Removal of gutta-percha and various sealers was assessed during retreatment of the root canal system. Sixty root canals were prepared and obturated with gutta-percha and Ketac-endo, Roth's 801 cement, or AH26. After 14 days, the canals were retreated by hand or ultrasonic technique and the retreatment time was recorded. The roots were split and the amount of debris that remained on the canal walls in three separate levels was recorded by use of a dissecting microscope. The mean scores were compared by one-way and two-way ANOVA. The total score was significantly the highest for Ketac-endo (p less than 0.04), followed by AH26 and Roth's 801 cement. The largest amount of debris was consistently found in the apical third, and generally it was comparable for both retreatment techniques. The ultrasonic technique was significantly faster than the hand technique for all sealers tested (p less than 0.0004, p less than 0.0003, p less than 0.02, respectively). Hand retreatment of Ketac-endo was the slowest, whereas ultrasonic retreatment of Roth's 801 cement was the fastest. The results indicated that Ketac-endo endodontic sealer can be effectively removed from the root canal by ultrasonic instrumentation.  相似文献

OBJECTIVE: To evaluate and compare the sealability and radiographic quality of root fillings in extracted teeth by using lateral condensation of gutta-percha or alpha-phase gutta-percha in conjunction with a single gutta-percha cone. STUDY DESIGN: A total of 108 single-rooted teeth were divided into 2 groups of 54 teeth based on canal shape. Canals were prepared to a minimum 0.055 taper and enlarged to a size 35 at the working distance. Irrigation was done with 17% EDTA and 2.5% sodium hypochlorite (NaOCl) to remove smear layer. One group was obturated by using cold lateral condensation of gutta-percha; the other group was obturated with thermomechanically compacted alpha-phase gutta-percha and a single cone of gutta-percha. Apical extrusion of sealer or gutta-percha was recorded. Sealability of each technique was assessed by dye penetration. The radiographic quality of obturation was also determined. RESULTS: Root canals filled with alpha-phase gutta-percha and a single cone had significantly more extrusion of sealer than canals filled by lateral condensation. Lateral condensation achieved significantly better scores for radiographic quality. There was no significant difference between the 2 filling techniques in terms of apical or coronal leakage. CONCLUSIONS: Thermomechanically condensed alpha-phase gutta-percha used in conjunction with a single gutta-percha cone had poorer radiographic quality than laterally condensed gutta-percha.  相似文献

This study compared halothane and chloroform used with hand or ultrasonic instrumentation to remove gutta-percha and sealer from root canals. Apically extruded debris, residual debris, time for filling removal, and amount of solvent used were determined. The differences in extruded apical debris and radiographically visible residual debris were not significant (p greater than 0.05). Ultrasonic instrumentation required significantly less time to remove the root canal filling than did hand instrumentation (p = 0.02). The only significant difference in the amount of solvent used occurred when the ultrasonic-chloroform group was compared with the hand instrumentation-chloroform group (p = 0.05). Halothane was found to be an acceptable alternative to chloroform for removing gutta-percha and sealer from the obturated root canal.  相似文献

Comparison of the obturation of lateral canals by six techniques.   总被引:11,自引:0,他引:11  
The following obturation techniques were compared on their ability to obturate lateral canals in vitro, lateral condensation (LC), continuous wave of condensation (CW), warm vertical condensation (WV), carrier-based thermoplasticized gutta-percha (CB), warm lateral condensation (WL), and vertically condensed high-temperature gutta-percha (HT). A root canal system with lateral canals in the coronal, middle, and apical thirds was prepared in resin blocks. Each block was obturated using each technique (n = 30, 15 each, with and without sealer). The length of gutta-percha and sealer in each of the lateral canals was measured with a measuring microscope and statistically compared. All techniques obturated all three levels of lateral canals with sealer. WV, CB, and CW were able to fill the lateral canals with gutta-percha significantly better when root canal sealer was used. WV, CB, CW, and HT filled the coronal and middle lateral canals significantly better with gutta-percha than LC or WL condensation. CB and CW filled the apical lateral canal significantly better with gutta-percha than HT, WV, WL, or LC.  相似文献

AIM: The aim of this study was to compare in vitro root fracture resistance following root canal filling with either Ketac-Endo or Roth's root canal sealer. METHODOLOGY: The roots of 40 teeth with single canals were instrumented and placed randomly into four groups of 10 teeth. The prepared canals were obturated with lateral compaction using gutta-percha and Roth's 801 sealer, lateral compaction and Ketac-Endo sealer or with a single cone and Ketac-Endo sealer; the fourth group acted as unfilled controls. The roots were stored for 2 weeks in 100% humidity prior to being mounted in acrylic resin blocks. A steel tipped rod attached to an Instron testing machine was positioned against the canal opening and a slowly increasing force was applied until root fracture occurred. The results were subjected to statistical analysis using Kruskal-Wallis one-way ANOVA. RESULTS: There were no significant differences between the groups in terms of force required to vertically fracture the roots. CONCLUSIONS: Under the conditions of this study, the use of Ketac-Endo sealer in conjunction with lateral compaction or single-cone obturation techniques does not increase the fracture resistance of root-filled teeth.  相似文献

One hundred freshly extracted human mature, single-rooted teeth were divided into four identical groups of 25 teeth on the basis of root canal shape. The root canals of two groups were prepared in such a way as to produce a relatively parallel canal shape with little or no flare towards the coronal orifice. The root canals of the other two groups were prepared in such a way as to produce a canal shape that was deliberately more flared to ensure that they were wider at the orifice than at the endpoint of the preparation. All root canals were flushed with 17% EDTA solution and 2.5% NaOCl to remove the dentinal smear layer. The canals of one flared and one parallel group were obturated using cold lateral condensation and the canals of the other two groups were obturated using the Trifecta system. The sealability of each technique was assessed by a dye penetration method. The radiographic quality of obturation was determined for each canal using a 4-point scale. Canals filled with thermoplasticised gutta-percha had significantly less apical dye penetration than those obturated by lateral condensation (P < 0.001). Lateral condensation achieved significantly better scores for radiographic quality than thermoplasticised gutta-percha from both the bucco-lingual (P < 0.001) and the mesio-distal views (P < 0.001). Root canals obturated using the Trifecta technique showed significantly more apical extrusion of sealer than those obturated by lateral condensation (P < 0.001). Under laboratory conditions the Trifecta technique had better sealability but poorer radiographic quality than lateral condensation.  相似文献

目的:评价根管显微镜联合超声技术在钙化根管治疗中的应用。方法:选取临床上常规方法根管预备失败的钙化根管患牙42颗,钙化根管91个,在根管显微镜下,采用超声根管锉疏通钙化根管上段,再用小号手用K锉(08#和10#)和15%EDTA疏通根管中下段,机用镍钛ProTaper完成根管预备,热牙胶完成根管充填,观察并评价疗效。结果:75个钙化根管疏通成功并完成根管充填,根管疏通率82.41%,治疗过程中有1例根管侧壁穿孔,2例出现根尖偏移,其余病例无根折、无根管台阶形成,无器械折断。结论:对于根管中上段钙化的根管,根管显微镜可提供良好的光源和清晰的术野,超声器械可有效地去除钙化组织,但对于根尖1/3的弯曲钙化根管则容易造成根管侧壁穿孔或台阶形成。  相似文献

AIM: The purpose of this study was to compare the efficacy in vitro of gutta-percha removal from obturated root canals using ProFiles. METHODOLOGY: Forty-eight human root canals with curvatures ranging between 25 and 45 degrees were instrumented by a standardized method to an apical ISO size 30 and 0.04 taper. They were obturated with vertically condensed gutta-percha. Retreatment was performed with the following techniques: K-Flexofiles with chloroform; Hedstrom files with chloroform; ProFiles 0.04 taper with chloroform; ProFiles 0.04 taper alone. The time for each method was measured. A microfocal macroradiographic technique was used to evaluate the amount of debris remaining within the root canals after the retreatment procedure. Roots were divided into apical, middle and coronal parts and scored on a scale of 0 (no debris) to 3 (> 50% of walls covered with debris) by trained observers on two separate occasions. RESULTS: The scores for debris remaining within root canals for K-Flexofiles with chloroform and ProFiles with chloroform were the lowest and not significantly different at all three levels of the roots examined (P > 0.05), and Hedstrom files with chloroform and ProFiles with chloroform were not significantly different in the apical part. In general, coronal parts were cleaner than apical parts. The difference in scores at the three levels between ProFiles with chloroform and ProFiles alone were each significant (P < 0.01). Instrumentation using ProFiles with chloroform (mean 6.42 min) was significantly faster than using hand files (mean 11.67 min) (P < 0.01). CONCLUSION: The results indicated that ProFiles or hand files with chloroform produced similarly clean canals, but that ProFiles were faster.  相似文献

AIM: To compare the area occupied by gutta-percha, sealer, or void in standardized .06 tapered prepared simulated curved canals and in mesio-buccal canals of extracted maxillary first molars filled with a single .06 gutta-percha point and sealer or lateral condensation of multiple .02 gutta-percha points and sealer. METHODOLOGY: Simulated canals in resin blocks with either a 30 degrees curve and radius of 10.5 mm (n = 20) or a 58 degrees curve and 4.7 mm radius (n = 20) and curved mesio-buccal canals of extracted maxillary first molars (n = 20) were prepared using .06 ProFiles in a variable tip crown-down sequence to an apical size 35 at 0.5 mm from the canal terminus or apical foramen. Ten 30 degrees and 58 degrees curved resin canals and 10 canals in the extracted teeth group were obturated with .02 taper gutta-percha cones and AH 26 sealer using lateral condensation. The time required to obturate was recorded. The remaining canals were obturated with a single .06 taper gutta-percha cone and AH 26 sealer. Excess gutta-percha was removed from the specimens using heat and the warm mass vertically condensed. Horizontal sections were cut at 0.5, 1.5, 2.5, 4.5, 7.5 and 11.5 mm from the canal terminus or apical foramen. Colour photographs were taken using an Olympus 35 mm camera attached to a stereomicroscope set at x40 magnification, and then digitized using a flatbed scanner. The cross-sectional area of the canal contents was analysed using Adobe PhotoShop. The percentage of gutta-percha, sealer or voids to the total root canal area were derived and data analysed using unpaired Student's t-test and the Mann-Whitney U-test. RESULTS: In the 30 degrees curved canals the levels had between 94 and 100% of the area filled with gutta-percha with no significant difference (P > 0.05) between the lateral condensation and single cone techniques. In the 58 degrees curved canals the levels had 92-99% of the area filled with gutta-percha, with the single cone technique having significantly (P < 0.05) more gutta-percha fill at the 2.5 mm level only. In the mesio-buccal canals of the teeth the levels had between 72 and 96% of the area filled with gutta-percha with no significant difference (P > 0.05) between the lateral condensation and single cone technique. The time for obturation was significantly (P < 0.05) greater for lateral condensation compared with the single cone technique in all groups. CONCLUSIONS: The .06 taper single cone technique was comparable with lateral condensation in the amount of gutta-percha occupying a prepared .06 tapered canal. The .06 single cone technique was faster than lateral condensation.  相似文献

The canals of one flared group and one parallel group were obturated using cold lateral condensation, and the canals of the other two groups were obturated using low-temperature thermoplasticized gutta-percha. The sealability of each technique was assessed by a dye penetration method. The radiographic quality of obturation was determined for each canal using a 4-point scale. Canals filled with thermoplasticized gutta-percha had significantly less apical dye penetration than those obturated by lateral condensation (p < 0.001). Lateral condensation achieved significantly better scores for radiographic quality than thermoplasticized gutta-percha from both the buccolingual (p < 0.005) and mesiodistal views (p < 0.001). Low-temperature thermoplasticized gutta-percha root fillings were associated with significantly more apical extrusion of sealer (p < 0.001) and gutta-percha (p < 0.005). Under laboratory conditions the low-temperature thermoplasticized gutta-percha had better sealability but poorer radiographic quality than lateral condensation.  相似文献

This study evaluated the sectional gutta-percha obturation technique advocated by Lightspeed Technology, Inc. Single relatively straight-canaled teeth were prepared with rotary instruments and divided into three groups. Groups A and B served as controls and were obturated using laterally condensed gutta-percha and either Roth's 801 or Ketac-Endo sealer. Group C was obturated using the Lightspeed technique that included placement of a 5-mm apical section of gutta-percha, followed by backfill with Ketac-Endo sealer and a single gutta-percha cone. All teeth were suspended in India ink for 14 days then cleared. Four additional teeth, which were obturated as in group C then sectioned, revealed a tightly adapted apical section of gutta-percha with a very thin layer of sealer. However, voids were noted in the middle and coronal areas. There was no significant difference in apical microleakage among the three groups. The sectional method was significantly faster than lateral condensation and seems to offer promise as an effective obturation method.  相似文献

The aim of this in vitro study was to determine the effect of the smear layer on apical and coronal leakage in root canals obturated with AH26 or RoekoSeal sealers. A total of 160 maxillary anterior teeth extracted for periodontal reasons were used. The root canals were prepared to a size #7 with ProFile.06 taper Series 29 rotary instruments in a crown-down manner. Eight groups were created by all possible combinations of three factors: smear layer (present/absent), leakage assessment (apical/coronal), and sealer used (AH26/RoekoSeal). All teeth were obturated using lateral condensation technique of gutta-percha. A fluid filtration method was used to test apical or coronal leakage. According to the results of this study, the smear (+) groups displayed higher apical and coronal leakage than those smear (-) groups for both root canal sealers (p < 0.05). There was no statistically significant difference in either apical or coronal leakage between RoekoSeal and AH26, regardless of the presence or absence of the smear layer (p > 0.05). However, apical leakage was significantly higher than coronal leakage for both root canal sealers used in this study (p < 0.05). Under the conditions of this study, the results indicate that removal of the smear layer has a positive effect in reducing apical and coronal leakage for both AH26 and RoekoSeal root canal sealers.  相似文献

Sealer is required during root canal obturation, although no studies have shown which method of sealer placement provides the best coverage. This study evaluated and compared four methods of sealer placement: file, lentulo spiral, ultrasonic files, and master gutta-percha cone. Forty mandibular incisors were prepared by a step-back technique. AH26 (0.04 ml) was applied to the placement device, the sealer was placed in the canal, and the canal was obturated with lateral condensation of gutta-percha. The specimens were cleared and evaluated for the presence of the sealer in the apical, middle, and coronal thirds of the canal. The results showed no statistically significant differences among the four groups. The greatest variation in sealer coverage was found in the apical level. Sealer coverage in the coronal and middle thirds was nearly identical regardless of sealer placement technique.  相似文献

The purpose of this study was to evaluate in vitro the apical seal of root canals prepared with a new rotary system, Anatomic Endodontic Technology (AET) and filled with a methacrylate based endodontic sealer and a single gutta-percha cone and a lateral condensation technique, using the methacrylate based endodontic sealer/filler or Grossman's cement. The root canals of 45 freshly extracted human maxillary anterior teeth were prepared and then randomly assigned to three groups of 15 teeth each. After cleaning and shaping the teeth were obturated as follows: in group 1 the canals were filled with a methacrylate based sealer and a single gutta-percha cone; in group 2 the canals were filled using a lateral condensation technique with gutta-percha and the methacrylate based sealer; and in group 3, the canals were filled by means of lateral condensation of gutta-percha and Grossman's cement (control group). The specimens were stored in 100% relative humidity at 37 degrees C for 72 h, after which the coronal portion and the root surface of each tooth was covered with three layers of nail varnish and a final layer of sticky wax. After immersion in 2% methylene blue dye for 7 days, the specimens were imbedded in clear orthodontic resin and sectioned. Dye penetration was evaluated by an independent investigator using a stereo microscope. The results demonstrated that more pronounced leakage occurred in root canals obturated with the lateral condensation technique and Grossman's cement (p < 0.05). The least amount of dye leakage was observed for group 1 and 2 in which the methacrylate based sealer was used, either with a single gutta-percha cone or with lateral condensation of gutta-percha.  相似文献

The efficacy of sealing root canal systems with a dentine bonding agent and resin cement was compared with that of a glass ionomer sealer. Scanning electron microscopic examination of the bonding agent-dentine interface was also performed. The root canals of 50 single rooted teeth with mature apices were prepared chemo-mechanically and the smear layer removed with 17% REDTA and 5.25% NaOCl. Specimens were divided into two groups of 24 teeth each. One group had the dentine conditioned with a 10:3 citric acid-ferric chloride solution and obturated with the dentine bonding agent and resin, radiopaque C & B Metabond. The other group was obturated with the glass ionomer sealer, Ketac-Endo and a single cone of gutta-percha. After immersion in Indian ink for 90 h, the teeth were cleared and the quality of the apical and coronal seal was assessed using India ink dye penetration. There was a significantly better seal in both the apical and coronal directions when using the dentine bonding agent and resin obturation material. Scanning electron microscopic examination of the demineralized dentine and the C & B Metabond interface revealed the presence of the characteristic hybrid layer along with microtags of resin penetrating deep into the dentine tubules.  相似文献

The change in original canal size and location after canal preparation and after reinstrumentation using a step-back technique was studied in small and large canals with less than 20-degree curvature. Teeth were mounted in jigs and then sectioned. The jig permitted the sectioned teeth to be repositioned exactly. Slides were made of the canal in the coronal, middle, and apical at the following intervals: before instrumentation, after instrumentation, and after reinstrumentation. The increase in canal area and the direction of movement was measured. The data were analyzed by ANOVA and Duncan's multiple range test. The results showed: (a) All canal areas increased after retreatment. The retreated apical canal increased in area significantly more (25.4%) than middle (4.0%) or coronal (3.0%) regardless of whether the canal was large or small. (b) The retreated canals tended to move in the same direction they moved after first instrumentation.  相似文献

IntroductionThe aim of this study was to compare the quality of root fillings completed by a modified single-cone (MSC) technique with 3 different sealers after minimal instrumentation and multisonic cleaning of root canals of maxillary first molars.MethodsRoot canals of 18 maxillary first molars were instrumented to size 15/.04 taper using rotary files. Sodium hypochlorite 5.25% was used during instrumentation; the final cleaning was performed by the GentleWave System (Sonendo Inc, Laguna Hills, CA). The specimens were allocated into 3 groups and root filled by the MSC technique using a size fitted gutta-percha master cone and GuttaFlow Bioseal (Coltene Whaledent GmBH + Co KG, Langenau, Switzerland), GuttaFlow 2 (Coltene Whaledent GmBH + Co KG), and MTA Fillapex (Angelus Industria de Produtos Odontológicos S/A, Londrina, PR, Brazil) sealers. Micro–computed tomographic scans were obtained before and after instrumentation, post-GentleWave, and after obturation. Reconstructed images were analyzed for the volumetric percentage of filling materials. Mesiobuccal roots of the selected teeth were sectioned at 0.5-mm increments starting at the apex of the root. The cross sections were further examined using a light microscope.ResultsThe 3 groups had 90%–99% of the canal space filled with the root filling material. The mean volume of the filling material was higher in the GuttaFlow Bioseal and GuttaFlow 2 groups than in the MTA Fillapex group (P < .05). There was no significant difference among the apical, middle, and coronal thirds. The cross-sectional images showed no obvious gaps or voids in the GuttaFlow groups. After instrumentation, 49 of the 189 canal thirds (25.9%) had hard tissue debris in the root canal system. After GentleWave cleaning, only 4 of 63 canals (6.3%) and 4 of the 189 canal thirds (2.1%) still had debris.ConclusionsThe MSC method with GuttaFlow 2 and GuttaFlow Bioseal sealers after multisonic cleaning of minimally instrumented molar canals resulted in high-quality root fillings. Multisonic cleaning of minimally instrumented molars seems to be effective in debris removal.  相似文献

目的:评价手术显微镜联合超声技术在塑化根管再治疗中的应用。方法:选取曾进行过塑化治疗的患牙63颗,在手术显微镜下,采用超声根管锉疏通塑化根管的上段,小号手用K锉配合EDTA溶液逐步疏通根管下段,机动镍钛器械Hero642完成根管预备,侧向加压充填法充填根管,观察并评价疗效。结果:54颗患牙的根管疏通成功,并完成根管治疗,根管再通成功率为85.7%。治疗过程中无牙根折裂、台阶形成、根管壁侧穿或器械折断发生。结论:手术显微镜和超声器械去除根管内塑化物快速高效,可作为临床上处理塑化再治疗患牙的有效方法,但不推荐将超声器械用于根管的下段或弯曲部位。  相似文献

Abstract This study assessed in vitro apical leakage in root canals filled with gutta-percha and Ketac-Endo using different dentin conditioning and filling techniques. The root canals of 63 single-rooted teeth were instrumented and randomly divided into the following four groups: group A – canals were irrigated with 25% tannic: acid and filled with Ketac-Endo and a standardized gutta-percha cone; group B – canals were irrigated with 1% NaOCl and filled as in group A; group C – canals were irrigated with 25% tannic acid and filled with Ketac-Endo and a chloroform-softened gutta-percha cone adapted to the canal walls; group D – canals were irrigated with 1 % NaOCl and filled with Grossman's sealer and laterally condensed gutta-percha. After storage for 72 hours at 37°C and 100% humidity, the gutta-percha was removed to a level 4 mm short of the working length, using a Peeso reamer. Teeth were immersed in India ink for 48 hours and cleared, and the coronal extent of dye penetration examined under a binocular dissecting microscope at × 15 magnification. The mean value of leakage for group A was 0.86 mm, for group B 2.23 mm, for group G 2.34 mm, and for group D 0.55 mm. Tukey's multiple comparison lest disclosed significant differences (P<0.05) between group A and groups B and C, and between group D and groups B and C. Differences between groups A and D, as well as between groups B and C, were not significant.  相似文献

Bacterial coronal leakage after obturation with three root canal sealers   总被引:9,自引:0,他引:9  
The purpose of this study was to compare the bacterial leakage of root canals obturated with three root canal sealers, using Endodontalis faecalis as a microbial tracer to determine the length of time for bacteria to penetrate through the obturated root canal to the root apex. Seventy-five, single-rooted teeth with straight root canals had the crown cut off at the cementoenamel junction. Root canals were instrumented by a step-back technique. The prepared teeth were randomly divided into 3 groups of 19 teeth each and another 2 groups as positive and negative controls (9 teeth each). The experimental groups were dependent on the sealer used: AH-Plus, Apexit, and Ketac-Endo. The root canals were obturated using a lateral condensation technique. After 24 h the teeth were attached to microcentrifuge tubes with 2 mm of the root apex submerged in Brain Heart Infusion broth in glass test tubes. The coronal portions of the root canal filling materials were placed in contact with E. faecalis. The teeth were observed for bacterial leakage daily for 30 and 60 days. With the chi2 test for comparing pairs of groups at the 0.05 level (p < 0.05), there was no statistical difference between Ketac-Endo and AH-Plus (p > 0.06), but Apexit had significantly higher leakage (p < 0.05) at 30 days. After 60 days there was no statistical difference between Ketac-Endo and Apexit (p > 0.05), but Apexit leaked more than AH-Plus. The conclusion drawn from this experiment was that epoxy resin root canal sealer was found to be more adaptable to the root canal wall and filling material than a calcium hydroxide sealer when bacterial coronal leakage was studied.  相似文献
