寻常型天疱疮虱一级亲属循环天疱疮抗体的检测分析

目的 探讨与寻常型天疱疮（ｐｅｍｐｈｉｇｕｓ ｖｕｌｇａｒｉｓ,ＰＶ）患者可能共享易感基因的亲属中ＰＶ抗体的存在情况。方法 对１２例ＰＶ患者的３０名一级亲属及１０名正常对照者同时应用免疫印迹及间接免疫荧光（ｉｎｄｉｒｅｃｔ ｉｍｍｕｎｏｆｌｕｏｒｅｓｃｅｎｃｅ,ＩＩＦ）方法,检测其外周血中循环ＰＶ抗体。结果 在３０名亲属中有１９名（６３％）同时用两种方法检出了ＰＶ抗体,抗体滴度〉１：１６,仅有９名     

A rare presentation of hydatid cyst

Hydatid disease, although known to occur in most body areas, is extremely rare in the female reproductive system. There are different modes of presentation for the disease; however, we report and discuss a case presented with cystic vesicles passing through the vagina, which is considered as a rare presentation for secondary involvement of the uterus and both ovaries. We confirmed diagnosis with radiological examinations and serological tests. We operated on the patient, and studied the excised cysts microscopically. The gynecologist should be aware of hydatid cyst when vaginally passing a grape like vesicle is presented by the patient.     

65例寻常型天疱疮的护理体会

目的探讨寻常型天疱疮的护理体会。方法对65例寻常型天疱疮应用消毒隔离、皮肤护理、黏膜护理、饮食护理、药物护理及心理护理等护理措施及方法,观察其临床治疗效果。结果应用上述护理措施及方法,有助于临床治疗效果。结论正确的心理护理和皮肤护理,严格的消毒隔离,指导患者合理、规则用药可提高临床治疗效果,有助于患者早日康复,减少复发。     

A rare presentation of splenic rupture.

Traumatic rupture of the spleen is a well known problem, complicating many types of abdominal injury. In contrast, a spontaneous rupture is a rare occurrence, mainly associated with a large spleen resulting from infections, the most common in this country being infectious mononucleosis. It is also a rare complication of leukaemias and lymphomas and the case presented here demonstrates a peculiar diagnostic problem.     

Trauma-related oral lesions; Angina bullosa haemorrhagica: a rare case presentation

Angina bullosa haemorrhgica is a relatively uncommon condition characterized by blood-filled subepithelial lesions in the oral mucosa that is idiopathic and not caused by a systemic disease or a hemostatic abnormality. Middle-aged and elderly patients are usually affected and lesions heal spontaneously without scarring. A rapidly expanding hemorrhagic blister in the oropharynx can induce upper airway obstruction, so recognizing the lesion as soon as possible is essential. Because of its rarity, we wanted to highlight a 42-year-old male patient who presented with hemorrhagic bullae associated with insignificant local trauma in the oral mucosa and to emphasize that Angina bullosa haemorrhagica is a rare but recognizable lesion that clinicians should be aware of.     

Pseudomyxoma peritonei: a rare presentation as an umbilical nodule

Background  Umbilical nodules are rare and may be the sole presenting sign of an advanced intra-abdominal malignancy associated with poor prognosis. Metastatic involvement of umbilicus was first described in 1846 by Sister Mary Joseph, who established the correlation between carcinomas and umbilical nodes. Aim  We aim to describe a case report of Pseudo Myxoma Peritonei in a 31-year-old man who presented with subcutaneous non tender umbilical nodule. We also aim to emphasize the importance of differential diagnosis, when one encounters the presence of mucoid material in unusual sites such as the umbilicus. Method  Patient’s clinical notes, operative findings, imaging studies, laboratory investigations including histopathology and immunohistochemistry were reviewed. A literature search was done to look into the incidence, pathophysiology, presentation and treatment options for this neoplastic process. Conclusion  PMP should be considered as one of the differential diagnosis when one encounters mucinous collection in unusual sites such as the umbilicus.     

Sheehan's syndrome presenting as psychosis: a rare clinical presentation

Sheehan''s syndrome (SS) refers to the occurrence of varying degree of hypopituitarism after parturition (1). It is a rare cause of hypopituitarism in developed countries owing to advances in obstetric care and its frequency is decreasing worldwide. However, it is still frequent in underdeveloped and developing countries. Sheehan''s syndrome is often diagnosed late as it evolves slowly (2,3). Reports of psychoses in patients with Sheehan''s syndrome are rare. Herein, a case report of psychosis in a 31 year old woman who developed Sheehan''s syndrome preceded by postpartum haemorrhage is presented. Treatment with thyroxine and glucocorticoids resulted in complete remission after attaining euthyroid and eucortisolemic state.

背景:大剂量静脉滴注免疫球蛋白(IV Ig)已成为寻常型天疱疮(PV)疗法的一部分。一些人将IV Ig作为减少类固醇使用的辅助制剂,而另外一些人将其作为能够单一使用的缓解病情的疗法。方法:作者介绍了将IV Ig作为辅助方法治疗12例重度PV患者的经验。结果:12例患者中有10例(83%)对6次

寻常型天疱疮(PV)是一类危及生命的自身免疫性大疱性皮肤黏膜病,以往认为PV的发生与体液免疫有关,目前认为,除了B细胞的功能异常产生致病性抗体外,T细胞所介导的细胞免疫异常介导了PV患者出现病理性免疫应答.文章介绍了与PV发病相关的HLA-Ⅱ类抗原,Dsg3特异性的自身反应性T细胞,以及调节性T细胞的最近研究发现.

Hemoptysis. A rare presentation of a vallecular cyst

Hemoptysis is a frightening experience for both patient and clinician and warrants prompt evaluation and diagnosis. We report a case of a vallecular cyst (VC) in a 30-year-old gentleman presenting with hemoptysis. Carbon dioxide laser dissection of the VC after needle decompression was carried out with no complications or recurrence in a 2 months follow-up. In such rare lesions, the authors tend to have conflicting opinions in their classification, evaluation, and management. In this report, these opinions are reviewed and discussed.
