Thirty-eight knees in 34 patients with an average age of 22 years were diagnosed as having lateral facet syndrome (LFS), a painful compressive arthropathy of the lateral facet of the patella. This diagnosis was based on the physical findings of tenderness at the lateral patellofemoral joint line, tenderness over the vastus lateralis obliquus (VLO) tendon just above the patella, a positive medial apprehension test, and marked resistance to medial patellar displacement with the knee flexed 30 degrees. The most common complaints were patellar pain with activity, pain with prolonged knee flexion, intermittent knee swelling, and giving way. At surgery, the VLO, the lateral retinaculum (LR), and the anterior fibers of the iliotibial tract (ITT) were sequentially divided from the lateral border of the patella. Each was temporarily reattached to a cuff of soft tissue left on the patella using surgical clamps to determine its contribution to lateral restraint. The VLO was found to be the primary restraint in one-half of the knees. In one-third of the knees, all three of the structures contributed equally. In six knees, the primary restraint was the anterior fibers of the ITT, whereas the LR was the primary restraint in only two. The distal ends of these three structures were then resected to prevent rescarring and retethering. At a minimum follow-up period of two years, 87% had satisfactory relief of their patellar pain, had returned to normal activities, and had no or minimal physical findings of LFS. The procedure is recommended for patients who have failed other procedures and in those whose symptoms cannot be controlled by activity modification, exercises, bracing, or medication.  相似文献

Excellent results have been reported with posterior stabilized total knee arthroplasty. A common complication relating to patellofemoral articulation is patellar clunk syndrome. Patellar clunk syndrome occurs when a fibrous nodule develops just proximal to the patellar button. At approximately 30 degrees to 45 degrees from full extension, the nodule catches the anterior flange of the femoral prosthesis, resulting in the clunk and a painful range of motion. The present study examines the use of arthroscopic debridement for this disorder. Thirty consecutive patients (32 knees) with the diagnosis of patellar clunk syndrome were evaluated at 1 year after arthroscopic debridement. All patients were evaluated clinically and radiographically according to the Knee Society score. In addition, patients were questioned specifically regarding anterior knee pain and patellofemoral symptoms. Radiographs were evaluated further regarding patella and component position. Patients were diagnosed with patellar clunk at an average of 12 months after their latest knee arthroplasty, with a range of 3 to 47 months. All patients complained of anterior knee pain and the clunk. All patients had a hypertrophic nodule at the junction of the proximal pole of the patella and quadriceps tendon and underwent arthroscopic debridement through a superolateral portal. All patients were free of patellar clunk postoperatively; one patient reported persistent anterior knee pain. Knee Society scores increased from an average of 64 points preoperative to 93 points postoperative. Radiographs showed patella alta in eight knees, patella baja in two. Four femoral components were in 5 degrees flexion. The present study represents the largest collection of data regarding patellar clunk syndrome. The data appear to support arthroscopic debridement as a successful treatment of patellar clunk syndrome.  相似文献

Influence of patellar resurfacing after knee replacement and the frequency of patella infera and its relation to the postoperative appearance of the knee pain were assessed in patients with juvenile rheumatoid arthritis. Seventy-seven total knee arthroplasties using the AGC prosthesis with nonconstrained components were done on 52 patients with a mean followup of 7.3 years (range, 3-13 years). Anterior knee pain was present in 14 of 30 patients (47%) with an unreplaced patella and in two of 18 patients (11%) with patella resurfacing. The patella was replaced in 18 patients (35%) and in 23 of 77 knees (30%). Neither revision surgery of implanted patellar components nor any later resurfacing of an unreplaced patella were done during the followup. Preoperatively using the Insall-Salvati ratio, the majority of knees (54 of 77) had a low-riding patella. Patella infera occurred commonly in patients with juvenile rheumatoid arthritis. No connection between patella infera and anterior knee pain was found.  相似文献

目的 研究全膝置换术后相同屈膝角度下矢状位髌骨和髌腱之间的角度变化,及其与术后活动度的关系.方法 材料为2002年5月~2003年12月期间做了初次全膝置换的33个关节(30例),诊断包括29个膝骨关节炎、4个类风湿性关节炎,其中20个关节置换了髌骨.男5例,女25例;年龄平均67.3岁.术前和术后1年摄0°、45°、90°侧位X线片,记录术后1年的活动度,测量术前术后不同屈膝度下的髌骨纵轴和髌韧带的夹角,并进行分析.结果 术后1年在屈膝90°时,髌骨与髌腱的角度较术前增大(P<0.01).更换髌骨和保留髌骨组术后都发生了相似的角度变化(P<0.01).术后屈膝90°的髌骨髌腱角和术后活动度呈负相关(r=-0.506,P=0.003).结论 全膝置换术后在屈膝活动时的髌骨髌腱角变化规律发生改变,与置换髌骨无关;术后屈膝90°时髌骨髌腱角的改变幅度与术后活动度限制有关.  相似文献

We report 2 cases of conversion of fused knee to total knee arthroplasty (TKA) by way of soft-tissue expansion. Case 1 had had multiple operations on the infected total knee, which was arthrodesed 7 months before conversion. Case 2 had had knee arthrodesis for an intra-articular comminuted fracture 5 years and 10 months before conversion. In both patients, a tissue expander was placed subcutaneously upon the patella and the patellar tendon to gradually expand the anterior knee skin for later coverage and knee flexion. After takedown of the arthrodesis, a fully constrained rotating-hinge prosthesis was implanted as the second-stage procedure. The final range of flexion was 95 degrees in both knees with patients' subjective satisfaction.  相似文献

Introduction Anterior cruciate ligament (ACL) reconstruction with the use of autograft tissue represents the standard treatment. The use of a bone-patellar tendon-bone transplant for symptomatic ACL deficiency achieves good long-term results. The purpose of the study was to investigate in a cadaveric model whether reproducible patellar tendon shortening changes the patellofemoral alignment.Materials and methods Using five cadaveric knees, an MRI investigation was performed with the patellar tendon left unchanged, shortened 5 mm and shortened 10 mm, respectively, in both 20 and 45° of knee flexion. The lateral patellofemoral and the congruence angles were measured and compared using a one-way analysis of variance for repeated measurements.Results Shortening the patella tendon by approximately 20% did not significantly influence the patellofemoral alignment.Conclusion Although anterior cruciate ligament-reconstruction using the patellar tendon has become a standard procedure, postoperative problems such as anterior knee pain, and patellofemoral osteoarthritis occur quite often. In this cadaver study we did not find a significant difference for the patellofemoral alignment, irrespective of the patellar tendon length.  相似文献

Three total knee designs were evaluated to test the hypothesis that femoral component design affects the clinical and mechanical functions of the unresurfaced patella after total knee arthroplasty. Patients with the Ortholoc II, Advantim, and Profix femoral components were followed up for as many as 14 years and revision rate, anterior knee pain, and generalized knee pain were compared. A laboratory protocol was devised to evaluate pressure in the patellofemoral joint of knees from cadavers with a pressure-sensitive transducer using the same three designs at various degrees of knee flexion. Thirty Ortholoc II knee components were followed up for 14 years. Nineteen patients (63%) had severe anterior knee pain and 15 patients (50%) had reoperation to resurface the patella within 2 years. Two hundred one patients (222 knees) with Advantim components were followed up for 10 years and 305 patients (330 knees) with Profix components were followed up for 5 years. No patients with these two knee designs had severe anterior knee pain or reoperation for patellar resurfacing. A significantly higher rate of mild anterior knee pain was seen in the patients with Advantim components than in the patients with Profix components. No apparent relationship was seen between the severity of patellar wear found at the time of surgery and the incidence of anterior knee pain. Patients with rheumatoid arthritis receiving either the Advantim or Profix knee component performed as well as patients with osteoarthritis when the patella was not resurfaced. Pressure was significantly higher in the patellofemoral joints of the laboratory knee specimens with Ortholoc II components than in the specimens with either the Advantim or Profix components. The specimens with Advantim components had significantly higher pressure than did the specimens with normal knees, and the specimens with Profix components differed little from those with normal knees.  相似文献

Postoperative anterior knee pain was evaluated in a consecutive series of 138 knees in 108 patients with rheumatoid arthritis treated by total knee replacement with Mark I Insall-Burstein prostheses. No knee had primary patellar resurfacing, and in the 119 knees followed up for a mean of 63.9 months, none had secondary resurfacing. Anterior knee pain was absent in 87 knees (73%), mild in 16 (13.5%) and moderate or severe in 16 (13.5%). The height of the patella above the prosthetic joint line was the only variable which was directly related to the incidence of anterior knee pain. The sensitivity and specificity of patellar height measurements for identifying patients with or without pain were derived. From these data, a selective policy of resurfacing the patella in those at risk was adopted. Choosing a patellar height of 15 mm or less, patellar resurfacing could be avoided in 80% of patients likely to have no pain, and the patella could be resurfaced in 65% of those likely to have anterior knee pain.  相似文献

Patella infera may occur after reconstruction of the anterior cruciate ligament (ACL), high tibial osteotomy and total knee replacement (TKR). Restriction of movement of the knee and pain may result. Our aim was to compare the incidence and to assess the effects of patella infera after TKR and unicompartmental knee replacement (UKR). We reviewed radiographs of the knees of 84 patients who had had either TKR or UKR as part of a randomised, controlled trial The length of the patellar tendon was measured on serial radiographs taken before, at eight months and at five years after operation. There was no significant change in the length of the patellar tendon after UKR, but a significant reduction was observed after TKR. Five years after the operation, the shortening of the tendon had increased to a mean of 3.5 mm. Of the knees with TKR reviewed at five years, 34% developed patella infera, defined as 10% or more of shortening, compared with 5% of those with UKR. Shortening was greatest in those knees which had required a lateral release; in this subgroup the mean shortening was 7.2 mm. Shortening correlated with restriction of movement and pain in the knee. Our study has shown that patella infera develops in most patients after TKR with lateral release, and in approximately 25% of patients after TKR without this additional procedure. Patella infera rarely occurs after UKR. It is associated with restriction of movement and pain in the knee. It may be an effect of the more extensive exposure required to perform TKR and may, in part, explain the better clinical results of UKR.  相似文献

目的 探讨膝骨关节炎患者行全膝关节表面置换时髌骨置换与否对术后疗效及并发症发生的影响.方法 对2007年1月至2011年12月之间行人工全膝关节置换术的170例(237膝)骨关节炎患者,按髌骨置换(126膝)和未置换(111膝)分为2组.对术前和末次随访时两组的HSS评分、膝前痛评分、膝关节活动度、最大屈曲度数、屈曲畸形及手术时情况和患者满意度等进行对比分析.结果使用SPSS 17.0统计软件包进行统计学分析.结果 在170例(237膝)OA患者中161例(21 3膝)获得随访,平均随访时间(40.94±8.02)个月,置换组HSS评分由术前的(40.19±8.14)分增加到末次随访时的(87.45±6.00)分(P<0.05),膝前痛评分由(4.13±1.08)分升高到(19.47±4.04)分(P<0.05);未置换组HSS评分由(40.00±6.74)分上升到(88.93±4.92)分(P<0.05),膝前痛评分由(3.58±2.26)分变化到(13.61±3.89)分(P<0.05);患者满意度置换组为80.91%,未置换组为61.17%(P<0.05).在HSS评分、膝关节活动度、最大屈曲度数、屈曲畸形等方面两组差异无统计学意义.而在膝前痛评分、患者满意度、手术时间、术中失血量等方面差异有统计学意义.结论 当膝关节骨关节炎患者进行全膝关节置换术时,若术前存在膝关节疼痛严重、患者期望较高、不适合长时间手术,应行髌骨置换.  相似文献

Purpose Congenital dislocation of the patella is permanent and manually irreducible, and it manifests immediately after birth with flexion contracture of the knee, genu valgus, external tibial torsion and foot deformity. We retrospectively reviewed the results of operative treatment of seven knees in six patients with congenital dislocation of the patella. Methods The age of the six patients at diagnosis ranged from 8 days to 3.6 years, with an average of 1.3 years, and their age at the time of operation ranged from 0.6 to 3.9 years, with an average of 2.1 years. Serial casting and/or a brace was attempted before surgery in five of seven knees, leading to improvement in the flexion contracture of the knee. All knees were treated operatively in combination with lateral release, medial plication, V-Y lengthening of the quadriceps, medial transfer of the lateral patellar tendon and posterior release of the knee. Results Although these deformities were noticed at birth in all seven knees, diagnosis was delayed in three knees due to the low suspicion of the disease and invisible patellae on radiographs. Ultrasonography confirmed the diagnosis of dislocation. The patella was centered in the groove of the femoral condyle after surgery in all knees, but subluxation of the knee with flexion was observed in one knee in which the operation was performed at 3.9 years. Genu valgus and external tibial torsion improved after surgery in all knees. The operated knee was mobile in all cases, with less than 10° flexion contracture of the knee. Flexion contracture did not increase in any of the knees. Conclusion Congenital dislocation of the patella should be suspected in every patient with knee flexion contracture, genu valgus, external tibial torsion, foot deformity and delayed walking. Successful results were obtained when the operation was performed in younger children. Other procedures, such as the semitendinosus tenodesis or tendon transfer, might have to be combined to achieve better stability with flexion in older children. Long-term outcomes were reported for 10 (77%) of 13 cases of revision anterior cruciate ligament (ACL) reconstruction using the lateral third of the ipsilateral patellar tendon as a graft. All primary ACL reconstructions were ipsilateral central-third bone-patellar tendon-bone graft procedures. Mean age at follow-up was 30.7 years, and mean time from revision ACL surgery to follow-up was 42.9 months. At follow-up, average KT-1000 difference between knees was 2.4 mm. All patients had a negative pivot shift, extension within 5 degrees of the contralateral knee, and flexion within 15 degrees. Mean bilateral comparison ratios for isokinetic strength and hop testing were: extension, 83.5%; flexion, 96%; and single-leg hop 96.9%. No patella fractures or tendon ruptures had occurred. All patients had returned to their previous work level, and 8 of the 10 patients could participate in at least "moderate" sports activities (e.g., skiing and tennis). The results were comparable to published outcome reports for both primary and revision ACL reconstruction. The lateral third of the ipsilateral patellar tendon is a good graft option for revision ACL reconstruction.  相似文献

This prospective clinical study evaluated 69 patients (136 knees) presenting with anterior knee pain after failure to respond to conservative treatment. All patients agreed to undergo minimally invasive intraosseous pressure measurements of the patella under local anesthesia. Forty patients (50 knees) demonstrated a positive pain provocation test (the typical pain sensation could be reproduced by raising intrapatellar pressure) and were treated with a new method of intraosseous drilling and decompression. Ninety percent of the patients treated experienced pain relief >3 years postoperatively. Repeated intraosseous pressure measurements in 33 knees 1 year postoperatively confirmed objective intraosseous pressure reduction in 88%. Patients with anterior knee pain and a positive pain provocation test were labeled as hypertension syndrome of the patella. These results indicate simple extra-articular patellar decompression may offer a valuable new form of treatment in select patients with anterior knee pain, ie, hypertension syndrome of the patella.  相似文献

Asymmetric patella resurfacing in total knee arthroplasty   总被引:3,自引:0,他引:3  
Three hundred consecutive primary, cemented, condylar total knee arthroplasties (TKAs) were reviewed for the presence of asymmetric patella resurfacing using a postoperative Merchant or sunrise patellar radiograph. Twenty-one knees in 14 patients were found to have the patella asymmetrically resurfaced. Asymmetric resurfacing typically involved the inadvertent preferential resurfacing of the lateral facet with underresection of bone from the medial patellar facet. All patients underwent follow-up for a minimum of 5 years, with a mean follow-up of 7.5 years. Of the 21 knees, 3 revisions were required for patellar complications. One patellar component was loose on radiographs and there was marked patellofemoral pain in 6 knees. Overall, 11 of 21 knees (52%) underwent revision or were recommended for revision for patellar complications or had anterior knee pain that limited activities. Inadvertent asymmetric patella resurfacing using the kinematic condylar implant adversely affects the outcome after TKA.  相似文献

目的探讨髌骨半脱位的治疗方法及其预后。方法回顾性分析70例(130膝)髌骨半脱位患者的临床资料,其中男30例,女40例;年龄10~48岁,平均23岁。患者髌骨的形态均为WibergⅡ、Ⅲ型,其中伴有外侧髌股关节骨关节炎者58例108膝,高位髌骨23例46膝,外伤后股四头肌内侧头肌张力不良10例10膝。Q角20°~40°。全部行手术治疗,手术方式包括:髌外侧支持带充分松解(130膝);髌内侧支持带紧缩、股内侧肌移位至髌骨前内侧(12膝);髌骨外侧部分切除(17膝);胫骨结节前、内侧移位(46膝),平均内移1.0cm、前移1.2cm;缝匠肌前移(11膝),半腱肌前移(13膝);关节镜下髌外侧支持带松解,髌内侧支持带紧缩(31膝)。术中见全部病例髌外侧支持带均明显增厚。结果随访3~180个月,根据Insall等评分系统评估分析疗效,术后疗效优78膝,良33膝,可12膝,差7膝。患者术后疼痛、关节活动功能均得到明显改善,尤其是解除膝前痛效果显著。结论根据病因及病理改变选择恰当的手术方式治疗髌骨半脱位均可取得满意的效果,特别是关节镜手术具有创伤小、恢复快的特点,值得进一步推广应用。  相似文献

BACKGROUND: The management of the patella in total knee arthroplasty is still problematic. We aimed to identify differences in the clinical outcome of total knee arthroplasty according to whether or not patellar resurfacing had been performed in a prospective, randomized study of 220 osteoarthritic knees. METHODS: Two hundred and twenty total knee arthroplasties in 201 patients were randomly assigned to be performed with either resurfacing or retention of the patella, and the results were followed for a mean of forty-eight months (range, thirty-six to seventy-nine months) in a double-blind (both patient and clinical evaluator blinded), prospective study. Evaluation was performed annually by an independent observer and consisted of assessment with the Knee Society clinical rating system, specific evaluation of anterior knee pain, a stair-climbing test, and radiographic examination. RESULTS: Fifteen (12%) of the 128 knees without patellar resurfacing and nine (10%) of the ninety-two knees with patellar resurfacing underwent a revision or another type of reoperation related to the patellofemoral articulation. This difference was not significant (chi square with one degree of freedom = 0.206, p = 0.650). At the time of the latest follow-up, there was a significantly higher incidence of anterior pain (chi square with one degree of freedom = 5.757, p = 0.016) in the knees that had not had patellar resurfacing. CONCLUSIONS: Patients who underwent patellar resurfacing had superior clinical results in terms of anterior knee pain and stair descent. However, anterior knee pain still occurred in patients with patellar resurfacing, and nine (10%) of the ninety-two patients in that group underwent a revision or another type of reoperation involving the patellofemoral joint. Weight but not body mass index was associated with the development of anterior knee pain in the patients without patellar resurfacing, a finding that suggests that patellofemoral dysfunction may be a function of joint loading rather than obesity.  相似文献

Patella nonresurfacing in total knee arthroplasty was reviewed retrospectively in 49 knees (42 patients). Thirteen patients (17 knees) died leaving 29 patients (32 knees) in the final study group. The minimum length of followup was 10 years and averaged 11.7 years (range, 10-13.4 years). The diagnosis was primary osteoarthritis in 26 knees, posttraumatic arthritis in four, osteonecrosis in one, and Paget's disease in one. Patients were evaluated using the Knee Society score, a patella score, and radiographs. The mean Knee Society score improved from 52.8 to 87.5 points postoperatively and functional score improved from 57.5 to 84.5 points postoperatively. Anterior knee pain was reported in six knees (20%). Only one patient required secondary resurfacing of the patella because of postoperative anterior knee pain. No significant correlation was found between anterior knee pain and patellofemoral joint space, patellar sclerosis, Insall-Salvati ratio, patellar tilt, gender, obesity, or age. Based on the need for secondary patellar resurfacing because of anterior knee pain, the 10-year survival was 97.5%. At minimum 10-year followup, retaining the patella in total knee arthroplasty in selected patients with osteoarthritis of the osteoarthritic knee can yield acceptable results.  相似文献

In many designs of total knee arthroplasty, the patella with one central peg has been replaced by a patella with three small pegs for cement fixation. There have been recent reports of failure of this design. This is a prospective, consecutive study of two types of patella component fixation in 228 posterior-stabilized knee arthroplasties done by one surgeon. A central peg all-polyethylene component was used for 84 consecutive knees in 63 patients (Group A) and a three-peg patella was used for the next 144 consecutive knees in 99 patients (Group B). The mean followup was 6.7 years (range, 2-10 years) for Group A and 3.5 years (range, 2-6 years) for Group B. Except for the patellar component fixation, all knees had the same posterior-stabilized prosthesis using a specific protocol for patellar resurfacing. No patient required reoperation for a patellofemoral complication. The prevalence of patella fracture was higher in Group A, 4.7% (four knees), compared with 2.1% (three knees) in Group B, but this difference was not statistically significant. The presence of anterior knee pain referable to the patella was 7.1% (five patients, six knees) in Group A (one patient with two knees had severe anterior knee pain) and 9% (13 knees in 13 patients) in Group B. There was no patella clunk syndrome, subluxation, or fracture of a fixation peg in either group. With this specific protocol for patella resurfacing, there was a higher rate of complications with the one central peg patella (4.7%) than with the three-peg patella (2.1%), but this did not reach statistical significance. The results do not support an increased risk of component failure with this three-peg patella design, but do not, at this length of followup, show any significant advantage of three-peg fixation.  相似文献

目的研究全膝置换(totalkneearthroplasty,TKA)术后的髌腱长度变化,置换髌骨对髌腱长度的影响,髌腱长度变化与术后膝关节活动度的关系。方法2002年5月~2003年12月对49例55个关节行初次全膝置换手术。包括48例骨关节炎、7例类风湿性关节炎,其中7例做了外侧支持带松解,测量术前的髌腱长度及术后1年的髌腱长度和膝关节活动度,按是否置换髌骨分成两组,对数据进行组间和组内统计学分析。结果术后1年,髌腱发生有统计学意义的短缩(P<0·01)。在短缩大于10%的发生率上,换与不换髌骨组没有统计学意义的差别(P>0·05)。髌腱短缩和关节活动度呈负相关(P<0·01)。结论全膝置换术后存在髌腱短缩的并发症。置换髌骨对髌腱短缩没有影响。髌腱短缩可能是导致术后活动度损失的原因之一。  相似文献

 目的 探讨髌骨置换与髌骨成形对全膝关节置换术后膝关节功能的影响。方法 对2010年8月至11月拟行全膝关节置换术的48例(69膝)膝关节骨关节炎患者进行随机分组,髌骨置换组24例(35膝)、髌骨成形组24例(34膝)。两组患者年龄、体重、身高、体重指数、美国膝关节协会(Knee Society Score,KSS)膝评分及功能评分、髌骨评分的差异均无统计学意义。比较两组术后6周、3个月、6个月、12个月、24个月的KSS膝评分及功能评分、髌骨评分、术后膝前痛发生率及影像学表现。结果 髌骨置换组20例(30膝)与髌骨成形组20例(29膝)获得随访。术后各时点两组KSS膝评分的差异无统计学意义;6个月以后髌骨置换组KSS功能评分高于髌骨成形组,12个月以后髌骨置换组髌骨评分高于髌骨成形组,差异有统计学意义。术后各时点髌骨置换组膝前痛发生率与髌骨成形组的差异有统计学意义。术后24个月髌骨置换组与髌骨成形组术后胫股角(174.25°±0.97°与173.63°±0.48°)、髌韧带比值(1.01±0.09与1.09±0.07)、髌骨倾斜角(4.58°±0.18°与4.41°±0.19°)、髌骨适合角(2.69°±4.15°与2.56°±3.72°)、髌骨移位距离[(1.53±1.34) mm与(1.68±1.23) mm]的差异均无统计学意义。结论 全膝关节置换术中行髌骨置换可以改善膝关节和髌骨功能,降低术后膝前痛的发生率。  相似文献
