Purpose: To examine the utility of low-field, dedicated extremity MRI for assessing acute upper extremity trauma in patients with radiographs that are negative for fracture. Secondly, to determine which sequences are most useful when screening for fractures. Methods: Forty-four of 46 patients with acute upper extremity trauma and initial radiographs negative for fracture were imaged with a 0.2-T MRI system. Findings were verified with follow-up clinical assessment and plain radiography. Results: In 21 patients, dedicated extremity MRI demonstrated 26 ultimately proven occult fractures, none of which were seen on the initial conventional radiographs. True fractures demonstrated marrow edema and a linear fracture line on low-field MRI. One bone contusion showed edema with no fracture line and was misdiagnosed as a fracture. Low-field MRI correctly identified 23 remaining patients with no fracture. Sensitivity and specificity for fracture in the 44 patients successfully imaged were 100 % and 96 %, respectively. Additionally, 21 soft-tissue injuries were found incidentally by MRI. However, these may not merely be incidental findings of an acute nature, but may very well be chronic, and therefore merit no specific treatment. T1-weighted gradient-echo and short-tau inversion recovery sequences demonstrated the fracture line and marrow edema to best advantage, and were thus the most useful sequences to assess fractures on low-field MRI. Conclusion: Extremity MRI is highly sensitive and specific for radiographically occult fractures of the upper extremity and can also identify associated soft-tissue injuries. In our study, the management was altered in 45 % of the patients following MRI. When adequate sequences are used this modality can direct appropriate therapy while obviating the added expense and morbidity of unnecessary immobilization and follow-up imaging of patients without fracture.  相似文献

Objective. To review the risk factors and the radiological appearance of insufficiency fractures of the sacrum and acetabular roof. Design and patients. Twenty patients with sacral and acetabular roof insufficiency fractures were reviewed retrospectively. There were 16 women (80%) and 4 males (age range 48–86 years, excluding an 8-year-old boy). Thirteen patients had a known tumour, and nine had received pelvic irradiation. All patients, except one who was asymptomatic, presented with low back or hip pain. In patients with a known tumor, metastases were suspected. Plain radiography (20), bone scintigrams (16), MR examinations (20), and bone densitometry (14) were performed. Nine patients also each had a CT scan. Results and conclusions. In three cases the CT scan performed 10–25 days after onset of symptoms was interpreted as normal. MR examination performed a few days after the CT scan showed in each of these three patients a fracture line with a band of edema. Scintigraphy was very sensitive, but the H-shaped pattern of sacral uptake, specific for an insufficiency fracture, was detected in only three of 16 cases. The earliest MR sign was medullary edema, seen as early as 18 days after the onset of symptoms. On spin echo (SE) T1-weighted images (T1WI), the hypointense signal of edema could mask a fracture line. On SE T2WI the fracture line could be detected within the hyperintense edema (10 of 17 patients with examinations including SE T2WI). However, in four patients a fracture of the sacrum was not seen on T2WI, these having been obtained in the axial plane. For this reason, intravenous gadolinium was injected, revealing a fracture line in 12 of 14 examinations, or fat suppression sequences were performed, revealing a fracture line in five of five cases. The total number of fractures detected was 17 [15 fractures of the sacrum (bilateral in 10 cases) and two of the acetabular roof]. At a later stage, the edema resolved and the fracture was clearly seen. The two cases of fracture of the acetabular roof were easily recognized at MRI, particularly in the sagittal plane.  相似文献

The classification of acetabular fractures is important for preoperative planning; interpretation of imaging studies, however, is often difficult. Plain radiography is the primary method for evaluation of these injuries, and computed tomography is now commonplace for the preoperative classification of patients with acetabular fractures. Eight patients with nondisplaced greater trochanteric (GT) fractures diagnosed by conventional radiographs were evaluated with magnetic resonance (MR) imaging. MR imaging demonstrated unsuspected intertrochanteric (IT) extension in each of the eight patients. This resulted in unanticipated surgical intervention in six of eight patients. Nondisplaced GT fractures are often more extensive and complex than they appear on conventional radiographs. MR imaging is useful in determining the extent and morphology of these fractures, information valuable for orthopedic management, and should be considered for definitive assessment in these patients.  相似文献

Our purpose was to quantify the intracranial cerebrospinal fluid (CSF) volume components using an original MRI-based segmentation technique and to investigate whether a CSF volume index is useful for diagnosis of normal pressure hydrocephalus (NPH). We studied 59 subjects: 16 patients with NPH, 14 young and 13 elderly normal volunteers, and 16 patients with cerebrovascular disease. Images were acquired on a 1.5-T system, using a 3D-fast asymmetrical spin- echo (FASE) method. A region-growing method (RGM) was used to extract the CSF spaces from the FASE images. Ventricular volume (VV) and intracranial CSF volume (ICV) were measured, and a VV/ICV ratio was calculated. Mean VV and VV/ICV ratio were higher in the NPH group than in the other groups, and the differences were statistically significant, whereas the mean ICV value in the NPH group was not significantly increased. Of the 16 patients in the NPH group, 13 had VV/ICV ratios above 30 %. In contrast, no subject in the other groups had a VV/ICV ratios higher than 30 %. We conclude that these CSF volume parameters, especially the VV/ICV ratio, are useful for the diagnosis of NPH. Received: 17 June 1999/Accepted: 12 October 1999  相似文献

We report a patient with a cerebral cryptococcal granuloma who presented with recent memory disturbance and deteriorating mental status followed by temporary loss of consciousness. To our knowledge, this is the first report of a cerebral cryptococcal granuloma examined by a combination of conventional MRI, fluid-attenuated inversion recovery and diffusion-weighted imaging and in which the surgical specimen was analysed histochemically. Spinal cord magnetic resonance imaging in suspected multiple sclerosis   总被引:1,自引:0,他引:1  
We examined the value of spinal cord magnetic resonance imaging (MRI) in the diagnostic work-up of multiple sclerosis (MS). Forty patients suspected of having MS were examined within 24 months after the start of symptoms. Disability was assessed, and symptoms were categorized as either brain or spinal cord. Work-up further included cerebrospinal fluid analysis and standard proton-density, T2-, and T1-weighted gadolinium-enhanced brain and spinal cord MRI. Patients were categorized as either clinically definite MS (n = 13), laboratory-supported definite MS (n = 14), or clinically probable MS (n = 4); four patients had clinically probable MS, and in nine MS was suspected. Spinal cord abnormalities were found in 35 of 40 patients (87.5 %), consisting of focal lesions in 31, only diffuse abnormalities in two, and both in two. Asymptomatic spinal cord lesions occurred in six patients. All patients with diffuse spinal cord abnormality had clear spinal cord symptoms and a primary progressive disease course. In clinically definite MS, the inclusion of spinal imaging increased the sensitivity of MRI to 100 %. Seven patients without a definite diagnosis had clinically isolated syndromes involving the spinal cord. Brain MRI was inconclusive, while all had focal spinal cord lesions which explained symptoms and ruled out other causes. Two other patients had atypical brain abnormalities suggesting ischemic/vascular disease. No spinal cord abnormalities were found, and during follow-up MS was ruled out. Spinal cord abnormalities are common in suspected MS, and may occur asymptomatic. Although diagnostic classification is seldom changed, spinal cord imaging increases diagnostic sensitivity of MRI in patients with suspected MS. In addition, patients with primary progressive MS may possibly be earlier diagnosed. Finally, differentiation with atypical lesions may be improved. 目的 探讨移位髋臼骨折的手术治疗及提高疗效的方法.方法 总结2000年6月-2008年9月对46例移位髋臼骨折患者的手术治疗经验.根据Letournel-Judet分型:双柱骨折15例,横行伴后壁骨折11例,"T"形骨折8例,后柱伴后壁骨折8例,前柱伴后壁骨折4例.分别采用Kocher-Langenbeck入路(K-L入路)、髂腹股沟入路、前后联合入路进行切开复位重建钢板内固定治疗.结果 46例患者获得随访6个月~8年(平均3.9年).按改良的Merle d'Aubigne和Postel评分标准进行评定:优23例,良14例,一般7例,差2例,优良率为80.4%.术后发生脂肪液化2例(4%),医源性坐骨神经损伤2例(4%),股骨头坏死3例(7%),异位骨化3例(7%),创伤性关节炎4例(9%),无手术死亡或骨不愈合发生.结论 术前正确判断骨折类型及移位方向、选择适当的手术时机和入路,以及术中良好的复位和内固定是提高移位髋臼骨折疗效的关键.  相似文献

Magnetic resonance (MR) imaging is not routinely used to exclude ligamentous and other soft tissue injury in the unconscious trauma patient. In order to determine the frequency and types of soft tissue injuries detected by MR imaging but not by computed tomography (CT), we performed a retrospective investigation comparing the MR findings in 59 trauma patients, either alert or unconscious, who previously had undergone a negative clearance multislice CT study of the cervical spine. Six patients had abnormalities not identified on the CT studies, including four cases of ligamentous injury.  相似文献

We describe the findings from various cross-sectional imaging modalities in patients with cardiac valve adherent masses. The techniques are discussed, and imaging findings are compared with the results of cardiac surgery. All three patients had neurological symptoms and/or cardiac murmurs. Transthoracic and/or transesophageal echocardiography revealed the cardiac mass in all three. For differentiation of thrombus and cardiac neoplasm magnetic resonance imaging (MRI) was also performed in all three patients and electron-beam computed tomography (EBCT) in two. Fast segmented cine gradient-echo MRI techniques provided mass depiction in all patients, while T1-weighted spin-echo imaging failed in mass detection in one patient. None of the patients showed evidence of valve regurgitation or stenosis in flow sensitive cine MRI. EBCT excluded mass calcifications in both patients and reliably demonstrated the valve attached lesions. Although echocardiography is the modality of choice in evaluating cardiac masses and especially valve attached masses, MRI and EBCT provide additional information about tissue characteristics and allows an excellent overview of the cardiac and paracardiac morphology. Fast segmented cine gradient-echo MRI is especially able to depict even small tumors attached to rapidly moving cardiac valves, and valve competence can be easily assessed within the same examination. AIMS: To report the magnetic resonance imaging (MRI) findings of osteomalacic insufficiency fractures of the pelvis. MATERIALS AND METHODS: In all, 12 persons presenting with chronic pelvic pain and with a definitive diagnosis of osteomalacia (OM) were enrolled in this study. Radiological work-up included direct radiographs (n = 12), computed tomography (n = 5), scintigraphy (n = 12) and MRI (n = 12). The location of the insufficiency fractures and corresponding MRI appearances were evaluated. Depending on the presence or absence of signal intensity around the fractures, the lesions were grouped into active and chronic forms. RESULTS: A total of 34 insufficiency fractures were depicted on imaging studies. MRI demonstrated 33 of the lesions. All the insufficiency fractures were seen as a hypointense lines or fissures on T1- and T2-weighted and STIR MR images. There was a high frequency of insufficiency fractures at the sacrum and iliac bone (16/34, 47%). Overall, 11 chronic-type (no abnormal signal intensity around the fissure) and 22 active-type (abnormal signal intensity around the fissure) insufficiency fractures were detected by MRI. Follow-up MR examinations of 2 subjects showed that abnormal signal intensity had disappeared after appropriate treatment. CONCLUSION: The results of this study showed that the iliac and sacral bones are frequently involved in patients with OM. MRI can determine the clinical activity of the disease, and can monitor the response to treatment of the active type of insufficiency fractures.  相似文献

We examined the value of dynamic magnetic resonance imaging (MRI) in chronic renal disease with renal insufficiency. In 33 consecutive patients (21 vascular nephropathy, 12 glomerular nephropathy) MRI was performed using a 1.5-T unit and a body coil, with SE T1-weighted (TR/TE = 600/19 ms) and dynamic TFFE T1-weighted sequences (TR/TE = 12/5 ms, flip angle = 25 °) after manual bolus injection (via a cubital vein) of 0.1 mmol/kg Gd-DTPA-BMA. Morphological evaluation was performed in unblinded fashion by three radiologists, evaluating renal size, cortical thickness, and corticomedullary differentiation. Functional analysis was performed by one reviewer. Time-signal intensity curves, peak intensity value (P), time to peak intensity (T), and the P/T ratio were obtained at the cortex, medulla, and pyelocaliceal system of each kidney. The relationship of these parameters to serum creatinine and with creatinine clearance was investigated. A good correlation between morphological features of the kidneys and serum creatinine values was found. Morphological findings could not distinguish between vascular and glomerular nephropathies. A statistically significant correlation (P <0.01) between cortical P, cortical P/T, medullary P, and serum creatinine and creatinine clearance was found. A significant correlation (P <0.01) was also found between cortical T, medullary P/T, T of the excretory system, and creatinine clearance. The cortical T value was significantly higher (P <0.01) in vascular nephropathy than in glomerular nephropathy. Thus in patients with chronic renal failure dynamic MRI shows both morphological and functional changes. Morphological changes are correlated with the degree of renal insufficiency and not with the type of nephropathy; the functional changes seem to differ in vascular from glomerular nephropathies. A subchondral insufficiency fracture of the femoral head (SIF) is generally observed in elderly women. In addition, SIF has been reported to occur in young military trainees, which are considered to be due to a fatigue stress fracture. This report reviewed five cases of SIF in young adults without any history of overexertion or antecedent trauma. The clinical findings, time to the initial radiograph, time to the initial magnetic resonance imaging (MRI), and follow-up period were investigated. Radiographs and MRI were obtained in both hips. Bone mineral density and bone scintigram were performed. Measurements of a bone mineral density indicated presence of osteopenia in three patients. MRI disclosed a discontinuous irregular-shaped low-intensity band surrounded by bone marrow edema in all cases. After conservative treatments, one patient's condition resolved, while four patients underwent femoral head collapse resulting in surgical treatment. In these four patients, the diagnosis of SIF was histologically confirmed. In addition, the histological findings showed thin disconnected bone trabeculae, thus indicating the presence of some degree of osteopenia. These patients were considered to demonstrate an insufficiency fracture rather than a fatigue stress fracture based on both clinical findings and histological findings. SIF needs to be considered in patients presenting with hip pain even in young cases.  相似文献

目的探讨移位髋臼骨折的手术治疗方法并初步探讨髋臼骨折Ⅰ期行全髋关节置换术的疗效。方法对69例髋臼骨折行手术治疗并随访63例,随访时间6~36个月,平均21.5个月。结果根据Matta对骨折复位情况的判定标准及美国矫形外科研究院髋关节功能评价标准:完全复位48例,满意复位17例,不满意4例;优40例,良18例,一般3例,差2例,优良率92%。并发切口感染1例,异位骨化6例,创伤性关节炎4例,股骨头缺血性坏死2例。髋臼骨折Ⅰ期全髋关节置换术随访6例均无感染、假关节松动、创伤性关节炎等并发症。结论手术是治疗移位髋臼骨折较为有效可靠的方法。全面的影像学检查是了解髋臼骨折部位及类型的主要手段;准确了解髋臼骨折的分类是提高疗效的关键。应根据髋臼骨折的分类正确选择合适的手术入路及复位固定。髋臼骨折Ⅰ期行全髋关节置换术近期疗效满意,但要谨慎掌握手术适应证,远期疗效有待进一步观察。  相似文献

Role of radiology in the diagnosis of neurosarcoidosis   总被引:2,自引:0,他引:2  
Clinical studies report a rate of 5 % and autopsy results a rate of 25 % of brain involvement in sarcoidosis. The aim of this study was to evaluate the role of radiology in the diagnosis of patients with neurosarcoidosis. The chest radiographs and MRI brain scans of 22 patients with sarcoidosis were retrospectively reviewed, along with the information that was provided in the request form and clinical charts. All patients had neurological signs and symptoms; 21 patients were examined with contrast enhancement. Facial nerve paralysis was the most common clinical manifestation identified in 10 patients. A wide spectrum of MR findings was noted: periventricular high-signal lesions on T2-weighted images (46 %); multiple supratentorial and infratentorial brain lesions (36 %); solitary intra-axial mass (9 %); solitary extra-axial mass (5 %); and leptomeningeal enhancement (36 %). Neurological signs and symptoms can be significant manifestations of sarcoidosis. Magnetic resonance imaging shows a wide spectrum of brain abnormalities associated with neurosarcoidosis. The patient's history and chest X-ray are helpful in arriving at the correct diagnosis, but in selected cases with isolated brain involvement biopsy may still be required. The “dedicated” MRI units have characteristics of high diagnostic accuracy and lower installation and management costs as compared with whole-body systems. The dedicated MRI units are easy to install. The low weight allows their installation also under unfavorable circumstances. In a dedicated system cost-effectiveness and ease of installation must be accompanied by the capability of providing high-quality images. In our experience, the high number of examinations performed, the most part of which provided with the surgical controls, allowed an accurate evaluation of the diagnostic potentialities of the dedicated magnet. We were not able to perform the examinations in only 3 % of cases due to the physical shape of the patient and the clinical condition of the patient which may hinder the correct positioning of the limb. The overlapping of the diagnostic accuracy of the E-scan and Artoscan units in the study of the lower limbs, compared with whole-body units and surgery, prompted us to exploit the potentialities of the E-Scan in the study of the shoulder. We had a good correlation between E-Scan, whole-body units, and surgical findings, which confirmed the high diagnostic accuracy of the dedicated system. In conclusion, in our experience carried out in the musculoskeletal system, the dedicated magnets showed promising results. Their diagnostic reliability and utility was comparable to that obtained from conventional units operating at higher magnetic fields.  相似文献

We examined possible age- and gender-specific differences in the function and mass of left (LV) and right (RV) ventricles in 36 healthy volunteers using cine gradient-recalled echo magnetic resonance imaging. Subjects were divided into four groups (nine men and nine women in each): men aged under 45 years (32 ± 7), women aged under 45 (27 ± 6), men aged over 45 (59 ± 8), and women aged over 45 (57 ± 9). Functional analysis of cardiac volume and mass and of LV wall motion was performed by manual segmentation of the endocardial and epicardial borders of the end-diastolic and end-systolic frame; both absolute and normalized (per square meter body surface area) values were evaluated. With age there was a significant decrease in both absolute and normalized LV and RV chamber volumes (EDV, ESV), while LV and RV masses remained unchanged. Gender-specific differences were found in cardiac mass and volume (for men and women, respectively: LV mass, 155 ± 18 and 110 ± 16 g; LV EDV, 118 ± 27 and 96 ± 21 ml; LV ESV, 40 ± 13 and 29 ± 9 ml; RV mass, 52 ± 10 and 39 ± 5 g; RV EDV, 131 ± 28 and 100 ± 23 ml; RV ESV, 53 ± 17 and 33 ± 15 ml). Normalization to body surface area eliminated differences in LV volumes but not those in LV mass, RV mass, or RV function. Functional parameters such as cardiac output and LV ejection fraction showed nonsignificant or only slight differences and were thus largely independent of age and gender. Intra- and interobserver variability ranged between 1.4 % and 5.9 % for all parameters. Cine magnetic resonance imaging thus shows age- and gender-specific differences in cardiac function, and therefore the evaluation of cardiac function in patients should consider age- and gender-matched normative values. The value of CT in management of severe acute pancreatitis is well established. Some, but not all, experimental studies suggest a detrimental effect of intravenous iodinated contrast agents in acute pancreatitis, but although initial clinical data tends to support this, the positive advantages of enhanced CT outweigh the possible risks. Magnetic resonance imaging has been shown to be as effective as CT in demonstrating the presence and extent of pancreatic necrosis and fluid collections, and probably superior in indicating the suitability of such collections for percutaneous drainage. Image-guided intervention remains a key approach in the management of severely ill patients, and the indications, techniques and results of radiological intervention are reviewed herein. Both CT and MRI can be used to diagnose advanced chronic pancreatitis, with the recent addition of MRCP as a viable alternative to diagnostic endoscopic retrograde cholangiopancreatography (ERCP). Both MRCP and CT/MR imaging of the pancreatic parenchyma still have limitations in the recognition of the earliest changes of chronic pancreatitis – for which ERCP and tests of pancreatic function remain more sensitive – but the clinical significance of these minor changes remains contentious. Objective. The objective of this study was to evaluate the effects of continuous leg traction on contrast-enhanced MR imaging of the hip joint and to determine whether MR imaging under these conditions is useful for demonstrating acetabular labral tears. Design and patients. Nineteen hips underwent MR imaging with a T1-weighted spin-echo sequence, followed by MR imaging under continuous leg traction after intravenous injection of gadolinium-DTPA. Joint fluid enhancement and labral contour detection were evaluated. Eleven hips had labral tears shown by conventional arthrography, arthroscopy and macroscopic surgical findings. Assessment of labral tears by MR imaging was correlated with the diagnosis based on these standard techniques. Results and conclusions. Joint fluid enhancement was obtained in all hips at 30 min after injection. Superior and inferior labral surfaces were completely delineated in 1 hip on the unenhanced MR images, and in 7 and 13 hips, respectively, on the enhanced images under traction. The enhanced images under traction depicted 9 of the 11 labral tears. Comparison between the unenhanced image and the enhanced image under traction avoided mistaking undercutting of the labrum for a tear in 4 hips. Contrast-enhanced MR imaging under traction was valuable for detecting labral tears non-invasively and without radiation. Follow-up examinations using this method in patients with acetabular dysplasia can help to clarify the natural course of labral disorders and enable better treatment planning.  相似文献

The aim of this study was to evaluate the efficacy of contrast-medium (CM)-enhanced MR imaging of operated pituitary macroadenomas with reduced dose of gadopentetate dimeglumine. In a prospective study 18 patients were examined with coronal T1-weighted MR imaging prior to and following intravenous CM injections. Two sets of contrast-enhanced coronal images were obtained in each patient; the first set after 50 % of the recommended dose of 0.1 mmol/kg body weight (b. w.) had been administered, and the second set immediately after additional CM had been given to make up a total dose of 0.1 mmol/kg b. w. The images were evaluated by three neuroradiologists. The SIPAP classification system was used to evaluate tumour extension, whereas tumour margin conspicuity was scored using an arbitrary scale of 1–5 (1 = indistinct, 5 = well defined). Signal intensity measurements obtained from the most enhancing part of the adenomas demonstrated increased enhancement with increased CM dose. Tumour delineation scores were significantly better on the reduced- and full-dose images than on pre-CM injection images, but, with one exception, tumour extension was identified as the same on all imaging sequences. Postoperative MR imaging of large macroadenoma residues can routinely be performed without intravenous CM.
