Purpose The quantitative assessment of muscle atrophy has a degree of importance in prognosticating rotator cuff treatment. However, it has been conjectured that muscle fat increases with aging. Therefore, we thought that the quantitative assessment of the supraspinatous would be better if made in comparison with a standard of reference such as the deltoid. Consequently, we performed a two-part study, first evaluating supraspinatous changes compared with the deltoid in “normals” with aging, and second, determining if in patients with cuff tears the supraspinatous fat exceeds that of the deltoid. Materials and methods In part 1, we studied 50 patients stratified by decade. In the first sitting, two blinded independent observers quantitatively graded the deltoid (with the supraspinatous obscured) and in the second sitting the same two observers quantitatively graded the supraspinatous (with the deltoid obscured). In part 2 of the study, we evaluated patients with moderate rotator cuff tears (>2 cm) and performed the same blinded, two-sitting, quantitative assessment (with the comparison muscle obscured). Results We found that muscle atrophy increases with age in patients without tears (0.011/0.028 U/year), although to a greater degree in the deltoid (p = 0.032). Also, in similarly aged patients, quantitative scores of the deltoid closely matched those of the supraspinatous (p = 0.071). Notably, however, in patients with large tears, the supraspinatous showed significant changes disproportionate to those of the deltoid, regardless of patient age (p = 0.044). Conclusion In the presence of a normal rotator cuff, fatty infiltration increases with age. Age-related changes occur more frequently in the deltoid, verifying this muscle’s potential as a standard of reference. With cuff tears, supraspinatous atrophy was disproportionate to that of the deltoid. Therefore, systematic assessment of supraspinatous muscle atrophy may be more reliable using the deltoid as a control for comparison than assessing it in isolation.  相似文献

MR arthrography of postoperative knee: for which patients is it useful?   总被引:9,自引:0,他引:9  
PURPOSE: To assess which postoperative patients benefit most from MR arthrography of the knee. MATERIALS AND METHODS: One hundred consecutive MR arthrograms obtained in patients who had previous knee surgery underwent retrospective review in consensus by three radiologists after prospective reading by one of the three radiologists. Criterion on MR arthrograms for a retear was abnormal tracking of an intraarticular dilute gadolinium-based contrast material and saline mixture into the substance of a meniscus. Patients were separated into three groups: those with more than 25% meniscal resection, those with less than 25% meniscal resection, and those with meniscal repair. All 100 patients had preoperative MR images to review directly in conjunction with the postoperative MR images. Fifty-seven of these 100 patients underwent second-look arthroscopy. RESULTS: Nine patients had MR findings consistent with avascular necrosis. Nineteen patients had marked degenerative arthrosis in the area of previous surgery. Seven patients had chondral defects or injuries. Twenty-nine patients had clear MR evidence of a meniscal retear without any contrast material injected into the joint. In 32 of the 100 patients, intraarticular contrast material was useful in demonstrating a retear. Of these 32 patients, 22 had MR arthrographic evidence of a retear, while 10 had no clear MR arthrographic finding to explain postoperative pain. Four additional patients had no clear MR imaging or MR arthrographic abnormality. All patients with meniscal repair (n = 16) needed MR arthrography to diagnose a residual or recurrent meniscal tear. No patient with less than 25% meniscal resection (n = 23) needed MR arthrography to demonstrate a residual or recurrent meniscal tear. Sixteen of 61 patients with more than 25% meniscal resection needed MR arthrography to demonstrate a residual or recurrent meniscal tear. CONCLUSION: All patients with meniscal repair required MR arthrography. All patients with meniscal resection of more than 25%, who did not have severe degenerative arthrosis, chondral injuries, or avascular necrosis required MR arthrography. Patients with less than 25% meniscal resection did not need MR arthrography.  相似文献

Magnetic resonance imaging (MRI) plays a central role in the modern imaging of musculoskeletal disorders, due to its ability to produce multiplanar images and characterise soft tissues accurately. However, computed tomography (CT) still has an important role to play, not merely as an alternative to MRI, but as being the preferred imaging investigation in some situations. This article briefly reviews the history of CT technology, the technical factors involved and a number of current applications, as well as looking at future areas where CT may be employed. The advent of ever-increasing numbers of rows of detectors has opened up more possible uses for CT technology. However, diagnostic images may be obtained from CT systems with four rows of detectors or more, and their ability to produce near isotropic voxels and therefore multiplanar reformats.  相似文献

Purpose

Dynamic laxity is clinically demonstrated with the pivot-shift (PS) test. Recently, a new system that measures the acceleration of the tibia during the PS test was validated. The goal of the present study was to use the accelerometer “KiRA” to evaluate the efficacy of measuring PS.

Methods

Between 2010 and 2011, a total of 100 patients with anterior cruciate ligament (ACL) lesions were enrolled. They underwent surgical reconstruction of the ACL. Among them, 30 patients were re-evaluated at least 6 months after surgery. Each patient underwent a clinical examination (Lachman test, anterior drawer test, and PS test) and then was subjected to an instrumental examination: KT1000 evaluation to quantify the Lachman test and KiRA to quantify the PS test.

Results

The accelerometer found a positive acceleration difference in favour of the pathologic knee. In the 100 patients evaluated preoperatively, the analysed acceleration parameters on the pathologic knee were found to be significantly different with respect to the contralateral joint. Correlating the clinical subjective data with numerical data, we identified mean reference values for every grade of the PS test (negative, glide, and clunk).

Conclusion

Our experience showed us that the use of KiRA accelerometer for quantitative measurement of the PS is both promising and reliable. The efficacy of this instrument is strictly related to an inevitable learning curve and to proper execution of the test. It has the value of being easy to set up and easy to use in both the clinic and the operating room.  相似文献

BACKGROUND: Pain is the most common symptom of patients with rotator cuff tendinopathy, but little is known about the relationship between the site of pain and the site of cuff pathologic lesions. Also, accuracies of physical examinations used to locate a tear by assessing the muscle strength seem to be affected by the threshold for muscle weakness, but no studies have been reported regarding the efficacies of physical examinations in reference to their threshold. HYPOTHESIS: Pain location is useful in locating a tear site. Efficacies of physical examinations to evaluate the function of the cuff muscles depend on the threshold for muscle weakness. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: The authors retrospectively reviewed the clinical charts of 160 shoulders of 149 patients (mean age, 53 years) with either rotator cuff tears (140 shoulders) or cuff tendinitis (20 shoulders). The location of pain was recorded on a standardized form with 6 different areas. The diagnostic accuracies of the following tests were assessed with various thresholds for muscle weakness: supraspinatus test, the external rotation strength test, and the lift-off test. RESULTS: Lateral and anterior portions of the shoulder were the most common sites of pain regardless of existence of tear or tear location. The supraspinatus test was most accurate when it was assessed to have positive results with the muscle strength less than manual muscle testing grade 5, whereas the lift-off test was most accurate with a threshold less than grade 3. The external rotation strength test was most accurate with a threshold of less than grade 4+. CONCLUSION: The authors conclude that pain location is not useful in locating the site of a tear, whereas the physical examinations aiming to locate the tear site are clinically useful when assessed to have positive results with appropriate threshold for muscle weakness.  相似文献

BackgroundThe variation and accuracy of computed tomography angiography (CTA) features of anomalous aortic origin of a coronary artery (AAOCA) have not been completely characterized. We evaluated anomalous right (AAORCA) and left (AAOLCA) coronary arteries by CTA, with an emphasis on reproducibility, comparison with surgical measurements, and effect of nitroglycerin.MethodsCTAs were interpreted for location of coronary origin & exit from the aorta, course, and dominance; minor & major diameters of the proximal & distal coronaries; shape of orifice; and intramural length. Relationships between vessel measurements, body surface area (BSA), and nitroglycerin use were evaluated. Comparisons between CTA and surgical measurements included intramural length, surgical probe size pre-intervention vs. CTA proximal AAOCA diameter, and surgical probe size post-intervention vs. CTA distal AAOCA diameter.ResultsOf 104 patients [81 (78%) AAORCA], all but 1 were intramural. Compared to AAOLCA, AAORCA patients were more likely to have a high origin (91% vs. 31%, p ​< ​0.01), and slit-like orifice (58% vs. 26%, p ​< ​0.01). When CTAs with nitroglycerin were compared to those without, no difference in proximal AAOCA dimensions was identified; however distal AAOCA and normal-origin coronary dimensions were larger in scans with nitroglycerin. Aside from slit-like orifice, reliability of coronary CTA measurements, including proximal AAOCA minor diameter and intramural length, was moderate to good between readers. In a subgroup of 54 patients, proximal AAOCA minor diameter and intramural length had good agreement and correlation with surgery.ConclusionProximal AAOCA dimensions and intramural length are reproducible variables. The lack of difference in proximal AAOCA dimensions with nitroglycerin may reflect abnormal vessel mechanics. Multicenter studies are an important next step in understanding the generalizability of our findings.  相似文献

Background  Studies directly evaluating the reliability of the Risser sign are few in number, possess small sample sizes, and offer conflicting results. This study establishes the reliability of the Risser sign on a large sample size in an effort to provide clarification on the subject. Methods  Two years’ worth of AP pelvis radiographs from patients age 8–20 were downloaded from our institution’s digital imaging system. One hundred of these images were selected for inclusion by an independent reviewer whose goal was to capture a spread of radiographs that included all Risser stages. Risser grading occurred in two rounds. In each round, three examiners randomly reviewed the 100 radiographs on three different occasions. The full AP pelvis radiograph was graded in Round 1 while only the iliac apophysis was visible in Round 2. Kappa coefficients and their confidence bounds are reported to indicate intra- and inter-observer reliability. The contrast between the rates of agreement about Risser stages in Rounds 1 versus 2 was assessed by McNemar’s test. The signed-rank test was used to evaluate differences in intra-observer values between rounds. Results  Round 1 inter-observer kappa was 0.76. Round 2 inter-observer kappa was 0.51. In Round 1, 63 radiographs showed perfect agreement within the same Risser stage for all observations compared to 44 radiographs with perfect agreement within the same Risser stage in Round 2 (p = 0.004). Round 1 intra-observer kappa values were 0.92, 0.86, and 0.88. Round 2 intra-observer kappa values were 0.91, 0.77, and 0.88. Intra-observer value differences between rounds were not significant for two observers (p = 0.074, 0.061) but was significant for the third observer (p = 0.002). Conclusion  The reliability of the Risser sign is acceptable and can be further improved when other markers of skeletal maturity on the pelvis radiograph are used to assist in grading. Purpose We hypothesized that combining pre- and postsynaptic quantitative information about the dopaminergic system would provide a higher diagnostic accuracy in the differential diagnosis of parkinsonism than specific striatal D2 receptor binding alone. Therefore, the aim of the study was to introduce new semi-quantitative parameters and evaluate their ability to discriminate between Parkinson’s disease (IPS) and non-idiopathic parkinsonian syndromes (non-IPS). Methods In 100 patients (69 IPS, 31 non-IPS), postsynaptic [123I]IBZM and presynaptic [123I]FP-CIT SPECT scans were evaluated by observer-independent techniques. The diagnostic performances of striatal dopamine transporter (DAT) and D2 receptor binding, their respective asymmetries, and a combination of pre- and postsynaptic asymmetry were evaluated with ROC analyses. A logistic regression model was generated combining factors to calculate the probability for each patient of belonging to either diagnostic group. Results D2 receptor binding provided a sensitivity of 87.1% and a specificity of 72.5% with an area under the curve (AUC) of 0.866. The AUCs of other single parameters were lower than that of D2 binding. A gain of diagnostic power (p = 0.026) was reached with a model combining pre- and postsynaptic asymmetries and D2 binding (sensitivity 90.3%, specificity 73.9%, AUC 0.893). Conclusion The combination of quantitative parameters of presynaptic DAT and postsynaptic D2 receptor binding demonstrates superior diagnostic power in the differentiation of patients with IPS and non-IPS than the established approach based on D2 binding alone. Striatal D2 receptor binding and the combination of DAT and IBZM binding asymmetries are the factors contributing most in separating these diagnostic groups.  相似文献

PURPOSE: To define the beneficial and detrimental effects of adding exercise to direct magnetic resonance (MR) shoulder arthrography. MATERIALS AND METHODS: Direct, intraarticular, gadolinium arthrography of the shoulder was performed in 41 patients, who underwent 1.5-T MR imaging before and after 1 minute of arm swinging. Fourteen milliliters of dilute gadolinium solution was injected. Two readers blinded to exercise independently graded the randomly distributed images with a five-point scale for capsular contrast material resorption; extraarticular contrast material leakage; rotator cuff, glenoid labrum, and anterior capsule conspicuity; and partial-thickness or full-thickness rotator cuff tear and labral tear detectability. The sign test was performed to evaluate the significance of differences between preexercise and postexercise grading for each reader. A second review was performed, with direct side-by-side comparison of preexercise and postexercise images. RESULTS: There was evidence of increased capsular resorption after exercise but no alteration in the depiction of the rotator cuff tendons or glenoid labrum. There was no significant extraarticular contrast material leakage after exercise and no alteration in depiction of the anterior capsule. There was no difference in the detectability of rotator cuff or labral tears. CONCLUSION: Exercise with direct shoulder MR arthrography has no beneficial or detrimental effect on image quality or on the depiction of rotator cuff or labral tears.  相似文献

AIM: To show whether the clinical and radiological features of newly developed ulcer-like projections (nULPs) in an aortic intramural haematoma (IMH) on follow-up computed tomography (CT) images, are different from those of the initial ULPs (iULPs) on the initial CT images. MATERIALS AND METHODS: A review of the radiological database revealed 98 patients with IMH with at least two follow-up CT examinations with a follow-up period of more than 1 month. The patients were divided into four groups: patients without iULPs or nULPs throughout the follow-up periods (group A); patients with iULPs on the initial CT images (group B); patients with nULPs on follow-up CT images but without iULPs on the initial CT images (group C); and patients with both iULPs and nULPs (group D). The type of IMH, aortic diameter, thickness of the haematoma, and complications were analysed. The clinical and CT findings and complications in the four groups were compared. RESULTS: Forty-two patients had no iULPs nor nULPs (group A); 27 patients had 45 iULPs on the initial CT images (group B); 16 patients had 17 nULPs on follow-up CT images without any ULP on the initial CT images (group C); and 21 nULPs developed in 13 patients with iULPs (group D). There was no significant difference in the demographic or initial CT findings in the four groups. There was no statistical difference in the incidence of complications between groups B (59.3%), group C (62.5%), and group D (69.2%; p=0.830), but there was a significant difference in the incidence of complications between the patients without any ULPs (21.5% in group A) and those with ULPs (62.5% in groups B, C, and D). CONCLUSION: There were no significant differences in the CT findings or complications between the patients with iULPs and nULPs. Regardless of the developing time of the ULPs, the incidence of complications of IMH in patients with ULPs was higher than that in those without ULPs. Careful and regular follow-up CT examinations are needed for patients with ULPs.  相似文献

Current forensic DNA profiling kits and techniques enable the detection of trace amounts of DNA. With advancements in kit sensitivity, there is an increased probability of detecting DNA from contamination. Research into DNA transfer within operational forensic laboratories provides insight into the possible mechanisms that may lead to exhibit contamination. To gain a greater understanding of the potential for evidence bags to act as DNA transfer vectors, the level of DNA accumulating on the exterior of evidence bags during the exhibit examination process was investigated. The exterior of 60 evidence bags were tapelifted before and after the examination of the exhibit inside of the bag resulting in 120 DNA profiles. These DNA profiles were compared to DNA profiles of staff working within the building and samples taken from the exhibit inside the bag. Common DNA profile contributors from each sample were also identified through STRmix™ mixture to mixture analysis. The average DNA quantity and number of profile contributors was higher in samples taken from the bag before exhibit examination than after examination. Fifty six percent of all samples taken identified a match between DNA recovered from the evidence bag and at least one staff member. On 11 bags, a common contributor was identified between the exhibit in the bag and the exhibit package post-examination. In one instance a DNA profile, matching that of a donor, on the exhibit bag before examination was also detected on a sample taken from the exhibit, raising the possibility of outer bag-to-exhibit DNA contamination. This study demonstrates that operational forensic laboratories must consider exhibit packages as a potential source of DNA contamination and evaluate their exhibit handling and storage procedures accordingly.  相似文献

OBJECTIVE: The meniscus is considered "extruded" when it extends beyond the tibial margin. We hypothesize that severe degeneration, large radial tears, complex tears, and tears involving the meniscal root would alter meniscal stability and cause more substantial extrusion. MATERIALS AND METHODS: The knee MRI database at Thomas Jefferson University Hospital was searched for reports describing meniscal extrusion; MR images were reviewed retrospectively. On mid coronal images, extrusion of the medial meniscus was quantified in millimeters. A separate, independent review of the meniscus evaluated degeneration severity and tear (type and extent). Radial tears were divided into those involving more (large) or less (small) than 50% of the meniscal width. Tears that involved the "root" at the tibial spine were recorded. Chi-square analysis compared these findings with extrusion extent, divided into minor ( 3 mm) extrusion. RESULTS: One hundred five knees were reviewed (12 men and 93 women; age range, 34-83 years; mean age, 56 years). Distribution of medial meniscus extrusion was 2 mm (n = 17), 3 mm (n = 17), 4 mm (n = 27), 5 mm (n = 14), 6 mm (n = 16), and 7-10 mm (n = 14). Mild, moderate, or marked degeneration was seen in 47%, 26%, and 27% with minor extrusion, respectively, and in 17%, 41%, and 42% with major extrusion, respectively (p = 0.003). Tears were seen in 59% (20/34) with minor extrusion versus 89% (63/71) with major extrusion (p = 0.001). Tears involved one third, two thirds, or all of the meniscus in 75%, 25%, or 0%, respectively, with minor extrusion and 46%, 40%, or 14% with major extrusion, respectively (p = 0.014). Longitudinal (nonradial) and horizontal tears were not associated with extent of extrusion (p = 1.0). Oblique tears were significantly associated with minor extrusion (minor, 26% [9/34]; major, 4% [3/71]; p = 0.003). Radial tears were seen in 9% (3/34) with minor extrusion versus 21% (15/71) with major extrusion (p = 0.20). All three radial tears with minor extrusion were small; conversely, 87% (13/15) of radial tears with major extrusion were large (p = 0.019). Complex tears were seen in 18% (6/34) with minor extrusion versus 59% (42/71) with major extrusion (p < 0.001). Tears involving the meniscal root were seen in 3% (1/34) with minor extrusion and 42% (30/71) with major extrusion (p < 0.001). CONCLUSION: Substantial medial meniscus extrusion (> 3 mm) is associated with severe meniscal degeneration, extensive tear, complex tear, large radial tear, and tear involving the meniscal root.  相似文献

OBJECTIVE: The purpose of this article is to describe a practical imaging approach to evaluating collections in the retropharyngeal space. CONCLUSION: The differential diagnoses for fluid in the retropharyngeal space include both noninfectious and infectious processes. The multiplanar capabilities of CT and MRI are ideal for characterizing and delineating collections. In this pictorial essay, we describe the anatomy of the retropharyngeal space and offer a four-step approach to evaluating retropharyngeal collections on multiplanar imaging.  相似文献

Whiplash injury has been a continuous source of controversy due to its association with litigation. We studied the incidence of whiplash associated disorder (WAD) in two similar socioeconomic areas and carried out a retrospective study based on the hypothesis that the Spanish law 30/1995 might have an affect on the incidence and duration of cervical symptoms and the persistence of impairment. More than 10,000 patients injured in traffic accidents were studied over a period three years. Of these, only patients with an initial diagnosis of whiplash injury were included in the study. Patients with other injuries were excluded. The patients were classified into two groups: Galicia-Spain and North-Central Portugal (depending on where the accident took place and the medico-legal evaluation procedure in force). Statistical analysis was made using SPSS 13.0 and Statistix 8.0. We found a statistically significant difference between Spain and Portugal in the incidence of WAD and in the duration of symptoms. The incongruities caused by the compulsory application of Spanish law arise from the fact that evaluation on a points scale of impairment does not always reflect the functional state of the injured person.  相似文献
