目的:探讨西比灵的临床疗效和脑电地形图的应用价值。方法:应用西比灵治疗300例缺血性血管性头晕患,通过临床症征和脑电地形图观察其疗效。结果:西比灵治疗缺血性血客性头晕有效率为100.0%,脑电地形图在治疗前后有明显改变。结论:西比灵是治疗缺血必血管性头晕的理想药物,脑电地形图是观察西比灵疗效和作用机理的有效指标。  相似文献

Objective:To observe clinical effect of combined mixture of traditional chineses and western medicine " PIAN TAN NING" in treatment of ischemic cerebrovascular diseases and change of brain atlas. Methods: In 6660 patients with ischemic cerebrovascular disease, the curative effect of "PIAN TAN NING" and brain atlas were observed and other 30 cases were compared with treatment of Dextran. Results: After treatment,patient's clinical symptoms and recovered degree of myodynamia and brain atlas improved significantly. Total effective rate was 96.7%. But the contral group was 80.0%. There was a significant difference between them ( p < 0. 05). Conclusion: The " PIAN TAN NING" produced a good clinical curative effect in treatment of ischemic cerebrovascular diseases and the side effect was only a few. So it can be applied very well in patients with ischemic cerebrovascular disease. The applied value of brain atlas was very good.  相似文献

目的观察中西药配伍合剂"偏瘫宁"治疗缺血性脑血管病的临床疗效及脑电地形图改变.方法对6660例缺血性脑血管病患者进行了"偏瘫宁"疗效及脑电地形图观察,并与采用脉通治疗的30例结果对比.结果临床症状和肌力恢复程度及脑电地形图治疗后有明显改善,总有效率为96.7%,而对照组脉通治疗的有效率为80.0%,两组对比,有显著性差异(P<0.05).结论"偏瘫宁"对缺血性脑血管病有较好的临床疗效,无毒副作用,可推广应用.脑电地形图有很好应用价值.  相似文献

西比灵治疗椎基底动脉供血不足的研究   总被引:7,自引:0,他引:7  
目的 :研究椎基底动脉供血不足 (VBI)所致的眩晕、共济失调、耳鸣、无力等 ,用西比灵治疗前后血液流变性、经颅多普勒 (TCD)及甲襞微循环的变化和症状改善情况。方法 :76例VBI患者随机分为二组 ,以维脑路通作对照 ,常规用药 ,治疗前后检查上述指标。结果 :西比灵治疗后血浆粘度、红细胞压积和红细胞聚集指数显著下降 ,TCD检查血流速度加快 ,频谱频窗异常状况有明显好转 ,甲襞微循环流速流态、管袢清晰度及临床症状显著改善 (P <0 .0 5~ 0 .0 1 )。结论 :西比灵能扩张血管 ,降低血粘度 ,改善微循环 ,治疗VBI。  相似文献

我们于1990~1992年将脑电地形图应用于儿科临床,并对其实用价值进行了研究探讨。本组102例,其年龄为新生儿期~14岁。疾病包括:癫痫27例,血管性头痛15例,病毒性脑炎14例,新生儿缺氧缺血性脑病14例,精神发育迟缓9例,脑外伤后遗症4例,脑发育不全6例,其他13例。对上述疾病的脑电地形图表现作了分析,并对其中67例同时作脑CT,37例同时作脑电图者的结果进行了对比。从而提出脑电地形图可客观地反映,直观地显示患者各种病变脑电活动变化,其不仅可提示病变的性质而且可确定病变的部位。因此在儿科有重要应用价值。  相似文献

背景:缺血缺氧性脑损伤后的神经再生、神经功能的恢复与缺血部位新生血管的形成和重塑有着密切的关系。血管内皮祖细胞参与出生后缺血组织的血管新生及修复,促使血液循环再通和氧气等营养物质的供应,为神经功能的恢复提供微环境。 目的:探讨自体血管内皮祖细胞治疗缺血缺氧性脑损伤的可行性、有效性及安全性,探索改善脑损伤患者神经功能修复的新方法。 方法:应用计算机检索PubMed、ScienceDirect、Springerlink、CNKI、万方等数据库近10年的相关文献。英文关键词为“EPCs、endothelial progenitor cell、stroke”等,中文关键词为“内皮祖细胞、干细胞移植、脑卒中”等,选择内容与血管内皮祖细胞治疗缺血缺氧性疾病相关的文献,同一领域文献则选近期发表的或发表在权威杂志上的文章,共纳入43篇参考文献。 结果与结论:脑缺血后神经再生、神经功能的恢复与缺血部位新生血管的形成和重塑有着密切的关系,内皮祖细胞参与出生后缺血组织的血管发生及修复,促进血液循环及氧气等营养物质的供应,为神经功能的恢复提供微环境。自体血管内皮祖细胞治疗缺血缺氧性脑损伤是可行的、安全的、有效的,但仍需大量的生物学及动物实验为其临床应用提供客观的理论依据。  相似文献

目的 探讨胫前动脉踝上支降支蒂胫骨远端骨瓣移位治疗距骨缺血性坏死的临床效果。方法 2010年3月~2013年12月收治8例距骨创伤性缺血坏死患者,均采用胫前动脉踝上支降支蒂胫骨远端骨瓣移位治疗,术后石膏托固定踝关节4周。 结果 随访6个月~24个月,平均12个月。8例患者术后均获满意愈合,最终随访检查踝关节功能按Kenwight疗效标准评估,其中优6例,良2例。 结论 该骨瓣血管恒定, 血供丰富,手术操作简便,是治疗距骨缺血坏死的较理想方法。  相似文献

Objectives To explore clinical application on brain atlas in paediatrics. Methode: Brainatlas was applied in diagnosis and treatment of paediatric diseases and its clinical value was discussed in 1990~2001. The manifestation of these diseases in brain atlas were analysed and the manifestation of CT of 67 cases andmanifestations of EEG of 37 cases with that of BA were compared. Results The changes of cerebral electrical ac-tivity of these diseases were reflected objectively and showed directly in BA. Conclusion Brain atlas not only canpoint out quality of disease but also define position of disease. Therefore, brain atlas has important clinical valuein paediatrics.  相似文献

目的 对我院缺血性脑血管疾病患者应用丁基苯酞软胶囊进行治疗,了解该药应用过程中的具体疗效。方法 2016年2月~2017年2月我院共收治缺血性脑血管疾病患者60例,随机将患者分为观察组与对照组,每组30例。对照组按照常规的治疗方式应用长春西汀注射液治疗,观察组患者在对照组患者治疗的基础上增加应用丁基苯酞软胶囊治疗,比较两组患者临床疗效。结果 治疗一个疗程结束后,观察组有效率76.67%高于对照组50.00%,差异有统计学意义(P<0.05);治疗结束后,观察组平均NIHSS评分(6.52±1.03)分,对照组平均NIHSS评分(7.97±1.43)分,观察组患者NIHSS评分较低,两组患者结果存在差异,有统计学意义(P<0.05)。结论 在临床治疗缺血性脑血管疾病时,与传统的治疗药物相比较,应用丁基苯酞软胶囊具有显著的疗效且具有一定的安全性,能够保护脑神经,减轻因血小板集中而造成的缺血性脑血管疾病症状。另外,该药物应用后用药患者均未发生不良反应,值得在临床治疗中推广应用。  相似文献

目的:探讨脑功能障碍治疗仪—仿真生物电刺激在缺血性脑血管病中的应用。方法:利用仿真生物电刺激对50例缺血性脑血管病合并偏瘫的患者进行治疗,同时选择50例相匹配的患者进行对比分析。结果:治疗组疗效明显高于对照组(P<0.05),治疗组与对照组神经功能缺损程度比较有统计学意义(P<0.05),治疗组明显优于对照组。结论:仿真生物电刺激有助于缺血性脑血管病患者运动功能的提高。  相似文献

目的 探讨阿司匹林肠溶片治疗中老年缺血性脑血管疾病的临床效果及其安全性。方法 选取我院2017年4月~2018年4月收治的缺血性脑血管疾病的中老年患者130例,随机分为对照组和观察组,每组65例。对照组采用普通阿司匹林片治疗,观察组采取阿司匹林肠溶片治疗,比较两组患者治疗总有效率、治疗中不良反应发生率和治疗依从率。结果 观察组治疗总有效率为90.77%,治疗中不良反应的发生率为7.69%,治疗依从性率为96.92%;对照组治疗总有效率为84.62%,治疗中不良反应的发生率为13.85%,治疗依从率为84.62%。观察组的各项指标均优于对照组,差异具有统计学意义(P<0.05)。结论 阿司匹林肠溶片治疗缺血性脑血管疾病较普通阿司匹林片具有明显优势,疗效显著,服用剂量小,毒副反应少。  相似文献

背景:胚胎干细胞移植是否能够成为脑血管疾病治疗有效的方法已成为目前研究的热点。 目的:探讨胚胎干细胞分化神经前体细胞移植治疗脑血管疾病的效果及可行性。 方法:分别建立帕金森病、缺血性脑损伤以及血管性痴呆大鼠模型,并进行胚胎干细胞体外培养,诱导分化为神经前体细胞,将胚胎干细胞分化神经前体细胞移植入相应脑血管疾病模型大鼠脑内,观察脑血管病变大鼠的旋转行为学变化、脑组织病理变化以及海马结构的变化和神经细胞数目的变化。 结果与结论:胚胎干细胞分化神经前体细胞移植入帕金森病大鼠脑内后,阿朴吗啡诱发的旋转次数逐渐减少,呈下降趋势,纹状体多巴胺的含量明显增高。胚胎干细胞分化神经前体细胞移植入缺血性脑损伤大鼠脑内后,能够长期存在于脑内,并迁移、分布于受损的海马,构成海马结构,进一步分化为神经元,并且受损海马内的神经细胞数量明显增加。说明移植的胶质细胞源性神经营养因子基因修饰的胚胎干细胞可改善血管性痴呆大鼠的学习记忆功能,增强神经的可塑性,诱导自身定向迁移并分化为成熟神经元。  相似文献

Functional disorders of the brain, such as Parkinson's disease, dystonia, epilepsy, and neuropathic pain, may exhibit poor response to medical therapy. In such cases, surgical intervention may become necessary. Modern surgical approaches to such disorders include radio-frequency lesioning and deep brain stimulation (DBS). The subthalamic nucleus (STN) is one of the most useful stereotactic targets available: STN DBS is known to induce substantial improvement in patients with end-stage Parkinson's disease. Other targets include the Globus Pallidus pars interna (GPi) for dystonia and Parkinson's disease, and the centromedian nucleus of the thalamus (CMN) for neuropathic pain. Radiosurgery is an attractive noninvasive alternative to treat some functional brain disorders. The main technical limitation to radiosurgery is that the target can be selected only on the basis of magnetic resonance anatomy without electrophysiological confirmation. The aim of this work is to provide a method for the correct atlas-based identification of the target to be used in functional neurosurgery treatment planning. The coordinates of STN, CMN, and GPi were identified in the Talairach and Tournoux atlas and transformed to the corresponding regions of the Montreal Neurological Institute (MNI) electronic atlas. Binary masks describing the target nuclei were created. The MNI electronic atlas was deformed onto the patient magnetic resonance imaging-T1 scan by applying an affine transformation followed by a local nonrigid registration. The first transformation was based on normalized cross correlation and the second on optimization of a two-part objective function consisting of similarity criteria and weighted regularization. The obtained deformation field was then applied to the target masks. The minimum distance between the surface of an implanted electrode and the surface of the deformed mask was calculated. The validation of the method consisted of comparing the electrode-mask distance to the clinical outcome of the treatments in ten cases of bilateral DBS implants. Electrode placement may have an effect within a radius of stimulation equal to 2 mm, therefore the registration process is considered successful if error is less than 2 mm. The registrations of the MNI atlas onto the patient space succeeded in all cases. The comparison of the distance to the clinical outcome revealed good agreement: where the distance was high (at least in one implant), the clinical outcome was poor; where there was a close correlation between the structures, clinical outcome revealed an improvement of the pathological condition. In conclusion, the proposed method seems to provide a useful tool for the identification of the target nuclei for functional radiosurgery. Also, the method is applicable to other types of functional treatment.  相似文献

BACKGROUND AND PURPOSE: The present study was designed to detect the abnormalities of the cerebral grey-matter density in subcortical ischemic vascular dementia patients by FSL-VBM method to promote the early diagnosis of it.METHODS: Nine subcortical ischemic vascular dementia patients and nine age-matched normal controls underwent MRI brain structure scanning that was performed on a SIEMENS AVANTO 1.5 Tesla scanner and standard T1-weighted high-resolution anatomic scans of MPRAGE sequence were obtained. The 3-demensional MPRAGE images were processed with FSL-VBM package and the cerebral gray matter density was compared between the subcortical ischemic vascular dementia patients and normal controls.RESULTS: Compared with the normal control group, the cerebral gray matter density of subcortical ischemic vascular dementia patients was found significantly decreasing, including brain regions of thalamus, parietal lobe, frontal lobe and temporal lobe (P<0.05).CONCLUSIONS: The cerebral gray matter density alterations have closed correlation with cognitive dysfunction in subcortical ischemic vascular dementia patient and can be detected by MRI. MRI has some potential value in the diagnosis of them.  相似文献

背景:目前对腰椎间盘突出症患者的疗效判定仍主要依靠患者的自觉症状和物理检查,尚缺乏客观评价手段。 目的:应用远红外热像技术评估射频热凝靶点消融术结合臭氧治疗腰椎间盘突出症的临床疗效。 方法:选择46例符合条件的腰椎间盘突出症患者,随机分为射频治疗组23例和保守治疗组23例,分别予以射频热凝靶点消融术结合臭氧治疗和传统保守治疗,治疗前后使用远红外热像仪检测双下肢温度差,并与临床改善情况进行对比分析。 结果与结论:射频治疗组与保守治疗组的显效率分别为52%和17%,有效率分别为96%和65%,两组差异均有显著性意义(P < 0.05);射频治疗组治疗后热图温差明显低于保守治疗组(P=0.00);射频治疗组治疗前后温差改变比保守治疗组明显增大(P=0.00)。结果可见红外热成像技术可作为射频热凝靶点消融术结合臭氧治疗腰椎间盘突出症疗效的一种客观评估指标。   相似文献

目的 研究分析早期颅骨修补治疗脑外伤的临床疗效。方法 选择2016年1月~2017年1月于我院脑外科就诊治疗的脑外伤患者108例,随机分为研究组和参考组两组,每组54例。研究组进行早期颅骨修补术,参考组进行晚期颅骨修补术。分析对比两组患者的治疗效果以及手术前后的神经功能缺损程度和日常生活能力评分情况。结果 研究组患者的整体治疗有效率(94.44%)显著高于参考组(66.67%),差异有统计学意义(P<0.05);研究组患者术后神经功能缺损程度评分为(14.3±3.6)分,低于参考组的(19.2±4.5)分,研究组患者术后日常生活能力评分为(74.2±12.7)分,高于参考组的(53.4±12.9)分,两组患者相比,差异有统计学意义(P<0.05)。结论 早期颅骨修补治疗脑外伤比晚期颅骨修补术的治疗效果更为良好,同时能够有效提高患者的神经功能与日常生活能力,值得临床推广与应用。  相似文献
