BACKGROUND. Shift work is related to fatigue and desynchronization with the external environment. This study investigates how 6-h shifts and 12-h shifts affects sleep and safety in workers onboard offshore supply vessels, and if any differences exist between the two working schedules. MATERIAL AND METHODS. A questionnaire study was carried out in the North Sea, Australia, Africa, South America, and the Far East, with 577 participants. The offshore fleet workers gave information on parameters related to sleep disturbances, causes of sleep disturbances, and safety. Regional differences in these parameters were also investigated. RESULTS. Workers on 6-hour shifts reported significantly more sleep problems than 12-hour shift workers did (p 〈 0.01). The 6-hour workers were more affected by noise (p 〈 0.01) and shift-work itself (p 〈 0.01). CONCLUSIONS. Those working 6-hour shifts suffer more from sleep disturbances than those on 12-hour shifts, but this is not reflected in the perception of safety within the individual. Noise and shift-work itself is more of a problem in the 12-hour workers. Differences in safety culture and work morale are likely to cause the differences between regions.  相似文献

The aim was to study the effect of job task and fast backward-rotating shifts on the time-of-day variations of cortisol, fatigue, and sleep disturbances in broadcasting sound engineers. The concentration of saliva cortisol and ratings of stress, sleepiness, and fatigue were followed at 3-hour intervals during the fast backward-rotating shifts in 26 sound engineers: 14 subjects from control rooms, aged 45.1 +/- 7.3 years, and 12 subjects working in direct transmissions, aged 51.7 +/- 6.0 years. Saliva cortisol was assessed using an radioimmunology kit. The participants reported for stress symptoms after the shifts and filled a sleep diary. The effects of job task, shift, and time-of-day were analyzed by tests of between-subjects effects (SPSS). Cortisol retained the typical diurnal pattern with a highly significant effect of the shift. The job task and the shift interacted significantly. Higher cortisol values during the morning and night shifts in engineers working in direct transmissions were found. Their stress ratings were also higher, as well as the ratings of sleepiness and fatigue. The quality of sleep was worse in engineers working in direct transmissions. In conclusion, our data indicate that stress and fast backward-rotating shifts in sound engineers working in direct transmissions affect physiological stress markers such as cortisol and increase sleepiness, fatigue, and sleep problems.  相似文献

This study was conducted among health care personnel (registered nurses and nurse aides) in a public hospital in S?o Paulo, Brazil. Work was organized in 12-hour daytime or nighttime shifts, followed by 36 hours off. The study aimed to evaluate how the nursing staff perceived the duration and quality of sleep both during and off work days, as well as their perception of alertness during working hours. There were significant differences between night and day in the duration of sleep (Student t test = 10.82; p < 0.000). Quality of daytime sleep after working night shifts was perceived as worse than nighttime sleep (Wilcoxon test, Z = 2.67; p < 0.007). Significant differences were detected in self-evaluation of alertness after the 2nd, 6th, and 10th hour of night shifts (Friedman = 63.0; p < 0.00). Alertness was perceived as worse during dawn hours. This is an indication of sleepiness at work and can have serious consequences for both health care workers and patients.  相似文献

This study examined the relationship between perceived adaptation to shift work and shift-related problems. A total of 608 male operators at nuclear power plants completed a set of validated questionnaires including a modified version of the Standard Shiftwork Index, which covered adaptation to shift work, fit to job content, chronotypes, chronic fatigue, sleep, naps, shift work locus of control (SHLOC), psychological health, social/family life, daytime sleepiness, workload, alertness on the job, and lifestyle factors. Participants were divided into two groups according to their perceived level of adaptation to shift work. The good adaptation group showed better outcomes than the poor adaptation group in terms of fit to job content, chronic fatigue, daytime sleep before night shifts, social and family disruption, SHLOC, psychological health, and alertness during night shifts (ps<0.001). Operators who reported good adaptation also took a more frequent, longer nap and more cigarettes during night shifts (ps<0.05). The cross-sectional study design cannot determine a causal relationship between perceived adaptation and shift work problems, yet the present results suggest that the effects of working shifts may be modified by perceptions of shift work adaptation.  相似文献

The aim was to study the effect of rotation direction on the time-of-day variations of cortisol, fatigue and sleep in sound engineers broadcasting in a life show. The salivary cortisol and ratings of stress, sleepiness and fatigue were followed at three hour intervals in 25 sound engineers: 13 working very fast forward-rotating shifts and 12 working very fast backward-rotating shifts. Cortisol was assessed with radioimmunological kits. The participants reported for stress symptoms and filled sleep diary. Cortisol retained the typical diurnal pattern. The rotation direction interacted with the shift significantly and as a result higher cortisol values during the morning and night shifts in the backward rotating group were found as well as worse quality of sleep. Higher salivary cortisol during morning and night shifts and worse quality of sleep in engineers working very fast backward-rotating shifts may be an indication for insufficient recovery.  相似文献

OBJECTIVES: This study attempted to clarify the health effects of implementing a 12-hour shift in place of the traditional 8-hour shift in work in a clean room in an electronic parts producing factory. METHODS: Health-check records during a year or longer before and after the shift change were reviewed regarding subjective symptoms, height, body weight, and blood pressure for 189 male workers who had moved to a 12-hour shift and for 16 male workers who remained on an 8-hour shift. RESULTS: The workers changing to a 12-hour shift showed significant increases in subjective symptoms, particularly psychological symptoms defined as related to fatigue and bodyweight gain, 1 kg on average, in the year after the shift change. Increased symptoms and body weight remained high even another year later. The workers remaining in the 8-hour shift did not show significant changes in symptoms or body weight during the observation. No changes in blood pressure were observed in relation to the shift move. CONCLUSIONS: The results suggested that implementing a 12-hour shift caused marked psychological fatigue and unhealthy weight gain among some cleanroom workers and that the adaptation to the new work shift did not seem easy for these workers. Working in a clean room can be considered a job in which special caution is needed regarding increased fatigue when a compressed working schedule is implemented. Attention should also be paid to possible weight gain among persons working 12-hour shifts.  相似文献

OBJECTIVES: To investigate of the effects of distribution of rest days in 12 hour shift systems. Although several studies have examined the effects of compressing work schedules by comparing 8 and 12 hour shift systems, there is little published research examining the various forms of 12 hour shift system. METHODS: An abridged version of the standard shiftwork index which included retrospective alertness ratings was completed by a large sample of industrial shiftworkers. The respondents worked 12 hour shift systems that either did or did not incorporate breaks of > 24 hours between the blocks of day and night shifts. For the purposes of the analysis, each of these two groups were further subdivided into those who started their morning shift at 0600 and those who started at 0700. RESULTS: Systems which incorporated rest days between the day and night shifts were associated with slightly higher levels of on shift alertness, slightly lower levels of chronic fatigue, along with longer sleep durations when working night shifts and between rest days. Early changeovers were associated with shorter night sleeps between successive day shifts, but longer and less disturbed day sleeps between night shifts. These effects of changeover time were broadly in agreement with previous research findings. CONCLUSIONS: The distribution of rest days in 12 hour shift systems had only limited effects on the outcome measures, although the few modest differences that were found favoured systems which incorporated rest days between the day and night shifts. It is conceded that the design of the study may have obscured some subtle differences between the shift systems. Nevertheless, it is concluded that the impact of distribution of rest days seems to be minor relative to previously found effects of other features of shift systems--for example, shift duration.

The objective of this study was to examine the perception of conflict between work and family roles and job, family, and life satisfaction among nurses in Croatia. One hundred and twenty-nine nurses (married mothers) working in hospitals in Zadar, ?ibenik, and Split were divided in four groups according to their worktime schedule. The participants completed a survey, which included a set of sociodemographic-type questions, questions about the level and allocation of family responsibilities between spouses, and scales measuring the perceived negative effects of worktime, psychological demands of the work, work-family conflict, and semantic differential scales for measuring the affective and cognitive-evaluative component of job, family, and life satisfaction. This was the first study in Croatia to deal with work-family conflict among nurses or workers with different shift systems.The results of this study indicate that nurses working morning shifts only experienced less conflict between work and family than other groups of nurses, who worked the morning, afternoon, and the night shift. The cognitive-evaluative component of job satisfaction was the highest among morning shift nurses and the lowest in nurses who worked 12-hour shifts, while the affective component of life satisfaction was the lowest in nurses working irregular and backward rotated shifts. These results confirm that shiftwork makes the work-family role conflict even worse. They also support the view that the type of shift rotation matters.  相似文献

This paper reviews briefly our research findings on sleep and health for shift workers at a bakery and a dish factory and aims to give some information on health protection with improved sleep. Our medical examinations revealed that rises in blood pressure (BP) were frequently observed in male bakery workers on the fixed night shift. They took a two-hour nap from 1:00 to 3:00 during the night shifts according to our advice. As a result, their diastolic BP significantly decreased in subsequent years. Women working shifts before 3:00 at a dish factory seemed to show more sleep disturbance, higher daytime sleepiness, and more fatigue than those working shits from 3:00. Total sleep hours taken during the daytime was similar between two groups. Among the dish, factory workers who switched to midnight or night shifts, 70% reported slow recovery from fatigue and 30% felt deteriorated health. These women also reported increased efforts to obtain a deep sleep, probably making up for shortened sleep. Occupational physicians can act to improve shift work conditions through monitoring the health of workers, applying naps, and designing possible schedules to secure deep, sufficient sleep.  相似文献

BACKGROUND: Night work is associated with disrupted circadian rhythms, fatigue, accidents, and chronic disease. Melatonin secretion helps regulate sleep and circadian rhythms. OBJECTIVE: Melatonin, sleep disturbances, and symptoms (sleep, fatigue, mental) were compared among workers on permanent day, swing, and night shifts. METHODS: Urinary 6-hydroxymelatonin sulfate (6-OHMS) was measured in postwork and postsleep samples. Disrupted circadian melatonin production was evaluated using the sleep:work 6-OHMS ratio. Wrist actigraphy characterized light exposures and sleep characteristics. RESULTS: Night workers had altered melatonin, disrupted sleep, and elevated symptom prevalence. Subjects grouped by their sleep:work 6-OHMS ratio rather than shift had even greater symptom prevalence. Risks for two or more symptoms were 3.5 to 8 times greater among workers with sleep:work ratios < or =1 compared to those with ratios >1. CONCLUSIONS: This ratio may help identify workers at increased risk for accidents or injuries.  相似文献

The productivity of female shift workers, working on a weekly rotating three-shift system, was examined. The afternoon shift was found to be the most productive and the night shift the least productive one. The greatest difference in productivity between shifts was found in the first two days of the week, when the productivity on night shift was significantly lower than that on the other two shifts. From the third day on there were no longer significant differences in productivity between shifts. The most productive and the least productive workers on night shift did not significantly differ in extraversion or in sleep duration after the night shift. Family responsibility was found to be associated with the duration of sleep after the night shift: married women slept significantly shorter after the night shift than unmarried women. However, this difference in sleep duration was not associated with productivity on night shift. Sleep duration after the afternoon shift (8 hours 40 minutes) was on average two hours longer than after the other two shifts. The difference in sleep duration after different shifts, along with circadian variations in alertness, readiness for work and performance efficiency, could be responsible for differences in productivity between shifts.  相似文献

The Shinkansen Super-Express trains of the Japanese National Railways, the so-called bullet trains, run from early morning until late at night. Accordingly the car-cleaners of the Shinkansen must do late night shift work. A self-administered questionnaire survey including items about health and work conditions was given to 246 workers at Osaka Station, who were then divided into 3 groups: Group A consisted of 102 night workers working 5 successive days with 2 days off, Group B consisted of 124 alternate-day 24-hour workers working 3 days a week, while Group C included 20 day-workers working 6 days a week. A subjective fatigue inventory was administered before and after work for a week to 20 workers selected from each of three groups, group A, B above group D whose shift was 2 times 2 successive night shifts with one day off, and one holiday a week. The results of the questionnaire survey revealed that the rates of complaints such as gastroenteric disorders and general fatigue were the highest in group A. The results of the subjective fatigue inventory show a tendency for the number of complaints in group A and B to increase significantly after the last work shift of the week, but not in the case of group D. The authors demonstrated that there was a close relationship between patterns of night-shift work and various kinds of health problems, and concluded that to insert a day off on the third day of the group D shift was effective in reducing the work load.  相似文献

Purpose  There has been increasing interest in the occupational health of workers in small enterprises, especially in developing countries. This study examines the association between psychosocial job characteristics and fatigue, and attempts to identify risk factors for fatigue among workers of small enterprises in southern Taiwan. Methods  A structured questionnaire was administered to workers receiving regular health examinations between August 2005 and January 2006. The questionnaire collected demographic information and data on working conditions, personal health status and life styles. It also collected information on psychosocial job characteristics, fatigue and psychological distress using three instruments. Results  A total of 647 workers with mean age of 43.7 were completed. Probable fatigue was found in 34.6% of the sample. Fatigue was found by multiple logistic regressions to be associated with the lack of exercise, working in shifts, depression score and lack of social support at workplace. Conclusions  This study found associations between life style, psychosocial job characteristics and fatigue. Because the high prevalence of probable fatigue was found in such small enterprises, the authors suggest that a short interview with some quick questionnaires in health checkup for these small enterprise workers are helpful to early detect psychosocial and fatigue problems.  相似文献

Objectives:Rotating shift work is associated with adverse outcomes due to circadian misalignment, sleep curtailment, work-family conflicts, and other factors. We tested a bright light countermeasure to enhance circadian adaptation on a counterclockwise rotation schedule.Methods:Twenty-nine adults (aged 20–40 years; 15 women) participated in a 4-week laboratory simulation with weekly counterclockwise transitions from day, to night, to evening, to day shifts. Each week consisted of five 8-hour workdays including psychomotor vigilance tests, two days off, designated 8-hour sleep episodes every day, and an assessment of circadian melatonin secretion. Participants were randomized to a treatment group (N=14), receiving intermittent bright light during work designed to facilitate circadian adaptation, or a control group (N=15) working in indoor light. Adaptation was measured by how much of the melatonin secretion episode overlapped with scheduled sleep timing.Results:On the last night shift, there was a greater overlap between melatonin secretion and scheduled sleep time in the treatment group [mean 4.90, standard deviation (SD) 2.8 hours] compared to the control group (2.62, SD 2.8 hours; P=0.002), with night shift adaptation strongly influenced by baseline melatonin timing (r2= -0.71, P=0.01). While the control group exhibited cognitive deficits on the last night shift, the treatment group’s cognitive deficits on the last night and evening shifts were minimized.Conclusions:In this laboratory setting, intermittent bright light during work hours enhanced adaptation to night work and subsequent readaptation to evening and day work. Light regimens scheduled to shift circadian timing should be tested in actual shift workers on counterclockwise schedules as a workplace intervention.  相似文献

OBJECTIVES: We sought to measure melatonin levels and 24-hour light intensity exposure in health care workers over a 7-day period in natural occupational and residential settings. METHODS: Five office workers and 17 nurses working either days or rotating night and day shifts wore a device to record light intensity exposure for one or two 7-day periods, completed a questionnaire, and provided three saliva samples for melatonin. RESULTS: Rotating shift workers had irregular light exposure patterns and abnormal melatonin levels compared with those working days. In addition to lower-than-normal melatonin levels during sleep periods, rotating shift workers exhibited higher-than-normal melatonin levels on arising and during work. Self-reported years of shift work were correlated with measured melatonin and light. CONCLUSIONS: Rotating shift work is supported as a surrogate for exposure to light-at-night and circadian disruption.  相似文献

Summary This study was designed to assess and compare the quantity and quality of sleep of morning, afternoon, and night shift workers during the winter and summer seasons, and to evaluate the effects of sleep deficiency on the work effort and fatigue of the workers. The subjects studied consisted of a sample of 150 textile workers, working on a monthly rotating 3-shift system: morning shift (06.00–11.00 and 14.00–17.00) afternoon shift (11.00–14.00 and 17.00–22.00), and night shift (22.00–06.00). Information concerning quantity and quality of sleep were registered daily over a week for every subject. The Ratings of Perceived Effort scale and the Feeling and Symptoms of Fatigue scale were used to collect information about feeling of effort and fatigue of the subjects. The results showed that the duration of sleep length is longest in the afternoon and shortest in the night shift both during summer and winter. The amount of sleep is shorter during summer during all three shifts. The workers spent more effort and felt more fatigued during summer than winter. The shift workers, particularly of the tropical region, like Bangladesh, seem to be worst affected in terms of quantity and quality of sleep, health and well-being due to extremely high temperature (30–45° Celsius) during summer than winter (5–25° Celsius). The workers think that the problems of shift work could be minimized by improving the quality of working and living conditions, reducing cycle of rotation, shortening working day and closing night shift during peak summer.  相似文献

The relationship between the type of job and/or position of male white-collar workers to their working life, life style, health behavior, and health condition was examined based on data obtained from a survey of 1129 male workers in a major computer manufacturing company in 1988. The results were as follows: 1. Work, life style, health behavior, and health condition of male white-collar workers are strongly affected by type of job and position when adjusted for education, income, and age. 2. Workers in the sales department and engineering department appeared to have the most problems associated with working life, life style, health behavior, and health condition. Workers in sales, especially, displayed Type A behavior and their job was characterized by a lot of business transactions in a working environment that requires them to work until late at night. This leads to an irregular diet, late bed time, and makes their family relationships worse. Workers in the engineering department had work loads that were heavy because they have the longest working hours, and the content of their jobs is a mixture of both research and sales. Consequently their health is not good. On the other hands, workers in the laboratory had the best situation, and with general workers about average in terms of working life, life style, health behavior, and the state of health. 3. The department heads have the best conditions of life and health over all, except for their diet. The chiefs of sections are generally in the middle between the department heads and ordinary workers in terms of life and health. They drink and smoke heavily and show Type A behavior characteristics. Ordinary workers appear to have the most problems in working life, life style, health behavior, and health condition. From these results it may be concluded that this is associated with the difference in work type and position of these workers compared to managerial class and also with a low level of morale.  相似文献

Shift work has been associated with increased risk of cardiovascular disease. This study was designed to determine the hemodynamic effects of 12-hour (12-h) shifts, and changes in blood pressure (BP) and heart rate variability (HRV) during 36 h rest time following 12-h shifts. Fifteen male shift workers with a mean age of 32.9 yr were recruited from a semiconductor factory. Ambulatory BP (AmBP) monitoring was performed for a total of 48 h for each participant. Six workers were monitored for 48 h by Holter electrocardiogram on both the day and night shifts. Paired self-comparison was used to estimate the difference between two hourly measurements of 12-h BP, HR, and HRV using the same timetable intra-individually. We also applied mixed models to estimate the effects of 12-h shifts on the delayed recovery of BP and heart rate (HR) in six workers who completed 96-h AmBP monitoring, including a 48-h night shift-rest period and another day shift period. Results showed that 12-h night shift work gave a persistently elevated systolic and diastolic BP (SBP and DBP) and HR, and decreased HRV compared to 12-h day shift work with the corresponding resting time. In addition, there was delayed SBP and DBP recovery on the first 12-h rest time in night shift workers, which was further demonstrated on the second 12-h rest time after adjustment for possible confounders through mixed models. In conclusion, 12-h night shift work may elevate BP and HR and decrease HRV. It is also associated with delayed BP recovery.  相似文献

OBJECTIVES: This study evaluated the effects of bright light and melatonin on adaptation to night work on an oil rig in the North Sea. METHODS: Seventeen persons working a schedule of 2 weeks on a 12-hour shift, with the first week on night shift and the second week on day shift (ie, the swing shift schedule) participated. In a randomized controlled crossover design, the shift workers received a placebo, melatonin (3 mg, 1 hour before bedtime), or bright light (30-minute exposure, individually scheduled) during the first 4 days on the night shift and during the first 4 days on the day shift. Subjective and objective measures of sleepiness (Karolinska Sleepiness Scale and a simple serial reaction-time test) and sleep (diary and actigraphy) were recorded. RESULTS: Subjective measures indicated that melatonin modestly reduced sleepiness at work during the day shift and increased sleep by 15-20 minutes per day. Bright light gave values in between those of melatonin and the placebo, but with few significant results. According to the objective measures, bright light improved sleep to a minor degree during the night shift. Hardly any side-effects were reported. CONCLUSIONS: Melatonin and bright light modestly improved sleep and sleepiness in this field study. In well-controlled simulated nightwork studies, both melatonin and bright light are more effective in alleviating sleepiness and sleep problems. The less effect in this field study may be due to competing or conflicting factors present in real life or to an inoptimal timing and duration of the treatments.  相似文献

Objectives:This study aimed to estimate acute effects of roster characteristics on fatigue and sleep quality and investigated whether these effects differed by individual characteristics.Methods:Using an ecological measurement assessment survey, fatigue and sleep quality were daily measured among 223 shift workers for up to eight weeks. A questionnaire assessed baseline characteristics, and roster data were retrieved from the company registers to determine roster parameters. The effects between each shift parameter on fatigue and sleep quality were estimated with random- and fixed-effects models.Results:Compared to day shifts, night shifts were related to fatigue [β=0.22; 95% confidence interval (CI) 0.05–0.39] and poorer sleep quality (β =0.64; 95% CI 0.47–0.80), and more successive night shifts with more fatigue (up to β=0.68; 95% CI 0.49–0.87 for ≥2 nights). Fatigue was increased after a quick return (<11 hours) (β=1.94; 95% CI 1.57–2.31) or 11–16 hours (β=0.43; 95% CI 0.26–0.61) compared to >16 hours between shifts. Compared to forward rotation, stable (β=0.22; 95% CI 0.01–0.43) and backward rotation (β=0.49; 95% CI 0.23–0.74) were also associated with more fatigue. Workers with a morning or intermediate chronotype had poorer sleep quality after a night shift, while workers with poor health reported poor sleep quality as well as more fatigue after a night shift.Conclusions:To alleviate acute effects of shift work on fatigue, shift schedules should be optimized by ensuring more time to recover and rotate forwards.  相似文献
