OBJECTIVE To investigate the clinical and pathological characteristics,diagnosis and treatment of stromal sarcoma of the breast(SSB).Methods:The clinical and pathological data of 6 patients with SSB treated between 1954 and 2007 were retrospectively analyzed.METHODS The clinical and pathological data of 6 patients with SSB treated between 1954 and 2007 were retrospectively analyzed.RESULtS Atl patients were female and one was menopausal.The median age of the patients was 39 years old(range,20-55).All cases had a history of a palpable mass.The tumor rapidly augmented in a short time period in 3 patients.One patient had discontinuous pain and 3 patients had masses located in the upper outer quadrant of the breast.The median tumor radius was 6.0 cm(range,3-15 cm).According to the AJCC breast cancer staging standard(6th edition),1 case was of stage ⅡA,2 cases were of stage ⅡB,2 cases were of stage ⅢB and one case couldn't be staged.Four patients were initially treated by excising the tumor and then undergoing mastectomy or modified radical mastectomy after recurrence.Radical mastectomy was suitable for those with pectoralis major muscle involvement.Two patients received simple mastectomy, 2 patients underwent radical mastectomy and another 2 patients received modified radical mastectomy,After surgery,all patients were identified as SSB through pathology,with focal ossification in one case and mucinous degeneration in another one case.Four patients who underwent axillary Iymph node dissection did not have lymph node metastases.Three patients received chemotherapy after surgery. After a median follow-up time of 36.5 months(8-204 months),4 patients had recurrence after local excision and 3 patients had recurrence more than 2 times with a median time to recurrence of 2.5 months(1to 4 months) after surgery.One patient had lung metastases at 7months after the initial surgery and the other 5 patients were alive without disease at the end of the follow-up period.CONCLUSION SSB is difficult to diagnose preoperatively and is characterized by its tendency to recur locally.To obtain negative margins,wide local excision or mastectomy must be performed.Axillary lymph node dissection is not mandatory.The roles of adjuvant chemotherapy and radiotherapy have still been controversial.  相似文献

背景与目的:隐匿性乳腺癌发病隐匿,诊断困难,治疗策略不确定,是乳腺癌领域的难点和热点。本研究探讨了隐匿性乳腺癌的临床病理特征及诊疗策略。方法:对56例隐匿性乳腺癌患者在术前采用乳腺彩超、钼靶、磁共振及PET/CT检查,对比不同检查方法对隐匿性乳腺癌可疑原发灶检出率及病理符合率的差异。治疗方案采用新辅助化疗、乳腺癌改良根治术、保乳+腋窝淋巴结清扫术、腋窝淋巴结清扫术,术后行化疗+放疗。结果:乳腺超声、X线、MRI、PET/CT对乳腺可疑原发灶的检出率分别为7.14%(4/56)、29.41%(15/51)、37.50%(18/48)和16.28%(7/43);结合术后病理学检查发现其病理符合率分别为66.67%(2/3)、50.00%(7/14)、50.00%(9/18)和50.00%(3/6);26例患者乳腺超声、X线及MRI均未发现可疑原发灶,其中21例接受病理学检查,阳性率为14.29%(3/21);对39例乳腺癌改良根治术标本行乳腺病理切片检查,检出原发灶15例,检出率38.46%。根据St. Gallen指南分子分型标准,Luminal A型、Luminal B型、HER-2阳性型和三阴性型比例分别为7.14%、46.43%、12.50%和33.93%。术后随访52例,随访时间10~104个月,中位时间35个月,复发转移4例,死亡0例。检出原发灶的15例患者中,复发或转移2例;未检出原发灶的24例患者中,无复发或转移;行新辅助化疗17例,达病理学完全缓解(pathological complete response,PCR)2例,复发或转移2例;行乳腺癌改良根治术39例,复发或转移2例;行保乳+腋窝清扫8例,复发或转移2例;腋窝清扫9例,无复发或转移。结论:乳腺MRI检查在隐匿性乳腺癌的排除性诊断中有重要价值;乳腺超声、X线及MRI均未发现可疑原发灶的患者其乳腺病理原发灶检出率较低;隐匿性乳腺癌的治疗策略可选择新辅助化疗、乳腺癌改良根治术、保乳+腋窝清扫术、腋窝清扫术;乳腺病理学未检出原发灶的患者复发转移率低于检出原发灶者。  相似文献

Background  Neoadjuvant chemotherapy (NAC) is the standard therapy for locally advanced breast cancer. Recently, several studies have revealed that clearance of axillary lymph node involvement is an independent factor for survival irrespective of the response of the primary lesion. However, in daily practice, it is difficult to fully examine every lymph node that has been surgically sampled, in view of pathology laboratory workload and cost. Therefore, in the present study, we adopted the more clinically relevant categorization of evaluating postoperative axillary lymph node status and retrospectively studied its significance in predicting patients’ survival. Methods  The study cohort consisted of 35 locally advanced breast cancer patients who are treated with concomitant epirubicin–docetaxel. The clinicopathological factors used for analysis were as follows: ERα, PgR, p53, HER2, pathological response (in the primary tumor), and axillary lymph node status at surgery. With regard to axillary lymph node status, we categorized patients into two groups: those with pathological lymph node involvement at surgery (pLNI) and those without. Using multivariate analysis, we evaluated the significance of these factors in predicting disease-free survival after surgery. Results  The median follow-up period was 23.4 months (range 4.3–45.4). Multivariate analysis showed significantly reduced disease-free survival associated with pLNI (P = 0.005). Conclusions  pLNI is an excellent prognostic factor for locally advanced breast cancer patients treated with concomitant epirubicin–docetaxel. Use of our criteria may enable large numbers of oncologists to participate in new studies of high-risk cohorts who are refractory to NAC.  相似文献

Background Paget’s disease of the breast is an uncommon presentation of breast malignancy, accounting for 1–3% of all the breast tumors and presents in different histopathologic patterns: in association with an underlying invasive or non invasive carcinoma, or without any underlying neoplasia. In the literature, different methods are used for the treatment. Mastectomy with or without axillary dissection has been considered as the standard treatment procedure for many years. Several studies have already shown that breast conservation with radiation therapy is an oncologically safe option. Regarding the axillary approach, several studies have documented the presence of positive sentinel lymph node even in Paget’s disease alone. The objective of this study was to retrospectively analyze outcome of patients affected by Paget’s breast disease and to define our institutional experience. Patients and methods Between May 1996 and February 2003, 114 patients with confirmed Paget’s disease of the breast were retrieved and underwent surgery at the European Institute of Oncology of Milan, Italy. The median age of the patients was 54 years at the time of the diagnosis. In our study, the histopathological examination of the operated specimen revealed one hundred seven patients with Paget’s disease associated with an underlying invasive or non invasive carcinoma, and seven patients without underlying carcinoma. Patients underwent either conservative breast surgery or mastectomy, with or without sentinel lymph node biopsy and/or axillary surgery. Each patient was evaluated after surgery at a multidisciplinary meeting to selecting systemic therapy. Results Seven patients had “pure” Paget’s disease of the breast and one hundred seven had the disease associated with an underlying carcinoma. As surgical techniques 71 mastectomies and 43 breast conserving surgeries have been performed. Complete axillary dissection was done in patients with clinically positive lymph node and/or sentinel lymph node biopsy positive. Sentinel lymph node biopsy was performed in nineteen patients with invasive component and five were positive and underwent axillary dissection. Eleven sentinel lymph node biopsies were done in patients with non invasive component and none of them was positive. Adjuvant systemic therapies were based on the final tumor, node and metastasis stage: thirty patients received adjuvant chemotherapy alone, fourteen received endocrine treatment alone, twenty-six patients were evaluated to receive both chemo and endocrine therapy. The median duration of follow up was 73 months and was updated in the last 6 months. Five patients developed local recurrence, one had regional recurrence, another two had loco-regional recurrences and fourteen had distant metastasis as a first event. Malignancy-related deaths were censored in the statistical analyses cancer for and due to another tumor in eleven patients. Additionally, deaths were not related to malignancy totally in thirteen patients. Conclusions Screening examination and imaging techniques are fundamental. Breast conserving surgery combined with breast irradiation for patients with invasive and non invasive breast carcinoma has become the treatment of first choice. All surgical conservative approaches should include the complete nipple–areolar complex and margins of resected specimen free of tumor. Thanks to the evolution of the conservative approach, good cosmetic result can be obtained. To be informed about the axillary lymph node status and to avoid the patient to have a second surgical approach, sentinel lymph node biopsy should be performed.  相似文献

背景与目的:乳腺化生性癌十分少见。对此,本文旨在探讨乳腺纯上皮化生性癌的病理和临床特征以及治疗。方法:回顾性分析1997-2007年9例在我院治疗的女性乳腺纯上皮化生性癌病例。结果:患者中位年龄50岁(37~78岁),9例均行根治术或改良根治术,病理诊断为梭形细胞化生性癌6例,鳞癌3例。雌激素受体(ER)和/或孕激素受体(PR)阳性者2例。2例淋巴结转移。中位随访时间为25(3~112)个月,术后局部复发1例,远处转移3例,肺转移2例,2例患者死亡,1例带瘤生存。结论:本病以梭形细胞化生性癌多见,可同时表达上皮和间叶组织抗原,但较少表达ER和PR。患者多以无痛性肿块就诊,肿块局切复发率高。腋窝淋巴结转移少见,肺转移多见。手术治疗宜选择单纯乳房切除加前哨淋巴结活检。  相似文献

目的探讨乳腺癌锁骨上淋巴结转移癌的综合治疗方法.方法所有病例均于术前行新辅助化疗,其中12例乳腺癌锁骨上转移癌患者再行乳癌根治术和颈淋巴结清扫术(手术组),并与15例同期术前行放疗,再行乳癌根治术、化疗和放疗("三明治"疗法)患者(放疗组)对比,观察1~3年存活率和复发时间.结果手术组1年生存12例,1~3年存活7例,复发时间18~47个月;放疗组1年生存12例,1~3年存活4例,复发时间11~43个月.结论乳腺癌锁骨上淋巴结转移癌行乳癌根治术和颈淋巴结清扫术及综合治疗,对提高患者生存率和延长复发时间有积极作用.  相似文献

BackgroundThe objective of this study was to explore the effect of conservative surgery plus postoperative axillary radiotherapy without axillary lymph node dissection vs. modified radical mastectomy in patients with stage I breast cancer.Patients and MethodsIn this study, 186 patients with stage I breast cancer were enrolled. Among them, 98 patients underwent breast-conserving surgery without axillary node dissection. From the first day after surgery, each of them received 6 cycles of CMF (cyclophosphamide, methotrexate, 5-fluorouracil) chemotherapy, and thereafter radical radiotherapy for 5 to 6 weeks. Eighty-eight patients received modified radical mastectomy with postoperative chemotherapy and radiotherapy. The clinical data of these 186 patients were analyzed.ResultsThere was no significant difference (P > .05) in local recurrence and survival rates between the conservative plus axillary radiotherapy group and the modified radical mastectomy group, although a significantly greater incidence of upper limb dysfunction and edema were observed in the modified mastectomy group (P < .05).ConclusionThe efficacy of conservative surgery plus axillary radiotherapy alone is superior to that of axillary node dissection for stage I breast cancer patients.  相似文献

Background Reconstruction of the breast after mastectomy is an integral part of the complete management of breast cancer. However, a delay in the reconstruction is usually proposed after the mastectomy in case of invasive cancer, while there is a general agreement for immediate breast reconstruction in case of in situ tumors. Patients and methods Among a total of 677 patients having undergone a mastectomy between 1997 and 2001, 518 (76.5%) underwent an immediate breast reconstruction (IBR). All the patients had a Patey mastectomy for invasive cancer (T1–T3). An adjuvant medical treatment was given according to the biological characteristics of the tumor and lymph node status. No patient received any kind of radiotherapy. The median follow up was 70 months (range 13–114). Results The local recurrence rate was 5.2% for the group of IBR and 9.4% for the mastectomy group without IBR (NoIBR). The regional metastases rate was 1.4 vs. 1.3%. The rate of distant metastases was 13.9 vs. 16.4%. Contra-lateral breast tumor was observed in 1.5 vs. 1.3%. Death rate was 10.4 vs. 16.4%. No statistical difference was observed between the two groups in terme of overall survival (OS) and disease free survival (DFS) (HRIBR vs. NoIBR: 1.03 and 0.99 for OS and DFS, respectively). Conclusion At our institution we have compared a large series that have undergone IBR (518) with a control group (159) and followed over a prolonged period of time (70 months). This study provides the best available results to suggest that IBR is a safe and reliable treatment option for the managing invasive breast cancer.  相似文献

Background Sentinel lymph node biopsy (SLNB) has almost totally replaced axillary lymph node dissection as the first-line axillary procedure for node-negative breast cancer. SLNB has a false-negative rate of 0–22%, and regional nodal recurrence is a major concern after SLNB. In this study, we assessed axillary recurrence and risk factors in breast cancer patients 40 months after negative SLNB. Methods Of 940 patients with node-negative breast cancer who underwent SLNB between December 2003 and January 2006 at Asan Medical Center, 720 were negative on SLNB, as determined using 99-m TC radiocolloid and subareolar injection technique. Of the 720 patients negative on SLNB, 174 underwent further axillary dissection, 253 underwent node sampling, and 293 received SLNB only. Results A mean of 2.1 SLNs was removed per patient. At a median follow-up of 40 months (range 24–49 months), recurrence in the axilla was observed in three patients, all of whom had undergone SLNB only; two of these patients also had recurrences in internal mammary lymph nodes. Tumors in all three patients were hormone-receptor negative, and two were c-erbb2 negative. Conclusion The axillary recurrence rate was low in patients negative on SLNB. Negative hormone-receptor status and high nuclear grade may be risk factors for regional nodal failure after SLNB.  相似文献

OBJECTIVE In Europe and America breast cancer commonly occurs in women of middle and old age, with a median age of about 57 years. Modified radical mastectomy now called standard radical mastectomy, has taken the place of traditional radical mastectomy. Patients with breast cancer at an early stage commonly receive BCT (breast conservative therapy). The TNM stage (especially the lymph node status) affects the prognosis, and adjuvant therapy can improve survival. In China, only a few reports have been pubIished studying large numbers of breast cancer patients. This study was designed to analyze the clinical features, surgical pattern and treatment outcome of resectable breast cancer, as well as to explore the prognostic factors and the effect of adjuvant therapy, with a goal to improve the level of diagnosis and treatment.METHODS Records of the 6,263 patients with resectable breast cancer who had been admitted into our hospital from June 1964 to June 2003 were analyzed retrospectively.RESULTS Of the 6,263 cases, 98.8% were female. Breast cancer occurred most frequently in patients of ages 40~49 years (41.0%), especially in patients 45~49 years old (25.2%). A breast lump, which occurred in 96.2% of the patients, was the main clinical manifestation. The overall 5- and 10-year survival rates were 75.16% and 40.44%. Of the patients in TNM stages 0-1,Ⅱ, and Ⅲ, the 5-year survival rates were 96.8%, 73.7% and 46.4% respectively and the 10-year survival rates were 78.7%, 64.6% and 33.5% respectively. The 5-, and 10-year survival rates were higher in the lymph node negative group than in the lymph node positive group (80.3% vs. 55.6%, and 59.2% vs. 31.9%, P<0.01). Since the 1980s there was no significant difference in survival rates of patients who received a radical mastectomy compared to a modified radical mastectomy(P>0.05). Of the 73 patients who underwent breast conservative therapy, no local recurrence or metastasis occurred during a maximal follow-up of 17 years. Of the patients in stage T2T4, the 5-, and 10-year survival rates were significantly higher in the group treated with adjuvant chemotherapy compared to the non-chemotherapy group (78.2% vs. 60.1%, and 48.9% vs. 30.7%, P<0.01).CONCLUSION According to our data, breast cancer most frequently occurred in patients of ages 45~49 years. The TNM stage (especially the lymph node status) relates to breast cancer prognosis. The prognosis was worse in patients with positive lymph nodes compared to the patients with negative lymph nodes. The efficacy of a modified radical mastectomy is equal to that of a radical mastectomy, and breast conservative therapy can be applied to patients in an early stage. Adjuvant chemotherapy and endocrine therapy can improve the survival of resectable breast cancer patients.  相似文献

BACKGROUND: The purpose of the current study was to evaluate the locoregional recurrence rate after treatment of patients with operable breast carcinoma with a modification of the Halsted radical mastectomy and the selective use of radiotherapy and to identify risk factors for locoregional recurrence. METHODS: Between 1979-1987, 691 consecutive patients underwent mastectomy after a negative biopsy of the axillary apical lymph nodes. The median age of the patients was 59 years (range, 26-89 years). The clinical tumor size was < 2 cm in 72 patients, 2-5 cm in 387 patients, and >5 cm in 169 patients; 16 patients had a T4 tumor. Surgery was comprised of a modification of the Halsted radical mastectomy, including at least part of the pectoralis major muscle and the entire pectoralis minor muscle, in 573 patients; 303 patients had positive axillary lymph nodes. Adjuvant radiotherapy to the chest wall and regional lymph nodes was given to 74 patients, whereas an additional 414 patients underwent irradiation to the internal mammary and medial supraclavicular lymph nodes. The median follow-up was 91 months. RESULTS: The actuarial overall survival rate was 82% at 5 years and 63% at 10 years. The 10-year chest wall and regional lymph node control rates, including patients with prior distant failures, were 95% and 94%, respectively. The only two significant prognostic factors for locoregional recurrence on multivariate analysis were lymph node status and pathologic tumor size. CONCLUSIONS: Excellent locoregional control can be achieved with a modified technique of radical mastectomy in patients with negative apical biopsy and the selective use of comprehensive radiotherapy. These results may serve as a reference outcome for comparison with other locoregional treatment strategies.  相似文献

目的 探讨改良根治术后腋窝淋巴结阳性乳腺癌放疗效果与雌激素受体(ER)、孕激素受体(PR)及人类表皮生长因子受体2(HER2)的相关性.方法 选取福建医科大学附属漳州市医院2008年1月至2013年1月乳腺外科收治的进行改良根治术的腋窝淋巴结阳性的乳腺癌患者350例,其中216例接受了放疗,134例未接受放疗,根据ER、PR及HER2的免疫组织化学结果,将所有患者分为三阴性乳腺癌(ER-/PR-/HER2-)、HER2阳性(ER-/PR-/HER2+)、Luminal B(ER +/PR+/HER2+)及Luminal A(ER +/PR +/HER2-)4组,比较放疗对4组患者局部复发率、远处转移率和无瘤生存率的影响.结果 中位随访48个月,放疗降低了三阴性乳腺癌、HER2阳性、Luminal B及Luminal A 4组患者的局部复发率(x2=6.23,P=0.01;x2=8.02,P=0.00;x2=15.43,P=0.00;x2 =4.47,P=0.03),且前3组放疗患者的远处转移率(x2=4.18,P=0.04;x2 =6.35,P=0.01;x2=43.31,P=0.00)和无瘤生存率(x2=9.78,P=0.00;x2=10.83,P=0.00;x2=16.95,P=0.00)相对于未放疗患者差异具有统计学意义.但放疗对Luminal A组患者的远处转移率和无瘤生存率无影响(P>0.05).结论 不同的ER、PR和HER2状态对于改良根治术后腋窝淋巴结阳性乳腺癌患者的放疗结果具有一定影响.  相似文献

Background  Early gastric cancer (EGC) has an excellent prognosis, but some patients with lymph node-positive disease will develop recurrence. In this study we investigated the risk factors for recurrence in this selected group of patients. Methods  The clinical and pathological records of 2368 patients who underwent gastrectomy for solitary EGC between 1980 and 1999 at the National Cancer Center Hospital, Tokyo, were examined. Two hundred and thirty-eight patients (10%) were lymph node-positive (positive for lymph node metastasis) and form the population of this study. Results  Nineteen (8%) of the 238 patients with lymph nodepositive disease developed recurrence. The most common site of recurrence was lymph node (37%), followed by liver (21%). The interval between surgery and the detection of recurrence ranged from 3 to 98 months, with a median of 26 months. Multivariate analysis demonstrated that the number of metastatic nodes was an independent risk factor for recurrence. Patients with seven or more metastatic nodes had the highest rate of recurrence, at 38%. Conclusion  The number of nodes positive for metastasis was the only independent risk factor for recurrence after curative surgery in patients with lymph node-positive early gastric cancer. These high-risk patients may obtain additional survival benefit if targeted with adjuvant chemotherapy.  相似文献

目的探讨前哨淋巴结活检术(sentinellymph node biopsy,SLNB)在早期乳腺癌保乳术中的应用效果。方法回顾性分析56例pT1.2N0M0期乳腺癌行保乳术+前哨淋巴结活检术的临床资料。56例SLN阴性,未行腋窝淋巴结清扫术(axillary lymph node dissection,ALND)。术后辅以化疗、放疗,激素受体阳性患者行内分泌治疗。结果56例成功施行保乳手术,保乳术后双乳对称。SLNB替代ALND者各项术后并发症少。中位随访时间36个月(1~72个月),1例发现局部复发,行乳腺癌改良根治术时发现腋窝淋巴结转移;1例发现腋窝淋巴结复发转移。结论SLNB可以缩小手术范围,减少术后并发症,保留腋窝形态,提高保乳质量。  相似文献

目的 分析隐匿性乳腺癌的诊断、治疗及预后情况,总结其诊断和治疗经验.方法 回顾性分析44例隐匿性乳腺癌患者的临床资料,44例隐匿性乳腺癌患者中,16例行乳腺癌根治术,19例行乳腺癌改良根治术,1例行保留乳腺的腋下淋巴结清扫,8例仅行腋下肿物切除.结果 在接受乳腺癌根治术的35例患者中,有4例(11.4%)在术后病理切片中发现原发灶,原发灶的最大直径为0.6~2.5 cm,其中3例为浸润性导管癌,1例为黏液腺癌;另有3例可在标本中见不典型增生.全组有38例患者获得随访,随访时间为12~132个月.32例行手术治疗且获得随访的患者中,2例死亡,3例术后局部复发并带瘤生存;未接受进一步手术治疗患者中,有2例分别于确诊后16和41个月后因浸润性导管癌行乳腺根治术;其余患者均无病生存.结论 对以腋下淋巴结肿大为惟一临床表现、经肿物活检确认为转移性腺癌的女性患者,应高度怀疑隐匿性乳腺癌的可能性.隐匿性乳腺癌的治疗方式可采用腋窝淋巴结清扫后全乳放疗或乳腺癌根治术.
Abstract:
Objective To summarize the experience of diagnosis and treatment of occult breast cancer in 44 cases. Methods Clinicopathological data of 44 cases of occult breast cancer initially presenting axillary mass alone treated in our department during Jan 1997 to Dec 2008 were retrospectively analyzed. Results The 44 patients with occult breast cancer accounted for 0.42% of all breast cancer patients admitted to our hospital and institute in the same period. The surgery included radical mastectomy in 16 cases, modified radical mastectomy in 19 cases, axillary clearance in 1 case, and simple axillary node excision in 8 cases. Follow-up, ranging from 12-132 months, was available in 38 cases. Among 32 cases who underwent mastectomy or axillary clearance, 2 cases died of distant metastases and 3 cases were still alive with local recurrence at the time of analysis. In two out of six cases who refused further surgical treatment received mastectomy 16 months and 41 months after the primary diagnosis of occult breast cancer, respectively. Others were alive without evidence of recurrence or metastases at the time of analysis. Conclusions Occult breast cancer should be taken into consideration in cases presenting with axillary metastasis of unknown primary origin. The treatment of occult breast cancer should include modified radical mastectomy/radical mastectomy or breast conserving surgery combined with breast irradiation.  相似文献

回顾分析临沂市肿瘤医院乳腺外科收治的36例隐匿性乳腺癌(OBC)患者的临床资料,并结合文献进行讨论。所有病例腋下肿物均经切检病理确诊。乳腺钼靶检查2例诊断为乳腺癌,3例患者为可疑乳腺癌;乳房彩超检查1例诊为乳腺癌,3例患者为可疑乳腺癌。行改良根治术28例,乳腺癌根治术4例,保留乳房手术2例,腋窝淋巴结清除加全乳放疗2例。30例患者获得随访,15例生存时间>5年。回顾分析结果提示,对腋下肿块应行切除活检以明确诊断;乳腺钼靶、彩超及腋下肿物激素受体检测有一定价值;乳腺核磁共振扫描能够提高OBC的检出率。手术方式宜采用改良根治术或保留乳房后全乳照射,并辅以化疗及内分泌治疗等以提高长期生存率。  相似文献

PurposeTo evaluate the prognostic value of Ki67 expression, breast cancer molecular subtypes and the impact of postmastectomy radiotherapy in breast cancer patients with pathologic negative lymph nodes (pN0) after modified radical mastectomy.Patients and methodsSix hundred and ninety-nine breast cancer patients with pN0 status after modified radical mastectomy, treated between 2001 and 2008, were identified from a prospective database in a single institution. Tumours were classified by intrinsic molecular subtype as luminal A or B, HER2+, and triple-negative using estrogen, progesterone, and HER2 receptors. Multivariate Cox analysis was used to determine the risk of locoregional recurrence associated with intrinsic subtypes and Ki67 expression, adjusting for known prognostic factors.ResultsAt a median follow-up of 56 months, 17 patients developed locoregional recurrence. Five-year locoregional recurrence-free survival and overall survival in the entire population were 97%, and 94.7%, respectively, with no difference between the postmastectomy radiotherapy (n = 191) and no-postmastectomy radiotherapy (n = 508) subgroups. No constructed subtype was associated with an increased risk of locoregional recurrence. A Ki67 above 20% was the only independent prognostic factor associated with increased locoregional recurrence (hazard ratio, 4.18; 95% CI, 1.11 to 15.77; P < 0.0215). However, postmastectomy radiotherapy was not associated with better locoregional control in patients with proliferative tumours.ConclusionKi67 expression but not molecular subtypes are predictors of locoregional recurrence in breast cancer patients with negative lymph nodes after modified radical mastectomy. The benefit of adjuvant radiotherapy in patients with proliferative tumours should be further investigated in prospective studies.  相似文献

Imaging diagnostic methods except for mammograms are not recommended for follow-up of postoperative breast cancer patients in order to detect small recurrences because of the poor survival improvement in earlier randomized trials. However, the use of new imaging modalities may improve survival by detection of small isolated regional lymph node recurrences which are potentially curable. Between April 2006 and December 2008, we used PET–CT to find small recurrences in follow-up of 1,907 postoperative breast cancer patients. A total of 3,280 PET–CT imagings were performed. The median age at PET–CT imaging was 58 years, with a median 48-month interval from definitive surgery to the PET–CT imaging. Twenty-two patients were found to have isolated ipsilateral regional recurrences only by PET–CT (axillary node recurrences in 6, infraclavicular node recurrences in 5, supraclavicular node recurrences in 6, and parasternal node recurrences in 5). All of those recurrences were missed by palpation or were nonpalpable. The pathological lymph node status at the definitive surgery for the primary breast cancer of 22 patients with the isolated ipsilateral regional lymph node recurrences was positive in 17 patients. If patients are limited to those who had pathologically positive node(s) at definitive surgery, the incidence of patients with isolated regional lymph node recurrences found only by PET–CT would be 2.6% (17/663 patients). Seventeen other asymptomatic cancers including contralateral breast cancers were found only by PET–CT. Early detection of isolated loco-regional recurrences of breast cancer is suggested to result in improved survival. Therefore, the use of PET–CT in follow-up of postoperative node-positive breast cancer patients may improve their survival because of early detection of isolated regional lymph node recurrences which are still potentially curable, and screening of other asymptomatic cancers.  相似文献

BACKGROUND: Neoadjuvant therapy results in a significant increase in breast-conserving surgery. However, traditional imaging methods are unable to accurately predict the extent of viable residual disease leading to uncertainty in surgical planning and some previous studies have shown a disproportionately high incidence of locoregional recurrence. Dynamic contrast enhanced-MRI (DCE-MRI) has been shown to provide a potentially more accurate prediction of residual disease. RESULTS: Patients undergoing neoadjuvant chemotherapy for breast cancer in our unit are staged with the DCE-MRI of the breast performed at 1.5 T before, during and after treatment and the final result was used to plan surgery. Two hundred and four patients with breast cancer were treated with neoadjuvant chemotherapy between 1996 and April 2005. Eighteen of these patients had distant metastases at the time of initial diagnosis and so were excluded from the present study. Following neoadjuvant chemotherapy, 186 patients underwent surgical treatment. Of these, 68 patients had breast-conserving surgery. At a median follow up of 30 months (range: 5.6-72 months) 21 patients in this group developed subsequent recurrence (21/68 - 30%) of whom 9 (9/68 - 13%) had locoregional recurrence, 7 had local recurrence (7/68 - 10%), and 17 (17/68 - 25%) had distant recurrence. Logistic regression analysis revealed only vascular invasion (p=0.006) of the tumour to be significantly associated with overall recurrence. None of the pathological factors (ER, PR status, vascular invasion, lymph node metastases, pathological complete response to neoadjuvant chemotherapy) showed a significant association with locoregional recurrence. CONCLUSION: Breast-conserving surgery with DCE-MRI planning after neoadjuvant chemotherapy provides an acceptable level of local recurrence without the need for mastectomy.  相似文献

We report a case of angiosarcoma of the breast and the autopsy findings. The patient was a 35-year-old premenopausal woman who complained of a tumor in her left breast. We found a tumor measuring 55 mm in diameter in the lower external quadrant. The tumor was elastic and soft, smooth surfaced, well-defined and mobile. Dimpling sign or change of skin color were not observed. Clinically it was diagnosed as phyllodes tumor, but tumorectomy revealed primary angiosarcoma of the breast. Further extended surgery was recommended, but the patient refused additional therapy. Histological findings revealed a free surgical margin and neither lymph node metastasis nor distant metastasis were clinically observed. Seven months later, local recurrence in the same breast was recognized and finally radical mastectomy was carried out. Histological findings showed recurrence of angiosarcoma in the left breast but lymph node metastasis was not detected. Two months after mastectomy, metastases to the cervical and thoracic vertebrae were observed and radiation therapy was performed. Sixteen months from onset, she died due to multi-organ failure as general metastases of angiosarcoma. At autopsy, metastases to many organs including the digestive system were observed. The incidence of primary angiosarcoma of the breast is low but its prognosis is poor. This case emphasized the difficulties in clinical diagnosis and treatment for the angiosarcoma of the breast.  相似文献
