PURPOSE: The management of lower pole kidney stones is controversial. We examined whether lower pole caliceal anatomy could predict the success of extracorporeal shock wave lithotripsy of primary lower pole kidney stones 20 mm. or less. MATERIALS AND METHODS: From December 1997 to June 2001, 246 adults with a single, 20 mm. or less radiopaque lower pole renal stone were treated with the Doli 50 lithotriptor (Dornier Medical Systems, Marietta, Georgia) while under general anesthesia. Of the 246 patients 190 (77%) had excretory urography available for review. Lower pole infundibular length and width, lower pole infundibulopelvic angle and caliceal-pelvic height were measurable on 161 (85%), 129 (68%), 128 (67%) and 163 (86%) excretory urograms, respectively. Extracorporeal shock wave lithotripsy was considered a failure if residual stone fragments remained after 1 month, or an auxiliary procedure or re-treatment was required.RESULTS The overall stone-free rate was 78% (32 of 41) for stones 5 mm. or less, 73% (98 of 135) for stones 6 to 10 mm., 43% (22 of 51) for stones 11 to 15 mm. and 30% (7 of 19) for stones 16 to 20 mm. in maximum linear dimension. The stone-free rates grouped according to stone surface area were 76% (48 of 63 stones) for stone surface area 25 mm.2 or less, 69% (97 of 141) for 26 to 100 mm.2 and 33% (14 of 42) for 101 to 400 mm.2. Caliceal anatomy was not predictive of success even with stones grouped as 10 or less or 11 to 20 mm. Grouping patients with favorable (lower pole infundibulopelvic angle 70 degrees or greater, lower pole infundibular length 30 mm. or less and lower pole infundibular width greater than 5 mm.) versus unfavorable (70 degrees or less, greater than 30 mm. and 5 mm. or less, respectively) anatomy was also not predictive of success. CONCLUSIONS: On the Doli 50 machine stone size rather than caliceal anatomy is predictive of treatment outcome. Initial treatment failures with this machine should be managed by alternative endoscopic procedures if necessary rather than by repeat shock wave lithotripsy.  相似文献

BACKGROUND AND PURPOSE: Several anatomic factors influence the clearance of lower pole stones treated with shockwave lithotripsy (SWL). One of these is the infundibulopelvic angle, but its measurement is complex. METHODS: We proposed a more simple measure of caliceal dependence, the caliceal pelvic height (CPH), which we defined as the distance between a horizontal line from the lowermost point of the calix containing the stone to the highest point of the lower lip of the renal pelvis. RESULTS: In 62 patients who had SWL for solitary lower pole stones, a CPH < 15 mm was associated with a stone clearance rate of 92%, whereas with a CPH > or = 15 mm, the clearance rate was only 52% (p < 0.05). A majority (74%) of the patients with an infundibular width of > or = 5 mm were rendered stone free compared with 40% of those with a width of < 5 mm (p < 0.05). CONCLUSION: Measurement of the CPH, in conjunction with other anatomic factors, may more accurately predict the outcome of SWL in patients with lower pole stones.  相似文献

Hysterectomy is responsible for numerous postoperative complications largely urological, but sometimes of sexual or colorectal function. The latter involve modifications of bowel function resulting in constipation as a result of delayed transit, infrequent evacuations and greater difficulty in expelling faeces. We have reviewed the topic, and consider the various hypotheses whereby these modifications may result from hormonal alterations, iatrogenic causes, or variations in recto-bladder sensitivity. Any of these could justify the modifications in bowel function detected in these patients. The studies carried out so far provide no clear information about the type of constipation that develops after hysterectomy, as we lack prospective studies that compare bowel function before and after operations. Nor is it clear whether we are dealing with constipation as a result of delayed transit or obstructed defaecation, nor which pathogenetic mechanisms are involved. In view of the large number of hysterectomies that are done, sometimes prophylactically, controlled prospective studies are warranted to address these issues more fully.  相似文献

Shock wave lithotripsy and flexible ureterorenoscopy are acceptable treatment options for lower pole stones smaller than 10 mm, while percutaneous nephrolithotomy is the favoured treatment for stones larger than 20 mm. For treatment of lower pole stones of 10–20 mm, flexible ureterorenoscopy has a significantly higher stone‐free rate and lower retreatment rate than shock wave lithotripsy.

OBJECTIVE

  • ? To compare the outcomes of flexible ureterorenoscopy (F‐URS) and extracorporeal shock wave lithotripsy (ESWL) for treatment of lower pole stones of 10–20 mm.

PATIENTS AND METHODS

  • ? The database of patients with a single lower pole stone of 10–20 mm was examined to obtain two matched groups who were treated with F‐URS or ESWL. Matching criteria were stone length, side and patient gender.
  • ? Stone‐free rates were evaluated 3 months after the last treatment session by non‐contrast computed tomography. Both groups were compared for retreatment rate, complications and stone‐free rate.

RESULTS

  • ? The matched groups included 37 patients who underwent F‐URS and 62 patients who underwent ESWL. Retreatment rate was significantly higher for ESWL (60% vs 8%, P < 0.001).
  • ? Complications were more after F‐URS (13.5% vs 4.8%), but the difference was not significant (P= 0.146). All complications were grade II or IIIa on modified Clavien classification.
  • ? The stone‐free rate was significantly better after F‐URS (86.5% vs 67.7%, P= 0.038). One failure of F‐URS (2.7%) and five failures (8%) of ESWL were treated with percutaneous nephrolithotomy.
  • ? Significant residual fragments in three patients (8%) after F‐URS were treated with ESWL, while significant residual fragments after ESWL in five patients (8%) were treated with F‐URS. Residual fragments (<4 mm) were followed every 3 months in one patient (2.7%) after F‐URS and in 10 patients (16%) after ESWL.

CONCLUSIONS

  • ? For treatment of lower pole stones of 10–20 mm, F‐URS provided significantly higher stone‐free rate and lower retreatment rate compared with ESWL.
  • ? The incidence of complications after F‐URS was not significantly more than after ESWL.

Cervical sprain/strain or whiplash injuries are a common cause of acute and chronic musculoskeletal impairments and are ubiquitous after rear-end automobile collisions. The diagnosis is largely subjective and the ideal treatment controversial. Unfortunately, the majority of compensated litigation claims are associated with whiplash-type injuries secondary to motor vehicle accidents. Fortunately, many recent advances have led to better understanding of the collision and injury biomechanics and to development of a prognostic classification system, objective diagnostic tests, an array of treatment modalities, and, most important, safer automobiles. These advances will undoubtedly lead to decreased incidence, a more accurate diagnosis, and a tailored management regimen resulting in improved outcomes and ultimately fewer legal proceedings.  相似文献

OBJECTIVE: To evaluate the potency in patients after radical perineal prostatectomy with wide excision of both neurovascular bundles. MATERIAL AND METHODS: In this prospective study, a quality-of-life questionnaire was completed by 128 patients at the preoperative stage, and 6 and 12 months postoperatively. Ten questions concerning the patient's sexuality were included on the pre- and both postoperative questionnaires. In addition, 6 patients who recorded some erectile function were sent a separate questionnaire containing eight more detailed questions. RESULTS: Preoperatively, 74/128 (57.8%) patients reported erections sufficient for sexual intercourse, and of these 74, 6 (8.1%) described having spontaneous erections 1 year postoperatively. These spontaneous erections occurred 1-5 times per week. The reply to the separate mailing made clear that the reported erections were insufficient for intercourse. CONCLUSIONS: Patients undergoing standardized radical prostatectomy with wide excision of the neurovascular bundle have a very small chance of spontaneous erections sufficient for intercourse postoperatively.  相似文献

Scar lymphedema: fact or fiction?   总被引:1,自引:0,他引:1  
BACKGROUND: Few concepts are as fundamental to plastic surgery as scarring, yet swelling within a scar and its adjacent tissues is a common observation which is not well understood. Mechanical forces, scar contracture, fibrosis, and lymph stasis have been considered as possible explanations for these edematous-appearing areas, but conclusive evidence of a cause of swelling has not been established. The purpose of this study was to evaluate the possible role of microlymphatic stasis or disruption as a causal factor. PATIENTS AND METHODS: Eleven patients (mean age: 43; range: 15 to 70) with localized swelling in conjunction with linear or curvilinear scars were evaluated, 9 with facial scars and 2 with scars of the chest wall and abdomen. Swelling within the scar had been present for an average of 4.5 years (range: 9 months to 13 years). Two patients had undergone previous Z-plasty revisions to the limbs of their curvilinear scars. Radiocolloid lymphoscintigraphy with technetium-99m Sb2S3 was performed on all patients by single or multiple injection technique into the site of the scar corresponding to local edema. RESULTS: Following injection, rapid egress of radiotracer was visualized along lymphatic pathways posterior to the scar, with continuation to locoregional nodes in all patients with U-shaped "trapdoor" or linear scar configuration. However, in 8 cases there was no evidence of lymphatic drainage traversing or bridging the scar. In 2 patients with multiple prior Z-plasty revisions to the limbs of curvilinear scars, no visualization of lymph channels across the Z-plasty flaps was apparent. In total, 8 patients were diagnosed with lymphedema of the area adjacent to or enclosed within the scar. CONCLUSIONS: These findings suggest that undrained lymphatic fluid contributes to the pathogenesis of the raised and swollen tissues seen abutting a U-shaped scar. Furthermore, as lymphatic pathways do not reestablish themselves across scars, attempts at improving lymphatic flow with Z-plasty revisions may not succeed in patients with clinical trapdoor scar deformities. Determination of scar lymphedema can assist in the selection of proper management for patients seeking scar revision.  相似文献

The angiosome concept delineates the human body into three-dimensional blocks of tissue fed by specific arterial and venous sources named "angiosomes." Adjacent angiosomes are connected by a vast compensatory collateral web, or "choke vessels." This concept may provide new information applicable to improving targeted revascularization of ischemic tissue lesions. A few dedicated studies available seem to favor this strategy, as encouraging ulcer healing and limb preservation are reported in connection with both bypass and endovascular techniques based on these principles. The theory on the angiosome model of revascularization (AMV) may help the clinician to better refine vessel selection, vascular access, and specific strategies in the revascularization of critically ischemic legs with tissue lesions. Specific applications of angiosome-guided revascularization were recently suggested for patients with diabetes or renal insufficiency, with ischemic tissue lesions of the lower limb, and extended large- and medium-size collateral network decay. For these cases, the concept may allow deliberate arterial reconstruction following individual wound topographies in specific ischemic areas, although deprived from "rescue-vessel" supply. The AMV theory may contribute to a shift in common reperfusion options. However, the data available is suggestive and does not provide strong evidence as factors such as case mix and the severity of ischemia are unsatisfactorily controlled. At present, the evidence is scarce as to the effect of the severity of the arterial disease. In all comparisons, the groups treated are likely to be dissimilar and mismatched. The angiosome concept is postulated to be valid especially in diabetics, whose ischemic lesions tend to heal worse than those of non-diabetics.  相似文献

The aim of the study was to evaluate the impact of age on the efficacy of extracorporeal shock wave lithotripsy (SWL), in a comparative study based on the principles of matched-pair analysis. Over a period of 4 years, 2,311 patients were treated with SWL in a tertiary referral center. Patient and stone data were recorded in a prospective electronic database. Among these patients, 115 (4.97%) were older than 70 years of age and fulfilled the criteria for inclusion in the study (Group A). For the purposes of the comparative analysis, Group A patients were matched for gender and stone parameters (side, location of stone, and diameter ±2 mm) with a control group of patients under the age of 70 (Group B). Following matching, the patients' electronic medical records were reviewed, to identify SWL success rates at 3 months and McNemar's test was used to compare the efficacy of SWL between the two groups. Matching was possible in all cases. The results indicate that there were no statistically significant differences in the mean number of SWL sessions or in the mean number of impulses per session between the two groups. The overall stone clearance rate achieved by SWL alone was 71.3% for Group A and 73.9% for group B. Discordant pairs were found in 37 cases (in 17 pairs only patients in Group A became stone-free, while in 20 pairs only patients in Group B became stone-free). By using McNemar's test, the difference in stone clearance rates between the two groups was not found to be statistically significant (p = 0.742). A total of 22 patients (19.1%) in Group A and 17 patients (14.7%) in Group B underwent an adjuvant procedure to achieve stone clearance. McNemar's test also revealed the absence of any statistically significant difference in SWL success rates between older and younger patients in the subgroups of patients presenting with either ureteric or renal stones (p = 0.727 and p = 0.571, respectively). In conclusion, SWL is still considered one of the first-line tools for geriatric patients suffering from urolithiasis, as increased age alone does not seem to adversely affect the efficacy of SWL.  相似文献
