The aim of this autopsy study was to investigate chest-compression associated injuries to the trunk in out-of-hospital and in-hospital non-traumatic cardiac arrest patients treated with automated external chest compression devices (ACCD; all with LUCAS II devices) versus exclusive manual chest compressions (mCC). In this retrospective single-center study, all forensic autopsies between 2011 and 2017 were included. Injuries following cardiopulmonary resuscitation (CPR) in patients treated with mCC or ACCD were investigated and statistically compared using a bivariate logistic regression. In the seven-year period with 4433 autopsies, 614 were analyzed following CPR (mCC vs. ACCD: n?=?501 vs. n?=?113). The presence of any type of trunk injury was correlated with longer resuscitation intervals (30?±?15 vs. 44?±?25 min, p?<?0.05). In comparison with mCC, treatment with ACCD led to more frequent skin emphysema (5 vs 0%, p?=?0.012), pneumothorax (6 vs. 1%, p?=?0.008), lung lesions (19 vs. 4%, p?=?0.008), hemopericardium (3 vs 1%, p?=?0.025) and liver lesions (10 vs. 1%, p?=?0.001), all irrespective of confounding aspects. Higher age and longer CPR durations statistically influenced frequency of sternal and rib fractures (p?<?0.001). The mean number of fractured ribs did not vary significantly between the groups (6?±?3 vs. 7?±?2, p?=?0.09). In this cohort with unsuccessful CPR, chest compression-related injuries were more frequent following ACCD application than in the mCC group, but with only minutely increased odds ratios. The severity of injuries did not differ between the groups, and no iatrogenic injury was declared by the forensic pathologist as being fatal. In the clinical routine after successful return of spontaneous circulation a computed tomography scan for CPR-associated injuries is recommended as soon as possible.  相似文献

The purpose of the study was to evaluate both CT image quality in a phantom study and feasibility in an initial case series using automated chest compression (A-CC) devices for cardiopulmonary resuscitation (CPR). Multidetector CT (MDCT) of a chest/heart phantom (Thorax-CCI, QRM, Germany) was performed with identical protocols of the phantom alone (S), the phantom together with two different A-CC devices (A: AutoPulse, Zoll, Germany; L: LUCAS, Jolife, Sweden), and the phantom with a LUCAS baseplate, but without the compression unit (L-bp). Nine radiologists evaluated image noise quantitatively (n = 244 regions, Student’s t-test) and also rated image quality subjectively (1–excellent to 6–inadequate, Mann-Whitney U-test). Additionally, three patients during prolonged CPR underwent CT with A-CC devices. Mean image noise of S was increased by 1.21 using L-bp, by 3.62 using A, and by 5.94 using L (p < 0.01 each). Image quality was identical using S and L-bp (1.64 each), slightly worse with A (1.83), and significantly worse with L (2.97, p < 0.001). In all patient cases the main lesions were identified, which led to clinical key decisions. Image quality was excellent with L-bp and good with A. Under CPR conditions initial cases indicate that MDCT diagnostics supports either focused treatment or the decision to terminate efforts.  相似文献

The purpose of this study was to compare sensitivity for detection of pulmonary nodules in MDCT scans and reading time of radiologists when using CAD as the second reader (SR) respectively concurrent reader (CR). Four radiologists analyzed 50 chest MDCT scans chosen from clinical routine two times and marked all detected pulmonary nodules: first with CAD as CR (display of CAD results immediately in the reading session) and later (median 14 weeks) with CAD as SR (display of CAD markers after completion of first reading without CAD). A Siemens LungCAD prototype was used. Sensitivities for detection of nodules and reading times were recorded. Sensitivity of reading with CAD as SR was significantly higher than reading without CAD (p < 0.001) and CAD as CR (p < 0.001). For nodule size of 1.75 mm or above no significant sensitivity difference between CAD as CR and reading without CAD was observed; e.g., for nodules above 4 mm sensitivity was 68% without CAD, 68% with CAD as CR (p = 0.45) and 75% with CAD as SR (p < 0.001). Reading time was significantly shorter for CR (274 s) compared to reading without CAD (294 s; p = 0.04) and SR (337 s; p < 0.001). In our study CAD could either speed up reading of chest CT cases for pulmonary nodules without relevant loss of sensitivity when used as CR, or it increased sensitivity at the cost of longer reading times when used as SR.  相似文献

Identifying inflammation- or fibrosis-predominant strictures in Crohn’s disease (CD) is crucial for treatment strategies. We evaluated the additive value of magnetisation transfer (MT) to conventional MRI for differentiating CD strictures using surgical histopathology as a reference standard. Twenty-eight consecutive CD patients who underwent MRI preoperatively were recruited. MRI parameters included T2-weighted imaging (T2WI) hyperintensity, bowel wall thickness, enhancement pattern changes over time, enhancement pattern and gain ratio in dynamic contrast-enhanced phases, and MT ratio. Correlation analysis was performed using Spearman’s rank test. Receiver operating characteristic curve analysis and Cohen’s κ were used. A model with combined MRI variables characterising intestinal strictures was proposed and validated in 14 additional CD patients. Significant correlations with histological inflammation scores were shown for wall thickness (r = 0.361, p = 0.001) and T2WI hyperintensity (r = 0.396, p < 0.001), whereas histological fibrosis scores were significantly correlated with MT ratio (r = 0.681, p < 0.001) and wall thickness (r = 0.461, p < 0.001). T2WI hyperintensity could differentiate mild from moderate-to-severe inflammation with a sensitivity of 0.871 and a specificity of 0.800. MT ratio could discriminate mild from moderate-to-severe fibrosis with a sensitivity and a specificity of 0.913 and 0.923, respectively. Combining MT ratio and T2WI hyperintensity, the MRI classification moderately agreed with the pathological stricture classification (p < 0.01, κ = 0.549). In the validation set, the diagnostic accuracy of T2WI hyperintensity and MT ratio were 86% and 89%, with good agreement between MRI and histopathological classification (p < 0.01, κ = 0.665). MT ratio combined with conventional MRI improves the differentiation of fibrotic from inflammatory components of small-bowel strictures in CD patients. • MT ratio from magnetisation transfer imaging combined with T2WI from conventional MRI can simultaneously characterise bowel fibrosis and inflammation in adult Crohn’s disease.  相似文献

The purpose of this study was to evaluate the reproducibility of CT-volumetric tumour response assessment of pulmonary metastasis using variable volume change thresholds (VCT) and target lesions with response evaluation criteria in solid tumours (RECIST). Fifty consecutive patients with pulmonary metastases undergoing follow-up multislice CT under chemotherapy were assessed for response to chemotherapy with modifications to RECIST: (1) decreasing the percentual VCT for diagnosis of tumour response (range = 70%–20%), (2) reducing the number of target lesions (range = 1–5). Continuous and categorical observer agreements were tested by Bland and Altman and extended (κe) or non-weighted kappa (κ) and correlated with percentual VCT to predict observer agreement. A total of 202 metastases were evaluated (average volume = 522.4 mm3±902.4 mm3). General agreement on treatment response was very high (κe = 0.93–1), but was reduced with VCT < 35% (κe < 0.95). Kappa correlation with VCT values was strong (r=0.94–0.96; p≤0.0002). Average confidence decreased significantly at VCT < 45% (p < 0.01) and agreement on stable disease at VCT < 35% (κe < 0.95; p < 0.01). Reduction of target lesions (n < 3; VCT = 35%) resulted in decreased reader confidence (for n = 1: κ = 0.49; p < 0.05). Agreement for evaluation of treatment response was robust using VCT ≥35% and ≥3 metastases. This may translate into shortening of follow-up intervals or enable for response assessment with tumours displaying minimal volume change.  相似文献

The purpose of this study was to evaluate the diagnostic efficacy of magnetic resonance (MR)-guided biopsy of focal liver lesions within a short, wide-bore 1.5-T MR system and to determine the duration and accuracy of needle placement using MR fluoroscopy guidance in 25 patients. Accuracy of needle placement was evaluated in two orthogonal planes, and the out-of-plane angle of needle deflection was measured. Needle positioning was characterised subjectively as centred, peripheral, or exterior relative to the lesion. Exterior positioning was corrected by a step-by-step procedure. Surgical resection (n = 6), previous histologies (n = 8), or clinical/radiological follow-up (n = 11) served as the ‘gold standard’. The guidance needle could be placed successfully using MR fluoroscopy in 20 of 25 patients (80%). Needle placement was rated as ‘centred’ in 11 and as ‘peripheral’ in nine patients. Median needle deflection was 2.6 degrees, with a median deviation of 3.4 mm. In five patients, the direct approach failed or was rated as ‘exterior’; therefore, repositioning after needle stabilisation with a stainless-steel stylet was necessary. The diagnostic yield of all biopsies was: sensitivity 95.5%, specificity 100.0% and accuracy 96.0%. In conclusion, MR-guided biopsies in a short, wide-bore MR system yielded highly reliable biopsy results, and in most cases the direct approach with MR fluoroscopy guidance proved to be fast and accurate.  相似文献

The main objective of this study was to assess the quality of CT pulmonary angiography (CTPA) for suspected pulmonary embolus (PE) in the pregnant population. We retrospectively identified 40 consecutive pregnant patients who underwent CTPA from January 2005 to December 2006. Forty consecutive age-matched non-pregnant women were used as a control group. Studies were subjectively graded according to overall image quality by two readers in consensus, in randomised and blinded manner. Moreover, contrast enhancement of pulmonary arteries was subjectively and objectively evaluated. The proportion of sub-optimal studies was more than three times higher in the pregnant group (27.5%, n = 11) compared with the non-pregnant group (7.5%, n = 3; p = 0.015). Mean contrast enhancement was consistently higher in the non-pregnant group compared with pregnant group, both subjectively and objectively. The percentage of inadequately opacified vascular segments was more than two times higher in the pregnant group (28.7%, n = 264) than in the non-pregnant group (13.3%, n = 122; p = 0.0001). The incidence of sub-optimal CTPA studies is higher in pregnancy when compared with an age-matched non-pregnant control group. In addition to radiation issues, this should also be considered when implementing diagnostic strategies for suspected PE in pregnancy.  相似文献

In computed tomography(CT) several contrast media with different iodine concentrations are available. The aim of this study is to prospectively compare contrast media with iodine concentrations of 300, 370 and 400 mg iodine/ml for chest- CT. 300 consecutive patients were prospectively enrolled, under a waiver of the local ethics committee. The first (second, third) 100 patients, received contrast medium with 300(370, 400)mg iodine/ml. Injection protocols were adapted for an identical iodine delivery rate(1.3 mg/s) and total iodine load(33 g) for all three groups. Standardized MDCT of the chest (16 × 0.75 mm, 120 kVp, 100 mAseff.) was performed. Intravascular attenuation values were measured in the pulmonary trunk and the ascending aorta; subjective image quality was rated on a 3-point-scale. Discomfort during and after injection was evaluated. There were no statistically significant differences in contrast enhancement comparing the three contrast media at the pulmonary trunk(p = 0.3198) and at the ascending aorta(p = 0.0840). Image quality(p = 0.0176) and discomfort during injection(p = 0.7034) were comparable for all groups. General discomfort after injection of contrast media with 300 mg iodine/ml was statistically significant higher compared to 370 mg iodine/ml(p = 0.00019). Given identical iodine delivery rates of 1.3 g/s and iodine loads of 33 g, contrast media with concentrations of 300, 370 and 400 mg iodine/ml do not result in different intravascular enhancement in chest-CT.  相似文献

Simon’s bleedings are stripe-like hemorrhages on the ventral surface of the intervertebral disks of the lumbar part of the spinal column. The aims of this study were to determine the appearance frequency of Simon’s bleedings in cases of hanging and in other cases of asphyxiations and to determine if the age of the deceased was in correlation with the occurrence of Simon’s bleedings. A prospective autopsic study included 147 cases of hanging, 39 other asphyxiation deaths, and 461 deaths other than asphyxiation (blunt trauma, natural deaths, etc.). Simon’s bleedings were present in 62.8% cases of hanging and in 61.5% cases of other types of asphyxiations. Simon’s bleedings are not specific for hanging (χ 2 = 0.022, p > 0.05). Simon’s bleedings were less frequent in other cases. It was established that the older the person was, the possibility of Simon’s bleedings to occur would be less (Spearman’s correlation coefficient = −0.225, p < 0.001; Wald coefficient = 29.798, p < 0.001). In the cases of hanging, there is statistically significant difference in average age between the groups with and without Simon’s bleedings (t = 2.875, p = 0.017). The older the person was, the lower the likelihood of Simon’s bleedings to occur: if the person was more than 60 years old, there was 70% probability of not having Simon’s bleedings, and if older than 70, this probability would rise to 88% (Wald coefficient = 7.609, p = 0.021). In older persons who died due to hanging, throat skeleton fractures accompanied by local hemorrhage could be considered as a vital sign. In younger persons, where throat skeleton fractures are less frequent, Simon’s bleedings could be the vital sign of premortem hanging. Simon’s bleedings, in cases of asphyxiation, most likely occur due to agonal convulsions and forced movements in lumbosacral part of spinal column. Additional factor for the appearance of Simon’s bleedings in hanging is traction of body and especially this part of spinal column due to gravity.  相似文献

This study aimed at evaluating the diagnostic benefits of maximum intensity projections (MIP) and a commercially available computed-assisted detection system (CAD) for the detection of pulmonary nodules on MDCT as compared with standard 1-mm images on lung cancer screening material. Thirty subjects were randomly selected from our database. Three radiologists independently reviewed three types of images: axial 1-mm images, axial MIP slabs, and CAD system detections. Two independent experienced chest radiologists decided which were true-positive nodules. Two hundred eighty-five nodules ≥1 mm were identified as true-positive by consensus of two independent chest radiologists. The detection rates of the three independent observers with 1-mm axial images were 22 ± 4.8%, 30 ± 5.3%, and 47 ± 2.8%; with MIP: 33 ± 5.4%, 39 ± 5.7%, and 45 ± 5.8%; and with CAD: 35 ± 5.6%, 36 ± 5.6%, and 36 ± 5.6%. There was a reading technique effect on the observers’ sensitivity for nodule detection: sensitivities with MIP were higher than with 1-mm images or CAD for all nodules (F-values = 0.046). For nodules ≥3 mm, readers’ sensitivities were higher with 1-mm images or MIP than with CAD (p < 0.0001). CAD was the most and MIP the less time-consuming technique (p < 0.0001). MIP and CAD reduced the number of overlooked small nodules. As MIP is more sensitive and less time consuming than the CAD we used, we recommend viewing MIP and 1-mm images for the detection of pulmonary nodules. The purpose of this retrospective study was to evaluate the impact of energy subtraction (ES) chest radiography on the detection of pulmonary nodules and masses in daily routine. Seventy-seven patients and 25 healthy subjects were examined with a single exposure digital radiography system. Five blinded readers evaluated first the non-subtracted PA and lateral chest radiographs alone and then together with the subtracted PA soft tissue images. The size, location and number of lung nodules or masses were registered with the confidence level. CT was used as standard of reference. For the 200 total lesions, a sensitivity of 33.5–52.5% was found at non-subtracted and a sensitivity of 43.5–58.5% at energy-subtracted radiography, corresponding to a significant improvement in four of five readers (p < 0.05). However, in three of five readers the rate of false positives was higher with ES. With ES, sensitivity, but not the area under the alternative free-response receiver operating characteristics (AFROC) curve, showed a good correlation with reader experience (R = 0.90, p = 0.026). In four of five readers, the diagnostic confidence improved with ES (p = 0.0036). We conclude that single-exposure digital ES chest radiography improves detection of most pulmonary nodules and masses, but identification of nodules <1 cm and false-positive findings remain a problem.  相似文献

The aim of this study was to assess the performance of a motion-map algorithm that automatically determines optimal reconstruction windows for dual-source coronary CT angiography. In datasets from 50 consecutive patients, optimal systolic and diastolic reconstruction windows were determined using the motion-map algorithm. For manual determination of the optimal reconstruction window, datasets were reconstructed in 5% steps throughout the RR interval. Motion artifacts were rated for each major coronary vessel using a five-point scale. Mean motion scores using the motion-map algorithm were 2.4 ± 0.8 for systolic reconstructions and 1.9 ± 0.8 for diastolic reconstructions. Using the manual approach, overall motion scores were significantly better (1.9 ± 0.5 and 1.7 ± 0.6, p < 0.05), but diagnostic image quality was reached in >90% of cases using either approach. Using the automated approach, there was a negative correlation between heart rate and motion scores for systolic reconstructions (ρ = −0.26, p < 0.05) and a positive correlation for diastolic reconstructions (ρ = 0.46, p < 0.01). For the manual approach, no significant correlation was found for systolic reconstructions (ρ = −0.1, p = 0.52), while there was a positive correlation for diastolic reconstructions (ρ = 0.48, p < 0.01). Thus, the motion-map algorithm is a useful tool to save time in finding an appropriate reconstruction window in patients with heart rates <70 bpm (diastolic reconstruction) and >80 bpm (systolic reconstruction).  相似文献

Some authors have suggested that in the fetus, neonate and infant, intradural hemorrhage (IDH) is relatively common and often presents alongside subdural hemorrhage (SDH). These authors have theorized that pediatric SDH may result from an IDH due to blood leakage from a dural vascular plexus. In this study, we report the inter-observer variation for detection of IDH from a retrospectively collected series of pediatric autopsy photographs, with and without SDH. Autopsy photographs of the falx and tentorium from 27 neonatal, infant and early childhood autopsies were assessed by two independent consultant forensic pathologists blinded to all case histories for the presence and extent (focal or diffuse) of IDH. Inter-observer agreement between the pathologists was calculated using Cohen’s kappa coefficient. The occurrence of subdural hemorrhage was also recorded at autopsy. A kappa coefficient value of 0.669 (p = 0.001), indicated a substantial level of agreement for the presence/absence of IDH between the pathologists. For the extent of IDH a kappa coefficient value of 0.6 (p = 0.038) indicated a moderate level of agreement. The pathologists agreed on the presence of IDH in 10 of the 27 cases. Subdural hemorrhage was recorded for 8 out of 27 cases. Of these 8 cases, it was agreed that 4 had IDH. Using standardized methods of image capture and assessment, inter-observer agreement for the presence/absence of IDH was substantial. In this paper, we report a much lower frequency of macroscopic IDH occurring alongside SDH than previous studies, which included both gross observation of IDH and histological examination.  相似文献

To evaluate the time course of reversed remodeling after pulmonary endarterectomy (PEA) in patients with chronic thromboembolic pulmonary hypertension(CTPEH), we studied 22 patients (age: 60 ± 13 years) with MRI immediately before, 1 month, 3 months, and 6 months after PEA. MRI included assessment of biventricular function, aortic and pulmonary artery(PA) flow, and right ventricular (RV) overload using the ratio of RV-to-biventricular diameter. Except in one patient, who died 2 months post-surgery, clinical improvement occurred early after PEA (NYHA class: 3.3 ± 0.6 to 1.5 ± 0.8, p < 0.0001) with a decrease of systolic pulmonary artery pressures (79 ± 14 to 44 ± 14 mmHg, p < 0.0001). At 1 month post PEA, RV end-diastolic volumes decreased (198 ± 72 to 137 ± 59 ml, p < 0.0001), and the RV ejection fraction (EF) improved (31 ± 9 to 47 ± 10%, p < 0.0001). No further significant improvement in pulmonary pressures or RV function occurred at 3 months or 6 months. Although no significant change was found in LV volumes or function, aortic flow increased early after surgery. PEA had only a beneficial effect on right PA flow. RV overload decreased early after PEA (ratio RV-to-biventricular diameter: before: 0.67 ± 0.04, after: 0.54 ± 0.06, p < 0.0001), showing a good correlation with the improvement in RVEF (r = 0.7, P < 0.0001). In conclusion, reversed cardiac remodeling occurs early after PEA, to slow down after 1 month. At 6 months, cardiac remodeling is incomplete as witnessed by low-normal RV function and residually elevated PA pressures.  相似文献

The aim of the study was to compare widely used ordered subset expectation maximisation (OSEM) algorithm with a new Bayesian penalised likelihood (BPL) Q.Clear algorithm in 18F-PSMA-1007 PET/CT. We retrospectively assessed 25 18F-PSMA-1007 PET/CT scans with both OSEM and Q.Clear reconstructions available. Each scan was independently reported by two physicians both in OSEM and Q.Clear. SUVmax, SUVmean and tumour-to-background ratio (TBR) of each lesion were measured. Reports were also compared for their final conclusions and the number and localisation of lesions. In both reconstructions the same 87 lesions were reported. Mean SUVmax, SUVmean and TBR were higher for Q.Clear than OSEM (7.01 vs 6.53 [p = 0.052], 4.16 vs 3.84 [p = 0.036] and 20.2 vs 16.8 [p < 0.00001], respectively). Small lesions (< 10 mm) had statistically significant higher SUVmax, SUVmean and TBR in Q.Clear than OSEM (5.37 vs 4.79 [p = 0.032], 3.08 vs 2.70 [p = 0.04] and 15.5 vs 12.5 [p = 0.00214], respectively). For lesions ≥ 10 mm, no significant differences were observed. Findings with higher tracer avidity (SUVmax ≥ 5) tended to have higher SUVmax, SUVmean and TBR values in Q.Clear (11.6 vs 10.3 [p = 0.00278], 7.0 vs 6.7 [p = 0.077] and 33.9 vs 26.7 [p < 0.00001, respectively). Mean background uptake did not differ significantly between Q.Clear and OSEM (0.42 vs 0.39, p = 0.07). In 18F-PSMA-1007 PET/CT, Q.Clear SUVs and TBR tend to be higher (regardless of lesion localisation), especially for small and highly avid lesions. Increase in SUVs is also higher for lesions with high tracer uptake. Still, Q.Clear does not affect 18F-PSMA-1007 PET/CT specificity and sensitivity.  相似文献

The purpose of this study was to determine the clinical implication of the myocardial FDG uptake patterns by comparing with the results of stress myocardial perfusion imaging (MPI) as the reference standard. By reviewing the medical records, 86 pairs of stress MPI and FDG PET/CT of 84 patients who underwent stress MPI and oncologic FDG PET/CT in 1 month were included in this study. The patterns of the myocardial FDG uptake were classified into five patterns such as ‘low’, ‘diffuse’, ‘basal ring’, ‘focal high’, and ‘focal defect on diffuse high’. MPI was evaluated using a 5-point scoring model ranging from 0 (normal uptake) to 4 (uptake absent) based on the 17-segment model. The summed stress score of 4 or higher was defined as ‘abnormal MPI’. Factors predictive of abnormal MPI were analyzed using a log-rank multivariate test and p < 0.05 was set as significant. Abnormal MPI was observed in only 16 of 36 (44%) patients with ‘low’ pattern, 10 of 23 (43%) patients with ‘diffuse high’ pattern, and 1 of 9 (11%) patients with ‘basal ring’ pattern, but in 8 of 9 (89%) patients with ‘focal high’ pattern, and 8 of 9 (89%) patients with ‘focal defect on diffuse high’ pattern. The log-rank multivariate test revealed that ‘focal high’ and ‘focal defect on diffuse high’ pattern were correlated with an abnormal MPI. These results indicate that further cardiac work-up might be helpful in the patients with ‘focal high’ pattern or ‘focal defect on diffuse high’ pattern of myocardial FDG at oncologic PET. A prospective study should be needed to further support this conclusion.  相似文献

Retrospective evaluation of computed tomographic (CT) pyelography before percutaneous nephrostolithotomy (PCNL). Twenty patients with renal calculi underwent CT pyelography using a dedicated protocol. Calculus size, uniformity of contrast excretion and accuracy of calculus and pelvicalyceal (PC) system reconstructions were scored and compared on axial and coronal maximum intensity projections (MIP) and volume reconstructions (VRmovie loops). After contrast medium administration, the size of calculi is accurate on axial images, but underestimated on coronal studies: mean 14.7 mm vs. 14.4 mm (axial) and 17.2 mm vs. 16.1 mm (coronal) for measurements before and after enhancement, respectively (p = 0.11 and 0.03). Uniform contrast medium excretion (median 228 HU; 95% CI 209–266 HU) was sufficiently lower than calculus density (median 845 HU; 95% CI 457-1,193 HU) for precise calculus and pelvicalyceal reconstructions in 87% and 85%, respectively. Coronal MIP scans were rated best for calculus depiction (mean score 2.68 vs. 2.50 and 2.41 for coronal, axial and VRs, respectively; p = 0.14) and VR studies best for PC anatomy (mean score 4.4 vs. 3.73 and 2.89 for VR, coronal and axial studies, respectively; p = <0.0001). Breast screening specificity is improved if previous mammograms are available, which presents a challenge when converting to digital mammography. Two display options were investigated: mounting previous film mammograms on a multiviewer adjacent to the workstation, or digitising them for soft copy display. Eight qualified screen readers were videotaped undertaking routine screen reading for two 45-min sessions in each scenario. Analysis of gross eye and head movements showed that when digitised, previous mammograms were examined a greater number of times per case (p = 0.03), due to a combination of being used in 19% more cases (p = 0.04) and where used, looked at a greater number of times (28% increase, p = 0.04). Digitising previous mammograms reduced both the average time taken per case by 18% (p = 0.04) and the participants’ perceptions of workload (p < 0.05). Digitising previous analogue mammograms may be advantageous, in particular in increasing their level of use.  相似文献

Progressive multifocal leukoencephalopathy (PML) is a severe demyelinating disease of the central nervous system due to JC polyoma virus infection of oligodendrocytes. PML develops in patients with impaired T-cell function as occurs in HIV, malignancy or immunosuppressive drugs users. Until now no imaging methods have been reported to correlate with clinical status. Diffusion-weighted imaging (DWI) is a robust MRI tool in investigating white matter architecture and diseases. The aim of our work was to assess diffusion abnormalities in focal white matter lesions in patients with PML and to correlate the lesion load measured with conventional MRI and DWI to clinical variables. We evaluated eight patients with a biopsy or laboratory-supported diagnosis of PML. All patients underwent MRI including conventional sequences (fluid attenuated inversion recovery-FLAIR) and DWI. Mean diffusivity (MD) maps were used to quantify diffusion on white matter lesions. Global lesion load was calculated by manually tracing lesions on FLAIR images, while total, central core and peripheral lesion loads were calculated by manually tracing lesions on DWI images. Lesion load obtained with the conventional or DWI-based methods were correlated with clinical variables such as disease duration, disease severity and survival. White matter focal lesions are characterized by a central core with low signal on DWI images and high MD (1.853 × 10−3 mm2/s), surrounded by a rim of high signal intensity on DWI and lower MD (1.1 × 10−3 mm2/s). The MD value of normal-appearing white matter is higher although not statistically significant (0.783 × 10−3 mm2/s) with respect to control subjects (0.750 × 10−3 mm2/s). Inter-rater correlations of global lesion load between FLAIR (3.96%) and DWI (3.43%) was excellent (ICC =0.87). Global lesion load on FLAIR and DWI correlates with disease duration and severity (respectively, p = 0.037, p = 0.0272 with Karnofsky scale and p = 0.0338 with EDSS on FLAIR images; p = 0.043, p = 0.0296 with Karnofsky scale and p = 0.0365 with EDSS on DW images). Central core lesion load on DWI correlates with disease duration and severity (respectively p = 0.043, p = 0.0103 with Karnofsky scale and p = 0.0112 with EDSS), while peripheral lesion load does not correlate with any clinical variable. The global lesion load in PML correlates with disease duration and severity. DWI images, which can distinguish within lesions a central core from a peripheral rim, reveal that a larger central core component correlates to a worsened clinical status and longer disease duration. On the other hand the peripheral rim lesion load visualized on DWI images does not correlate with clinical variables and does not achieve obtaining further prognostic information with respect to conventional imaging.  相似文献

We assessed with computed tomography (CT) densitometry the prevalence of emphysema in 266 (175 men and 91 women; mean age 64 ± 4 years) smokers and former smokers enrolled in the ITALUNG trial of lung cancer screening with low-dose thin-slice CT. Whole-lung volume and the relative area at −950 Hounsfield units (RA950) and mean lung attenuation (MLA) in 1 of every 10 slices (mean, 24 slices per subject) were measured. Lung volume, MLA and RA950 significantly correlated each other and with age. Average RA950 >6.8% qualifying for emphysema was present in 71 (26.6%) of 266 subjects, with a higher prevalence in men than in women (30.3% vs 19.8%; p = 0.003). Only in smokers was a weak (r = 0.18; p = 0.05) correlation between RA950 and packs/year observed. In multiple regression analysis, the variability of RA950 (R2 = 0.24) or MLA (R2 = 0.34) was significantly, but weakly explained by age, lung volume and packs/year. Other factors besides smoking may also have a significant role in the etiopathogenesis of pulmonary emphysema.
