男性不育症患者精子DNA完整性与精液分析参数相关性研究   总被引:1,自引:0,他引:1  
目的 探讨男性不育症患者精子DNA完整性与精液参数间的相关性.方法 已有生育男性健康者50例(对照组);男性不育症患者100例(不育组),根据精子常规参数检测结果分为参数正常不育组及参数异常不育组.采用计算机辅助精液分析系统分析精液常规和运动参数,采用末端脱氧核苷酸转移酶介导的dUTP末端标记法检测精子DNA碎片化指数(DFI).结果 参数正常不育组和参数异常男性不育组精子DFI均高于对照组(P<0.05),参数异常不育组精子DFI高于参数正常不育组(P<0.05);不育组精子DFI与精子密度、正常形态率、活动率、存活率、a级和b级精子活力及精子运动参数(直线速度、曲线速度、平均路径速度和侧摆幅度)呈不同程度负相关(P<0.05),与c级和d级精子活力呈正相关(P<0.05),与精子直线性、前向性、摆动性和鞭打频率无相关性(P>0.05).结论 精子DFI是可用于评估精液质量和男性生育能力的理想指标.  相似文献

目的通过研究精子DNA碎片指数(DFI)与其他精液参数的相关性,探讨精子DFI在诊断和治疗男性不育中的临床应用价值。方法收集2019年1月至2020年1月在该院生殖中心就诊的959例男性不育患者的精液标本,通过计算机辅助分析系统进行精液常规检测,运用Diff-quik染色法对精液进行染色处理后再作形态学分析,采用精子染色质扩散试验检测精子DFI,根据精子DFI值分组,Ⅰ组精子DFI<30%,Ⅱ组精子DFI≥30%,并对精子DFI与其他精液参数的相关性进行分析。结果Ⅰ组有754例;Ⅱ组有205例。Ⅰ组的精子存活率、前向运动精子百分率(PR)、非前向运动精子百分率(NP)和正常形态精子百分率明显高于Ⅱ组,差异有统计学意义(P<0.05)。Ⅰ组患者的年龄、不动精子百分率(IM)明显低于Ⅱ组,差异有统计学意义(P<0.05)。精子DFI与精子存活率、PR、NP、正常形态精子百分率呈负相关(r=-0.409、-0.402、-0.198、-0.216,P<0.05),而与患者年龄和IM呈正相关(r=0.181、0.402,P<0.05)。结论精子DFI与多项精液参数相关,因此精子DFI检测应与精液常规检测及精子形态学分析相结合,为临床评估男性不育患者的生育力提供可靠且准确的评价指标。  相似文献

目的探讨孕前保健男性年龄与精子DNA碎片、精液常规参数之间的临床关联。方法收集2018年5月至2019年6月于深圳市宝安区妇幼保健院生殖健康科就诊的孕前保健检查男性1237例,依据年龄截点将其分为3组(20~29岁,30~39岁和≥40岁)。采用染色质结构分析实验检测精子DNA碎片指数(DFI)、精子成熟度,同时依据世界卫生组织第五版标准操作规程检测各项精液常规临床参数。结果≥40岁男性的前向运动精子百分率、精子存活率与20~29岁男性相比,差异均有统计学意义(P<0.05)。≥40岁男性的前向运动精子百分率、精子存活率、精液体积、不动精子百分率与30~39岁男性相比,差异均有统计学意义(P<0.05)。同时≥40岁男性精子DFI显著高于20~29岁、30~39岁的男性(P<0.001),且DFI与年龄呈正相关关系(r=0.11,P<0.0001),尤表现在≥40岁男性精子DFI与年龄呈显著性正相关关系(r=0.18,P<0.0001)。男性精子DFI与正常形态精子百分率、精液浓度、前向运动精子百分率、精子存活率、非前向运动精子百分率呈负相关关系(r=-0.21、-0.11、-0.45、-0.45、-0.21,均P<0.05);而与禁欲天数、精液体积、不动精子百分率、头部畸形精子百分率呈正相关关系(r=0.14、0.13、0.46、0.21,均P<0.05)。年龄、精液体积、正常形态精子百分率、前向运动精子百分率、精子成熟度、非前向运动精子百分率和不动精子百分率是男性精子DNA碎片的独立风险因素。结论年龄影响精子活力,精子DNA碎片显著增加;男性精子DFI与其他精液常规参数呈紧密相关,可作为一项重要的男性精子质量评价指标,为全面客观评价孕前保健男性生育能力提供了有利证据。  相似文献

目的:探讨精子DNA碎片指数(DNA fragmentation index, DFI)与精子形态学之间的相关性。方法:回顾性分析3 068例男性患者的精液检查结果,将精子DFI分为<15%、15%~30%、>30%共3组,再根据精子形态学检查结果,按照正常形态精子所占比例<4%、4%~10%、>10%将精子分为3组,分析精子不同形态学分组与精子DFI分组间的关系。结果:不同精子DFI分组的正常形态、头部畸形、混合畸形百分比差异无统计学意义(P均>0.05);不同的精子正常形态率分组间的精子DFI差异无统计学意义(P均>0.05),用Spearson相关性分析精子DFI与精子形态之间的相关性,结果发现两者间无相关性。结论:本研究发现精子DFI与精子形态学之间没有相关性。  相似文献

目的分析精子形态、DNA碎片指数(DFI)和精浆锌对体外受精-胚胎移植(IVF-ET)妊娠结局的影响。方法选取2015年6月至2019年2月于我院生殖中心行IVF-ET治疗的278例夫妇为研究对象,依据临床妊娠结局将其分为妊娠组(92例)与未妊娠组(186例)。比较两组的一般资料、精液参数及DFI。结果两组的年龄、不育年限、BMI、获卵数、避孕时间、鲜胚移植周期比较,差异无统计学意义(P>0.05)。妊娠组IVF、ICSI周期的精子浓度、前向活动力、精浆锌水平均高于未妊娠组,DFI低于未妊娠组;妊娠组ICSI周期的精子畸形率低于未妊娠组(P<0.05)。结论精子形态、DFI及精浆锌对IVF-ET妊娠结局有明显影响,且DFI可影响精子前向运动,导致精子畸形,影响早期胚胎发育。  相似文献

目的通过比较精索静脉曲张不育患者与健康婚育检查人士各项精液分析指标水平,探讨精索静脉曲张不育患者各项精液分析指标的特征,并分析精子DNA碎片指数(DFI)与其他精液分析指标的相关性,以及各项指标联合检测在精索静脉曲张不育中的诊断效能。方法将该院2019年经B超确诊为精索静脉曲张,并同时进行精子DNA完整性检查、精液常规参数分析和精子形态分析的107例不育患者纳入精索静脉曲张组,选择同期60例健康婚育检查人士纳入健康对照组。检测并比较两组研究对象精子DFI、高DNA可染性(HDS)、精子浓度、前向运动精子(PR)、正常形态精子百分比(PNS)、畸形精子指数(TZI)、精子畸形指数(SDI),分析精子DFI与其他精液分析指标的相关性,绘制受试者工作特征(ROC)曲线,判断联合检测在精索静脉曲张不育患者中的诊断效能。结果两组精子DFI、HDS、精子浓度、PR、PNS、TZI、SDI比较,差异均有统计学意义(P<0.05)。精子DFI与HDS、TZI、SDI呈正相关(r=0.530、0.454、0.306,P<0.05),与PR、PNS呈负相关(r=—0.559、—0.245,P<0.05),与精子浓度无明显相关性(r=—0.112,P>0.05)。多因素Logistic回归分析显示,精子DFI与HDS是精索静脉曲张不育的独立危险因素[精子DFI(OR=1.179,95%CI:1.094~1.271,P<0.05)、HDS(OR=1.150,95%CI:1.029~1.286,P<0.05)]。ROC曲线分析显示,各项指标联合检测ROC曲线下面积(AUC)为0.871(95%CI:0.819~0.924,P<0.05),灵敏度为71.0%,特异度为95.0%,阳性预测值为95.0%,阴性预测值为65.5%,约登指数为0.660,诊断效能均优于单独检测。结论精索静脉曲张不育患者精子DNA完整性、常规精液分析结果与健康人有差异,精子DFI与部分精液分析指标存在相关性,精子DFI与其他精液常规分析参数联合检测能为临床筛查和治疗精索静脉曲张不育提供更好的依据。  相似文献

目的探究精子DNA完整性检测相对传统的精液常规在男性生育力评价中的辅助预估价值。 方法采集收集2017年4月至10月共1076例不育男性精液标本,男性患者来源于安徽医科大学第一附属医院妇产科,每份标本分成2份,分别行精液常规及精子DNA完整性检测,根据精子DNA碎片指数(DFI)分为精子DNA完整性良好组(DFI≥30%)、完整性一般组(15%6/ml)及少精症(<15×106/ml),按照精子前向活动率分为正常(≥32%)及弱精子症(<32%),采用χ2检验分析不同精子DNA完整性组之间的年龄、浓度和前向活动率的差异,采用Pearson相关分析分析患者年龄、精子浓度及前向活动率分别与精子DFI的相关性。 结果不同精子DNA完整性组间,精子浓度正常与少精症水平差异具有统计学意义(χ2=96.82,P<0.001)、活力正常与弱精子症水平差异均具有统计学意义(χ2=2.06,P<0.001)。年龄与DFI成正相关(r=0.154,P<0.001),精子浓度及前向活动率与DFI成负相关(r=-0.231,P<0.001;r=-0.564,P<0.001)。 结论精子DFI与精子浓度及活动率呈负相关,与年龄呈正相关,精子DNA完整性检测一定程度上可以弥补精液常规参数分析在评价男性生育能力上的不足,具有重要参考价值。  相似文献

BackgroundSperm DNA fragmentation measured by different techniques make comparisons impossible due to lack of standardization. Induction of DNA damage after sperm preparation in the entire fraction has been observed on independent occasions but findings are not consistent.MethodsMen presenting at a University hospital setup for infertility treatment. DNA damage via TUNEL assay was validated on fresh semen samples, as conventional semen parameters, to reduce variability of results.ResultsSperm motility in neat semen inversely correlated with sperm DNA fragmentation in the total fraction, but, total count, leukocytes and immature germ cells significantly affected the vital fraction. Sperm DNA fragmentation was observed both in normal and subnormal semen samples, but was significantly different in the total fraction of astheno-, asthenoterato- and oligoteratozoospermic men. After density gradient centrifugation, sperm DNA fragmentation increased significantly in the total but decreased in the vital fraction. Advancing male age significantly influenced damage in the total but not in the vital population.ConclusionsThese findings provide opportunities to investigate the significance of the total and the vital fractions both in natural conception and after different assisted reproductive technologies.  相似文献

目的分析腹腔镜精索静脉高位结扎术对单侧精索静脉曲张(VC)伴不育患者精液参数的影响。方法选取2014年3月至2016年3月该院收治的单侧VC伴不育患者56例,均行腹腔镜精索静脉高位结扎术。观察并对比术前及术后3个月乳酸脱氢酶同工酶X(LDH-X)在精子及精浆中的含量、精子碎片率(DFI)及其他精子参数。结果腹腔镜精索静脉高位结扎术前精浆与全精子LDH-X的比值、DFI及精浆LDH-X明显高于术后,术前精子LDH-X低于术后,差异均有统计学意义(P0.05);腹腔镜精索静脉高位结扎术前患者精子含量、精子活动率、精子存活率、前向运动精子百分率低于术后,差异有统计学意义(P0.05)。结论单侧VC伴不育患精液的质量、LDH-X的含量和DFI可以通过腹腔镜精索静脉高位结扎术得到积极的改善。  相似文献

目的探讨精子形态学分析在诊断男性不育症中的应用。方法选择2008年7月1日至2010年6月29日在英德市妇幼保健院就诊的不孕夫妇,男方至少行精液分析2次,在第一次精液分析时即进行精子形态学分析。精液分析采用JZF-Ⅰ型精液分析系统,精子形态学分析采用改良巴氏法染色,应用Microsort Excel进行分析。结果进行精液分析共402例次,进行精子形态学分析共173例次。精子密度0~180×109/L,平均(55.3±32.2)×109/L,75.1%的病例精液密度为20~100×109/L,仅13.3%为少精症,无精症占1.5%;a级精子为(14.0±9.9)%,b级精子为(10.6±7.8)%,a级+b级大于50%者仅占0.6%,超过半数的为30%~50%(51.1%),精子形态学分析,仅有20.5%病例是正常的(>14%)。结论无精症显然与不育相关,精子活动力低下或精子畸形率过高更能预示不孕因素,精液分析和精子形态学分析可以作为男性不育的首要筛查手段,以防止过度治疗。  相似文献

目的探讨不同类型人类乳头瘤病毒(HPV)感染对汉中地区不育男性精液质量和精子DNA完整性的影响。方法将我院收治的326例不育男性患者根据PCR-膜杂交法检测精液HPV感染状况分为HPV高危型感染组、HPV低危型感染组和HPV未感染组。分析HPV检测阳性精液标本的各种亚型分布情况;比较三组患者的精液质量和DNA完整性。结果汉中地区不育男性的精液HPV感染以高危型为主。三组的正常精子形态率、精子DNA碎片指数比较,差异具有统计学意义(P<0.05)。HPV高危型感染组的正常精子形态率低于HPV低危型感染组及HPV未感染组,精子DNA碎片指数高于HPV低危型感染组及HPV未感染组,且HPV低危型感染组劣于HPV未感染组(P<0.05)。结论HPV高危型感染对精子正常形态和DNA完整性具有显著影响。  相似文献

目的探讨不育男性生殖道沙眼衣原体感染对精子核DNA碎片化指数的影响。方法选择2017年1月至2017年5月在成都市锦江区妇幼保健院生殖医学中心(成都西囡妇科医院)门诊初诊的117例生殖道沙眼衣原体感染的不育男性(感染组)和92例体检正常的男性(对照组),对两组的精液常规参数及精子核DNA完整性(以精子核DNA碎片化指数DFI表示)检测结果进行回顾性分析。结果⑴感染组的精子浓度低于对照组,但差异无统计学意义(P>0.50);感染组的前向运动精子百分比低于对照组,但差异无统计学意义(P>0.20);⑵与对照组相比,感染组的精子DFI值显著升高,差异有统计学意义(P<0.05)。结论生殖道感染沙眼衣原体的男性不育患者的精子核DNA损伤程度明显高于未感染者。精子核DNA碎片化指数可能是评估男性精液质量的独立指标。  相似文献

目的探讨吸烟对男性精液质量及精子核DNA完整性的影响。方法 120例男性不育患者根据吸烟习惯和烟龄分为3组:A组日吸烟量小于10支,且烟龄不超过1年;B组日吸烟量10~20支,且烟龄5~10年;C组日吸烟量大于或等于20支,且烟龄大于或等于10年。选用35例已生育健康男性作为对照。对其精液常规参数及DNA片段化指数(DFI)值进行检测。结果不育吸烟组精子活力显著低于不育非吸烟组,DFI值显著增高;A、B、C3组比较,C组精子活力显著低于B组和A组,DFI值显著增高。结论吸烟可以破坏精子核DNA完整性,严重影响精液质量。  相似文献

Introduction: Sperm DNA damage is the major molecular cause of male infertility, which has a negative effect on reproductive outcomes in couples. Sperm DNA damage originates either during production/maturation or transport of spermatozoa through male genital tract. Though several assays have been used to assess the sperm chromatin integrity and sperm DNA fragmentation (SDF), routine application of SDF testing in semen analysis is generally not reinforced by professional societies. SDF testing is now emerging as a valuable tool and recent clinical practice guidelines (CPG) published by the Society for Translational Medicine recommends SDF testing in various clinical scenarios.

Areas covered: This review discusses the origin and factors contributing to sperm DNA damage, the molecular changes, especially proteomic alterations caused due to SDF, risk factors associated with SDF, methods used to analyze SDF, clinical implications of SDF, and CPG recommendations for SDF testing.

Expert opinion: Recent clinical practice recommendations suggest the potential role of SDF testing in specific clinical scenarios. This would expand the horizon of SDF testing globally as a prognostic and diagnostic tool in various male infertility scenarios and their treatment management.  相似文献

BackgroundChromosome translocation is a genetic factor associated with male infertility. However, cases of Y chromosome/autosome translocation are rare. Individuals with translocation between the Y chromosome and an autosome have a variety of different clinical phenotypes. There is a need for further study of molecular cytogenetic feature of those with Y chromosome translocation.MethodsWe reported that an apparently healthy 31‐year‐old man, 168 cm tall and weighing 65 kg, had a 2‐year history of primary infertility after marriage. Clinical diagnostic techniques included semen analysis, hormone measurements, cytogenetic analysis, fluorescence in situ hybridization (FISH), and high‐throughput multiplex ligation‐dependent probe amplification semiconductor sequencing. Detailed genetic counseling was provided to the patient. Intracytoplasmic sperm injection treatment combined with preimplantation genetic diagnosis was chosen with the aim of achieving a successful pregnancy.ResultsSemen analysis revealed cryptozoospermia. Hormone levels were within the normal limits. Sequencing results indicated the presence of the sex‐determining region on Yp, and AZFa, AZFb, and AZFc regions on Yq. The patient''s karyotype was 45,X,psu,dic(Y;14)(p11.3;q11.2), which was confirmed by cytogenetic analysis and FISH.ConclusionThis study reports a case of cryptozoospermia in a male patient with a Y;14 chromosomal translocation. When clinical karyotyping has revealed potential Y chromosome abnormality, FISH or molecular detection should be further performed to facilitate identification of the chromosomal breakpoint.  相似文献

Semen analysis is a basic step in the investigation of several disturbances affecting the male genital tract. Analysis of seminal parameters provides important clinical information on the spermatogenesis and functional competence of spermatozoa, as well as on the secretory pattern of the accessory genital glands. Semen analysis is particularly useful in the evaluation of couples requiring fertility investigation (to detect genital infections and pathologies) and in verifying the influence of environmental factors, drugs, lifestyle, chemical products, and professional activities on several diseases affecting male reproductive health. Measure of semen quality is of substantial interest for diagnoses in clinical urology, andrology, and gynecology. Currently, basic requirements for semen analysis are standardized by World Health Organization (WHO) guidelines that describe several procedures for an objective evaluation of the semen quality with diagnostic purposes. These guidelines include: parameters for the physical and biochemical evaluation of semen; parameters for the analysis of sperm characteristics; and other seminal parameters that can be easily adopted in any laboratory. This report summarizes current concepts on semen analysis and the significance of the seminal parameters for reaching a diagnosis based on the procedures recommended by WHO guidelines.  相似文献

目的观察分析不孕不育家庭男方精液检查结果,了解长沙地区不孕不育家庭男方精液质量状况。方法选取2015年1月至2018年12月长沙市妇幼保健院爱心助孕义诊下乡活动中不孕不育家庭前来就诊的182例男性作为不育组,选取同期在该院行体外受精-胚胎移植术(IVF)的445例男性作为IVF组,另选取同期孕前检查结果均正常已正常生育的272例男性作为正常组,统计分析各组精液参数。结果 3组间进行两两比较,不育组年龄、精液pH值、精液液化时间、精子浓度、精子总数、精子总活力、精子前向运动百分率和精子畸形率与IVF组结果相近,差异均无统计学意义(P>0.05);不育组精液量低于IVF组,差异有统计学意义(P<0.05);不育组精液量、精子浓度、精子总数、精子总活力和精子前向运动百分率均低于正常组,精子畸形率高于正常组,差异均有统计学意义(P<0.05);正常组精液量、精子浓度、精子总数、精子总活力和精子前向运动百分率均明显高于IVF组,差异均有统计学意义(P<0.05)。结论不育组男性年龄和精液参数与IVF组相近,且两组年龄均大于正常组,两组精液参数明显劣于正常组,精子质量异常是不孕不育家庭男性不育的主要原因。  相似文献

BackgroundGastrin‐17 (G‐17) and Helicobacter pylori (H pylori) antibody are widely used in the screening of gastric diseases, especially in gastric cancer. In this study, we aimed to evaluate the value of G‐17 and H pylori antibody in gastric disease screening.MethodsHealthy males and females (1368 and 1212, respectively) aged between 21‐80 years were recruited for the study. Serum G‐17 value was measured using ELISA, and H pylori antibodies were measured using Western blotting. Statistical analyses were performed using the chi‐square, Mann‐Whitney U, and Kruskal‐Wallis H tests.ResultsSerum G‐17 level was higher in the H pylori‐positive group than in the negative group. Serum G‐17 level was higher in the type 1 H pylori‐positive group than in the type 2 H pylori‐positive group. Further, serum G‐17 level was higher in females than in males and showed significant differences among different age‐groups, with changes in trend proportional to the age. The positive rate of H pylori infection in all the subjects was 58.29% and did not show a significant difference between males and females. However, it showed significant differences among different age‐groups, with the changing trend proportional to the age.ConclusionAnalysis of serum G‐17 level and H pylori antibody typing is valuable in gastric disease screening. Every laboratory should establish its own reference interval for G‐17 level.  相似文献

目的观察非细菌性前列腺炎对精子顶体酶活性及精液参数的影响,并探讨其在男性不育中的意义。方法检测100例男性不育合并慢性非细菌性前列腺炎患者(观察组)及42例正常生育男性(对照组)的精液参数及精子顶体酶活性并进行对比分析。结果观察组前列腺炎患者精子顶体酶活性、活精子率及a b级精子百分率与对照组相比,差异具有显著性(P<0.01),而精液pH、密度及畸形率两组间差异无显著性。结论非细菌性前列腺炎可以通过改变精液质量,降低精子活力及受精能力而导致不育。  相似文献

Background and objective(S)Tribulus terrestris L. is an herbal plant that has long been used as sex stimulant and to treat male infertility. This systematic review collected the clinical trials and/or quasi-experimental studies on the effect of T. terrestris on sperm parameters in idiopathic male infertility.Materials and methodsTo search the related articles, Cochrane Library, EMBASE, ProQuest, Clinicaltrial.gov, WHO, Google Scholar, MEDLINE via Pubmed, Web of Science, SID, Magiran, Irandoc, and Iranmedex databases were used without any time limitation. Words used to search were T. terrestris L., Tribestan, male infertility and sperm parameters based on the MeSH glossary. To assess the eligibility of the articles, the views of the two authors and in cases where there was no agreement, the third person was used.ResultsThrough searching of the databases, 5775 articles were identified, of which 3509 were entered after removing the duplicates. Afterward, 102 articles were screened for inclusion. Finally, 7 articles were included in this systematic review. Only one quasi-experimental without control arm article reported that the use of T. terrestris L. in the treatment of idiopathic male infertility was not effective, In the others, T. terrestris L. was reported to be effective in improving some or all parameters of the sperm, namely number, motility and morphology.ConclusionThe results of the present systematic review showed that the consumption of T terrestris L., in general, resulted in the improvement of sperm parameters. This result was obtained from 6 out of the 7 articles.  相似文献
