目的总结成人活体肝移植后再次肝移植的体会。方法6例曾经接受活体肝移植者因胆道并发症(2例)、血管并发症(2例)、慢性排斥反应(1例)和肝炎复发(1例)而接受再次肝移植,再次肝移植均采用改良背驮式原位肝移植术。除1例将供肝动脉改为与受者腹主动脉吻合外,其余均为同名血管的端端吻合。结果1例术后因原发性移植肝无功能死亡;1例肝动脉吻合口狭窄,经介入治疗放置支架后缓解。随访至今,5例已分别存活12、9、6、4和3个月,目前肝功能良好。结论腹腔内粘连是成人活体肝移植后再次肝移植的手术难点。  相似文献

目的研究并改进成人间活体供肝移植的手术技术。方法自2002年1月至2005年8月,施行了16例成人间活体右半供肝移植。手术中改进了技术,包括右肝静脉重建、肝中静脉分支搭桥、肝动脉搭桥及胆道吻合等。结果所有供者均无严重并发症及死亡。移植肝与受者重量比(GRWR)为0.72%~1.24%,其中9例〈1.0%,2例〈0.8%。手术除了采用移植肝的右肝静脉与受者下腔静脉(IVC)直接吻合外,5例加行右肝下静脉重建、5例取自体大隐静脉行肝中静脉分支与IVC间搭桥,保证了右肝流出道通畅。最早手术的2例受者中,1例发生肝静脉吻合口狭窄,另1例发生小肝综合征,最终导致死亡。后阶段手术的14例受者均未发生小肝综合征;发生并发症5例,分别为急性排斥反应、肝动脉栓塞、胆漏、左膈下脓肿及肺部感染;1例再次肝移植后因肺部感染,多器官功能衰竭(MOF)死亡。结论活体供肝移植中采用改进的手术技术,特别是肝静脉流出道重建的方法,可有效避免发生小肝综合征。  相似文献

目的总结再次肝移植的临床经验,提高再次肝移植患者的存活率。方法14例再次肝移植患者,其再次肝移植的原因分别为缺血性胆道并发症(9例)、急性肝动脉血栓形成(2例)、慢性排斥反应(2例)及重度药物性肝损害(1例)。再次肝移植均采用改良背驮式,供、受者的门静脉均采用端端吻合;8例供、受者的肝动脉仍行端端吻合,6例供肝动脉经搭桥与受者的腹主动脉吻合;2例的供肝胆管与受者的空肠行Roux-en-Y吻合,12例的胆管行端端吻合。结果14例患者首次和再次肝移植术前终末期肝病模型评分分别为16.4±7.2和22.5±7.9(P〈0.05),手术耗时分别为(553.3±85.4)min和(796.4±125.2)min(P〈0.05),无肝期时间分别为(63.4±29.6)min和(102±32.8)min(P〈0.05),术中出血量分别为(1537.3±348.3)ml和(2589.3±429.7)ml(P〈0.05)。14例中,死亡4例,均为MELD评分≥25分者。结论术前科学评估患者的病情、把握手术时机、提高手术技巧及防治术后感染是提高再次肝移植成功率的关键。  相似文献

目的探讨应用供者髂动脉行腹主动脉搭桥重建移植肝动脉对肝移植受者预后的影响。 方法回顾性分析中国医科大学附属第一医院2006年1月至2018年4月应用供者髂动脉行腹主动脉搭桥重建肝动脉的肝移植受者临床资料,观察其术后肝功能恢复情况及肝动脉血栓等并发症的发生情况,分析采用搭桥方式进行移植肝动脉重建的原因。 结果共纳入8例受者,其中1例存在脾动脉盗血综合征导致肝总动脉供血不足,3例肝总动脉纤细,4例肝总动脉壁薄弱或分层。重建后肝动脉平均血流为(315±178)mL/min。术后2例受者分别因肝脏流出道和胆管吻合口狭窄导致黄疸,其余受者移植肝功能恢复良好。1例受者术后2个月出现肝动脉血栓形成,继发肝脓肿,半年后因多脏器功能衰竭死亡。其余7例受者随访至2018年11月均存活,肝动脉均通畅,无狭窄或血栓形成。 结论当供、受者常规肝动脉端端吻合无法实施时,应用供者髂动脉行腹主动脉搭桥重建移植肝动脉是一种可行的肝动脉重建方法。  相似文献

合并门静脉血栓形成患者的肝移植21例   总被引:3,自引:2,他引:1  
目的探讨肝移植术中门静脉血栓的处理方法,评价移植术前门静脉血栓形成对肝移植疗效的影响。方法回顾性分析267例270次肝移植的临床资料。267例受者中,术中明确存在门静脉血栓者21例,占7.8%,其中12例术前通过彩色多普勒超声、计算机断层扫描血管造影或磁共振成像明确诊断。按照Yerdel门静脉血栓分级法,1级8例,2级5例,3级6例,4级2例。肝移植术中根据门静脉血栓分级的不同,采取相应处理方式:1级和2级者选择单纯血栓切除或血栓累及段门静脉切除,然后将供、受者的相应血管行端端吻合;3级和4级者采取供者肠系膜上静脉或髂静脉在供肝门静脉与受者肠系膜上静脉或门静脉系统属支间架桥等方式重建供肝门静脉循环。结果21例术前存在门静脉血栓形成的患者均顺利完成肝移植手术。4例Yerdel分级为3级的受者术后早期(8~21d)死亡,死亡率为19.0%,显著高于术前无门静脉血栓者(8.5%,P(0.01)。21例受者中,1例术后3个月时再次发生门静脉血栓形成,再次血栓形成发生率为4,8%,显著高于术前无门静脉血栓形成者(0.8%,P〈0.01)。17例术前存在门静脉血栓形成的患者肝移植后康复出院,其1年存活率为94.1%,与术前无门静脉血栓形成的良性肝病受者(93.8%)比较,差异无统计学意义(P〉0.05)。结论术前存在的门静脉血栓形成并非肝移植的绝对禁忌证,术中根据门静脉血栓分级选择适宜的手术处理方式可顺利完成肝移植手术,并取得与无门静脉血栓形成者相近的远期疗效。  相似文献

目的 探讨成人间活体肝移植的肝动脉重建和并发症处理的经验.方法 自2002年1月至2006年7月,共施行50例成人间右半肝活体肝移植.在供受者间肝动脉的重建中,供者右肝动脉与受者右肝动脉吻合24例,与受者肝固有动脉吻合12例,与受者左肝动脉吻合3例,与受者肝总动脉吻合2例,与受者肠系膜上动脉发出的副右肝动脉吻合2例,与受者肝总动脉自体大隐静脉间置搭桥3例.受者腹主动脉与供者右肝动脉自体大隐静脉搭桥2例,用保存的尸体髂血管行受者腹主动脉与供者右肝动脉搭桥2例.供者肝动脉直径1.5~2.5 mm,采用显微外科技术在3.5倍手术放大镜和5~10倍手术显微镜下完成肝动脉重建.结果 50例成人间右半肝活体肝移植中,有2例(4%)分别于术后1d、7d发生肝动脉血栓形成,立即采用自体大隐静脉从肾下腹主动脉至供者右肝动脉搭桥术,恢复供肝血流,痊愈出院.1例术后1.5个月后发生肝动脉血栓形成,随访期无临床症状未行处理.术后和随访期未发现肝动脉狭窄、肝动脉假性动脉瘤等并发症.围手术期未有与肝动脉并发症有关的死亡病例.全部病例获得随访,随访时间2~52个月(中位随访时间9个月),1年实际生存率为92%.结论 选择恰当的肝动脉重建方式和应用显微外科技术可显著降低肝动脉并发症,及时处理肝动脉并发症是保证供肝存活的关键.  相似文献

目的探讨成人间右半肝移植手术中保证供、受者安全的方法。方法2002年1月至2006年9月四川大学华西医院对56例受者施行了成人右半肝移植,其中52例不含肝中静脉,4例双供肝肝移植。受者原发病为乙型肝炎肝硬化35例(62.5%,含急性肝功能衰竭12例),肝细胞肝癌17例(30.4%),其他4例;MELD评分〉25分者10例。供者常规行三维CT计算全肝体积及右半肝体积,并进行了移植手术技术改进。结果58例供者共摘取55例右半供肝及3例左半供肝。右半供肝均不含肝中静脉,重量为400—860g(中位数550g),右半供肝与受者标准肝重比为31.7%-71.7%(中位数45.4%),供者残肝体积均大于全肝体积的35%。58例供者发生并发症7例(12.5%),无死亡。术后住院时间7—30d(中位数11d)。术后对56例受者随访2—52个月(中位数11个月),发生并发症15例(26.8%),死亡4例(7.2%)。1年实际生存率92.8%。结论采用不包含肝中静脉的右半供肝,术前CT测量残肝体积〉35%,同时右半供肝与受者标准肝重比〉40%者进行右半肝移植可保证供受者安全,反之则应考虑采用双供肝肝移植。  相似文献

目的 总结再次肝移植治疗移植肝失功能的临床经验。方法 回顾分析2004年1月至2006年6月期间中山大学附属第三医院施行22例再次肝移植受者的临床资料,结合文献加以讨论。再次肝移植的原因分别为移植术后胆道并发症(12例)、移植术后肝癌复发(4例)、肝动脉栓塞(2例)、肝动脉狭窄(2例)以及乙肝复发(2例)。再次移植率为3.62%,供肝植入均采用改良背驮式肝移植技术。结果 全组无手术死亡,8例随访至今分别存活21、14、8、3个月各1例,12、1个月各2例;14例存活2周到28个月不等。首次肝移植术后8~30d行再次肝移植病人围手术期病死率最高,为66.7%;1年内死亡10例,主要死亡原因为感染(60%)。结论 再次肝移植是移植肝失功能的惟一有效的治疗方法,正确掌握手术时机及适应证,钻研手术技巧,合理的个体化免疫抑制方案以及围手术期有效的抗感染治疗是提高再次肝移植病人存活率的关键。  相似文献

目的 探讨再次肝移植的手术技巧及其临床效果.方法 回顾性分析31例患者接受32次再次肝移植手术的临床资料,手术方式均采用附加腔静脉整形的改良背驮式原位肝移植,其中11例采用了股静脉-颈内静脉转流术.肝动脉的重建采用供肝动脉通过供者髂动脉间置搭桥与受者腹主动脉行端侧吻合24例次,采用供肝动脉与受者肝固有动脉行端端吻合8例次.胆道的重建采用胆管-空肠Roux-en-Y吻合28例次,采用胆道端端吻合4例次.术后常规使用抗排斥反应和抗感染治疗,并对患者进行了长期随访.结果 术后死亡17例,死亡时间为术后2周~28个月,死亡原因为术后严重感染8例、多器官功能衰竭和肝癌复发各3例、血管并发症和心肌梗塞以及颅内出血各1例,其中首次肝移植术后8~30 d行再次肝移植者围手术期死亡率最高,为66.7%.其余14例均痊愈出院,随访至今已存活1~29个月,肝功能及生活质量良好.再次肝移植与首次肝移植的手术时间及术中出血量比较,差异无统计学意义.结论 附加腔静脉整形的改良背驮式肝移植是再次肝移植的最佳术式,正确掌握手术时机,并针对患者进行个体化的处理是手术成功的关键.与首次肝移植相比,再次肝移植面临着较高的并发症发牛率和死亡率.  相似文献

目的构建一个稳定的重建肝动脉血液供应和胆道外引流的大鼠原位肝移植模型,为肝移植术后胆道问题的研究提供一个更加符合正常生理的动物实验平台。方法在经典的“双袖套”法的基础上,将与肝动脉相连的供者主动脉直接与受者的腹主动脉行端侧吻合,重建肝动脉血液供应,同时横断受者的胆总管,行胆道外引流和空肠造瘘,重建胆汁回流通路,以维持肠一肝循环。结果共施行改良的大鼠原位肝移植31次,成功28次,手术成功率为90.32%,术后受者24h存活率为80.65%(25/31),术后1周存活率为74.19%(23/31),术后1个月存活率为70.97%(22/31)。结论用带肝动脉的供者主动脉直接与受者的腹主动脉行端侧吻合重建肝动脉血液供应可靠、易行,通过建立体外胆汁回流通路不仅能有效维持肠一肝循环,而且便于胆汁采集。该模型易于标准化,是研究肝移植术后胆道问题的理想动物模型。  相似文献

Arterial complications after orthotopic liver transplantation (OLT), including hepatic artery thrombosis (HAT), are important causes of early graft failure. The use of an arterial conduit is an accepted alternative to the utilisation of native recipient hepatic artery for specific indications. This study aims to determine the efficacy of arterial conduits and the outcome in OLT. We retrospectively reviewed 1,575 cadaveric adult OLTs and identified those in which an arterial conduit was used for hepatic revascularisation. Data on the primary disease, indication for using arterial conduit, type of vascular graft, operative technique and outcome were obtained. Thirty-six (2.3%) patients underwent OLT in which arterial conduits were used for hepatic artery (HA) revascularisation. Six of these were performed on the primary transplant, while the rest (n=30) were performed in patients undergoing re-transplantation, including six who had developed hepatic artery aneurysms. The incidence of arterial conduits was 0.4% (6/1,426 cases) in all primary OLTs and 20.1% (30/149 cases) in all re-transplants. Twenty-nine procedures utilised iliac artery grafts from the same donor as the liver, six used iliac artery grafts from a different donor, and a single patient underwent a polytetrafluoroethylene (PTFE) graft. Two techniques were used: infra-renal aorto-hepatic artery conduit and interposition between the donor and recipient native HAs, or branches of the HAs. The 30-day mortality rate for operations using an arterial conduit was 30.6%. Three conduits thrombosed at 9, 25 and 155 months, respectively, but one liver graft survived without re-transplantation. The arterial conduits had 1- and 5-year patency rates of 88.5% and 80.8%. The 1- and 5-year patient survival rates were 66.7% and 44%. We can thus conclude that an arterial conduit is a viable alternative option for hepatic revascularisation in both primary and re-transplantation. Despite a lower patency rate than that of native HA in the primary OLT group, the outcomes of arterial conduit patency and patient survival rates are both acceptable at 1 and 5 years, especially in the much larger re-OLT group.  相似文献

BACKGROUND: Vascular conduits may be required to gain arterial inflow to the donor hepatic artery in orthotopic liver transplantation. METHODS: From January 1986 to December 2003, arterial conduits were required in 31/582 (5.3%) adult liver transplant procedures. RESULTS: Indications for the conduit included recipient hepatic artery problems (20); hepatic artery thrombosis previous allograft (7) and other (4). The conduits used in 28/31 cases (90%) were deceased donor iliac arteries and the remainder prosthetic grafts. Patients requiring conduits were more likely to be already hospitalized (P = 0.038) or undergoing a retransplant procedure (P = 0.001) than patients not requiring conduits. Both sepsis and haemorrhage caused death in 8/31 (26%) patients requiring conduits versus 42/551 (7.6%) patients not requiring conduits. Death from thrombosis of the iliac artery conduit occurred in two cases and from bacterial infection of a prosthetic conduit in one case. For retransplant procedures, allograft loss was seen in 11/13 (84%) conduit cases versus 11/28 (39%) non-conduit cases (P = 0.016). Overall allograft survival was significantly lower in the conduit cases than in the non-conduit cases (P = 0.0001), with 12/31 (39%) allografts being lost within the first 3 months post-transplantation for the conduit cases. CONCLUSION: Arterial vascular conduits are more commonly required in adult liver transplant recipients who are hospitalized or undergoing retransplant procedures. Allograft survival is poorer in the conduit cases and is associated with complications, particularly sepsis and haemorrhage, following retransplantation procedures.  相似文献

BACKGROUND: In case of anomal hepatic arterial inflow, it can be necessary to perform revascularization of the liver allograft by iliac arterial interposition graft. METHODS: We analyzed retrospectively 613 liver transplants in a 16-yr period. The hepatic artery (HA) graft group (n = 101) consisted of patients with arterial inflow based on recipient infrarenal aorta using donor iliac artery graft tunneled through the transverse mesocolon. The control group (n = 512) consisted of patients who underwent liver transplantation with routine HA reconstruction. RESULTS: Both groups are homogeneous and comparable. In case of retransplantation, arterial conduit with iliac graft was adopted more frequently instead of conventional arterial anastomosis (24.8% vs. 9%, p < 0.0001). The 1-, 3- and 5-yr overall survival was 85.41, 79.42, 76.57% in the control group and 76.21, 73.43, 73.43% in the HA graft group, respectively (p = ns). The 1-, 3- and 5-yr graft survival was better in the control group (81.51, 73.66, 69.22% vs. 71.17, 62.50, 53.42%) (p = 0.01). In case of retransplantation, the 1-, 3- and 5-yr overall (57.81, 53.95, 41.96% vs. 60, 51.95, 49.85%) and graft survival (57.52, 53.68, 41.75% vs. 56, 50.4, 40.3%) was similar in control and HA graft group, respectively (p = ns). Hepatic artery thrombosis (HAT) rate is 21.8% vs. 8.6% (p < 0.0001) in HA graft group and control group, respectively. The only factor independently predictive of early HAT resulted arterial conduit (p = 0.001, OR = 3.13, 95% CI: 1.57-6.21). Retransplant procedure, donor age and arterial iliac conduit were found to be a significant risk factors for late HAT, at univariate analysis. At multivariate analysis, donor age >50 yr old resulted the only factor independently associated with late HAT (p < 0.0001, OR = 1.05, 95% CI: 1.02-1.07). CONCLUSION: Iliac arterial interpositional graft is an alternative solution for arterial revascularization of liver allograft in case of retransplantation when the use of HA is not possible. In case of primary transplantation, is better not to perform arterial conduit if it is possible, for poor graft survival and high incidence of early HAT, especially in case of liver donor aged over 50 yr.  相似文献

Hepatic artery complications after orthotopic liver transplantation are associated with a high rate of graft loss and mortality (23% to 35%) because they can lead to liver ischemia. The reported incidence of hepatic artery thrombosis (HAT) after adult liver transplantation is 2.5% to 6.8%. Typically, these patients are treated with urgent surgical revascularization or emergent liver retransplantation. Since January 2007, we have recorded the postanastomotic hepatic artery flow after revascularization. The aim of this study was to assess the relationship between hepatic blood flow on revascularization and early HAT. Retrospectively, we reviewed perioperative variables from 110 consecutive liver transplantation performed at the Virgen del Rocío University Hospital (Seville, Spain) between January 2007 and October 2010. We evaluated the following preoperative (donor and recipient) and intraoperative variables: donor and recipient age, cytomegalovirus serology, ABO-compatibility, anatomical variations of the donor hepatic artery, number of arterial anastomoses, portal and hepatic artery flow before closure, cold ischemia time, and blood transfusion. These variables were included in a univariate analysis. Of the 110 patients included in the study, 85 (77.7%) were male. The median age was 52 years. ABO blood groups were identical between donor and recipient in all the patients. The prevalence of early HAT was 6.36% (7 of 110). Crude mortality with/without HAT was 22% versus 2% (P = .001), respectively. Crude graft loss rate with/without HAT was 27% versus 4% (P = .003), respectively. Early HAT was shown to be primarily associated with intraoperative hepatic artery blood flow (93.3 mL/min recipients with HAT versus 187.7 mL/min recipients without HAT, P < .0001). No retransplantation showed early HAT. In our experience, intraoperative hepatic artery blood flow predicts early HAT after liver transplantation.  相似文献

BackgroundHepatic artery thrombosis (HAT), a serious complication after orthotopic liver transplantation, almost always leads to morbidity and mortality without urgent revascularization or retransplantation, especially if HAT occurs within a few days after transplantation.Case PresentationHerein we describe a case report of an orthotopic liver transplantation patient surviving without hepatic artery flow due to HAT on postoperative day 1. Reanastomosis, thrombectomy, and intra-arterial thrombolysis were performed, but only retrograde arterial flow by Doppler ultrasound, not by angiography, could be demonstrated in the hepatic artery. This case report is in compliance with the Declaration of Helsinki and the Declaration of Istanbul.ConclusionBased on the evidence from this patient, we believe that patients with failed revascularization can experience a long-term survival with conservative treatment. Retransplantation should be evaluated based on laboratory findings because graft function in individual patients can recover.  相似文献

Early hepatic artery thrombosis (HAT) after orthotopic liver transplantation remains a significant cause of graft loss and patient death. The most effective treatment approach is still controversial. The purpose of this study was to assess the effect of continuous transcatheter arterial thrombolysis in the treatment of early HAT. Routine posttransplant color Doppler imaging (CDI) was performed to monitor hepatic artery blood flow. HAT was confirmed by arterial angiography in suspected cases. HAT was identified in 8 patients (8/287, 2.8%) which occurred on days 2 to 19 (mean, 5.2 days) after liver transplantation. Patients with HAT were treated with continuous transcatheter arterial thrombolysis using urokinase. Successful revascularization through thrombolysis was obtained in all eight cases. One patient died of a pulmonary infection at 2 months after liver transplantation. Another patient underwent retransplantation because of resistant allograft rejection and recurrence of HAT 6 months after the first operation, but died from multiple system organ failure 2 months later. The other six patients remained in good health during the follow-up period of 3 to 27 months. Our results demonstrate that CDI is an effective method to monitor the occurrence of early HAT after liver transplantation. Furthermore, continuous transcatheter arterial thrombolysis with urokinase could be a rational therapeutic approach to rescue the allograft following early HAT diagnosis confirmed by arterial angiography.  相似文献

目的 探讨彩色多普勒超声 (CDI)监测肝移植术后肝动脉并发症的应用价值。方法 术后连续CDI检查监测 180次原位肝移植。监测指标包括肝门部及肝内肝动脉左、右分支的峰值速度 (HAV) ,加速度 (HAAC) ,加速时间 (SAT) ,阻力指数 (RI) ,观察有无血流信号中断、侧支循环形成、肝内有无梗死灶和肝内、外胆管改变等。结果  8例病人经选择性动脉造影证实为动脉并发症 (血栓形成 5例 ,肝动脉狭窄 3例 )。CDI表现有 :RI降低 <0 5 (8/ 8) ,SAT延长 >0 0 8s(6 / 8) ,HAAC降低<30 0cm/s2 (6 / 8) ,HAV降低 <4 0cm/s(7/ 8) ,肝内胆管扩张、回声改变等 (4 / 8) ,肝内梗死灶 (2 / 8) ,肝内外动脉血流信号消失 (2 / 8) ,肝门部侧支循环形成 (1/ 8)。CDI对动脉并发症诊断的敏感度和特异度分别为 87 5 % (7/ 8)和 95 3% (16 4 / 172 )。结论 CDI可有效监测肝移植术后肝动脉并发症并对其治疗有一定的指导作用。RI、SAT、HAAC、HAV是CDI诊断肝动脉并发症的敏感指标 ,联合应用可以提高CDI的诊断特异度。  相似文献

目的 探讨成人肝移植术后肝动脉血栓形成(hepatic artery thrombosis,HAT)的诊断与治疗,及其对患者预后的影响.方法 2007年6月至2010年10月我中心共实施成人尸体肝脏移植387例.术后采用床边彩色多普勒超声监测移植肝血流.疑有肝动脉血栓形成时,采用超声造影或肝动脉造影明确诊断,根据病情采用介入溶栓治疗、手术再血管化治疗及再次肝移植等治疗.结果 387例中术后共有10例患者发生HAT,发生率2.6%.发生HAT的中位时间为肝移植术后7(范围2~18)d.2例采用介入溶栓治疗,其中1例伴肝动脉狭窄放置支架,均痊愈;3例再次手术行肝动脉重建联合肝动脉局部溶栓治疗,其中1例术后再次出现HAT,死亡;2例行再次肝移植,痊愈;3例出现肝内脓肿,严重感染,肝功能恶化死亡.死亡率为40%(4/10).结论 肝移植术后常规彩色多普勒超声监测肝动脉血流是早期发现HAT的关键,超声造影及肝动脉造影可明确诊断;及时采用介入溶栓、手术再血管化及再次肝移植等治疗虽然可减少患者死亡,但预防HAT发生更为重要.  相似文献

目的探讨原位肝移植术后肝动脉血栓形成的诊断与治疗。方法回顾性分析我科1996年10月至2004年3月施行的98例次原位肝移植术.其中8例原发性肝癌患者术前有多次TACE史。结果本组病例仅术前有多次TACE史的患者发生肝动脉血栓5例,均经选择性血管造影证实。3例、1例和1例受者分别接受介入溶栓、再血管化手术和再次肝移植。与肝动脉栓塞有关的死亡率为60%(3/5)。结论术前多次TAcE史是移植术后发生肝动脉血栓的高危因素。彩色多普勒超声是监测血管并发症的首选方法。再血管化手术或再次肝移植是治疗肝动脉血栓的有效方案。  相似文献

Early hepatic artery thrombosis (HAT) after pediatric orthotopic liver transplantation (OLT) can cause significant morbidity and mortality, leading to liver failure or septic complications requiring urgent retransplantation. Experimental evidence that hyperbaric oxygen (HBO) may ameliorate hepatic ischemic-reperfusion injury led to this study of HBO in pediatric liver transplant recipients who developed HAT. Children undergoing OLT under primary tacrolimus immunosuppression and University of Wisconsin organ preservation between August 1, 1989, and December 31, 1998, who developed HAT were the basis for this study. Patients who developed HAT between March 1, 1994, and December 31, 1998, were treated with HBO therapy until signs of ischemia resolved (absence of fever, normalizing liver injury test results) or for 2 weeks. The pediatric OLTs performed from August 1, 1989, to February 28, 1994, who developed HAT served as a control group. Primary outcome measures were survival, retransplantation rate, time to retransplantation, incidence of hepatic gangrene, and days to collateral formation. Three hundred seventy-five consecutive pediatric patients underwent 416 OLTs between August 1, 1989, and December 31, 1998. Thirty-one patients (7.5%) developed HAT at a mean time of 8.2 days (range, 1 to 52 days) post-OLT. In 17 patients, HBO treatment was begun within 24 hours of HAT or immediately after the revascularization attempt and performed twice daily for 90 minutes at 2.4 atmospheres pressure. Fourteen patients were treated without HBO. None of the HBO-treated patients developed hepatic gangrene. Eight HBO patients (47%) were bridged to retransplantation at a mean time of 157 days (range, 3 to 952 days) after initial OLT and all survived. Mean time to retransplant in the control group was 12.7 days (range, 1 to 64 days). HBO was well tolerated without significant complications. Although there was no significant difference in survival or retransplantation rates, HBO significantly delayed retransplantation, potentially by hastening the development of hepatic artery collaterals.  相似文献
