The objective of this study was to arthroscopically analyse the morphology and dynamics of variants of the anterior horn of the medial meniscus of the knee (VAMM) and to then consider the pathological significance of these variants. VAMM was defined as knees in which the anterior horn of the medial meniscus is not attached to the tibia. Between April 1992 and March 1995, arthroscopy was performed on 953 knees of 903 patients. At the time of this examination, observation and probing were performed to determine the condition of the synovium, the synovial plica, the cartilage in all compartments, the meniscus, the cruciate ligaments, and the popliteal tendon. In particular, detailed examination was made of the anterior horn of the medial meniscus with regard to the point of insertion to the tibia and the degree of movement in knee flexion/ extension. Cases of VAMM diagnosed on the basis of the arthroscopic findings were classified into the following four categories: the ACL (anterior cruciate ligament) type, where the anterior horn of the medial meniscus was attached to the ACL; the transverse ligament type, where the anterior horn of the medial meniscus was attached to the transverse ligament; the coronary ligament type, where the anterior horn of the medial meniscus was attached to the coronary ligament; and the infrapatellar fold type, where the anterior horn of the medial meniscus was attached to the infrapatellar synovial fold. These patients were then analyzed with regard to the arthroscopic findings and the intra-articular lesions other than VAMM. In 98 (10.9%) of the total patients, 103 knees were classified as VAMM. Classification of those 103 knees using the above criteria showed 39 ACL type knees, 51 transverse ligament type knees, 11 coronary ligament type knees, and 2 infrapatellar fold type knees. The arthroscopic findings indicated that the anterior horn of the medial meniscus was not attached directly to the tibia in any of these knees. Probing and flexion/extension of the knee revealed hypermobility at the anterior horn of the medial meniscus. In this study, anterior knee pain syndrome was diagnosed in 12 (11.7%) of the 103 VAMM knees. In addition, there was no clear history of trauma in 20 of 23 knees found to have an isolated medial meniscus tear. In these cases, even detailed arthroscopic observation proved the causes of the symptoms or injury. On the basis of these findings, we surmised that the anterior portion shows hypermobility at the time of flexion/extension of the knee, regardless of the type of VAMM. In this study, we discussed the possibility that the existence of VAMM may become the cause of pain or injury to the meniscus.  相似文献

 目的 研究前十字韧带损伤合并内侧半月板后角 Ramp(内侧半月板后角至后内侧关节 囊滑膜移行区域)损伤的发生率, 并分析其影响因素。方法 2002年 4月至 2007年 10月, 采用韧带重 建手术治疗并确诊为前十字韧带损伤的患者 868例, 男 609例, 女 259例;年龄 15~55岁, 平均 25.2岁。根据手术记录以及术中关节镜探查, 筛选其中存在内侧半月板后角 Ramp损伤的病例, 计算其发生率。 按照性别、年龄段及受伤至手术时间分组, 分析 Ramp损伤发生率的影响因素。结果 868例膝关节确诊为前十字韧带损伤并接受韧带重建手术的患者中, 内侧半月板后角 Ramp损伤的患者为 144例, 男 113例, 女 31例;平均年龄 24.7岁。受伤至手术时间平均 29.3个月。 Ramp损伤的发生率为 16.59% (144/868), 伤后 6周、3、6、12、24个月 Ramp损伤的发生率分别为 12.66%、13.51%、13.17%、14.49%、 15.92%;男性发生率为 18.56%(113/609), 女性为 11.97%(31/259), 两者比较差异有统计学意义;30岁以下的患者 Ramp损伤发生率为 21.59%(114/528), 30岁以上者为 8.82%(30/340), 两者比较差异有统 计学意义。结论 内侧半月板后角 Ramp损伤是前十字韧带断裂的常见合并损伤, 其发生率为 16.59%; 伤后 2年内, Ramp损伤的发生率随受伤时间延长明显增加;男性及 30岁以下患者 Ramp损伤发生率明 显增高。  相似文献

This study investigated 11 patients with localized pigmented villonodular synovitis of the knee that was diagnosed and treated by arthroscopic technique. There were six male and five female patients between the ages of 15 and 59 years (mean, 34.6 years). Seven patients reported extension limitation without joint line tenderness. Four of the 11 patients had a history of trauma before the onset of knee symptoms. All patients were treated by arthroscopic resection with partial synovectomy. The most common involved site was the anteromedial synovium near the anterior horn of the medial meniscus (five patients). The remaining cases were located in the anterior fat pad (two patients), suprapatellar pouch, posteromedial compartment, medial gutter, and the anterior horn of the lateral meniscus. Nine patients had one mass, and the remaining patients each had two or three masses. There was no evidence of recurrence at followup for an average of 29.9 months (range, 24-48 months). Arthroscopy is effective in the diagnosis of localized pigmented villonodular synovitis with minimal morbidity, and complete arthroscopic excision can be considered the definitive treatment for localized pigmented villonodular synovitis.  相似文献

We wanted to determine the frequency of occurrence and types of anomalous insertions of the medial meniscus to the anterior cruciate ligament (ACL) in Finnish patients. In 987 consecutive arthroscopies from January 1996 to August 1998, the meniscal pathology was mapped using videoprints and drawings. We found 11 anomalous medial meniscus insertions to the ACL. There were 4 female and 6 male patients in the series. In 1 woman, the anomaly was bilateral. The mean age of the patients was 30.6 years (range 14 to 63 years). The anomalous band was excised in all patients. The anomaly usually was not the finding that caused the need for arthroscopy. In 3 knees, the anomaly was the only pathologic finding. The frequency of this anomaly occurring was approximately 1.2%. The anomalous medial meniscus insertion to the ACL is a very rare clinical finding. In 1936 Campbell asserted that "impairment of the anterior crucial and mesial ligaments is associated with injuries of the internal cartilage." O'Donoghue in 1950 called attention to "that unhappy triad (1) rupture of the medial collateral ligament, (2) damage to the medial meniscus, and (3) rupture of the anterior cruciate ligament" and recommended early surgical intervention. In 1955 he reported 33 cases with both medial collateral (MCL) and anterior cruciate ligament (ACL) tears, but there were only three lateral meniscus tears reported. Based on a recent report by Shelbourne and Nitz that questions the validity of this unhappy triad, a review of all arthroscopically confirmed acute injuries of second degree or worse to the ACL and MCL was undertaken. Of a total of 52 knees reviewed, 50 knees had third-degree ACL tears and two had second-degree ACL tears. One of the second-degree tears was associated with a second-degree MCL and one with a third-degree ACL tear. Neither had an associated meniscus tear. Forty-five third-degree ACL tears were associated with third-degree MCL tears (group 1) and five with second-degree MCL tears (group 2). Eighty percent (36 knees) of group 1 had lateral meniscus tears. Only 29% of group 1 (13 knees) had associated medial meniscus tears. None of these medial meniscus tears was isolated. Eighty percent (four knees) of group 2 had lateral meniscus tears with only one associated medial meniscus tear. Again, there were no medial meniscus tears in the absence of a lateral meniscus tear. We did not find the combination of injury originally described as the unhappy triad.  相似文献

Many types of meniscal anomalies have been reported. The authors encountered two cases of anomalous insertion of the anterior horn of the medial menisci to the lateral femoral condyle, which ran up along the course of the anterior cruciate ligament (ACL), but was independent of the ACL. These anomalies were noted during arthroscopic examination and surgery of the ipsilateral knee for a torn discoid meniscus and a patellar fracture. A 34-year-old woman had a horizontal tear of the lateral discoid meniscus. We performed arthroscopic partial meniscectomy of the inner torn portion of the lateral discoid meniscus and contoured it to resemble a normal meniscus. An anomalous insertion of the medial meniscus was found on examination of the joint during surgery. A 32-year-old man had a patellar fracture and we performed reduction under arthroscopy and internal fixation with cannulated screws. The same anomalous insertion of the medial meniscus was also found on examination of the joint during surgery. Introduction Total meniscus resection usually leads to osteoarthritis of the knee joint. Preservation and refixation of the injured menisci are therefore of great clinical importance.Materials and methods The present study examines 40 meniscal injuries in 37 patients that were arthroscopically treated with Clearfix meniscal screws (Mitek, Norderstedt, Germany) in the period from August 1999 to December 2002. The mean patient age was 27.7 (range 16–62) years. Nine patients were female. A total of 24 patients (27 meniscal tears) also had an anterior cruciate ligament (ACL) lesion, and 5 patients had cartilage injuries. Twenty-two patients were treated within the first 2 weeks following the trauma, 10 patients within 8 weeks, and 5 patients after 8 weeks. The lesions were a bucket-handle tear of the medial meniscus in 11 cases and a bucket-handle tear of the lateral meniscus in 2 cases. The posterior horn of the medial meniscus was torn in 13 cases, and the pars intermedia in 3 cases. The posterior horn of the lateral meniscus was torn in 8 cases, and the pars intermedia in 3 cases. The mean tear length was 2.9 (±1.5) cm and was fixed with an average of 1.8 (±0.7) screws. In 7 cases, the anterior horn was treated with an additional meniscal suture. A total of 35 patients were examined after an average of 18 (range 7–45) months.Results In the event of a moderate outcome, MRI was performed as part of the follow-up investigation. The average Lysholm score was 93 (±7.4), the Tegner activity index was 6.3 (±2.0) before the accident and 5.8 (±2.0) at the follow-up, the Marshall knee score was 47 (±3.8). The VAS pain assessment was 1.6 (±1.3) and the VAS function assessment was 7.9 (±1.6). Of the 7 patients with a moderate result, 2 patients without additional ligament lesions suffered re-ruptures after 6 and 13 months, respectively. The other 5 patients with a moderate result each had multiple accompanying injuries or pre-existing damage to the affected knee joint.Conclusion The Clearfix screws achieved a clinical success rate of 82% in isolated meniscal tears in stable knee joints and a clinical success rate of 100% with additional ACL reconstruction. In view of the good clinical results and the simple procedure for use, the implant should be recommended for meniscal refixation.  相似文献

OBJECTIVE: Application of an arthroscopic suture system to restore the form and function of the meniscus by adaptation of a longitudinal tear close to the base. INDICATIONS: Unstable longitudinal tears near the base of the meniscus, mainly in the posterior horn of the medial or lateral of the meniscus. Dislocated bucket-handle tears of the medial and lateral meniscus close to the base. CONTRAINDICATIONS: Poor tissue quality with fibrillated meniscal tissue. Meniscal tears in the avascular zone (zone I). Insufficient blood supply from the joint capsule and the base of the meniscus. Degenerative meniscal lesions. Anterior or posterior knee joint instability. Allergic reactions to nonresorbable suture material. SURGICAL TECHNIQUE: Standard anterior arthroscopic portals. Arthroscopic assessment of the meniscal tear using the probe. Revitalization of the tear margins and perforation of the meniscal base to induce bleeding. Adaptation of the tear margins and fixation with a suture-anchor system using an ipsilateral standard portal for tears in the posterior horn or by way of a contralateral standard portal for tears in the lateral horn. POSTOPERATIVE MANAGEMENT: Full weight bearing with the knee in extension in a knee immobilizer, relative to the pain threshold in the 1st postoperative week. Range of motion exercises without weight bearing from full extension to 90 degrees knee flexion (0/0/90). If simultaneous reconstruction of the anterior cruciate ligament (ACL) is being performed, rehabilitation protocols follow the principles for ACL reconstruction. RESULTS: Since the year 2000, meniscal tears in more than 300 patients have been repaired with the all-inside suture system. In the context of a multicenter study by ESSKA (European Society for Sports Medicine, Surgery and Arthroscopy) involving 20 patients, the result was evaluated by direct MRI arthrography (gadolinium). The suture bar anchors were generally not detectable, the incision channels produced a hypodense signal in the meniscus tissue. If re-rupture occurred, it was because the meniscus had pulled out of the suture loop. Cartilaginous lesions were not found. There were no complications related to the nonresorbable suture bar anchors.  相似文献

Between December 1984 and March 1987, i.e. a period of 27 months, the authors operated on 59 "isolated" fresh ruptures of the ACL with routine evaluation of the posterior horns of the medial and lateral menisci. Lesions were investigated either by arthroscopy (23 cases) or by anterior arthrotomy with routine medial and lateral retro-ligamentous counter-incision (36 cases). This revealed 21 lesions of the medial meniscus (i.e. 35.5%) and 38 lesions of the lateral meniscus (i.e. 64.5%). Lesions of both menisci were present in 16 knees (27%) and only 16 knees (27%) were found to be free of any meniscal lesion. The majority of meniscal lesions were viable and could be sutured in 86% of cases for the medial meniscus and 87% of cases for the lateral meniscus. From the standpoint of operative technique, posterior lesions are relatively poorly visualized by arthroscopy (notably concerning the posterior horn of the medial meniscus though it is easier to assess the stability of the meniscus by this technique using the palpating hook. Lesions are well visualized by medial and lateral retroligamentous counter-incisions, but it is difficult to assess meniscus stability. Finally it should be noted that all of these ruptures of the ACL were dealt with by reconstruction of the central pivot either by suture and a strengthening procedure (semitendinous) or by ligament plasty from the outset.  相似文献

This article describes a case of bilateral sagged knees presenting as posterior, posterolateral rotatory instability with tears of the anterior horn of the lateral meniscus. Each knee had identical tears of the lateral meniscus anterior horn. A 42-year-old woman reported bilateral anterior knee pain and painful instability during running or jarring exercises. She reported no major trauma to her knees. Arthroscopic findings of her right knee revealed a posterior cruciate ligament that looked hypoplastic but was without acute injury, and the anterior horn of the lateral meniscus showed chronic complex tears with some degeneration. Posterior cruciate ligament reconstruction, posterolateral corner sling, and meniscal repair of the lateral meniscus anterior horn was performed on her right knee. Three months later, a similar operation was performed on her left knee. However, menisectomy was performed because the lateral meniscus anterior horn tear was in the junction of the red-white and white zones. At 18 months postoperatively, the patient reported no symptoms and was satisfied with her results. Physical examination showed no joint line tenderness, and posterior stress radiographs on both knees showed grade I posterior instability. She showed no posterolateral subluxaion by supine dial test, and her prone dial test also improved approximately 15° on both knees. Lysholm score was 74 preoperatively and improved to 92 postoperatively.  相似文献

Discoid medial meniscus is very rare, and bilateral discoid medial meniscus is extremely rare. We report a case of bilateral discoid medial meniscus. The patient had symptoms in the right knee but was asymptomatic in the left knee. We performed magnetic resonance imaging for both knees. The patient was treated by saucerization and by all inside repair of the anterior horn tear in the right knee. We performed second-look arthroscopy after 18 months.  相似文献

Sixty-four patients out of a much larger population who presented within 2 weeks of an acute injury to the knee were found at arthroscopy to have either a chronic tear or chronic incompetent anterior cruciate ligament (group I, 32 patients) or to have an acutely torn anterior cruciate ligament (ACL) (group II, 32 patients). Arthroscopy of both groups allowed the author to compare and contrast the lesions found in each group. Chondromalacia of the femorotibial joint is significantly more frequent in chronic ACL-deficient knees (p less than 0.05), and there is a tendency to degenerative meniscus lesions and chondromalacia patella in these knees too.  相似文献

Combined injury to the anterior cruciate ligament (ACL) and meniscus is associated with earlier onset and increased rates of post-traumatic osteoarthritis compared with isolated ACL injury. However, little is known about the initial changes in joint structure associated with these different types of trauma. We hypothesized that trauma to the ACL and lateral meniscus has an immediate effect on morphometry of the articular cartilage and meniscus about the entire tibial plateau that is more pronounced than an ACL tear without meniscus injury. Subjects underwent magnetic resonance imaging scanning soon after injury and prior to surgery. Those that suffered injury to the ACL and lateral meniscus underwent changes in the lateral compartment (increases in the posterior–inferior directed slopes of the articular cartilage surface, and the wedge angle of the posterior horn of the meniscus) and medial compartment (the cartilage-to-bone height decreased in the region located under the posterior horn of the meniscus, and the thickness of cartilage increased and decreased in the mid and posterior regions of the plateau, respectively). Subjects that suffered an isolated ACL tear did not undergo the same magnitude of change to these articular structures. A majority of the changes in morphometry occurred in the lateral compartment of the knee; however, change in the medial compartment of the knee with a normal appearing meniscus also occurred. Statement of clinical significance: Knee injuries that involve combined trauma to the ACL and meniscus directly affect both compartments of the knee, even if the meniscus and articular cartilage appears normal upon arthroscopic examination. © 2019 Orthopaedic Research Society. Purpose

To investigate the knee arthroscopic findings of pediatric patients with knee pain.

Subjects

Ninety-five knees of 94 patients (46 males and 48 females) aged 15 years or younger who underwent knee arthroscopy during a 4-year period from January 2007 were studied. The mean age at surgery was 13.5 (7–15) years. The mean interval from symptom onset to arthroscopic examination was 6.8 months (5 days to 2 years 10 months).

Results

The most common cause of knee pain was sports-related activities (64 knees). Other causes included falling from a moving bicycle (5 knees), while knee pain appeared with no defined reason in 14 knees. The most frequent final diagnosis based on knee arthroscopic findings was anterior cruciate ligament (ACL) injury (35 knees), followed by discoid lateral meniscus (16 knees), lateral meniscal tear (11 knees), and medial plica syndrome (9 knees), while no arthroscopic abnormality was observed in 8 of 95 knees. Among the 95 knees, the diagnosis based on preoperative physical tests and imaging findings was different from the arthroscopic diagnosis in 16 knees, 8 of which were diagnosed preoperatively as medial meniscal tear.

Conclusion

ACL injury and discoid lateral meniscus were the predominant conditions in pediatric patients who underwent knee arthroscopic surgery for knee pain. Knee arthroscopy is useful to provide a definitive diagnosis for knee pain in pediatric patients. Preoperative evaluations had a diagnostic accuracy of only 83.2 % and failed to diagnose conditions such as medial plica syndrome and chondral injury. Therefore, diagnosis before knee arthroscopy has to be interpreted with caution.  相似文献

Background:

The clinical relationship between medial meniscus tear and anterior cruciate ligament (ACL) rupture has been well documented. However, the mechanism of this clinical phenomenon is not exactly explained. Our aim is to investigate the biomechanical impact of partial and complete ACL rupture on different parts of medial meniscus.

Materials and Methods:

Twelve fresh human cadaveric knee specimens were divided into four groups: ACL intact (ACL-I), anteromedial bundle transection (AMB-T), posterolateral bundle transection (PLB-T), and ACL complete transection (ACL-T) group. Strain on the anterior horn, body part, and posterior horn of medial meniscus were measured under 200 N axial compressive tibial load at 0°, 30°, 60°, and 90° of knee flexion, respectively.

Results:

Compared with the control group (ACL-I), the ACL-T group had a higher strain on whole medial meniscus at 0°, 60°, and 90° of flexion. But at 30°, it had a higher strain on posterior horn of meniscus only. As to PLB-T group, strain on whole meniscus increased at full extension, while strain increased on posterior horn at 30° and on body of meniscus at 60°. However, AMB-T only brought about higher strain at 60° of flexion on body and posterior horn of meniscus.

Conclusions:

Similar to complete rupture, partial rupture of ACL can also trigger strain concentration on medial meniscus, especially posterior horn, which may be a more critical reason for meniscus injury associated with chronic ACL deficiency.  相似文献

According to our observation in ACL reconstruction, we find root tears of the posterior horn of the lateral meniscus as a common concomitant injury in ACL-deficient knees. This might be a consequence of initial trauma or of the increased anterior–posterior translation of the tibia and an overload impact on the posterior meniscus root in ACL-deficient knees. A tear of the posterior horn of the medial meniscus causes a 25% increase in peak pressure in the medial compartment compared with that found in the intact condition. The repair restores the peak contact pressure to normal (Allaire et al. in J Bone Joint Surg Am 90(9):1922–1931, [2008]). A tear of the posterior horn of the lateral meniscus might have similar consequences. We hypothesize the surgical anatomical reattachment of the root at the tibia helping to restore knee joint kinematics and helping to advance ACL-graft function. This article presents an arthroscopical technique to reattach the posterior meniscus root in combination with ACL double-bundle reconstruction. The procedure uses the tibial PL tunnel to fix the meniscus suture.  相似文献

Identifying and treating medial meniscal ramp lesions in conjunction with ligament reconstruction restores critical stability in knees with ligament injuries. This must begin with obtaining high-quality magnetic resonance imaging (MRI) and critical evaluation of the MRI and include a subsequent thorough arthroscopic examination of these knees. As evident in previous studies, most surgeons associate medial meniscal ramp lesions with anterior cruciate ligament (ACL) tears. Biomechanical studies have reported that a ramp lesion produces significant anterior tibial translation and external rotational instability in ACL-deficient knees that is not reestablished with an isolated ACL reconstruction. In addition, recent research identified ramp lesions in one-third of multiligament knee injuries with an intact ACL and two-thirds of patients with posteromedial tibial plateau bone bruises on MRI. Restoring knee stability and biomechanics is necessary in treating all knee ligament injuries. Don’t miss the meniscal ramp lesion. Have a high index of suspicion, obtain a high-quality MRI,and arthroscopically evaluate the meniscocapsular junction of the medial meniscus, especially if there is a bone bruise seen on MRI.  相似文献

Summary: This article adds 2 more cases of bilateral discoid medial menisci to the previously reported 9 cases. The radiographs of the first case revealed bilateral cupping of the medial tibial plateaus and a small lytic lesion of the left distal medial femoral metaphysis. Beside the posteriorly unstable incomplete type discoid medial meniscus, the left knee had several associated arthroscopic findings: depression of the medial tibial plateau with exposed subchondral bone, anomalous insertion of the anterior horn of the discoid meniscus to the anterior cruciate ligament, a pathological medial plica, and longitudinal deep fissures in the trochlea. The second case was a complete type with peripheral horizontal cleavage tears. Following arthroscopic surgery, magnetic resonance imaging (MRI) scans of the contralateral knees were obtained in both cases to search for bilaterality. MRI revealed discoid medial menisci in the unoperated knees as well. Meniscal transplantation in the human knee: a preliminary report   总被引:4,自引:0,他引:4  
Meniscectomy can result in degenerative disease, with younger patients developing problems in middle age (1). In addition, loss of a major portion of a medical meniscus may increase instability in an anterior cruciate deficient knee (2). For these two reasons, it is theoretically desirable to transplant menisci in selected patients. In this paper, six patients who underwent allograft transplantation of a meniscus (four medial and two lateral) are presented to demonstrate the apparent success of the operative procedure. Follow-up extends from 24 to 44 months, June 1986 to March 1988. All patients are free of episodes of locking. Four have undergone arthroscopy, which demonstrated healing of the meniscus and no evidence of shrinkage. KT 1,000 values returned to normal in four cases, with concomitant reconstruction of the anterior cruciate ligament (ACL). It is hoped that meniscal transplantation will offset the development of degenerative changes; however, this supposition will remain unproven for decades. Meniscal transplantation seemed to improve stability in the knees undergoing ACL reconstruction. Arthroscopy performed after transplantation revealed that healing does indeed occur. There was no evidence of rejection, confirming the feeling that meniscal tissue is "immunologically privileged." Meniscal transplantation is offered as an adjunct to other reconstructive procedures in cases of meniscal deficient knees.  相似文献

目的 探讨采用关节镜技术诊治膝关节局灶性色素沉着绒毛结节性滑膜炎(localized pigmented villonodular synovitis,LPVNS)的临床疗效.方法 2004年6月至2007年9月,关节镜下辅助诊治12例LPVNS患者,男5例,女7例;年龄22~53岁,平均36.3岁.病程最短1个月,最长84个月,平均19.5个月.关节镜下行关节内肿物和局部滑膜切除术,根据膝关节Lysholm评分评估术后疗效.结果 膝关节镜检查发现2例伴内侧半月板前角损伤,1例伴外侧半月板损伤,4例内侧滑膜皱襞增生Ⅲ度,3例髌股关节炎Ⅱ度,其中1例伴内外侧髁软骨Ⅲ度磨损.3例肿物来源于内侧半月板前角附着处滑膜,1例在外侧半月板前角附着处滑膜,3例在髌下脂肪垫,1例在髁间窝,2例在内侧沟,1例在外侧沟.1 例来源于膝关节后纵隔内侧面滑膜.肿物大小平均1.7 cm×1.4 cm×0.8 cm(0.5 cm×0.5 cm×0.5 cm~3 cm×3 cm×2 cm);2例无蒂,10例带蒂,术后组织切片均明确LPVNS的诊断.术后无一例发生膝关节感染.术后随访6~45个月,平均19.9个月.7例无任何不适,3例膝关节下蹲时存在疼痛,1例活动后肿胀,1例行走后疼痛.术后膝关节Lysholm评分为78~100分,平均96.3分;8例优,3例良,1例中.结论 关节镜技术是诊治膝关节LPVNS的一种有效方法 .  相似文献
