To assess the impact of scanning direction on the image quality of coronary artery bypass grafts (CABGs), native coronary arteries (NCAs) were examined by electrocardiographically (ECG) gated 16-row multidetector computed tomography (16-MDCT). Eighty-two patients with 209 grafts were studied by 16-MDCT. Forty-one patients with 111 grafts were scanned craniocaudally. Forty-one patients with 98 grafts were scanned caudocranially. CABG, native coronary arteries were examined in four (proximal, middle, distal, distal anastomoses), three (proximal, middle, distal) segments, respectively. Subjective image quality on a four-point scale was calculated for segments. Scores of groups were compared. Results Image quality scores of proximal, distal segments of the right coronary artery (RCA) were better in caudocranially scanned group (P<0.05). When we subgrouped patients according to initial heart rates (IHR) (group 1, <65 beats/min; group 2, ≥65 beats/min), there was no statistical significance between image quality scores of coronary arteries, CABG when IHR was <65 beats/min in groups regardless of scanning direction. Scores of anastomotic segment of CABG to RCA, middle segments of circumflex coronary artery, proximal and distal segments of RCA in caudocranially scanned group were better when the IHR is ≥65 beats/min compared with the craniocaudally scanned group. When the IHR of the patient is ≥65 beats/min, performing ECG-gated 16-MDCT angiography in the caudocranial direction provides better image quality for evaluation of coronary arteries and CABGs.  相似文献

OBJECTIVE: The objective of our study was to assess the feasibility of using 16-MDCT angiography for the preoperative assessment of the radial and ulnar arteries and the palmar arches in patients scheduled for radial artery harvesting for coronary artery bypass graft (CABG) surgery. CONCLUSION: Sixteen-MDCT angiography shows promise for the noninvasive preoperative assessment of the radial artery as a CABG donor site.  相似文献

OBJECTIVE: The objective of our study was to assess the effect of beta-blockers on image quality of ECG-gated 16-MDCT coronary angiography. MATERIALS AND METHODS: Coronary CT angiography was performed in two groups: group 1, 24 volunteers (mean age, 50 years; mean heart rate, 69 beats per minute [bpm]; range, 47-97 bpm); and group 2, 15 patients with current ischemic heart disease (mean age, 54 years; mean heart rate, 54 bpm; range, 48-69 bpm) who were premedicated with 20-40 mg of oral propranolol 1 hr before the study. CT scans were obtained on a 16-MDCT scanner with a 12 x 0.75 mm collimation and 420-msec rotation using nonionic contrast material (80 mL; injection rate, 4 ml/sec). Images were reconstructed at 30-80% of the cardiac cycle in increments of 5%. Image quality of the following eight coronary segments was assessed by two radiologists in consensus: left main coronary artery; proximal and middle segments of the left anterior descending (LAD) and left circumflex (LCX) coronary arteries; and the proximal, middle, and distal segments of the right coronary artery (RCA). Image quality was assessed, using a 5-point grading scale, as grades 1-5. Images assessed as grade 4 or 5 were considered to be of diagnostically acceptable quality. RESULTS: In group 1, grade 4 or 5 image quality was achieved for visualization of 92% of the left main coronary arteries; 96% of the proximal LAD coronary arteries; 88% of the middle LAD, proximal LCX, and middle LCX coronary arteries; 83% of the proximal RCAs; 58% of the middle RCAs; and 96% of the distal RCAs. In group 2, this level of image quality was achieved in 100% of the left main coronary arteries, proximal and middle LAD arteries, and proximal LCX arteries; 87% of the middle LCX arteries; and 93% of the proximal, middle, and distal RCAs. CONCLUSION: Reduction of heart rates with beta-blocker premedication improves the image quality of CT coronary angiography, especially in terms of the visualization of the right coronary artery.  相似文献

Early identification and evaluation of relatively frequent anomalous coronary anatomy is quite relevant because of the occurrence of sudden cardiac death or related symptoms of myocardial ischemia. Selective coronary angiography (CAG) is invasive, expensive and cannot always provide the required information adequately. Recently, non-invasive imaging techniques such as magnetic resonance imaging and multidetector-row computed tomography (MDCT) have been shown to provide a good anatomical view of the coronary artery tree. This study aims to demonstrate the value of 16-MDCT for evaluation of anomalous coronary anatomy. In 13 patients scanned using 16-MDCT, six different coronary anomalies were diagnosed [two absent left main, one single vessel left coronary artery (LCA), three LCA originating from the right (two with interarterial course), six right coronary artery originating from the left, one double left anterior descending (LAD)]. Mean diagnostic quality, recorded by two observers using a 5-point scale (1= non-diagnostic to 5= excellent diagnostic quality), resulted in a mean score of 3.73 (SD 1.19) without any non-diagnostic result. MDCT offers an accurate diagnostic modality to visualize the origin and course of anomalous coronary arteries by a three-dimensional display of anatomy. Shortcomings in CAG can be overcome by the use of contrast-enhanced MDCT.  相似文献

PURPOSE: To evaluate accuracy of velocity encoded cine MR imaging for determination of blood flow in coronary arteries and coronary bypass grafts. MATERIALS AND METHODS: 12 patients with 22 coronary bypass grafts underwent intraoperative flow quantification using the transit time ultrasound method. These values were compared to postoperative MR phase shift measurements. Flow measurements were performed preoperatively in 28 coronary arteries of 20 patients. For flow measurement, we used a velocity-encoded k-space segmented gradient echo sequence with a temporal resolution of 110 or 125 ms, respectively. 6-8 pase shift images could be acquired during one cardiac cycle. RESULTS: There was a significant correlation between intraoperative and flow measurements using velocity-encoded MR imaging (r = 0.74, p < 0.0001, t-test). Flow volumes determined by MR imaging were systematically larger than those determined by the transit time ultrasound method. Mean flow in coronary arteries was reduced in severely stenosed vessels (> 70%) compared to normal vessels. Intra- and interobserver variability were 10.5 and 15% (coronary bypass grafts), and 12.3 and 15.8% (coronary arteries), respectively. CONCLUSIONS: Velocity-encoded MR imaging enables determination of flow in coronary artery bypass grafts and coronary arteries. Future developments should aim at the improvement of spatial and temporal resolution of the method.  相似文献

PURPOSE: The aim of our study was to assess the influence of heart rate on the selection of the optimal reconstruction window with 40-slice multidetector-row computed tomography (40-MDCT) coronary angiography. MATERIALS AND METHODS: We studied 170 patients (114 men, age 60+/-11.3 years) with suspected or known coronary artery disease with 40-MDCT coronary angiography. Patients [mean heart rate (HR) 62.9+/-9.3 bpm, range 42-94 bpm] were clustered in two groups (group A: HR 65 bpm). Multiphase reconstruction data sets were obtained with a retrospective electrocardiogram (ECG)-gated 40-MDCT coronary angiography scan from 0% to 95% every 5% of the R-R interval. Two radiologists in consensus evaluated the best data sets for diagnostic purposes. RESULTS: In group A, the optimal reconstruction windows were at 70% (55/110, 71/110 and 69/110 for the right coronary artery, left anterior descending and the left circumflex, respectively) and 75% (26/110, 28/110 and 28/110, respectively) of the R-R interval. In group B, a wide range of reconstruction windows were employed, both in the end-systolic phase at 40% (32/60, 18/60 and 17/60, for the right coronary artery, left anterior descending and circumflex, respectively) and diastolic phases at 70% (12/60, 22/60 and 19/60, respectively). Six scans were excluded due to severe respiratory artefacts. CONCLUSIONS: Optimal position of the image reconstruction window relative to the cardiac cycle is significantly influenced by the heart rate during scanning. Diastolic reconstruction phases often allowed an optimal assessment in group A. Reconstruction phases from 30% to 45% are advisable for higher heart rates.  相似文献

OBJECTIVE: The purpose of our study was to prospectively evaluate the usefulness of CT coronary angiography versus invasive coronary angiography for the detection of clinically significant coronary artery disease in patients hospitalized for acute chest pain syndrome. SUBJECTS AND METHODS: Sixty-six consecutive patients (52 men and 14 women; average age, 57 +/- 11 [SD] years) who were hospitalized for acute chest pain syndrome underwent CT coronary angiography and invasive coronary angiography within an average time interval of 4 days. ECG-gated CT coronary angiography was performed with a 16-MDCT scanner (0.42-sec rotation time, 16 x 0.75 mm detector collimation). Beta-blockers were not administered routinely, and thus the average heart rate was 71 +/- 11 beats per minute. CT coronary angiographic images were evaluated concurrently by two radiologists, who were blinded to invasive coronary angiography results, for stenoses having a diameter of 50% or more, using a 15-segment classification, including all segments 2 mm or more in diameter. The consensus interpretation was compared with results of invasive coronary angiography. RESULTS: CT coronary angiography was technically successful in 59 patients (89%). After exclusion of 20 (3.1%) of 649 coronary segments, which were classified as nonevaluable by CT coronary angiography, the sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of CT coronary angiography for identifying significant coronary artery disease in the remaining 629 coronary segments were 80% (68/85), 89% (482/544), 52% (68/130), 97% (482/499), and 87% (550/629), respectively. The overall accuracy for the main vessels (left main, left anterior descending, left circumflex, and right coronary arteries) was 93%, 88%, 86%, and 86%, respectively. CONCLUSION: CT coronary angiography using a 16-MDCT scanner enables accurate noninvasive detection of significant coronary artery disease in patients hospitalized for acute chest pain syndrome. Furthermore, relative high sensitivity and specificity of CT coronary angiography can be achieved without pharmacologic manipulation of patient heart rates.  相似文献

目的探讨双源 CT 冠状动脉成像对冠状动脉搭桥术后桥血管通畅状况的诊断价值.资料与方法38例患者于冠状动脉搭桥术后行双源 CT 冠状动脉成像与冠状动脉造影检查,以冠状动脉造影作为“金标准”,对双源 CT 冠状动脉成像结果进行分析.结果38例患者共检出桥血管92支,其中内乳动脉桥31支,大隐静脉桥61支;31支内乳动脉桥中,26支(83.9%)桥血管通畅,5支(16.1%)管腔狭窄、闭塞;61支大隐静脉桥血管中35支(57.4%)桥血管通畅;26支(42.6%)管腔狭窄、闭塞.大隐静脉桥血管狭窄、闭塞的发生率明显高于内乳动脉桥血管(P<0.05).双源 CT 冠状动脉成像评价桥血管狭窄、闭塞的敏感性为93.9%,特异性为100.0%.结论双源 CT 冠状动脉成像对冠状动脉搭桥术后桥血管通畅状况的评价有较高的诊断价值,是一种快速、无创、准确、安全的检查方法.  相似文献

目的:回顾性研究2000-05-2001-07我院25例行冠脉旁路移植手术病人的治疗效果。结果:术后22例患者心绞痛症状完全消失,1例冠状动脉弥漫性病变的患者仅偶尔需服用小剂量硝酸甘油制剂,症状即可明显改善,1例术后3例死于严重的感染,1例术中死于严重的鱼精蛋白过敏。结论:药物及介入治疗无效的冠心病病人采用冠状动脉旁路移植术是有效的方法。  相似文献

Arterial coronary bypass grafts [internal mammary arteries and gastroepiploic artery (GEA)] are in widespread use for coronary surgery. Since selective catheterisation of the GEA graft to monitor patency, is often unsuccessful, a non-invasive protocol to visualise the GEA-graft from origin to anastomosis is presented using 16-slice multidetector computed tomography (MDCT). Twenty-six male patients (mean age 58.1±6.7 years) with GEA grafts were scanned according to a protocol of an ECG-synchronised cardiac scan followed by a thoracoabdominal scan. To terminate the scan at the correct anatomical level, the lowest level of the GEA was coded based on the lumbar vertebrae level. Scores ranging from one (excellent) to four (bad) were assigned to evaluate visualisation quality of the grafts. GEA grafts were assessable in 62% of the thoracoabdominal scans and 69% of the cardiac scans. On average, the lowest part of the GEA corresponded with a level between L1 and L2, in two cases in the upper part of L3. Mean visualisation score in the thoracoabdominal scans and cardiac scans was good (respectively 1.4±0.6 and 1.4±1.0). Sixteen-slice MDCT is a promising alternative for catheterisation in evaluating patency of GEA grafts, using the presented protocol with thoracoabdominal scan including L3 for complete coverage of the GEA graft.  相似文献

PURPOSE: To report our preliminary experience with multi-slice spiral CT coronary angiography of the coronary arteries. MATERIALS AND METHODS: 50 volunteers (mean age 61 years, range 45-72) with baseline heart rates below 70 bpm underwent multi-detector row CT coronary angiography (GE Light Speed Plus, 140 kVp, 270 mA, 1.25-mm collimation, 0.5-second rotation time, high quality pitch) with retrospective ECG gating after receiving 140-150 ml of iodinated contrast medium (Iopamiro 300 mg-dl, Bracco, Italy) at a flow rate of 4 ml/sec. Three of the 50 patients had previously undergone coronary procedures (1 anterior descending artery stent, 1 left circumflex artery stent and 1 anterior descending artery percutaneous angioplasty) and three were undergoing follow-up examinations after by-pass surgery. The remaining 44 patients were asymptomatic and had no history of coronary artery disease. All CT angiograms were back-reconstructed from 20 to 80% of the diastolic cycle with 10% increments to establish the phase with fewer "stair-step" motion artefacts for each artery. Patients with heart rates above 70 bpm were administered beta-blockers during the five days preceding the examination in doses appropriate for the patient's clinical characteristics. RESULTS: The CT room occupation time ranged from 25 to 35 minutes (mean time 27') and the post-processing time from 30 to 60 minutes (mean time 40'). The left anterior descending artery was best visualised in middle diastole (70% of cardiac cycle), the circumflex artery at 60% of the cardiac cycle, and the right coronary artery at 40%. Out of 132 arteries, 19 (14.4%) were considered non-assessable due to "stair-step" motion artefacts, whereas 2 (1.5%) were only partially visualised owing to the presence of extensive wall calcifications. Among the 113 assessable arteries, we observed: 72 normal coronary arteries without stenosis or wall calcifications (54.5%); 28 arteries with minimal wall irregularities and stenoses below 50% (21.2%); 7 stenoses >50% involving the right coronary artery (no. 2), the anterior descending arteries (no. 4) and the left circumflex artery (no. 1) (5.3%). In the remaining 6 patients, optimal visualisation of the stents and venous and arterial surgical by-passes was obtained. CONCLUSIONS: Although further larger-scale studies are required to compare MSCT coronary angiography with CT coronary angiography, the application of MSCT technology to the study of the coronary arteries is a promising technique with a good potential for use in routine clinical practice. In selected patients (with baseline heart rates <70 bpm, or after beta-blocker therapy) it is able to provide very interesting results and could be used as a method of choice for following patients after interventional procedures or as a mass-screening tool to select patients to be referred for coronary angiography.  相似文献

OBJECTIVE: Six patients who had undergone minimally invasive direct coronary artery bypass surgery were examined to evaluate an MR imaging protocol that provided information about cardiac function, bypass graft patency, and flow characteristics with a single examination. CONCLUSION: Preliminary results suggest that our imaging protocol allows accurate follow-up of patients after minimally invasive direct coronary artery bypass surgery. Bypass graft patency was correctly determined in all patients. In four patients, anastomoses were visualized by MR angiography, and flow measurements revealed a volume range of 28-84 ml/min (native and grafted internal mammary arteries) and a trend for the flow values of bypass grafts to be lower than those of native vessels. Interobserver reproducibility was good (r = .99; slope, .98).  相似文献

PURPOSE: The study was undertaken to evaluate the 3-year outcome of patients undergoing coronary artery bypass grafting (CABG) involving the use of the radial artery (RA) in comparison with the left internal mammary artery (LIMA) and saphenous vein (SV) grafts by using 16-slice multidetector computed tomography (MDCT). MATERIALS AND METHODS: Fifty-one patients underwent electrocardiogram (ECG)-gated 16-MDCT 32+/-4 months after surgery. A total of 50 LIMA grafts, 55 SV grafts and 51 RA grafts were studied. Approximately 68.6% or RAs were free, 21.5% sequential and 9.8% composite. Grade 0 was defined as complete patency, grade 1 as focal stenosis (>70%) and grade 2 as graft occlusion. The Fisher exact test was used to analyse variables (p<0.05 significant). Concordance between readers for the detection of patency was calculated by the kappa-value. RESULTS: LIMA had the best patency rate (94.0%), followed by SV (83.6%) and RA (74.5%). Regarding RA, the patency rate by territory was 79.4% in the left circumflex coronary artery (LCX), 72.7% in the left anterior descending (LAD) and 50% in the right coronary artery (RCA); the occlusion rate was 20.0% among free grafts, 18.2% among sequential grafts and 20.0% among composite grafts. The kappa-value was 0.86. CONCLUSIONS: Sixteen-slice MDCT scanners enable accurate analysis of CABG status and are a useful noninvasive diagnostic tool for midterm clinical follow-up of patients who have undergone CABG involving the use of RA.  相似文献

OBJECTIVE: The purpose of this study was to determine the accuracy of three-dimensional coronary angiography obtained with electron beam CT in the assessment of the patency of coronary artery bypass grafts. SUBJECTS AND METHODS: Twenty-five patients who had undergone coronary artery bypass graft surgery were included. All patients underwent electron beam CT and conventional coronary angiography for the evaluation of the status of their bypass grafts. Three-dimensional reconstructions of the heart and bypass grafts were compared with selective angiographic images of the bypass grafts. RESULTS: Fifty-seven saphenous vein grafts and 22 left internal mammary artery grafts were evaluated for occlusion or patency. Sensitivity and specificity of electron beam CT in revealing left internal mammary artery patency were 80% and 82.4%, respectively. Sensitivity and specificity of electron beam CT in revealing saphenous vein graft patency were 91.7% and 91.1%, respectively. Sensitivity and specificity of electron beam CT for evaluating saphenous vein grafts according to coronary area were as follows: saphenous vein grafts to left anterior descending artery, 100% and 100%, respectively; to diagonal branch, 100% and 100%; to left circumflex artery, 100% and 88.9%; and to right coronary artery, 75% and 85.7%. CONCLUSION: Three-dimensional coronary angiography obtained with electron beam CT is a promising, useful, and relatively accurate diagnostic imaging technique for the evaluation of graft patency in patients who have undergone coronary artery bypass graft surgery.  相似文献

To determine whether thallium-201 washout profile analysis can detect regional myocardial ischemia caused by coronary artery bypass graft occlusion or progression of disease in nonbypassed coronary arteries, 19 consecutive patients with chest pain following bypass grafting were evaluated with coronary arteriography and thallium-201 scintigraphy. Twenty of the 55 coronary artery regions were perfused by an occluded bypass graft or a significantly stenosed (greater than or equal to 70% diameter narrowing) nonbypassed coronary artery, while 35 coronary regions were perfused by patent bypass grafts or insignificantly diseased coronary arteries. The tomographic thallium-201 washout profile results correlated with the bypass graft and coronary arteriographic findings (k = 0.67, P less than 0.001). The sensitivity of tomographic thallium-201 washout profile abnormalities for arteriographic abnormalities was 75%, while the specificity was 86%. The authors conclude that tomographic thallium-201 washout profile analysis may be very useful in the evaluation of patients with chest pain following coronary artery bypass grafting by detecting regional myocardial ischemia caused by occlusion of specific bypass grafts or progression of disease in nonbypassed coronary arteries.  相似文献

目的评价电子束CT血管造影(EBA)及其三维血管成像技术对冠状动脉疾病的诊断与随访价值. 资料与方法 87例经EBA检查的患者,其中支架置入术后随访16例,搭桥术后随访9例.除9例搭桥患者外,其余78例均有冠状动脉造影(CAG)结果证实. 结果 EBA可评价的冠状动脉中,对左主干和前降支的诊断敏感性、特异性最高,尤其是近段,其次为右冠,对回旋支的诊断敏感性和特异性则较低;16例支架置入术后患者共放置支架47枚,EBA准确定位45枚(95.7%),对支架内开通诊断的符合率为74.5%;9例冠状动脉搭桥术后的患者,共搭建冠状动脉桥血管27支,EBA对桥血管开通与否诊断的符合率为81.5%. 结论 EBA对冠状动脉狭窄的诊断具有较高的术前筛选价值,对术后的随访也有较大的潜力,是一项很有优势的无创性冠心病检查方法.  相似文献

目的:评价心率对64层螺旋CT冠状动脉成像图像质量的影响.方法:采用GE Light speed 64层螺旋VCT,以心脏扫描模式对心脏动态体模进行扫描.心脏动态体模由3个部分组成:动力部分、解剖结构模拟部分和控制部分.心脏动态体模的心率设置为40、45、50、55、60、65、70、75、80、85、90、95、100、105、110和115次/min,心律齐.以球管转速0.35 s对不同心率下的心脏动态体模进行冠状动脉成像扫描.所有扫描数据在R-R间期90%时相分别进行单扇区和多扇区重建.重建数据传至工作站后处理成像.后处理方法采用VR、MPR重组模式.分别对重建图像进行评分.结果:①心率与图像质量呈负相关(P<0.01);随着心率的增加,图像质量评分呈下降趋势;②在同一条件下多扇区重建算法较单扇区重建算法提高了图像质量评分.结论:采用心脏动态体模评价心率对64层螺旋CT冠状动脉成像图像质量的影响,对临床研究和应用有着重要价值.  相似文献

In a preliminary evaluation of the use of dynamic computed tomography (CT) for the detection of patent aortocoronary bypass grafts, 30 patients were scanned either during the early postoperative peroid or after graft patency was determined by angiography. To visualize the proximal grafts, CT scans were taken through the aortic root following an intravenous bolus injection of contrast medium. Patent bypass grafts to the left anterior descending and right coronary arteries were demonstrated in 77.5%, while posterior grafts to the circumflex and obtuse marginal coronary arteries were detected in 40%. With its pontential for extracting dynamic events, CT scanning provides a new, noninvasive modality for the diagnosis of patent bypass grafts, which heretofore have only been visualized by selective angiography.  相似文献

PURPOSE: To investigate the ability of 16-slice multidetector-row computed tomography (MDCT) to visualize coronary artery bypass graft (CABG) patency and to detect bypass stenoses. MATERIALS AND METHODS: Thirty-two patients with 94 grafts (20 mammary artery grafts, 74 venous grafts) were investigated by 16-slice MDCT using a scan protocol with 12 x 0.75 mm slice collimation (pitch 0.3), 420 ms rotation time and simultaneous electrocardiogram (ECG)-registration. One hundred milliliters iodinated contrast agent were injected with a delay according to the individually determined contrast agent transit time. Patients with heart rates above 60 bpm received oral beta-blockade. Cross-sectional images with a slice width of 1.0 mm (0.5 mm increment) were reconstructed using an ECG-gated half-scan reconstruction or a multisegment reconstruction algorithm depending on the heart rate. Bypass grafts were evaluated concerning patency and presence of stenoses > or = 50% diameter reduction on cross-sectional images, multiplanar reformations and maximum intensity projections by two independent observers. Results were compared to coronary bypass angiography. RESULTS: Sixteen-slice MDCT results were compared to those of invasive coronary angiography concerning absence or presence of bypass graft occlusion or relevant stenosis > or = 50% lumen reduction. Coronary CT angiography (CTA) permitted detection of bypass occlusion with 100% sensitivity (28/28) and 98% specificity (64/65). Seventy-eight percent (observer 1) and 84% (observer 2) of all patent grafts were found to be evaluable concerning presence or absence of stenosis. In 34 of 40 (observer 1) and 38 of 43 (observer 2) bypass grafts, high-grade stenoses were correctly ruled out (specificity 85% versus 88%, sensitivity 80% and 82%). Yet, if all patients with either unevaluable grafts/graft anastomosis or relevant graft stenosis were excluded, only 8/32 patients (25%) had fully diagnostic "negative" graft-CTA. According to Kappa statistics, agreement between the observers was 1.0 and 0.93 concerning occlusion and relevant stenosis, respectively. CONCLUSION: Sixteen-slice coronary CTA with sub-millimeter spatial resolution and premedication with oral beta-blockade permits non-invasive assessment of coronary artery bypass grafts with decreasing numbers of unevaluable graft segments.
