A mortality study was conducted in workers with at least 90 days' exposure to polychlorinated biphenyls (PCBs) between 1946 and 1977. Vital status was established for 98.7% of the 7075 workers studied. In hourly male workers, the mortality from all cancers was significantly below expected (standardized mortality ratio [SMR] = 81; 95% confidence interval [CI], = 68 to 97) and comparable to expected (SMR = 110; 95% CI, 93 to 129) in hourly female workers. No significant elevations in mortality for any site-specific cause were found in the hourly cohort. All-cancer mortality was significantly below expected in salaried males (SMR = 69; 95% CI, 52 to 90) and comparable to expected in salaried females (SMR = 75; 95% CI, 45 to 118). No significant elevations were seen in the most highly exposed workers, nor did SMRs increase with length of cumulative employment and latency. None of the previously reported specific excesses in cancer mortality were seen. This is the largest cohort of male and female workers exposed to PCBs. The lack of any significant elevations in the site-specific cancer mortality of the production workers adds important information about human health effects of PCBs.  相似文献

We evaluated mortality among subjects employed in an oil refinery plant in Rome, Italy. We studied two subgroups of refinery employees: blue collar and white collar workers. A total of 682 men (505 blue collars, 148 white collars, 29 unknown) employed between 1965 and 1992, were followed up for mortality since employment in the plant to July 1999. Standardized Mortality Ratios (SMR), and their 90% Confidence Intervals (90% CI), comparing mortality rates of the cohort members with those of the general population of the Lazio region. For blue collar workers, we performed analyses by latency since first employment and by duration of employment for selected cancer sites. We observed 94 total deaths (100.8 expected) (SMR = 0.93; 90% CI = 0.78-1.11) among blue collar workers and 16 total deaths (31.7 expected) (SMR = 0.50; 90% CI = 0.32-0.77) among white collar workers, and a large deficit of deaths from non-neoplastic cardiovascular diseases (respectively SMR = 0.60; 90% CI = 0.41-0.86 and SMR = 0.18; 90% CI = 0.03-0.56). All cancer mortality was slightly increased only in blue collars (SMR = 1.27 CI = 0.97-1.65). There was an excess risk from cancer of the lung (20 obs SMR = 1.80, 90% CI = 1.19-2.62), bladder (5 obs SMR = 3.19, 90% CI = 1.26-6.72), and benign/unspecified cancer of the brain (4 obs SMR = 4.11, 90% CI = 1.12-10.6). The lower mortality from cardiovascular disease indicates the presence of a strong healthy worker effect. The findings of elevated mortality from cancer of the lung and bladder in blue collar workers are in agreement with those of other studies. Confounding factors from cigarette smoking might have played only a marginal role in influencing the results. Exposure to polynuclear aromatic hydrocarbons (PAHs) is a plausible explanation for the excesses found.  相似文献

Mortality through 1988 was studied for 5,932 male employees who worked between January 1, 1946 and December 31, 1967 at a New Jersey plastics manufacturing and research and development facility. The cohort was followed for an average of 32 years and included 1,859 deaths. Potential exposures included asbestos, formaldehyde, and polyvinyl chloride (PVC). Mortality rates for the cohort were compared to both U.S. and state mortality rates, and analyses were also performed by lagging duration of employment. Based on U.S. rates, mortality among hourly males (n = 3,853) from all cancers was similar to expected [standardized mortality ratio (SMR), 102; 95% confidence interval (CI), 92-114]. Excess mortality among hourly workers was seen for pancreatic cancer (SMR, 146; 95% CI, 95–216) and “malignancies of other parts of the respiratory system” (SMR, 373; 95% CI, 121–870). The latter excess was due entirely to five deaths from pleural mesothelioma. There were no deaths identified due to nasal cavity or nasopharyngeal cancers, or angiosarcoma of the liver. Mortality from leukemia among research and development workers (n = 1,421) was significantly elevated (SMR, 265; 95% CI, 115–524) and related to assignment to process development. This study verifies the excess of pancreatic cancer among workers at the facility seen in earlier studies and observes excesses of mesothelioma due to asbestos exposure and leukemia in process development workers.  相似文献

BACKGROUND: We expanded an existing cohort of workers (n = 2,588) considered highly exposed to polychlorinated biphenyls (PCBs) at two capacitor manufacturing plants to include all workers with at least 90 days of potential PCB exposure during 1939-1977 (n = 14,458). Causes of death of a priori interest included liver and rectal cancers, previously reported for the original cohort, and non-Hodgkin lymphoma (NHL), melanoma, and breast, brain, intestine, stomach, and prostate cancers, based on other studies. METHODS: We ascertained vital status of the workers through 1998, and cumulative PCB exposure was estimated using a new job exposure matrix. Analyses employed standardized mortality ratios (SMRs; U.S., state, and county referents) and Poisson regression modeling. RESULTS: Mortality from NHL, melanoma, and rectal, breast, and brain cancers were neither in excess nor associated with cumulative exposure. Mortality was not elevated for liver cancer [21 deaths; SMR 0.89; 95% confidence interval (CI), 0.55-1.36], but increased with cumulative exposure (trend p-value = 0.071). Among men, stomach cancer mortality was elevated (24 deaths; SMR 1.53; 95% CI, 0.98-2.28) and increased with cumulative exposure (trend p-value = 0.039). Among women, intestinal cancer mortality was elevated (67 deaths; SMR 1.31; 95% CI, 1.02-1.66), especially in higher cumulative exposure categories, but without a clear trend. Prostate cancer mortality, which was not elevated (34 deaths; SMR 1.04; 95% CI, 0.72-1.45), increased with cumulative exposure (trend p-value = 0.0001). CONCLUSIONS: This study corroborates previous studies showing increased liver cancer mortality, but we cannot clearly associate rectal, stomach, and intestinal cancers with PCB exposure. This is the first PCB cohort showing a strong exposure-response relationship for prostate cancer mortality.  相似文献

OBJECTIVES: Mortality from diseases of the nervous system and nonmalignant diseases of the respiratory and genitourinary systems was examined for workers exposed to styrene. METHODS: Altogether 15,826 styrene-exposed workers in 30 plants in the reinforced plastics and composites industry were included. Vital status was ascertained through 31 December 1989. Individual exposure estimates were developed based on job functions, existing industrial hygiene data, process changes, engineering controls, work practices, and the use of personal protective equipment. Analyses were based on cause-specific standardized mortality ratios (SMR) and the Cox proportional hazards model. Mortality data were analyzed by latency, duration of exposure, average exposure, cumulative exposure, and process category. RESULTS: For diseases of the nervous system, the SMR was 0.56 [95% confidence interval (95% CI) 0.31-0.95]. Mortality from nonmalignant genitourinary diseases was not increased (SMR 0.87, 95% CI 0.46-1.50). Latency, duration of exposure, average exposure, cumulative exposure, and process category showed no association between styrene exposure and these 2 types of disease. A small increase in mortality from nonmalignant respiratory diseases was found (SMR 1.21, 95% CI 0.98-1.47), mainly due to "other nonmalignant respiratory diseases" (SMR 1.40, 95% CI 1.04-1.84). The highest increase occurred for short exposure duration (SMR 1.79 for <1 year's exposure) or low exposure (SMR 2.15 for <10 ppm-years); there were no increased risks in the high exposure categories. The Cox proportional hazard model revealed no association between styrene exposure and the diseases. CONCLUSIONS: No relationship was found between mortality from any of the diseases examined and any of the styrene exposure indices. The findings were compared with those reported in a European study of styrene-exposed workers.  相似文献

A cohort study was conducted to evaluate the mortality pattern among female and male workers in a cigarette factory. The study followed 972 female workers and 761 male workers with at least 6 months of cumulative employment from 1 January 1962 through 1 July 1996. Among women, mortality from all causes of death [standardized mortality ratio (SMR) = 0.9; 95% confidence interval (CI) = 0.8-1.1] and mortality from all malignant neoplasms (SMR = 1.1; CI = 0.9-1.3) were consistent with reference rates. Male workers had a significantly reduced overall mortality (SMR = 0.8; CI = 0.7-0.9), while mortality from all malignant neoplasms was as expected (SMR = 0.9; CI = 0.7-1.0). Among female workers the frequencies of deaths from diseases of nervous system (SMR = 2.0; CI = 1.1-3.4) and from non-Hodgkin's lymphoma (NHL) (SMR = 2.7; CI = 1.0-5.6) were elevated at a statistically significant level. No association between duration of employment and diseases of nervous system was observed. A higher risk for NHL, based on three deaths, was reported among female workers with 15 or more years of employment (SMR = 8.1; CI = 2.2-21.0). Although based on small numbers, the excess of NHL here reported suggests that potential exposure to foliar residues of pesticides should be thoroughly considered in tobacco manufacturing.  相似文献

Cancer mortality in a cohort of male German iron foundry workers   总被引:1,自引:0,他引:1  
BACKGROUND: Observations of an increased incidence of cancers of the upper aero-digestive tract (pharynx, esophagus, larynx, lung) among workers of local German foundries gave rise to concern about a potentially elevated occupational risk of those cancer sites. The purpose of the study was to examine whether occupational exposure in iron foundries increases the risk of cancer. METHODS: A historical cohort study of 17,708 male German production workers in 37 iron foundries who were first employed in 1950-1985 with a minimum employment period of 1 year was initiated. Employment and occupational histories were collected. Mortality was compared with that of the German general population during 1950-1993 using a new method for computing the SMR when not all causes of death are available (called SMR*). RESULTS: Mortality from all causes was elevated to SMR = 115.4 (95% confidence interval (CI) = 111.9-119.1), as was for total cancer (SMR* = 123.8, CI = 102.1-152.6), especially cancers of the lung (SMR* = 163.9, CI = 123.9-223.0) and liver (SMR* = 322.5, CI = 149.5-844.8), and diseases of the respiratory system (SMR* = 147.6, CI = 100.4-221.5). Non-significant elevations of mortality were also found for cancers of the mouth and pharynx (SMR* = 153.5, CI = 82.3-359.8) and larynx (SMR* = 173.1, CI = 85.5-550.5). Mortality from various causes of death was higher among workers with shorter exposure periods than among long-term employees. The elevated mortality persisted for years and decades after termination of employment. CONCLUSIONS: The results provide further evidence for an increased risk of lung cancer and possibly other cancers of the upper aero-digestive tract among foundry workers. Special attention should be paid to the strongly increased mortality from liver cancer and the mortality pattern among employees having terminated work.  相似文献

This paper reports the mortality experience from 1948 to 1989 of 2,504 maintenance employees who had a minimum of one year of employment in jobs with potential exposure to asbestos at a Texas refinery and petrochemical plant. For the purposes of this study, “potential exposure” is equated with those jobs or crafts having the greatest direct potential proximity to, or which worked directly with, asbestos-containing materials, especially asbestos-containing thermal insulation. Approximately one-half of the study population had 10 years or longer potential exposure, and 80% had their first potential exposure before 1970. The total population exhibited significantly lower mortality for all causes, the standardized mortality ratio (SMR = 77); and for all cancer (SMR = 85), as compared to residents in the surrounding communities. Statistically significant deficits in mortality were also observed in a number of noncancerous diseases such as heart disease (SMR = 78; 95% CI = 69-88), nonmalignant respiratory disease (SMR = 70; 95% CI = 50-95), and cirrhosis of the liver (SMR = 44; 95% CI = 22-79). Mortality among employees who had 20 years or longer since their first potential exposure was also examined; the pattern of mortality was similar to that exhibited by the total cohort, with a slight increase in the SMR for most of the causes. The only statistically significant excess of mortality found was a fourfold increase in mesothelioma (5 observed and 1.2 expected deaths); the SMR was 428 (95% CI = 139-996) for the total cohort and was 469 (95% CI = 152-1093) for those who had 20 years or more since first potential exposure. In contrast to asbestos industry worker studies, mortality for lung cancer was substantially lower than the general population (SMR = 81; 95% CI = 63-103). The observed number of deaths for cancer of the larynx was virtually the same as expected (3 observed vs. 2.8 expected). This study also showed decreased mortality for cancers of gastrointestinal organs such as the esophagus (SMR = 78), stomach (SMR = 63), large intestine (SMR = 91), rectum (SMR = 55), or pancreas (SMR = 90)—cancers that have been reported to be elevated in studies of various industry workers directly exposed to asbestos. © 1996 Wiley-Liss, Inc.  相似文献

OBJECTIVES: Concern about the health hazards of exposure to workers in the ferroalloy industry has initiated this historical cohort study. The aim was to examine the mortality pattern among male employees in 12 Norwegian ferroalloy plants. METHODS: All men employed for at least six months who started their first employment during 1933-91 were eligible for the cohort. Deaths observed during 1962-90 were compared with expected figures calculated from national mortalities. Internal comparisons of rates were performed by Poisson regression analysis. The final cohort comprised 14,730 male employees who were observed for 288,886 person-years. RESULTS: Mortality from all causes of death was slightly increased (3390 deaths, standardised mortality ratio (SMR) 1.08, 95% confidence interval (95% CI) 1.04-1.11). Regression analysis of total mortality showed a significant negative trend for the rate ratios with increasing duration of employment. An increased mortality was found among employees in urban plants compared with employees in rural plants (rate ratio (RR) 1.21, 95% CI 1.13-1.29). Excess deaths from cancer (SMR 1.11) and sudden death (SMR 1.47) were found among employees with at least three years of employment. Mortality from accidents, poisonings, and violence was increased among all employees (SMR 1.28). Excess deaths from this cause were however only found for the time after the end of employment in this industry and not during employment (SMR 0.90). CONCLUSIONS: The increased mortality from cancer and sudden death could be related to work exposures, at least in subgroups, and these results warrant further studies. The excess deaths from accidents, poisonings, and violence were probably not related to work exposures. The mortality results for short term workers and other information indicate that systematic errors contribute to the increased overall mortality.  相似文献

An Indiana capacitor-manufacturing cohort (n=3,569) was exposed to polychlorinated biphenyls (PCBs) from 1957 to 1977. The original study of mortality through 1984 found excess melanoma and brain cancer; other studies of PCB-exposed individuals have found excess non-Hodgkin lymphoma and rectal, liver, biliary tract, and gallbladder cancer. Mortality was updated through 1998. Analyses have included standardized mortality ratios (SMRs) and 95% confidence intervals (CIs) using rates for Indiana and the United States, standardized rate ratios (SRRs), and Poisson regression rate ratios (RRs). Estimated cumulative exposure calculations used a new job-exposure matrix. Mortality overall was reduced (547 deaths; SMR, 0.81; 95% CI, 0.7-0.9). Non-Hodgkin lymphoma mortality was elevated (9 deaths; SMR, 1.23; 95% CI, 0.6-2.3). Melanoma remained in excess (9 deaths; SMR, 2.43; 95% CI, 1.1-4.6), especially in the lowest tertile of estimated cumulative exposure (5 deaths; SMR, 3.72; 95% CI, 1.2-8.7). Seven of the 12 brain cancer deaths (SMR, 1.91; 95% CI, 1.0-3.3) occurred after the original study. Brain cancer mortality increased with exposure (in the highest tertile, 5 deaths; SMR, 2.71; 95% CI, 0.9-6.3); the SRR dose-response trend was significant (p=0.016). Among those working >or= 90 days, both melanoma (8 deaths; SMR, 2.66; 95% CI, 1.1-5.2) and brain cancer (11 deaths; SMR, 2.12; 95% CI, 1.1-3.8) were elevated, especially for women: melanoma, 3 deaths (SMR, 5.99; 95% CI, 1.2-17.5); brain cancer, 3 deaths (SMR, 2.87; 95% CI, 0.6-8.4). These findings of excess melanoma and brain cancer mortality confirm results of the original study. Melanoma mortality was not associated with estimated cumulative exposure. Brain cancer mortality did not demonstrate a clear dose-response relationship with estimated cumulative exposure.  相似文献

An update of a cohort study of 4855 employees at a Paulsboro, New Jersey refinery was conducted to further examine mortality patterns. The earlier study investigated refinery workers employed for a minimum of 1 year between 1 January 1946 and 1 January 1979. The vital status of these workers was ascertained through 1979. The update extended enrollment in the study and vital status follow-up for an additional 8 years (1980–1987). As in the previous study, mortality from all causes [standardized mortality ratio (SMR) = 87; 95% confidence interval (95% CI): 83–91] was significantly lower than expected compared with the general population. Total cancer mortality was also lower than expected (SMR = 96; 95% CI: 86–106). A borderline significant mortality increase in prostatic cancer was found (SMR = 144; 95% CI: 106–190). This increase was similar to the nonsignificant increase reported in the original study (SMR = 135; 95% CI: 90–196). The excess was of comparable magnitude among white males and nonwhite males, although it was not significant for the latter. Detailed analysis indicated that the prostatic cancer was not likely to be related to employment at the refinery. Mortality from lymphatic and hematopoietic cancers was similar to the expected mortality. Mortality from overall leukemia was as expected and detailed analyses by specific cell type showed no increase. An increase in mortality occurred from non-Hodgkin's lymphoma among male workers (SMR = 132; 95% CI: 74–217). The increase was not statistically significant and unlikely to be associated with refinery employment. Mortality from multiple myeloma among male employees was lower than expected (SMR = 74; 95% CI: 20–190). Mortality from asbestos-related diseases (pulmonary fibrosis, lung cancer, malignant mesothelioma) was also lower than expected among male workers. No cause-specific mortality was found to be associated with duration of employment at the refinery, including several causes which have been reported to be elevated in previous studies. The findings of this updated study indicate, as in the previous report, the generally favorable mortality experience of Paulsboro refinery workers.The work for this study was performed at Mobil Corporate Medical Department  相似文献

A historical cohort study of 406 workers was undertaken to evaluate the possible health hazards of employment in traditional electric power plants with particular regard to cancer mortality. The total mortality was below that expected [observed 40, standardized mortality ratio (SMR) 87] on the basis of national rates. Cancer mortality was slightly increased (SMR 112), mainly because of an excess of respiratory cancer (observed 9, SMR 171, 90% CI 89-299). Lung cancer deaths were higher than expected among those less than 60 years of age and among those with a duration of exposure and a latency period since first employment of over 10 years (observed 7, SMR 184). Furthermore, the increase was more pronounced among maintenance workers and workers in general services. Previous data indicated that there had been past exposure to some known respiratory carcinogens (ie, asbestos, polycyclic aromatic hydrocarbons). This study suggests that a possible lung cancer risk from traditional electricity generating plants should be considered and further studied.  相似文献

Mortality from cancer among workers exposed to ethylene oxide (EtO) has been studied in 10 distinct cohorts that include about 29,800 workers and 2540 deaths. This paper presents a review and meta-analysis of these studies, primarily for leukaemia, non-Hodgkin's lymphoma, stomach cancer, pancreatic cancer, and cancer of the brain and nervous system. The magnitude and consistency of the standardised mortality ratios (SMRs) were evaluated for the individual and combined studies, as well as trends by intensity or frequency of exposure, by duration of exposure, and by latency (time since first exposure). Exposures to other workplace chemicals were examined as possible confounder variables. Three small studies by Hogstedt initially suggested an association between EtO and leukaemia, but in seven subsequent studies the SMRs for leukaemia have been much lower. For the combined studies the SMR = 1.06 (95% confidence interval (95% CI) 0.73-1.48). There was a slight suggestion of a trend by duration of exposure (p = 0.19) and a suggested increase with longer latency (p = 0.07), but there was no overall trend in risk of leukaemia by intensity or frequency of exposure; nor did a cumulative exposure analysis in the largest study indicate a quantitative association. There was also an indication that in two studies with increased risks the workers had been exposed to other potential carcinogens. For non-Hodgkin's lymphoma there was a suggestive risk overall (SMR = 1.35, 95% CI 0.93-1.90). Breakdowns by exposure intensity or frequency, exposure duration, or latency did not indicate an association, but a positive trend by cumulative exposure (p = 0.05) was seen in the largest study. There was a suggested increase in the overall SMR for stomach cancer (SMR = 1.28, 95% CI 0.98-1.65 (CI 0.73-2.26 when heterogeneity among the risk estimates was taken into account)), but analyses by intensity or duration of exposure or cumulative exposure did not support a causal association for stomach cancer. The overall SMRs and exposure-response analyses did not indicate a risk from EtO for pancreatic cancer (SMR = 0.98), brain and nervous system cancer (SMR = 0.89), or total cancer (SMR = 0.94). Although the current data do not provide consistent and convincing evidence that EtO causes leukaemia or non-Hodgkin's lymphoma, the issues are not resolved and await further studies of exposed populations.  相似文献

To study whether exposure to low levels of vinyl chloride monomer (VCM) causes increased risk for cancer morbidity and death from ischemic heart disease, a cohort study was performed among 2,031 male workers at a polyvinyl chloride (PVC) processing plant who had been employed for at least 3 months during the period 1945-1980. An almost significantly increased total mortality (SMR = 116, 95% CI 99-136) was found. Deaths caused by violence or intoxication were significantly increased (SMR = 153, 95% CI 109-213), but not deaths from ischemic heart disease (SMR = 100, 95% CI 73-135). A significant increase in total cancer morbidity was observed (SMR = 128, 95% CI 101-161). Respiratory cancers were significantly increased (SMR = 213, 95% CI 127-346). Furthermore, six brain tumors (vs. 2.6 expected) were observed. This increase, however, was not significant (SMR = 229, 95% CI 84-498). No liver hemangiosarcoma was observed. Applying a latency period of greater than or equal to 10 years from start of employment did not change the risk patterns. There were no significant exposure-response associations between exposure estimates for VCM, asbestos, and plasticizers and cancer morbidity.  相似文献

This cohort included 25,321 active, retired, and terminated workers at the Exxon refineries and chemical plants in Baton Rouge, Louisiana; Baytown, Texas; and Bayway/Bayonne, New Jersey. An emphasis was placed on the cancers previously reported to be increased in at least one petrochemical worker population. The population included a high proportion of retirees, providing adequate work experience and latency to evaluate risks associated with employment. Vital status was determined for 98.3% of the population and death certificates were obtained for 99.9% of the decedents. Mortality for "all causes of death" (standardised mortality ratio (SMR) = 89, 95% confidence interval (95% CI) 87-91) and for "all cancers" (SMR = 94, 95% CI 89-100) were lower than expected based on state rates. History of cigarette smoking collected from individual medical records and summarised work histories were used for subgroup analyses. There was no indication that cancers of primary interest were increased; however, blue collar workers at one plant had raised rates of kidney cancer (SMR = 246, 95% CI 146-390), which may require further evaluation.  相似文献

Aims: To evaluate the mortality experience of a cohort of employees in the UK silica sand industry exposed to respirable crystalline silica (RCS).

Methods: A retrospective cohort mortality study followed all workers to 2001 with at least one year's employment at one of seven UK silica sand producing quarries between 1950 and 1986. Each worker was assigned a job category and cumulative exposure to RCS was estimated using a job-exposure matrix.

Results: A total of 764 deaths were identified in 2703 cohort members. The overall mortality rate for the cohort was lower than would be expected in the general population. Mortality from circulatory and respiratory disease was also less than expected, but death due to pneumoconiosis was slightly raised (two deaths). Mortality from all cancers was slightly decreased. Mortality was not raised in any job category. Cancer mortality was raised at one quarry due to a significant increase in lung (standardised mortality rate (SMR) 162.0, 95% CI 113.5 to 224.3) and bladder (SMR 366.5, 95% CI 167.6 to 695.7) cancers. Mortality from lung cancer and other causes did not show a trend with cumulative exposure to RCS.

Conclusions: This study did not show any consistent relation between RCS exposure (in the absence of other known carcinogens) and the development of lung cancer. This contrasts with a number of studies that have shown positive findings in similar and related industries.

We examined mortality in a retrospective follow-up study of 3,241 workers employed between 1970–1992, in four pulp and paper mills in Catalonia, Spain. Vital status was determined for 95% of the cohort. Exposure was reconstructed using job histories and a company exposure questionnaire. Standardized mortality ratios (SMR) were derived using mortality rates of Spain as the reference. For all workers, mortality from all causes (SMR = 76; 95%; confidence intervals [CI] = 65–88; 189 deaths) and all malignant neoplasms (SMR = 93; CI = 72–119; 65 deaths) were less than the expected. Excess risk was observed for mortality from all neoplasms in females (SMR = 168; CI 84–303; 11 deaths), for large intestine cancer in both sexes (SMR = 250; CI = 115–525; 8 deaths), particularly after 10 years of employment and latency (SMR = 355; CI = 154–701; 8 deaths), and for breast cancer in females (SMR = 286; CI = 77–732; 4 deaths). These findings suggest that workers employed in the pulp and paper industry may have an excess risk of specific cancers. © 1996 Wiley-Liss, Inc.  相似文献

BACKGROUND: This study has examined cancer mortality of a cohort of male U.S. workers exposed to lead. METHODS: The cohort consisted of 4,518 workers at lead battery plants and 2,300 at lead smelters. Vital status was ascertained between 1947 and 1995. Site-specific cancer standardized mortality ratios (SMRs) and 95% confidence intervals (95% CIs), based on the mortality rates of the U.S. male population and adjusted for age and calendar time, were calculated for the total cohort as well as subcohorts stratified by various exposure parameters. In addition, a nested case-control study of stomach cancer (30 cases and 120 age-matched controls) was also conducted. RESULTS: Mortality from all cancers was as expected (897 observed deaths, SMR = 103.8, 95% CI: 97.1-110.8). Mortality was significantly raised for stomach cancer (SMR = 147.4, 95% CI: 112. 5-189.8), lung cancer (SMR = 116.4, 95% CI: 103.9-129.9), and cancer of the thyroid and other endocrine glands (SMR = 308.0, 95% CI: 133. 0-606.8). There was a nonsignificant mortality deficit from kidney cancer (SMR = 63.6, 95% CI: 33.9-108.7). For bladder cancer, mortality was significantly lower than expected (SMR = 55.5, 95% CI: 31.7-90.1). Nonsignificant mortality deficits were also reported for cancer of the central nervous system (SMR = 74.8, 95% CI: 41.9-123. 4) and lymphatic and hematopoietic cancer (SMR = 92.2, 95% CI: 72. 4-115.7). Additional analyses by type of facility (lead battery plants vs. lead smelters), length of employment, latency, and period of hire were also performed. In the nested case-control study of stomach cancer, odds ratios were calculated for various exposure indices, and none was found to be elevated. Furthermore, no exposure-response relationship between lead exposure and stomach cancer was found in the nested case-control study. CONCLUSIONS: A significant mortality increase from stomach cancer was found. However, based on the analyses in the cohort study and the nested case-control study, the increase did not appear to be related to lead exposure. A small, but statistically significant mortality increase from lung cancer was also observed. The small increase, in the absence of an exposure-response relationship, could be the result of confounding due to smoking, and was not likely causally related to lead exposure. Although the significant increase in cancer of the thyroid and other endocrine glands appeared to be consistent with an occupational interpretation, the small number of deaths (8), the lack of information on potential confounding factors, and the lack of reporting of a similar increase in other studies underscore the need to view this finding with caution. No increased mortality was found for kidney cancer, bladder cancer, cancer of the central nervous system, or lymphatic and hematopoietic cancer.  相似文献

BACKGROUND: The possible associations between asphalt work and mortality from non-malignant diseases in a cohort of male Norwegian asphalt workers that formed part of the European asphalt worker mortality study are examined. METHODS: The mortality experience among 8,610 male workers ever employed in asphalt work was observed from 01.01.1970 until 12.31.1996, yielding 127,636 person years of observation. The cause-specific deaths observed in the study cohort were compared with the expected figures calculated from 5-year period- and age-specific national death rates. A study-specific job exposure matrix was used for individual-individual exposure estimates. RESULTS: Eight hundred and three workers died during the observation period (SMR = 0.92, 95% confidence intervals (CI) = 0.58, 1.40). The mortality from non-malignant respiratory diseases was elevated (SMR = 1.25, 95%CI = 0.97, 1.58) and was associated with years since first employment in the asphalt industry. Mortality from respiratory diseases was highest among the workers first employed in the 1960s. Among the different job types held by the workers the pavers and mastic asphalt workers had the highest mortality from respiratory diseases. A weak dose-response was found with cumulative exposure to PAH and bitumen fume and mortality from non-malignant respiratory diseases in analyses employing the job-exposure matrix. Overall mortality, the mortality from circulatory diseases (SMR = 0.93, 95%CI = 0.83, 1.03) and external causes (SMR = 0.90, 95%CI = 0.72, 1.11) was lower than expected. CONCLUSIONS: Mortality from respiratory diseases was found somewhat elevated among the asphalt workers. There was some evidence of exposure-response with both bitumen fume and PAH exposure. PAH from coal tar contributes to the PAH exposure.  相似文献
