PURPOSE: To evaluate the effect of astigmatic correction on the accuracy of the myopic and astigmatic correction in patients having photorefractive astigmatic keratectomy (PARK) and in those having photorefractive keratectomy (PRK). SETTING: Specialist excimer laser refractive clinic. METHODS: This prospective consecutive case series comprised 6097 eyes with a preoperative mean spherical equivalent (MSE) of -4.63 diopters (D) +/- 1.95 (SD) (range -0.75 to -13.00 D) and a mean cylinder of -1.13 +/- 0.73 D (range -0.50 to -6.00 D) having PARK with a Nidek EC-5000 excimer laser. Visual and refractive outcomes were assessed 12 months postoperatively and compared with those in 3004 eyes that had spherical PRK. RESULTS: At 12 months, the MSE was -0.02 +/- 0.79 D and the mean cylinder was -0.49 +/- 0.47 D in the PARK group; the MSE was -0.07 +/- 0.66 D in the PRK group. An MSE within +/-0.05 D of emmetropia was achieved by 69.8% and within +/-1.00 D, by 87.9%. The uncorrected visual acuity (UCVA) was 20/20 or better in 42.6% and 20/40 or better in 91.2%. Statistical significance (P <.001, analysis of variance) was achieved for MSE, sphere, cylinder, haze, and visual acuity (best corrected [BCVA] and UCVA) based on the preoperative cylinder. The loss of BCVA varied from 1.1% to 5.8% depending on the degree of astigmatism treated. Accuracy varied with the attempted myopic correction and the attempted astigmatic correction. CONCLUSIONS: Excimer laser PARK was an effective treatment for compound myopic astigmatism, but predictability decreased and complications increased as the attempted astigmatic correction increased.  相似文献

PURPOSE: The aim of this study was to compare the results of excimer laser photorefractive keratectomy (PRK) in patients who underwent PRK using the Summit Apex (Omnimed) excimer laser in one eye and the Nidek EC-5000 excimer laser in the other. METHODS: All consecutive patients who underwent PRK with the Summit Apex laser (Omnimed) in one eye and the Nidek laser (EC-5000) in the second and had at least 12 months of follow-up were included in this retrospective study (n=30). Uncorrected and spectacle-corrected visual acuity, final spherical equivalent refraction, and grade of subepithelial haze were compared. The average preoperative spherical equivalent refraction of eyes treated with the Summit laser was -6.00 D (range, -2.50 to -8.75 D), and for Nidek-treated eyes it was -5.57 D (range, -2.50 to -8.80 D). RESULTS: Forty-seven percent of Summit-treated eyes and 53% of Nidek-treated eyes had uncorrected visual acuity of 6/6 or better; 61% of Summit-treated eyes and 63% of Nidek-treated eyes had uncorrected visual acuity of 6/7.5 or better; 95% of Summit-treated eyes and 95% of Nidek-treated eyes had uncorrected visual acuity of 6/12 or better (difference not statistically significant). Seventy-three percent of eyes treated with the Summit laser and 80% of eyes treated with the Nidek laser had a postoperative refraction within +/-0.50 D of emmetropia; 97% of Summit-treated eyes and 87% of Nidek-treated eyes had a postoperative spherical equivalent refraction within +/-1.00 D of emmetropia; the difference between the two lasers was not statistically significant. However, the percent of eyes with persistent hyperopia was smaller in the Nidek group after 3 months (P=.0062) and after 6 months (P=.07) than in the Summit group. Videokeratography was not done. CONCLUSION: Both lasers were effective with relatively low side effects. No significant difference was found between the two lasers in postoperative uncorrected visual acuity or refractive outcome. Eyes operated with the Nidek laser had less persistent hyperopia and stabilized earlier.  相似文献

PURPOSE: To evaluate the visual and refractive results of laser in situ keratomileusis (LASIK) for mixed and simple myopic astigmatism using bitoric ablation. METHODS: A retrospective study was performed in 65 eyes of 38 consecutive patients to evaluate uncorrected (UCVA) and best spectacle-corrected visual acuity (BSCVA) and cyclopegic and manifest refraction, before and 3 and 6 months after LASIK (Moria LSK-ONE microkeratome, Nidek EC-5000 excimer laser). RESULTS: At 3 and 6 months after LASIK, 40 eyes of 24 patients (64.5%) were available for follow-up examination. Mean age was 25.9 +/- 6.6 years (range 18 to 43 yr). Mean preoperative manifest spherical equivalent refraction was -1.40 +/- 0.80 D (range -3.80 to +0.50 D) mean preoperative cylinder was -3.30 +/- 1.30 D (range -1.00 to -6.00 D). At 6 months follow-up, mean manifest spherical equivalent refraction was +0.30 +/- 0.46 D (range -0.38 to +1.88 D), mean cylinder was -0.73 +/- 0.61 D (range -2.25 to 0 D). There was a 77.8% decrease in astigmatism magnitude. According to vector analysis, mean achieved vector magnitude was 80% of intended. Fifty percent (20 eyes) had a cylinder within +/- 0.50 D of emmetropia. Twenty-three eyes (57.5%) had a spherical component within +/- 0.50 D. Eighty-five percent (34 eyes) had postoperative UCVA of 20/40 or better. Ten percent (four eyes) lost two lines of Snellen BSCVA, whereas 35% (14 eyes) gained one or more lines. CONCLUSIONS: Bitoric LASIK with the Moria LSK-ONE microkeratome and Nidek EC-5000 excimer laser was effective and safe for the reduction of cylinder in mixed and simple myopic astigmatism. Moderate undercorrection of the cylinder was evident.  相似文献

PURPOSE: To compare the refractive results of eyes with low to moderate myopia that underwent laser in situ keratomileusis with either the Nidek EC-5000 or the Alcon LADARVision 4000 excimer laser systems. METHODS: We performed a retrospective review of 114 LASIK procedures with either the Nidek EC-5000 (54 eyes) or the Alcon LADARVision 4000 (60 eyes) excimer laser systems. Preoperative refractive errors were similar and both populations were treated during the same time period. Data analyzed included uncorrected visual acuity (UCVA), spherical magnitude, spherical equivalent refraction, astigmatism power, astigmatism axis, and vector astigmatism change. RESULTS: Results at 6 months were analyzed. Spherical correction change was a mean -3.95 D for Nidek treated eyes and a mean -4.53 D for LADARVision treated eyes (P = .20). Mean spherical equivalent refraction change was -3.70 D for Nidek eyes and -4.20 D for LADARVision eyes (P = .23). Mean change in UCVA (LogMAR) was 1.05 for Nidek eyes and 0.99 for LADARVision eyes (P = .40). Mean astigmatism magnitude change was 0.71 D for Nidek eyes and 0.77 D for LADARVision eyes (P = .63). Mean vector-corrected astigmatism change was 0.93 D for Nidek eyes and 1.00 D for LADARVision eyes (P = .63). Mean vector-corrected astigmatism axis for Nidek eyes was 3.08 D and for LADARVision eyes 6.58 D (P = .70). CONCLUSION: There was no significant difference in refractive results in eyes treated with the Alcon LADARVision 4000 or the Nidek EC-5000 excimer laser systems. Inherent differences between the two laser systems are highlighted.  相似文献

PURPOSE: To evaluate the results of laser in situ keratomileusis (LASIK) for the correction of low and moderate myopia and astigmatism using the Nidek EC-5000 excimer laser with a modified algorithm. METHODS: Three hundred and ninety-six eyes of 257 patients with myopia ranging from -1.50 to -6.00 D and astigmatism of no more than 4.00 D were included. Manifest spherical equivalent refraction, uncorrected and spectacle-corrected visual acuity were assessed at 1 month (357 eyes, 90%), 3 months (220 eyes, 56%), and 6 months (168 eyes, 42%) after LASIK. RESULTS: Mean preoperative spherical equivalent refraction was -3.60 +/- 1.27 D and mean cylinder was -1.01 +/- 1.08 D. Mean postoperative spherical equivalent refraction was -0.12 +/- 0.31 D and mean cylinder was -0.19 +/- 0.33 D. At 6 months follow-up, 158 eyes (94%) had a mean postoperative spherical equivalent refraction within +/- 0.50 D of emmetropia; 168 eyes (100%) were within +/- 1.00 D. Uncorrected visual acuity was 20/20 or better in 81% (136 eyes) and 20/40 or better in 100%. Two eyes (1.2%) lost 1 line of spectacle-corrected visual acuity due to a decentered ablation. No eye lost 2 or more lines of spectacle-corrected visual acuity. CONCLUSION: Our modified LASIK algorithm with the Nidek EC-5000 excimer laser resulted in safe and predictable outcomes for treatment of low and moderate amounts of myopia and astigmatism.  相似文献

PURPOSE: To analyze long-term results of photorefractive keratectomy (PRK) for myopia and myopic astigmatism. METHODS: This retrospective study included 15 eyes of 8 patients who were examined annually for 5 years after PRK. The Nidek EC-5000 laser with an ablation zone of 5.0 mm was used. Evaluations included spherical equivalent manifest refraction, uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), corneal haze, and corneal astigmatism calculated by Fourier analysis of videokeratography data. RESULTS: A tendency toward myopic regression was most evident within the first postoperative year, with manifest refraction changing from +0.80 +/- 1.62 D at 1 week to -0.45 +/- 0.70 D at 1 year postoperatively (P = .007). Regression continued after the second postoperative year. There was a statistically significant difference between manifest refraction at 2 years (-0.36 +/- 0.75 D) and 5 years (-1.11 +/- 1.12 D) (P = .002). Postoperative UCVA stabilized from 3 months up to 3 years, but slightly deteriorated at 4 years and thereafter due to the myopic refractive shift. BSCVA remained stable throughout the 5-year follow-up period. Several eyes developed mild corneal haze after surgery, but haze was minimal in the majority of patients by 1 year and continued to fade over time. The asymmetry component of the cornea significantly increased after surgery, with all postoperative values significantly higher than before PRK (P < .05). Higher order irregularity increased after surgery, with a statistically significant difference between preoperative and 1 year postoperative (P < .05), but values after 2 years were not different from preoperative baseline. CONCLUSION: Fifteen eyes with a baseline refraction of -3.00 to -9.00 D had PRK with the Nidek EC-5000 laser and a 5-mm-diameter ablation zone. Myopic regression occurred in the first year, with continued mild regression of approximately -0.75 D between 2 and 5 years. Nevertheless, the results show the procedure was relatively safe and effective in this group.  相似文献

PURPOSE: To evaluate the safety and efficacy of photorefractive keratectomy for the treatment of primary compound myopic astigmatism. METHODS: In a prospective study, 93 eyes from 56 patients with a mean spherical equivalent of -4.98 +/- 1.80 diopters (range, -1.75 to -8.5) underwent photoastigmatic refractive keratectomy with the Summit Apex Plus excimer laser using erodible mask technology and were followed for 2 years. Primary outcome measures included an assessment of astigmatic correction through vector analysis, manifest refraction, uncorrected visual acuity, corneal clarity, and the presence of adverse symptoms.RESULTS: Eighty-five eyes (91.4%) were available for analysis at 6 months. Mean spherical equivalent refraction was reduced 85% (mean, -0.75 +/- 0.85 diopter) and the target-induced astigmatism was reduced 70% (mean, 0.98 +/- 1.88 diopters). Forty-eight eyes (56%) had an uncorrected visual acuity of 20/20 or greater, whereas 70 eyes (82%) had an uncorrected visual acuity of 20/40 or greater. Twenty-four eyes (26% ) required re-treatment because of undercorrection of the spherical equivalent and astigmatic components after the 6-month follow-up. Fifty-nine of the remaining eyes were available at the 24-month visit. Mean spherical equivalent refraction was reduced to -0.39 +/- 0.72 diopter (91.8%). The target-induced astigmatism was reduced 64% from 1.74 diopters. Forty-one eyes (81.3%) were within +/-1.0 diopter of attempted spherical equivalent correction. Stability within a spherical equivalent of +/-0.5 diopter occurred after the first postoperative month. Fifty-six eyes (94.9%) had an uncorrected visual acuity of 20/40 or greater, whereas 34 eyes (57.6 %) demonstrated an uncorrected visual acuity of 20/20 or greater. One eye (1.7%) lost 2 or more lines of best spectacle-corrected visual acuity.CONCLUSION: Photoastigmatic refractive keratectomy with the Summit Apex Plus excimer laser is a safe and effective method of reducing compound myopic astigmatism. However, higher re-treatment rates may result from significant undercorrections because of current laser algorithms and variability in the mean angle of error.  相似文献

OBJECTIVE: To study photorefractive keratectomy (PRK) or laser in situ keratomileusis (LASIK) after clear lens extraction (CLE) with intraocular lens (IOL) implantation for hyperopia or astigmatism. DESIGN: Retrospective, noncomparative interventional case series. PARTICIPANTS: Sixty-five eyes (55 subjects) had CLE with posterior chamber IOL implants for hyperopia up to 12.25 diopters (D); 31 eyes were retreated with PRK, and 34 eyes were retreated with LASIK for residual ametropias. INTERVENTION: For PRK and LASIK, the refractive surgery was performed with the slit-scanning excimer laser Nidek EC-5000, Nidek Co., Tokyo, Japan. MAIN OUTCOME MEASURES: Manifest refraction, best-spectacle and uncorrected Snellen visual acuity, haze, and halos were evaluated before surgery and at 1, 3, 6, and 12 months postoperative. RESULTS: Forty-seven eyes were evaluated at the 12-month postoperative examination: 96% of these eyes had spherical equivalents (SE) within +/-2 D of emmetropia, 79% of eyes had SE within +/-1 D of emmetropia and 51% of eyes had SE within +/-0.50 D of emmetropia. Eighty-five percent of the eyes at 12 months postoperative had uncorrected visual acuity of 20/40 or better, and 46% of eyes had uncorrected visual acuity of 20/20 or better. Eighty-seven percent of the eyes at 12 months postoperative had uncorrected visual acuity within 1 Snellen line of their initial best spectacle-corrected visual acuity (BSCVA) before all treatment. No eye lost 2 Snellen lines of BSCVA at 3, 6, or 12 months after PRK or after LASIK. CONCLUSIONS: IOL implantation for CLE, although an invasive technique, resulted in better refractive outcomes without laser-related clinical complications after PRK or LASIK adjustment.  相似文献

PURPOSE: To evaluate the safety and efficacy of laser-assisted subepithelial keratectomy (LASEK) for the treatment of low to high myopia and astigmatism. SETTING: Solo private practice, Mountain View, California, USA. METHODS: Laser-assisted subepithelial keratectomy was performed in 146 eyes of 83 consecutive patients with myopia or myopic astigmatism using a VISX Star S2 excimer laser (72 eyes) or a Nidek EC-5000 excimer laser (74 eyes). The mean preoperative myopic spherical equivalent was -5.32 diopters (D) (range -1.25 to -14.38 D). Data were collected prospectively with a follow-up of 1 to 12 months. Outcome measurements included uncorrected visual acuity (UCVA), manifest refraction, best spectacle-corrected visual acuity (BSCVA), corneal haze, and complications. RESULTS: After 6 and 12 months, no eye lost 2 or more lines of BSCVA. After 6 months, the UCVA was 20/20 in 57% of eyes and 20/40 or better in 96%. After 12 months, it was 20/20 in 56% of eyes and 20/40 or better in 96%. No eye developed corneal haze that affected visual acuity. There were no serious or vision-threatening complications. CONCLUSIONS: Laser-assisted subepithelial keratectomy was safe and effective in treating a wide range of myopia and astigmatism. The potential advantages of LASEK over laser in situ keratomileusis (LASIK) include the elimination of stromal flap complications and greater choice in patient selection. The disadvantages include varying degrees of pain for 2 days and blurry vision for several days postoperatively.  相似文献

PURPOSE: We studied the efficacy, predictability, and safety of laser in situ keratomileusis (LASIK) for moderate to high simple and compound myopic astigmatism. METHODS: Ninety-two eyes of 46 consecutive patients who had LASIK for myopic astigmatism (64 eyes, astigmatism 3.00 to 9.00 D; myopia 0 to -20.00 D), or simple myopia (28 eyes, myopia -4.00 to -20.00 D; astigmatism 0 to 0.50 D) were retrospectively studied. Mean baseline spherical equivalent refraction (SE) in the myopia group was -8.11 +/- 3.94 D and in the astigmatism group, -8.55 +/- 4.49 D. All eyes underwent LASIK using the Nidek EC-5000 laser by the same surgeon. RESULTS: At 6 months after LASIK in the myopia group versus the astigmatism group, 24 eyes (85%) vs. 54 eyes (84%) were available for follow-up, 12 eyes (50%) vs. 13 eyes (24%) had uncorrected visual acuity (UCVA) of 20/20, 19 eyes (79%) vs. 44 eyes (81%) had UCVA of 20/40, 8 eyes (33%) vs. 18 eyes (33%) had SE within +/- 0.50 D, 15 eyes (62%) vs. 39 eyes (72%) had SE within +/- 1.00 D, and mean SE was -1.22 +/- 1.17 D vs. -0.74 +/- 1.46 D. Mean astigmatism (vertexed to the corneal plane) in the astigmatism group was 2.77 D at 0 degrees before surgery and 0.32 D at 7 degrees at 6 months. None of the myopic eyes and three of the astigmatic eyes (5%) lost > or = 2 lines of best spectacle-corrected visual acuity. CONCLUSION: LASIK with the Nidek EC-5000 laser for myopic astigmatism was reasonably effective, predictable, and safe. Astigmatism was under-corrected with the nomogram implemented in this study.  相似文献

PURPOSE: To assess the safety, efficacy, predictability and stability of photorefractive keratectomy in compound myopic astigmatism with a moderate and high cylinder component. METHODS: Photorefractive keratectomy was done in 42 eyes with compound myopic astigmatism with the spherocylindrical algorithm of the MEL-70 excimer laser, with wide ablation zones. RESULTS: Spherical equivalent refraction changed from -4.19 +/- 1.65D to -0.05 +/- 0.31D, refractive cylinder from -2.01 +/- 0.71D to -0.09 +/- 0.20D and mean sphere from -3.22 +/- 1.76D to -0.02 +/- 0.26D. Mean uncorrected visual acuity rose from 0.12 +/- 0.17 to 0.91 +/- 0.10. No eye lost lines of spectacle-corrected visual acuity. The safety index was 1.03 and the efficacy index 0.98. Six months from the treatment all eyes were within +/- 1D, 8.9% of eyes were within 0.50D and 44% were plano of target refraction. Refractive and topographical stability were achieved between one and three months after treatment. Transient haze was observed between one and three months after PRK. CONCLUSIONS: Photorefractive keratectomy with the MEL-70 excimer laser to correct myopic astigmatism was a safe and effective procedure with good stability at six months' follow-up. Refractive and visual outcome confirmed that excellent predictability can be expected.  相似文献

PURPOSE: To prospectively evaluate the safety, efficacy, and visual performance of excimer laser enhancement following photoastigmatic refractive keratectomy (PARK) with the Summit Apex Plus. SETTING: Stanford University School of Medicine Eye Laser Clinic, Stanford, California, USA. METHODS: As part of a Food and Drug Administration clinical trial, 93 eyes of 56 patients with a mean spherical equivalent (SE) of -4.98 diopters (D) +/- 1.80 (SD) (range -1.75 to -8.50 D) had PARK for compound myopic astigmatism using the Summit Apex Plus excimer laser and a poly(methyl methacrylate) erodible mask. Seventeen eyes with a mean SE of -2.08 +/- 0.76 D required excimer laser refractive keratectomy for residual spherical myopia or compound myopic astigmatism. Patients were prospectively followed 1, 3, 6, 9, and 12+ months after the enhancement procedure. Primary outcome variables included uncorrected visual acuity (UCVA), refraction, vector analysis, best spectacle-corrected visual acuity (BSCVA) under standard ambient conditions (photopic, scotopic, and glare), corneal clarity, and contrast sensitivity function curve under photopic and scotopic conditions. RESULTS: At the last postoperative visit, the mean sphere had been corrected 82% to a residual of -0.29 +/- 1.23 D and mean SE had been corrected 65% to a residual of -0.74 +/- 1.27 D. Eighty-two percent of eyes were within +/-1.0 D of attempted correction. Eighty-eight percent had a UCVA of 20/40 or better. Vector analysis demonstrated a difference vector of within +/-1.0 D in 75% of eyes that had PARK retreatment. There was no significant loss in the contrast sensitivity curve. Late regression associated with corneal haze and loss of BSCVA occurred in 2 eyes (11.7%). CONCLUSIONS: Retreatment following PARK for compound myopic astigmatism results in effective reduction in residual spherical myopia and compound myopic astigmatism. An improvement in UCVA without loss of contrast sensitivity can be expected in most eyes. However, regression, corneal haze, and loss of BSCVA may occur. Further studies are indicated to predict risk factors for these complications.  相似文献

Prospective follow-up is analyzed at 1-year postoperatively. MAIN OUTCOME MEASURES: Postoperative reduction in myopic sphere, myopic astigmatism, spherical equivalent, angle of error, magnitude of error, difference vector, uncorrected visual acuity, and corneal haze. RESULTS: Seventy-one eyes were available at 1-year follow-up. Mean spherical equivalent was reduced 86.5% to a mean residual of -0. 65 D +/-0.70 D (-2.88-+1.13 D). Astigmatism was reduced 66.2% to a mean residual of -0.76 +/- 0.52 D (-2.25-0.00 D); 77.5% were within 1.0 D of attempted correction; 1.4% of eyes experienced an overcorrection >1.0 D of spherical equivalent; 0.27 D of mean myopic regression was demonstrated between 1 and 12 months; 93.0% of eyes achieved an uncorrected visual acuity of 20/40 or better; and 49.3% of eyes achieved an uncorrected visual acuity of 20/20 or better. CONCLUSIONS: PARK with the Summit Apex Plus excimer laser is effective at reducing compound myopic astigmatism. However, current laser algorithms result in consistent undercorrection of both the compound myopia and the astigmatic component.  相似文献

BACKGROUND: Photorefractive keratectomy (PRK) is an accepted procedure to correct myopia in Germany. The purpose of this study was to evaluate photoastigmatic refractive keratectomy (PARK) for myopic astigmatism. MATERIAL AND METHODS: PARK using the VISX 20/20 excimer laser was performed in 50 eyes of 36 patients (25 female, 16 male) aged 22 to 68 years (mean 38 +/- 10 years) in the period of 1995 to 1996. Retrospective evaluation was done after dividing the patients into three astigmatic groups between -0.5 diopters (D) and -1.5 D (group I, n = 28), between -1.75 D and -3.0 D (group II, n = 16) und between -3.25 D and -5.5 D (group III, n = 6). The correction of myopic astigmatism was assessed concerning the criteria safety, efficacy, predictability and stability. Mean follow-up was 12 months (8-15 months). RESULTS: Seventy-eight % of all eyes achieved an uncorrected visual acuity of > 0.5. Sixty-eight % of the eyes were within +/- 1.0 D of the desired correction. Loss of 2 or more lines of best corrected visual acuity occurred in 6 eyes (12.0%). We observed in group I an astigmatic reduction of 36.0%, in group II of 66.5% and in group III of 68.0%. The surgically induced astigmatism was calculated with 1.29 +/- 1.0 D. Mean axis rotation of the refractive cylinder was 22.12 +/- 23.05 degrees. CONCLUSIONS: PARK using the VISX 20/20 excimer laser is an acceptable procedure to correct myopic astigmatism. However, the study revealed that astigmatic reduction with a broad beam laser is not yet perfect in terms of efficacy, predictability and safety.  相似文献

PURPOSE: To report the results of laser in situ keratomileusis (LASIK) in a series of adult patients with amblyopia. METHODS: A retrospective noncomparative review was performed on patients with amblyopia who underwent LASIK for correction of ametropia, using the Summit Krumeich Barraquer microkeratome and the Nidek EC-5000 excimer laser. Data were collected on uncorrected visual acuity, best spectacle-corrected visual acuity, manifest refraction, anterior segment evaluation, intraocular pressure, corneal topography, and dilated fundus examination (preoperative and postoperatively on day 1, months 2 and 6). RESULTS: Eight eyes of seven patients were included, with a mean patient age of 30 +/- 10 years (range 21 to 49 yr). Mean preoperative spherical equivalent refraction was -4.70 D (range -12.62 to +4.71 D) and the best spectacle-corrected visual acuity varied from 20/32 to 20/80. At 2 months after LASIK, mean spherical equivalent refraction was -0.37 +/- 0.60 D (range -1.37 to + 0.60 D), uncorrected visual acuity ranged between 20/20 to 20/30, and a mean gain of 3 Snellen lines (range 2 to 4 lines) was observed. All patients reported significant subjective improvement in their perception of vision. The visual acuity and subjective improvement were maintained throughout 6 months postoperatively. CONCLUSIONS: LASIK with the Nidek EC-5000 laser for correction of ametropia in adult amblyopic eyes provided encouraging results for visual acuity improvement beyond correction of the refractive error.  相似文献

PURPOSE: To assess the efficacy, predictability, and safety of LASIK for the surgical correction of low to moderate hyperopia and hyperopic astigmatism using the NIDEK EC-5000 excimer laser. METHODS: In a multicenter United States Food and Drug Administration (FDA) regulated study of LASIK, 7 centers enrolled 293 eyes with manifest refraction sphere that ranged from +0.50 to +6.00 diopters (D) with or without astigmatism up to 3.00 D. The intended outcome was plano in all eyes. Patients were treated bilaterally. One year postoperative outcomes are reported. RESULTS: The mean spherical equivalent refraction (MRSE) for all eyes changed from +2.61 +/- 1.19 D (range: +0.50 to +6.63 D) preoperatively to +0.35 +/- 0.54 D (range: -1.63 to +2.00 D) 1 year postoperatively. Overall, 61% (170/279) of eyes achieved distance uncorrected visual acuity (UCVA) of 20/20 or better, 82% (228/279) of eyes saw 20/25 or better, and 99% (277/279) of eyes saw 20/40 or better. Refractive accuracy was demonstrated as 63.1% (176/279) of eyes achieved a MRSE within +/- 0.50 D and 90.3% (252/279) of eyes within +/- 1.00 D. Less than 2% (4/279) of eyes lost 2 lines of distance best spectacle-corrected visual acuity. Stability of refraction was demonstrated by 6 months, with a mean hyperopic shift of < 0.03 D from 3 to 6 months. CONCLUSIONS: The NIDEK EC-5000 corrected hyperopia and hyperopic astigmatism with UCVA, refractive accuracy, 1-year stability, and safety that surpassed all FDA criteria.  相似文献

PURPOSE: To evaluate and compare the efficacy, safety, predictability, and surgically induced astigmatism (SIA) of photorefractive keratectomy (PRK) and photoastigmatic refractive keratectomy (PARK). SETTING: Department of Ophthalmology, National Taiwan University Hospital, Taipei, Taiwan. METHODS: In this retrospective study, 70 eyes were treated for myopia and 70 eyes were treated for myopic astigmatism. Refraction, corneal topography, slitlamp findings, and visual acuity in the 2 groups at 1, 3, and 6 months were evaluated and compared. Vector analysis was performed to determine the SIA in both groups. RESULTS: The mean preoperative spherical equivalent at the glasses plane in the PRK and PARK groups was -6.06 diopters (D) and -7.18 D, respectively. At 6 months, the mean reduction in astigmatism in the PARK group was 61.0%. Predictability was within +/-1.0 D in 85.2% of eyes in the PRK group and 62.5% in the PARK group. An uncorrected visual acuity of 20/40 or better was achieved in 91.8% and 83.9% of eyes, respectively. The mean SIA was 0.64 D in the PRK group, with a general with-the-rule axis shift. The results of vector analysis were more favorable when calculated from refractive values than from Sim-K corneal topography values. The mean astigmatism correction index and index of success calculated from refractive data were 0.75 and 0.38 in the PARK group. The mean magnitude and angle of error were 0.22 +/- 0.52 D and -2.13 +/- 24.41 degrees, respectively. CONCLUSIONS: Photorefractive keratectomy and PARK were effective and safe procedures for the correction of myopia and myopic astigmatism. However, SIA occurred with spherical myopic treatments. This small SIA may be a confounding factor in low astigmatic treatments.  相似文献

PURPOSE: We evaluated the efficacy, predictability, stability, and safety of laser in situ keratomileusis (LASIK) for hyperopia and hyperopic astigmatism. METHODS: A retrospective study was performed for 92 eyes of 62 consecutive patients to evaluate uncorrected (UCVA) and best spectacle-corrected visual acuity (BSCVA) and manifest refraction before and 3 and 6 months after LASIK (Moria LSK-ONE microkeratome, Nidek EC-5000 excimer laser). Eyes were divided into groups: Group 1 (low hyperopia) for spherical correction of +1.00 to +3.00 D (22 eyes), Group 2 (low hyperopic astigmatism) for toric correction with spherical equivalent refraction of +1.00 to +3.00 D (18 eyes), Group 3 (moderate hyperopia) for spherical correction of +3.25 to +6.00 D (10 eyes), and Group 4 (moderate hyperopic astigmatism) for toric correction with spherical equivalent refraction between +3.25 and +6.00 D (18 eyes). RESULTS: At 3 and 6 months after LASIK, 68 eyes (73.9%) were available for follow-up examination. Percentage of eyes with a spherical equivalent refraction within +/-0.50 D of emmetropia for Group 1 was 54.5% (12 eyes); Group 2, 50% (9 eyes); Group 3, 40% (4 eyes), and Group 4, 38.8% (7 eyes). UCVA > or =20/20 in Group 1 was 14% and in Groups 2, 3, and 4, 0%. One eye (5.5%) lost two lines of BSCVA. CONCLUSION: LASIK with the Moria LSK-ONE microkeratome and the Nidek EC-5000 excimer laser reduced low and moderate hyperopia and was within +/-0.50 D of target outcome in approximately 50% of eyes. Undercorrection was evident in all groups. The procedure was safe.  相似文献

PURPOSE: To evaluate the efficacy, predictability, and safety of excimer laser photoastigmatic refractive keratectomy (PARK) to correct compound myopic astigmatism. SETTING: Departments of Ophthalmology, Robert Debré Hospital and Rothschild Foundation, Paris, France. METHODS: This retrospective study included 27 eyes with compound myopic astigmatism treated with a Nidek EC 5000 excimer laser. The refractive results were measured at 1 year, and the cylindrical component was analyzed by the Alpins method. Mean preoperative myopia was -4.50 diopters (D) (range -0.75 to -4.00 D) and mean preoperative cylinder, -1.64 D (range -0.75 to -4.00 D). RESULTS: At 1 year, the spherical equivalent was -0.47 D (range +1.00 to -3.00 D) and residual subjective astigmatism, -0.40 (range -0.25 to -1.50 D). Uncorrected visual acuity of 20/40 or better was obtained in 22 of the 27 eyes; 21 eyes were within +/- 1.0 D of emmetropia. Vector analysis showed a mean coefficient adjustment of 1.50 D +/- 0.53 (SD), a mean axis shift of 2.64 +/- 12.10 degrees, and a mean magnitude of error of 0.45 +/- 0.56 D. Haze was absent in 22 eyes and grade 1+ in 5 eyes. Five eyes gained 1 line of best corrected visual acuity and 3 lost 1 line. No patient lost more than 1 line. CONCLUSION: Excimer laser PARK successfully corrected low and moderate myopia combined with up to 4.0 D of astigmatism with a low mean angle of error. To increase the accuracy of toric ablation, specific algorithms for the cylinder component are needed.  相似文献

BACKGROUND AND OBJECTIVE: To determine the efficacy, safety, and predictability of excimer laser photorefractive keratectomy of high myopia and myopic astigmatism. PATIENTS AND METHODS: 76 eyes of 52 patients with myopia from -8.00 to -23.50 diopters (D) with or without astigmatism up to -5.50D were treated with the VISX 20/20 excimer laser (VISK, Santa Clara, CA) and a multi-zone ablation technique. Visual acuity, manifest refraction, corneal haze, and topography were evaluated at 1 week and 1, 3, 6, 12, and 18 months postoperatively. RESULTS: Postoperative refractions were generally stable after 12 months. At the last follow-up all patients were within - 1.96 D of the intended correction. Eighteen months postoperatively, 68% of patients undergoing photorefractive keratectomy (PRK), and 65% of patients undergoing photo astigmatic refractive keratectomy (PARK), were within 1 D of planned refraction. Furthermore, 87% of patients after PRK and 80% of patients after PARK had a visual acuity of 20/40 or better. CONCLUSIONS: High myopia with or without astigmatism was successfully treated in most of the patients using PRK. The stability of the postoperative refraction during the first 18 months seems to be good. The incidence of adverse effects was low but improvements in the future should further reduce complications, thus increasing the safety of refractive procedures.  相似文献
