The complete mouth reconstruction of a periodontally compromised dentition is a tremendous challenge. Although the treatment is complex, the rewards can be satisfying. In some cases, the use of telescopic copings overcomes several of the problems associated with periodontal prosthetic treatment. Combined orthodontic-prosthodontic procedures will enhance the results of restorative treatment in adult patients. Restoration of malposed teeth using prostheses alone will fail both esthetically and functionally. Rotation, spacing or crowding and tipped teeth will compromise the final restoration if left untreated. Correction of axial inclination will prevent pulp exposure during preparation, as well as improving the periodontal status of the involved teeth. Cooperation between the orthodontist and the prosthodontist will ensure a more favorable final result. Cases treated in this manner are presented, illustrating the orthodontic treatment and the successful end results.  相似文献

The use of resin cements in combination with dentin bonding agents can result in superior attachment of prostheses to tooth structure. This paper describes four clinical cases in which dentin-bonded resin cements were used to overcome retention problems. In the first case, a detached fixed partial denture, which was in good condition when separated, was recemented to abutment teeth prepared with less-than-ideal angle of convergence. In the second case, a detached all-porcelain crown was recemented with a dentin-bonded resin cement after appropriate surface treatment. In the third case, a porcelain-fused-to-metal crown made for a molar tooth was cemented to a short clinical crown, avoiding crown-lengthening surgery. In the fourth case, a 3-unit fixed partial denture was recemented to abutments with less-than-ideal supporting features. Dentin-bonded resin cements can help to extend the life of detached prostheses until the patient is financially prepared for replacement or it can help to avoid crown-lengthening surgery.  相似文献

Statement of problem

Microstereolithography (μ-SLA), a form of additive manufacturing, can produce one or more platforms of resin copings. However, no evaluation has been made of the variation in marginal discrepancy using this method, even though this is an important factor for a successful restoration.

Purpose

The purpose of this in vitro study was to evaluate the reproducibility and marginal discrepancy of resin copings fabricated using dental μ-SLA.

Material and methods

A master die of a mandibular right first molar tooth was made from Type IV stone and scanned to produce a stereolithography file. Resin copings were then fabricated using μ-SLA additive manufacturing by repeating 1, 3, or 6 arrays to give a total number of 18. The marginal discrepancies of these resin copings were measured using digital microscopy (at ×160 magnification), and the data obtained were analyzed using a nonparametric Kruskal-Wallis H test, post hoc Mann-Whitney U-test, and Bonferroni correction.

Results

The mean ±SD total marginal discrepancies of 1, 3, and 6 arrays were found to be 72.2 ±39.1 μm, 61.2 ±37.3 μm, and 92.5 ±54.1 μm. Statistically significant differences were found among the compared groups (P<.05).

Conclusions

Based on the marginal discrepancy, μ-SLA of additive manufacturing is more precise when 3 arrays are used than when 1 or 6 arrays are used on a single build platform. Because the fit is affected by the number of copings fabricated, further research of multiple resin copings is required.  相似文献

ObjectivesThis study aims to determine the effects of multiple translucent layers, the alignment of composite structures, and specular reflecting backings as goniochromatic mechanisms relevant to dentistry.MethodsRectangular composite specimens were filled with very short E-glass fibers (120 μm length, 16 μm diameter). The fibers were oriented random, perpendicular, and parallel to the surface normal using an electric field. A Minolta CS-100 colorimeter was used to measure the color at various angles of samples with various filler alignments, translucent sublayers, and specular reflecting backings.ResultsAll three investigated mechanisms were proven to have a goniochromatic effect. Filler alignment perpendicular to the surface made the composites more transmissive and reduced the dependence of lightness on observation angle. Backing composites with a pigmented sublayer caused the color of the sublayer to be more apparent when the observation angle is perpendicular to the surface. The specular reflection of a gold backing was only partially diffused by the specimen.SignificanceThe determination of the effects of these goniochromatic mechanisms is relevant to dentistry, because teeth are naturally aligned composites composed of multiple translucent layers. Therefore, understanding these goniochromatic effects is important to recreating them in restorative materials. Also, gold backings were previously found to give restoratives a more vital appearance, and this perceived vitality may be related to goniochromism.  相似文献
