Objective To explore the changes of the nerve fibers from median and ulnar nerves after cutting the branches of lower trunk which was repaired by the contralateral C7.Methods Forty female SD rats were divided into A, B, C and D groups randomly.In group A,the contralateral C7 root was transferred to lower trunk directly, and the posterior division of lower trunk, medial anterior thoracic nerve and the medial antebrachial cutaneous nerve were severed at the beginning of them;In group B, the contralateral C7 root was trarsferred to lower trunk directly, and the posterior division of lower trunk, medial anterior thoracic nerve and the medial antebrachial cutaneous nerve were severed at the point which was 1 cm away from the beginning of above branches;In group C, the contralateral C7 root was transferred to lower trunk directly, and the posterior division of lower trunk was severed at the beginning of it;In group D, control group.After the operation, myelinated fiber count, nerve fiber density, the percentage of the number of nerve fiber from branch accounting for that from lower trunk, nerve fiber diameter,the percentage of nerve fibers with different diameters and N Ratio were carried out to evaluate the outcome of each group.Results Myelinated fiber count, nerve fiber density, the percentage of the number of nerve fiber from branch accounting for that from lower trunk, nerve fiber diameter,the percentage of nerve fibers with different diameters and N Ratio in ulnar and median nerve, there were no difference between group A, group B and group C ( P > 0.05).Conclusion After the medial anterior thoracic nerve and medial antebrachial cutaneous nerve, repaired by the contralateral C7, were severed at the beginning and at the point which was 1 cm away from the beginning of above branches, the changes of the quantity and quality of the nerve fibers from median and ulnar nerves were not significant.  相似文献

目的 以大鼠健侧颈7直接修复臂丛神经下干为模型,探讨阻断下干分支前后,尺神经和正中神经神经纤维数量和质量的变化.方法 雌性SD大鼠40只,随机分成4组.A组:健侧颈7直接修复下干,并从下干发出处阻断下干后股、胸前内侧神经、前臂内侧皮神经;B组:健侧颈7直接修复下干,并从下干发出处以远1 cm处阻断下干后股、胸前内侧神经、前臂内侧皮神经;C组:健侧颈7直接修复下干,并从下干发出处切断后股;D组:对照组.术后比较尺神经、正中神经、胸前内侧神经和前臂内侧皮神经的神经纤维数量、神经纤维密度(p)、神经纤维数占下干神经纤维总数百分比、神经纤维直径、有髓神经纤维面积与相应分支神经总面积比(N Ratio).结果 尺神经和正中神经中,神经纤维数量、神经纤维密度、正中神经与尺神经分别占下干神经纤维百分比、神经纤维直径、不同直径神经纤维百分比、N Ratio,A、B、C三组间差异均无统计学意义.前臂内侧皮神经和胸前内侧神经中,上述各检测指标B、C组间均无明显差异.结论 健侧颈7直接移位下干后,在根部及根部以远1 cm处阻断胸前内侧神经及前臂内侧皮神经后,对尺神经、正中神经、前臂内侧皮神经残端和胸前内侧神经残端中神经纤维的数量和质量无明显影响.

Objective To observe the ultrastructure of sciatic nerve of GFP transgenic mice under normal and damaged conditions as well as to explore the applications of laser scanning confocal micoscope for in vivo and real time observation of peripheral nerve morphology. Methods The GFP mice were anesthetized and the sciatic nerve was exposed.The mouse was then mounted on the platform of a laser scanning confocal microscope in a way that the nerve was in direct contact with the slide for real time morphological observation in vivo.The diameters of the nerve trunk,nerve fascicles and nerve fibers were measured and the number and morphology of nerve fascicles and nerve fibers were reconted.Then the sciatic nerve was subjected to crush or transection to create a nertve iniury model.Morphological changes of the nerve proximal to,at and distal to the injury site were observed immediately after the injury and 2 hours later by laser scanning confoeal microscope in vivo. Results The diameter of normal sciatic nerve trunk of mice was about 800 μm,the diameter of nerve fascicle and nerve fiber Were 70 to 100 μm and 7.5 μm respectivdy.Each normal sciatic nerve trunk contained about 2 tiscicles and 70 to 200 nerve fibers.Immediately after and 2 hours after nerve injury vaeuolization was seen at the nerve proximal to the injury site.The nerve fibers were:disoriented and disconnection of the fibers was also seen.Nerve fibers of the nerve at the injury site were distorted,with vacuolization and breaking down of the fibers.Nenve continuity was lost.At the nerve 250 μm and 500 μm distal to the injury site,the nerve fiber fluoescence intensity gradually increased and the nerve fiber structure restored gradually.Orientation of the fibers tended to be organized. Conclusion The use of GFP transgenic mice and laser scanning confocal microscope provides a technical platform for in vivo real time observation of peripheral nerves.It can be applied to observe the morphological features and changes of mouse peripheral nerves.  相似文献

目的 探讨修复腕掌尺侧皮肤神经同时缺损的新方法.方法 2000年4月至2009年8月,应用游离足底内侧皮瓣修复腕掌尺侧皮肤并神经缺损5例.足拇趾胫侧趾底固有神经修复小指尺掌侧固有神经缺损1例;桡神经浅支修复尺神经及其深浅支缺损2例,修复尺神经浅支、第4指掌侧总神经及小指尺掌侧固有神经缺损1例;尺神经手背支修复尺神经浅支、第4指掌侧总神经及小指尺掌侧固有神经缺损1例.足底内侧血管与尺血管吻合.供区取同侧大腿皮片移植修复.结果 术后皮瓣及移植皮片全部成活.5例获得6个月至4年的随访,皮瓣质地好、外观满意,无手内肌萎缩和爪形手畸形,皮瓣和手指感觉恢复达S3~S3+,皮瓣两点辨距觉为7~10 mm.尺神经深浅支缺损病例术后综合评价均为优.结论 游离足底内侧皮瓣是修复腕掌尺侧皮肤神经缺损的有效方法.
Abstract:
Objective To explore a new method for repair of concurrent skin and nerve defect at palm and carpal on ulnar side. Methods From April 2000 to August 2009, five cases with concurrent skin and nerve defect at palm and carpal on ulnar side were reconstructed with free medial plantar flaps.Palmar nervous proprii defect at ulnar side of little finger was repaired by the first toe tibia nervous proprii in one case. The superficial branch of radial nerve was applied to repair the defect of ulnar nerve, as well as its deep or superficial branch in two cases. The superficial branch of radial nerve was also used to repair the defect of superficial branch of ulnar nerve, common palmar digital nerve of the fourth finger, Little finger ulnar palmar nervous proprii in one case. The dorsal branch of ulnar nerve was applied to repair the defect of superficial branch of ulnar nerve, common palmar digital nerve of the fourth finger, little finger ulnar palmar nervous proprii in one case. The vascular bundle of medial plantar flap was anastomozised with ulnar vascular bundle. The wounds at donor sites were covered with free skin grafts which were obtained from upper leg. Results All the flaps and skin grafts were survived completely. The five patients were followed up for six months to four years with no muscular atrophy or claw hand deformity. The esthetic result was satisfied. The Sensory of flaps and fingers recovered to S3 to S3+. The two-point discrimination distance on flaps was range from 7 mm to 10 mm. The postoperative comprehensive evaluation was excellent in the cases whose superficial and deep branches of ulnar nerve were repaired.Conclusions Free medial plantar flap is an effective method to repair concurrent skin and nerve defect at palm and carpal on the ulnar side.  相似文献

Objective:To compare the effect of using partial median and ulnar nerves for treatment of C5-6 orC5-7 avulsion of the brachial plexus with that of using phrenic and spinal accessary nerves.Methods:The patients were divided into 2groups randomly according to different surgical procedures.Twelve cases were involved in the first group.The phrenic nerve was transferred to the musculocutaneous nerve or through a sural nerve graft,and the spinal accessary nerve was to the suprascapular nerve.Eleven cases were classified into the second group.A part of the fascicles of median nerve was transferred to be coapted with the motor fascicle of musculocutaneous nerve and a part of fascicles of ulnar nerve was transferred to the axillary nerve.The cases were followed up from 1to 3years and the clinical outcome was compared between the two groups.  相似文献

Objective To study the feasibility of subclavicular brachial plexus block with localized puncture at the apex of the axilla.Methods 520 adult patients who underwent upper limb emergency surgery were randomly assigned to receive subclavicular brachial plexus block with localized puncture at the apex of the axilla ( experimental group) and block anesthesia with intraclavicular route ( control group).Patients in both groups received 0.375% ropivacaine, volume (40.0± 2.7)ml.During the 30 minutes after the blockade, the onset and effect of motor and sensory block, the sensory block effect of each nerve branches and integrated effect of nerve block were recorded.The VAS score and acesodyne duration with tourniquet were compared between the two groups at 2, 4, 6 and 8 hours after the drug had been used for 30 minutes.Results Within 30 minutes after the drug was delivered, the onset of motor and sensory block in the experimental group was evidently shorter than that in the control group, while the duration of motor and sensory block was evidently longer ( P < 0.01).There were no statistically significant differences in motor and sensory block score, median nerve block effect and ulnar nerve block effect between the two groups.The musculocutaneous nerve and radial nerve block effects in the experimental group were better than those in the control group ( P < 0.05).After the drug had been used for 30 minutes, VAS score of the experimental group at each point time was significantly lower than that of the oontrol group ( P <0.01).The analgesia duration of tourniquet in the experimental group was significantly longer than that of the control group ( P < 0.01 ).Conclusion The modified subclavicular brachial plexus block with localized puncture at the apex of the axilla can provide perfect blockade, satisfactory anesthetic effect and higher safety in upper limb surgery.  相似文献

肘下动脉穿支蒂前臂外侧皮神经营养血管皮瓣的解剖基础   总被引:1,自引:0,他引:1  
目的 为肘下动脉穿支蒂前臂外侧皮神经营养血管皮瓣修复肘部软组织缺损提供解剖学基础.方法在30侧动脉内灌注红色乳胶的成人上肢标本上解剖观查:①前臂外侧皮神经的走行与分布;②肘下动脉与前臂外侧皮神经营养血管间吻合关系.另在1侧新鲜标本上进行摹拟手术设计.结果①前臂外侧皮神经主干行于前臂桡侧,分布于前臂外侧1/3区域;②营养血管为多节段、多源性,其中肘下动脉的位置相对恒定,在头静脉与深静脉系统的交通支形成的倒"V"状顶点到达皮肤,并分出众多的细小血管与前臂外侧皮神经的神经旁和神经干血管链的分支密切吻合.结论 可形成肘下动脉穿支蒂前臂外侧皮神经营养血管皮瓣顺行转位修复肘部软组织缺损.
Abstract:
Objective To provide anatomical basis for lateral antebrachial neurocutaneous flap pedi-cled with inferior cubital artery perforator in repairing tissue defects around elbow joint. Methods Thirty embalmed upper limbs of adult cadavers perfused with red latex were used for this study, and followings were observed:①The course and distribution of lateral antebrachial cutaneous nerve; ②Anastomoses between inferior cubital artery and nutrient vessels of lateral antebrachial cutaneous nerve. Mimic operation was performed on other side of fresh specimen. Results ①The main trunk of lateral antebrachial cutaneous nerve (LACN) lined in the radial forearm and distributed in the 1/3 region of lateral forearm. ①The nutritional vessels of the flap were plurisegmental and polyphyletic. The inferior cubital artery which was relatively constant reached to skin through "V"-shaped peak formed by communicating branches of cephalic vein and deep venous system. They also gave off large number of small veins, which closely aligned with perineural branches and neural stem vascular chain of lateral antebrachial cutaneous nerve. Conclusion The lateral antebrachial neurocutaneos flap pedicled with inferior cubital artery perforator can be formed to repaire tissue defects around elbow joint.  相似文献

Objective To evaluate the therapeutic effect of in situ ulnar nerve decompression at the cubital tunnel via a small incision assisted with electromyography localization and discuss the surgical indications.Methods Twelve patients who were diagnosed with idiopathic cubital tunnel syndrome (CuTS) without intrinsic muscle atrophy and elbow deformity were involved in the study.Before the operation, short-segment nerve conduction test (SSCT) was carried out.The exact compression site was determined by the > 50%reduction in amplitude or > 0.5 ms lengthening in latency of action potentials recorded upon stimulation of the ulnar nerve around the elbow at 1 cm intervals.An in situ ulnar nerve release at the compression site was performed.Compression of the ulnar nerve was observed and documented to verify the accuracy of pre-operative SSCT localization.Results Intraoperative findings confirmed that lesions were located from 3 cm above to 1 cm below the medial epicondyle, which coincided with the compression sites determined by SSCT.All the patients reported alleviation of hand discomfort postoperatively.Follow-up at 3 months postoperatively showed that paresthesia in the distribution of the ulnar nerve in the hand disappeared.Pinprick sensation recovered.There was no subjective or measurable weakness in pinch or grip strength and no clumsiness or loss of coordination.Claw deformity disappeared.Six months after the surgery, the strength of abductor digiti minimi returned to normal.Two-point discrimination of the little finger was 5.0 mm on average.Nerve conduction velocity returned to > 45.0 m/s.Action potential amplitude increased and SSCT yielded no positive findings.Mild atrophy was reversed one year postoperatively.Elbow flexion test, Tinel' s sign and Froment' s test were all negative.Conclusion In situ ulnar nerve decompression via a small incision assisted with electromyography localization is a suitable procedure for certain CuTS cases.  相似文献

[目的]探讨胸腰椎骨折椎弓根螺钉内固定系统内固定术后,椎弓根螺钉断裂与植骨融合方式之间的关系,以探讨胸腰椎骨折植骨融合的最佳方式。[方法]回顾性研究1995年5月~2005年12月本院脊柱外科收治的胸腰椎骨折病人197例,其中A组单纯内固定(不植骨)患者14例,B组“H”形椎板植骨21例,C组横突间植骨67例,D组椎间、椎内联合横突间植骨95例。[结果]术后随访6~32个月,内固定断裂12例,其中A组4例,B组3例,C组5例,D组0例,4组中D组内固定断裂率显著低于其他3组(P<0.05)。[结论]椎间、椎体内联合横突间植骨重建脊柱三柱的稳定性,符合人体生物力学原理,能有效降低内固定断裂的发生。  相似文献

We reviewed 39 patients with displaced three- and four-part fractures of the humerus. In 21 patients (group A) we had used an anatomical prosthesis for the humeral head and in 18 (group B) an implant designed for fractures. When followed up at a mean of 29.3 months after surgery the overall Constant score was 51.9 points; in group A it was 51.5 and in group B 52.4 points. The subjective satisfaction of the patients was assessed using a numerical rating scale and was similar in both groups. In group A complete healing of the tuberosities was found in 29% and 50% in group B. Partial integration was seen in 29% of group A and in only one patient in group B, while resorption was noted in 43% of group A and 44% of group B. The functional outcome was significantly better in patients with complete or partial healing of the tuberosities (p=0.022). The specific trauma prosthesis did not lead to better healing of the tuberosities. The difference in clinical outcome obtained by the two designs did not reach statistical significance.  相似文献
