[Purpose] To investigate the effects of a seven-week quadriceps stretching program on the muscle fibre orientation of the vastus medialis oblique and vastus lateralis in the lower limbs by ultrasound imaging. [Participants and Methods] Twenty-seven healthy, physically fit, asymptomatic females and males (age 21.5 ± 1.3, Tegner activity level score ≥4) were recruited. Their initial vastus medialis oblique and vastus lateralis fibre angles were determined using ultrasound. They then undertook a seven-week quadriceps stretching program, 3 sets of stretches to be performed on both lower limbs, 3 times a week on 3 separate days. One volunteer was assigned as an intra-rater control and did not take part in the stretching program. The vastus lateralis and vastus medialis oblique fibre angles were measured again on completion of the exercise regime. [Results] A statistically significant decrease in muscle fibre angle was observed in both the right and left vastus medialis oblique, and the right and left vastus lateralis. [Conclusion] A 7-week stretching program can result in a significant decrease in muscle fibre angle in both the vastus medialis oblique and the vastus lateralis. This can help in understanding the effects of prescribed stretching exercises on athletic patients with PFP.  相似文献

[Purpose] This study evaluated the effective selective activation method of the vastus medialis oblique for knee joint stabilization in patients with patellofemoral pain syndrome. [Subjects and Methods] Fifteen healthy college students (9 males, 6 females); mean age, height, and weight: 22.2 years, 167.8 cm, and 61.4 kg, respectively) participated. The knee angle was held at 60°. Muscle activities were measured once each during an ordinary squat and a squat accompanied by hip joint adduction. The muscle activities of the vastus medialis oblique and vastus lateralis were measured by electromyography for five seconds while maintaining 60° knee flexion. Electromyography signals were obtained at a sampling rate of 1,000 Hz and band pass filtering at 20–50 Hz. The obtained raw root mean square was divided by the maximal voluntary isometric contraction and expressed as a percentage. The selective activity of the vastus medialis oblique was assessed according to the muscle activity ratio of the vastus medialis oblique to the vastus lateralis. [Results] The activity ratio of the vastus medialis oblique was higher during a squat with hip joint adduction than without. [Conclusion] A squat accompanied by hip joint adduction is effective for the selective activation of the vastus medialis oblique.Key words: Patellofemoral pain syndrome, Vastus medialis oblique, Squat  相似文献

[Purpose] This study investigated the effects of the slow speed-targeting squat exercise on the vastus medialis oblique/vastus lateralis ratio. [Subjects] Ten asymptomatic men were recruited. [Methods] The EMG activities of the vastus medialis oblique and vastus lateralis muscles were recorded using surface electrodes. The subject performed the squat exercise under 3 different conditions. [Results] The vastus medialis oblique/vastus lateralis ratio in condition 2 (1.5 ± 0.7) was significantly higher than that in conditions 1 and 3 (1.0 ± 0.5, 1.1 ± 0.8, respectively) [Conclusion] Therefore, an effectively slow movement speed is recommended for selective strengthening of vastus medialis oblique using a slow speed-targeting device that provides biofeedback.Key words: Slow movement speed, Squat exercise, VMO  相似文献

[Purpose] This study examined the effects of neuromuscular training on knee joint stability after anterior cruciate ligament reconstruction. [Subjects and Methods] The subjects were 16 adults who underwent arthroscopic anterior cruciate reconstruction and neuromuscular training. The Lysholm scale was used to assess functional disorders on the affected knee joint. A KT-2000 arthrometer was used to measure anterior displacement of the tibia against the femur. Surface electromyography was used to detect the muscle activation of the vastus medialis oblique, vastus lateralis, biceps femoris, and semitendinosus before and after neuromuscular training. [Results] There was significant relaxation in tibial anterior displacement of the affected and sound sides in the supine position before neuromuscular training. Furthermore, the difference in the tibial anterior displacement of the affected knee joints in the standing position was reduced after neuromuscular training. Moreover, the variation of the muscle activation evoked higher muscle activation of the vastus medialis oblique, vastus lateralis, biceps femoris, and semitendinosus. [Conclusion] Neuromuscular training may improve functional joint stability in patients with orthopedic musculoskeletal injuries in the postoperative period.Key words: Arthrometer, Anterior cruciate ligament reconstruction, Neuromuscular training  相似文献

[Purpose] This study aimed to investigate the relationships among the changes in iliopsoas muscle thickness, hip angle, and lower limb joint moment during squatting in different pelvic positions to help in performing hip-dominant squatting exercises. [Participants and Methods] The participants were seven healthy adult males. The measurement task consisted of squatting with 60 degrees of knee flexion in three positions: the anterior, neutral, and posterior pelvic tilt positions. The iliopsoas muscle thickness was measured in the center of the inguinal region using ultrasonography. A three-dimensional motion analysis system was used to measure the joint angles and joint moments. [Results] There were no significant differences in pelvic angles between the pelvic positions. The hip angle differences were significantly higher in the anterior and neutral pelvic tilt positions compared to those in the posterior tilt position. Only the anterior pelvic tilt position had a significantly positive correlation with iliopsoas muscle thickness and hip angle differences. [Conclusion] Squatting in the neutral or posterior pelvic tilt position was not associated with hip angle and iliopsoas muscle thickness changes, whereas squatting in an anterior pelvic tilt position was associated with changes in the iliopsoas muscle thickness and hip flexion angle. OBJECTIVE: Investigation of the influence of weakness in the vastus medialis oblique muscle on patellar tracking. DESIGN: In vitro biomechanical study. BACKGROUND: Currently, the influence of weakness in the vastus medialis oblique muscle on patellar tracking has not been well understood. METHODS: Seven human cadaveric knees were used. The direction of loading forces in the rectus femoris/vastus intermedius, vastus medialis oblique, and the vastus lateralis muscles was decided by the muscle alignment of each cadaver knee measured at the time of dissection. The loads used were 60 N in the rectus femoris, 50 N in the vastus lateralis, and 40 N in the vastus medialis oblique, according to the ratio calculated from the cross-sectional study. The weakness of vastus medialis oblique was simulated at 30 N (75%), 20 N (50%), 10 N (25%), and 0 N (0%), and the patellar position was measured for each condition using a magnetic 3 Space Tracker System. The influence of weakness in the vastus medialis oblique muscle on patellar position was investigated with seven cadaver knees using a magnetic 3 Space Tracker System. RESULTS: At 0 degrees and 15 degrees of knee flexion, 75%, 50%, 25% and 0% of the normal vastus medialis oblique muscle led to a significant difference in lateral patellar shift compared to the normal (P<0.05). CONCLUSIONS: Weakness of the vastus medialis caused the patellar lateral shift at 0 degrees and 15 degrees of knee flexion. RELEVANCE: Weakness of the vastus medialis is thought to be an important factor causing patellar subluxation and dislocation. Understanding the relationship between the vastus medialis weakness and patellar tracking will be useful in diagnosis, treatment and prevention of patellar subluxation and dislocation.  相似文献

[Purpose] The purpose of the present study was to examine the effects of squat exercises performed on different surfaces on the activity of the quadriceps femoris muscle in order to provide information on support surfaces for effective squat exercises. [Subjects and Method] Fourteen healthy subjects performed squat exercises for five seconds each on three different support surfaces: hard plates, foam, and rubber air discs. Their performance was measured using electromyography. As the subjects performed the squat exercises on each surface, data on the activity of the vastus medialis oblique and the vastus lateralis, and the vastus medials oblique/vastus lateralis ratio, were collected. [Results] The activity of the vastus medialis oblique and the vastus medialis oblique/vastus lateralis ratio were found to be statistically significantly higher on rubber air discs than when the squats were performed on hard plates or foam. [Conclusion] To activate the vastus medialis obilique, and to enhance the vastus medialis oblique/vastus lateralis ratio, unstable surfaces that are highly unstable should be selected.Key words: Support surfaces, Vastus medialis obilique, Vastus lateralis  相似文献

BACKGROUND AND PURPOSE: Reduced motor unit activity of the vastus medialis muscle relative to the vastus lateralis muscle has been implicated as a cause of lateral patellar subluxation. The purpose of this study was to assess the influence of vastus muscle motor unit activity on patellar kinematics. SUBJECTS: Twenty-three women (mean age=26.8 years, SD=8.5, range=14-46) with a diagnosis of patellofemoral pain and 12 women (mean age=29.1 years, SD=5.0, range=24-38) without patellofemoral pain participated. Only female subjects were studied because of potential biomechanical differences between sexes. METHODS: Patellar kinematics (kinematic magnetic resonance imaging) and vastus muscle electromyographic (EMG) activity using indwelling electrodes were measured during resisted knee extension. Measurements of medial and lateral patellar displacement and tilt obtained from magnetic resonance images were correlated with normalized vastus lateralis:vastus medialis oblique muscle and vastus lateralis:vastus medialis longus muscle EMG ratios at 45, 36, 27, 18, 9, and 0 degrees of knee flexion using a stepwise regression procedure. RESULTS: The vastus lateralis:vastus medialis longus muscle EMG ratio contributed to the prediction of lateral patellar glide at 27 degrees of knee flexion (r=-.48), with increased vastus medialis longus muscle activity being associated with greater lateral patellar displacement. A similar inverse relationship was evident with lateral patellar tilt at 36, 27, 18, and 9 degrees of knee flexion. CONCLUSION AND DISCUSSION: These results suggest that increased motor unit activity of the vastus medialis muscle appears to be associated with abnormal patellar kinematics in women, but it is not necessarily a cause of abnormal patellar kinematics.  相似文献

[Purpose] The purpose of this study was to determine the effect of ground tilt on the lower extremity muscle activity of stroke patients performing squat exercises. [Subjects] Fifteen hemiparetic patients volunteered to participate in this study. [Methods] The subjects performed squat exercises at three different ground tilt angles: 15° plantar flexion, a neutral position, and 15° dorsiflexion. A surface electromyogram (sEMG) was used to record the electromyographic activities of the leg extensor muscle in the vastus lateralis (VL), vastus medialis (VM), gastrocnemius lateralis (GL), and gastrocnemius medialis (GM). The sEMG activity was analyzed using a one-way repeated measures ANOVA and a post hoc Bonferroni correction. [Results] The results of this study are summarized as follows. Significant differences were noted for the VL and the GL when the angle of the ankle joint was between the 15° plantar flexion and neutral positions during squat exercises involving the VL and when the angle of the ankle joint was between the neutral position and 15° dorsiflexion during squat exercises involving the VM. [Conclusion] In this study, sEMG showed that the VL and GL changed significantly during squat exercises according to the ground tilt angle of hemiparetic patients. IntroductionThis study aimed to evaluate the effectiveness of proprioceptive neuromuscular facilitation (PNF) in promoting muscle irradiation to the lower limbs and to relate these findings with muscle activation during the sit-to-stand task.MethodsTwenty-four healthy women were recruited via convenience sampling and submitted to four PNF patterns: upper limb pattern with flexion-abduction-external rotation (UL), lower limb pattern with flexion-adduction-external rotation with knee flexion (lower limb pattern 1 [LL1]), lower limb pattern with flexion-abduction-internal rotation with knee flexion (lower limb pattern 2 [LL2]), and lifting to the right. Electromyography was recorded from the gluteus maximus, gluteus medius, vastus medialis, vastus lateralis, and soleus during the PNF patterns and during the sit-to-stand task.ResultsThe most recruited muscles during the sit-to-stand task were the vastus lateralis, vastus medialis, and soleus (p < 0.05). Lifting to the right induced the greatest activation of the gluteus maximus (p < 0.001). Lifting to the right and LL2 were better than UL at recruiting the gluteus medius (p < 0.05). Lifting to the right and LL1 were better than UL at recruiting the vastus medialis and lateralis (p < 0.05; p < 0.05). and Lower limb pattern 1 and 2 were better than UL at recruiting the soleus muscle (p < 0.05).ConclusionPNF can be used to induce muscle irradiation to maintain or improve muscle activity to the muscles recruited during the sit-to-stand task. The best PNF pattern for inducing muscle irradiation to muscles recruited during the sit-to-stand task are lifting to the right, LL2, and LL1.  相似文献

OBJECTIVE: This study examined the immediate effect of patellar taping with a standard force on the onset of vastus medialis obliquus and vastus lateralis activities before and after muscle fatigue in able-bodied subjects. DESIGN: This study tested 29 mature able-bodied subjects. The surface electromyographic onset time of their vastus medialis obliquus and vastus lateralis was measured after a posteroanterior perturbation at the knee in a single-legged standing position. The measurements were taken under three conditions in random order of true patellar taping, sham patellar taping, and no patellar taping. Afterward, subjects performed a knee-extension exercise until their quadriceps fatigued, and the above tests were repeated to test the effect of fatigue on the outcome. RESULTS: There was no statistically significant difference with patellar taping on the electromyographic onset time of the vastus medialis obliquus and vastus lateralis compared with the placebo-taping and no-taping conditions (P = 0.455). There was no statistically significant difference in onset of vastus medialis obliquus and vastus lateralis muscles before and after muscle fatigue (P = 0.304). CONCLUSIONS: The present study suggests that patellar taping does not enhance the temporal activation of vastus medialis obliquus in both fatigue and nonfatigue conditions on able-bodied subjects.  相似文献

OBJECTIVE: We measured the surface electromyographic activities of vastus medialis obliquus and vastus lateralis in 16 subjects with patellofemoral joint pain syndrome. DESIGN: Each subject performed bilateral static knee extension exercises at 60% of his or her maximal voluntary effort under different combinations of hip rotation (30 degrees of medial rotation, neutral, 45 degrees of lateral rotation) and knee flexion (20 and 40 degrees) in a standing position. The ratio of surface-integrated electromyographic signals of vastus medialis obliquus over vastus lateralis was calculated for each of the six conditions. Because of significant interaction of hip rotation and knee flexion in the two-way analysis of variance, data were analyzed separately with paired t tests for the effect of knee positions and one-way repeated measures analysis of variance for hip positions. RESULTS: At 20 degrees of knee flexion, there was no significant difference among the three hip positions, whereas at 40 degrees of knee flexion, medial rotation of the hip resulted in significantly higher vastus medialis obliquus over vastus lateralis activity ratio than lateral rotation (P < 0.05). CONCLUSIONS: There was relatively more activation of vastus medialis obliquus than vastus lateralis at 40 degrees of semisquat with the hip medially rotated by 30 degrees. This finding has clinical implications for training the vastus medialis obliquus in patients with patellofemoral joint pain syndrome.  相似文献

[Purpose] This study attempted to identify how genu varum or valgum affects the electromyographic activities of the vastus medialis, vastus lateralis, and rectus femoris during knee isometric contraction. [Subjects] Fifty-two healthy young adults were enrolled in this study. They were enrolled and classified into three groups by knee alignment conditions: the genu varum, genu valgum, and control groups. [Methods] The electromyographic activity ratio of the vastus medialis to the vastus lateralis and rectus femoris were calculated using the percentage of maximum voluntary contraction. The participants contracted their quadriceps during isometric contraction at 30 and 60° of knee flexion. [Results] The genu varum group had more activity in the vastus medialis than in the vastus lateralis and rectus femoris, whereas the genu valgum group had more activity in the vastus lateralis and rectus femoris than in the vastus medialis. There was a significant difference in the muscle activity ratio between the vastus medialis and vastus lateralis by angle of knee flexion degree only in the genu valgum. There were no significant differences in any of the three groups in terms of the muscle activity ratio of the vastus medialis to the rectus femoris by angle of knee flexion. [Conclusion] The quadriceps femoris was used for different strategies according to knee alignment during isometric contraction at 30 and 60°. This study suggests that rehabilitation training programs used to strengthen the quadriceps should consider the knee alignment conditions of the target subjects.Key words: Electromyography, Quadriceps muscle, Isometric contraction  相似文献

[Purpose] The purpose of this study was to compare the effects of visibility and types of ground surface (stable and unstable) during the performance of squats on the muscle activities of the vastus medialis oblique (VMO) and vastus lateralis (VL). [Subjects and Methods] The subjects were 25 healthy adults in their 20s. They performed squats under four conditions: stable ground surface (SGS) with vision-allowed; unstable ground surface (UGS) with vision-allowed; SGS with vision-blocked; and UGS with vision-blocked. The different conditions were performed on different days. Surface electromyogram (EMG) values were recorded. [Results] The most significant difference in the activity of the VMO and VL was observed when the subjects performed squats on the UGS, with their vision blocked. [Conclusion] For the selective activation of the VMO, performing squats on an UGS was effective, and it was more effective when subjects’ vision was blocked.Key words: Vision, Unstable, VMO  相似文献

[Purpose] The purpose of this study was to investigate the difference in onset timing between the vastus medialis and lateralis among the different knee alignments, as well as the best isokinetic angular velocity for an isokinetic concentric contraction. [Subjects] Fifty-two adults (20 with genu varum, 12 genu valgum, and 20 controls) were enrolled in this study. Subjects with > 4 cm between the medial epicondyles of the knee were placed in the genu varum group, whereas subjects with > 4 cm between the medial malleolus of the ankle were placed in the genu valgum group. [Methods] Surface electromyography was used to measure the onset times of the vastus medialis and vastus lateralis during concentric contractions at 30, 60, and 90°/sec. [Results] The vastus lateralis showed more delayed firing than the vastus medialis in the genu varum group, whereas vastus medialis firing was delayed more than that of the vastus lateralis in the genu valgum group. No differences in onset timing were observed between the vastus medialis and lateralis according the different angular velocities during concentric contractions in all three groups. [Conclusion] Genu varum and valgum affect quadriceps firing. OBJECTIVE: To use a musculoskeletal model and simulation of running to examine: (1) the influence of two commonly prescribed treatments for patellofemoral pain (vastus medialis oblique strengthening and orthoses) and (2) the functional significance of timing differences between vastus medialis oblique and vastus lateralis on lateral patellofemoral joint loads. DESIGN: A three-dimensional musculoskeletal model of the lower extremity was used to simulate running at 4 m/s. BACKGROUND: Repetitive and excessive joint loading is often associated with overuse injuries that require clinical treatments to reduce pain and restore function. Affecting one in four runners, patellofemoral pain is one of the most common injuries in running. Although conservative treatments have been reported to successfully treat patellofemoral pain, the effectiveness is often based on subjective or empirical data, which have generated disagreement on the most effective treatment. METHODS: Nine subject specific running simulations were generated and experiments were performed by applying the treatments and timing differences to the nominal simulations. RESULTS: Both treatments significantly reduced the average patellofemoral joint load and the vastus medialis strengthening also significantly reduced the peak patellofemoral joint load. In addition, when the vastus medialis oblique timing was delayed and advanced relative to the vastus lateralis timing, a significant increase and decrease in the joint load was observed, respectively, during the loading response.Conclusions. Increasing vastus medialis oblique strength yielded more consistent results across subjects than the orthosis in reducing patellofemoral joint loads during running. The effect of orthoses was highly variable and sensitive to the individual subject's running mechanics. Vastus medialis oblique activation timing is an important determinant of lateral patellofemoral joint loading during the impact phase. RELEVANCE: These findings indicate that a reduction in patellofemoral pain may be achieved through techniques that selectively increase the vastus medialis oblique strength. Therefore, future studies should be directed towards identifying such techniques. Additionally, the functional significance of timing differences between the vastus medialis oblique and vastus lateralis is an important consideration in patellofemoral pain treatment and orthoses may be beneficial for some patients depending on their running mechanics.  相似文献

OBJECTIVE: Knee osteoarthritis has a major impact on quadriceps function, yet its effect on the specific temporal recruitment of vastus medialis obliquus and vastus lateralis is unknown. The aim of this study was to determine the electromyographic onset of vastus medialis obliquus activity relative to that of vastus lateralis in individuals with symptomatic knee osteoarthritis and in an asymptomatic control group during the functional task of stair climbing. DESIGN: Cross-sectional, comprising 41 participants with symptomatic knee osteoarthritis and 33 controls matched for age, sex, and body mass. RESULTS: No significant differences were detected in the timing of onset of vastus medialis obliquus relative to that of vastus lateralis between the osteoarthritis and control groups. Radiographic osteoarthritis severity, presence of patellofemoral joint disease, and pain intensity did not seem to influence the temporal relationship of the vastii in the osteoarthritis group. CONCLUSION: The presence of symptomatic, radiographic knee osteoarthritis is not associated with deficits in the temporal recruitment of vastus medialis obliquus and vastus lateralis during stair climbing. This implies that selective retraining of the individual components of the quadriceps is not indicated in rehabilitation programs for patients with this disease.  相似文献

[Purpose] The purpose of this study was to investigate muscle activation related to postural stability depending on the pelvic position and frequency of whole body vibration (WBV) during quiet standing, and to identify the most effective training conditions that elicit the highest neuromuscular responses. [Subjects and Methods] Eighteen healthy subjects voluntarily participated in this single-group, repeated-measures study in which surface electromyography (EMG) data for the upper trapezius, rectus abdominis, external oblique abdominis, erector spinae, gluteus maximus, rectus femoris, semitendinosus, and medial gastrocnemius were collected at three frequencies (0 Hz, 10 Hz, and 20 Hz) of WBV and three pelvic positions (neutral, anterior tilt, posterior tilt) for each subject during quiet standing. [Results] The EMG activities of all the recorded muscles showed significant differences between the three frequencies of WBV and three pelvic positions during quiet standing. [Conclusion] The study findings suggest that a higher WBV frequency (20 Hz) should be used to strengthen most muscles, and that using the posterior pelvic tilt during WBV is much more effective at strengthening and training muscles related to core stability.Key words: Whole body vibration, Frequency, Pelvic tilt  相似文献

[Purpose] The purpose of this study was to identify the effects of performing squat exercises with visual feedback on the activation of the vastus medialis oblique (VMO) and vastus lateralis (VL) muscles in young adults with an increased quadriceps angle (Q-angle). [Subjects] This study used a motion analysis program (Dartfish, Switzerland) to select 20 young adults with an increased Q-angle, who were then divided into a squat group that received visual feedback (VSG, n=10) and a squat group that received no visual feedback (SG, n=10). [Methods] The intensity of exercises was increased every two weeks over a six-week exercise period in both groups. A visual marker was attached to the patella of the subjects in the VSG, and they then performed squat exercises with a maximum of 90° of knee flexion within a route marked on a mirror. The SG performed squat exercises with a maximum 90° of knee flexion without attaching a visual feedback device. [Results] Analysis of the muscle activation due to 90° squat exercises indicated that both groups had statistically significant increases in activation of the VL. The VSG exhibited statistically significant increases in activation of the VMO. [Conclusion] This study confirmed that squat exercises with visual feedback are effective in activation of the VMO and VL muscles.
