目的:采取核酸检测(NAT )技术进行血液筛查,减少由献血者感染“窗口期”及隐匿感染献血引起的疾病传播。方法采用酶联免疫吸附法(ELISA)对54358份献血者标本进行乙型肝炎表面抗原(HBsAg)、人类免疫缺陷病毒抗体(抗-HIV)、丙型肝炎病毒抗体(HCV-Ab)检测,同时用转录介导扩增技术(TMA)进行 HBV-DNA 、HCV-RNA 检测,人类免疫缺陷病毒1型(HIV-1)RNA 核酸检测。结果 NAT 检测阳性 ELISA 检测阴性标本共51份。51份标本有鉴别试验结果的33份,其中32份 HBV-DNA 阳性,1份 HIV-1 RNA 阳性,HIV-1 RNA 阳性的献血者经南京市疾病预防控制中心免疫印迹法确认为 HIV-1疑似阳性(gp160和 p24±)。半年后再次回访该献血者,抽取血样送南京市疾控中心经免疫印迹法确认为 HIV-1阳性(gp160、gp120、p66、p51、gp41、p31、p24、p17+)。结论 NAT 技术可以减少由献血者感染“窗口期”及隐匿感染献血引起的疾病传播,从而减少输血风险。  相似文献

Background

Few cases of transfusion-transmitted hepatitis E virus (HEV) have been published in Spain. Here, we describe a well-characterized lookback investigation of a transfusion-transmitted HEV case at the Community Centre for Blood and Tissues of Asturias (Spain).

Case Report

A female patient with chronic myeloid leukemia underwent an allogeneic bone marrow transplant in March 2019 and showed alterations in liver function shortly afterwards. This patient received blood components from 30 different donors in the 3 months before the transplant. Frozen plasma samples from these donations were investigated for the presence of HEV-RNA. One frequent donor was identified as asymptomatic HEV RNA-positive at the time of his whole blood donation. The investigation revealed that this donor's plasma unit, originally intended for the fractionation industry, had a viral RNA concentration of 1.9 × 104 copies/mL. HEV RNA was detected initially in the index patient who received the red cell concentrate from this donor 25 days after the transfusion. HEV RNA isolated from both donor and recipient were identified as subtype 3f. The recipient of platelet concentrate (PC), treated with a riboflavin-based pathogen reduction technology (PRT) was not infected, being negative for the presence of HEV IgM, IgG, and HEV RNA before and after the transfusion.

Conclusion

This case study shows that HEV was transmitted through red cell transfusion to a recipient, while the patient who received riboflavin/UV light treated PC did not develop signs of infection. A causal relationship between PRT treatment of the PC and the non-transmission of HEV remains to be established.  相似文献

目的探讨住院患者输血前检测4项指标在医院感染管理和控制中的临床意义,以便做好自我防护及预防交叉感染,同时避免医疗纠纷的发生。方法对本院2007~2010年3 237例住院输血前患者采用酶联免疫吸附试验(ELISA)进行血清学乙型肝炎病毒表面抗原(HBsAg)、人类免疫缺陷病毒(Ⅰ型、Ⅱ型)抗体(抗-HIV1/2)、丙型肝炎抗体(抗-HCV)、梅毒抗体(抗-TP)检测分析。结果检测样本总阳性数531例,总阳性率16.40%。其中检出HBsAg阳性362例、抗-HIV阳性13例、抗-HCV阳性50例、抗-TP阳性116例,经梅毒螺旋体血细胞凝集试验确证106例。不同性别的检测情况来看,男性抗-TP阳性率(4.16%)明显高于女性(2.52%),差异具有统计学意义(P<0.05)。结论对输血患者进行输血前血清学4项检测,明确患者的健康状况,可避免或减少因输血产生的有关血源性传播疾病的医疗纠纷,为医疗纠纷的处理提供法律依据,同时有利于医患双方的自我保护。  相似文献

Safety in blood transfusion has all along been focused on blood borne viruses like HIV, hepatitis B and C. However, infective agents that are not usually transmitted through blood may also pose risk to transfusion if the donor gives blood in the early-viraemic phase. A case report of potential transfusion-transmitted hepatitis E (HEV) is described. It shows the virus can be transmissible via blood because of the presence of HEV RNA in the blood donated. Pre-donation health screening was unable to exclude this asymptomatic donor. But donor-initiated call back system which acts as an additional safety net prevented the release of the potential infective blood products.  相似文献

The 2022 monkeypox (MPX) outbreak was a source of consternation and stress among men who have sex with men (MSM)—the population disproportionately affected by the recent MPX outbreak. Individuals at high-risk for MPX (e.g., MSM, sex workers) were prioritized for vaccination during early state and city health department vaccine initiatives across the U.S., but little is known about how the MSM sex work market was affected by the MPX outbreak in U.S. cities where MPX incidence was particularly high. To better characterize the potential relationship between MPX incidence and MSM sex worker advertising at the height of the U.S. MPX outbreak, we monitored advertising volume on an Internet advertising platform popular among MSM sex workers and compared these data to city and county health department MPX case reporting data during a 13-week period (July 2nd, 2022- September 24th, 2022) in three U.S. cities with peak MPX incidence: Chicago, New York City, and San Francisco. In each of these cities, we observed a decline in advertising during or immediately following study week(s) with the highest number of weekly MPX cases during the study period, recognizing that observed decreases in advertising could be attributed to transient increase(s) in advertising during large-scale LGBT festivals occurring during the study period.  相似文献
