目的:分析57例氩激光光凝治疗视网膜静脉阻塞(retinal vein occlusion, RVO)的临床疗效。方法:随访我院57例被确诊为视网膜静脉阻塞并经氩激光光凝治疗后的患者,观察患者的视力、眼底改变以及并发症情况并评价氩激光光凝治疗视网膜静脉阻塞的临床价值。激光治疗方法分为黄斑区格栅样光凝,局灶性视网膜光凝及全视网膜光凝。结果:经氩绿激光光凝治疗57例后3~6mo给予复查:末次随访视力提高23眼,视力无明显变化25眼,视力下降9眼;激光治疗后复查荧光素眼底血管造影 (观察视网膜毛细血管无灌注区面积变化及新生血管消退情况),治疗有效54眼, 3眼发展为新生血管性青光眼(NVG)。而激光光凝对于黄斑部晚期并发症无明显效果。结论:激光光凝治疗可提高中心视力,可促进视网膜水肿、出血、渗出的吸收,同时减少视网膜静脉阻塞引起的新生血管,对于预防并延缓增殖性玻璃体视网膜病变和继发性新生血管性青光眼的发生有明显效果。  相似文献

目的:通过对视网膜母细胞瘤(retinoblastoma,RB)患者实施眼动脉化疗,观察化疗效果及副反应。方法:根据RB最新国际分期,对11例11眼A~D期RB患者,通过股动脉插管至眼动脉,使用马法兰(melphalan)经眼动脉灌注,对RB患者实施动脉化疗。结果:所选患者共实施眼动脉灌注化疗23次,随访3~12mo,其中8例肿瘤明显缩小、钙化,2例病情进展,实施眼球摘除,1例失访。除部分患者术后短暂的恶心、呕吐、发热,没有出现死亡、中风、骨髓抑制等严重并发症。结论:所选患者中,A、B、C期9例,7例有效,1例继续进展,1例失访,有效率88%;2例D期患者1例有效,1例继续进展,有效率50%。  相似文献

目的:观察和血明目片联合氪激光治疗视网膜静脉阻塞(retinal vein occlusion,RVO)的疗效。方法:选择经眼底血管荧光造影(FFA)、光学相干断层扫描(OCT)确诊的57例60眼视网膜静脉阻塞患者,随机分为治疗组29例30眼采用和血明目片联合氪激光光凝治疗。对照组28例30眼采用单纯氪激光光凝治疗。观察两组治疗前后视力、眼底和FFA,全部病例随访6mo。结果:氪激光联合中成药和血明目片治疗视网膜静脉阻塞较对照组视力改善明显,出血吸收快(P<0.05)。结论:在视网膜静脉阻塞行氪激光光凝时联合中成药和血明目片治疗,能较好的促进视网膜出血的吸收,有效的改善患者视功能。  相似文献

目的:观察氩激光视网膜光凝治疗视网膜静脉周围炎(Eales病)的效果。方法:对经荧光素眼底血管造影(FFA)检查确诊为Eales病患者28例52眼,按眼底病变区域行氩激光光凝,每周一次,分别为1~4次,3mo后FFA复查,按眼底病变好转与否决定是/否补充激光光凝,6mo后再行FFA检查;追踪复查30例50眼药物治疗Eales病患者的治疗效果,并行FFA检查。比较氩激光视网膜光凝与药物治疗的治疗前、后视力、眼底变化,评价治疗效果。结果:氩激光视网膜光凝治疗Eales病28例52眼,有效49眼(94%),无效3眼(5.8%)。药物治疗Eales病30例50眼,有效30眼(60.0%),无效20眼(40.0%)。结论:氩激光视网膜光凝治疗Eales病效果优于药物治疗。FFA检查对Eales病的发病机制、诊断、鉴别诊断、氩激光视网膜光凝、早期治疗及疗效观察提供重要的指导依据。  相似文献

目的探讨倍频Nd:YAG532激光光凝治疗视网膜静脉阻塞新生血管的方法和疗效。方法对26例视网膜静脉阻塞伴新生血管的31眼采用倍频Nd:YAG532激光进行视网膜光凝治疗。光凝后经3月~2.5a随访,对比分析光凝前后的视力变化及眼底改变。结果有效21眼(68%),好转7眼(22%),无效3眼(10%)。视力提高19眼(61%)。结论倍频Nd:YAG532激光光凝术对视网膜静脉阻塞新生血管的消退及预防玻璃体积血具有显著疗效。  相似文献

目的:观察半导体激光视网膜光凝术治疗增殖前期和增殖期糖尿病视网膜病变(diabeticretinopathy,DR)疗效。方法:对50例(92眼,其中增殖前期30眼,增殖期62眼)DR患者,依病变程度分别行次全视网膜光凝及全视网膜光凝治疗,术后随访视力、眼底、眼底荧光血管造影(FFA)。结果:光凝治疗后,视力增进25眼(27%),视力无变化48眼(52%),视力下降19眼(21%),FFA结果有效76眼,有效率83%,需补充光凝4眼。结论:半导体激光全视网膜光凝治疗糖尿病视网膜病变不仅对屈光间质清晰者效果好,而且对屈光间质混浊的病例效果也较满意。  相似文献

目的 探讨氩激光视网膜光凝联合中药活血化瘀治疗视网膜中央静脉阻塞(CRVO)的临床疗效.方法 选择88例(88只眼)CRVO患者随机分为A、B组,A组应用氩激光视网膜光凝治疗,B组应用氩激光视网膜光凝治疗同时,应用活血化瘀中药,观察两组患者治疗前后视力变化情况.结果 A组45例(45只眼)经氩激光视网膜光凝治疗后,77.7%视力提高,B组43例(43只眼)经氩激光视网膜光凝联合中药-活血化瘀治疗后,95.4%视力提高.结论 氩激光视网膜光凝联合中药活血化瘀治疗CRVO可取得良好疗效.  相似文献

PURPOSE: Because of the long-term complications associated with external beam radiation in retinoblastoma, alternative treatment methods have been investigated. We conducted a retrospective study to evaluate the functional results of new treatment modalities. METHODS: Thirty-seven eyes were treated without external beam irradiation in 31 patients. The median diameter of the largest tumor in each eye was 6 mm. Primary chemotherapy was used in 25 cases, chemothermotherapy was used in 32 cases, cryotherapy was used in 28 cases, iodine 125 Plaques were used in 15 cases, diode laser thermotherapy was used alone in 11 cases, and photocoagulation was performed in 5 cases. The median follow-up after diagnosis of retinoblastoma was 41 months. The visual results were evaluated at a median age of 54 months. RESULTS: The median visual acuity of the treated eyes was 20/33. Twenty-four eyes presented a visual acuity better than 20/40, 4 eyes had a visual acuity between 20/200 and 20/40, and 9 eyes had a visual acuity less than 20/200. Maculopathy was observed in 16 cases, associated with papillopathy in 1 case. A cataract was observed in 1 case and a vitreous hemorrhage was observed in another case. Twenty-one eyes did not develop any complications. No corneal dryness and very few lens changes were observed. CONCLUSION: The functional results after local treatments for retinoblastoma are very good. The most frequent complication is maculopathy, particularly when the tumor involves or is situated close to the macula.  相似文献

The management of retinoblastoma (RB) has dramatically changed over the past two decades from previous radiotherapy methods to current chemotherapy strategies. RB is a remarkably chemotherapy-sensitive tumor. Chemotherapy is currently used as a first-line approach for children with this malignancy and can be delivered by intravenous, intra-arterial, periocular, and intravitreal routes. The choice of route for chemotherapy administration depends upon the tumor laterality and tumor staging. Intravenous chemotherapy (IVC) is used most often in bilateral cases, orbital RB, and as an adjuvant treatment in high-risk RB. Intra-arterial chemotherapy (IAC) is used in cases with group C or D RB and selected cases of group E tumor. Periocular chemotherapy is used as an adjunct treatment in eyes with group D and E RB and those with persistent/recurrent vitreous seeds. Intravitreal chemotherapy is reserved for eyes with persistent/recurrent vitreous seeds. In this review, we describe the various forms of chemotherapy used in the management of RB. A database search was performed on PubMed, using the terms “RB,” and “treatment,” “chemotherapy,” “systemic chemotherapy,” “IVC,” “IAC,” “periocular chemotherapy,” or “intravitreal chemotherapy.” Relevant English language articles were extracted, reviewed, and referenced appropriately.  相似文献

目的 分析总结氩离子激光治疗黄斑裂孔的疗效。方法 采用氩绿激光治疗36例36眼黄斑裂孔,其中13眼为玻璃体切割气液交换术后的补充光凝,治疗后观察孔封闭情况及我涨前后的视力变化。结果 经1次或光凝有33例裂孔封闭,治愈为91.7%,其中1次光台愈率为69.4%,激光治疗前后视力无显著差异。结论 氩绿激光治疗黄斑裂孔儿确切,对视力功能的损害轻快。  相似文献

目的 探讨玻璃体手术治疗眼内晚期视网膜母细胞瘤的效果。方法 2012年7月至2013年6月,选择眼内晚期(D期或E期)视网膜母细胞瘤患儿11例(12眼),采用博士伦23G玻璃体切割系统,进行玻璃体视网膜手术,包括玻璃体切割、肿瘤切除、视网膜光凝、眼内填充等。手术后随访6~18个月(平均9.4个月)。结果 所有患眼均完成视网膜肿物切除。术后10眼视网膜裂孔封闭、视网膜复位;2眼因肿瘤范围大,视网膜大部分切除。8眼硅油填充,2眼行C3F8填充,2眼未作眼内填充。随访末,病情稳定者8眼,肿瘤复发4眼,眼球摘除1眼,因肿瘤眼外转移去世1例,2眼再行玻璃体内注射马法兰。硅油取出3眼,硅油置换1眼,4眼硅油长期填充。视力保持0.2以上5眼,患儿因年龄小不配合视力检查者7眼。并发白内障2眼,行白内障摘出术。结论 玻璃体手术能够有效切除眼内晚期视网膜母细胞瘤,对保留眼球和患眼视力,提高患儿生存率有一定帮助,但其安全性和长期疗效还有待进一步研究。  相似文献

Nineteen eyes of 17 patients were examined for complications associated with a retinal macroarterial aneurysm. 13 of 17 patients were feminine and over 70 years of age and 15 had longstanding systemic hypertension. Visual loss associated with retinal macroarterial aneurysms was related to macular hemorrhage, exudate, or serous retinal detachment. Twelve eyes had retinal hemorrhages associated with the macroarterial aneurysm. Hemorrhages were subretinal and/or preretinal of which some diffused into the vitreous. Seven eyes were treated by direct argon laser photocoagulation of the aneurysm giving an arterial occlusion. Five eyes received perianeurysmal photocoagulation without occlusion of the arteriole. Five eyes received no laser treatment because their exudate or hemorrhage was not threatening the fovea and these evolved towards cicatrisation without a drop in visual acuity. Improvement in visual acuity was noted with both types of photocoagulation. But, direct photocoagulation was found less preferable since an absolute scotoma may be created by the area deprived of arterial flow. Perianeurysm photocoagulation was found to be effective, and is indicated in cases of deep exudate and edema that may threaten the fovea. Treatment is questionable when hemorrhage is the predominant complication.  相似文献

目的:探讨玻璃体注射雷珠单抗联合物理治疗和手术治疗Coats病的效果.方法:选取2013-05/2016-05在我院治疗的Coats病患者13例13眼,所有患眼给予玻璃体腔注射雷珠单抗治疗,同时联合巩膜外放液、激光光凝等治疗,观察患眼视力及视网膜复位情况.结果:选取的13眼中,玻璃体腔注射雷珠单抗联合巩膜外放液治疗4眼,联合激光光凝治疗3眼,联合玻璃体切除、剥膜、冷冻、硅油填充等治疗6眼,眼球保留率100%;患者治疗后6mo视力明显较治疗前改善,差异有统计学意义(P<0.05),视力比较:0.05~0.1、0.01~<0.05、指数、光感和手动眼数比例术前为0、0、38%、38%和23%,术后6mo为23%、23%、38%、15%和0;视网膜完全复位5眼(38%);基本复位3眼(23%);未复位5眼(38%).结论:玻璃体腔注射雷珠单抗联合物理治疗和手术治疗Coats病有一定效果,能提高或保持患者有限的视功能,避免因病变进展摘除眼球.  相似文献

目的:评估532nm激光治疗糖尿病视网膜病变(diabetic retinopathy,DR)的治疗效果。方法:应用532nm激光对糖尿病患者48例96眼[包括增殖前期糖尿病视网膜病变(PPDR)54眼,增殖期糖尿病视网膜病变(PDR)42眼(包括高危PDR20眼,非高危PDR22眼),合并黄斑水肿21眼]行视网膜光凝术。根据DR分期及病变严重程度行次全视网膜光凝(Sub-PRP)、标准全视网膜光凝(S-PRP)和超全视网膜光凝(E-PRP)及局限光凝和格栅样光凝,随访3mo~4a,观察视力和视网膜病变进展情况。必要时再补充激光治疗。结果:视力:提高或不变为有效,其中PPDR组有效45眼(83%),非高危PDR组有效18眼(82%),高危PDR组有效12眼(60%)。视网膜病变进展情况:视网膜水肿、渗出、出血部吸收,视网膜新生血管部分或全部消退为有效,其中PPDR组有效50眼(93%),非高危PDR组有效19眼(86%),高危PDR组有效13眼(65%)。结论:倍频532nm激光治疗DR安全有效,早期病变治疗效果好。及早发现和治疗病变,注意光凝方法和技巧,以及随访和跟踪治疗,是提高视网膜光凝疗效的关键。  相似文献

Therapeutic options for capillary papillary hemangiomas   总被引:7,自引:0,他引:7  
PURPOSE: To describe the results and therapeutic complications of treatment of papillary capillary hemangiomas over the last 13 years. DESIGN: Retrospective, noncomparative, interventional case series. PARTICIPANTS: Five cases of unilateral capillary papillary hemangiomas were reviewed. Three patients had von Hippel-Lindau disease, and two had no associated familial or systemic disease. METHODS: Two patients were treated with argon green laser, one patient received argon green and diode photocoagulation, and two patients underwent argon green photocoagulation and diode transpupillary thermotherapy (TTT); two eyes also underwent vitreoretinal surgery. Follow-up ranged from 1 to 13 years. MAIN OUTCOME MEASURES: The presence of exudative retinal detachment associated with the activity of the hemangioma and the visual acuity were the two main parameters. RESULTS: Prompt argon green laser treatment was the most effective therapy; -II was ineffective. Vitreoretinal surgery, transscleral drainage, and argon endolaser photocoagulation in one case resolved bullous retinal detachment secondary to tumor exudation. Pretreatment visual acuity (VA) levels ranged from 20/25 to counting fingers; posttreatment VA levels ranged from 20/25 to light perception. CONCLUSIONS: If left untreated, papillary hemangiomas may evolve to exudative retinal detachment and marked VA decreases. Although we have not established an ideal therapy, we recommend appropriate treatment on diagnosis. Close follow-up and multiple treatments with argon laser are likely the best therapeutic course.  相似文献

A minimally invasive method for repair of rhegmatogenous retinal detachment following treatment of retinoblastoma is described. Two children with retinoblastoma developed rhegmatogenous retinal detachments following treatment with a combination of chemotherapy and cryotherapy. Each eye underwent a non-drainage scleral buckling procedure without retinopexy of the retinal break. Postoperatively, the retinas reattached and retinopexy with indirect laser photocoagulation was performed during subsequent examinations under anesthesia. The retinas have remained attached with 2 to 5 years of follow-up. The tumor recurred in case 2, was treated with additional chemotherapy, and has undergone type 1 regression. Non-drainage scleral buckling without retinopexy is a useful technique for repairing rhegmatogenous retinal detachment in eyes with retinoblastoma. The retinal break can be treated postoperatively with indirect laser photocoagulation to minimize the chance of viable tumor cell dissemination.  相似文献
