The short section of the urethra that passes through the urogenital diaphragm is termed the membranous urethra. It is accompanied by several vessels and nerve fibers. In surgery on the membranous urethra the perineal body is of crucial importance, and precise knowledge of the course of the urethra through the pelvic floor is essential. Furthermore, the relationship between the membranous urethra and its accompanying structures is of great importance. These include the branches of the internal pudendal artery, the prostatic venous plexus and the cavernous nerves coursing ventrally from the pelvic plexus. The nerves supplying the membranous urethra were studied in fetuses with the help of a magnifying lens. The results of the dissections are documented by histological sections, photographs and paintings.  相似文献

Posterior urethral injuries are not common and take a low priority in the management of patients with pelvic fracture injuries as these individuals almost always have multiple injuries of more serious consequence. Most patients are best treated by a suprapubic catheter for 3 months and then an end-to-end anastomotic urethroplasty in those who have developed urethral occlusions. There are roles for delayed primary repair and for endourological management in selected patients, but their exact roles have yet to be defined.  相似文献

The majority of strictures are treated by intermittent dilatations in order to achieve and maintain an adequate passage through the urethra. Some strictures are resistant to this treatment and call for a more radical therapy. Numerous surgical procedures have been designed with the goal of finding a method which would definitely remove urethral strictures. Strictures of the posterior urethra pose particularly serious problems. In this paper a surgical procedure is described which corrects strictures of the posterior urethra, based on marsupialization of the stricture by an inlay graft of the scrotal skin and on recontsruction of the urethra in a second stage. The author evaluates his own results in 22 patients and shows the adequacy of a radical solution in patients who are otherwise entirely dependent on life-long treatment by intermittent dilatations or on continuous derivation by cystostomy.  相似文献
