specificity. Studies of plaque progression with NIVE are required to identify vulnerable plaques.
