Renal oncocytomas are uncommon, benign tumors that classically are treated by local excision or heminephrectomy. Preoperative differentiation from renal cell carcinoma is invaluable in the planning of treatment. Cases of renal oncocytoma treated conservatively have been reported. This is a case report of bilateral multifocal renal oncocytomas of which only three previous cases have been reported. Diagnosis was made from multiple fine needle biopsies and present treatment consists only of conservative observation.  相似文献

A 20-year-old woman, who presented with a several-week history of abdominal pain, was referred for magnetic resonance imaging (MRI) and 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) after an ultrasound showed complex cystic masses arising from both ovaries. The MRI and 18F-FDG PET/CT imaging characteristics of the ovarian masses were strongly suspicious for malignancy, and the masses were surgically removed. Histopathological evaluation revealed a bilateral tubo-ovarian abscess, with no evidence of malignancy. This case highlights a potentially serious pitfall in the evaluation of suspicious pelvic masses by 18F-FDG PET/CT, whereby a complex bilateral tubo-ovarian abscess may mimic the PET/CT imaging characteristics of an ovarian or pelvic malignancy.  相似文献

Renal oncocytomas are uncommon, benign tumors that can be treated by local excision or heminephrectomy; their preoperative differentiation from renal cell carcinoma, treated by radical nephrectomy, would be invaluable. A particularly important finding, a central scar--not stressed in previous reports, is frequently demonstrated by CT examination. We evaluated radiographic studies of 18 pathologically confirmed cases of oncocytoma and compared findings with results of CT, sonography, and angiography studies of 18 renal cell carcinoma cases. Oncocytomas can be suggested if a stellate scar is identified within an otherwise homogenous tumor on ultrasound (US) and CT; if the mass appears homogeneous but no scar is present, angiography should be performed. An oncocytoma can be suggested in these cases if a spoke-wheel configuration and homogeneous blush are present. These criteria, which are reliable only if the mass is 3 cm or larger, would have resulted in the correct diagnosis of oncocytoma in 16/18 cases.  相似文献

Renal metastasis of thyroid cancer is extremely rare. We report the case of a 62-year-old woman with Hürthle cell thyroid cancer (HCTC) with lungs, bones, and bilateral kidneys metastases. The renal metastatic lesions were clearly demonstrated by 131I whole body scan (WBS) with SPECT/CT. However, they exhibited false-negative results in 18F-FDG PET/CT, kidney ultrasonography, and contrast-enhanced CT scan. The findings imply that tumors have low glucose metabolism and are able to accumulate radioiodine, which is not commonly found in the relatively aggressive nature of HCTC. The patient received two sessions of 200 mCi 131I therapy within 6 months duration. There was complete treatment response as evaluated by the second post-therapeutic 131I SPECT/CT and serum thyroglobulin. To our knowledge, renal metastasis from HCTC with positive 131I but negative 18F-FDG uptake has not been reported in the literature. This case suggests that 131I SPECT/CT is useful for lesion localization and prediction of 131I therapy response.  相似文献

Although controversy exists among pathologists regarding the existence of a benign renal adenoma of proximal tubular origin (oncocytoma), certain “typical” angiographic features have been proported to characterize such renal masses. These include “spoke-wheel” vasculature, homogeneous blush, orderly vascular pattern, and absence of vascular puddling or venous shunting. These features may characterize oncocytomas but may also be shared by an occasional renal cell carcinoma. Nonetheless, their demonstration should alert radiologists and surgeons to the possibility that a given renal mass may be a benign oncocytoma. Such a presumptive diagnosis may have significant surgical implications.  相似文献

Metastatic struma ovarii is an extremely rare disease, and the treatment of choice has not been established. Here, we introduce the case of a 36-year-old female pregnant patient with metastatic struma ovarii. Initial treatment was an exploratory laparotomy to remove multiple peritoneal masses. After delivery, a total thyroidectomy was done for the further 131I-therapy. 131I-SPECT/CT and 18 F-FDG PET/CT showed multiple hepatic metastases and extensive peritoneal seeding nodules. Multiple 131I and retinoic acid combination therapies were performed, resulting in marked improvement. 131I-SPECT/CT and 18 F-FDG PET/CT were quite useful for evaluating the biologic characteristics of the metastases.  相似文献

Radiologic findings in renal oncocytoma   总被引:2,自引:0,他引:2  
The imaging findings of 36 renal oncocytomas in 32 patients were evaluated retrospectively. Twenty-two patients underwent ultrasound (US), 16 CT, 25 angiography, 19 urography, and 6 fine-needle biopsy. At US 8 of the oncocytomas were isoechoic, 7 hyperechoic, and 7 hypoechoic. The structure was homogeneous in 59% and nonhomogeneous in 41%. At plain CT 12 oncocytomas were homogeneously hypodense, 3 isodense, and one hyperdense. Contrast enhancement was homogeneous in 10 and inhomogeneous in 4 cases. A stellate scar was detected in one case, and in one case a contrast study was not performed. At angiography the oncocytomas were hypervascular in 21, hypovascular in 3 cases, and one was not detected. Nineteen of the tumors were well delineated. A spokewheel arterial pattern was detected in 3 tumors. The cytologic diagnosis was benign renal oncocytoma in 4 cases and highly suggestive of oncocytoma in 2 cases. We conclude that the homogeneity of a renal tumor at US and at CT may suggest the possibility of oncocytoma. US-guided fine-needle biopsy is a useful diagnostic tool in patients who need a precise presurgical diagnosis.  相似文献

The findings of an ovarian mass with marked ascites and pleural effusion are highly suggestive of malignancy, especially in a postmenopausal female with an elevated level of CA-125. However, benign conditions such as Meigs’ syndrome should be considered in the differential diagnosis if the primary mass shows benign features. 18F-FDG is known to be useful to differentiate between malignant and benign diseases, and this utility is also promising in the case of ovarian tumor. We present here a case of Meigs’ syndrome that was evaluated by 18F-FDG PET/CT, and this helped the preoperative diagnosis be made. 18F-FDG PET/CT is a promising modality to diagnose the pathological character of an ovarian tumor preoperatively, which can lead to a proper therapeutic plan.  相似文献

Increased 18F-fluorodeoxyglucose (FDG) uptake of brown fat on 18F-FDG positron emission tomography (PET) originating from physiological activation is a common incidental finding and is usually located in the neck, shoulder, and supraclavicular areas. We present a case of an incidental pheochromocytoma showing diffusely increased 18F-FDG uptake in bilateral perirenal fat tissue as well as supraclavicular and paravertebral fat tissue on 18F-FDG PET/CT. The patient had no clinical symptoms except hypertension, and a pheochromocytoma was confirmed in a postsurgical specimen. A pheochromocytoma should be considered a cause in cases of increased 18F-FDG uptake of perirenal brown fat.  相似文献

Renal oncocytomas are benign, solid tumours of the kidney. An angiographic spoke-wheel pattern is known to be associated with oncocytomas, although it is not pathognomonic. On review of the literature, we found two reports of sonographic spoke-wheel appearance in oncocytomas. These were sufficiently characteristic to enable a confident preoperative diagnosis of oncocytoma. We present a case of a surgically proven oncocytoma with a distinct helical CT appearance, -commensurate with the angiographic and sonographic spoke-wheel appearance from which the diagnosis was suspected preoperatively.  相似文献

Giant cell tumor (GCT) is a generally benign bone tumor accounting for approximately 5 % of all primary bone neoplasms. Cystic components in GCTs that indicate secondary aneurysmal bone cysts (ABCs) are reported in 14 % of GCTs. Although both of them have been described separately in previous reports that may show considerable fluorodeoxyglucose (FDG) uptake despite their benign nature, the findings of GCT with secondary ABC on 18F-FDG positron emission tomography/computed tomography (PET/CT) have not been well-known. We report a case of GCT with secondary ABC in a 26-year-old woman. 18F-FDG PET/CT revealed a heterogeneous hypermetabolic lesion in the left proximal femur with the maximum standardized uptake value of 4.7. The solid components of the tumor showed higher FDG uptake than the cystic components. These observations suggest that the ABC components in GCTs show heterogeneous metabolic patterns on 18F-FDG PET/CT.  相似文献

Metastatic mucinous adenocarcinoma in an inguinal hernia is a rare disease and the image findings of 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET)/computed tomography (CT) are little known. Here, we introduce a 57-year-old man with metastatic mucinous adenocarcinoma in an inguinal hernia. On initial 18F-FDG PET/CT, hypermetabolism was observed in mucinous adenocarcinoma of the cecum, and adenocarcinomas of the transverse and ascending colon, respectively. Follow-up 18F-FDG PET/CT revealed newly developed multiple hypermetabolism in peritoneal seeding masses and nodules in the pelvic cavity and scrotum. Peritoneal carcinomatosis in the right pelvic side wall was extended to the incarcerated peritoneum and mesentery in the right inguinoscrotal hernia.18F-FDG PET/CT was useful to reveal unexpected peritoneal seeding within the inguinal hernia. Also, this case demonstrated that metastatic mucinous adenocarcinomas had variably intense FDG uptake.  相似文献

Objective The aim of this study is to evaluate an imaging approach using computed tomography (CT), magnetic resonance imaging (MRI), technetium-99m pertechnetate scintigraphy (99mTc pertechnetate scintigraphy), gallium-67 scintigraphy (67Ga scintigraphy) and fluorine-18 fluorodeoxyglucose positron emission tomography (18F-FDG PET). Material and Methods We reviewed retrospectively 59 patients with parotid masses. CT, MRI,67Ga scintigraphy,99mTc pertechnetate scintigraphy, and18F-FDG PET were performed. Results All of the benign tumors had smooth margins on CT and MRI. Patients with inflammatory lesions and malignant lesions showed well-defined margins or ill-defined margins. All Warthin's tumors showed high technetium accumulation. Many of malignant tumors showed high FDG accumulation. Both pleomorphic adenomas and Warthin's tumors showed high accumulation in some cases on18F-FDG PET. Among 15 patients with pleomorphic adenoma, 14 patients showed marked hyperintensity relative to CSF on T2-weighted images and partial enhancement on contrast-enhanced T1-weighted images. Combination of several imaging modarity offered usefulness of differential diagnosis for parotid masses. Conclusion An efficient combination of imaging methods may be helpul for achieve the correct diagnosis.  相似文献

A 60 year old woman who presented with multiple small subcutaneous nodules in the upper back and arms, was referred for an [18F] fluoro-2-deoxy-D-glucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) after histological evaluation revealed metastatic leiomyosarcoma of unknown origin. The PET/CT showed multiple 18F-FDG-avid subcutaneous nodules, bone lesions, as well as a large left renal mass, which was biopsied to confirm a primary renal leiomyosarcoma arising from the renal parenchyma. A post therapy PET/CT showed overall progression of disease. The use of 18F-FDG PET/CT in the staging and evaluation of response to therapy of a renal leiomyosarcoma has not been previously described in the literature.  相似文献

Tetrasomy 8 is a relatively rare chromosomal abnormality that has been reported in only 33 cases in hematologic disorders. It is known for its association with aggressive acute myeloid leukemia (AML) and myeloid sarcoma and is considered a very poor prognostic factor. Myeloid sarcoma is a rare hematologic malignancy characterized by tumor masses consisting of immature myeloid cells, presenting at an extramedullary site. We present a case of a 17-year-old boy referred for an 18F-FDG PET/CT for the evaluation of pleural masses and spinal bone lesions seen on CT, after presenting with a 4 month history of chest pain. The PET/CT revealed extensive FDG-avid extramedullary disease in the soft tissues of the chest, abdomen, and pelvis, which were biopsy-proven to be myeloid sarcoma, as well as extensive intramedullary disease biopsy proven to be AML. This is the first report of the use of 18F-FDG PET/CT to stage a subset of aggressive AML and myeloid sarcoma in a patient with an associated chromosomal abnormality (tetrasomy 8).  相似文献

Adrenocortical oncocytoma is a very rare tumor, which is not malignant and nonfunctioning in most cases. We report a case of a 53-year-old male with a 9.8 cm sized hyperfunctioning, well-encapsulated adrenal mass, which exhibited by high FDG uptake on a PET/CT scan. The patient had complained of symptoms of Cushing’s syndrome for 4 months. Laparoscopic adrenalectomy was performed and the mass was pathologically confirmed as adrenocortical oncocytoma of uncertain malignant potential. Four years after surgery, the tumor recurred with distant metastases, which was proven by subsequent biopsy. 18F-FDG PET/CT also showed hypermetabolism in the recurred tumor and multiple metastatic lesions. Adrenocortical oncocytoma of borderline malignant potential with high FDG uptake may require long-term follow-up with clinical, hormonal, and imaging evaluations.  相似文献

A 66-year-old man, who presented with bright red blood per rectum, was referred for an 18F-FDG PET/CT after colonoscopy showed two suspicious colon masses, which were biopsied to reveal an adenocarcinoma of unknown origin. PET/CT showed two intensely FDG-avid colon masses as well as an unsuspected FDG-avid lung mass, which was biopsied to reveal a primary lung adenocarcinoma. Immunohistochemistry confirmed the two colon metastases were of pulmonary origin. It is extremely rare for lung carcinoma to present with symptomatic colon metastases, with only 11 cases described in the literature. We report the first case of the utility of 18F-FDG PET/CT in staging a patient who presented with symptomatic colon metastases of an unknown primary lung malignancy.  相似文献

目的探讨多层螺旋CT(MSCT)四期扫描对肾嗜酸细胞腺瘤(renal oncocytoma,RO)与嫌色细胞癌(chro-mophobe cell renal carcinoma,CCRC)的鉴别诊断价值。资料与方法搜集经手术病理证实的8例(9个)RO和15例CCRC患者的资料,术前全部行CT平扫和三期增强扫描,采用SPSS 16.0软件对两种肿瘤的影像学表现进行统计学处理。结果病变平扫时密度值、钙化、强化的均匀性及中央星状瘢痕在RO组和CCRC组间的差异具有统计学意义(P<0.05);病变的大小、增强扫描各期的强化程度以及病变出现包膜、出血、坏死囊变、"辐轮"状强化及节段性增强反转等征象的比较,差异无显著性统计学意义(P>0.05)。结论 MSCT对术前RO和CCRC的鉴别诊断具有重要价值,平扫密度值、钙化、强化的均匀性及中央星状瘢痕有助于两者的鉴别。  相似文献

Fluid levels in aneurysmal bone cysts: a CT feature   总被引:2,自引:0,他引:2  
The sonographic features of 15 histologically confirmed renal oncocytomas in 14 patients are presented. Lesions were evaluated for homogeneity, echogenicity, margination, venous invasion, and nodal enlargement. Nine were less than 5.5 cm in diameter, homogeneous, and well circumscribed. Three lesions were greater than 8 cm in diameter, contained areas of central necrosis or calcification, and were nonspecific for renal oncocytoma. One of these lesions (12 cm) contained a central scar. Two masses were slightly inhomogeneous and, at best, moderately circumscribed. In no case were metastatic nodes or venous invasion present. Since about 6% of renal cell carcinomas may have this appearance, the homogeneous, well marginated renal mass that is isoechoic with cortex and less than 5.5 cm in diameter is as likely to represent renal cell carcinoma as it is an oncocytoma. Although only present in one of the cases, the central fibrotic scar in a larger mass has been described in oncocytoma and may be the most specific feature. Preoperative investigation with fine-needle aspiration biopsy for cytology may be indicated in an attempt to avoid radical nephrectomy in selected patients.  相似文献

Purpose

To evaluate the role of 64 multidetector CT (MDCT) with multiplanar reformation in evaluation of bilateral ovarian masses and to correlate the findings with histopathological results and surgery.

Material and methods

MDCT was performed to 23 patients with sonographically detected bilateral ovarian masses. All scans were performed on a 64 row CT scanner at the oncology center, Mansoura University. The axial and reformatted images were evaluated for lesion characterization and the relationship to adjacent structures. The radiological findings were correlated with operative and pathological findings.

Results

There were 11 bilateral malignant lesions and 9 bilateral similar benign lesions. Three cases showed different benign lesions in both sides.All malignant lesions were read by MDCT as malignant, also all benign lesions were read by MDCT as benign except for one case with endometrioid cyst which was diagnosed as malignant tumor by CT.

Conclusion

64 Multidetector CT with MPR in different planes allows good visualization and characterization of bilateral ovarian masses with greater diagnostic accuracy and improves the detection of peritoneal metastases.  相似文献
