B-type natriuretic peptide (BNP) is increasingly being used as a diagnostic marker in the diagnosis of heart failure. Here we evaluate the evidence base for its utility in the ED. Clinical trials suggest that it is more accurate than clinical acumen especially when emergency physicians have diagnostic problems. BNP appears more accurate than any clinical findings or radiological signs. In conjunction with considered clinical judgement, knowledge of its limitations and variable cut-off points, BNP can be of considerable utility to the emergency physician.  相似文献

目的探讨慢性心力衰竭(心衰)患者血浆B型钠尿肽(BNP)水平变化及其临床意义。方法采用免疫化学发光分析法,测定63例住院心力衰竭患者血浆B型尿钠肽(BNP)水平(心衰组)和30例健康成人(对照组)血清BNP浓度,并按照纽约心脏病协会(NYHA)分级,比较不同NYHA级别BNP水平差异。结果 (1)心衰组BNP水平(868.5±302.8)pg/mL,明显高于对照组[(101.4±32.8)pg/mL](P〈0.01)。(2)NYHA分级Ⅱ级患者BNP水平为(629.37±224.56)pg/mL,NYHA分级Ⅲ级患者BNP水平为(853.39±302.43)pg/mL,NYHA分级Ⅳ级患者BNP水平为(1 378.62±433.25)pg/mL,3组间两两比较差异有统计学意义(P〈0.01)。结论 BNP水平随着心衰患者严重程度的加重而增加,对心衰患者的诊断、病情评估、预后判断具有重要临床意义。  相似文献

目的探讨B型钠尿肽(BNP)在心力衰竭(简称心衰)、高血压患者中的应用价值。方法采用荧光免疫分析技术,对181例心衰患者、50例原发性高血压病患者及健康对照人群30例进行血浆BNP水平检测,同时以彩色多普勒超声心电动仪测定心衰患者左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF),并与BNP作相关性分析。结果心衰组患者血浆BNP水平高于高血压组,心衰组和高血压组均高于对照组,且差异有显著性(P<0.05~0.01)。BNP水平与LVEF、LVESD、LVEDD的数值具有相关性(r分别为-0.57、0.65和0.68,P<0.01)。结论BNP是反映心室功能的灵敏指标。  相似文献

The use of BNP as a diagnostic and therapeutic tool in the management of heart failure is promising. Additional studies need to be done regarding the use of BNP as a diagnostic tool to clarify its intrapatient and interpatient variability, especially over time. Nesiritide is the first new intravenous agent for the treatment of acute decompensated heart failure since the introduction of milrinone. It is an effective vasodilator and enhances the effect of concomitant diuretic therapy. Nesiritide may have some benefit on long-term outcomes by prolonging survival, decreasing hospitalizations, or enhancing quality of life. Whether it can or should be used as chronic therapy in end-stage patients remains to be determined.  相似文献

The B-type or brain natriuretic peptide (BNP) assay, a 15-minute bedside blood test, is highly sensitive and fairly specific for diagnosing heart failure and is useful in evaluating suspected heart failure in outpatients and in emergency care. BackgroundThe purpose of this study was to investigate the applicability of urinary B-type natriuretic peptide (BNP) levels to the diagnosis and prognosis of heart failure (HF) by comparing urinary and plasma BNP levels.MethodsUrinary and plasma BNP levels of 160 patients with HF classifiable into NYHA stages I, II, III, or IV, and 30 healthy control subjects were measured using the microparticle enzyme immunoassay (MEIA). The heart function of each patient was examined by ultrasonic cardiogram and classified according to the NYHA standard.ResultsThe levels of urinary and plasma BNP among patients in the HF group were significantly higher than those in the control group. Urinary BNP levels were correlated with plasma BNP levels and NYHA grades of HF and were negatively correlated with LVEF. According to the Cox model multivariate regression analyses on age, gender, heart function class, LVEF, and urinary BNP among patients with post-treatment cardiac events, age and the urinary BNP level were predictors for the independent post-treatment cardiac events.ConclusionsMeasurement of urinary BNP is applicable in clinical diagnosis and prognosis of HF; it provides similar accuracy to plasma BNP in the detection of HF, and it has the advantage of being a noninvasive test.  相似文献

Heart failure has a significant impact on health and its prevalence increases with age (Cowie et al, 1997). Coronary heart disease (CHD) and hypertension are the most common causes (Cowie et al, 1999). The National Service Framework for Coronary Heart Disease identifies that people with suspected heart failure should be offered appropriate investigations to confirm the diagnosis and identify the cause (DoH, 2000). Also, those with confirmed heart failure should be offered treatment to relieve symptoms and reduce the risk of death. The National Institute for Clinical Excellence will publish guidelines this summer, which will aim to improve the management of heart failure.  相似文献

The incidence of heart failure (HF) is on the increase with the aging population. Heart failure can manifest as either systolic or diastolic dysfunction. Systolic dysfunction causes impaired ventricular contractility with an ejection fraction of less than 45%. In contrast, diastolic dysfunction is evidenced by impaired ventricular relaxation and an ejection fraction greater than 45%. The diagnosis of HF is challenging with patients who present with acute dyspnea and a history of chronic obstructive pulmonary disease or pneumonia. The pathophysiology of HF and the resulting compensatory mechanisms involve a complex neuroendocrine response that includes a release of natriuretic peptides including B-type natriuretic peptides (BNPs). Elevation of BNP is in response to ventricular wall stress and volume overload from HF. BNP promotes natriuresis, diuresis, and vasodilitation and therefore counteracts some of the deleterious effects of the neuroendocrine response in HF Recently, a new laboratory test for BNP has been developed to assist in rapid identification of patients with HF. Research studies have shown that BNP testing assists in differentiating between cardiac and pulmonary causes of acute dyspnea and could be used to evaluate effectiveness of therapy and as a predictor for length of stay and readmission.  相似文献

目的 探讨尿液BNP水平对慢性心力衰竭的诊断价值。方法采用微粒酶免疫测定法检测83例慢性心力衰竭患者(NYHAⅠ ~Ⅳ级)和30名健康对照者的尿液和血浆BNP浓度,同时采用NYHA标准对心功能进行分级,并用超声心动图检查评定患者心功能。结果慢性心力衰竭组尿液BNP水平为[90.0(38.3~209.5)]ng/L,血浆BNP水平为[680.0(289.7 ~ 1543.5)]ng/L,明显高于健康对照组的[17.0(13.0 ~33.0)]ng/L和[84.5(56.0~158.0)]ng/L,差异均有统计学意义(U值分别为207.5、71.0,P均<0.01)。随心功能NYHA分级的增高,尿液BNP水平逐渐升高。尿液BNP水平与血浆BNP水平呈正相关(r=0.842,P<0.01),与NYHA分级呈正相关(r =0.742,P<0.01),与LVEF呈负相关(r=-0.801,P<0.01)。LVEF< 40%患者的尿液BNP水平为[143.0(85.0 ~258.0)]ng/L,LVEF≥40%患者的尿液BNP水平为[31.5(17.3~38.8)]ng/L,差异有统计学意义(U=80.0,P<0.01)。以36.5 ng/L为临界值时,尿液BNP诊断慢性心力衰竭的敏感度为84%,特异度为80%。结论 尿液BNP对慢性心力衰竭的诊断有重要意义,与血浆BNP具有相似的诊断价值。  相似文献
