目的 对75例肝硬变患血清透明质酸(HA)及T3、T4的联合检测,探讨其对肝硬变诊断及预后判断的价值。方法 采用放射免疫方法检测肝硬变血清HA及T3、T4。结果 Child B、C级组HA升高与T3、FT3降低均较ChildA级组有显性差异,且HA的升高与T3、FT3的降低有相关性(P〈0.01)。结论 随肝功能损害程度加重,血清HA升高及T3、FT3降低明显,联合检测HA及T3、FT3对肝硬  相似文献

Zusammenfassung Tödliche basale Subarachnoidalblutungen nach Gewalteinwirkungen gegen den Kopf bereiten bei der Begutachtung erhebliche Schwierigkeiten. Wie sich aus der Kasuistik ergibt, dürften Gewalteinwirkungen gegen das Gesicht und den Hinterkopf, d. h. in einer Ebene mit der Schädelbasis, besonders gefährlich sein. Dies läßt sich vielleicht auch aus dem Ergebnis von Tierversuchen herleiten. Die Begutachtung kann sich nicht nur auf die Sektionsbefunde stützen, sondern auch auf die Begleitumstände. Eine wesentliche Bedeutung für das Zustandekommen der Blutung dürfte der Alkoholgenuß bzw. -einfluß haben.Die Besonderheiten bei der Beurteilung der Fälle nach dem schwedischen Strafrecht werden besprochen.  相似文献

Overtraining syndrome (OTS) occurs where an athlete is training vigorously, yet performance deteriorates. One sign of OTS is suppressed immune function, with an increased incidence of upper respiratory tract infection (URTI). An increased incidence of URTIs is also associated with high volume/intensity training, as well as with excessive exercise (EE), such as a marathon, manifesting between 3-72 hours post-race. Presently, there is no encompassing theory to explain EE and altered immune competence. Recently, it has been conclusively established that T helper lymphocytes (T(H)), a crucial aspect of immune function, represent two distinct functional subsets: T(H)1 and T(H)2 lymphocytes. T(H)1 lymphocytes are associated with cell-mediated immunity (CMI) and the killing of intracellular pathogens, while T(H)2 lymphocytes are associated with humoral immunity and antibody production. When T(H)-precursor cells are activated, the balance is tipped in favour of one or the other. Furthermore, the most appropriate means of determining the T(H)-subset, is by the prevailing cytokine 'pattern'. This paper hypothesises that exercise-related immunosuppression is due to tissue trauma sustained during intense exercise, producing cytokines, which drive the development of a T(H)2 lymphocyte profile. A T(H)2 cell response results in simultaneous suppression of CMI, rendering the athlete susceptible to infection. Additionally, increased levels of circulating stress hormones (cortisol and catecholamines), as well as prostaglandin E(2), support up-regulation of T(H)2 lymphocytes. Marathon-related data are presented to support this hypothesis. It is concluded that an increased incidence of illness associated with OTS and in response to EE is not due to immunosuppression per se, but rather to an altered focus of immune function, with an up-regulation of humoral immunity and suppression of CMI.  相似文献

Purpose

To prospectively evaluate four non-invasive markers of cartilage quality—T2* mapping, native T1 mapping, dGEMRIC and ΔR1—in healthy volunteers and rheumatoid arthritis (RA) patients.

Materials and methods

Cartilage of metacarpophalangeal (MCP) joints II were imaged in 28 consecutive subjects: 12 healthy volunteers [9 women, mean (SD) age 52.67 (9.75) years, range 30–66] and 16 RA patients with MCP II involvement [12 women, mean (SD) age 58.06 (12.88) years, range 35–76]. Sagittal T2* mapping was performed with a multi-echo gradient-echo on a 3?T MRI scanner. For T1 mapping the dual flip angle method was applied prior to native T1 mapping and 40?min after gadolinium application (delayed gadolinium-enhanced MRI of cartilage, dGEMRIC, T1Gd). The difference in the longitudinal relaxation rate induced by gadolinium (ΔR1) was calculated. The area under the receiver operating characteristic curve (AROC) was used to test for differentiation of RA patients from healthy volunteers.

Results

dGEMRIC (AUC 0.81) and ΔR1 (AUC 0.75) significantly differentiated RA patients from controls. T2* mapping (AUC 0.66) and native T1 mapping (AUC 0.66) were not significantly different in RA patients compared to controls.

Conclusions

The data support the use of dGEMRIC for the assessment of MCP joint cartilage quality in RA. T2* and native T1 mapping are of low diagnostic value. Pre-contrast T1 mapping for the calculation of ΔR1 does not increase the diagnostic value of dGEMRIC.  相似文献

目的 研究Ki-67蛋白表达与胃癌生物学特性的相关性。方法 利用免疫组化方法对12例Ti期胃癌,20例T2期胃癌组织的Ki-67蛋白表达率进行测定,并分别在有淋巴结转移组和无淋巴结转移组之间进行对比研究,结果Ki-67蛋白表达率与胃癌的淋巴结转移,浸润深度及生长方式明显相关。结论Ki-67蛋白可作为判断胃癌淋巴结转移情况的分子生物学指标。  相似文献

Purpose. The HIRE sequence utilizes the very long T2 value of CSF to suppress its high signal contribution in T2-weighted imaging by an image subtraction technique. Methods. To assess the diagnostic potential of a new dark fluid sequence HIRE (High Intensity REduction) in the diagnostic work-up, 20 patients with histologically confirmed cerebral gliomas were examined with T2-weighted FSE, T1-weighted SE, fast FLAIR and HIRE using identical scan parameters. In patients with enhancing lesions fast FLAIR and HIRE were added to the contrast-enhanced T1-weighted SE images. Images were analyzed in a qualitative and quantitative evaluation. In the qualitative lesion analysis, lesion delineation and differentiation between enhancing and non enhancing tumor tissue were by two readers. For the quantitative analysis lesion-to-background and lesion-to-CSF contrast and contrast to noise ratios were determined in an region of interest analysis. Results. HIRE achieved a significant reduction of the CSF signal without loosing the high gray-to-white matter contrast of T2 weighted sequences. In the quantitative analysis, the contrast ratios of the HIRE were lower compared to the FLAIR images due to a relative high background and CSF signal. After the application of contrast media HIRE images revealed a significant signal increase in enhancing lesions, which subsequently increased the contrast and contrast-to-noise ratios. In the qualitative analysis, both readers found all tumors clearly delineated on HIRE imaging. Compared to T2-weighted FSE the tumor delineation with HIRE was better in nine patients, equal in four patients and less in one patient. Compared to the FLAIR images HIRE was rated superior in three patients, equal in nine patients and inferior in another three patients. Delineation of the enhancing tumor parts was possible with HIRE in all patients. HIRE images present significant less image artifacts than FLAIR images due to reduced inflow effects. Conclusions. The presented T2 based HIRE sequence is an alternative to the T1 based FLAIR sequence with the advantage of a better gray to white matter contrast and shorter measurement time. Due to the subtraction technique signal intensities from tissues with T2 relaxation times in the range between white matter and CSF are also partially affected depending on their T2 values. With respect to this undesired effect, an improvement in HIRE imaging will be expected by a self-weighted subtraction algorithm.  相似文献
