内肾外肾论

提出内肾外肾学说。内肾主水,相当于西医解剖学中的泌尿系统;外肾主精,相当于西医解剖学中下丘脑-垂体-性腺轴系统和解剖学的外生殖器官;内肾、外肾合而为中医肾,相当于泌尿系统、生殖系统、下丘脑-垂体-腺轴(甲状腺、胸腺、肾上腺、性腺)系统及神经内分泌免疫网络系统。内、外肾在解剖上相互联系,生理上密切相关,病理上相互关联,治疗上互相影响。内肾、外肾是中医肾的物质及功能基石,肾阴、肾阳是中医肾的物质及功能的运用和体现。

<正>209467 Diagnosis and treatment of multifocal urothelial carcinomas of kidney recipients following renal transplantation/Wu Jianhui(吴建辉,Dept Urol,Tianjin 1st Central Hosp,Tianjin 300192)…∥Chin J Organ Transplant.-2009,30(6).-331～333Objective To analyze the etiology,clinical diagnosis and outcomes of 6 cases of multifocal urothelial carcinomas following transplantation.Methods A retrospective analysis was performed on 6 cases of multifocal urothelial carcinomas following renal transplantation in our center.Results Six patients were diagnosed as having multifocal urotheial carcinomas,including 1 case of related renal transplantation.Five cases were diagnosed by painless gross hematuria 2~48 months after renal transplantation,and 1 patients was diagnosed as having bladder tumors by B-ultrasound.All lesions happened un-simultaneously,from 1.5~16 months,and each case accepted operation 2~5 times.One case accepted radical cystectomy and cutaneous ureterostomy of the graft,and one accepted radical cystectomy,cutaneous ureterostomy of the graft and urethectomy.All of 6 cases received OKT3 or Daclizumab as the induced-immunosuppressive therapy and tacrolimus or cyclosporine A+mycofenolate mofetil+steroid as the maintenance therapy.Intravesical chemotherapy started in all patients immediately after the surgery.All operations were successful and most patients got satisfactory results.Conclusion Kidney recipients have a higher rate of transitional cell carcinomas which are characterized by easy metastasis,and mostly have unsatisfactory prognosis.Kidney recipitents with hematuria should be examined carefully with cystoscopy,retrograde pyelography and cystourethroscopy.A routine examination at regular intervals after transplantation is very important.9 refs.

<正>I209303 The operative techniques of renal transplantation of cynomolgus monkey/Wang Hang(Dept Urol,Zhongshan Hosp,Fudan Univ,Shanghai 200032)…∥Chin J Organ Transplant.-2009,30(3).-156~158Objective To study the operative techniques of renal transplantation in cynomolgus monkeys.Methods Twenty-six cynomolgus monkeys first served as donors,and were subsequently used as recipients.The vessel of graft was implanted hetertopically end-to-side in the aorta and vena cava,and ureterocystostomy or urteroureterostomy was constructed subsequently.Results Operating time was(180.9±25.3)min;The warm ischemia time of kidney was(1.5±0.4)min,and the cold ischemia time was(71.4±10.5)min.The time of arterial anastomosis,anastomaosis of vein,ureterocystostomy and ureteroureterostomy was(29.5±6.3),(2507±5.7),(19.8±5.7),and(15.1±3.2)min.There was one intraoperative death because of bleeding.Postoperative complications were observed in 32%(8/25)live monkeys.Conclusion Renal transplantation of nonhuman primate provides an optimal model for the evaluation fo tolerance in the preclinical setting.General anesthesia with endotracheal tube,vascular anastomosis with atraumatic technique and ureteral anastomosis with stent are the key points for success.9 refs.

保留原位肾的多囊肾患者肾移植研究

目的 研究多囊肾患者保留原位肾的肾移植效果.方法 回顾性分析12例多囊肾患者保留原位肾直接进行肾移植术的效果.结果 除1例术后因并发症切除1侧原位肾以外,其余11例均保留原位肾,术后移植肾功能良好.结论 多囊肾患者保留原位肾直接进行肾移植效果满意.
