男性生殖器硬化性苔藓样病(MGLSc)是一种慢性复发性疾病,病程较长,逐渐加重,并有恶变倾向。该病既往被称为闭塞性干燥性阴茎头炎(BXO),主要发生于包皮、阴茎头,疾病进行性进展可累及尿道,形成瘢痕组织而引起尿道狭窄,出现尿线变细、排尿困难及痛性勃起,严重影响患者生活质量。该病的病因及发病机制目前尚不明确,普遍认为与自身免疫、遗传、感染、局部创伤及尿液慢性刺激有关。通过临床表现并结合病理检查该病不难确诊,重点在于早期诊断及治疗。该病很难被治愈,但是通过积极的治疗可以减轻症状、阻止疾病进展,有效阻断远期后遗症的发生。局部皮质类固醇类药物是MGLSc治疗的首选药物,对于药物保守治疗无效者和尿道狭窄引起的排尿梗阻及痛性勃起者,可采用合理的手术方式进行手术治疗,并重视长期随访。本文对MGLSc及其所致的尿道狭窄的诊治现状作一综述,以期对该病的诊治有所提高。  相似文献

目的:提高对阴茎头硬化性苔藓样变(LS)及由此引起的前尿道狭窄的认识,并探讨合理的手术治疗方法。方法:15例LS并发前尿道狭窄的患者,年龄27~75岁,尿道狭窄段长4~16cm。采用舌黏膜尿道成形11例、结肠黏膜尿道成形2例;尿道外口切开及前尿道劈开术各1例。所有患者手术同时行LS病变组织病理学检查。结果:术后随访6~12个月(平均10.07个月)。1例游离结肠黏膜尿道成形患者术后2月发生尿道外口狭窄。行尿道外口切开后排尿道通畅;余者术后排尿通畅,Qmax:17.2~32ml/s(平均18.70ml/s)。结论:采用游离黏膜尿道成形治疗LS性尿道狭窄可取得较好效果,但需密切随访病变迁延致尿道再狭窄。  相似文献

舌黏膜尿道成形治疗尿道狭窄的初步报告   总被引:3,自引:0,他引:3  
目的:探讨舌黏膜补片法尿道成形治疗尿道狭窄的疗效。方法:采用舌黏膜尿道成形治疗尿道狭窄14例。尿道狭窄段3.5~14cm,平均6.2cm;术前耻骨上膀胱造瘘8例,余6例排尿均较为困难,最大尿流率2.5~5.5ml/s,平均3.8ml/s。结果:术后随访2~8个月,1例因伤口感染致尿道皮肤瘘,余患者均排尿通畅,最大尿流率增至22~51ml/s,平均29.6ml/s。结论:舌黏膜具有取材方便、对患者创伤小、抗感染力强等特点,是一种较好的尿道替代物,尤其适合尿道狭窄段<6cm的患者。  相似文献

目的 观察结肠黏膜尿道成形术治疗复杂性超长段尿道狭窄的长期效果和影响因素. 方法 2000年10月至2009年9月采用结肠黏膜尿道成形治疗复杂性超长段尿道狭窄46例.年龄17 ~70岁,平均39岁.尿道狭窄段长10.0~20.0 cm,平均15.2 cm.术前有平均2.7次不成功的尿道修复史.通过定期门诊或电话随访进行术后疗效评估,包括排尿情况和尿流率检查,部分患者行尿道造影和尿道镜检查等.以不需要任何处理包括尿道扩张,能正常排尿,尿流率在正常范围内者视为手术成功. 结果 结肠黏膜重建尿道的长度为11.0~21.0 cm,平均15.4 cm.1例失访,余45例随访20~120个月,平均62个月.发生与手术相关的并发症4例(8.9%),其中3例于术后3、8和24个月发生尿道外口狭窄,1例术后29个月发生结肠黏膜新尿道与尿道近端吻合口狭窄.另2例发生与结肠黏膜尿道成形术无关的尿道狭窄. 结论 结肠黏膜尿道成形术治疗复杂性超长段尿道狭窄术后长期效果理想;影响术后效果的因素是尿道口狭窄和吻合口狭窄.  相似文献

舌黏膜尿道成形治疗前尿道狭窄(附80例报告)   总被引:1,自引:0,他引:1  
目的:研究舌黏膜尿道成形治疗尿道狭窄的有效性和安全性。方法:2006年8月~2008年12月采用舌黏膜尿道成形治疗80例前尿道狭窄,尿道狭窄段2.5~18cm,其中30例为长段尿道狭窄(9~18cm)采用双侧舌黏膜或舌黏膜与颊黏膜或与带蒂包皮拼接尿道成形治疗。尿道成形采用两种术式:保留原尿道板的扩大尿道腔37例;埋藏黏膜条43例。结果:术后随访4~30(平均16.8)个月,7例发生并发症,其中尿瘘4例,再次发生尿道狭窄3例,其余患者排尿通畅,最大尿流率从15.2~54.6(平均28.7)ml/s。结论:舌黏膜是一种修复前尿道狭窄较好的尿道替代物,双侧舌黏膜尿道成形能成功治疗长段、复杂性尿道狭窄。  相似文献

目的:探讨口腔黏膜尿道成形治疗复杂性尿道狭窄影响疗效的相关因素。方法:对采用口腔黏膜替代尿道成形术76例患者的尿道狭窄位置与长度、口腔黏膜宽度、术前手术次数、支架管留置时间等与狭窄复发率进行单因素分析。结果:76例随访3~60个月,平均24.1个月,术后初期排尿通畅61例(80.3%);再次狭窄15例,其中3例同时伴尿道皮肤瘘,4例伴尿道假性憩室。2例经数次尿道扩张、8例再次手术后排尿通畅,总成功率为93.4%。结论:尿道狭窄复发率与黏膜宽度明显相关(P〈0.05)。在0.8~2.0cm范围内口腔黏膜条越宽,尿道狭窄发生率越低;狭窄长度与术前手术次数对短期狭窄复发率无明显影响。  相似文献

目的 探讨口腔内黏膜尿道成形治疗尿道狭窄的长期效果. 方法 2001年1月至2010年12月,应用口腔内黏膜(颊黏膜和舌黏膜)尿道成形治疗前尿道狭窄255例.尿道狭窄段长度3 ~18 cm,平均6 cm.尿道成形采用保留原尿道板的扩大尿道成形术或埋藏黏膜条背侧替代尿道成形术.对49例尿道狭窄段≥8 cm者采取双侧颊黏膜拼接、颊粘膜与舌黏膜拼接或双侧连续长条舌黏膜尿道成形. 结果 术后随访8 ~120个月,平均37个月.230例患者排尿通畅,尿线粗,最大尿流率为16~51 ml/s,平均26 ml/s.尿道造影显示重建段尿道管腔通畅.总成功率90.2%.25例患者于术后1年内发生并发症,其中尿道再次狭窄17例,尿道皮肤瘘8例.17例尿道再狭窄患者中15例再次行口腔内黏膜尿道成形,2例吻合口狭窄行尿道内切开,术后排尿通畅;8例尿道皮肤瘘均接受尿瘘修补术后治愈. 结论 口腔颊黏膜和舌黏膜均是良好的尿道替代物,舌黏膜取材较颊黏膜更为便利;口腔内多种黏膜的组合移植重建尿道是治疗长段前尿道狭窄( ≥8 cm)的有效方法.  相似文献

目的 探讨舌黏膜与颊黏膜或包皮拼接尿道成形治疗多节段或长段尿道狭窄的安全性和治疗效果.方法 尿道狭窄患者11例,年龄24~56岁,平均32岁.其中前尿道长段狭窄7例,狭窄长度10~15 cm,平均12 cm;尿道2~3处狭窄4例.病程6个月~8年.取舌黏膜条与颊黏膜条拼接尿道成形治疗7例,舌黏膜条与包皮瓣拼接尿道成形治疗4例.结果 11例手术顺利.1例舌黏膜与包皮拼接尿道成形患者术后3个月并发尿道外口狭窄,经手术矫正后排尿通畅,术后8个月最大尿流率27.5 ml/s.余10例排尿通畅、尿线粗,术后随访5~12个月,平均10个月,最大尿流率21~36 ml/s,平均26.8 ml/s.结论 舌黏膜与颊黏膜或包皮拼接尿道成形具有取材方便、患者创伤小的特占点,是治疗长段尿道律窄的较好术式.尤其适用于尿道多处狭窄者.  相似文献

游离黏膜组织重建尿道治疗复杂性尿道狭窄的临床研究   总被引:11,自引:0,他引:11  
目的 探讨利用游离黏膜一期尿道成形治疗复杂性尿道狭窄或闭锁的疗效。方法 2000年8月至2004年7月采用2种游离黏膜一期尿道成形术治疗73例复杂性尿道狭窄。术前42例已行耻骨上膀胱造瘘,余31例最大尿流率1.2~6.5ml/s。用游离结肠黏膜(n=22)重建尿道长10~18cm,平均13cm;用口腔黏膜(n=51)重建尿道长3~11cm,平均5cm。术后随访分别行逆行尿道造影及尿流率,部分患者行尿道镜检查。结果随访2~48个月,平均19个月。术后排尿通畅67例(91.8%)。发生再次狭窄4例,其中结肠黏膜重建者1例,口腔黏膜重建者3例;排尿欠畅2例,定期行尿道扩张;尿道皮肤瘘2例;结肠腹壁瘘1例。1例结肠黏膜重建尿道者术后47个月移植物活检示结肠黏膜的组织形态学基本无变化。结论口腔与结肠黏膜均可作为较理想的尿道替代物,口腔黏膜较适合狭窄段不长的尿道修复,结肠黏膜较适合复杂性超长段尿道狭窄或缺损的治疗。  相似文献

PURPOSE: We report our experience with buccal mucosa grafts for anterior urethral strictures. We compared outcomes in the pendulous and bulbar urethra as well as the impact of lichen sclerosus on success. MATERIALS AND METHODS: A total of 53 men underwent buccal mucosa graft urethroplasty from 1997 to 2004 for strictures of all etiologies, including lichen sclerosis in 13. Of the patients 46 underwent 1-stage repair and 7 with full-thickness circumferential disease underwent multistage repair. For 1-stage repair strictures were limited to the bulb in 33 cases and they involved the pendulous urethra in 13. A dorsal onlay was used in 24 cases and a ventral onlay was used in 22. For multistage urethroplasty 2 strictures were in the bulbar urethra and 5 were in the pendulous urethra. Success was defined as no postoperative procedures or complications. RESULTS: The success rate of all urethroplasties was 81% (43 of 53 cases) at a mean followup of 52 months. For bulbar vs pendulous urethroplasty the success rate was 86% (30 of 35 cases) vs 72% (13 of 18, p = 0.23). For 1-stage urethroplasty by graft location success was achieved in 20 of 24 cases (83%) for dorsal onlay vs 17 of 22 (77%) for ventral onlay (p = 0.61), in 18 of 21 (86%) for bulbar-dorsal onlay, in 10 of 12 (83%) for bulbar-ventral onlay, in 2 of 3 (66%) for pendulous-dorsal onlay and in 7 of 10 (70%) for pendulous-ventral onlay. For multistage urethroplasty success was achieved in 2 of 2 cases (100%) for bulbar repair vs 4 of 5 (80%) for pendulous repair. In the 13 patients with lichen sclerosus success was achieved in 4 of 8 (50%) with 1-stage repair vs 4 of 5 (80%) with multistage repair (p = 0.28). Complications developed in 10 of 53 cases (19%), including fistula in 1, urinary tract infection in 1 and stricture in 8 that required treatment, including dilation in 3, internal urethrotomy in 4 and perineal urethrostomy in 1. Five of these 8 recurrent strictures (63%) developed in patients with lichen sclerosus, including 4 in urethras in which 1-stage repair was done for lichen sclerosus. There were no donor site complications, postoperative erectile dysfunction or chordee. CONCLUSIONS: A buccal mucosa graft placed dorsally or ventrally remains an excellent graft material in the bulbar and pendulous urethra. When lichen sclerosus is present, careful consideration should be given to complete excision of the diseased urethra with multistage repair vs accepting a higher rate of stricture recurrence with 1-stage repair.  相似文献

目的 评价颊黏膜尿道背侧镶嵌技术尿道修复重建手术的有效性和安全性.方法 男性前尿道狭窄患者57例.平均年龄36(17~52)岁.尿道狭窄长度平均3.0(2.5~7.0)cm.颊黏膜取材长度平均4.0(3.5~8.0)cm,平均宽度2.3(I.8~2.5)am.57例均有经尿道内窥镜冷切电切手术史,其中1次手术史29例(51%),2次20例(35%),3次8例(14%),行耻骨上膀胱造瘘48例,9例采用尿道扩张维持.手术游离并旋转尿道,并于尿道背侧纵行切开,6-0可吸收线缝合颊黏膜与尿道切缘.保留尿管4周,拔管时行顺行尿道造影.患者排尿困难和尿线变细、尿道造影或尿道镜发现尿道管腔狭窄<16 F确定为尿道狭窄复发.结果 57例手术均成功,平均手术时间135(105150)min.平均随访11.2(1~23)个月.尿道通畅54例(95%),术后2~3个月复发3例(5%),狭窄部位均为远端的颊黏膜与尿道结合部,行尿道内窥镜冷刀切开,定期尿道扩张治疗.伤口感染3例,未发生尿瘘及尿道憩室.患者口腔疼痛持续2~5 d,平均2.3 d.术后无张VI困难、颊部麻木及腮腺导管梗阻等并发症发生,VI腔颊部均未见瘢痕形成.结论 VI腔颊黏膜尿道背侧镶嵌补片修复重建尿道,效果确切、并发症少、狭窄复发率低,是理想的长段前尿道狭窄的修复重建手段.  相似文献

Objective  To evaluate the efficacy and safety of using oral mucosal grafts for urethroplasty in the treatment of complex segmented urethral strictures. Methods  Between January 2002 and January 2008, 25 cases of long or multi-segmented urethral strictures (10–18 cm, mean 11.72) were treated using combined two oral mucosal graft urethroplasty. Of the 25 patients, combined double buccal mucosal graft (BMG) urethroplasty was performed in nine patients, double lingual mucosal graft (LMG) urethroplasty in seven patients and combined lingual and buccal mucosal graft urethroplasty in nine patients. Results  Follow-up was obtained for 6–72 months (mean 26.83) post-operatively. Urethrocutaneous fistulas developed in two patients. Urethral strictures developed in one patient undergoing BMG urethroplasty; the patient underwent five urethral dilations, after which he voided well with a urinary peak flow of 26.4 ml/s. Meatal stenosis developed in one patient undergoing LMG and a second operation was required, after which the patient voided well (urinary peak flow of 28.7 ml/s). The other patients voided well and urinary peak flow rates ranged from 16.8 to 49.2 ml/s (mean 28.65 ml/s). Conclusion  Combined two oral mucosal grafts substitution urethroplasty is an effective technique for the treatment of long, segmented urethral strictures.  相似文献

Study Type – Therapy (case series) Level of Evidence 4 What's known on the subject? and What does the study add? Male genital lichen sclerosis is a disease of unknown aetiology which is believed to be a risk factor for penile cancer. This study reports that the disease can develop in non‐genital skin grafts used for reconstruction following penile cancer surgery.

OBJECTIVE

? To describe the incidence of the development of male genital lichen sclerosus (LS) in non‐genital skin grafts used in penile reconstruction after cancer surgery.

PATIENTS AND METHODS

? Between 1997 and 2009, 177 patients received surgical treatment for penile cancer in the Urology Department at Sunderland Royal Hospital, the regional penile cancer centre for the north‐east of England. ? Patients who had organ‐sparing surgery and non‐genital penile graft reconstructions were identified. ? Histology reports for specimens obtained from those grafts were reviewed to identify the presence of male genital LS and the incidence of recurrence of squamous cell carcinoma (SCC).

RESULTS

? The mean (range) age of patients at diagnosis was 61.8 (32–89) years. Of the 177 patients, 139 had SCC, 32 had carcinoma in situ and six had verrucous carcinoma. ? In total, 56 penile reconstructive procedures were performed using split‐thickness skin grafts obtained from the inner thigh. ? From those grafts, 18 specimens were obtained later for cosmetic, diagnostic or curative purposes. ? Male genital LS was found in six of the 18 specimens, and one of them was associated with recurrent verrucous carcinoma.

CONCLUSIONS

? This is the first published series to describe the incidence of male genital LS in penile skin grafts taken from a remote site after penile cancer surgery. ? These results represent new information that might help explain the aetiology of male genital LS.  相似文献

BACKGROUND: Management of lichen sclerosus (LS) in boys is still controversial. Although in most cases only the prepuce is affected, meatal and urethral involvement may require major surgical reconstruction with substantial morbidity. OBJECTIVE: Because the frequency of such complicated courses is still unclear, an adjuvant postoperative treatment is highly desirable. Therefore, we addressed safety and tolerability of tacrolimus 0.1% ointment in the postoperative period. DESIGN, SETTING, AND PARTICIPANTS: Among 222 penile surgeries, in 25 cases LS was confirmed histologically and 20 of those patients participated in the adjuvant treatment study. Moreover, 18 patients of the same cohort showed a lichenoid inflammatory reaction pattern suggestive of early but not fully established LS. INTERVENTIONS AND MEASUREMENTS: Subsequent to the operation and after explicit information about off-label use, parents applied tacrolimus 0.1% ointment twice daily to the glans and the meatus for 3 wk in cases of proven LS. The 18 patients with possible early LS were followed up only without any treatment. Clinical follow-up was performed up to 13 mo (median). RESULTS AND LIMITATIONS: All 20 LS patients completed the topical treatment without any relevant side-effects. Two relapses occurred in the treatment group and were clinically cured with an additional 3-wk cycle of topical tacrolimus 0.1% ointment. None of the 18 early LS cases progressed to full-scale LS. CONCLUSIONS: This is the first study showing that tacrolimus 0.1% ointment applied immediately after surgery of fully established LS is a tolerable and most probably safe adjuvant novel treatment option. Because the therapy led to disease control in all treated individuals for >1 yr (median), this study establishes the groundwork for future trials with expanded treatment and follow-up periods to verify the true clinical benefit of tacrolimus in patients after LS surgery. Lichenoid tissue reactions suggestive of early LS seem to require no adjuvant treatment.  相似文献
