目的研究股骨头坏死病程各阶段的生物力学改变,探讨其生物力学发生作用的机制。方法取杂种犬24只,固定一侧前肢建立三足负重犬模型。随机取三足负重犬一侧后肢为实验侧,在股骨头内注射无水酒精,致股骨头坏死;对侧为对照侧,在股骨头内注入等量生理盐水。造模后1、3、6及12周处死动物,每组6只,分别对两组股骨头行大体观察以及放射学、组织学、生物力学检测。结果术后3周股骨头坏死侧点压硬度及中部松质骨弹性模量相对对照侧分别下降29%和32.9%,此时仅MRI可见股骨头出现低密度坏死区,组织学主要表现为骨坏死。术后6、12周股骨头坏死侧点压硬度相对对照侧分别下降了45.5%和48.7%,中部松质骨弹性模量相对对照侧分别下降了34.1%和32.4%。6周时坏死侧X线可见股骨头内密度不均,组织学表现为坏死和修复反应并存,坏死区向软骨下骨区进展。12周时坏死侧X线可见股骨头负重区下出现局部骨密度减低区,组织学出现关节软骨面塌陷和关节间隙狭窄。结论生物力学是股骨头病程进展的重要影响因素;病程进展中,特别是修复期力学性能的显著下降可能是后期塌陷的最直接原因。治疗股骨头坏死不仅应促进骨修复,更应提供病变区一个有利、稳定的生物力学环境。  相似文献

背景:目前评价保髋治疗的疗效标准较多,没有统一标准,因此保髋治疗的疗效缺乏一定的可比性。 目的:通过回顾及分析现有的用于保髋治疗疗效的各种评价标准,试图研制一款特异性的用于评价保髋治疗疗效的生存质量量表。 方法:应用计算机检索Pubmed数据库(2000/2010-08),以“Osteonecrosis Femoral Head、quality of life”或“Osteonecrosis Femoral Head、curative effect”为检索词;应用计算机检索CNKI数据库(1994-01/2009-12),以“股骨头坏死、疗效”或“股骨头坏死、生存质量”为检索词。选择与股骨头坏死保髋治疗疗效或生存质量相关,同一领域文献则选择近期发表或发表在权威杂志文章。根据纳入标准共31篇文章进行综述。 结果与结论:现有的评价保髋治疗的疗效标准过分的侧重髋关节的疼痛、功能、关节活动等方面,局限于髋关节或下肢的局部改变情况,对患者整体的生存质量反应不够,而以SF-36为代表的生存质量量表评价标准却缺乏一定的特异性,都未能更全面更具特异性的用于评价保髋治疗的疗效。因此,研制一款特异性的用于评价股骨头坏死保髋治疗疗效的生存质量量表成一种必要。  相似文献

To date, there has been no report clarifying the existence of sensory nerve fibers as the origin of the hip joint pain of osteoarthritis. We examined the existence of sensory nerve fibers in osteoarthritis (OA), osteonecrosis of the femoral head (ONFH), and femoral neck fracture of the human hip joint. Ten labra of 10 human hip joints were harvested during a total hip arthroplasty. Each labrum was separated into 12 sections and we used three sections for analysis, which included 2 weight-bearing and 1 non-weight-bearing portion. Protein gene product 9.5 (PGP 9.5) immunoreactive sensory nerve fibers were found in the labrum and synovium harvested from the weight-bearing portion in the OA group. Some of these sensory nerve fibers were also positive for tumour necrosis factor alpha (TNF). The PGP 9.5 immunoreactive sensory nerve fibers existed in the labrum tissue and inflammatory TNF positive cells were observed in the hyperplastic synovium. On the other hand, we could not demonstrate PGP 9.5 or TNF immunoreactive sensory nerve fibers and cells in any of the ONFH group or the non-weight-bearing portion in the OA group. These data suggest that the pain of ONFH and OA of the hip joint have different pathogenetic mechanisms and that the invasion of sensory nerve fibers containing TNF may be involved in the pathogenesis of pain in the human hip joint affected by OA.  相似文献

Non-traumatic osteonecrosis of the femoral head (ONFH) usually affects adults younger than 50 years and frequently leads to femoral head collapse and subsequent arthritis of the hip. It is becoming more prevalent along with increasing use of corticosteroids for the adjuvant therapy of leukemia and other myelogenous diseases as well as management of organ transplantation. Alcohol‐induced osteonecrosis of the femoral head (ONFH) is observed in alcohol abusers and patients with alcoholic fatty liver disease. It has been reported that Toll‐like receptor 4 (TLR4) signalling plays a crucial role in the pathogenesis of alcoholic fatty liver disease. We previously reported a corticosteroid‐induced ONFH rat model, and suggested that TLR4 signalling contributes to the pathogenesis of ONFH. Thus, it is thought that the pathogenesis of alcohol‐induced ONFH is probably similar to that of corticosteroid‐induced ONFH. The aim of this study was to develop a new animal model for alcohol‐induced ONFH and to evaluate the relationship between the pro‐inflammatory response via TLRs and the development of ONFH in rats. Male Wistar rats were fed a Lieber–DeCarli liquid diet containing 5% ethanol (experimental group) or dextran (control group) for 1–24 weeks. Histopathological and biochemical analyses were performed. Feeding the ethanol‐containing liquid diet resulted in the development of ONFH with hepatic steatosis, hepatic dysfunction and hyperlipidaemia, whereas feeding the dextran‐containing diet did not cause ONFH. However, we could not recognize any relationship between the pro‐inflammatory response via TLR4 and the development of alcohol‐induced ONFH. Thus in this study we have developed a new rat model for alcohol‐induced ONFH based on the feeding of an ethanol liquid diet. ONFH was observed within seven days from the start of feeding with 5% ethanol‐containing liquid diet. Although this was linked to hepatic steatosis, a TLR4 association was not a feature of this model.  相似文献

背景:目前还难以建立合适的早期股骨头坏死的动物模型。 目的:建立一个简单、标准和可靠的股骨头坏死动物模型用于实验研究。 方法:X射线透视下将新西兰兔右侧股骨头中心钻孔并注入无水乙醇,左侧不做处理做对照。经过2,4和6周麻醉下处死动物获取股骨头。 结果与结论:大体及X射线观察显示,造模侧股骨头第2周关节软骨颜色变暗、骨质密度不均匀;第6周,关节面有轻微凹陷,骨质低密度影较前进一步增大。MRI显示,造模侧股骨头第2周,T1加权股骨头负重区显示线样或不规则低信号,T2加权呈高信号;第6周,股骨头变性,软骨下骨折,关节面塌陷,新月体形成。对照侧第2,4,6周股骨头结果均常。组织病理学观察显示,造模侧股骨头第2周后,骨细胞核固缩、变性坏死。结果证实,2周后兔股骨头均发生了部分坏死,股骨头坏死的完整的外观形态,大体循环和关节软骨与人类早期股骨头坏死是相似的,提示实验建立了良好的股骨头坏死动物模型。  相似文献

股骨头坏死(ONFH)好发于青壮年。病变进展会显著影响患者的生活质量以及工作能力。因此在ONFH早期进行治疗以防止股骨头塌陷,保护关节功能并延迟关节置换的时间显得极为重要。而ONFH的保头治疗的重点都在于如何有效重建股骨头内支撑结构,延缓股骨头坏死的病情进展,尽可能的保留髋关节功能。用于重建股骨头的修复材料是股骨头早期手术治疗的重点,直接关系到股骨头的存活及髋关节功能的保留程度。本文就各种用于重建股骨头内支撑的修复材料的研究进展做一综述。  相似文献

Osteonecrosis (ON) is one of the serious and well recognized complicationscausing severe pain and disability in patients with systemic lupus erythematosus (SLE), and its manifestation and pathogenesis are only partially understood. This review provide an update of the recent progress in etiology, pathogenesis, diagnosis, and treatment of systemic lupus erythematosus related osteonecrosis (SLE-ON). Despite the concomitant use of corticosteroids, alcohol and obesity, the dysregulated immune micro-environment and the complex pathogenesis of SLE synergistically play important roles in the development of ON. Osteonecrosis of femoral head (ONFH) is the most often involved joint in SLE. The current classification and staging system of ONFH is based on imaging techniques, particularly relating to MRI and CT, for the identification and quantification of necrotic lesions. For SLE-ONFH patients, both SLE specific clinical symptoms and ONFH imaging findings should be comprehensively evaluated. Even though advances concerning bone grafting and arthroplasty procedures have resulted in improved clinical outcomes, early pharmacological treatment at the pre-collapse stage may prevent joint collapse and reduce the joint arthroplasty rate, and this needs to be accounted. Although some progress has been made, considerably more research is needed before we fully understand SLE-ONFH. Future treatments of SLE-ONFH may involve genetic or cell-based therapies that target potential biomarkers, and this will lead to effective measures for saving thefunction of hip joint and preventing osteonecrosis.  相似文献

背景:非创伤性股骨头坏死的发病机制存在各种理论学说,脂质代谢异常是其中一种假说,但是脂质代谢如何参与骨坏死的发病机制仍不清。 目的:通过对激素性和酒精性骨股头坏死患者血脂改变及股骨头置换标本形态计量学观察分析,探讨脂质代谢改变在非创伤性骨坏死中的作用。 方法:选择股骨头坏死住院患者162例,包括酒精性骨股头坏死57例,激素性骨股头坏死63例,创伤性骨股头坏死42例,其国际骨循环分期为(ARCO)Ⅱ~Ⅳ期。患者入院后常规抽空腹静脉血行三酰甘油、低密度脂蛋白等血脂检测。各组选取12例关节置换股骨头标本,股骨头冠状面正中剖开,在负重区和非负重区取材,对骨髓造血面积、脂肪细胞密度、脂肪细胞直径和脂肪细胞面积等行形态计量学观察。 结果与结论:酒精性股骨头坏死患者血三酰甘油升高率显著高于激素性和创伤性股骨头坏死患者(P < 0.05)。酒精性股骨头坏死患者血低密度脂蛋白降低率明显高于激素性和创伤性股骨头坏死(P < 0.05)。与激素性骨坏死患者相比,酒精性股骨头坏死患者脂肪细胞面积和脂肪细胞直径大,但造血面积减小,且差异有显著性意义。推测脂质代谢改变在酒精性股骨头坏死晚期起重要调节作用,而在激素性股骨头坏死晚期作用不明显。  相似文献

Background

Standard radiographic views for patellofemoral joint assessment do not reflect loading at which TKA patients may describe post-operative anterior symptoms. A novel weight bearing (WB) Merchant view has been described and demonstrated a number of tracking changes that correlated with clinical outcomes. In this study, we aim to validate the WB Merchant view and assess relationships with patient outcome scores.

Methods

Patients were randomly allocated to receive one of the three commonly used patellar implants with a single TKA prosthesis. Patients were evaluated at six months post-operatively using both NWB and WB Merchant views. Indicators of patellar tracking were correlated with improvement in KOOS, WOMAC and Kujala scores. For reliability assessment, radiographs were assessed twice by two readers.

Results

The WB Merchant view showed a reduction in the percentage of outliers of tracking indices in comparison to the NWB view (Congruence angle: NWB?=?37%, WB?=?24%; Displacement: NWB?=?2%, WB?=?0%; Tilt angle: NWB?=?60%, WB?=?56%). There was an increase in the lateral contact state with the WB Merchant view (Type I: NWB?=?19%, WB?=?28%; Type II: NWB?=?3%, WB?=?4%). The state of lateral contact had a consistent and statistically significant correlation with the improvement in KOOS, WOMAC and Kujala scores (p value?=?0.01, 0.01 and 0.03, respectively). All radiographic indices had good reliability with accepted variability.

Conclusion

The WB Merchant radiograph is an easy to perform and reliable view for the evaluation of patellar tracking and may provide additional information to the routinely used NWB view.  相似文献

背景:股骨头坏死发病机制仍需要进一步研究,因此需要建立一个能够高度模拟人类股骨头坏死的动物模型,而如何正确评价股骨头坏死动物模型,是建立股骨头坏死动物模型的前提条件。目的:归纳总结近几年国内外对股骨头坏死动物模型的评价方法,综述各评价方法的优缺点,并探索新的评价方法,为动物模型的建立提供参考。方法:以“股骨头坏死,动物模型,评价方法”为检索词检索CNKI中国知网、万方、维普数据库,以“osteonecrosis of femoral head,femoral head necrosis,animal model,evaluation methods”为检索词检索PubMed、Web of Science及Medline数据库,检索2010年1月至2020年9月发表关于股骨头坏死动物模型评价方法的文献。结果与结论:①根据纳入标准通过阅读文献进行初步筛选,共获得51篇文章进行综述,其中中文文献33篇,英文文献18篇;②近年来,随着影像技术的发展和新技术的引入,股骨头坏死动物模型评估方法的选择增多,而选择合理的评价方式能减少建立动物模型过程中的工作量,并为后续研究提供参考,因此需要继续完善和改进。  相似文献

目的回顾性分析髓芯减压术后二次减压术治疗早中期股骨头坏死(ONFH)的临床疗效。 方法选取2008年7月至2013年4月于安徽医科大学解放军第三〇七医院只接受股骨头髓芯减压、死骨刮除、人工骨打压植入术治疗的30例(50髋)ONFH患者,设为单次减压组;选取2010年1月至2013年4月于安徽医科大学解放军第三〇七医院接受单次髓芯减压术术后半年同意行二次减压治疗的30例(42髋)ONFH患者,设为二次减压组。按照国际骨循环研究会分期,所有ONFH均为Ⅰ期、Ⅱ期。采用髋关节Harris评分系统评估术后患髋功能改善情况,应用视觉模拟评分法(VAS)进行临床疼痛测定,通过影像学(X线、CT、磁共振成像)评定股骨头是否塌陷及病灶修复情况。组间数据采用t检验、χ2检验进行比较分析。 结果60例(92髋)ONFH患者术后均获得随访,随访时间为25~84个月。术后25个月时,两组髋关节Harris评分较术前均有显著提高,差异均有统计学意义(P值均小于0.05);二次减压组髋关节Harris评分[(91.21±3.32)分]显著高于单次减压组[(81.60±2.60)分],差异有统计学意义(t=-15.592,P<0.05)。两组VAS评分较术前均有显著降低,差异均有统计学意义(P值均小于0.05);二次减压组VAS评分[(2.20±0.71)分]显著低于单次减压组[(3.27±1.51)分],差异有统计学意义(t=3.503,P=0.001)。单次减压组髋关节Harris评分优2髋,良36髋,尚可5髋,优良率为76.0%,股骨头保存率为86.0%;二次减压组髋关节Harris评分优35髋,良5髋,尚可2髋,优良率为95.2%,股骨头保存率为100.0%。两组髋关节Harris评分优良率比较差异有统计学意义(χ2=6.548,P=0.011),两组股骨头保存率比较差异有统计学意义(χ2=4.529,P=0.033)。两组髋关节Harris评分为优的37髋的股骨头外形修复均完整;评分为良的41髋中有6髋外形不圆,但股骨头内未出现骨折,剩余基本维持球形;评分为差的7髋出现软骨下骨折、关节间隙狭窄,其中3髋行全髋关节置换术。 结论髓芯减压术后二次减压术可再次诱发股骨头内创伤修复过程,进一步改善患髋功能、减轻髋关节疼痛,起到较好的治疗作用。  相似文献

Femoral head avascular necrosis is a process leading to femoral head deformity and osteoarthritic changes in the hip joint. Alendronate slows down bone resorption and remodelling in rats, while core decompression hastens the healing processes. We evaluated the influence of daily alendronate treatment on the rat femoral head shape after surgical osteonecrosis with core decompression, compared with controls. No differences were found in shape factor and femoral head height/length ratios. It was concluded that alendronate treatment slows down the process of replacing osteonecrotic bone by new bone and prevents early immature new bone collapse resulting from early revascularization because of core decompression.  相似文献

目的探讨经大粗隆单一入路双孔道减压植骨+异体腓骨支撑治疗早期股骨头坏死(ONFH)的临床疗效。 方法采用经大粗隆单一入路双孔道减压植骨+异体腓骨支撑的方法对2011年3月至2017年3月北京积水潭医院矫形骨科收治的30例(41髋)早期ONFH患者进行治疗。所有患者术前均行双髋关节正侧位X线、CT平扫及MRI检查,根据国际骨循环研究会(ARCO)的骨坏死分期标准对其分期,其中ARCO ⅡA期1髋、ARCO ⅡB期1髋、ARCO ⅡC期33髋、ARCO ⅢC期6髋,术前评估患髋Harris评分。术后随访时复查髋关节正侧位X线及CT,评估患者术后髋关节Harris评分,以综合评价术后临床疗效。数据比较采用配对t检验及χ2检验。 结果所有患者平均随访(33.6±13.2)个月,术前Harris评分为(74.2±5.81)分,优良率为12.20%;末次随访Harris评分为(87.7±7.2)分,优良率为80.49%;末次随访时Harris评分及术后优良率均较术前明显提高,差异均有统计学意义(t=3.129,χ2=38.45;P值均小于0.05);其中,1例女性患者术前左髋ARCOⅡ期,因病情进展于术后22个月行左侧全髋关节置换术;2例3髋术前ARCOⅡC期,末次随访时病情已进展到ARCO Ⅲ期,但髋功能良好,末次随访时手术成功率为97.56%。 结论经大粗隆单一入路双孔道减压植骨+异体腓骨支撑治疗是治疗早期股骨头坏死的有效方法,该术式具有手术创伤小、术后恢复快、并发症少、临床疗效确切等优点。  相似文献

Autologous implantation of bone mesenchymal stem cells (BMSCs) has achieved promising clinical efficacy for the treatment of early-stage osteonecrosis of the femoral head (ONFH). However, the underlying mechanisms are not completely elucidated. Here, we investigated the effect of BMSCs on the early ONFH in vitro and in vivo. In co-cultured system, primary BMSCs enhanced the activity and inhibited the apoptosis of primary OB. The concentrations of VEGF and BMP-2 in the co-cultured medium were significantly higher than those without co-culture. Importantly, BMSCs implantation increased OB, capillaries and VEGF and BMP-2 expressions of the necrotic areas of femoral head in the ONFH rabbits. In conclusion, our results indicated that BMSCs treated the early ONFH possibly through increasing OB and capillaries, as well as VEGF and BMP-2 expression in the femoral head. These results provided possible mechanisms for the treatment of early-stage ONFH with BMSCs transplantation.  相似文献

目的:比较髓芯减压联合金属钽棒植入术与带血管蒂髂骨瓣转移术治疗成年早期股骨头坏死(osteonecrosis of the femoral head,ONFH)的近期临床疗效.方法:回顾性收集本院2012年1月至2015年2月间收治的83例(97髋)非创伤性ONFH患者病历资料,A组43例52髋行髓芯减压并钽棒植入术,B组40例47髋,行自身带血管蒂髂骨瓣转移术,术后随访1年,比较两组手术参数指标、术后1年Harris髋关节功能评分及手术疗效.结果:A组手术时间(40.7±10.3) min、术中失血量(88.0±27.1) mL、术后镇痛泵应用时间(1.68±0.32)d、拆线天数(12.8±1.5)d均显著低于B组[(82.5±17.2) min,(239.4±45.2) mL,(2.15±0.48)d,(14.2±1.9)d;P<0.05].两组术后12个月Harris总评分及各项目评分均较术前显著提高(P<0.0s),A组术后Harris总评分(88.4±5.9)及关节功能评分(41.5±4.8)均显著高于B组[(83.6±5.3),(37.5±6.1);P<0.05].两组术后12个月髋关节功能优良率均较术前显著提高(P<0.05),A组术后12个月优良率(92.3%)显著高于B组(76.6%,P<0.05).B组有1例出现髓塌陷,A组无髓塌陷病例.结论:髓芯减压联合金属钽棒植入术较带血管蒂髂骨瓣转移术治疗早期ONFH手术操作相对简单,近期疗效更为明显.  相似文献

背景:近年来随着器官移植技术在临床上的广泛开展,糖皮质激素引起股骨头缺血性坏死占到首位,且并有逐年增高的趋势。 目的:主要对他汀类药物治疗股骨头坏死的机制进展进行综述分析,为进一步的实验研究及临床应用奠定基础。 方法:第一作者应用计算机检索CNKI、PubMed、万方和维普数据库中1980-01/2010-12关于他汀类药物在治疗股骨头坏死机制的文章,在标题和摘要中以“股骨头坏死;他汀类药物;糖皮质激素;成骨细胞”或“ONFH;Corticosteroids;  statins;  Osteoblasts”为检索词进行检索。选择文章内容与激素性股骨头坏死与他汀类药物修复治疗相关,同一领域文献则选择近期发表或发表在权威杂志文章。排除陈旧及重复实验文章,初检得到169篇文献,根据纳入标准选择34篇文章进行综述。 结果与结论:股骨头缺血性坏死是骨科常见,多发性的疾病,其治疗是世界性的难题。他汀类药物能有效纠正激素所导致机体内脂质代谢紊乱的状态,减轻股骨头内的骨细胞损害,可以延缓或阻止股骨头缺血坏死、塌陷、变形的病理过程。  相似文献

Developing the radiographic images from two to three-dimensional, finite element analysis(FEA) technology can set up the model, predicting diagnosis, treatment design, as well as surgical plan. FEA provides an accurate three-dimensional finite element biomechanical study in osteonecrosis of femoral head(ONFH). The papers in the latest 5 years related to femoral head osteonecrosis and finite element analysis application are concentrated on. 同时基于个体股骨头坏死患者的X-ray、CT和MRI图像,采用图像配准和融合技术对包含坏死股骨头的髋关节进行三维重建,获取具有高度几何相似性的三维有限元网格模型。选择1例中年女性股骨头坏死患者,分别获取X-ray、CT和MRI三套图像,采用Mimics 13.1和Pro/E 5.1软件分别基于这三套数据建立相关三维实体模型,经图像投影转换后,确定图像之间的匹配点,进行二维图像配准,配准后对成功融合的图像进行三维有限元网格模型显示。建立了具有良好几何相似性的髋关节三维有限元网格模型,包括正常皮质骨、松质骨、关节软骨和股骨头坏死区、断裂骨小梁等六部份,较真实地反映了包含坏死股骨头的髋关节的形态特征及毗邻关系,为进一步的生物力学分析和手术模拟提供了较理想的研究平台。  相似文献
