Black men who have sex with men (MSM) are disproportionately affected with HIV in the US. Limited event-specific data have been reported in Black MSM to help understand factors associated with increased risk of infection. Cross-sectional National HIV Behavioral Surveillance Study data from 503 MSM who reported ≥1 male sexual partner in the past year in New York City (NYC) were analyzed. Case-crossover analysis compared last protected and last unprotected anal intercourse (UAI). A total of 503 MSM were enrolled. Among 349 tested for HIV, 18% were positive. Black MSM (N = 117) were more likely to test HIV positive and not know their HIV-positive status than other racial/ethnic groups. Case-crossover analysis of 208 MSM found that men were more likely to engage in protected anal intercourse with a first time partner and with a partner of unknown HIV status. Although Black MSM were more likely to have Black male partners, they were not more likely to have UAI with those partners or to have a partner aged >40 years. In conclusion, HIV prevalence was high among Black MSM in NYC, as was lack of awareness of HIV-positive status. Having a sexual partner of same race/ethnicity or older age was not associated with having UAI among Black MSM.  相似文献

[目的]了解男男性行为者(men who have sex with men,MSM)人群社会网络及性行为特征,为MSM人群干预提供参考。[方法]采用统一设计的问卷进行面对面调查,并用描述性、卡方检验等方法进行分析。[结果]151名被调查对象拥有128个社交网络;网络大小为2~6个节点,其中≥4个节点的占32.93%;网络密度为0.40~1.00,中位数为1.00;社会网络成员中,男性占85.86%,≤30岁者占73.73%,大学及以上文化程度者占49.93%,被调查对象与社交对象年龄、文化程度的构成差异无统计学意义。297名社交对象中,属于朋友或同学者占61.62%,同事占15.15%,性伴占12.12%,亲属占11.11%;56.23%是MSM;24.58%曾与被调查对象发生性行为,其中76.71%在过去6个月发生过性关系。被调查对象首次与男性发生性行为的年龄为13~40岁,平均(22.73±5.14)岁;在过去6个月中,男性伴个数为1~101个,中位数为2个;60.93%有2个以上男性伴,与固定男性伴、多次男性伴、偶遇男性伴、商业男性伴、异性性伴不安全性行为发生率分别为51.65%、46.05%、36.84%、75.00%和53.33%。[结论]MSM人群多性伴、无保护性行为普遍,社会网络是艾滋病危险行为扩散的重要影响因素,应根据各地区网络社交的特点,开展针对性艾滋病防治。  相似文献

This study evaluates associations between internalized homonegativity and demographic factors, drug use behaviors, sexual risk behaviors, and HIV status among men who have sex with men (MSM) and with men and women (MSM/W). Participants were recruited in Los Angeles County using respondent-driven sampling (RDS) and completed the Internalized Homonegativity Inventory (IHNI) and questionnaires on demographic and behavioral factors. Biological samples were tested for HIV and for recent cocaine, methamphetamine, and heroin use. The 722 MSM and MSM/W participants were predominantly African American (44%) and Hispanic (28%), unemployed (82%), homeless (50%), and HIV positive (48%) who used drugs in the past 6 months (79.5%). Total and Personal Homonegativity, Gay Affirmation, and Morality of Homosexuality IHNI scores were significantly higher for African American men than for other ethnicities, for MSM/W than for MSM, for recent cocaine users than for recent methamphetamine users, and for HIV-seronegative men than for HIV-seropositive men. Linear regression showed the Gay Affirmation scale significantly and inversely correlated with the number of sexual partners when controlling for effects of ethnicity/race and sexual identification, particularly for men who self-identified as straight. Highest IHNI scores were observed in a small group of MSM/W (n = 62) who never tested for HIV. Of these, 26% tested HIV positive. Findings describe ways in which internalized homophobia is a barrier to HIV testing and associated HIV infection and signal distinctions among participants in this sample that can inform targeted HIV prevention efforts aimed at increasing HIV testing.  相似文献

This study examined demographic characteristics, sexual risk behaviors, sexual beliefs, and substance use patterns in HIV-positive, methamphetamine-using men who have sex with both men and women (MSMW) (n = 50) as compared to men who have sex with men only (MSM) (n = 150). Separate logistic regressions were conducted to predict group membership. In the final model, of 12 variables, eight were independently associated with group membership. Factors independently associated with MSMW were acquiring HIV through injection drug use, being an injection drug user, using hallucinogens, using crack, being less likely to have sex at a bathhouse, being less likely to be the receptive partner when high on methamphetamine, having greater intentions to use condoms for oral sex, and having more negative attitudes about HIV disclosure. These results suggest that, among HIV-positive methamphetamine users, MSMW differ significantly from MSM in terms of their HIV risk behaviors. Studies of gay men and HIV often also include bisexual men, grouping them all together as MSM, which may obscure important differences between MSMW and MSM. It is important that future studies consider MSM and MSMW separately in order to expand our knowledge about differential HIV prevention needs for both groups. This study showed that there were important differences in primary and secondary prevention needs of MSM and MSMW. These findings have implications for both primary and secondary HIV prevention among these high-risk populations.  相似文献

The purpose of the study was to determine the potential contribution of bisexual men to the spread of HIV in Los Angeles. We compare the characteristics and behaviors of men who have sex with men and women (MSMW) to men who have sex with only women (MSW) and men who have sex with only men (MSM) in Los Angeles. Men (N = 1,125) who participated in one of the two waves of data collection from 2005 to 2007 at the Los Angeles site for NIDA’s Sexual Acquisition and Transmission of HIV—Cooperative Agreement Program were recruited using Respondent Driven Sampling. Participants completed Audio Computer Assisted Self Interviews and received oral HIV rapid testing with confirmatory blood test by Western Blot and provided urine specimens for detection of recent powder cocaine, crack cocaine, methamphetamine, or heroin use. MSM, MSW, or MSMW were defined by the gender of whom they reported sex with in the past 6 months. Chi-square tests and ANOVAs were used to test independence between these groups and demographic characteristics, substance use, and sexual behaviors. We fit generalized linear random intercept models to predict sexual risk behaviors at the partner level. Men were mostly of low income, unemployed, and minority, with many being homeless; 66% had been to jail or prison, 29% had ever injected drugs, and 25% had used methamphetamine in the past 30 days. The sample had high HIV prevalence: 12% of MSMW, 65% of MSM, and 4% of MSW. MSMW were behaviorally between MSW and MSM, except that more MSMW practiced sex for trade (both receiving and giving), and more MSMW had partners who are drug users than MSW. Generalized linear random intercept models included a partner-level predictor with four partner groups: MSM, MSMW-male partners, MSMW-female partners, and MSW. The following were significantly associated with unprotected anal intercourse (UAI): MSW (AOR 0.15, 95% CI 0.08, 0.27), MSMW-female partners (AOR 0.4, 95% CI 0.27, 0.61), HIV-positive partners (AOR 2.03, 95% CI 1.31, 3.13), and being homeless (AOR 1.37, 95% CI 1.01, 1.86). The factors associated with giving money or drugs for sex were MSMW-female partners (AOR 1.70, 95% CI 1.09, 2.65), unknown HIV status partners (AOR 1.72, 95% CI 1.29, 2.30), being older (AOR 1.02, 95% CI 1.00, 1.04), history of incarceration (AOR 1.64, 95% CI 1.17, 2.29), and being homeless (AOR 1.73, 95% CI 1.27, 2.36). The following were associated with receiving money or drugs for sex: MSW (AOR 0.53, 95% CI 0.32, 0.89), African American (AOR 2.42, 95% CI 1.56, 3.76), Hispanic (AOR 1.85, 95% CI 1.12, 3.05), history of incarceration (AOR 1.44, 95% CI 1.04, 2.01), history of injecting drugs (AOR 1.57, 95% CI 1.13, 2.19), and had been recently homeless (AOR 2.14, 95% CI 1.57, 2.94). While overall HIV-positive MSM had more UAI with partners of any HIV status than MSMW with either partner gender, among HIV-positive MSMW, more had UAI with HIV-negative and HIV status unknown female partners than male partners. Findings highlight the interconnectedness of sexual and drug networks in this sample of men—as most have partners who use drugs and they use drugs themselves. We find a concentration of risk that occurs particularly among impoverished minorities—where many men use drugs, trade sex, and have sex with either gender. Findings also suggest an embedded core group of drug-using MSMW who may not so much contribute to spreading the HIV epidemic to the general population, but driven by their pressing need for drugs and money, concentrate the epidemic among men and women like themselves who have few resources.  相似文献

BackgroundGay apps are smartphone-based geosocial networking apps where many men who have sex with men (MSM) socialize and seek sex partners. Existing studies showed that gay app use is associated with greater odds of high-risk sexual behaviors and potentially more HIV infections. However, little is known about this behavior among young MSM.ObjectiveWe conducted this study to understand gay app use and its influencing factors among MSM attending university in China.MethodsFrom January to March 2019, participants were recruited from 4 regions with large populations of college students in China: Chongqing, Guangdong, Shandong, and Tianjin. The eligibility criteria were MSM aged 16 years or older, self-identified as a university student, and being HIV negative. A self-administered online structured questionnaire was used to collect data on sociodemographic information, sexual behaviors, gay app use, substance use, and HIV testing history. We performed multivariable log-binomial regression to assess correlates of seeking sex partners via gay apps.ResultsA total of 447 MSM attending university with an average age of 20.4 (SD 1.5) years were recruited. Almost all participants (439/447, 98.2%) reported gay app use at some point in their life, and 240/439 (53.7%) reported ever seeking sex partners via gay apps. Blued (428/439, 97.5%) was the most popular gay app. Higher proportions of sexual risk behaviors (including seeking sex partners via apps [P<.001], engaging in group sex [P<.001], having multiple sex partners [P<.001], unawareness of sex partners’ HIV status [P<.001], and using recreational drugs during sex [P<.02]) were positively associated with the increase in the frequency of gay app use. In multivariable analysis, participants who used gay apps to seek sex partners might be more likely to have multiple sex partners in the past 3 months (adjusted prevalence ratio [APR] 1.53, 95% CI 1.33-1.76; P<.001), engage in group sex in the past 3 months (APR 1.55, 95% CI 1.35-1.78; P<.001), and have sex partners with unknown or positive HIV status (APR 1.72, 95% CI 1.46-2.01; P<.001).ConclusionsSeeking sex partners via gay apps may associate with the increased high-risk sexual behaviors among MSM attending university. [目的]调查男男性行为者(menwhohavesexwithmen,MSM)中双性性行为的发生情况及艾滋病相关的高危行为发生情况,分析MSM发生异性性行为的原因及影响因素,为减少艾滋病由高危人群向一般人群扩散,更好地开展艾滋病高危行为干预工作提供思路。[方法]采用横断面调查研究的方法,于2010年6_12月按年龄段在上海市浦东新区进行分层抽样,对符合入选标准的研究对象进行一对一的问卷调查,收集一般人口学信息、工作生活状况、社交网络与性行为情况、艾滋病相关知识和态度等资料并进行相关的统计描述与分析。[结果]共收集有效问卷158份;66.5%(105/158)的MSM来自上海以外省市;17.7%(28/158)的MSM表示自己是双性恋,27.8%(44/158)的MSM表示过去5年和女性有过性活动,差异有统计学意义(x2=4.60,P〈0.05);和女性有过性活动的MSM人群中最近一次性行为中安全套使用率为80.O%(12/15),最近6个月每次安全套使用率为53-3%(8/15);非条件logistic回归分析结果表明,影响MSM人群与女性发生性行为的因素有婚姻状况(OR=10.53,95%CI:3.07~36.13)和性取向(OR=82.86,95%CI:16.35-419.86)。[结论]MSM人群中的双性恋是将人类免疫缺陷病毒从MSM人群向一般人群扩散的主要人群,一部分男男同性恋由于婚姻原因和女性发生性行为,进一步增加了人类免疫缺陷病毒向一般人群扩散的力度,应从家庭和社会环境的角度加以防治。  相似文献

Our objective was to investigate sexualcommunication and risk-taking behaviors among young menwho have sex with men (MSM). MSM aged 17 to 25 yearswere recruited from four areas throughout California. The construct Safer Sex Communication, assessedthrough principal-components analysis, was defined asperceived efficacy or experience in minimizing the riskof sexual HIV infection through communication with partners. In the 6 months prior to thesurvey, 35.6% of the MSM had participated in unprotectedanal intercourse (UAI). Adjusted logistic regressionanalyses found low Safer Sex Communication to be predictive of UAI. In addition, negativeattitudes toward safe sex, high perceived risk of HIVinfection, 10 or more male sex partners during theprevious 6 months, and use of cocaine in the past 6months were also independently related to UAI. SaferSex Communication skills should be an outcome ofinterventions designed to prevent HIV transmission amongyoung MSM.  相似文献

[目的]探索男男性接触者(MSM)对自己同性恋身份的认同及其与艾滋病相关危险因素的关系。[方法]使用统一设计的调查问卷,对利用"滚雪球"、"网络动员"等途径招募的151名MSM采用匿名面对面询问方式调查其自我认同、性行为、性伴网络、吸毒史和健康相关行为。[结果]消极自我认同的比例为56.29%(85/151)。文化程度与自我认同有关联性(χ2=8.022,P〈0.05)。自认性取向为同性恋的积极自我认同比例高于自认性取向为双性恋和异性恋者(χ2=5.108,P〈0.05)。不同自我认同和与男性首次性行为年龄、与异性结婚可能性、人类免疫缺陷病毒(HIV)抗体检测率、过去6个月有异性性伴/固定男性伴/多次男性伴等因素的关联有统计学意义(P值均〈0.05),其中积极自我认同的MSM与男性首次性行为更多发生在20岁以前、与异性结婚可能性小、更易接受HIV检测服务、拥有固定男性伴比例高、拥有异性性伴和多次男性伴的比例小。[结论]MSM人群消极自我认同的比例较高,其无保护性交和多性伴较广泛地存在,主动求医进行HIV抗体检测率较低,感染HIV危险性和传播桥梁作用也更大。应广泛开展社会反歧视宣传,促进MSM积极自我认同,增进艾滋病预防服务利用,有效降低艾滋病传播的风险。  相似文献

[目的]了解上海市男男性行为人群(menwhohavesexwithmen,MSM)人类免疫缺陷病毒(HIV)感染相关行为学特征,并分析与男男无保护性行为相关的影响因素。[方法]利用信息网络中的上海市男男活动小组和男男论坛,共招募符合筛选条件的调查对象121名。采用中一加项目全球课题设计的统一表格对121人进行面对面问卷调查。[结果]调查对象平均年龄为(27.99±5.93)岁;大学及以上教育程度者占74.4%;月均收入≥3000元人民币者占62.9%;单身者占86.0%。121名MSM初次和男性发生性行为的平均年龄为(22.14±5.32)岁;过去半年内受访者的平均性伴数为(3.27±2.77)人。目前有固定男友的受访者占56.14%,但和固定男友之间在性关系上相互忠诚的仅占其中的36.84%。16人在过去半年内和女性有性行为,其中62.8%的受访者曾和女性发生过无保护性行为。45.45%的受访者曾与男性有过无保护性行为,受访者和固定男友间发生无保护性行为的比例高于和多次男性伴间发生无保护性行为的比例(x2=4.32,P〈0.05),也明显高于和偶遇男性伴间发生无保护性行为的比例(x2=10.13,P〈0.01)。77.7%的受访者认为自己有感染HIV的风险。多因素分析表明,与男性发生无保护性行为相关的影响因素为受教育程度、半年内性伴数量和HIV感染自我风险评估情况。[结论]受教育程度越低、性伴数量越多的MSM发生无保护性行为的概率越高。认为自己有HIV感染风险的受访者更有可能发生过无保护性行为。因此在今后对存在上述情况的MSM更应加强健康干预。  相似文献

BackgroundFinding casual sex partners on the internet has been considered a huge challenge for HIV transmission among men who have sex with men (MSM) in China.ObjectiveThis study aimed to identify the characteristics and risk factors of finding causal sex partners on the internet among MSM in Zhejiang Province, China.MethodsThis was a cross-sectional study. Participants were enrolled by 4 community-based organizations (CBOs) and 10 Voluntary Counselling and Testing (VCT) clinics through advertisements in bathrooms, bars, and gay hook-up apps from June to December 2018. A CBO- or physician-assisted survey was conducted to collected information on finding casual sex partners, perceived HIV infection, and HIV risk behaviors.ResultsAmong 767 participants, 310 (40.4%) reported finding causal sex partners on the internet. Factors associated with finding casual sex partners on the internet included watching pornographic videos on the internet more than once a week (adjusted odds ratio [aOR]=1.881, 95% CI 1.201-2.948), discussing “hooking-up online” with friends (aOR=4.018, 95% CI 2.910-5.548), and perceiving that the likelihood of HIV infection among casual sex partners sought on the internet was “medium” (aOR=2.034, 95% CI 1.441-2.873) or “low” (aOR=2.548, 95% CI 1.524-4.259). Among the participants who reported finding casual sex partners on the internet, 30.2% (91/310) reported having unprotected sex with casual sex partners encountered on the internet in the past 6 months. On multivariate logistic regression analyses, knowing the HIV infection status of casual sex partners sought on the internet was significantly associated with performing inserted intercourse (aOR=1.907, 95% CI 1.100-3.306) and a decreased risk of inconsistent condom use (aOR=0.327, 95% CI 0.167-0.642).ConclusionsWeb-based casual sexual behavior is becoming more prevalent, and the rate of unprotected sex among MSM in Zhejiang Province is high. Future HIV prevention approaches should emphasize the importance for MSM to proactively determine the HIV infection status of potential casual sex partners sought on the internet.  相似文献

ObjectivesIn South Korea, men who have sex with men (MSM) are rather understudied, but are known to be at high risk for human immunodeficiency virus infection (HIV)/acquired immunodeficiency syndrome (AIDS). This study was to access HIV/AIDS knowledge, attitudes, and risk behaviors, and to identify the factors of condom use in HIV prevention.MethodsWe recruited 1070 MSM in Korea, using the Internet to maximize the confidentiality of the MSM.ResultsThe prevalence of self-reported and sexually transmitted infections and HIV in the total sample was 10.7% and 2.7%, respectively. Factual knowledge and phobias regarding HIV/AIDS and self-efficacy were relatively high among the MSM. After controlling for age, education, marital status, and sexual identity, predictors of condom use at most recent anal sex included knowledge (OR = 1.25; p < 0.0001); self-efficacy (OR = 1.33; p = 0.02), additionally, having HIV testing (OR = 1.45; p = 0.02); and having a regular partner (OR = 0.53; p < 0.0001) were also positively associated with condom use.ConclusionThe intervention programs for MSM in Korea may need to take the idiosyncratic societal and cultural pressures of the region into consideration in order to reduce infection risk.  相似文献

[目的]了解男男性行为(MSM)人群性伴网络特征及该人群中高危性行为的发生情况,探讨性病(STDs)、人类免疫缺陷病毒(HIV)在该人群中传播及向普通人群传播的模式,为开展性病、艾滋病预防控制提供依据。[方法]通过艾滋病自愿咨询检测门诊、同性恋人群活动场所、同性恋QQ群内动员、"滚雪球"等途径寻找MSM人群,招募151人,采用面对面匿名问卷调查方式收集信息。[结果]151名被调查者中最近6个月有30人(19.9%)与异性发生过性行为,高危性行为的发生率为53.3%;与同性性活动情况,其中91人(60.3%)有同性固定性伴,76人(50.3%)与非固定同性性伴的人发生过多次男男性行为(简称有多次男性伴),77人(51.0%)发生过偶遇性行为(简称有偶遇男性伴),10人(6.6%)从事商业性行为。而在与固定性伴、多次男性伴、偶遇男性伴、商业性行为中高危性行为的发生率依次是69.2%、57.9%、50.6%、70.0%。与固定男性伴高危性行为的发生率高于与偶遇男性伴高危性行为的发生率(P=0.014)。最近6个月有2种及以上性伴类型占58.9%。30名已感染HIV者中有多次男性伴及偶遇男性伴类型的比例分别为53.3%和63.3%;有过多次男性伴、偶遇男性伴性行为者中发生高危性行为的比例分别为50.0%和63.2%。感染HIV者与固定男性伴发生高危性行为的率低于未感染HIV者(P=0.029)。[结论]MSM人群性伴网络复杂多样,高危性行为发生率高。不仅使HIV在该人群中容易传播,且该人群已成为向普通人群传播HIV的"桥梁"人群。  相似文献

Using the Internet to meet sexual partners is associated with increased HIV risk behavior, including substance use, sex with multiple or anonymous partners, and unprotected anal sex (UAS), among diverse samples of MSM, yet little is known about Internet use and HIV risk among Black MSM specifically. In 2008, a sample of 197 Black MSM completed an interviewer-administered assessment and voluntary HIV counseling and testing. One fifth of the sample (20 %) reported meeting a sexual partner via the Internet in the past 12 months. Men who met sexual partners over the Internet had significantly more male sex partners (M = 13.44, SD = 20.01) than men who did not meet partners in this manner (M = 4.11, SD = 4.14, p < 0.001) and reported significantly higher rates of UAS (p < 0.05). Adjusting for sociodemographic and other HIV-related covariates, factors significantly associated with the increased odds of engaging in at least one episode of UAS with a male partner in the past 12 months included: meeting sexual partners on the Internet, identifying as gay, and lower knowledge about HIV transmission. These findings highlight the unique HIV risk behaviors among Black MSM meeting sexual partners via the Internet and warrant tailoring of prevention activities to address the specific behaviors and social influences that may contribute to increased HIV spread among this population.KEYWORDS: MSM, Internet, African American/Black, HIV, Sexual risk  相似文献

Disproportionately high HIV/AIDS rates and frequent non-gay identification (NGI) among African American men who have sex with men or with both men and women (MSM/W) highlight the importance of understanding how HIV-positive African American MSM/W perceive safer sex, experience living with HIV, and decide to disclose their HIV status. Thirty predominately seropositive and non-gay identifying African American MSM/W in Los Angeles participated in three semi-structured focus group interviews, and a constant comparison method was used to analyze responses regarding condom use, sexual activity after an HIV diagnosis, and HIV serostatus disclosure. Condom use themes included its protective role against disease and pregnancy, acceptability concerns pertaining to aesthetic factors and effectiveness, and situational influences such as exchange sex, substance use, and suspicions from female partners. Themes regarding the impact of HIV on sexual activity included rejection, decreased partner seeking, and isolation. Serostatus disclosure themes included disclosure to selective partners and personal responsibility. Comprehensive HIV risk-reduction strategies that build social support networks, condom self-efficacy, communication skills, and a sense of collective responsibility among NGI African American MSM/W while addressing HIV stigma in the African American community as a whole are suggested.Harawa is with the Charles B. [目的]了解大连市男男性行为者(MSM)的性行为特征、艾滋病病毒(HIV)感染状况及其影响因素,为MSM人群行为干预工作提供依据。[方法]2009年4-6月,在大连市采用定向抽样方法,对402名MSM进问卷调查和HIV抗体检测。[结果]调查402人,最近半年发生同性肛交的357人,占88.81%。这357人中,同性肛交时安全套从未使用的占9.52%,有时使用的占35.29%,每次都用的占55.18%;付钱寻找男男性服务的占2.24%,平均2.38个性伴;提供商业男男性服务的占21.01%,平均服务17人。调查的402人中,最近半年发生异性性行为的136人,占33.83%。这136人中,安全套从未使用的占41.91%,有时使用的占21.32%,每次都用的占36.76%。检测402人,HIV抗体阳性检出率为4.48%,梅毒感染率为13.93%,有4名同时感染HIV和梅毒。多因素非条件Logistic回归分析结果,年龄大、未接受过艾滋病干预服务者是促进HIV感染的因素。[结论]MSM人群存在感染HIV的危险行为,艾滋病干预服务措施有效,中老年MSM是艾滋病预防控制工作的重点人群。  相似文献

Employing data from two Chicago-based household probability samples of men who have sex with men (MSM) implemented 5 years apart (the “UMHS 1997” and the “2002 MSM supplement” studies), we evaluated changes in risk behavior as well as the potential viability of two alternative perspectives for explaining these changes—risk management and safe-sex norm abandonment. We found significantly increased rates of unprotected insertive and receptive anal intercourse in the 2002 study. Sixty-eight percent of UMHS men reported having sex with partners having HIV positive or unknown status, compared with 38% of the MSM supplement men (p < .0001). Serosorting mediated and moderated the most extreme forms of risk behavior. Positive statistical associations between drug use and unprotected sex were stronger in the UMHS sample than in the MSM supplement. Findings suggesting that “risk management” strategies have shaped MSM behavior as it emerged in the early part of this decade have considerable implications for HIV prevention strategies.  相似文献

Little is known about the prevalence of intimate partner violence (IPV) among men who have sex with men (MSM) or about childhood adversity as a predictor of IPV among MSM. Studies have documented high rates of childhood sexual abuse among MSM. To evaluate associations of early-life sexual and physical abuse with IPV among African American heterosexual men or MSM, prevalence of early-life (≤21 years) sexual and physical abuse was measured among 703 nonmonogamous African American men. Men were classified as (1) MSM who disclosed male sex partners; (2) MSM who initially denied male sex partners but subsequently reported oral-genital and anal-genital behaviors with men; (3) non-MSM. MSM who initially disclosed male sex partners reported significantly (P < 0.0001) higher rates of early physical abuse (36%) and lifetime abuse (49%) compared with non-MSM (15 and 22%), respectively. These MSM reported significantly higher rates of sexual abuse by age 11, age 21, and over a lifetime compared with non-MSM (P < 0.0001). Being an MSM who initially disclosed male sex partners (OR: 2.1; 95% CI: 1.2, 3.6) and early-life sexual abuse (OR: 2.8; 95% CI: 1.8, 4.3) was associated with IPV victimization in current relationships. Similarly, being an MSM with early-life physical and sexual abuse was associated (0.0004 ≤ P ≤ 0.07) with IPV perpetration. Early-life physical and sexual abuse was higher among MSM who disclosed male sex partners compared with heterosexual men; however, all MSM who experienced early-life abuse were more likely to be IPV victims or perpetrators.  相似文献

Much of our understanding of the association between the Sexual Compulsivity Scale (SCS) and sexual risk behavior among men who have sex with men (MSM) has been limited to samples of HIV positive MSM only. Using data from a community-based survey of gay and bisexual men (n = 1214), this analysis sought to further evaluate the association between the SCS and sexual risk behavior. The SCS was significantly associated with a variety of sexual risk behaviors, including having sex under the influence of club drugs, engaging in unprotected anal sex (receptive or insertive) with partners of the same and/or different HIV serostatus, identity as a barebacker, intentions to have bareback sex, number of recent sex partners, and temptation for unsafe sex. The SCS was also significantly associated with having engaged in a variety of specialized sexual behaviors (i.e., fetishes), many of which can increase HIV transmission risks. Finally, in multivariate analyses, the SCS significantly predicted unprotected sex with a non-main partner even when controlling for race, HIV serostatus, age, identity as a barebacker, and club drug use. These data indicate that the SCS may be able to serve as an indicator to detect HIV-associated sexual risk behavior in community-based samples of gay and bisexual men.  相似文献
