目的评价抗环瓜氨酸肽(CCP)抗体和葡萄糖-6-磷酸-异构酶(GPI)抗原对类风湿关节炎(RA)的诊断价值。方法用酶联免疫吸附试验(ELISA)法分别测定RA患者112例、非RA其他风湿病患者44例以及健康志愿者119名血清中的抗CCP抗体、GPI抗原和IgM—RF,应用ROC比较抗CCP抗体和GPI抗原对RA的诊断价值,用Spearman方法分析上述指标与lgM—RF的相关性。结果在诊断RA时.抗CCP抗体和GPI抗原的ROC曲线下面积均大于IgM—RF(P均〈0.05)。抗CCP抗体、GPI抗原与IgM—RF显著相关。CCP抗体与GPI抗原的联合试验的灵敏度达到0.946。结论联合测定抗CCP抗体和GPI抗原对RA具有较高的诊断价值,具有良好的应用前景。  相似文献

目的 探讨抗Sa抗体、抗环瓜氨酸肽(CCP)抗体、葡萄糖6-磷酸异构酶(GPI)抗原及类风湿因子(RF)联合检测对类风湿关节炎(RA)诊断的意义.方法 采用酶联免疫吸附试验(ELISA)检测抗Sa抗体、抗CCP抗体及RF亚型RF-IgA、RF-IgG、RF-IgM,免疫比浊法检测RF.结果 抗Sa抗体、抗CCP抗体、GPI抗原、RF对RA诊断的特异性和敏感性分别为95.69%和59.79%、98.28%和90.72%、76.14%和78.35%、67.24%和91.75%.RF-IgA、RF-IgG、RF-IgM对于RA诊断的特异性和敏感性分别为89.66%和58.76%、90.52%和68.04%、79.31%和79.38%.两项指标联合检测对RA诊断的特异性和敏感性为92.24% ~ 100.00%和47.42%~86.60%;3项指标联合检测对RA诊断的特异性和敏感性为99.14%~100%和46.39%~71.13%;4项指标联合检测对RA诊断的特异性和敏感性为100.00%和46.39%.抗Sa抗体阳性组患者C反应蛋白水平高于阴性组(P<0.05).结论 两项及两项以上指标联合检测可以显著提高RA诊断的特异性,有利于RA的早期诊断及治疗.  相似文献

目的 了解抗突变型瓜氨酸波形蛋白(MCV)抗体、葡萄糖-6-磷酸异构酶(GPI)、抗环瓜氨酸多肽(CCP)抗体检测在类风湿关节炎(RA)诊断中的价值.方法 检测109例RA患者,24例其他风湿病患者及19名健康对照血清中抗MCV抗体、GPI和抗CCP抗体,比较3种抗体单独或联合检测对诊断RA的敏感性和特异性.结果 RA组抗MCV抗体和GPI阳性率及平均浓度显著高于其他风湿病组及健康对照组(P<0.05).抗CCP抗体与GPI阳性率在RA患者中的差异有统计学意义.抗MCV抗体对RA敏感性最高(99.1%);抗CCP抗体对RA特异性最高(90.7%);联合检测以MCV+CCP、MCV+GPI以及MCV+CCP+GPI特异件最高(均为98.1%).结论 单独或联合检测抗MCV抗体、抗CCP抗体和GPI,有助于提高RA诊断的敏感性和特异性.  相似文献

葡萄糖-6-磷酸异构酶抗原对类风湿关节炎诊断的意义   总被引:3,自引:0,他引:3  
目的 研究葡萄糖-6-磷酸异构酶(G6PI)抗原在类风湿关节炎(RA)中的诊断意义.方法 用酶联免疫吸附试验(ELISA)双抗体夹心法检测106例RA患者,53例其他风湿性疾病和35名健康志愿者血清中的G6PI抗原的分布.还同时检测了类风湿因子(IgM-RF)、抗环瓜氨酸肽(CCP)抗体、抗角蛋白抗体(AKA)、C反应蛋白(CRP)、红细胞沉降率(ESR).并对98例RA患者行手足x线片,将其按修正的Sharp评分,分为有骨侵蚀组与无骨侵蚀组.结果 ①RA患者血清中G6PI浓度显著高于其他风湿病组和健康对照组(P<0.05).检测G6PI对RA的诊断价值,采用受试者工作曲线(ROC),诊断RA的最佳临界值为0.225μg/ml,此时的灵敏性为0.868,特异性为0.853.②G6PI与IgM-RF之间具有相关性(rs=0.504,P<0.05),G6PI与抗CCP抗体之间未显示出相关性(rs=0.078,P=0.503).③通过Spearman相关性分析.未发现G6PI抗原与病情活动性指标如ESR、CRP等有相关性.④经单因素分析G6PI抗原与骨侵蚀无关(P<0.05),IgM-RF与骨侵蚀具有相关性(P<0.05).结论 G6PI抗原在部分RA患者血清中显著升高,抗G6PI抗原对RA具有良好的敏感性(86.8%)和特异性(85.3%),能作为RA患者的一种新的标志物.G6PI抗原与IgM-RF有相关性,但不完全重叠.此研究未发现G6PI与病情活动性指标及关节侵蚀之间有关联.  相似文献

目的:探讨血清中葡萄糖-6磷酸异构酶(G6PI)对类风湿性关节炎(RA)诊断及与其他自身免疫性疾病鉴别诊断中的价值。方法:采用酶联免疫吸附试验(ELISA)检测137例RA患者、141例其他风湿病患者和60例健康对照血清中GPI抗原的浓度,RA患者还检测了抗环胍氨酸肽抗体(抗CCP抗体)及类风湿因子(RF)的浓度。结果:137例RA患者血清中GPI浓度为(3.01±1.33)mg/L,141例其他风湿性疾病组为(0.27±0.16)mg/L,60例健康对照组为(0.07±0.03)mg/L。RA患者血清中GPI浓度显著高于其他风湿病组和健康对照组(P〈0.01);在RA活动组和RA非活动组中亦差异有统计学意义(P〈0.01)。GPI抗原对RA诊断的敏感性为62.8%,特异性为95.7%。3个指标联合检测的特异性达100%。结论:GPI在部分RA患者血清中显著升高,有可能成为诊断RA及判断其疾病活动性的一个新指标。  相似文献

目的 检测葡萄糖-6-磷酸异构酶(GPI)在类风湿关节炎(RA)活动期膝关节滑液中的含量,并探讨滑液中GPI与抗环瓜氨酸肽(CCP)抗体的关系.方法 用酶联免疫吸附法(ELISA)检测22例RA活动期患者和37例骨关节炎活动期患者滑液中GPI和抗CCP抗体的浓度.组间比较采用t检验,相关性分析采用Spearman相关分析.结果 RA活动期患者膝关节滑液中的GPI浓度高于骨关节炎活动期患者[(9.6±8.4)和(0.9±1.8)μg/ml],差异有统计学意义(P<0.01);RA活动期患者与骨关节炎活动期患者滑液的抗CCP抗体浓度分别为(14.61±18.64)和(1.42:±0.09) U/ml,差异有统计学意义(P<0.01);RA活动期患者膝关节滑液中GPI含量和抗CCP抗体浓度呈正相关(r=0.447,P=0.037).结论 GPI在RA活动期患者滑液中高表达,可能与RA发生发展过程中慢性滑膜炎、骨质破坏有关.GPI与抗CCP抗体呈正相关,两者可作为RA实验室诊断的参考指标.  相似文献

目的探讨血清葡萄糖-6-磷酸异构酶(GPI)对类风湿关节炎(RA)的诊断价值。方法对RA患者(RA组)、其他自身免疫性疾病(其他AID组)、正常体检者(正常对照组)分别采用ELISA法测定血清GPI水平,并检测类风湿因子(RF—IgM、RF—IgG、RF-IgA)、抗环瓜氨酸肽(CCP)抗体,RA组同时检测C反应蛋白、血沉、免疫球蛋白、补体和血小板。结果RA组的GPI水平明显高于另外两组,且GPI水平在早期RA和非早期、活动期与非活动期之间均无统计学差异(P均〉0.05);当GPI为0.23mg/L时,诊断准确性最高;CPI与疼痛关节数、RF-IgM、RF-IgA相关(P〈0.05);GPI与抗CCP抗体、RF—IgM和RF-IgA的敏感度相似,与RF-IgM的特异度相似。结论GPI可作为RA诊断的实验室指标之一,其与抗CCP抗体、RF—IgA联合应用对RF—IgM阴性患者在诊断上可起到互补作用。  相似文献

目的探讨4种抗瓜氨酸化蛋白,肽抗体(anti-citrufllinated protein/peptide autoantibodies,ACPAs)-抗核周因子(anti-perinuclear fator,APF)、抗角蛋白抗体(anti-keratin antibody,AKA)、抗角蛋白微丝聚集蛋白抗体(antiprofillagrin/fillagrin antibody,AFA)、第二代抗环瓜氨酸肽抗体(second generation of anti-cylic citrulline peptide antibody,CCPⅡ)对类风湿关节炎(rheumatoid arthritis,RA)的诊断意义及其结果的一致性。方法 对129例RA、153例其他风湿性疾病患者和94名健康对照者的血清进行类风湿因子(rheumatoid factor,RF)和ACPAs检测。RF采用乳胶间接凝集法检测。APF和AKA采用间接免疫荧光法检测,AFA、抗CCP(Ⅱ)抗体采用酶联免疫吸附法检测。结果APF、AKA、AFA和抗CCP(Ⅱ)抗体对RA的诊断特异性分别为90.3%、94.7%、93.5%和94.7%,敏感性分别为58.1%、40.3%、62.0%和67.4%;而RF的诊断特异性和敏感性为77.7%和46.5%。APF与AFA、APF与抗CCP(Ⅱ)和AFA与抗CCP(Ⅱ)抗体间检测结果一致性较好,Kappa值分别是0.596、0.572和0.576。结论ACPAs是RA的诊断特异性抗体,其中抗CCP(Ⅱ)抗体的敏感性和特异性最高,且与APF和AFA抗体间结果具有明显的一致性。  相似文献

目的比较第1代与第2代抗环瓜氨酸肽(anti-cyclic citrutlinated pepfide,CCP1,CCP2)抗体对类风湿关节炎(rheumatoid arthritis,RA)的诊断价值。方法对129份RA、153份其他风湿性疾病和94份健康对照血清进行检测。类风湿因子(rheumatoid factor,RF)检测采用乳胶间接凝集法,抗CCP1和抗CCP2抗体检测采用酶联免疫吸附法。结果对RA的诊断敏感性,抗CCP2抗体高于RF和抗CCP1抗体,分别为67.4%、46.5%和46.5%,差异有显著性意义(P=0.001);抗CCP1和抗CCP2抗体对RA的诊断特异性均为94.7%,而RF的诊断特异性仅为77.7%,差异亦有显著性意义(P〈0.001)。结论抗CCP2抗体对RA诊断有较好的敏感性和较高的特异性。  相似文献

目的探讨葡萄糖6磷酸异构酶(GPI)在老年起病的类风湿关节炎(EORA)患者血清中的临床意义,探讨其与EORA病情活动性之间的相关性。方法对36例EORA患者及48例健康老年人的血清进行了GPI的测定,并在EORA患者中对GPI与病情活动性之间的关系进行了分析。结果 36例EORA患者中,GPI阳性率为75%(27/36)。48例健康老年人中,仅有5例检测到GPI,GPI阳性率为10.4%(5/48)。GPI在EORA患者中的敏感性及特异性分别为75%及82.7%。以28处关节疾病活动度积分(DAS28)来评价EORA患者的病情活动性,在EORA患者中,GPI阳性组患者的病情活动性显著高于GPI阴性组患者(P〈0.05)。结论 GPI有助于EORA的诊断及病情活动性判断。  相似文献

Glucose-6-phosphate dehydrogenase (G6PD) is an essential enzyme in the pentose phosphate pathway that prevents oxidative damage to cells. This study determined the genotypic and allelic frequencies of G6PD G202A and A376G and also investigated correlation between G6PD polymorphisms and hemoglobin (Hb) phenotypes in children in Lagos, Nigeria. Seventy-eight children [55 with Hb AA (βΑA) and 23 with Hb AS (βΑS) trait] and 65 Hb SS (βSS) (HBB: c.20A>T) subjects in steady state with age range between 5–15 years were recruited for the study. Hemoglobin phenotypes of all study participants were carried out using alkaline electrophoresis and solubility tests. Genomic DNA was extracted from whole blood and restriction fragment length polymorphism-polymerase chain reaction (RFLP-PCR) was used to determine the G202A and the A376G mutations of the G6PD gene. The genotype and allele distributions of G6PD G202A and A376G according to the Hb phenotypes were not statistically significant (p?>?0.05). The minor allele frequency 202A was 0.15 (15.0%) and 0.14 (14.0%) in cases and controls, respectively. The overall frequency of 376G allele in the case group was 0.35 (35.0%) and 0.38 (38.0%) in the control group. No statistical significance was observed in the genotype and allele distributions of A376G in both the case and control groups (p?>?0.05). The G6PD A? frequency in Hb SS subjects and the control group were 6.2 and 2.6%, respectively. G6PD G202A and A376G polymorphisms were not associated with Hb phenotypes and the allele distributions of 202A and 376G in this study are typical of West African populations.  相似文献

Glucose-6-phosphate dehydrogenase (G-6-PD) deficient erythrocytes are particularly sensitive to oxidant stress. In order to evaluate if these cells are protected against oxidant damage, we assayed the antioxidant enzymes superoxide dismutase (SOD), catalase and glutathione peroxidase (GSH-Px) in erythrocytes of G-6-PD deficient (hemizygous and heterozygous) subjects. Normal levels of antioxidant enzymes were found in all subjects examined both with positive and negative histories of haemolytic crisis after fava bean or drug ingestion. In contrast, high levels of catalase and GSH-Px were found in a small group of G-6-PD deficient subjects (hemizygous and heterozygous) with β-thalassaemia trait, probably by reason of the chronically enhanced oxidant stress which is present in β-thalassaemia.  相似文献

目的探讨联合检测抗环瓜氨酸肽抗体(anti-CCP)与类风湿因子(RF)对类风湿关节炎(RA)早期诊断的意义。方法选取2008年1月—2009年1月收治确诊RA患者中随机抽取100人作为RA组,以健康人50例作为对照组,采用酶联免疫吸附法(ELISA)检测抗CCP抗体,采用速率散射比浊法检测RF。结果RA组的anti-CCP阳性率为67%,RF的阳性率为78%。对比分析发现RA患者血清中的anti-CCP对RA诊断的灵敏度比RF的灵敏度低,差异具有显著性(P〈0.05);而anti-CCP的96.0%的特异性比RF的88.0%要高,差异具有显著性(P〈0.05)。两者联合检测灵敏度可提高,特异性可达100.0%。结论RF与anti-CCP联合使用可进一步提高RA诊断的灵敏度和特异性,有利于对RA的早期检出。  相似文献

目的通过检测类风湿关节炎(RA)患者血清中白细胞介素(IL)-33水平,分析血清IL-33水平与RA的临床及实验室指标之间的相关性,验证IL-33参与RA的发病过程,探讨IL-33对于RA的临床意义。方法采用ELISA方法测定70例RA患者和30例健康对照者血清IL-33水平。结果 RA组血清IL-33水平为(231±335)pg/ml,高于健康对照组(6±23)pg/ml,P<0.01。高疾病活动度组血清IL-33水平为(351±391)pg/ml,高于中低疾病活动度组(145±266)pg/ml,P<0.05。RA患者血清IL-33水平与类风湿因子(RF)及抗环瓜氨酸肽(CCP)抗体呈正相关(P<0.01)。结论 IL-33参与了RA的发病过程,IL-33的升高可能提示RA预后不良。  相似文献

Thalassemias and glucose-6-phosphate dehydrogenase (G6PD) deficiency are the most common inherited blood disorders. They are distributed among populations living in malaria endemic regions resulting in survival advantage from severe malaria disease. The aims of this study were to analyze the prevalence of thalassemias and G6PD deficiency at the Ramathibodi Hospital, Bangkok, Thailand. A total of 616 adult and 174?cord blood samples were collected and analyzed for red blood cell (RBC) parameters, hemoglobin (Hb) typing and DNA analysis for G6PD mutations and α-thalassemia (α-thal). The two most prominent types of thalassemia were heterozygous Hb E (HBB: c.79G>A), (19.5% in newborns and 35.6% in adults) followed by heterozygous α-thal-2 [–α3.7 (rightward) deletion] at 18.7% in newborns and 19.5% in adults. After performing G6PD genotyping using multiplex amplification refractory mutation system-polymerase chain reaction (multiplex ARMS-PCR) for 10 G6PD mutations, the prevalence of G6PD mutation was found in 12.0% of newborns and 11.7% of adults. The G6PD Viangchan [871 (G>A)] is the most common G6PD mutation in newborns (42.9%) and adults (52.8%). In addition, coinheritance of various types of thalassemia with G6PD deficiency were found. The results indicated that heterozygous Hb E and G6PD Viangchan are predominant both in newborns and adults in this study.  相似文献
