目的探讨血管内微弹簧圈栓塞术治疗颅内动脉瘤的临床价值。方法对92例动脉瘤患者实行血管内微弹簧圈栓塞术,其中单纯微弹簧圈栓塞80例,Solitaire支架辅助弹簧圈栓塞12例。结果 92例动脉瘤均成功栓塞,其中完全栓塞67例,大部分栓塞17例,部分栓塞8例,术后死亡2例,皆为术前Hunt-Hess分级Ⅳ级,术中动脉瘤破裂出血,1例为广泛蛛网膜下腔出血,1例为大量额叶脑内血肿。应用Logistic回归分析显示患者年龄、高血压及Hunt-Hess分级是影响动脉瘤预后的危险因素。结论血管内微弹簧圈栓塞术治疗颅内动脉瘤可有效防止动脉瘤破裂出血,有很好的临床应用价值。  相似文献

目的 观察支架辅助弹簧圈栓塞治疗大脑中动脉分叉部宽颈动脉瘤的安全性和有效性。方法 分析60例接受支架辅助弹簧圈栓塞治疗的大脑中动脉分叉部宽颈动脉瘤患者共65个动脉瘤,观察术后即刻及随访动脉瘤栓塞程度,评估术后并发症、疗效及安全性。结果 术中2例(2/60,3.33%)动脉瘤破裂出血,4例(4/60,6.67%)支架内血栓形成;术后即刻栓塞程度为Raymond Ⅰ级50个(50/65,76.92%)、Ⅱ级12个(12/65,18.46%)、Ⅲ级3个(3/65,4.62%)。术后1个月内3例(3/60,5.00%)动脉瘤再出血,2例致死;1例(1/60,1.67%)死于肺栓塞;4例(4/60,6.67%)并发脑梗死。1例术后6个月瘤颈复发,1例术后25个月出现无症状性血管闭塞;末次复查数字减影血管造影显示43个(43/51,84.31%) RaymondⅠ级、8个(8/51,15.69%)Ⅱ级。末次随访,49例(49/57,85.96%)改良Rankin量表评分为0~2分,8例(8/57,14.04%)为3~4分。结论 支架辅助弹簧圈栓塞技术治疗大脑中动脉宽颈动脉瘤安全、有效。  相似文献

目的 探讨Neuroform Atlas支架在颅内动脉分叉处宽颈动脉瘤栓塞中的价值。方法 回顾性分析2021年1~9月16例采用Neuroform Atlas支架辅助弹簧圈栓塞治疗颅内动脉分叉处宽颈动脉瘤的临床和影像学资料。动脉瘤位于大脑中动脉分叉处8例,大脑前动脉分叉处4例,大脑前、中动脉分叉处2例,大脑前动脉A2远端分叉处1例,大脑后动脉P2分叉处1例。破裂动脉瘤10例(术前Hunt-Hess分级Ⅰ级6例,Ⅱ级2例,Ⅲ级2例),未破裂动脉瘤6例。结果 均在单一Neuroform Atlas支架辅助下完成栓塞,技术成功率100%。术后即刻造影显示动脉瘤完全闭塞13例,瘤颈残留2例,瘤体残留1例。术中及围手术期未观察到介入操作相关并发症。出院前改良Rankin量表(mRS)0~1分11例,2分3例,3分2例。16例随访时间3~14个月,(7.8±3.2)月。mRS评分0~1分14例,2分1例,3分1例。9例术后3~6个月行DSA,动脉瘤完全闭塞8例,瘤颈残留1例,9例均未见载瘤动脉狭窄或支架内闭塞。结论 Neuroform Atlas支架辅助弹簧圈栓塞治疗颅内动脉分叉处宽颈动脉瘤安全,...  相似文献

颅内动脉瘤弹簧圈栓塞治疗术中动脉瘤再破裂的防治   总被引:2,自引:0,他引:2  
目的颅内动脉瘤在弹簧圈栓塞过程中发生破裂是最可怕的术中并发症之一,本文探讨处理、预防这一并发症的初步经验。方法2002年4月-2006年12月,共有153例患有颅内动脉瘤的患者在我院接受了可脱卸弹簧圈栓塞治疗,其中141例患者曾有过动脉瘤破裂引起蛛网膜下腔出血史。5例有动脉瘤破裂出血史的患者术中再次发生动脉瘤破裂。术中动脉瘤再破裂时,常规使用鱼精蛋白中和肝素,并设法用弹簧圈尽快填塞动脉瘤腔。微导丝引起动脉瘤破裂时,尽量保持微导丝不动,微导管尽快送到瘤腔中进行填塞治疗。若微导管引起破裂而微导管头端位于瘤壁外蛛网膜下腔时,微导管且勿退入瘤腔内,应将弹簧圈经微导管送入蛛网膜下腔一部分后,再将微导管头撤入瘤腔内,继续弹簧圈填塞。若弹簧圈引起破裂,要将弹簧圈完全或部分送出去,将破裂口堵住后,调整微导管头端位置继续弹簧圈填塞。结果在接受动脉瘤栓塞治疗的153例患者中,141例曾有过动脉瘤破裂引起蛛网膜下腔出血,治疗中5例发生了术中再破裂,占动脉瘤破裂引起蛛网膜下腔出血的3.5%,总发生率为3.3%。1例破裂由导丝引起,1例由微导管引起,1例由弹簧圈过度填塞引起,弹簧圈穿孔1例,其余1例由微导管和弹簧圈共同引起。2例死亡,死亡率占术中破裂的40%,占总例数的1.3%;1例患者出院时遗留有右下肢瘫痪,其余2例患者无残留神经系统并发症。结论动脉瘤栓塞术中动脉瘤的再破裂是一少见、威胁生命但又不可避免的事件。应该立即采取妥善措施以挽救患者生命、改善预后、降低可怕并发症的发生。如处理恰当,多数术中动脉瘤破裂的患者能够存活,无后遗症。  相似文献

目的观察血管内介入治疗大脑后动脉夹层动脉瘤的效果。方法回顾性分析12例接受血管内介入治疗的大脑后动脉夹层动脉瘤患者,评价治疗效果。结果12例大脑后动脉夹层动脉瘤中,P1段2例,P1-P2段2例,P2段6例,P3段2例,均成功实施栓塞治疗;对其中2例单纯以弹簧圈栓塞动脉瘤,8例以弹簧圈闭塞载瘤动脉,2例植入支架。术后即刻行复查造影显示Raymond分级1级10例,2级2例。术后4例诉头痛,经对症治疗后2周内好转;未见动脉瘤破裂出血及脑梗死。随访期间影像学检查均未见动脉瘤复发,无新发神经功能障碍及颅内再次出血;末次随访时改良Rankin量表0分10例,1分1例,2分1例。结论血管内介入治疗大脑后动脉夹层动脉瘤效果较好,且安全性较高。  相似文献

目的探讨NeuroformⅡ支架植入联合MatrixⅡ弹簧圈栓塞治疗颅内宽颈动脉瘤的疗效、技术要点、安全性及并发症防治。方法诊断为颅内宽颈动脉瘤的病人11例,其中大脑前动脉瘤2例,后交通动脉瘤4例,眼动脉瘤1例,大脑中动脉瘤2例,椎基底动脉瘤2例。7例先行NeuroformⅡ支架瘤颈成形,将微导管通过支架网眼置入动脉瘤内,填塞弹簧圈;4例先置入微导管于动脉瘤内,再释放支架后栓塞,术后3~6个月随访。结果所有病例栓塞操作均顺利完成,无手术并发症;其中致密填塞8例,部分致密填塞3例,术后病人均恢复良好,4例短期随访无再出血及血栓栓塞症状发生。结论NeuroformⅡ支架联合MatrixⅡ弹簧圈治疗颅内宽颈动脉瘤安全、有效。  相似文献

目的评价动脉瘤模型行腔内微弹簧圈栓塞前后血流动力学的改变,用以判断疗效。方法运用改进的显微外科技术建立犬颈总动脉(CCA)动脉瘤模型22个,其中侧壁型12个。分叉部4个,末端型6个。术后7~14d行彩色多普勒超声、经颅多普勒(TCD)、数字减影动脉血管造影(IADSA)及经微导管动脉瘤内测压,然后以微弹簧圈紧密填塞动脉瘤腔,栓塞后重复进行上述检查,比较栓塞前后血流动力学变化。结果所建模型均获成功。实验证实,动脉瘤微弹簧圈栓塞前后其血流动力学参数的差异有统计学意义(P〈0.01)。结论实验所建动物模型是研究动脉瘤血管内栓塞治疗的理想方法;动脉瘤微弹簧圈栓塞后,能减低、改变或消除载瘤动脉及动脉瘤内异常血流动力学状态,终止动脉瘤行为,防止动脉瘤扩大和破裂。  相似文献

目的:探讨小脑上动脉动脉瘤的临床特点及治疗方法和效果。方法回顾性分析5例小脑上动脉囊性动脉瘤患者的临床资料。3例采用血管内介入治疗的患者中,2例使用支架辅助弹簧圈技术栓塞,1例单纯弹簧圈栓塞;2例行开颅夹闭术的患者,其中1例经颞下入路,1例经翼点入路。结果除1例经颞下入路开颅夹闭的患者术后遗留同侧动眼神经麻痹外,其余患者无神经功能障碍,痊愈出院;术后随访3~18个月,未见动脉瘤复发,无再出血病例。结论小脑上动脉动脉瘤发病率较低,临床表现以后循环蛛网膜下腔出血为主,可以根据具体情况采用血管内治疗或开颅手术,血管内治疗具有微创、疗效好和并发症少等特点,可以作为首选治疗方法。  相似文献

目的观察血管内治疗大脑前动脉远端(DACA)动脉瘤的效果。方法回顾性分析14例接受血管内治疗的DACA动脉瘤患者(共15个动脉瘤),对10个动脉瘤行单纯弹簧圈栓塞、4个动脉瘤行支架辅助下弹簧圈栓塞,1个以Onyx胶栓塞。之后复查DSA,根据Raymond分级评价即刻疗效。术后6个月复查DSA,以改良Rankin量表(mRS)评估预后,mRS评分0~2分为结局良好。结果术后即刻12个动脉瘤Ⅰ级栓塞,3个Ⅱ级栓塞。术中、术后均未发生缺血等并发症。1例术后12 h死于动脉瘤再次破裂出血。术后6个月随访显示1例复发,10例结局良好(mRS评分0~2分),另2例mRS评分分别为3分、4分。结论个体化血管内治疗DACA动脉瘤安全、有效。  相似文献

A 61-year-old woman suffered cisternal coil migration in the follow-up period after endovascular coil embolization for a ruptured cerebral aneurysm. She presented with sudden onset of headache. Computed tomography demonstrated diffuse subarachnoid hemorrhage, and cerebral angiography disclosed a left anterior choroidal artery aneurysm. The aneurysm was treated by endovascular embolization with Guglielmi detachable coils. During the embolization procedure, the microcatheter perforated the aneurysm. For direct closure of the perforation site with coils, the microcatheter was withdrawn and coils were deployed partially in the subarachnoid space and partially in the aneurysm sac. The coil mass was spread in the subarachnoid space around the aneurysm immediately after embolization. The patient was discharged with no neurological deficit. Three months later, follow-up radiography demonstrated obvious reduction in the size and compaction of the coil mass. Magnetic resonance angiography and digital subtraction angiography demonstrated stable occlusion of the aneurysm. The coil mass probably spread in the cistern around the aneurysm and was compacted by the shape memory of the coils and pulsation of the brain and vessels, as the subarachnoid clots around the aneurysm had disappeared. This case suggests that cisternal coil migration should be considered in the follow up of intracranial aneurysm treated with detachable coils.  相似文献

BACKGROUND: The treatment of very small cerebral aneurysms with maximal diameter less than 3 mm remains a challenge for endovascular and surgical treatment. Endovascular treatment of these lesions may be difficult and associated with high risk of complications because of their small size. Our purpose was to assess the feasibility and results of endovascular treatment of these lesions. METHODS: We conducted a retrospective review of our experience and results of endovascular treatments for a series of 11 consecutive patients with 11 very small aneurysms. Of 11 aneurysms, 10 were acutely ruptured, and 1 was unruptured with a previous subarachnoid hemorrhage from another aneurysm. Aneurysms were located at the internal carotid artery (n = 4), the anterior communicating artery (n = 6), and the vertebral artery (n = 1). Seven patients were treated with coil embolization, and remodeling technique was used in 1 case. Three cases underwent intravascular stent implantation. Coil packing was done after in 2 of 3 aneurysms, and stent implantation alone was used in the remaining aneurysm. RESULTS: Coil embolization and stent deployment were carried out without difficulty in all cases. Coil packing was not available after stent implantation in 1 case for unsuccessful navigation of microcatheter into the aneurysm sac. Immediate angiography demonstrated complete occlusion in 10 cases and nearly complete occlusion in 1 case with stent implantation alone. No stent thrombosis and aneurysmal rupture was encountered during treatment. With the exception of 1 patient (Hunt and Hess grade 4) who died of pneumonia 4 weeks after treatment, no clinical evidence of neurologic deterioration and hemorrhagic complication was seen during the follow-up period in the remaining 10 patients. Follow-up angiography for 3 to 12 months (mean, 5.3 months) was available in 6 (60%) of 10 surviving patients, and no aneurysm recanalization was found. CONCLUSIONS: Endovascular treatment may be a feasible and effective therapeutic alternative for very small aneurysms. The long-term efficacy and durability of endovascular treatment for these lesions remains to be determined in a large series.  相似文献

BACKGROUND: Fenestration of vertebrobasilar junction is a rare congenital anomaly and often associated with aneurysm formation. We describe five cases of vertebrobasilar junction aneurysms in four patients associated with fenestration, which were treated with endovascular coil occlusion using Guglielmi detachable coils (GDCs). The importance of preoperative computed tomography (CT) angiography to understand the complex anatomy of fenestration and aneurysm is emphasized. CASE REPORTS: Three patients presented with subarachnoid hemorrhage and one patient presented with headache only. Among 3 patients with subarachnoid hemorrhage, 1 patient was referred for endovascular coil occlusion after clipping of ruptured distal ACA aneurysm. A six-French guiding catheter was placed in the left vertebral artery via right femoral artery, except for 1 patient who had two vertebrobasilar junction aneurysms with complex anatomic relationship, accessed bilaterally. Five vertebrobasilar junction aneurysms with fenestration were treated with endovascular coil occlusion using GDCs. Postoperative angiography demonstrated successful occlusion of aneurysmal sac with preservation of basilar artery. CONCLUSIONS: Vertebrobasilar junction aneurysms are frequently associated with fenestrations. In addition to vertebral angiography on both sides, CT angiography may be a valuable tool for better understanding of complex anatomy of aneurysms associated with fenestration. The surgically difficult aneurysms such as vertebrobasilar junction aneurysm with fenestration can be successfully treated with GDCs.  相似文献

We describe a case of the combined application of endovascular stent implantation and Guglielmi detachable coil packing for the treatment of a vertebro-basilar fusiform aneurysm and review the literature on stent placement to treat cerebral aneurysms. A 70-year-old female presented with an acute headache from subarachnoid hemorrhage. A fusiform aneurysm with a broad-based neck and dome, measuring 15 mm, involving the union of the vertebral arteries and the proximal basilar artery was demonstrated on cerebral angiography. The aneurysm was judged to be inoperable and treated conservatively. Twelve days later the patient was transferred to our hospital for endovascular therapy. An intravascular stent (MultiLink) was placed across the base of the aneurysm through the right vertebral artery. After this, coil placement in the aneurysm around the stent was performed via a microcatheter guided from the left vertebral artery. After that a microcatheter was guided from the right vertebral artery through the interstices of the stent into the aneurysm, and additional coils were placed. Final angiography showed subtotal occlusion of the aneurysm and excellent blood flow of the parent artery through the stent. There were no new neurological deficits. Neither rerupture nor ischemic event has occurred. The use of stents provides another treatment for managing the difficult entity of intracranial aneurysms.  相似文献

BACKGROUND: Endovascular treatment of middle cerebral artery (MCA) aneurysms has not been extensively studied. We report our experience on a select group of patients that underwent coil embolization of an MCA bifurcation aneurysm. METHODS: From August 1999 to January 2005, 29 patients harboring 30 MCA aneurysms were treated with coil embolization. These patients were felt to have favorable characteristics for endovascular therapy including absence of thrombus in the aneurysm, absence of an efferent artery off of the aneurysm, and ability to reconstruct the wide neck with stent reconstruction. We retrospectively reviewed their records and angiographic images to evaluate for technical result and complications. RESULTS: The mean age of our cohort was 59 +/- 13 years with 19 patients presenting with a ruptured aneurysm. Complete obliteration was achieved in 24 (80%) of 30 of aneurysms on postprocedural angiography and no patient showed aneurysm regrowth at 6-month follow-up. Twenty-seven (93%) of 29 patients had no change in baseline neurological function post-embolization. There were two procedural-related complications: one intraprocedural rupture of an aneurysm and one thromboembolic stroke in the ipsilateral MCA territory. CONCLUSIONS: Coil embolization of MCA bifurcation aneurysms has a high rate of complete obliteration with acceptable morbidity in our selected group of patients.  相似文献

The rupture of an intracranial aneurysm leads to subarachnoid hemorrhage (SAH). To prevent SAH, unruptured lesions can be treated by either endovascular or microsurgical approach. Due to their complex anatomy, middle cerebral artery (MCA) aneurysms represent a unique subgroup of intracranial aneurysms. Primary objective was to determine radiological and clinical outcomes in patients with middle cerebral artery aneurysms who were interdisciplinary treated by either endovascular or microsurgical approach in a single center. Secondary objective was to determine the impact of the lesions’ angiographic characteristics on treatment outcome. Clinical and radiological data of 103 patients interdisciplinary treated for unruptured MCA aneurysms over a 5-year period were analyzed in endovascular (n?=?16) and microsurgical (n?=?87) cohorts. Overall morbidity (Glasgow Outcome Score <5) after 12-month follow-up was 9 %. There was no significant difference between the two cohorts. Complete or “near complete” aneurysm occlusion was achieved in 97 and 75 % in the microsurgical, respective endovascular cohort. A “complex” aneurysm configuration had a significant impact on complete aneurysm occlusion in both cohorts, however, not on clinical outcome. Treatment of unruptured MCA aneurysms can be performed with a low risk of repair using both approaches. However, the risk for incomplete occlusion was higher for the endovascular approach in this series.  相似文献

Embolization of Hypogastric Artery Aneurysm: 17 Cases   总被引:2,自引:0,他引:2  
Surgical management of hypogastric artery aneurysm is associated with high morbidity due to hemorrhage and ischemia. Occlusion by embolization is an attractive alternative treatment. Between 1991 and 1995, we used Gianturco coils to embolize 17 hypogastric aneurysms in 14 patients. All patients were men with a mean age of 77 years. Ten patients had previously undergone aortic repair. Complete occlusion of the aneurysm was achieved in 16 cases but placement of an iliac stent was required in 1 case. Embolization failed in one case involving rupture of a large aneurysm. No complications were observed. Moderate buttock claudication was noted after bilateral embolization in three cases. Embolization of hypogastric artery aneurysm using coils resolves the long-term problems associated with surgical ligation. Extensive aneurysm of the origin to the bifurcation is the main indication for nonresective treatment but embolization can also be a useful alternative to open surgery for high-risk patients. Availability of stent grafts may extend the indication for endovascular treatment.  相似文献

OBJECTIVE: Subarachnoid hemorrhage (SAH) often results in devastating neurological deficits requiring hospitalization and loss of independence. This is often a difficult time for patients and their families who are struggling to cope with this sudden illness. Current treatment options include surgical clipping of the aneurysm or endovascular obliteration using Guglielmi detachable coils. Our purpose in writing this paper was to review the factors that determine the choice of treatment. In addition to this we wanted to study the benefits of surgical clipping for ruptured aneurysms over endovascular coiling. MATERIAL AND METHODS: We studied--retrospectively--450 cases of ruptured cerebral aneurysms admitted to our institution from 1997 to 2003. Out of these, 324 were subjected to surgical clipping and 126 to endovascular techniques. The outcome was studied using the Glasgow Outcome Score (GOS). RESULTS: Of the 324 cases of surgical clipping 222 had a good recovery, 38 had moderate disability, 15 had severe disability, 13 became vegetative and 36 patients died. In the endovascular group 34 had a good recovery, 22 had moderate disability, 18 had severe disability, 15 became vegetative and 37 patients died. Grade to Outcome was compared for both forms of treatment. In our series clipping for ruptured aneurysm was preferred to coiling in fusiform-shaped aneurysms, large or giant aneurysms, MCA aneurysms, blister aneurysms, complex configurations, partially thrombosed aneurysms and aneurysms associated with cerebral hemorrhage. Coiling was performed for basilar tip and trunk aneurysms, high anterior communicating artery aneurysms, patients in subacute stages of subarachnoid hemorrhage, and those with associated medical complications. CONCLUSION: Based on this study we were able to formulate a few definite indications for clipping, even in the times of advanced endovascular techniques. In addition we could also prove the benefits of surgical clipping over the endovascular technique in severe subarachnoid hemorrhage.  相似文献

Four women, aged 39 to 46 years, were urgently admitted to our neurosurgical unit after strokes. On admission, all appeared moribund, presenting with deep coma, pupils bilaterally dilated and fixed, decerebrate posture, and markedly abnormal respiratory patterns. Computed tomography revealed subarachnoid hemorrhage with an associated large intracerebral hematoma and pronounced shift of midline structures in all four cases. Because of the clinical appearance, the patients were given urea and were operated without preceding angiography. The origin of the hemorrhage was identified as a middle cerebral artery (MCA) bifurcation berry aneurysm in one patient and giant MCA aneurysms in the other three. The hematomas were evacuated, and the aneurysms were occluded. All four patients received intravenous nimodipine, none showed any sign of delayed ischemic deterioration, and all regained full consciousness within a few days. One patient died 3 weeks later from a pulmonary embolus. Three patients are presently at home with moderate focal neurological deficits and moderate to marked cognitive impairment. The psychosocial readjustment was very good in a patient with a left giant aneurysm, satisfactory in a patient with a right giant aneurysm, and unsatisfactory in a patient with a right berry aneurysm. The indications, ethical considerations, and technical aspects of operating on seemingly moribund patients who probably harbor a ruptured MCA aneurysm are discussed.  相似文献

目的总结无蛛网膜下腔出血(SAH)的大脑中动脉(MCA)动脉瘤破裂的临床特点。方法对6例在首次CT扫描上表现为脑内出血(ICH),或壁内出血(IMH)而无SAH的MCA动脉瘤破裂患者的临床表现、影像学检查、治疗方法和预后进行回顾性总结。结果本组首次CT扫描时间为起病后0—2d,表现为单纯ICH者4例,IMH者2例。DSA和手术证实为MCA动脉瘤破裂。开颅动脉瘤切除1例,夹闭5例,其中4例同时行血肿清除术。术后无死亡。结论MCA动脉瘤破裂首次CT扫描可仅表现为ICH或IMH而无SAH,与动脉瘤的部位、出血量以及CT扫描时间相关。  相似文献
