A review of the literature and a report of a case of necrotizing sialometaplasia of the hard palate is presented. Because necrotizing sialometaplasia may be misdiagnosed as squamous cell carcinoma or mucoepidermoid carcinoma, it is important that the clinician and pathologist take the necessary precautions to prevent any further mutilative surgery for this benign disease entity.  相似文献

The purpose of this report was to present two unusual cases of necrotizing sialometaplasia of the hard palate, in which the patients were suffering from bulimia and chronic self-induced vomiting. A case of NSM presenting as bilateral ulceration of the palate developing in the seventh month of pregnancy is reported. The lesions caused initial problems of differential diagnosis because they were more extensive than is often the case in NSM. However, healing followed simple palliative treatment for the ulcers. The etiology of NSM is discussed in relation to the vascular and hormonal changes of pregnancy.  相似文献

Necrotizing sialometaplasia is a nonneoplastic, inflammatory, variably ulcerated, occasionally bilateral, self-healing lesion of human salivary glands which is often confused clinically and histologically with squamous cell or mucoepidermoid carcinoma. On the basis of thirty-three documented cases, the lesion occurs primarily in the minor salivary glands of the palate (87.9 percent), with 63.6 percent arising in the fifth and sixth decades (average age 46.0 years, range 22 to 68 years). There is a marked predilection for males (ratio of males to females. 2.7:1) and a questionable predominance in Caucasians (60.6 percent). Numerous etiologic factors have been proposed; however, on the basis of previous animal experimentation, the lesion appears to be infarctive in nature.  相似文献

A review of 127 documented cases of necrotizing sialometaplasia is presented, mainly focusing on 96 palatal lesions. Necrotizing sialometaplasia is a benign, self limiting inflammatory lesion of the minor salivary glands. The cause is unknown but it may be related to oral trauma, tobacco use or inadequate blood supply. Male (mean age 49.8 yrs) to female (mean age: 36.3 yrs) ratio is 2.31:1. Because the clinical and histologic features mimic malignancy, proper diagnosis is of utmost importance in order to avoid unnecessary and extensive surgery and radiation therapy. The lesion heals spontaneously, regardless of therapy in two to eight weeks.  相似文献

Salivary gland involvement, particularly salivary gland enlargement (sialosis), is a recognised complication in bulimia. We report the rare association of sialosis and necrotising sialometaplasia with bulimia in the same patient. The association of sialosis and necrotising sialometaplasia in the same patient with bulimia has been reported previously in two patients and may be coincidental, but the appearance in this additional patient suggests it may be prudent to explore this further.  相似文献

Necrotizing sialometaplasia is a benign, self-limiting disease that requires no treatment. It is often confused with malignancy, resulting in unnecessary, extensive surgery. Clinicians need to be aware of this lesion and include it in a differential diagnosis of ulcerated lesions. Necrotizing sialometaplasia poses many diagnostic problems, both clinically and microscopically. Caution had most frequently been urged for distinguishing between malignancy and NS, so as to avoid mutilating surgical treatment resulting from the misdiagnosis of a lesion that is benign and self-limiting. This case has been presented to alert the practitioner to the possibility that NS may obscure an underlying malignant disease process, possibly resulting in delays in referral or treatment. The exceedingly rapid growth of this mesenchymal neoplasm may have caused obstruction of the blood supply to the overlying mucosa and submucosa, thereby producing the ischemic alterations observed in the first biopsy specimens. Some of the difficulties encountered in the detection and diagnosis of neoplasms arising in the parapharyngeal space were discussed, and some previously published suggestions to aid in these efforts were reiterated.  相似文献

Necrotizing cervical fasciitis is a rare, fulminating infection causing extensive necrosis of the subcutaneous tissue and fascial planes with resultant skin gangrene. To reduce the morbitity and mortality from this condition, it requires early recognition and aggressive surgical debridment with appropriate antibiotic therapy. The origin is generally odontogenic and presents more often in immunocompromised patients. This article presents a case with typical clinical features and appropriate management of this condition. In the present study, a rare pathology, described for the first time in 1973 has been examined. In particular, the nosologic picture of the disease, the etiopathogenetic hypotheses, symptoms and diagnostic procedures reported in the literature have been analysed. The case of a man showing a neo-formation, in the midline region of the hard palate, that appeared only following ingestion of very hot food is examined. The surgical procedure is reported and discussed in detail, and the histopathological analysis showing a necrotizing sialometaplasia is reported. Prognosis and possible therapeutic approaches are also discussed. Finally, the attention has been focused on the histological similitary between this pathology and squamous cell carcinoma or epidermoid carcinomas.  相似文献

Necrotizing sialometaplasia is a self-limiting, variably ulcerated benign process affecting minor salivary glands. Accurate histological diagnosis is paramount, as it has been mistaken for malignancy, which has resulted in excessively aggressive and unnecessary radical surgery. A unique case of bilateral necrotizing sialometaplasia, presenting with anaesthesia of the greater palatine nerves, is described. An attempt at active therapy with intralesional steroids had no effect on the course of the condition.  相似文献

Necrotizing sialometaplasia (NS) is a self‐limiting, benign, inflammatory disease of the minor salivary glands of the hard palate. The main significance of the NS lesion lies in the fact that it may be mistaken for mucoepidermoid or squamous cell carcinoma. A case report is presented of a patient with NS who was HIV positive; the lesion was located in the minor salivary glands of the hard palate. Clinically, the lesion presented as a deep ulcer with slightly elevated irregular borders and a necrotic base in the hard palate. Histologically, the tissue was characterized by squamous metaplasia of ducts and acini, lobular coagulation necrosis, and pseudoepitheliomatous hyperplasia of the overlying epithelium. The lesion disappeared completely after 2 weeks.  相似文献

The clinicopathologic findings in 69 cases of necrotizing sialometaplasia (NS) were analyzed and compared with 115 reported cases of NS in the English-language literature. Data comparing age, sex, race, location, clinical presentation, and possible predisposing factors are summarized. Analysis of the data indicates that NS can occur in a variety of clinical settings and may exhibit a spectrum of histologic features. Recognition of NS, regardless of its clinical or microscopic presentation, is essential to avoid inappropriate or unnecessary treatment for this benign reactive process.  相似文献

Cocaine abuse and the consequent prevalence of the damage associated with it have increased in the global population. The damage depends on the route of administration, where an important sequel of snorting is perforation of the nasal septum and palate. Necrosis of the palate, meanwhile, is also the manifestation of other diseases such as tertiary syphilis, necrotizing sialometaplasia and malignant neoplasms. The differential diagnosis of such lesions is important for determining appropriate treatment. A case of an ulcerated lesion in the palate of a cocaine-using patient whose diagnosis was necrotizing sialometaplasia is reported.  相似文献
