目的调查护理人员对设置高级实践护士角色职能的认同情况。方法调查北京市某三级甲等医院140名护士,问卷内容包括对设置高级实践护士的总体看法以及对其角色职能的认同情况。结果护士对设置高级实践护士角色持肯定态度;对绝大多数高级实践护士角色职能(39个条目)高度认同,对少数角色职能(7个条目)认同率不高甚至不认同。结论设置高级实践护士角色是临床护理人员的共识,护理人员对其角色职能的认同将有力推动我国高级实践护士的发展;现阶段我国高级实践护士的发展应以临床护理专家为主。 相似文献

目的 调查外科护士对外科高级实践护士角色的认知与态度情况。方法 采取方便抽样法,于2019年6月27日—7月9日抽取四川大学华西医院外科护士作为研究对象。采用外科高级实践护士角色的认知与态度调查问卷对其进行调查。结果 外科护士对于高级实践护士相关名词了解比例最高的前3位分别为专科护士(630/92.1%)、临床护理专家(529/77.3%)、高级助产士(366/53.5%),对于国内Advanced Practice Nurse译名认可度最高的为临床护理专家(416/60.8%)。97.1%的外科护士认为外科高级实践护士最重要的核心能力为临床实践护理能力,62.3%的护士认为APN应负责疑难复杂病例的临床业务管理,不具体分管床位。48.0%~74.3%的外科护士认为APN应具备本科及以上学历、10年以上工作经验、主管护师及以上职称,且需要有国外交流经验,由国家级卫生行政部门定期认证。95.6%的外科护士对国内发展外科高级实践护士表示支持。结论 本组外科护士对于外科高级实践护士的发展态度积极,认为外科高级实践护士应该是以临床护理业务管理为核心、以提高护理质量为目的指导者、沟通者和研究者,需要定期考核并由国家级卫生行政部门给予认证资质。但目前国内发展外科高级实践护士仍存在一定阻碍,未来发展外科高级实践护士应以临床护理专家角色为抓手,对名称选择、岗位职责和资质认定等进行统一与规范,从而体现高级实践护士在提升临床护理质量中的作用。 相似文献

目前国际上较认可的高级实践护士(APN)角色能力评价工具有APN评估工具包、Scarpa的APN角色评估工具、Strong高级实践角色描述工具(M-Strong APRD)和胜任力角色评价工具(PEAC Tool)等。角色能力评价指标包括患者质量安全指标、护理专业发展指标、多学科合作指标和对医疗保健系统的影响指标等。本文旨在总结国际上APN角色能力评价的方法,为我国APN角色能力评价提供理论依据。在临床应用时,可将角色能力评价工具和角色能力评价指标相结合,综合应用多维度的评价方式进行全面地评价,以更好地明确APN的独立贡献。 相似文献

Purpose: To examine the development of advanced nursing practice globally.
Methods: Data were collected from documentary resources available in the International Nurse Practitioners/Advanced Practice Nurse Network (INP/APNN) of the International Council of Nurses. The areas examined were guided by the "key informant survey on advanced nursing practice self-administered questionnaire." Two core members of the INP/APNN who have rich experience in global advanced nursing development analyzed the data. A total of 14 countries and three regions from five continents were included in the analyses. The development of advanced nursing practice in these areas is facilitated by a need for better access to care in a cost-containment era and the enhancement of nursing education to postgraduate level. The mechanism for regulation of practice is in place in some countries.
Conclusions: Confirms the development of advanced practice in nursing is a global trend.
Clinical Relevance: APNs can improve global health with points to enhanced education in nursing and regulation of advanced practice. 相似文献

Advanced practice registered nurses (APRNs) have been used by organizations to meet the needs of their patients. However, there is a lack of organizational structure in place for appropriate APRN supervision. Oftentimes, there are non-APRN supervisors who lack understanding of APRNs, leading to poorly defined roles, credentialing, understanding of scope of practice, and lack of shared governance. With an advanced practice nursing organizational leadership model, there is evidence for improved job satisfaction, retention, and accountability for APRNs. AIM: The aim of this paper is to discuss six issues influencing the introduction of advanced practice nursing (APN) roles: confusion about APN terminology, failure to define clearly the roles and goals, role emphasis on physician replacement/support, underutilization of all APN role domains, failure to address environmental factors that undermine the roles, and limited use of evidence-based approaches to guide their development, implementation and evaluation. BACKGROUND: Health care restructuring in many countries has led to substantial increases in the different types and number of APN roles. The extent to which these roles truly reflect advanced nursing practice is often unclear. The misuse of APN terminology, inconsistent titling and educational preparation, and misguided interpretations regarding the purpose of these roles pose barriers to realizing their full potential and impact on health. Role conflict, role overload, and variable stakeholder acceptance are frequently reported problems associated with the introduction of APN roles. DISCUSSION: Challenges associated with the introduction of APN roles suggests that greater attention to and consistent use of the terms of the terms advanced nursing practice, advancement and advanced practice nursing is required. Advanced nursing practice refers to the work or what nurses do in the role and is important for defining the specific nature and goals for introducing new APN roles. The concept of advancement further defines the multi-dimensional scope and mandate of advanced nursing practice and distinguishes differences from other types of nursing roles. Advanced practice nursing refers to the whole field, involving a variety of such roles and the environments in which they exist. Many barriers to realizing the full potential of these roles could be avoided through better planning and efforts to address environmental factors, structures, and resources that are necessary for advanced nursing practice to take place. CONCLUSIONS: Recommendations for the future introduction of APN roles can be drawn from this paper. These include the need for a collaborative, systematic and evidence-based process designed to provide data to support the need and goals for a clearly defined APN role, support a nursing orientation to advanced practice, promote full utilization of all the role domains, create environments that support role development, and provide ongoing evaluation of these roles related to predetermined goals. 相似文献

A variety of terms are used to describe advanced practice nursing roles internationally. This has resulted in confusion in terminology around these roles. The aim of this concept analysis was to clarify what is meant by advanced practice nursing internationally, what attributes signify advanced practice nursing and what are its antecedents, consequences, references and related terms. Rodgers's evolutionary method of concept analysis was used. Data sources included Medline, CINAHL, Applied Social Sciences Index and Abstracts (ASSIA), Cochrane Library, Science Direct, SCOPUS, Web of Science, Dissertation Abstracts and DARE as well as relevant nursing texts and professional organization websites. The analysis reveals that there are many different articulations of the advanced practice nursing role outlined in the literature. This variety in terminology hinders developments in advanced practice nursing roles. Consensus on advanced practice nursing definitions, terminology, educational requirements and regulatory approaches is integral to the implementation of the advanced practice nursing role internationally. 相似文献

Aims and objectives. This paper aims to explore the critical elements of advanced nursing practice in relation to policy, education and role development in order to highlight an optimal structure for clinical practice. Background. The evolution of advanced nursing practice has been influenced by changes in healthcare delivery, financial constraints and consumer demand. However, there has been wide divergence and variations in the emergence of the advanced nurse practitioner role. For the successful development and implementation of the role, policy, educational and regulatory standards are required. Conclusion. The paper highlights the value of a policy to guide the development of advanced nursing practice. Educational curricula need to be flexible and visionary to prepare the advanced nurse practitioner for practice. The core concepts for the advanced nursing practice role are: autonomy in clinical practice, pioneering professional and clinical leadership, expert practitioner and researcher. To achieve these core concepts the advanced nurse practitioner must develop advanced theoretical and clinical skills, meet the needs of the client, family and the community. Relevance to clinical practice. In a rapidly changing people‐centred healthcare environment the advanced nurse practitioner can make an important contribution to healthcare delivery. The challenges ahead are many, as the advanced nurse practitioner requires policy and appropriate educational preparation to practice at advanced level. Distance education programs are increasingly common in nursing education, and educators are challenged with creating online courses that provide students with quality learning experiences. For advanced practice nursing (APN) students, online courses need to engage students in learning while meeting their unique needs. A key component of workforce reform is the international growth in Advanced Practice Nursing (APN) roles. This study evaluated one APN role in Australia, the Clinical Nurse Consultant (CNC). All 56 CNCs employed in a tertiary hospital in New South Wales took part in the study. Demographic and work activity data were collected by an online questionnaire. Face‐to‐face interviews included the administration of a 50‐point tool to score the level of practice of each CNC against five domains. The domains of practice did not appear to have played a central role in the design of these CNC roles despite being defined in the industrial legislation and linked to a pay structure. There was widespread variability in the level of practice both within and between the CNC grades as well as significant differences in job content. Few CNCs managed to achieve a moderate level of practice across all five domains. The findings suggest that the distinctive features of the CNC roles as articulated in the domains of practice are often not realized in practice. 相似文献
