In patients with internal carotid artery (ICA) occlusion, the external carotid artery (ECA) can be both a source of collateral flow and a pathway for emboli. We identified 11 patients with ICA occlusion and ipsilateral ECA stenosis who underwent ECA endarterectomy to determine its role in treating extracranial cerebrovascular disease. Follow-up ranged from 1–65 months, with a mean of 27 months. Seven of eight patients with unilateral disease remained symptom free. The eighth patient had recurrent symptoms that were subsequently diagnosed as hemi-Parkinsonism. Two of three patients with bilateral occlusive disease had developed non-hemispheric symptoms at 12 and 24 months, respectively; the third remains asymptomatic after extracranial-intracranial bypass. None of the seven patients who presented with amaurosis fugax had recurrent visual symptoms. ECA endarterectomy is a safe and effective operation in treating symptomatic patients with ICA occlusion, especially those with transient monocular blindness or unilateral occlusive disease. It is less effective in those patients who have diffuse bilateral occlusive disease.  相似文献

目的 探讨颈动脉血运重建治疗完全性颈内动脉闭塞的临床疗效.方法 2001年6月~2010年4月,收治颈动脉狭窄患者397例,术前行磁共振血管造影(MRA)检查,确诊并行颈动脉内膜切除术(CEA)治疗颈内动脉闭塞患者28例,术中切除标本送病理检查,术后复查颈部MRA,并对术后情况进行随访.结果 术后即时通畅率为92.8%,术后平均随访时间10个月,22例颈内动脉通畅,通畅率为78.5%,无脑缺血事件发生;6例颈内动脉闭塞患者中,2例在术后4个月发生短暂性脑缺血及腔隙性梗死;3例术后仍偶有头晕,其中2例单侧肢体麻木;1例记忆力减退.结论 对于有症状的颈内动脉闭塞患者,CEA加取栓术是安全有效的方法.  相似文献

The authors performed external carotid artery (ECA) angioplasty and stenting in a 45-year-old man who had presented with right hemispheric crescendo ischemic symptoms stemming from acute right internal carotid artery occlusion (ICAO). This unique application of ECA angioplasty and stenting augmented cerebral perfusion and improved clinical symptoms. In certain situations, ECA stenting can increase cerebral perfusion in the setting of ICAO and ECA stenosis. The authors are the first to describe this approach in this context.  相似文献

The authors report a novel stepwise carotid revascularization method to prevent perioperative complication. A 68-year-old man presented with left hemiparesis and dysarthria caused by severe stenosis of the right cervical internal carotid artery. According to the preoperative cerebral blood flow evaluation and plaque characterization, the patient was at risk for postoperative hyperperfusion and ischemic complications after carotid artery stenting. Initially, the patient underwent percutaneous angioplasty using an undersized balloon. Fifteen days later, the patient underwent a carotid endarterectomy. The surgical specimen obtained during the carotid endarterectomy showed the presence of typical vulnerable plaque. Of note was the complete preservation of the thin fibrous cap. The postoperative single-photon emission tomography images showed no signs of hyperperfusion, and the patient developed no neurological symptoms after each of the procedures.  相似文献

A 15-year experience with 98 patients who underwent extracranial artery reconstruction for symptomatic internal carotid artery occlusion is reviewed. Thromboendarterectomy of the occluded carotid artery resulted in unacceptably high mortality and morbidity rates, and long-term patency of the internal carotid artery was rarely achieved. Carotid endarterectomy on the side opposite the occlusion proved to be successful in relieving nonlateralizing symptoms of cerebral ischemia, whereas results were less encouraging in patients with focal symptoms in the hemisphere ipsilateral to the occlusion. External carotid artery reconstruction on the side of the occlusion was successful in relieving focal symptoms. Surgical treatment in patients with symptomatic internal carotid artery occlusion should be planned in each patient on the basis of symptoms and anatomic pattern.  相似文献

A 59-year-old male and a 74-year-old male presented with occlusion of the right internal carotid artery and stenosis at the origin of the ipsilateral external carotid artery manifesting as cerebral ischemia. External carotid endarterectomy with patch angioplasty using the internal jugular vein was performed. Special care was taken to obliterate the stump of the carotid artery using a Weck clip in one case and plication with non-absorbable sutures in the other. Cerebral blood flow in the affected hemisphere was increased after surgery and the patients remained asymptomatic. External carotid endarterectomy has several special aspects such as patch angioplasty and elimination of the stump which must be understood.  相似文献

The results of every carotid endarterectomy performed contralateral to an internal carotid artery occlusion (n = 36) (group I) were compared with those performed contralateral to a patent internal carotid artery (n = 169) (group II) over the last 10 years. The patients in each group were evenly matched with respect to male gender (66% vs 69%); mean age (66.7 vs 65.9 years); and incidence of hypertension (55.6% vs 53.2%), diabetes (16.7% vs 20.1%), and hyperlipidemia (8.3% vs 11.8%). Patients in group I had a higher incidence of previous myocardial infarction (25% vs 11.8%, p less than 0.05) and exertional angina (55.6% vs 29.6%, p less than 0.01). Indications for carotid endarterectomy were equivalent, including stroke (19.4% vs 21.9%), transient ischemic attacks (36.1% vs 35.5%), amaurosis fugax (16.7% vs 11.8%), nonhemispheric symptoms (5.6% vs 8.3%), and asymptomatic stenoses (22.2% vs 22.5%), respectively. Perioperative strokes occurred in one (2.8%) patient in group I and seven (4.1%) patients in group II (NS). Among the patients in group II the incidence of perioperative stroke did not correlate directly with the degree of contralateral ICA stenosis: greater than 90% (4%); 70% to 90% (6.7%); 50% to 70% (8.7%); and less than 50% (2.8%). The operative mortality rate was 0% among patients in group I and 1.2% among patients in group II (NS). The authors report a case of emergency carotid artery (CA) stent placement for a symptomatic acute CA occlusion following carotid endarterectomy (CEA). This 43-year-old man underwent a right-sided CEA for an asymptomatic 80% CA stenosis detected using duplex ultrasound testing. The patient experienced hypotension and possibly a myocardial infarction intraoperatively and a left hemiplegia immediately postoperatively. He was referred to the authors' institution for consideration of emergency coronary intervention and evaluation of stroke. A computerized tomography scan of the head demonstrated subtle early ischemic changes in the right posterior parietal region. Cerebral angiography revealed occlusion of the right common CA (CCA) at the CA bifurcation. Two coronary stents (Magic Wall; Boston Scientific Scimed, Maple Grove, MN) were placed in tandem in the right CCA and internal CA (ICA), overlapping at the proximal cervical ICA. Complete recanalization of the CA was achieved, and the patient made a clinically significant recovery. Diagnostic angiography can provide important information about CA and intracranial circulation that will aid in the evaluation of postoperative stroke after CEA. Stent placement should be considered as an alternative method of treatment for acute CA occlusion or dissection following CEA.  相似文献

BACKGROUND: Controversy about the optimal method of performing a carotid endarterectomy (CEA) exists despite its widespread application and support from various randomized clinical trials. Many surgeons selectively or routinely use electroencephalography (EEG) monitoring as well as shunting when performing this operation. ETHODS: We conducted this retrospective study to assess the maximum carotid clamp time without shunting or EEG monitoring during a CEA without the development of neurological deficits in an already compromised cerebral circulation. RESULTS: Fifteen consecutive patients who underwent CEAs between 1988 and 1999 met our criteria of angiographically documented ipsilateral internal carotid artery (ICA) stenosis with contralateral ICA occlusion. The patient presentations included asymptomatic (14%), transient ischemic attack (TIA) (50%), and stroke (36%). All patients were operated under general anesthesia without shunting and only 4 patients underwent EEG monitoring. On angiography, all 15 patients had ipsilateral ICA stenosis (70-99%) and contralateral occlusion. In 54% of patients, the vertebral arteries (VAs) were both patent, while in 46% of patients only 1 VA was patent. Eighty-five percent of patients had at least 1 patent anterior communicating (Pcomm) artery, while 15% had nonvisualized Pcomm arteries bilaterally. Of the 15 patients, 14 had a patent anterior communicating artery. The mean clamp time of the CCA was 18.5 minutes (range 14-30 minutes). None of the 15 patients had new neurological changes immediately postoperatively or during the 6 weeks of follow-up. CONCLUSION: We propose that shunting may not be necessary during CEA for high-grade stenosis with contralateral ICA occlusion, presumably because of adequate distal small vessel collaterals.  相似文献

Patients with one internal carotid artery occlusion and a contralateral stenosis run a significantly higher risk of stroke. We performed endarterectomy of the stenotic carotid in 44 such patients and followed them for mean 54 months (range 1–172 months). Early mortality was 2%. Life-table analysis shows that the incidence of a new stroke was 0.6% per year, the survival rate was 78% after three years, and 70% after five years. We conclude that carotid endarterectomy can be safely performed in patients with contralateral internal carotid artery occlusion and can significantly improve the long-term prognosis of these patients.  相似文献

Occlusion of the common and internal carotid arteries in a patient with symptomatic severe cerebral ischemia, with or without contralateral carotid disease, portends a poor prognosis. The present study has described our experience with subclavian and external carotid artery revascularization for symptomatic severe cerebral ischemia from common and internal carotid artery occlusion. Nine patients (five men and four women) with a mean age of 62 (range 41 to 82 years) were diagnosed as having symptomatic severe cerebral ischemia. All patients had ipsilateral hemispheric symptoms, seven had amaurosis fugax, and two had associated syncope. Four patients (three men and one woman) were hypertensive, four (two men and two women) had diabetes, eight smoked, and all had a history of coronary artery disease. All of the patients had noninvasive laboratory studies and preoperative angiography, and three had postoperative angiography. Five patients were successfully revascularized to a patent external carotid artery despite nonvisualization by angiography. Six patients had unilateral and three bilateral occlusion of the common and internal carotid arteries appropriate to their symptoms. Using regional anesthesia, four patients underwent a subclavian-external carotid bypass with polytetrafluoroethylene; saphenous vein was used in five; and three had concomitant axilloaxillary bypass grafting with polytetrafluoroethylene. Neurologic improvement (that is, no subsequent deficit and no progression of symptoms) was noted in all nine patients with a follow-up of 4 to 28 months (mean 11.2 months). Two patients died from myocardial infarction 4 and 7 months after operation. Subclavian-external carotid artery bypass is a safe addition to the options for the treatment of symptomatic severe cerebral ischemia with occlusion of the common and internal carotid arteries, visualization of a superior thyroid collateral vessel on the recipient end, and nonvisualization of the external carotid artery.  相似文献

OBJECT: Definitive data characterizing the safety and efficacy of carotid angioplasty with stent placement (CAS) for symptomatic, occlusive carotid artery (CA) disease require further refinements and standardization of techniques as well as large prospective studies on a par with the North American Symptomatic Carotid Endarterectomy Trial (NASCET). Despite the absence of such data, many surgeons have performed angioplasty and stent placement in patients with clinical or anatomical features known to add significant perioperative risk and capable of disqualifying the patients from participation in NASCET. There exists no cost analysis comparing high-risk endarterectomy with percutaneous angioplasty and stent insertion. METHODS: Forty-five patients (29 men and 16 women) with high-risk, symptomatic CA stenosis have been treated with CAS at the authors' institution since 1996. Indications for this procedure included symptomatic recurrent stenosis following CA endarterectomy (CEA), active coronary disease, high CA bifurcation, and severe medical comorbidities. A longstanding CEA computer database was screened for control patients with similar risk factors; 391 patients (276 men and 115 women) were identified. Actual cost data, duration of hospital stay, and relevant clinical data from the time of treatment until hospital discharge were collected in each patient. The median total cost of CAS was dollar 10,628, whereas that for CEA was dollar 10,148 (p = 0.495). CONCLUSIONS. In patients with high-risk, NASCET-ineligible CA stenosis there was no overall statistically significant cost difference between CEA and CAS. Given that there may not be a cost advantage for either procedure, procedural risk, efficacy, and durability should be key factors in determining the optimal treatment strategy.  相似文献

Heart rate, central venous pressure, radial artery pressure and electrocardiograph were recorded in 14 patients undergoing carotid endarterectomy under general anaesthesia supplemented with fentanyl and halothane and muscle relaxation as required. Induction of anaesthesia was followed by a significant reduction in systolic arterial pressure and a rise in central venous pressure (CVP). Thereafter CVP did not vary significantly and heart rate did not change significantly at any stage. After internal carotid artery occlusion, mean systolic pressure at one minute, 143 (SD 17) mmHg, and three minutes, 160 (SD 27) mmHg, were both significantly higher than prior to occlusion, 132 (SD 17) mmHg (P less than 0.01 and P less than 0.002 respectively). Similar significant changes occurred in diastolic pressure after carotid occlusion (P less than 0.02 and P less than 0.002 respectively). The restoration of flow through the internal carotid artery in patients operated on without a shunt was associated with a significant reduction in mean systolic pressure. The mean systolic pressure at one minute, 145 (SD 20) mmHg, and three minutes, 135 (SD 19) mmHg, were both significantly lower than that before restoration of flow, 159 (SD 17) mmHg (P less than 0.02 and P less than 0.05 respectively). Changes in mean diastolic pressure in this group at these times, while in the same direction, were not significant. The observed hypertensive response to carotid occlusion may assist in preserving cerebral perfusion while the internal carotid artery is occluded, but may be hazardous for patients with ischaemic heart disease.  相似文献

OBJECTIVE: This analysis of the outcome of carotid endarterectomy (CEA) was performed during a period of transition from the frequent use of autologous greater saphenous vein grafting to the frequent use of Dacron graft patch reconstruction and from the infrequent use to the moderate use of eversion plication shortening of the endarterectomized internal carotid artery segment. METHODS: From 1990 to 1997, 697 consecutive primary CEAs were performed on 326 men (61 bilateral CEAs) and 272 women (38 bilateral CEAs) with a mean age (+/- SD) of 68 +/- 9 years. The indications were transient ischemic attack in 31% of the patients, stroke or reversible ischemic neurologic deficit in 18%, global ischemia in 12%, and asymptomatic stenosis >/=70% in 39%. Patch reconstruction was performed in the 678 CEAs in which the arteriotomy extended distal to the internal carotid artery bulb (97%; 370 saphenous vein grafts, 308 Dacron grafts). Primary closure was used in the other 19 CEAs. Early in this series, saphenous vein patching frequently was performed, with a gradual transition to the frequent use of knitted Dacron grafts. Concurrent with this, the frequency of the shortening of the internal carotid artery increased from 7% to 40%. Postoperative duplex scans were obtained on 619 CEAs (91%). RESULTS: There were four deaths (0.6%) in 30 days-three from myocardial infarction and one from hyperperfusion stroke. Thirteen strokes (1.9%), nine ipsilateral and four contralateral, occurred in 30 days. Four nonfatal strokes and one death occurred in the saphenous vein group (3.2%), and eight strokes and two deaths occurred in the Dacron graft group (1.4%; P =.16). The combined 30-day stroke or death rate was 2.3% (16/697), and the hospital rate was 1.7% (12/697). Of the three internal carotid artery occlusions, two were identified at 2 months (one Dacron graft, one saphenous vein) and one was identified at 1 year (Dacron graft). Nonocclusive (>/=50%) restenosis was identified in 16 CEAs. Fifteen of these were in the internal carotid artery. The cumulative Kaplan-Meier method of life-table analysis for the >/=50% restenosis rate at 2 months, 6 months, 1 year, and 3 years for Dacron graft patched CEA was 1.7%, 2.3%, 8.8%, and 12.3% and for saphenous vein patched CEA was 0.3%, 0.3%, 0.3%, and 1.1% ( P <.0001). At the same time intervals, the >/=50% restenosis rate for internal carotid artery shortening was 1.0%, 2.5%, 13.7%, and 19.5%, and, when shortening was not done, the rate was 0.8%, 0.8%, 1.1%, and 3.1% (P <.0001). The >/=50% restenosis rate at the same intervals for women was 0.8%, 1.3%, 5.2%, and 8.9%, and, for men, the rate was 0.9%, 0.9%, 1.8%, and 2.5% (P =.11). Univariate analysis of the rate of >/=50% restenosis in 3 years for the 346 vein patched (2; 0.6%) and 186 Dacron graft patched (7; 3.8%) CEAs that did not have internal carotid artery shortening gave a P value of .015. Similarly, Kaplan-Meier method analysis of this subset of nonshortened CEAs gave a higher restenosis rate with Dacron graft patching (P =.012). With multiple logistic regression, the >/=50% restenosis rate was significantly associated with Dacron graft patching (P =.023; odds ratio, 4.5) and internal carotid artery shortening (P =.025; odds ratio, 3.1) and was weakly associated with female gender (P =.15; odds ratio, 2.0). Cox proportional hazards model analysis for >/=50% restenosis gave relative risk ratios of 3.0 (1.6 to 6.8; 95% confidence interval [CI]) for Dacron graft versus vein patching, 2.0 (1.2 to 3.3; 95% CI) for shortening versus not shortening, and 1.5 (0.9 to 2.4; 95% CI) for female versus male gender. CONCLUSION: CEA patching with Dacron grafts and saphenous vein grafts have similar low perioperative thrombosis, stroke, and death rates, although the stroke and death rates were slightly higher but not statistically different when Dacron grafts were used. The author presents a technique for endarterectomy and reconstruction of the carotid bifurcation in difficult cases when the plaque extends high into the internal carotid artery. The technique combines the aspects of the 2 most commonly performed procedures: carotid endarterectomy after a longitudinal arteriotomy extending from the common carotid artery into the internal carotid artery and eversion endarterectomy in which the plaque is removed from the internal carotid artery sectioned from the common carotid artery and everted. The author suggests applying this technique selectively in patients in whom the atherosclerotic plaque extends very high into the internal carotid artery. The technique offers the advantages of removing the plaque into the common carotid artery under direct vision and leaving the original dimensions and geometry of the internal carotid artery, theoretically decreasing the probability of early thrombosis and recurrent carotid disease. For routine cases, the author prefers and recommends standard carotid bifurcation endarterectomy with patch closure when the size of the arteries is reduced like in women and selected male patients.  相似文献
