枢椎后路侧块螺钉与椎弓根螺钉固定强度的生物力学评价   总被引:7,自引:2,他引:5  
目的评价单皮质和双皮质枢椎侧块螺钉与枢椎椎弓根螺钉的固定强度,为临床选择枢椎后路螺钉的固定方式提供生物力学依据。方法利用12具新鲜尸体枢椎标本,进行单皮质和双皮质的椎弓根螺钉或侧块螺钉固定,测试比较其螺钉拔出强度。结果双皮质枢椎椎弓根螺钉的拔出力量最大,为(1 726.5±433.3)N;单皮质枢椎椎弓根螺钉〔(1 279.9±432.0)N〕与双皮质枢椎侧块螺钉〔(1 054.8±411.3)N〕之间差异无统计学意义;单皮质枢椎侧块最小为(689.4±128.0)N。结论枢椎后路螺钉固定宜首选椎弓根螺钉,侧块螺钉可作为补充固定技术,且以双皮质骨固定为宜。  相似文献

目的探讨采用微创经皮经伤椎短节段椎弓根螺钉固定治疗单节段胸腰段骨折的效果及优势。方法选择2010年1月~2013年8月收治的86例单节段胸腰段椎体骨折经伤椎短节段椎弓根螺钉固定手术患者进行回顾性分析,其中经皮置钉44例[A组,男性26例,女性18例;年龄19~50岁,平均(45.3±3.5)岁],开放置钉42例[B组,男性25例,女性17例;年龄18~53岁,平均(44.6±2.9)岁]。对两组围手术期相关指标、影像学表现、视觉模拟评分(VAS)及Oswestry功能障碍指数(ODI)评分进行分析。结果A组术中X线曝光次数较B组多,但手术时间、术中出血量、术后引流量均少于B组,并能获得更高的术后3d VAS评分,差异均有统计学意义(P0.001),而两组之间术前、术后即刻、术后随访过程中测量伤椎高度变化及Cobb角恢复情况大致相当,无统计学差异(P0.05),但术后与同组术前比较明显改善,有统计学差异(P0.05),A组术后早期及随访过程中ODI评分优于B组同一时间节点,差异有统计学意义(P0.05)。结论经伤椎置钉短节段固定治疗胸腰段骨折固定可靠,伤椎高度恢复良好且维持稳定,而经皮置钉在不影响疗效同时减少手术创伤,加快术后恢复,值得应用推广。  相似文献

目的探讨O-arm导航辅助下后路椎弓根螺钉复位内固定治疗寰枢椎骨折的疗效。方法采用回顾性病例对照研究分析2016年1月至2018年6月河北医科大学第三医院收治的37例寰枢椎骨折患者临床资料,其中男22例,女15例;年龄29~68岁[(50.9±9.8)岁]。24例行O-arm导航辅助下后路椎弓根螺钉复位内固定术(导航组),13例行徒手后路椎弓根螺钉复位内固定术(徒手组)。比较两组手术时间和术中出血量。术前、术后7 d及末次随访采用日本骨科学会(JOA)评分和颈椎功能障碍指数(NDI)评分对患者临床疗效进行评估。观察患者并发症情况。采用Neo分级评价螺钉位置分级及皮质穿透率。结果者均获随访24~38个月[(27.7±4.0)个月]。导航组手术时间[(189.8±35.4)min]显著短于徒手组[(221.5±48.6)min](P<0.05),术中出血量[300.0(250.0,537.5)ml]〖显著少于徒手组[500.0(425.0,625.0)ml](P<0.05)。两组患者术后7 d及末次随访JOA评分较术前显著升高(P<0.05),NDI评分较术前显著降低(P<0.05)。而组间差异无统计学意义(P>0.05)。术中未见神经、血管损伤等严重相关并发症。导航组共置入椎弓根螺钉86枚,包括Neo分级0级83枚,1级2枚,2级1枚;徒手组共置入椎弓根螺钉44枚,包括Neo分级0级36枚,1级5枚,2级2枚,3级1枚(P<0.05)。导航组螺钉皮质穿透率为3%(3/86),徒手组为18%(8/44)(P<0.05)。结论对于寰枢椎骨折,在行后路椎弓根螺钉复位内固定术时,应用O-arm导航辅助可以显著缩短或减少手术时间和术中出血量,提高椎弓根螺钉置入的精确率。  相似文献

目的探讨重度不稳定型胸腰椎骨折采取后路短节段经伤椎椎弓根螺钉固定治疗的疗效。方法 2016年3月—2017年3月深圳市宝安区沙井人民医院脊柱外科收治的82例重度不稳定型胸腰椎骨折患者,根据治疗方式分为传统跨伤椎固定41例(跨伤椎组)与后路短节段经伤椎椎弓根螺钉固定41例(经伤椎组),比较两组患者手术时间、术中出血量、术后下地时间、伤椎相对高度、伤椎相邻椎Cobb角、VAS评分及ODI评分。结果手术时间及术中出血量,两组比较差异有统计学意义(P<0.05);两组术后下地时间比较差异无统计学意义(P>0.05);两组术后1年伤椎相对高度、伤椎相邻椎Cobb角较术前均显著改善,但经伤椎组伤椎复位率及Cobb角矫正度显著优于跨伤椎组(P<0.05);术后1年,两组患者VAS评分及ODI评分相比于术前均显著降低。而相比跨伤椎组,经伤椎组患者上述评分降低幅度更显著(P<0.05)。结论重度不稳定型胸腰椎骨折采取后路短节段经伤椎椎弓根螺钉固定治疗,治疗效果确切,具有临床推广价值。  相似文献

目的:探讨经伤椎椎弓根置钉治疗胸腰椎骨折的临床效果.方法:纳入2013年1月-2015年1月我科治疗的21例胸腰椎骨折患者,按照TLICS评分均大于4分,其中按AO分型,A型6例,B型15例,所有患者只累及一个椎体,均给予后路伤椎置钉内固定治疗.结果:本组研究所有患者均顺利完成手术,随访资料完整.术前Cobb角为(21.34±4.55)°,伤椎高度为(53.56±10.83)%,椎管面积为(45.35±16.24)%;末次随访Cobb角为(10.42±6.41)°,伤椎高度为(82.68±8.93)%,椎管面积为(90.33±10.56)%,术前及末次随访比较有统计学意义(P<0.05).结论:后路伤椎置钉治疗胸腰椎骨折临床效果满意.  相似文献

目的 探讨附加经伤椎椎弓根固定治疗胸腰椎骨折的临床疗效。方法 病例选自 2010 年 3 月-2013 年 3 月在我院骨科住院治疗的 55 例胸腰椎骨折患者,随机分为二组,对照组采用单纯行跨伤椎的 4 钉固定术(4 钉组),观察组采用附加经伤椎椎弓根螺钉内固定的 6 钉固定术(6 钉组)。比较两组患者的手术时间、术中出血量、术后住院时间及伤椎前缘高度、伤椎后缘高度、矢状位 Cobb 角等情况。结果 两组患者在手术时间、术中出血量、术后住院时间指标差异无统计学意义(P > 0. 05)。伤椎前、后缘高度、矢状位 Cobb 角较术前均有明显改善(P < 0.05 或 P < 0. 01),且 6 钉组的伤椎前缘、后缘术后高度及随访期间均优于 4 钉组(P < 0. 05)。结论 附加经伤椎椎弓根螺钉内固定是治疗胸腰椎骨折的可靠、有效方法,值得在临床上推广应用。  相似文献

目的 探讨寰枢椎椎弓根钉固定治疗伸直型齿状突骨折并寰枢关节半脱位临床疗效.方法 2002年12月-2006年12月,采用后路寰楸椎椎弓根钉内固定术治疗伸直型齿状突骨折并寰枢关节半脱位患者7例,其中男5例,女2例;年龄21~59岁,平均39.2岁.齿状突骨折均为Anderson Ⅱ型新鲜骨折,术前神经功能日本骨科协会(JOA)评分8.6~14.9分,平均10.7分.术前均行X线、CT等影像学检查及颅骨牵引术.均于全身麻醉下采用枢法模Vertex寰枢椎椎弓根钉系统,直视下行C1、C2复位固定术.结果 本组患者未发生椎动脉、脊髓及神经根损伤,寰枢椎骨折脱位均完全复位,术口均Ⅰ期愈合;术后3~6 d(平均4 d)颈托保护下离床活动.随访12~36个月(平均22个月),临床症状得到明显改善.术后6个月X线片示螺钉位置良好,无松动、断钉,所有骨折均呈骨性愈合.神经功能JOA评分13.5~16.9分,平均15.8分.结论 寰枢椎椎弓根钉内固定技术具有术中复位、固定可靠、骨愈合率高等特点,为齿状突骨折合并伸直型寰枢关节半脱位患者提供了一种较好的治疗方法.  相似文献

目的 探讨枢椎前路椎弓根螺钉通道的安全性和可行性,为经前路枢椎椎弓根螺钉固定提供理论依据.方法 对32具干燥枢椎标本行前路椎弓根螺钉置入,测量枢椎前路椎弓根螺钉的钉道长度.行加密薄层螺旋CT扫描和重建,然后在重建后的横断面和矢状面图像上运用软件测量枢椎前路椎弓根螺钉的外倾角度及尾倾角度.结果 枢椎椎弓根螺钉均位于椎弓根内.枢椎前路椎弓根螺钉的进钉点选择在枢椎椎体与上关节突之间凹陷的顶点,螺钉长度为(28.4±2.5)mm,外倾角度为(28.5±2.3)°,尾倾角度为(15.5±2.0)°.结论 枢椎前路椎弓根螺钉不仅具有可行性,而且可以避开横突孔、椎动脉沟、椎管等重要解剖结构,安全空间较大.
Abstract:
Objective To investigate feasibility and safety of anterior pedicle screw fixation tunnel in the axis so as to provide theoretic evidence for further clinical application.Methods Thirty-two dry axis specimens were used for anterior pedicle screw placement,and the length of pedicle crews tunnel was measured by a digital caliper.The extraversion angle and the downslope angle were determined by CT cross-sectional scanning and reconstruction.Results All the anterior pedicle screws were positioned well,with no perforation of the pedicle.The anchoring point was at the vertex between the vertebral body and the superior articular process,with axis pedicle length of(28.4 ± 2.5)mm,the extraversion angle of (28.5 ±2.3)°and the downslope angle of(15.5 ±2.0)°.Conclusions Anterior pedicle screw fixation tunnel is feasible in the axis and can keep away from some important anatomy structures including transverse foramen,vertebral artery groove and vertebral canal to provide large safety space.  相似文献

Methods From December 2002 to December 2006, seven patients with extension-type odontoid fracture combined with at-lantoaxial subluxation were reduced and fixed with atlantoaxial pedicle screw instrument of Vertex system under general anesthesia. There were five males and two females, at mean age of 39.2 years (range 21-59 years). All odontoid fractures were fresh type Aderson Ⅱ. JOA scores of spinal core function was 8.6-14.9 (average 10.7) preoperatively. The X-ray examination, CT scanning and skull traction were performed in all patients preoperatively. Results There found no severe complications such as injuries of vertebral artery, nerve root and spinal cord postoperatively. All patients obtained complete reduction and healing of the fracture and dislocation. The mean off-bed time was four days (3-6 days) after opera-tion. A follow-up for 12-36 months (average 22 months) in all patients showed that the clinical symptom was improved significantly six months postoperatively and that all screws were in proper position verified by X-ray and CT scanning. All patients obtained solid bony union on radiographs, with no loosing or breakage of instrument. The postoperative JOA scores was 13.5-16.9 (average 15.8). Conclusions Allantoaxial pedicle screw fixation has advantages of intraoperative reduction, reliable fixation and high fusion rate and can be used as an effective method for extension-type odontoid fracture combined with at-lantoaxial subluxation.  相似文献
