Objectives:To clarify the relationship between tooth length and three-dimensional mandibular morphology in a healthy Japanese population.Materials and Methods:This study included 181 Japanese adults: 66 men and 115 women. Cone-beam computed tomography (CBCT) images were acquired with a dentofacial cone-beam x-ray CT scanner. Tooth length was measured with open-source OsiriX medical image processing software. Crown height and root length were measured in the maxillary and mandibular central incisors, lateral incisors, canines, first premolars, second premolars, first molars, and second molars. Based on these measurements, principal component (PC) analysis was performed. The following measurements were used to assess three-dimensional mandibular morphology: CD-GO, GO-GN, RCD-LCD, RGO-LGO, RCP-LCP, and the gonial angle. Stepwise multiple regression analysis was performed to examine the associations between three-dimensional mandibular morphology and the patterns of crown and root lengths using the mandibular measurements as explanatory variables and each PC as the dependent variable.Results:CD-GO was positively associated with PC1, which represented overall tooth length. RGO-LGO was positively associated with PC2, whereas GO-GN, RCP-LCP, and gonial angle were negatively associated with PC2, which was the axis denoting relatively longer root (+) vs higher crown (−). Being female was associated with PC3, which was the axis denoting relatively longer posterior tooth (+) vs anterior tooth (−).Conclusions:The present clinical study effectively used CBCT images and PC analysis to reveal significant correlations between tooth length and mandibular morphology in a modern human population, confirming in part the statement that “large teeth necessitate large jaws.”  相似文献

STATEMENT OF PROBLEM: Dimensions of teeth have been available for a century. Some significant and clinically relevant aspects of dental esthetics, however, such as the crown width/length ratios, have not been presented in tooth morphology sources until recently. PURPOSE: The purpose of this study was to analyze the anatomic crowns of 4 tooth groups (central incisors, lateral incisors, canines, and first premolars) of the maxillary dentition with respect to width, length and width/length ratios and determine how these parameters are influenced by the incisal edge wear. MATERIAL AND METHODS: Standardized digital images of 146 extracted human maxillary anterior teeth from white subjects (44 central incisors, 41 lateral incisors, 38 canines, 23 first premolars) were used to measure the widest mesiodistal portion "W" (in millimeters) and the longest inciso-cervical/occluso-cervical distance "L" (in millimeters). The width/length ratio "R" (%) was calculated for each tooth. A 1-way analysis of variance was used to compare the mean values of W, L, and R for the different groups ("unworn" and "worn" subgroups, except for premolars). Multiple least significant difference range tests (confidence level 95%) were then applied to determine which means differed statistically from others. RESULTS: There was no influence of the incisal wear on the average value of W (width) within the same tooth group. The widest crowns were those of central incisors (9.10 to 9.24 mm) > canines (7.90 to 8.06 mm) > lateral incisors (7.07 to 7.38 mm). Premolars (7.84 mm) had similar width as canines and worn lateral incisors. The L-value was logically influenced by incisal wear (worn teeth were shorter than unworn teeth) except for lateral incisors. The longest crowns were those of unworn central incisors (11.69 mm) > unworn canines (10.83 mm) and worn central incisors (10.67 mm) > worn canines (9.90), worn and unworn lateral incisors (9.34 to 9.55 mm), and premolars (9.33 mm). Width/length ratios also showed significant differences. The highest values were found for worn central incisors (87%) and premolars (84%). The latter were also similar to worn canines (81%), which constituted a homogeneous group with worn lateral incisors (79%) and unworn central incisors (78%). The lowest ratios were found for unworn canines and unworn lateral incisors (both showing 73%). CONCLUSIONS: Along with other specific and objective parameters related to dental esthetics, average values for W (mesiodistal crown dimension), L (inciso-cervical crown dimension), and R (width/length ratio) given in this study for white subjects may serve as guidelines for treatment planning in restorative dentistry and periodontal surgery.  相似文献

Objective:To examine the relationship between three-dimensional mandibular morphology and growth hormone receptor (GHR) gene variants in a healthy Japanese population.Materials and Methods:The subjects, who were unrelated Japanese orthodontic patients, consisted of 64 men and 114 women. Using the Taqman genotyping assay, GHR gene rs6184 and rs6180 variants were detected in genomic DNA extracted from saliva. Mandibular volume and length were measured from cone-beam computed tomography images that were analyzed using Analyze image-processing software. The relationship between GHR gene variants and three-dimensional mandibular morphology was statistically examined.Results:Statistical significance for the relationship between the distance between the left and right coronoid processes and rs6180 was noted (P < .05).Conclusion:Our results indicate that the GHR variant rs6180 is associated with the distance between the left and right coronoid process in the Japanese subjects.  相似文献

OBJECTIVE: Oculo-facio-cardio-dental (OFCD) syndrome is a very rare condition that requires comprehensive dental management because of associated multiple dental anomalies such as canine radiculomegaly, delayed dentition, oligodontia, persistent primary teeth, microdontia, and macrodontia. This report presents a case of OFCD in a Japanese girl (13 years 1 month old). We analyzed cephalograms, panoramic roentgenograms, and dental casts and discuss our integrated orthodontic-prosthodontic treatment. DESIGN: The sizes of the tooth crown and root as well as lateral cephalograms were compared with those from a Japanese control group. The outcome of orthodontic treatment was evaluated by comparing cephalograms taken before and after treatment. RESULTS: Radiculomegaly was found not only in the upper and lower canines but also the upper central incisors and lower first premolars. Macrodontia was found in the upper central incisors, upper canines, lower canines, and lower first premolars. Microdontia was found in the upper lateral incisor. Lateral cephalometric analysis showed a remarkable hypoplastic midface in both the sagittal and vertical dimensions, coupled with a significantly decreased cranial base length and an increased gonial angle. Serial lateral cephalograms during orthodontic treatment from 13 to 23 years of age demonstrated only slight maxillary growth and significant downward mandibular growth with clockwise rotation in addition to pronounced labial tipping of the upper central incisors. CONCLUSIONS: Characteristic dental anomalies together with a unique craniofacial dysmorphology were clarified. Successful oral rehabilitation was achieved by integrated orthodontic-prosthodontic treatment.  相似文献

Abstract In a previous study on 42 young adult, periodontally healthy subjects without any attrition, abrasion or crown restoration, gingival thickness (GTH) was determined at facial aspects of premolars, canines and incisors by a novel ultrasonic device. GTH strongly depended on periodontal probing depth, width of gingiva (WG), and tooth type. Whereas the ratio of crown width to its length (CW/CL) was not identified as an explanatory variable, a significant influence of the subject was ascertained. The aim of the present study was to extend these analyses in order to identify subjects with different morphological characteristics of gingiva, i.e., gingival phenotypes. When employing cluster analysis on standardized parameters mean GTH, WG and CW/CL of upper canines, lateral and central incisors, 3 clusters were identified. Cluster A comprised 2/3 of subjects, displaying “normal” GTH, WG and CW/CL. Cluster B (n=9, 21%) had a significantly thicker and wider gingiva, and a more quadratic form of upper front teeth. A 3rd cluster (cluster C, n=5, 12%) was identified showing “normal” GTH, high CW/CL, but a narrow zone of keratinized tissue. Some characteristics of gingival phenotype of the upper front tooth region were also found at upper premolars (WG, CW/CL) but in general not at mandibular teeth. Present results clearly indicate evidence for the existence of different gingival phenotypes. Clinical relevance of these observations has to be tested in longitudinal studies.  相似文献

To evaluate the clinical crown length relative to fixed-appliance orthodontic treatment of excessive overjet and deep overbite and to correlate such changes to the vertical dental height, the following measurements were undertaken for 12 females and 8 males, between the ages of 16 and 20 years, on three separate occasions--2 days before banding, 2 days after debanding, and 12 months after debanding: (1) overjet, overbite and dental height measured from right lateral cephalometric x-ray films; (2) clinical crown length, measured from study models, of 400 teeth divided into four groups--maxillary incisors and canines (120 teeth), maxillary second premolars and first molars (80 teeth), mandibular incisors and canines (120 teeth) and mandibular second premolars and first molars (80 teeth); and (3) gingival condition by means of the gingival index of L?e and Silness. Fixed edgewise orthodontic appliances were used and the four first premolars were extracted. From the results of the investigation, the following conclusions were evident: after a 12-month follow-up observation period, the achieved reduction in overjet, overbite, and dental height showed relapses of 9%, 11%, and 29%, successively; only 7% of the 400 teeth examined showed reductions in clinical crown length. This change was probably the result of gingival hyperplasia. The gingival condition greatly improved by approximately 64% after 12 months of debanding, accompanied by 25% to 50% relapse in the amount of change in clinical crown length (noted 2 days after debanding); and the intrusive tooth movement during orthodontic correction of deep overbite was the result of vertical movement of the tooth, with its investing tissues and soft-tissue attachment, into the jaws.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

Objectives:To measure the root lengths of maxillary central incisors (U1) and evaluate the relationship among U1 root length, tooth movement, and type of treatment appliance in patients with unilateral cleft lip and palate over a long-term follow-up period.Materials and Methods:Occlusal radiographs of 30 patients with unilateral cleft lip and palate, acquired less than 6 months before secondary alveolar bone grafting (SBG, T1) and after edgewise treatment (T2), were measured for U1 root length (R1 and R2, root lengths at T1 and T2, respectively). Frontal and lateral cephalometric radiographs acquired at eruption of U1 (T0), T1, and T2 were evaluated to determine the inclination and position of U1.Results:The average values of R1 and R2 on the cleft side were significantly lower than those on the noncleft side. Frontal cephalometric analysis revealed that the horizontal distance of the root apex from the median vertical line at T0 on the cleft side was significantly smaller than that on the noncleft side and was correlated with short U1 root length on the cleft side. On the other hand, R1 in patients treated with maxillary protraction appliances between T0 and T1 was significantly shorter than that in patients without maxillary protraction appliances. However, none of the changes in cephalometric measurements were correlated with root length.Conclusions:In patients with unilateral cleft lip and palate, the short root length of cleft-adjacent central incisors might be associated with the horizontal position of the root apex. In addition, orthodontic treatment with a maxillary protraction appliance before secondary alveolar bone grafting might be associated with short U1 root length.  相似文献

The purpose of this study was to clarify the interrelation of the tooth crown diameters using statistical methods. The material consisted of 414 dental casts from orthodontic patients in the Japanese female. The mesiodistal diameters of all permanent teeth on the left side were measured, excluding the third molars. These values were examined by correlation coefficient matrix and multivariate analyses such as multiple regression, principal component and canonical correlation, etc. The results were as follows: 1) The tooth crown diameters showed a strong intercorrelation within each jaw and between both jaws. 2) When the incisors and the canine were large in size, the premolars and the molars tended to be small. 3) When the incisors and the molars were large in size, the canine and the premolars tended to be small. 4) When the incisors and the premolars were large in size, the canine and the molars tended to be small. 5) All of the teeth except the upper lateral incisor were classified according to size into the four groups of incisors, canines, premolars and molars. 6) The dentition with small teeth tended to show a spaced arch.  相似文献

ObjectiveThis study aims to analyze the relationship between the central incisor crown inclination and occlusal relationship via three-dimensional study.MethodsTen Chinese volunteers with normal occlusion were selected from physical examination of college students. Lateral cephalograms and CT scans were taken, and the CT data were imported into Simplant pro 11.04 to reconstruct three-dimensional model. In the three-dimensional model, every tooth was separated independently. The upper and lower central incisors were moved labial inclination(up) or lingual inclination(down) and then the upper and lower tooth were aligned based on the central incisor’s position according to the principles of Andrew’s six keys. Four groups were set up with different changes of central incisors. The upper and the lower arch were aligned again based on the moved central incisors. After every motion, the mesio-distal distances of upper and lower canines and first molars were recorded. All data were analyzed using t-test via SPSS19.0, and the significance level was set at 5%.ResultsThe results showed that the mesio-distal distances of occlusal relationship were different from normal occlusal relations, when either upper incisor crown inclination or lower incisor crown inclination was changed, and both upper and lower incisor crown inclination were moved in same or opposite direction. According to the statistical analysis, there was no significant difference when the changes of U1-L1 were no more than 10° (P > 0.05). But there was statistically significant difference (P < 0.05) when changes of U1-L1 were more than 20°.ConclusionsThe incisor crown inclination has influence on the establishment of occlusal relationships. The bigger difference upper and lower incisor crown inclination has, the more deviations from the normal occlusal relationship exist.  相似文献

Objectives:To determine root lengths of fully developed permanent teeth of cleft lip and palate (CLP) patients and to define their crown-root (C/R) ratios.Method:Crown height and root length of permanent teeth were measured from panoramic radiographs of 44 CLP patients and 37 controls. A total of 1397 teeth were measured, and C/R ratios were calculated.Results:Higher C/R ratios were found in CLP patients; this was statistically significant for both maxillary and mandibular incisors and canines. Bilateral CLP subjects showed higher C/R ratios in general than unilateral CLP subjects. Roots of maxillary incisors, canines, and some other teeth were significantly shorter in CLP patients than in controls.Conclusions:CLP patients should be considered to have unfavorable C/R ratios, which could be the result of short root lengths for some teeth.  相似文献

There is limited knowledge concerning tooth and gingival display in the maxillary premolar area during smiling. The purpose of this study was to investigate maxillary premolar and associated gingival display during smiling, and to reveal possible differences related to gender and age. The study consisted of 90 males and 113 females (mean age 38 years, s.d. = 13.83) receiving regular dental care. Two pictures of each subject, one with lips in maximum smile, and the other a retracted frontal view, were made. Clinical crown length, tooth and gingival display were measured for the left maxillary premolars. Central incisors and canines were used as controls. anova with Bonferroni post hoc multiple comparisons tests was used to assess any statistically significant impact of gender and age on tooth and gingival display, and determine differences among gender and age groups (at 5% significance level). Premolars were partially visible in more than 80% of the smiles, and displayed more than 65% of their clinical crown length during smiling. The least display (P < 0.001) was recorded for first and the greatest for second premolars. Tooth display was greater in females, but statistically significant only in central incisors (P < 0.05). No significant differences were revealed between age subgroups. Gingival display was greater for premolars compared with both central incisors (P < 0.001) and canines (P < 0.05). Displayed gingiva was found in first and second premolars in 44 and 49% of subjects, respectively. More gingival display was recorded in females and in younger subjects.  相似文献

Objective:To evaluate the effectiveness of computer-assisted orthodontic treatment technology to produce the tooth position prescribed by the virtual treatment plan.Materials and Methods:Posttreatment models of 23 patients treated with SureSmile were digitally superimposed on their corresponding virtual treatment plan models utilizing best-fit surface-based registration. Individual tooth-position discrepancies between virtual treatment plan and actual outcome were computed. Discrepancies less than 0.5 mm in mesial-distal, facial-lingual, and vertical dimensions, and less than 2° for crown torque, tip, and rotation were considered clinically ideal. One-sided test of equivalence was performed on each discrepancy measurement, with P < .05 considered statistically significant.Results:Mesial-distal tooth position was clinically ideal for all teeth with the exception of maxillary lateral incisors and second molars. Facial-lingual tooth position was clinically ideal for all teeth except maxillary central incisors, premolars, and molars, and mandibular incisors and second molars. Vertical tooth position was clinically ideal for all teeth except mandibular second molars. For crown torque, tip, and rotation, discrepancy exceeded the limits considered clinically ideal for all teeth except for crown torque on mandibular second premolars and crown tip on mandibular second premolars and first molars.Conclusions:The effectiveness of computer-assisted orthodontic treatment technology to achieve predicted tooth position varies with tooth type and dimension of movement.  相似文献

目的: 利用锥形束CT(cone beam computed tomography,CBCT)研究上颌侧切牙和尖牙区骨开裂和骨开窗的危险因素。方法: 实验对象选自2014~2016年就诊于中国医科大学附属口腔医院正畸一科患者治疗前的CBCT,存在骨开裂或骨开窗的上颌侧切牙44例和上颌尖牙43例为实验组,不存在骨开裂和骨开窗的上颌侧切牙43例和上颌尖牙41例为对照组。结果: 牙体倾斜度、牙体长轴与牙槽突长轴的角度、牙槽骨曲度、牙槽骨宽度、牙根宽度与牙槽骨宽度差、牙齿在牙槽骨中的位置有统计学差异(P<0.05),牙根长度、根尖距牙槽骨最凹点的距离无统计学差异(P>0.05)。结论: 牙齿舌倾、牙体长轴与牙槽突长轴角度大、牙槽骨薄、牙槽骨弯曲大、牙齿位置偏离牙槽骨中心(偏唇侧或舌侧)等是上颌侧切牙和尖牙区发生骨开裂与骨开窗的危险因素。  相似文献

Objectives:To investigate the relationships among different intrusion patterns of clear overlay aligners and the corresponding orthodontic forces and to provide guidance for clinical treatment.Materials and Methods:Five sets of removable thermoplastic-formed aligners with the same thickness, designed for different intrusion procedures (G0 aligners as a control group, with no activation; G1 aligners for intruding canines; G2 aligners for intruding incisors; G3 aligners for intruding canines and incisors with the same activations; G4 aligners for intruding canines and incisors with different activation), were manufactured, and the corresponding intrusion forces were measured with a multiaxis force/torque transducer measurement system in real time.Results:With the same activation (0.2-mm intrusion) and rectangular attachments placed on the premolars and first molars, the canines experienced the largest intrusive force when intruded alone using G1 aligners. The canines received a larger intrusive force than incisors in G3. The incisors received similar forces in G2 and G3. First premolars endured the largest extrusive forces when all anterior teeth were intruded with G3 aligners. Extrusion forces were exerted on canines and lateral incisors when using G4 aligners.Conclusions:Aligners with different intrusion patterns exert different forces on incisors, canines, and premolars, and the forces were closely related to the designed activation, shape and position of the attachment and relative movement of the adjacent teeth.  相似文献

IntroductionThe purpose of this study was to investigate the root canal configuration of the mandibular anterior teeth using cone-beam computed tomographic (CBCT) imaging in a Chinese subpopulation.MethodsA total of 3,871 CBCT images of mandibular anterior teeth were collected from 648 patients who accepted CBCT projection as a preoperative assessment for implants or orthodontic treatment. The following items were recorded and evaluated: tooth position, root number, canal number, root canal type, the distance between the anatomic apex and the point at which the canal divided into 2 for mandibular anterior teeth with 2 root canals (excluding canines with 2 roots), and the distance between the 2 root canal orifices. The Fisher exact test was used to analyze the correlation between the number of root canals and tooth position.ResultsAll of the incisors in this study had 1 root, and 1.32% of the canines had 2 roots. The prevalence of 2 root canals in the lateral incisors (354, 27.36%) was higher than that in the central incisors (202, 15.71%) (P < .05) and the canines (81, 6.27%) (P < .05).ConclusionsThere is a high prevalence of 2 root canals in the mandibular anterior teeth of the studied Chinese subpopulation. This study provides detailed information about the root canal morphology of mandibular anterior teeth in a Chinese subpopulation.  相似文献

Objective:To determine the root-crown (R/C) ratio and dental root length of teeth in patients with open bite and seek any relationships with occlusal contact (OC) and the mandibular plane (Mp) angle.Materials and Methods:Thirty-one patients with open bite with negative overbite of at least four anterior teeth and 31 control patients with clinically normal overjet and overbite were enrolled. R/C ratios, dental root length, OC, and Mp angle were measured using panoramic radiographs, dental casts, and cephalograms, respectively. Mean differences between the groups, and variations between the R/C ratio or root length and Mp angle in patients with open bite were statistically analyzed.Results:R/C and OC ratios from the incisors to premolars were significantly lower for patients with open bite than for controls, and some teeth had short dental roots. Relationships between low R/C ratio or root length and high Mp angle were significant in patients with open bite.Conclusion:Patients with open bite, especially those with a high Mp angle, have an unfavorable R/C ratio and short dental roots in some teeth, which may be related to the loss of OC.  相似文献

Objectives:To determine upper incisor root resorption, volume loss, and the relationship between root volume loss and tooth movement after 1 year of orthodontic treatment in patients with marginal bone loss.Materials and Methods:A total of 30 women (46.3 ± 5.4 years old) with moderate upper incisor bone loss who required intrusion during orthodontic treatment were recruited. Pre- and post-treatment cone beam computed tomography images were reconstructed. Upper incisors at pre- and post-treatment were superimposed; labio- and palato-apical, middle, and coronal third root volumes were assessed. Tooth movement and alveolar bone height were measured from lateral cephalometric radiographs and cone beam computed tomography. Changes in root volume/alveolar bone height were compared using paired-sample t-tests, percentage root volume loss for each tooth/segment was evaluated by one-way analysis of variance, and the relationship between percentage root loss and degree of tooth movement was assessed by linear regression.Results:Mean root volume significantly decreased on the labio- and palato-apical aspects of 12 and labio-apical aspects of 21 and 22 (P ≤ .024). Palato-apical segment volume loss was greater on lateral than central incisors (P ≤ .016). Two-dimensional root length and cementoenamel junction-bone crest distance did not change between T0 and T1, with no significant relationship between tooth movement amount and percentage root volume loss.Conclusions:Delivery of 40 g intrusive force to the four upper incisors using a T-loop and the leveling phase lead to more apical root volume loss on lateral than central incisors. There was no relationship between extent of tooth movement and upper incisor root volume loss.  相似文献
