Objective To evaluate the long following-up outcome of the medial gastrocnemius muscle transferring reconstruction the patella tendon after the wide resection of aggressive bone tumors in the proximal tibia. Methods With the 69 patients of the osteogenetic sarcoma in the proximal tibia were treated with the wide resection and reconstruction the patella tendon. After the long following up the knee extensor,function and complications were evaluated. Results With the 69 patients, the 45 survival patients were followed up for the average 68.6 (24-128) months. The local recurrence rate was 8.7%(6/69). The strength of knee extending was in the average of grade 4.2(3.6-5.0), the degree of knee flexion was in the average of 95°(75°-135°), the degree of knee extension was in the average of-2°(0°-12°), the knees of five patients cannot fully extension. The MSTS functional score was in the average of 77% (23.1/30). Conclusion During the limb salvage of the proximal tibial aggressive bone tumors, the medial gastrocnemius muscle transferring reconstruction the patella tendon could offer the knee extension strength; improve the soft tissue coverage and functional results.  相似文献

Objective To investigate the relationship between tibial anteroposterior axis and other anatomic landmarks in determining the rotation of tibial prosthesis in total knee arthroplasty using computed tomography. Methods Transverse CT scans of 40 volunteers' right knee in full extension were made. The anteroposterior axis of the tibia was defined as a line which was perpendicular to the transepicondylar axis and passing through the middle point of the posterior crnciate ligament. At the tibial plateau and optimum re-section level, the mean medial percentage width of intersection point of the patellar tendon anti the antero-posterior axis was measured. The mean angle between the anteroposterior axis and a line connecting the middle of the posterior cruciate ligament and the medial of the patellar tendon and the medial 1/3 of the patellar tendon were measured. Results At the tibial plateau level, the mean medial percentage width of intersection point of the patellar tendon was 10.1%±8.3%. At the optimum resection level, the mean medial percentage width of intersection point of the patellar tendon was 0.2%±10.0%. At the optimum resection lev-el, the mean angle between the anteroposterior axis and a line connecting the middle of the posterior cruciate ligament and the medial of the patellar tendon was 0.1°±2.7°. The mean angle between the anteroposterior axis and a line connecting the middle of the posterior cruciate ligament anti the medial 1/3 of the patellar tendon was 10.3°±3.6°. Conclusion There was a tendency to align the tibial component in external rotation relative to the femoral component when the medial 1/3 of the patellar tendon was used. The line connecting the middle of the posterior cruciate ligament and the medial of the patellar tendon can be used as a reliable axis for correct rotational orientation of the tibial component.  相似文献

Objective: To evaluate the therapeutic effect of combined reconstruction of anterior cruciate ligament ( ACL ) and posterior cruciate ligament ( PCL ) simultaneously by using allograft patellar tendon under arthroscopy. Methods: From May 2003 to November 2005, 10 cases of ruptured ACL and PCL were fixated with compressed screws and reconstructed under arthroscopy with allograft patellar tendon simultaneously. The clinical results were evaluated according to IKDC, Lysholm, and Tegner clinical rating scales. Results. All patients were followed up for 12-30 months (mean: 18 months ). At the last follow-up, there was no knee extension limitation and knee flexion was between 120° and 135°, with an average of 128.38°. The Lysholm score of the 10 cases was 66. 5 ± 5. 6 before operation and 89.8 ± 3.4 at last follow up. The difference was statistically significant ( P 〈 0.01 ). The average Tegner activity score decreased from 6.9 ± 1.7 ( range : 4-9 ) before injury to 5.5 ± 1. 6 (rang: 2-9) at the follow-up (P=0.53). At the end of follow-up, IKDC score was graded as A in 4 cases (40.0 % ), B in 5 (50.0 % ), and C in 1 (10.0%). Of the 10 patients, 8 returned to the same sports level as before injury and 2 were under the level. Conclusion. Arthroscopic combined reconstruction of ACL and PCL with allograft patellar tendon has the advantages of minimal trauma in surgery and reliable satisfactory outcome.  相似文献

Objective: To study the anatomic basis of the bi-pedicled V-Y gastrocnemius myocutaneous flap for repairing the composite Achilles tendon defect. Methods : The pedicle anatomy of the bi-pedicled V-Y gastrocnemius myocutaneous flap was examined on 30 cadaver specimens. The sliding distances of the flap were measured at different knee flexion degrees. The bi-pedicled V-Y gastrocnemius myocutaneous flap was applied in 12 cases of Achilles tendon defect with simultaneous skin and soft tissue defect. Results: The sural arteries could be classified into four types. After cutting off the gastrocnemius origin with a " Z-shaped" incision, the sliding distance of the flap reached (3. 7±0. 5) cm when the knee flexed 0°, (4.9±0.7) cm when the knee flexed 30°, (6. 7±0. 7) cm when the knee flexed 60 and (9.2±0.9) cm when the knee flexed 90°. All the defects healed. The patients recovered ambulation with satisfactory knee and ankle function. The follow-up was 4 months -12 years. Conclusions: Different sural artery types should be noticed during the harvest of the bi-pedicled V-Y gastrocnemius myocutaneous flap. With 90°knee flexion, this flap is suitable for one-stage repair of composite Achilles tendon defect within 9.2 cm±0.9 cm.  相似文献

Objective To explore the indication and observe short-term clinical outcome of the medial Oxford Ⅲ mobile bearing unicompartmental knee replacement (UKR) through a minimal invasive approach for the patients with osteoarthritis of the knee. Methods A retrospective study of the clinical outcomes from May 2006 to May 2008 was done on 30 knees in 21 patients after receiving the UKR with the mobile bearing Oxford Ⅲ prosthesis. There were 9 males (12 knees), 12 females (18 knees), with the mean age of (61.4±7.3) years (ranged 46-76 years). All patients were evaluated pre and postoperatively using X-ray image, the Knee Society Score (KSS) and the WOMAC osteoarthritis index and examined the ROM and the varus deformity of the knee with osteoarthritis. Results The average follow-up time was 17 months. The mean operation time was (83.0±12.9) min and the mean hospital stay was (9.3±2.0) days. No complications of infection, thrombosis, mobile beating dislocation were found. The loss of flexion of the operated knee was not significant. The average femorotibial angle decreased from 182.0°±4.3° preoperatively to 177.0°±3.5° postoperatively in the anterioposterior view of X-ray. The KSS clinical score and function score were improved from average 44.0±3.7 and 54.0±5.1 to 93.0±3.2 and 92.0±2.4 respectively. The WOMAC Score was average 48.0±4.2 preoperatively, 14.0±2.5 at the last evaluation. Statistically significant differences were found pre and postoperatively from the aforementioned evaluation criterions. Conclusion The short-term clinical outcomes of medial Oxford Ⅲ mobile bearing UKR through a minimally invasive approach are ideal, and it is a good therapy choice for the patients with medial osteoarthritis of the knee.
