卷烟厂男性职工恶性肿瘤死亡回顾性队列研究   总被引:1,自引:0,他引:1  
为分析烟草尘对卷烟厂男性职工恶性肿瘤死亡专率的影响,采用回顾性队列研究的方法,对河南、黑龙江两省某两卷烟厂1982~1994年间在册的男性职工进行恶性肿瘤死因分析。以队列成员中的非暴露于烟草尘组为内对照,比较两组间的恶性肿瘤死亡专率的差异。结果表明男性职工暴露组的恶性肿瘤死亡专率显著高于非暴露组,且可能存在剂量-效应关系。提示接触烟草尘可能是恶性肿瘤死亡的危险因素,且吸烟和接触烟草尘对上述疾病可能存在协同作用。  相似文献

目的 比较接尘、吸烟对呼吸系统疾病死亡的影响.方法 在1989年至1992年广州建立职业健康监护档案的基础上对30岁以上、职业接触粉尘和无粉尘接触的80 987名工人进行前瞻性队列研究.结果 (1)队列平均年龄43.5岁,职业性粉尘接触率16.3%,吸烟率43.7%,饮酒率33.5%.(2)队列平均随访8年,35人失访,失访率0.04%,总死亡1 593人中,219人死于肺癌,90人死于非恶性肿瘤性呼吸系统疾病.(3)调整相关因素后,相对于不吸烟者,吸烟者肺癌死亡相对危险度(RR)为3.32,是粉尘接触者相对于无粉尘接触者肺癌死亡RR(1.53)的2.2倍.粉尘接触者非恶性肿瘤性呼吸系统疾病死亡RR为2.41,是吸烟者(1.89)的1.28倍.尤其是矽尘接触者,其非恶性肿瘤性呼吸系统疾病死亡RR为5.72,是吸烟者的3.03倍.既接触粉尘又吸烟,呼吸系统疾病死亡RR增加更明显.(4)男性吸烟者呼吸系统疾病死亡危险分别随日吸烟量、烟龄的增加而增加.结论 职业接触粉尘和吸烟均可引致超额的呼吸系统疾病死亡,两者存在协同作用.吸烟致肺癌死亡RR高于接尘,而致非恶性肿瘤性呼吸系统疾病死亡RR则低于接尘.吸烟与呼吸系统疾病死亡危险存在明显的剂量-效应关系.  相似文献

目的 比较接尘、吸烟对死亡的影响。方法 以1989~1992年广州市实施并建立的职工职业健康监护档案为基础资料,选年龄≥30岁的80987名接尘和无接尘职工为研究对象,进行前瞻性队列研究。结果 (1)队列平均43.5岁,主要为工人、中学文化程度、已婚,接尘率16.3%,吸烟率43.7%,饮酒率335%。(2)队列平均随访8年,失访35人,死亡1539人,以恶性肿瘤死亡为主。(3)调整相关混杂因素后,全死因、恶性肿瘤、心脑血管疾病等死亡相对危险度(RR),接尘者和吸烟者基本一致,但鼻咽癌、呼吸系统疾病死亡RR值,接尘者高于吸烟者,而肺癌、胃癌死亡RR值,吸烟者分别是接尘者的2.2倍和1.5倍;接尘可协同吸烟致死亡危险性明显增加。(4)男性总死因、恶性肿瘤和呼吸系统疾病死亡RR值,矽尘接触者高于吸烟者,心脑血管疾病死亡RR值,木尘接触者也高于吸烟者。(5)人群死亡归因危险度百分比(PARP)吸烟者是接尘者的2.5倍。(6)男吸烟者全死因、恶性肿瘤、肺癌、胃癌的死亡危险随日吸烟量、烟龄的增加而明显递增,冠心病、呼吸系统疾病的死亡危险则分别随日吸烟量、烟龄的增加而增加。结论 接尘、吸烟者死亡RR值基本一致,接尘与吸烟存在协同作用,某些死因死亡危险吸烟者较明显,某些接尘者较明显;吸烟者PARP较接尘者高;吸烟与死亡危险存在明显的剂量效应关系。  相似文献

本文报告橡胶制品工业恶性肿瘤死亡率情况。采用回顾性队列流行病学研究方法,共调查3 573人,3 455人进入队列,失访率为3.3%,得67 693人年。1965~1984年间共死亡346人,其中恶性肿瘤占33.2%,为全部死因之首。全部职工和男性职工全癌死亡、肝癌死亡、膀胱癌死亡与当地居民比较相差具有显著意义。按工种分析,压胶工种恶性肿瘤SMR191,P<0.05。将全部职工分为直接接触组和间接接触组分析比较,直接接触组肝癌和膀胱癌的死亡率高于间接接触组。  相似文献

本文对安徽省某卷烟厂1981年1月1日至1990年12月31日十年间94例职工死亡原因进行回顾性调查。结果显示:卷烟厂职工标化死亡率明显高于当地居民。女性残废年龄提前16.45岁,男性提前8.76。恶性肿瘤死亡顺位较居民提高,肺癌死亡率为34.78/10万,女性恶性肿瘤中肺癌死亡纺为44.44/10万,乳腺癌死亡率为22.22/10万,均高于当地居民。提示烟草尘致癌的可能性不能排除,有待进一步研究  相似文献

目的 研究接尘和吸烟对死亡的影响。方法 以1989-1992年广州实施并建立的职工职业健康监护档案为基础资料,选年龄≥30岁的接尘工人和无任何粉尘及其他有害物质接触的职工(对照组)61648名为研究对象,进行前瞻性队列研究。结果 (1)队列失访28例,死亡1186例,以恶性肿瘤死亡为主;(2)本队列男性多于女性,年龄35-44岁,主要为工人,中学文化,已婚,率42.2%,饮酒率32.8%;(3)调整相应混杂因素后,全死因、恶性肿瘤和呼吸系疾病死亡接尘者相对危险度(RR)分别为1.24、1.34和1.96,与对照组比较,差异均有显著性(P<0.01),吸烟者RR分别为1.16、1.37和1.63,与不吸烟者比,前二者差异有显著性(P<0.01);既接尘又吸烟者上述相应死因死亡1RR分别为1.48、1.85和3.12,明显高于单纯接尘或单纯吸烟者,差异有显著性(P<0.01);(4)接尘对死亡的影响主要表现在职业接触矽尘和木尘。结论 接尘与吸烟均可致死亡危险性增加,并存在协同作用。  相似文献

目的 探讨单纯接触温石棉的工人恶性肿瘤,尤其肺癌是否超高。方法 采用固定队列研究方法。研究队列为515例男性石棉工人,人列条件为1972年1月1日工资在册,工龄满1年,没有明显心肺疾患者。对照队列为650例不接尘男性工人,其余条件同研究队。追踪27年(1972-1998)。结果 (1)研究队列全癌死亡50例(SMR=144),其中肺癌22例(SMR=652);.对照队列全癌死亡1例(SMR=34),肺癌3例(SMR=89)。两队列间全癌和肺癌差异均有显著性(P<0.05)。(2)不接触石棉的吸烟者肺癌RR为2.6,不吸烟的石棉接触者肺癌RR为12.2,而接触石棉的吸烟者RR高达32.1。吸烟和石棉暴露协同指数为2.2。(3)研究队列发生胸膜间皮瘤2例。结论 单纯暴露于温石棉的工人肺癌显著超高。  相似文献

BACKGROUND: An association between diesel exhaust exposure and lung cancer mortality in a large retrospective cohort study of US railroad workers has previously been reported. However, specific information regarding cigarette smoking was unavailable. METHODS: Birth cohort, age, job, and cause of death specific smoking histories from a companion case-control study were used to impute smoking behavior for 39,388 railroad workers who died 1959-1996. Mortality analyses incorporated the effect of smoking on lung cancer risk. RESULTS: The smoking adjusted relative risk of lung cancer in railroad workers exposed to diesel exhaust compared to unexposed workers was 1.22 (95% CI = 1.12-1.32), and unadjusted for smoking the relative risk was 1.35 (95% CI = 1.24-1.46). CONCLUSIONS: These analyses illustrate the use of imputation in record-based occupational health studies to assess potential confounding due to smoking. In this cohort, small differences in smoking behavior between diesel exposed and unexposed workers did not explain the elevated lung cancer risk.  相似文献

BACKGROUND: We describe the results of a cancer mortality study among asphalt workers in Israel. METHODS: Personal identifiers and employment histories of 2,176 workers were extracted from company records. RESULTS: Mortality from all malignant neoplasms was significantly reduced in the whole cohort (SMR 0.68, 95% confidence interval (CI) 0.56-0.83). SMR for lung cancer was elevated in workers exposed to bitumen (SMR 1.05, 95% CI 0.62-1.66). No significant elevation or reduction in mortality was observed in relation to a specific site. SMRs for lung cancer was higher among ever exposed to bitumen than among unexposed. There was no association between lung cancer risk and estimated exposure to bitumen fume, and no dose-response was apparent. CONCLUSIONS: While the results of this cohort study indicate a slightly increased SMR for lung cancer, it did not produce evidence of a causal link between lung cancer and exposure to bitumen fume.  相似文献

The mortality risk of iron ore (haematite) miners between 1970 and 1982 was investigated in a retrospective cohort study of workers from two mines, Longyan and Taochong, in China. The cohort was limited to men and consisted of 5406 underground miners and 1038 unexposed surface workers. Among the 490 underground miners who died, 205 (42%) died of silicosis and silicotuberculosis and 98 (20%) of cancer, including 29 cases (5.9%) of lung cancer. The study found an excess risk of non-malignant respiratory disease and of lung cancer among haematite miners. The standardised mortality ratio for lung cancer compared with nationwide male population rates was significantly raised (SMR = 3.7), especially for those miners who were first employed underground before mechanical ventilation and wet drilling were introduced (SMR = 4.8); with jobs involving heavy exposure to dust, radon, and radon daughters (SMR = 4.2); with a history of silicosis (SMR = 5.3); and with silicotuberculosis (SMR = 6.6). No excess risk of lung cancer was observed in unexposed workers (SMR = 1.2). Among current smokers, the risk of lung cancer increased with the level of exposure to dust. The mortality from all cancer, stomach, liver, and oesophageal cancer was not raised among underground miners. An excess risk of lung cancer among underground mine workers which could not be attributed solely to tobacco use was associated with working conditions underground, especially with exposure to dust and radon gas and with the presence of non-malignant respiratory disease. Because of an overlap of exposures to dust and radon daughters, the independent effects of these factors could not be evaluated.  相似文献

对全国12个城市的13个聚氯乙烯制造工人中5459名氯乙烯作业工人和6461名对照厂工人进行了从投产到1989年12月31日为止的恶性肿瘤发病情况的回顾性和前瞻性队列调查。结果表明,男工接触组肝癌的发病率、死亡率均显著高于对照组,男性肝癌的死亡率也显著高于我国中等城市男性肝癌的死亡率。在恶性肿瘤的分类构成中,接触组男工的肝癌居首位。接触组男工肝癌的发病年龄比对照组显著提前。  相似文献

Mortality in a cohort of 770 workers with potential pentachlorophenol (PCP) exposure was evaluated from 1940 through 1989. The study cohort is a subset of a larger cohort of workers with potential exposure to higher chlorinated dioxins. Total mortality and cancer mortality, in the PCP cohort were slightly lower than expected in comparison to the U.S. white male population. There were 229 total deaths with 242.5 expected (SMR = 94, 95% confidence interval 83–108), and 50 cancer deaths with 52.6 expected (SMR = 95, 95% confidence interval 71–125). In comparison with unexposed employees, the risk ratio for total mortality was 1.03 (95% confidence internal 0.90–1.17), and the risk ratio for all cancer mortality was 0.95 (95% confidence interval 0.71–1.26). In most cause of death categories of a priori interest no deaths were observed in the cohort. A small excess of other and unspecified lymphopoietic cancer deaths was observed but did not appear to be related to exposure. Excesses of deaths due to cancer of the kidney, gastric and duodenal ulcer, cirrhosis of the liver, and all accidents were observed in comparison with the U.S. white male population and with unexposed employees. These were associated with increasing estimated cumulative PCP exposure after lagging exposures by 5 and 15 years. Despite the limited size and the generally favorable total mortality experience of the cohort, it was concluded that cohort members may have incurred increased risk of death due to some specific causes. The risks could not, however, be attributed conclusively to PCP exposure and may have been associated with other occupational and nonoccupational factors. 目的调查接触温石棉女工的恶性肿瘤死亡率。方法以1960年1月1日至1980年12月31日曾经从事家庭内温石棉手工纺织作业一年以上的女工为调查对象,采用历史性队列流行病学方法进行研究。结果在5681名接触温石棉纺织女工中,有144名死于癌症,其中有74名女工死于肺癌,病死率最高;其次为肝癌,死亡数为27例;占第三位的是胃癌,死亡数为18例。以当地女性年龄组别恶性肿瘤死亡率均值为对照组计算标准化死亡比(SMR),所有除肺癌外的恶性肿瘤与肺癌的SMR分别为1.16(P<0.05)和4.17(P<0.01)。结论接触温石棉的女性纺织工人有明显的肺癌死亡高发倾向。  相似文献
