A case of personation.

A case from the 19th century is described. The patient was admitted to hospital as a man but was revealed to be a woman. The woman's history is examined and the psychological implications of her impersonation are discussed. A hypothetical diagnosis of transsexualism is made.     

Electrocardiographical case. A case of wide complex tachycardia

A 43-year-old Chinese woman complained of a one week history of irregular rapid palpitations associated with chest discomfort and dyspnoea. Her heart rate was more than 160 beats per minute and blood pressure was 154/95 mmHg. 12-lead electrocardiogram (ECG) showed a wide complex tachycardia with a slightly irregular rhythm. Delta waves were also present. She was treated appropriately with intravenous procainamide but developed torsades de pointes secondary to prolonged QT interval. Electrophysiology study revealed atrial tachycardia with a left-sided accessory pathway which was successfully ablated.     

A case of pulsating proptosis.

The long-term follow-up of a case of unilateral pulsating proptosis is reported. The clinical signs in this patient were due to a defect in the bony wall of the orbit which is an unusual manifestation of neurofibromatosis.     

A case of resolving idiopathic hypoparathyroidism.

A 72 year old man developed idiopathic hypoparathyroidism. The condition spontaneously resolved which, to our knowledge, has not been previously described. Causes of fluctuating hypocalcaemia are outlined and their possible relevance to our patient discussed.
