Buttock abscess brucellosis

Human brucellosis is a multisystem disease that may present with a broad spectrum of clinical manifestations, and its complications can affect almost all organs and systems with varying incidence. Soft-tissue infection due to Brucella species is a rare complication of brucellosis. This report presents a case of a 30-y-old previously healthy woman whose right buttock abscess culture grew Brucella spp. The patient was treated successfully with abscess drainage and antibiotic therapy.     

Prostatic abscess due to community-acquired methicillin-resistant Staphylococcus aureus

We report what we believe to be the second case of a prostatic abscess due to community-acquired methicillin-resistant Staphylococcus aureus (MRSA). A previously healthy diabetic man presented with dysuria, fatigue, weight loss, a tender prostate, and leukocytosis. Computerized tomography of the abdomen and pelvis demonstrated a large prostatic abscess at the base of the bladder. Blood, urine, and pus obtained by percutaneous aspiration grew MRSA. Percutaneous drainage and prolonged therapy with intravenous vancomycin resulted in cure. Prostatic abscess is most often caused by Gram-negative organisms. Community-acquired MRSA, which usually causes skin and soft tissue infections, may also cause prostatic abscess. The mainstay of treatment of prostatic abscess is drainage, which can be accomplished either percutaneously or transurethrally. Gram stain and culture of the drainage will direct proper antibiotic selection.     

Prostatic abscess caused by Streptococcus mutans

The first reported case of prostatic abscess caused by Streptococcus mutans isolated in pure culture is described. Urethral dilation for obstruction was unsuccessful, so suprapubic cystostomy was performed. Perineal aspiration under ultrasonic guidance resulted in 10 mL of pus containing pure Strep mutans. Diagnosis of prostatic abscess is difficult since the clinical manifestations are nonspecific.     

Medically treated splenic abscess due to Brucella melitensis

Brucellosis may lead to complications that affect different organ systems, including the liver and spleen. In acute disease, hepatosplenic abscess is a rare complication. We report herein a woman with splenic abscess due to acute Brucella melitensis infection who was successfully treated with antibiotics alone.     

Unusual manifestation of brucellosis: liver abscess and pancytopenia caused by Brucella melitensis

Brucellosis is a zoonosis with a worldwide distribution, and still endemic in Greece. Although hepatic involvement is common, cholecystitis and hepatic abscesses represent rare complications of the disease. Hematological manifestations of brucellosis include various abnormalities such as anemia and leucopenia, but the disease rarely presents with pancytopenia. We present a case of liver abscess and pancytopenia caused by Brucella melitensis in a 54-year-old worker who consumed unpasteurized dairy products. The patient was admitted twice to our hospital and finally diagnosed as suffering from a gallbladder empyema complicated by a liver abscess, for which he underwent surgery. Blood cultures drawn on his second admission isolated Brucella melitensis. Repeated serum agglutination testing for brucella turned out negative.     

Ovarian abscess due to Brucella melitensis

The case of a 25-y-old woman with brucellar ovarian abscess is reported. Cultures of blood, ascites and a pus specimen yielded Brucella melitensis. The possibility of ovarian abscess being caused by Brucella melitensis should be considered in countries where the infection is endemic.     

Brucella pelvic tubo-ovarian abscess mimicking a pelvic malignancy

A 57-y-old woman presented with recurrent abdominal and pelvic pain of 6 months' duration with low-grade fever. A computed tomographic scan indicated an ovarian tumor. Laparotomy revealed a pelvic abscess. Her symptoms resolved following surgery and antibiotic therapy. Pathology revealed an extensive inflammatory process. Tissue culture grew Brucella sp. The diagnosis and management of this previously undescribed pelvic tubo-ovarian abscess present a particular challenge.     

Fatal systemic infection with multifocal liver and lung nodules caused by Brucella abortus

We experienced a fatal case caused by Brucella abortus with multifocal necrosis confirmed by culture and polymerase chain reaction. Our case highlights that the clinician should be aware of the potential for fatality when a patient with brucellosis shows dissemination of abscess or nodules with no calcification in the liver, lung, pleura, and spine.     

Splenic abscess due to Brucella infection: is the splenectomy necessary? Case report and literature review

The case of a woman with splenic abscess due to Brucella is reported. There was no response with antibiotics and surgical treatment was required. On the basis of this case and the literature review we consider that surgical treatment must be considered in patients with splenic abscess due to Brucella infection.

A case of cervical Brucella spondylitis with paravertebral abscess and neurological deficits.

Spondylitis is one of the more frequent osteoarticular complications of Brucella infection, but cervical spine involvement is rare. We report here a case of cervical Brucella spondylitis with paravertebral anterior epidural abscess which resulted in neurological deficits. The diagnosis is based on clinical history supported by Brucella serology, radiological findings and histological evidence.     

Emphysematous prostatic abscess due to candidiasis: a case report

Prostatic abscess is an uncommon condition and clinical diagnosis is difficult. The classical symptoms and signs of prostatic abscess are variable and nonspecific. Here, we report a rare case of emphysematous prostatic abscess due to candidiasis in a 68-year-old man with diabetes and liver cirrhosis. The diagnosis was confirmed by pelvic computed tomography (CT) and successfully treated by antibiotics and CT-guided percutaneous abscess drainage. This case highlights the importance of early and accurate diagnosis of emphysematous prostatic abscess followed by appropriate treatment.     

Epididymal and prostatic histoplasmosis in a renal transplant recipient from southern India

An unusual case of epididymal and prostatic histoplasmosis is reported from Karnataka State of southern India, in a 37-year-old male renal transplant recipient, who had presented with gastrointestinal histoplasmosis a year before the second presentation and had responded successfully to 9 months' treatment with itraconazole. The patient presented a second time with high-grade fever and pain in the scrotum and the lower abdomen. Ultrasound of the abdomen revealed a retrotrigonal abscess, which was confirmed on computed tomography scan of the pelvis. Pus was drained via transrectal route and biopsy specimens were taken from the epididymis and the prostate. Histopathology revealed the picture suggestive of histoplasmosis, and the pus on culture confirmed the infection with Histoplasma capsulatum. To our knowledge, only 5 cases of epididymal histoplasmosis have been reported in the literature, our case being the second report from India and sixth in the world; prostatic histoplasmosis is also a rare occurrence.     

Brucella endocarditis caused by reinfection of an aortic Starr valve. Apropos of a case with a favorable development after valvular replacement

We report the case of a 36-year old male patient who was admitted for subacute endocarditis on an aortic prosthetic valve implanted 14 years previously for endocarditic valve regurgitation; no pathogen had been isolated at that time. Nine blood cultures were positive for Brucella abortus. The conventional antibiotic therapy did not prevent the formation of an abscess below the aortic annulus. A third antibiotic combination (quinolone and rifampicin) resulted in pyrexia prior to surgery. An atrioventricular block initially noted raised the problem of an associated myocarditis. The risk of Brucella infection existed before the first operation, and five serological tests were positive for that organism between the two episodes, which makes reinfection highly probable. This is the third case of Brucella endocarditis on a prosthetic valve. Its originality lies in its mechanism.     

Bilateral mammary abscess and uveitis caused by brucella melitensis — Report of a case

Summary Soft tissue locations ofBrucella are a rare finding. We report the isolation ofBrucella melitensis from a suspected breast tumour of a woman who also showed signs of uveitis. Three weeks after surgical drainage and despite antimicrobial therapy a new abscess developed on the other breast whose culture also yieldedBrucella. As observed, unusual localization of brucellosis without previous penetrating injury may be the only manifestation of chronic infection.

Zusammenfassung Weichteilaffektionen durchBrucella sind selten. Wir berichten über eine Frau mit Verdacht auf Mammatumor und Uveitis, bei derBrucella melitensis aus der entzündlichen Schwellung isoliert wurde. Trotz antimikrobieller Therapie und chirurgischer Ausräumung des entzündlichen Gewebes entstand auf der anderen Seite ebenfalls ein Abszeß, aus demBrucella isoliert wurde. Der Fall zeigt, daß ohne penetrierendes Trauma als einzige Manifestation einer chronischen Brucella-Infektion ein Brucelloseherd mit ungewöhnlicher Lokalisation auftreten kann.

     

Klebsiella pneumoniae liver abscess associated with septic endophthalmitis

Metastatic septic bacterial endophthalmitis is a rare, but devastating disease. We encountered seven cases of pyogenic liver abscess associated with septic endophthalmitis during a recent four-year period. The causative organism was a pure culture of Klebsiella pneumoniae. The diagnosis was made by results of a blood culture in seven of the cases, liver aspirate culture in four, and eye contents or conjunctival culture in four. Chest roentgenographic examination showed pulmonary embolization in four patients, purulent meningitis in one patient, and suspicious prostatic abscess in one patient. Despite diligent antibiotic therapy, six patients lost their vision and one had impaired vision. This poor outcome for septic endophthalmitis seems to result from delayed diagnosis and lack of scheduled periocular injections of antibiotics. A combination course of treatment by the internist and ophthalmologist acting aggressively, both diagnostically and therapeutically, is needed in the future. To have seven cases of K pneumoniae liver abscess complicated by septic endophthalmitis during a period of four years in one hospital is very unusual. To our knowledge, it has never been reported in the literature.     

Prostatic abscess as a delayed complication following cryosurgery for primary prostate cancer

Prostatic abscess is a rare and potentially life-threatening disease when left untreated. We report the first case of a man who developed a prostatic abscess as a long-term complication of cryoablation for primary prostate cancer. Timely diagnosis with non-contrast computed tomography (CT) scan, retrograde CT cystogram to ensure no bladder involvement, percutaneous drainage under CT-guidance, and antibiotic treatment sensitive to the causative organism were crucial for prompt recovery.     

Prostatic abscess of Klebsiella pneumoniae complicating septic pulmonary emboli and meningitis: A case report and brief review

Prostatic abscess is a rare entity with an incidence of 0.5% to 2.5% in all prostate diseases and usually occurs in the 5th and 6th decades of life with immunocompromised status,Prostatic abscess might be a process of evolution from acute prostatitis,Klebsiella pneumoniae is the leading microorganism in the diabetic patients of prostatic abscess in Taiwan,A 60-year-old diabetic man,with a one-week history of acute bacterial prostatitis was reported in this study,presenting to the emergency department with sudden altered mental status,The abdominal computed tomographic scan demonstrated lobulated prostatic abscess and multiple septic pulmonary emboli with lung abscesses,Analysis of cerebrospinal fluid showed white blood cells of 10 771 counts/mm3 with segmented neutrophils of 99%,Cultures of blood,cerebrospinal fluid and sputum yielded Klebsiella pneumoniae,We concluded that computed tomographic scan can make a definite diagnosis of prostatic abscess associated with complications and management with empiric antibiotics and adequate drainage is suggested.     

Persisting fever in a patient with brucella endocarditis: occult splenic abscess

Brucella endocarditis, despite its high mortality rate with combined medical and surgical treatment, has a low occurrence rate in cases of brucellosis and has been endemic in regions surrounding Turkey. Rarely, patients with infective endocarditis with common microorganisms develop a splenic abscess. A patient is reported on with brucella endocarditis and persistent fever. An occult splenic abscess was found. This is the second reported case in the literature of brucella endocarditis with splenic abscess.     

Melioidosis--an unusual cause of septic arthritis

Melioidosis is an infection caused by Burkholderia pseudomallei. It is an important human pathogen in the tropical area. The clinical manifestations are protean with multisystem involvement. Septic arthritis and prostatic abscess are rare but well-recognized forms of the disease. Herein we report a case of melioidosis presenting with a rare combination of septic arthritis, prostatic abscess, and septicemia.
