目的观察心脏再同步治疗慢性心力衰竭的临床疗效。方法36例慢性心力衰竭合并室内传导阻滞的患者行双心室再同步起搏治疗。全部患者均经冠状静脉窦植入左心室导线至心脏静脉。治疗后随访12个月,观察心功能,6min步行距离,QRS波宽度,心室间运动延迟,左心室收缩、舒张末内径,左心室射血分数,二尖瓣反流面积。结果31例治疗后心功能改善,有效率86.1%(31/36)。心功能从Ⅲ~Ⅳ级(纽约心脏协会心功能分级)改善为Ⅱ~Ⅲ级;6min步行距离从(362±153)m提高至(528_+165)m,差异有统计学意义(P〈0.01);QRS波从(164.74±33.76)ms缩短至(129.45±42.27)ms,差异有统计学意义(P〈0.01);心室间运动延迟时间从(65.19±21.50)ms缩短至(33.25±13.62)ms,差异有统计学意义(P〈0.01);左心室舒张末内径从(66.52±10.23)mm缩小至(60.63±9.97)mm(P〈0.05),左心室收缩末内径从(55.73±10.62)mm缩小至(47.45±11.35)mm,差异有统计学意义(P〈0.01);左心室射血分数从30.35%±4.69%提高至42.27%±8.40%,差异有统计学意义(P〈0.01);二尖瓣反流面积从(7.52±3.62)cm2减少至(4.33±2.07)cm2,差异有统计学意义(P〈0.01)。结论心脏再同步治疗是治疗慢性心力衰竭的有效方法,能使心脏活动再同步化,改善心功能。  相似文献

目的随访慢性心力衰竭患者心脏再同步治疗(CRT)的临床疗效。方法入选1999年至2008年行CRT的慢性心力衰竭患者,收集这些患者基线和最后一次临床资料。采集数据包括临床心功能评估,心脏超声学指标和体表心电图QRS时限宽度。结果95例患者行CRT治疗,分为早期组和近期组。早期组随访时间(57±17)个月,随访生存率为66.6%;近期组随访时间(18±10)个月,生存率为89.1%。两组患者CRT治疗后心功能显著提高,但自身状态下体表心电图QRS时限宽度变化差异无统计学意义。随访中患者药物治疗变化明显。结论中长期和短期随访均证实CRT治疗在一部分慢性心力衰竭患者中能提高心功能,但缺乏预测疗效的敏感指标。  相似文献

目的观察双心室再同步起搏治疗慢性心力衰竭(CHF)的临床疗效。方法28例慢性心力衰竭合并室内阻滞患者行双心室再同步起搏治疗,全部患者均经冠状静脉窦植入左心室导线1根至心脏静脉,术后平均随访20.6个月,观察心功能,QRS波宽度,左心室收缩、舒张末内径,左心室射血分数,二尖瓣反流面积。结果24例患者治疗后心功能改善,有效率85.7%,心功能从Ⅲ~Ⅳ级(NYHA分级)改善为Ⅱ~Ⅲ级,QRS波从(160±58)ms缩短至(132±53)ms,P<0.05,左心室舒张末内径、收缩末内径分别从(73.54±9.96)mm、(64.25±11.32)mm缩小至(68.75±8.63)mm和(58.51±10.78)mm,P<0.01,左心室射血分数从0.23±0.09提高至0.32±0.10,P<0.01,二尖瓣反流面积从(8.03±4.27)cm2减少至(5.15±4.02)cm2,P<0.01。结论双心室再同步起搏是慢性心力衰竭治疗的有效方法。  相似文献

目的 观察心脏再同步化治疗(CRT)心力衰竭伴传导阻滞患者心功能改善情况.方法 连续入选在我科住院的终末期心力衰竭患者45例,按照QRS时限、形态分为完全性左束支传导阻滞组、完全性右束支传导阻滞组和非特异性室内传导阻滞组,分别评估、测量三组患者术前及术后6个月的NYHA分级、左室舒张末内径(LVED)、左室射血分数(LVEF)及血浆B型利钠肽(BNP)浓度.结果 完全性左束支传导阻滞组和非特异性室内传导阻滞组患者术后NYHA分级改善[(2.8±0.3)级比(3.6±0.2)级,(2.9±0.3)级比(3.5±0.4)级,P<0.05],LVED缩小[(67.2±7.5)mm比(74.2±6.5)mm,(66.4±9.9)mm比(75.8±9.1)mm,P<0.05],LVEF增大[(33.6±7.7)%比(26.6±7.2)%,(44.4±5.1)%比(28.6±5.7)%,P<0.05],血浆BNP浓度下降[(3988.4±628.3)pg/ml比(8542.6±1384.9)pg/ml,(3573.2±749.5)pg/ml比(6666.2±1110.5)pg/ml,P<0.05];完全性右束支传导阻滞组NYHA分级没有改善[(3.3±0.2)级比(3.4±0.3)级,P>0.05],LVED无变化[(61.2±5.0)mm比(62.4±4.3)mm,P>0.05],LVEF没有增大[(35.2±12.3)%比(33.4±11.8)%,P>0.05],血浆BNP浓度无明显变化[(6844.7±774.8)pg/ml比(7558.6±1327.4)pg/ml,P>0.05].结论 心脏再同步化治疗能够改善完全性左束支传导阻滞及非特异性室内传导阻滞患者的心脏功能,不能改善完全性右束支传导阻滞患者的心脏功能.  相似文献

目的:分析左束支传导阻滞(left bundle branch block,LBBB)心力衰竭患者对心脏再同步(cardiac resynchronization therapy,CRT)治疗的疗效反应情况。方法:回顾性分析在本院成功置入CRT伴LBBB的心力衰竭患者。所有患者术前、术后1w及最终随访均行12导联心电图、超声心动图、组织多普勒。均对比术前、术后左心室大小、射血分数及生存现状。结果:共44例完成随访,随访6~61个月,平均27.12个月,4例再同步治疗后仍反复出现心力衰竭并因此再入院治疗,无死亡病例。共有38例(86.4%)患者对CRT治疗有反应,真性LBBB亚组中29例(93.5%)有反应,非真性LBBB亚组中9例(69.2%)有反应。真性LBBB亚组左心室射血分数绝对值、左心室舒张末径(left ventricular end diastolic diameter,LVEDD)及左心室射血分数的改善程度更显著,差异有统计学意义。结论:CRT治疗可使大部分患者获益,真性LBBB患者心功能改善更为显著,但终末期心脏失代偿严重者难于从中获益。  相似文献

目的评价心脏再同步治疗(CRT)难治性心力衰竭的临床疗效。方法21例难治性心力衰竭患者接受CRT治疗;左心室导线采用经心脏静脉血管径路植入或开胸直接缝合左心室心外膜导线。植入术后定期随访,分析患者心功能、活动耐量、生活质量评分、QRS时限、左心室射血分数(LVEF)、左心室舒张末内径(LVEDD)等指标的变化,评价CRT的临床疗效,测试起搏参数并在超声心动图指导下优化AV间期和VV间期。结果21例患者成功植入CRT,其中3例患者因心脏静脉血管畸形而采用开胸植入左心室心外膜导线。平均随访(9.8±9.5)个月,死亡3例;2例患者因心力衰竭症状复发再住院;其余患者心功能分级、生活质量评分、活动耐量较植入术前有显著改善;植入术后1个月LVEF明显提高(0.31±0.06对0.37±0.05,P〈0.01);二尖瓣反流程度[(2.24±0.89)级对(1.43±0.75)级,P〈0.01];LVEDD植入术后6个月明显减小((71.91±8.38)mm对(68.82±10.80)mm,P〈0.05];QRS时限分析显示CRT治疗有效组术后明显变窄((162.50±24.08)ms对(142.50±19.15)ms,P〈0.05],无效组变化不明显[(148.00±10.95)ms对(138.00±4.47)ms,P〉0.05]。1例患者左心室导线脱位。右心房和右心室起搏导线的阈值、感知灵敏度及阻抗均在正常范围,左心室导线各参数,除1例患者阈值升高外余均正常。3例开胸植入心外膜导线患者除1例于植入术后第5个月心脏性猝死,余2例分别随访18个月和26个月,左心室导线阈值分别为0.5V/0.4ms和1.0V/0.4ms。CRT治疗的总临床有效率为76.2%。结论CRT治疗难治性心力衰竭有效,能恢复患者心脏电活动和机械活动的同步性,显著改善心功能。QRS时限变窄是CRT疗效的重要预测指标。  相似文献

目的:观察窄QRS波慢性心力衰竭(CHF)患者的双心室再同步治疗的临床疗效。方法:筛选16例窄QRS波CHF患者行双心室再同步治疗,所有病例均经冠状静脉窦植入左心室导线至心脏静脉,术后平均随访1~43个月,平均13个月,观察心功能、左心室射血分数、二尖瓣反流面积、左心室舒张末和收缩末内径等的变化。结果:16例患者治疗后心功能明显改善,有效率75%,心功能从Ⅲ~Ⅳ级(NYHA分级)改善为Ⅱ~Ⅲ级,左心室射血分数从(0.31±0.11)%提高至(0.38±0.10)%,P0.05,每搏输出量从(74±8)ml提高至(96±4)ml,P0.05,二尖瓣反流面积从(8±4)cm2减少至(6±3)cm2,P0.05,左心室舒张末内径、收缩末内径虽有缩小趋势但没有显著差异[(69±7)mmvs.(68±9)mm;(59±10)mmvs.(56±11)mm]。结论:双心室再同步治疗对某些窄QRS波CHF患者同样有效。  相似文献

目的观察心脏再同步治疗(CRT)对慢性心力衰竭的长期临床疗效。方法24例接受CRT的慢性心力衰竭患者,心功能Ⅲ~Ⅳ级(NYHA分级),左心室射血分数(LVEF)≤0.35,左心室舒张末内径(LVEDD)/〉55mm,QRS时限≥120/TIS。植入前超声心动图及组织多普勒检查以评价心功能及心脏收缩不同步指标,并指导左心室电极导线的植入。植入后3、6个月及随后的每6个月进行随访,随访内容包括临床症状、心电图、LVEDD、LVEF及多普勒超声评价心脏收缩同步性指标。结果随访时间(12.0±4.6)个月,结果显示患者临床症状明显改善,QRS时限植入后较植入前缩短[(137.50±38.96)ms对(144.60±45.78)ms,P=0.67],但差异无统计学意义。植人后LVEDD较植入前明显缩小[(6.24±0.89)cm对(6.78±0.42)cm,P=0.03];LVEF则明显提高(0.36±0.09对0.31±0.03,P〈0.01),左心房内径(LAD)也较术前明显缩小[(4.22±8.43)cm对(5.32±7.63)cm,P=0.01]。心脏收缩不同步指标与植入前相比也明显改善。结论对慢性心力衰竭的患者,CRT治疗可改善左心室功能,逆转左心室重构。  相似文献

目的 观察双心室同步起搏治疗慢性心力衰竭 (心衰 )的临床效果。方法 慢性心衰伴心室内传导延迟患者 56例 ,男性 43例 ,女性 1 3例 ,平均年龄 52 9岁。心功能均在Ⅲ~Ⅳ级 (NYHA) ,体表心电图QRS波平均为 (1 4 3 1± 1 9 4)ms。其中 5例为慢性心房颤动 (房颤 ) ,其余为窦性心律患者 ,患者均进行了双心室同步起搏。慢性房颤患者先行房室结消融术。术前先进行逆行冠状静脉窦造影了解冠状静脉窦分支血管分布 ,然后将起搏电极导线分别置于右心房、右心室及冠状静脉窦分支血管起搏左心室 ,慢性房颤患者仅进行左、右心室起搏。观察双心室起搏前后左心室射血分数变化以及体表心电图QRS波变化。结果 双心室同步起搏后 ,患者心功能得到明确改善 ,左心室射血分数从 (2 6 1± 7 6) % ,提高至 (34 2± 8 9) % (P <0 0 5) ,左心室充盈时间延长 ,二尖瓣反流量减少。心功能平均改善Ⅰ级。QRS波时限由术前的 (1 4 3 1± 1 9 4)ms缩短至 (1 2 4 8± 1 6 7)ms(P <0 0 5)。结论初步临床观察提示 ,双心室同步起搏可有效改善慢性心衰伴心室内传导延迟患者的近期心功能 ,远期效果有待进一步观察  相似文献
