Objective PET with 18F-fluorodeoxyglucose (FDG) has been used to beth detect and stage a variety of malignancies. The aim of this retrospective study was to evaluate the clinical value of 18F-FDG PET/CT for recurrence and metastasis in gastric carcinoma patients after total gastrectomy. Methods A total of 45 gastric carcinoma patients who underwent total gastrectomy were included. PET/CT scans were obtained for restaging. The " gold standard" of 18F-FDG PET/CT diagnostic accuracy was based on either histopathology or clinical follow-up. By using the t-test from SPSS 11.5, the cut-off of maximum standard-ized uptake values (SUVmax) from 18F-FDG to differentiate benign from malignant lesion at stomach were determined and calculated. Results (1) Of the 45 patients, 22 were suspicion recurrent lesion at stomach. Of the 22 patients, 12 were confirmed to have recurrent lesions. The diagnostic accuracy were 100.0% (12/ 12) for sensitivity, 70.0% (7/10) for specificity, and 86.4% (19/22) for accuracy, respectively. A higher SUVmax in malignant than benign lesion was noted (6.27 ± 3.42 vs 3.92 ± 2.24), though not reached to the significance (t = 1. 862, P > 0.05). (2) For extra-gastric lesion detection, the sensitivity, specificity, and accuracy for region lymph nodes were 78.9% (15/19), 92.3% (24/26), and 86.7% (39/ 45), for peritoneal spreading were 6/9, 97.2% (35/36), 91. 1% (41/45), and for distant sites were 86.7%(13/15), 93.3% (28/30), 91.1% (41/45). (3) False positive were found in eight sites. All were either inflammatory or physiological uptake at intestine. False negative were found in nine sites. Either due to small in size (less than 1.0 cm in diameter), well differentiation of the tumor cell or with a nature of signet ring. Conclusion 18F-FDG PET/CT had a potential to detect local, regional, and distant metastasis in gastric cancer.  相似文献

目的探讨18F-FDG 符合线路 SPECT 显像联合传统显像(CT、B 超、MRI)在结直肠癌术后复发及转移中的诊断价值.资料与方法107例结直肠癌术后患者行18F-FDG 符合线路 SPECT 显像,并联合 CT、B 超、MRI 诊断结直肠癌术后复发及转移.结果107例患者最终确定复发及转移共29例.18F-FDG 符合线路 SPECT 显像与传统显像诊断均为阳性的15例患者中,传统显像检出病灶20个,符合线路 SPECT 显像检出病灶26个.传统显像检出3个符合线路 SPECT 显像未发现的转移病灶,而符合线路 SPECT 显像检出9个传统显像未发现的病灶.18F-FDG 符合线路 SPECT 显像诊断结直肠癌术后复发及转移的灵敏度、特异性和准确率分别为79.3%、89.7%和86.9%;传统显像的诊断效能分别为62.1%、88.5%和81.3%;二者联合的诊断效能分别为89.7%、92.3%和91.6%.二者联合诊断结直肠癌术后复发及转移的灵敏度和准确率明显优于传统显像(P <0.05).结论18F-FDG 符合线路 SPECT 显像联合 CT、B 超、MRI 在监测结直肠癌术后复发及转移中具有重要临床价值.  相似文献

Objective

Until now, serum tumor markers, physical examination, and conventional imaging modalities, such as CT or MRI, have been used in assessment of recurrence of cervical cancer after treatment. However, CT and MRI provide only anatomical data, which makes analysis of post-treatment change difficult. This study aims to explore the effectiveness of PET/CT, a new scanning device that combines PET and CT, in evaluation of cervical cancer lesions in patients with suspected recurrence.

Methods

We studied 51 patients suspected of recurrence among those who underwent F-18 FDG PET/CT for cervical cancer follow-up at Gachon University Gil Hospital between June 2006 and August 2009. Patients were considered to be at risk for recurrence if they reported symptoms that were clinically suggestive of recurrence, or if physical examination showed abnormalities, serum tumor marker levels rose, or follow-up images revealed changes, such as new lesions or swelling of previous sites. Sensitivity, accuracy, specificity, and positive and negative predictive values of PET/CT were measured.

Results

A total of 37 patients were confirmed with recurrence or metastasis, 13 of whom were diagnosed histologically. Measured across all patients, PET/CT scored 97.3% on sensitivity, 71.4% on specificity, a positive predictive value of 90%, a negative predictive value of 90.9%, and an accuracy of 90.2%. PET/CT yielded only one false negative diagnosis and four false positives.

Conclusion

As F-18 FDG PET/CT has high sensitivity and negative predictive value in diagnosis of recurrent cervical cancers, it is expected that it will be useful for clinical determination of recurrence and prevention of unnecessary additional treatments. The hope is that a future study on a larger scale will contribute further to determination of the efficacy of PET/CT.  相似文献

目的分析肺癌肾上腺转移患者的18F-脱氧葡萄糖正电子发射计算机断层显像(18-fluorodeoxyglucose positron emission tomography/computed tomography,18F-FDG PET/CT)特征,并评价其临床应用价值.资料与方法回顾性分析本院PET-CT中心检查发现并最终确诊的肺癌肾上腺转移患者31例,共35个肾上腺转移灶,分析其CT和PET图像特征,将同期转移患者原发灶和转移灶的最大SUV(SUVmax)进行相关性分析,并用t检验比较同期转移和异期转移病灶的SUVmax的差异。结果肺癌肾上腺转移瘤在CT表现为类圆形或不规则形的结节灶,35个病灶中密度均匀者23个,不均匀者12个,直径为0.8~10cm,平均为2.56±1.68cm;PET图像上最大SUV为1.9~22.1,平均为7.02±4.28。肺癌原发灶SUVmax为2.3~18.9,平均为8.07±4.59;肾上腺转移灶的SUVmax与原发灶的SUVmax无相关性(P=0.009,P=0.972)。初诊转移与随诊转移的肾上腺转移瘤的SUVmax的差异无统计学意义(t=-0.904,P=0.373)。显示3例病灶肾上腺转移瘤的变化随原发灶的好转而同步改善。结论 PET/CT检查对肺癌肾上腺转移的诊断有重要价值,可作为肾上腺转移癌的检查方法。  相似文献

目的 探讨18F-FDG PET/CT显像在不明原因腹腔积液患者积液性质鉴别诊断中的作用.方法 回顾性分析因不明原因腹腔积液入院的初诊患者70例,所有患者均行18F-FDG PET/CT 显像、同机CT平扫和腹腔积液细胞学检查,PET/CT显像计算病灶SUV.结果经组织病理学、腹腔镜、胃镜、肠镜等检查及临床随访确定,随访时间均大于6个月.对疑有胃肠道病变患者行PET/CT 延迟显像以明确诊断.有无腹膜转移组间SUVmax差异行两样本t检验;PET/CT、同机CT平扫及腹腔积液细胞学检查对腹膜转移诊断效能比较用McNemar检验;三者与最终结果吻合度检验用Kappa检验.结果 腹腔积液患者70例,经手术、腹膜活组织检查或腹腔积液细胞学检查证实48例为腹膜转移癌引起的恶性积液,22例为良性积液.2组腹膜的SUVmax分别为6.08±3.92和2.66±1.74,差异有统计学意义(t=-5.063,P<0.001).PET/CT诊断45例为恶性积液,其中2例为假阳性,另有5例恶性积液被误诊为良性积液,PET/CT诊断腹膜转移的灵敏度、特异性和准确性分别为89.6% (43/48)、90.9% (20/22)和90.0% (63/70).定位CT诊断27例为腹膜转移,其中4例为假阳性,CT 准确诊断良性腹腔积液18例,另有25例假阴性;定位CT诊断腹膜转移的灵敏度、特异性和准确性分别为47.9% (23/48)、81.8% (18/22)和58.6% (41/70).PET/CT与定位CT诊断腹膜转移的灵敏度差异有统计学意义(x2=14.286,P<0.001).70例腹腔积液患者中仅21例腹腔积液中见癌细胞,其余患者细胞学检查均为阴性,腹腔积液细胞学对腹膜转移诊断的灵敏度、特异性和准确性分别为43.8% (21/48)、100% (22/22)和61.4% (43/70);PET/CT与细胞学检查对腹膜转移诊断的灵敏度差异有统计学意义(x2=13.885,P<0.001).PET/CT检查结果与最后诊断结果吻合度较强(Kappa=0.776,P<0.001),定位CT、腹腔积液细胞学检查结果与最后诊断结果吻合度均较弱(Kappa值分别为0.236和0.328,P均<0.05).结论 18F-FDG PET/CT显像对不明原因腹腔积液患者腹膜转移诊断有重要临床价值,可用于不明原因腹腔积液的良恶性鉴别诊断.  相似文献

目的 探讨18F-FDG PET/CT在胸段食管鳞癌淋巴结转移中的诊断价值,并与增强CT及术后病理结果进行对比。 方法 胸段食管鳞癌患者54例,术前一周内行18F-FDG PET/CT和增强CT扫描,术后以病理学诊断为“金标准”,比较两种显像方法对胸段食管鳞癌淋巴结转移的灵敏度、特异度、准确率及不同短径范围的诊断准确率。应用SPSS13.0软件进行四格表χ2检验。 结果 手术共切取肿大淋巴结186枚,均经病理证实,其中淋巴结转移77枚,18F-FDG PET/CT诊断淋巴结转移的灵敏度、特异度及准确率分别为77.9%(60/77)、95.4%(104/109)和88.2%(164/186),增强CT分别为40.3%(31/77)、75.2%(82/109)和60.8%(113/186),二者诊断效能差异有统计学意义(χ2=24.04、15.77和36.77,P均 < 0.01)。对于短径>5 mm(5~10 mm及≥10 mm)的淋巴结,18F-FDG PET/CT诊断的准确率高于增强CT,且差异有统计学意义(χ2=26.03和26.47,P均 < 0.01);对于短径≤5 mm的淋巴结,二者间诊断的准确率差异无统计学意义(χ2=0.24,P>0.05)。 结论 18F-FDG PET/CT在胸段食管鳞癌淋巴结转移中具有较高的诊断价值,对于短径>5 mm的淋巴结优势明显。  相似文献

目的评价^18F-脱氧葡萄糖(FDG)PET/CT监测胃癌术后复发及转移的临床价值。方法回顾性分析胃癌术后临床疑复发或转移而行^18F-FDG PET/CT检查的45例患者临床资料,根据细胞学、病理或随访结果分析PET/CT检查结果,计算残胃及转移灶最大标准摄取值(SUVmax)。采用SPSS11.5软件进行数据处理,SUVmax两样本均数比较采用t检验。结果(1)45例中22例临床疑残胃复发,12例患者最终确定残胃复发,^18F-FDG PET/CT诊断残胃复发的灵敏度、特异性及准确性分别为100.0%(12/12)、70.0%(7/10)和86.4%(19/22),12例复发病灶SUVmax为6.27±3.42(其中2例低分化腺癌的SUVmax较低,分别为2.5和3.3),另10例无复发的残胃SUVmax为3.92±2.24(其中3例吻合口炎性病灶局部SUVmax较高,分别为8.3,5.2和6.3),差异无统计学意义(t=1.862,P〉0.05)。(2)对残胃以外转移灶的检出:PET/CT灵敏度、特异性、准确性在区域淋巴结转移中分别为78.9%(15/19)、92.3%(24/26)和86.7%(39/45),在腹膜转移中分别为6/9、97.2%(35/36)和91.1%(41/45),在远处转移中分别为86.7%(13/15)、93.3%(28/30)和91.1%(41/45)。(3)PET/CT假阳性8处为炎性或肠管摄取,假阴性9处多为直径〈1.0cm的转移灶和低分化腺癌及印戒细胞癌的转移灶。结论^18F-FDG PET/CT虽然检测胃癌术后区域淋巴结和腹膜转移的灵敏度较低,但仍是监测其术后复发转移的有效手段。  相似文献

目的 探讨18F-FDG PET/CT对临床怀疑宫颈癌局部复发和(或)远处转移患者的诊断价值.方法 回顾性分析95例宫颈癌治疗后患者的18F-FDG PET/CT检查资料,采用视觉分析和半定量方法(SUVmax)分析病灶特点,最终诊断以活组织病理检查、诊断性治疗及影像学随访结果为准,采用Kappa一致性检验进行分析.结果 共有54例患者18 F-FDG PET/CT检查发现局部复发和(或)远处转移病灶,其中局部复发24例,远处转移30例;18F-FDG PET/CT诊断肿瘤复发与转移的灵敏度、特异性和准确性分别为98.1%(52/53)、95.2% (40/42)和96.8% (92/95),阳性预测值与阴性预测值分别为96.3% (52/54)和97.6%(40/41).18F-FDG PET/CT检查结果与病理及临床随访结果一致性良好(Kappa=0.936,P<0.05).结论 18F-FDG PET/CT对临床可疑宫颈癌复发的诊断具有较高的灵敏度和特异性,有助于局部复发和(或)远处转移病灶的检测,对临床进一步的诊疗具有指导作用.  相似文献

目的 分析前列腺癌骨转移的形态及脱氧葡萄糖(FDG)代谢特点,探讨PET/CT的临床应用价值.方法 回顾性分析27例依据影像、临床或组织病理诊断为前列腺癌骨转移患者的PET/CT资料,总结骨转移瘤的分布、数目、大小、密度及FDG摄取等特点,分析病变大小与最大标准化摄取值(SUVmax)的相关性.结果 27例中,弥漫性转移11例,其中10例PET与CT检出病灶数目大致相符,1例PET发现病灶明显多于CT;多发及单发转移16例,选择测量病变51个,大小平均(1.9±1.0)cm,SUVmax平均为6.5±3.7,二者呈正相关.48个病变代谢活跃,其中成骨性46个(3个病灶PET浓聚范围大于CT所示),溶骨性1个,1个病灶CT未显示;3个病灶代谢与周围骨质接近,其中1个为成骨性改变,另2个CT未显示.结论 PET/CT诊断前列腺癌骨转移的敏感性较高,可能会漏诊代谢不活跃的小病灶.  相似文献
