目的:研究近视患者双眼暗视至明视状态下Kappa角水平偏移量及垂直偏移量的变化规律。方法:横断面研究。选取2020年11月至2021年12月在宁夏医科大学总医院门诊近视中心明确诊断为近视的患者,并采用单纯随机抽样的方法抽取120例(240眼),采用博士伦Orbscan II角膜地形图仪分别于暗视及明视条件下进行检查,记录明暗视下瞳孔直径大小、Kappa角的大小及水平偏移量和垂直偏移量。采用配对t检验比较左右眼在暗视和明视下的瞳孔直径和Kappa角大小;采用散点分布图分析双眼在明暗视下Kappa角的象限分布。结果:左右眼在暗视状态下的瞳孔直径均明显大于明视状态下的瞳孔直径,差异有统计学意义(t=13.67,P<0.001;t=13.48,P<0.001)。暗视与明视状态下左眼的Kappa角均大于右眼的Kappa角,差异有统计学意义(t=4.15,P=0.021;t=5.27,P=0.008)。由暗视转向明视状态时,双眼Kappa角的分布呈现镜像对称,水平方向主要向鼻侧位移,垂直方向主要向上方位移。右眼矢量位移(0.168±0.100)mm,左眼矢量位移(0.171±0.069)mm;87.5%右眼矢量位移小于0.3 mm,92.5%左眼矢量位移小于0.3 mm,绝大多数近视患者的矢量位移在0.3 mm以内。结论:从暗视至明视状态下,随着瞳孔缩小,近视患者的Kappa角分布主要向鼻上方位移。 相似文献

PurposeTo learn more about the locations of dopamine D2 receptors (D2Rs) that regulate form-deprivation myopia (FDM), using different transgenic mouse models.MethodsOne eye of D2R-knockout (KO) mice and wild-type littermates was subjected to four weeks of monocular FDM, whereas the fellow eye served as control. Mice in both groups received daily intraperitoneal injections of either the D2R antagonist sulpiride (8 µg/g) or vehicle alone. FDM was also induced in retina- (Six3creD2Rfl/fl) or fibroblast-specific (S100a4creD2Rfl/fl) D2R-KO mice. A subset of retina-specific D2R-KO mice and D2Rfl/fl littermates were also given sulpiride or vehicle injections. Refraction was measured with an eccentric infrared photorefractor, and other biometric parameters were measured by optical coherence tomography (n ≈ 20 for each group).ResultsFDM development was attenuated in wild-type littermates treated with sulpiride. However, this inhibitory effect disappeared in the D2R-KO mice, suggesting that antagonizing D2Rs suppressed myopia development. Similarly, the development of myopia was partially inhibited by retina-specific (deletion efficiency: 94.7%) but not fibroblast-specific (66.9%) D2R-KO. The sulpiride-mediated inhibitory effects on FDM also disappeared with retinal D2R-KO, suggesting that antagonizing D2Rs outside the retina may not attenuate myopia. Changes in axial length were less marked than changes in refraction, but in general the two were correlated.ConclusionsThis study demonstrates that D2Rs located in the retina participate in dopaminergic regulation of FDM in mice. These findings provide an important and fundamental basis for further exploring the retinal mechanism(s) involved in dopamine signaling and myopia development. 相似文献

目的:探讨配戴角膜塑形镜6个月后去片裸眼视力低下的影响因素及近视控制效果。方法:回顾性系列病例研究。收集2015年4月至2017年1月于北京同仁医院验光配镜中心验配角膜塑形镜并随访至少6个月的青少年儿童,以去片后即刻裸眼视力能否达到0.8为标准将入选儿童分为2组。比较2组间年龄、视力、屈光度、眼压、角膜曲率、e值、角膜直径、瞳孔直径、非对称性指数、规则性指数的异同,以及对近视的控制效果。采用单因素及多因素线性回归分析去片裸眼视力的影响因素。结果:共50例(96眼)入组,年龄(10.6±2.4)岁,球镜度(-3.62±1.18)D,柱镜度(-0.33±0.43)D,眼轴长度(25.18±0.75)mm。其中裸眼视力≤0.8组52眼,裸眼视力>0.8组44眼,通过单因素分析,较差的裸眼视力与基础较长的眼轴长度(b=-0.09,β=-0.29,P=0.003)、较高的球镜度(b=0.07,β=0.36,P<0.001)、较高的柱镜度(b=0.12,β=0.22,P=0.030)有关。通过多因素线性回归分析,较差的裸眼视力与基础较高的球镜度(b=0.05,β=0.27,P=0.018)有关。6个月后裸眼视力≤0.8组儿童眼轴增长(0.07±0.15)mm,裸眼视力>0.8组儿童眼轴增长(0.11± 0.15)mm,差异无统计学意义。结论:影响角膜塑形镜去片裸眼视力的主要因素是屈光度和眼轴长度,屈光度越高、眼轴越长,去片后裸眼视力越差,但其对近视仍有较好的控制作用。 相似文献

Of all the functions that define visual performance, the mesopic luminous efficiency function is probably the most complex and hardest to standardise or model. Complexities arise because of the substantial and often rapid visual changes that accompany the transition from scotopic to photopic vision. These are caused not only by the switch from rod to cone photoreceptors, but also by switches between different post-receptoral pathways through which the rod and cone signals are transmitted. In this review, we list several of the complexities of mesopic vision, such as rod-cone interactions, rod saturation, mixed photoreceptor spectral sensitivities, different rod and cone retinal distributions, and the changes in the spatial properties of the visual system as it changes from rod- to cone-mediated. Our main focus, however, is the enormous and often neglected temporal changes that occur in the mesopic range and their effect on luminous efficiency. Even before the transition from rod to cone vision is complete, a transition occurs within the rod system itself from a sluggish, sensitive post-receptoral pathway to a faster, less sensitive pathway. As a consequence of these complexities, any measure of mesopic performance will depend not only on the illumination level, but also on the spectral content of the stimuli used to probe performance, their retinal location, their spatial frequency content, and their temporal frequency content. All these should be considered when attempting to derive (or to apply) a luminous efficiency function for mesopic vision. 相似文献

Objectives: To evaluate the safety indexes and efficacy of Artisan phakic intraocular lens (IOL) for the correction of high myopia.
Methods: Retrospective interventional case series reports. Thirty-one eyes (22 patients) with myopia from −5.25 to −19.00 diopters
underwent implantation of an Artisan phakic IOL. Follow-up examinations were performed at 1 day, 1 week, 1 month, and 3 months.
The following parameters were recorded: manifest refraction, slit-lamp examination, applanation tonometry, uncorrected visual
acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), complications or adverse reactions. The primary variable was
the refractive outcome at 3 months. Secondary variables were change in BSCVA, and efficacy and safety indexes. Results: At 3 months, mean spherical equivalent was −0.50 ± 0.36 diopters (range, −1.25 – plano). In 29 eyes (93.5%) UCVA was 6/12
or better; the other three eyes had UCVA of 6/15. The difference between preoperative and 1 week postoperative BSCVA was
not statistically significant (p=0.25). Comparison of BSCVA at 1 week and at 1 month was statistically significant (p=0.05); this difference was even more significant at 3 months (p=0.01). The BSCVA remained the same or improved for all the eyes. BSCVA for 20 eyes (64.5%) had improved one or more lines
in visual acuity. Mean endothelial cell loss at 3 months was 3.96%. Conclusion: The Artisan lens is a safe, predictable, and efficacious method to correct high myopia. Postoperative gain in BSCVA was
achieved for the majority of eyes. Future study to assess safety indexes and risk of complications is required with long-term

PurposeThe purpose of this study was to determine the longitudinal changes in macular retinal and choroidal microvasculature in normal healthy and highly myopic eyes.MethodsSeventy-one eyes, including 32 eyes with high myopia and 39 healthy control eyes, followed for at least 12 months and examined using optical coherence tomography angiography imaging in at least 3 visits, were included in this study. Fovea-centered 6 × 6 mm scans were performed to measure capillary density (CD) of the superficial capillary plexus (SCP), deep capillary plexus (DCP), and choriocapillaris (CC). The rates of CD changes in both groups were estimated using a linear mixed model.ResultsOver a mean 14-month follow-up period, highly myopic eyes exhibited a faster rate of whole image CD (wiCD) loss (−1.44%/year vs. −0.11%/year, P = 0.001) and CD loss in the outer ring of the DCP (−1.67%/year vs. –0.14%/year, P < 0.001) than healthy eyes. In multivariate regression analysis, baseline axial length (AL) was negatively correlated with the rate of wiCD loss (estimate = −0.27, 95% confidence interval [CI] = −0.48 to −0.06, P = 0.012) and CD loss in the outer ring (estimate = −0.33, 95% CI = −0.56 to −0.11, P = 0.005), of the DCP. The CD reduction rates in the SCP and CC were comparable in both groups (all P values > 0.05).ConclusionsThe rate of CD loss in the DCP is significantly faster in highly myopic eyes than in healthy eyes and is related to baseline AL. The CD in the outer ring reduces faster in eyes with longer baseline AL. 相似文献

目的 探讨Kappa角特点及明暗状态下的变化规律。方法 系列病例研究。选取2016年5月至2017年10月于厦门大学附属厦门眼科中心行白内障手术的患者394例(788眼),采用OPD-Scan Ⅲ进行检查,搜集的数据包括患者的临床数据(年龄、性别、眼部病史及手术史),及术前生物测量数据如平均角膜曲率(Mean K)、角膜球差(CSA)、白到白距离(WTW)、明视瞳孔直径(Photopic)、暗视瞳孔直径(Mesopic),明视瞳孔时Kappa角的大小和位移角度(PDist@Angle)、暗视瞳孔时Kappa角的大小和位移角度(MDist@Angle)、暗视瞳孔中心相对于明视瞳孔中心位移的大小和角度(MPDist@Angle)等。数据采用秩和检验、卡方检验、多元回归统计分析处理。结果 双眼各生物测量因素对比中Mean K、MPDist、PDist、WTW、CSA差异无统计学意义(P>0.05),而Photopic、Mesopic、MDist左右眼差异有统计学意义(Z=2.276、3.284、2.388,均P<0.05)。双眼明暗状态下均为正Kappa角占多数;双眼由明瞳向暗瞳转变时瞳孔中心多向颞下方位移。分别将双眼临床、生物测量数据与MPDist进行多元回归分析,右眼回归方程为:MPDist=0.033+0.505×MDist+0.041×CSA-0.319×PDist(F=45.0,P<0.001),左眼回归方程为:MPDist=-0.002+0.547×MDist+0.013×Mesopic-0.350×PDist(F=61.6,P<0.001)。结论 白内障患者正Kappa角占多数;不同明暗度下瞳孔大小的改变会影响Kappa角的大小,而且明暗瞳孔大小及明暗交替时瞳孔中心的大小和位移角度在不同患者间存在较大差异;MPDist的变化与PDist、MDist、Mesopic、CSA存在线性回归关系,其中双眼MPDist均与PDist存在负相关,双眼MDist均对MPDist的变化影响相对较大。 相似文献

PurposeThe purpose of this study was to evaluate the role of the canonical Wnt signaling in the development of the myopia.MethodsPlasma from adult patients with myopia, myopic animal models including the adenomatous polyposis coli (APC) gene mutation mouse model, and the form deprivation (FD) induced mouse model of myopia were used. Niclosamide, a canonical Wnt pathway inhibitor, was orally administrated in animal models. Plasma levels of DKK-1 were determined by using enzyme-linked immunosorbent assay. Refraction, vitreous chamber depth (VCD), axial length (AL), and other parameters, were measured at the end of the FD treatment. Canonical Wnt signaling changes were evaluated by Western blot analysis and immunostaining analysis.ResultsPlasma level of Wnt inhibitor DKK-1 was markedly decreased in patients with myopia. Meanwhile, the canonical Wnt pathway was progressively activated during myopia development in mice. Moreover, inhibition of canonical Wnt signaling by niclosamide in mouse models markedly reduced lens thickness (LT), VCD, and AL elongation, resulting in myopia inhibition.ConclusionsDysregulation of canonical Wnt signaling is a characteristic of myopia and targeting Wnt signaling pathways has potential as a therapeutic strategy for myopia. 相似文献

Purpose: To determine the effect of conventional laser in situ keratomileusis (LASIK) and laser epithelial keratomileusis (LASEK) for myopia on contrast sensitivity (CS) using the Pelli‐Robson and Vector Vision CSV‐1000E CS tests. Methods: A prospective, comparative study was conducted on 36 eyes of 36 patients with myopia undergoing LASIK (18 eyes) and LASEK (18 eyes). Surgery was performed using the Technolas 217z laser (Bausch & Lomb). CS was recorded preoperatively and at 3, 6 and 12 months postoperatively. Results: No statistically significant difference was found in LogMAR uncorrected visual acuity post‐LASIK (?0.02 ± 0.16) and LASEK (?0.04 ± 0.14). Using the Pelli‐Robson, CS was significantly lower in the LASIK group 3 and 6 months postoperatively. No significant postoperative reduction in CS was observed in either treatment group. Using the CSV‐1000E test, CS was significantly reduced post‐LASIK at 3 (p = 0.05) and 6 (p = 0.05) cycles/degree under photopic conditions. No significant postoperative change occurred in the LASEK group under photopic or scotopic conditions. There was no significant difference in postoperative CS between the LASIK and LASEK groups at 3, 6, 12 or 18 cycles/degree using the CSV‐1000E test. Conclusion: One year postoperatively, there was no difference in CS between both treatment groups using the Pelli‐Robson and CSV‐1000E tests. CS was reduced postoperatively in the LASIK group at the lower spatial frequencies under photopic conditions. No postoperative change was detected in CS following LASIK or LASEK using the Pelli‐Robson test. 相似文献

Effectiveness of peripheral defocus spectacle lenses in myopia control: a Meta-analysis and systematic review 下载免费PDF全文
AIM: To evaluate the effectiveness of peripheral defocus spectacle lenses (PDLs) in myopia control.
METHODS: Literature retrieval on PubMed, Cochrane Library, Embase, and Web of Science databases, and the search time limit was from the establishment of each database to December 29, 2021 were conducted. Change of spherical equivalent refraction (SER) and axial change (AL) were extracted from the literatures that met the inclusion criteria, and RevMan5.3 software was used for Meta-analysis.
RESULTS: A total of 4 randomized controlled trials (RCTs) were included in this Meta-analysis, involving 770 myopic children. The results showed that PDLs could delay the progression of myopia in children with myopia compared with single vision spectacle lenses (SVLs; WMD=0.21 D, 95%CI: 0.01, 0.41, P=0.04). However, there was no significant difference in controlling the growth of axial length (AL) in myopic children (WMD=-0.10 mm, 95%CI: -0.21, 0.01, P=0.07). The results of the effectiveness of myopia control between the two spectacle lenses showed that PDLs were more effective in controlling the progression of myopia (OR=5.73, 95%CI: 2.58, 12.70, P<0.001) and delaying the growth of AL (OR=44.25, 95%CI: 8.84, 221.58, P<0.001) than SVLs, and the differences were statistically significant.
CONCLUSION: PDLs can control the progression of myopia compared with SVLs, but cannot delay the growth of AL, and the effectiveness of PDLs in myopia control better than SVLs. 相似文献

目的:探讨在现代白内障囊外摘除的条件下,高度近视白内障囊外摘除术后视网膜脱离的发生率及其危险因素。方法:对1993年7月至1994年1月在我院施行白内障囊外摘除术及白内障囊外摘除并后房型人工晶体植入的高度近视及老年性白内障的病人按病历序号进行登记随访,排除随访条件不良的病例,随访时间为27~33个月,观察术后视网膜脱离的发生率。结果:高度近视组视网膜脱离的发生率为1.11%(1/90),老年性白内障组无视网膜脱离发生(0/373),差别无统计学意义(P>0.05)。高度近视组中Nd:YAG激光后囊膜切开术的使用率高于老年性白内障组(P<0.005)。结论:保留完整后囊膜的白内障囊外摘除术使高度近视白内障术后视网膜脱离的发生率下降,术后发生视网膜脱离的危险因素是:年龄较轻、眼轴长及继之而施行的后囊膜切开术。避免与减少后发性白内障的形成以及术前、术后的眼底检查与治疗是预防术后视网膜脱离的有效措施。眼科学报1997;13:90~92。 相似文献

目的 以临床资料验证SRK-T公式和SRK-Ⅱ公式计算植入人工晶状体度数的准确性.方法 (1)对43例(43只眼)高度近视(眼轴长度≥26.0mm)白内障患者施行超声乳化联合人工晶状体(IOL)植入术,将病例按眼轴长度分为二组,A组(25眼):眼轴长度<29.0mm;B组(18眼):眼轴长度≥29.0mm.(2)比较A、B两组,分别采用SRK-T和SRK-Ⅱ公式计算的平均绝对预测屈光误差值.(3)比较A、B两组,分别采用SRK-T和SRK-Ⅱ公式计算的IOL度数之间的差值幅度.结果 (1)A组按SRK-T和SRK-Ⅱ公式计算其平均绝对预测屈光误差值分别为(1.33±0.67)D和(1.40±0.59)D,差异无统计学意义(P>0.05);B组按SRK-T和SRK-Ⅱ公式计算其平均绝对预测屈光误差值分别为(0.84±0.65)D和(1.42±0.87)D,差异有统计学意义(P<0.05).(2)A组采用两公式分别计算的IOL度数相差≤±0.5D者为84%;B组采用两公式分别计算的IOL度数相差≥±2.0D者为61.11%.结论 A组SRK-T公式和SRK-Ⅱ公式都适用;B组SRK-T公式更适用,其结果的准确性优于SRK-Ⅱ公式. 相似文献

PurposeTo identify key retinal pigment epithelium (RPE) genes linked to the induction of myopia in guinea pigs.MethodsTo induce myopia, two-week-old pigmented guinea pigs (New Zealand strain, n = 5) wore −10 diopter (D) rigid gas-permeable contact lenses (CLs), for one day; fellow eyes were left without CLs and served as controls. Spherical equivalent refractive errors (SE) and axial length (AL) were measured at baseline and one day after initiation of CL wear. RNA sequencing was applied to RPE collected from both treated and fellow (control) eyes after one day of CL-wear to identify related gene expression changes. Additional RPE-RNA samples from treated and fellow eyes were subjected to quantitative real-time PCR (qRT-PCR) analysis for validation purposes.ResultsThe CLs induced myopia. The change from baseline values in SE was significantly different (P = 0.016), whereas there was no significant difference in the change in AL (P = 0.10). RNA sequencing revealed significant interocular differences in the expression in RPE of 13 genes: eight genes were significantly upregulated in treated eyes relative to their fellows, and five genes, including bone morphogenetic protein 2 (Bmp2), were significantly downregulated. The latter result was also confirmed by qRT-PCR. Additional analysis of differentially expressed genes revealed significant enrichment for bone morphogenetic protein (BMP) and TGF-β signaling pathways.ConclusionsThe results of this RPE gene expression study provide further supporting evidence for an important role of BMP2 in eye growth regulation, here from a guinea pig myopia model. 相似文献

目的:分析中央孔型有晶状体眼后房型人工晶状体(ICL)植入矫正中低度近视的短期有效性、安全性和稳定性。方法:回顾性队列研究。连续纳入2020年7—12月在成都爱尔眼科医院植入V4c ICL晶状体的低度近视患者490例(707眼),均完成术后6个月随访,术前及术后1、3、6个月测量裸眼视力、矫正视力、眼压、拱高和角膜内皮细胞计数,并计算手术安全性、有效性、预测性以及稳定性。术前、术后数据采用配对t检验进行统计分析。结果:随访至术后6个月,患者裸眼视力(-0.076±0.079)LogMAR,手术有效性指数为1.26±0.21,最佳矫正视力(-0.079±0.080)LogMAR,安全指数为1.13±0.18。术后屈光度在±0.5 D以内的占比94.9%,可预测性为99.1%;等效球镜度在术后1个月为(-0.063±0.135)D,术后6个月为(-0.071±0.126)D;角膜内皮细胞计数术前(3 078±258)个/mm2,术后6个月为(2 953±206)个/mm2,损失率为(1.05±0.39)%。术后1个月拱高为(498±143)μm,术后6个月为(474±177)μm。ICL更换比例为1.4%。术后未出现严重威胁视力的并发症。结论:ICL 植入术(中央孔型,V4c ICL)矫正中低度近视具备良好的效果,手术安全、可靠,且预测性高,患者视力稳定。 相似文献

AIM: To investigate the impact of multifocal gas permeable contact lens (MFGPCL) in various add power and distance/near area allocation on short-term changes of choroidal thickness (ChT), axial length (AL), and retinal defocus profile in young adults.
METHODS: Seventeen young adults (2 males and 15 females; age 23.17±4.48y) were randomly assigned to wear two designs binocularly with a one-week washout period in between. Total of four MFGPCL designs were assessed. All designs were distance-center that varied in two add power (+1.50 and 3.00 D) and/or two distance zone (DZ) diameters (1.50 and 3.00 mm; design A: DZ 1.5/add 3.0, B: DZ 1.5/add 1.5, C: DZ 3.0/add 3.0, D: DZ 3.0/add 1.5). ChT, AL, and peripheral refraction data were collected on each subject at baseline, on days 1 and 7 of MFGPCL daily wear. ChT was assessed in four quadrants using a spectral-domain optical coherence tomography.
RESULTS: AL was shortened by -26±44 µm with lens C, -18±27 µm with lens D, -13±29 µm with lens A, and -8±30 µm with lens B (all P<0.05). A significant overall increase in ChT was observed with all 4 designs (lens A: +6±6 µm, B: +3±7 µm, C: +8±7 µm, and D: +8±7 µm). Temporal and superior choroid exhibited more choroidal thickening associated with MFGPCL. All designs induced significant relative peripheral myopia (RPM) beyond the central 20o across the horizontal meridian in both nasal and temporal fields (P<0.05).
CONCLUSION: MFGPCLs show a significant influence on ChT and AL, which are associated with significant increase in RPM after short-term wear. The reliability and feasibility of quantifying short-term changes in ChT support its use as a promising marker for the long-term efficacy of myopia-controlling treatments. 相似文献
