An international course on strategic information management for medical informatics students: international perspectives and evaluation

All over the world, countries more and more take part in the international society and economy. To meet the stringent requirements of this globalization asks for internationally oriented and well-educated graduates. A major challenge of academia thus lies in qualifying graduates for international positions in this new world. A crucial element in the training and education of tomorrow's medical informatics specialists is exposure to health care systems across national borders. In this contribution, we report on the international aspects of and experiences with an inter-university course for medical informatics students on hospital information systems, in particular on their strategic information management. From 2001 onwards, this course was offered jointly for students of the University of Amsterdam, the University of Heidelberg/University of Applied Sciences Heilbronn and the University of Health Informatics and Technology, Tyrol (UMIT). Based on our experiences, future establishment of international courses in the medical and health informatics field is recommended.

医院信息系统是利用计算机技术、网络技术、数据库技术对医院进行现代化、数据化管理,为各级领导、部门提供可靠的参考数据与决策支持。在医院管理、临床医疗、护理、财务、后勤物资、医保等多层次多部门之间发挥着重要的作用,并可促使医院的管理模式发生重大变革。医院信息的特点是信息量大,增长快速且复杂,同时各部门之间(包括院外,如医保)的数据需要实现共享及交换。     

医学影像设备管理系统的设计与开发

目的：设计和开发出一套功能全面、操作简便而又实用的放射科设备管理信息系统。方法：本论文从软件工程的角度出发进行分析和设计,采用J2EE、SPRING、HIBERNATE等先进流行的Web框架技术,对一款医学影像设备管理维护系统进行初步设计和开发。结果：根据对现有医院影像设备管理的需求分析和实地调研,完成了一款医学影像设备管理维护系统的具体开发,系统主要功能模块包括系统管理、采购管理、设备信息管理、故障信息管理、设备维护管理、设备说明书管理等等。结论：该医学影像设备管理系统总体来说,功能全面、操作简便而又实用,能够满足医院影像设备管理的大部分需求,极大的提高了医学影像设备的管理效率。     

Virtual polyclinic--consultant health service for rural areas and islands

The development of medical sciences and digital revolution in health care have made possible the creation of new models for hospital care and primary health care. Health care management usually does not follow all possible solutions of new computer technology. Croatian health system is based on large hospital centers in the mainland and small, poorly equipped remote medical units. The possibility of sharing medical information using telematic technologies brings all knowledge of the large, powerful hospital centers close to remote medical units. Working in a small, remote consultation office on the island Cres, the author A. M. became part of medical periphery far from the security of hospital on the mainland. After five years of the work on telemedicine projects for the islands, we have introduced a new medical practice concept of communication between remote medical units and clinical hospital centers in the mainland. The pilot project Virtual Polyclinic-Medical Consultation System for the Islands and Remote Areas is a good example of the possible new models in health care structure. Our experience indicates that organization of health care in remote areas depends not only on advanced technical solutions but also on team work and medical professionals.     

The Impact of Public Hospital Closure on Medical and Residency Education: Implications and Recommendations

Background:Challenges around safety-net hospital closure have impacted medical student and resident exposure to urban public healthcare sites that may influence their future practice choices.Objective:To assess the impact of the closure of a public safety-net teaching hospital for the clinical medical education of Charles Drew University medical students and residents.Method:Retrospective cohort study of medical students’ and residents’ and clinical placement into safety-net experiences after the closure of the primary teaching hospital.Results:The hospital closure impacted both medical student and residency training experiences. Only 71% (17/24) of medical student rotations and 13% (23/180) of residents were maintained at public safety-net clinical sittings. The closure of the public safety-net hospital resulted in the loss of 36% of residency training spots sponsored by historically black medical schools in the United States and an even larger negative impact on the number of physicians training in underserved urban areas of Los Angeles County.Conclusion:While the medical educational program changes undertaken in the wake of hospital closure have negatively affected the immediate clinical educational experiences of medical students and residents, it remains to be seen whether the training site location changes will alter their long-term preferences in specialty choice and practice location.     

Medical equipment inventory and service scheduling by computer: experience with a working scheme

In Liverpool Health Authority a computer inventory of over 6000 items of medical equipment is used as an aid to servicing and management. Extracts from the inventory are used to guide work in several different servicing units and the cross-referencing facility is employed to provide lists of equipment by category (for example, manufacturer, hospital, department, cost band, age etc.). After four years' experience of this scheme we conclude that the costs are justified by the benefits to the equipment servicing programmes and to the management of equipment. However, the information made available for other purposes within the health authority (administration, supplies, finance etc.) has not been made use of to the extent that the whole cost could be justified as a management service. We have also found that the quality of inventory records and usage are improved where technicians can use the computer directly, and we find that medical physics technicians quickly come to understand such systems thereby minimizing support costs.     

An international course on strategic information management for medical informatics students: aim, content, structure, and experiences

We report on a course for medical informatics students on hospital information systems, especially on its strategic information management. Starting as course at the Medical Informatics Program of the University of Heidelberg/University of Applied Sciences Heilbronn, it is now organized as international course in the framework of the International Partnership for Health Informatics Education (http:// www.iphie.org) jointly for medical information science students from the University of Amsterdam, medical informatics students, as well as health information management students from the Universities of Heidelberg/Heilbronn. In 2002, medical informatics students from the Master of Science program of the newly founded University for Health Informatics and Technology Tyrol (UMIT) at Innsbruck, Austria, joined. We report about the aim of this course, its audience, and the educational programs involved, about its content and structure, as well as about our experiences gained so far.
