Background

Birmingham hip resurfacing (BHR) has proven to be a good alternative for younger patients with osteoarthritis. Some have asserted that BHR may yield outcomes which are superior to total hip arthroplasty (THA), and that some studies which failed to show a difference were plagued by ceiling effects and lack of sensitivity of outcome measures. The purpose of this study is to compare outcomes of BHR and THA using the “Forgotten Joint” Score-12 (FJS), a more sensitive score with lesser vulnerability to the ceiling effect.

Methods

Patients who underwent BHR were matched to patients who underwent posterior THA by computing a propensity score using 5 covariates: age, body mass index, gender, worker's compensation claims, and previous hip surgery. Surgical outcomes were assessed using 6 patient-reported outcome measures, including the FJS, the visual analog scale for pain, and patient satisfaction.

Results

There were 42 patients in the BHR group and 18 patients in the THA group. The FJS was 78.0 for the BHR group and 76.0 for the THA group. The Veterans RAND 12-Item Health Survey Mental Component Summary and Short Form 12 Mental Component Summary were significantly higher in the BHR group. No differences were seen between all other patient-reported outcomes.

Conclusion

BHR offers excellent results in young patients that are comparable to THA. As no clinical difference could be shown between BHR and THA, even with the use of the FJS, the choice between BHR and THA should not be based solely on any expectation that either yields superior clinical outcomes compared to the other at short-term follow-up.  相似文献

BackgroundCurrent literature suggests that 8%-35% of patients undergoing total hip arthroplasty (THA) undergo a subsequent contralateral THA. This study aims to determine if functional outcomes after primary THA predict outcomes in the subsequent primary THA of the contralateral side.MethodsA retrospective cohort of patients undergoing staged bilateral primary THA was reviewed. The Oxford Hip Score (OHS) was utilized as the functional outcome measurement tool and was assessed preoperatively and at one year postoperatively. The minimal clinically important difference (MCID) was assessed. Based on the first-side THA one-year outcomes, the odds of maintaining an MCID, or not, for the second-side THA were determined.ResultsThe study cohort consisted of 551 patients and 1102 primary THAs. The average postoperative OHSs were similar after the first and second THA. Patients achieving the MCID with the first-side surgery were 2.6 times (95% confidence interval 1.0 to 6.64, P = .04) more likely to achieve the MCID for the second-side surgery than patients failing to reach the MCID for their first-side surgery. After the first THA, 29 (5.3%) patients failed to reach the predefined MCID for the OHS compared with 54 (9.8%) patients undergoing their second THA (odds ratio: 1.96 [95% confidence interval: 1.23 to 3.1], χ2 = 8.14, P = .005).ConclusionsFunctional outcomes after the first THA are predictive of functional outcomes of the second THA. Patients are more likely to achieve a clinically significant improvement after their first THA related to higher preoperative OHSs before the second THA.  相似文献

Background

Hip resurfacing is an alternative to total hip arthroplasty.

Questions/Purpose

We aimed to compare an experienced hip surgeon’s initial clinical results of hip resurfacing with a new cementless total hip arthroplasty (THA).

Methods

The first 55 consecutive hip resurfacing arthroplasties were compared to 100 consecutive cementless THAs using a cylindrical tapered femoral stem. The learning curve between the two procedures was compared utilizing the incidence of reoperation, complications, Harris Hip Scores (HHS), and implant survivorship.

Results

The reoperation rate was significantly higher (p = 0.019) for hip resurfacing (14.5%) versus THA (4%). The overall complication rate between the two groups was not significantly different (p = 0.398). Preoperative HHS were similar between the two groups (p = 0.2). The final mean HHS was similar in both the resurfacing and THA groups (96 vs. 98.3, respectively, p < 0.65). Kaplan–Meier survival analysis with an endpoint of reoperation suggests complications occurred earlier in the resurfacing group versus the THA group (log-rank test, p = 0.007).

Conclusions

In comparison to our initial experience with a cementless THA stem, operative complications occur earlier and more often after hip resurfacing during the learning period. The clinical outcomes in both groups however are similar at 5 year follow-up.

Background

Pseudotumors are a common finding in metal-on-metal (MoM) total hip arthroplasty (THA) and resurfacing hip arthroplasty (RHA). However, information on pseudotumors in metal-on-polyethylene (MoP) THA is limited.

Methods

One hundred eleven patients with 148 hip articulations—30 MoM THA, 47 MoM RHA, and 71 MoP THA—participated in a cross-sectional study at mean 7.1 (range: 0.2-21.5) years postoperatively. Patients were evaluated with metal artifact reducing sequence magnetic resonance imaging, measurements of metal ions, clinical scores of Harris Hip Score, Oxford Hip Score, the Copenhagen Hip and Groin Outcome Score, and conventional radiographs.

Results

Pseudotumors were present in 13 of 30 (43%) MoM THA, 13 of 47 (28%) MoM RHA, and 29 of 71 (41%) MoP THA patients, which was a similar prevalence (P = .10). The prevalence of mixed or solid pseudotumors was significantly higher in patients with MoP THA (n = 10) compared to MoM THA (n = 3) and MoM THA (n = 0), (P = .01). Hips with a mixed or solid pseudotumor had significantly poorer scores of Harris Hip Score (P = .01) and OHS (P = .002) and higher metal ion levels of cobalt (P = .0009) compared to hips without a pseudotumor or with a cystic pseudotumor.

Conclusion

Pseudotumors have primarily been associated with MoM hip articulations, but we found a similar pseudotumor prevalence in MoP THA, which is the most common bearing worldwide. Mixed or solid pseudotumors were more often seen in MoP THA compared with MoM hip articulations, and patients with a mixed or solid pseudotumor had poorer clinical scores and higher metal ion levels than patients without a pseudotumor or with a cystic pseudotumor.  相似文献

This systematic review compared 2 treatments for hip disease in active young patients: modern metal-on-metal total hip resurfacing and standard total hip arthroplasty. We conducted a literature search to identify relevant randomized and clinical controlled trials and included 968 patients from 4 trials in our analysis. Our results indicated increased rates of revision, femoral neck fractures, and component loosening among patients who received modern metal-on-metal hip resurfacing. No significant differences in the rates of mortality, dislocation, or deep hip joint infection were found between treatment groups. Hip function scores were similar between the 2 groups, but the resurfacing group showed higher activity levels. These results have provided insufficient evidence to determine whether modern metal-on-metal total hip resurfacing offers clinical advantages over standard total hip arthroplasty.  相似文献

Large head total hip arthroplasty (THA) and hip resurfacing arthroplasty (HRA) are alternatives to standard THA that generally have head sizes larger than 36 mm. This study examined 20 patients (10 large head THA and 10 HRA), at an average of 18 months postoperatively, and 15 healthy control subjects during stair negotiation. Hip kinetic and kinematic variables and ground reaction forces were measured. The THA and HRA groups ascended the stairs with increased peak hip flexion angles and decreased hip extension angles as compared with controls. The operative groups also descended the stairs with decreased hip flexion moments. No differences between the operative groups were observed. Eighteen months postoperatively, patients with large head THA or HRA display abnormal flexion and extension during a physically-demanding task.  相似文献

Newer arthroplasty designs claim to provide superior range of motion (ROM) and greater stability than their predecessors. However, there is no way to compare ROM of implant systems in an equivalent anatomical environment in a clinical setting. This study used computer-aided design to compare ROM after hip resurfacing, 28 mm THA, 36 mm THA, and anatomic dual mobility (ADM) THA in 3D models of 5 cadaver pelvises. ROM to impingement was then tested in 10 different motions and a one-way ANOVA was used to compare results. The hip resurfacing resulted in restricted ROM compared to the other 3 models in all motions except adduction. The ADM, 36 mm, and 28 mm THA resulted in similar ROM. Dual mobility constructs provide comparable ROM in patients where large head THA is not appropriate.  相似文献

Background

Studies regarding postoperative outcomes after primary total hip arthroplasty (THA) in patients who have comorbid factors tend to focus on medical diseases. However, there is a paucity of literature examining the effect of a patient's orthopedic surgical history on outcomes after THA. Significantly, there are currently no studies on the effect of spinal fusion surgery on THA outcomes.

Methods

A review of 82 consecutive patients who had prior spinal fusion surgery who underwent elective THA from January 1, 2006 to December 31, 2015, was conducted. A matching cohort of 82 patients was selected from the remaining THA patients to maintain a 1:1 ratio control group. This cohort of 82 patients was matched for age, gender, body mass index ±5, preoperative Oxford score ±10, total Short Form-36 score ±10, and total Western Ontario and McMaster Universities Arthritis Index (WOMAC) score ±50. Data on the same functional outcomes were prospectively collected at 6-month and 2-year follow-up for comparison.

Results

Patients without spinal fusion had better outcome scores than patients with prior spinal fusion, specifically in their 6-month WOMAC scores (253.33-225.07; P = .046), their 2-year Short Form-36 total scores (79.71-69.21; P = .041), and their 2-year WOMAC scores (213.5-267.41; P = .054).

Conclusion

This study demonstrates that patients with prior spinal fusion had worse outcomes after THA than patients without prior spinal fusion. This has clinical significance in counseling patients with previous spinal fusion undergoing THA.  相似文献

We compared the results of the metal-on-metal hip resurfacing with the ceramic-on-ceramic total hip arthroplasty (THA) in 2 groups. The preoperative and postoperative ranges of motion (ROMs) were recorded. At the latest follow-up, both of the groups make satisfactory clinical and radiographic results. There was no significant difference in Harris hip score of the 2 groups, but the ROM of the hips in hip resurfacing group was significantly wider than THA group (P < .01). Hip resurfacing has better ROM improvement than THA, with the same pain relief. Its high stability and low dislocation rate allow patients to do early function exercises, which is important for ankylosing spondylitis (AS) patients to avoid reankylosis. Hip resurfacing may be a reasonable option for young AS population.  相似文献

Our aim was to investigate the changes in bone mineral density (BMD) of acetabulum and proximal femur after total hip resurfacing arthroplasty. A comparative study was carried out on 51 hips in 48 patients. Group A consisted of 25 patients (26 hips) who had undergone total hip resurfacing and group B consisted of 23 patients (25 hips) who had had large-diameter metal-on-metal total hip arthroplasty (THA). BMDs around the acetabulum and proximal femur were measured using dual-energy x-ray absorptiometry (DEXA) at 2 weeks, 6 months, 1 year and annually thereafter during the 3 years after surgery. At final follow-up, the acetabular net mean BMD decreased by 11% in group A and 10% in group B with no differences between two groups (P = .35). For the femoral side, in Gruen zone 1, the mean BMD increased by 4% in group A, whereas it decreased by 11% in group B (P = .029). In Gruen zone 7, the mean BMD increased by 8% at the final follow-up in group A, whereas it decreased by 13% in group B (P = .02). In both groups the mean BMD increased by 3% in Gruen zones 3, 4, 5, and 6. Stress-related bone loss of the acetabulum was comparable for MOM THA and resurfacing devices, but proximal femoral bone density increased in the resurfacing group and decreased in the THA group.  相似文献

In this in vitro study, 4 different resurfacing implants have been evaluated with a simulated femoral head: 2 types of cement have been used (low and high viscosity) and 2 different cementing techniques have been performed: direct (cement apposition directly on the femoral head) and indirect (cement poured into the femoral component). High-viscosity cement shows a homogeneous distribution all over the femoral head. Low-viscosity cement shows a massive polar concentration with insufficient if not absent distribution in the equatorial zone. Polar cement concentration could be held as a risk factor for early implant failure. It could have 2 effects on femoral head: biologic (excessive local exothermic reaction could cause necrosis of the osteocytes) and biomechanical (it could lead to uneven load distribution on the femoral head).  相似文献

The purpose of this study was to compare the outcomes of total hip arthroplasty (THA) in systemic lupus erythematosus (SLE) compared to a cohort of osteonecrosis patients who did not have this disease. Between 2001 and 2008, 60 THAs in 44 SLE patients who had a mean age of 42 years (range, 18 to 87 years) and a mean follow-up of 7 years (range, 4 to 11 years) were evaluated. These SLE patients were compared to a cohort of 82 THAs in 70 osteonecrosis patients who did not have this disease. Outcomes evaluated included implant survivorship, Harris hip scores, complications, as well as radiographic results. There was no significant difference in implant survivorship (98% vs. 97.5%), Harris hip score (87 vs. 88 points) and complications (1.7% versus 2.4%) between the SLE and the comparison cohort. The authors believe that SLE does not negatively affect the outcomes of primary total hip arthroplasty at mid-term follow-up.  相似文献

BackgroundObesity is a growing public health concern. This study aims to identify the association of body mass index (BMI) on postoperative Forgotten Joint Score-12 (FJS-12) in patients undergoing primary total hip arthroplasty (THA).MethodsWe retrospectively reviewed 2130 patients at a single urban, academic, tertiary institution who underwent primary THA from 2016-2020 with available postoperative FJS-12 scores. Patients were stratified into two groups based on their BMI (kg/m2):<30 (nonobese) and ≥30 (obese). FJS-12 scores were collected postoperatively at 3 months, 1 year, and 2 years. Demographic differences were assessed with chi-square and independent sample t-tests. Mean scores between the groups were compared using multilinear regression analysis, controlling for demographic differences.ResultsOf the 2130 patients included, 1378 were nonobese, and 752 were obese. Although obese patients reported lower FJS-12 scores all time periods, there were no statistical differences between the two groups at 3 months (53.61 vs 49.62;P = .689), 1 year (68.11 vs 62.45; P = .349), and 2 years (73.60 vs 65.58; P = .102). A subanalysis comparing patients who were of normal BMI (<25), overweight (25.0-29.9), and obese (≥30) followed a similar inverse trend in scores but showed no statistical differences at all postoperative time points (3m:P = .612,1y:P = .607,2y:P = .253). Mean improvement in FJS-12 scores from 3 months to 1 year (14.50 vs 12.83; P = .041), 1 year to 2 years (5.49 vs 3.13; P = .004), and from 3 months to 2 years (20.00 vs15.96; P < .001) were significantly greater for nonobese patients compared to obese patients.ConclusionWhile obesity trended toward lower FJS-12 scores, the differences in scores were not statistically significant compared to nonobese patients. BMI did not influence overall FJS-12 scores; however, obese patients achieved a slightly smaller statistical improvement during the first 2 years, though this may not be clinically significant.Level III EvidenceRetrospective Cohort Study.  相似文献

BackgroundPurpose of this study was to determine implant survivorship and resultant outcomes, including modes of failure, for metal-on-metal hip resurfacing through the Hueter anterior approach (HAA).MethodsRetrospective review of cases from 2006 to 2015, resulted in 555 metal-on-metal hip resurfacing via HAA, mean age 49.4 ± 6.9 years and mean BMI 28.1 ± 5.3. Kaplan-Meier curves were used to assess implant survivorship. Evaluation of technique was based on radiographic assessment of component position at 6 weeks. Patient-reported outcome measures were assessed using 12-Item Short Form Survey 12, University of California Los Angeles activity, Western Ontario and McMaster Universities Osteoarthritis Index, and hip disability osteoarthritis outcome scores.ResultsAt a mean follow-up of 9.18 years, survivorship was 95.0% at 5 years (95% CI: 93.2-96.8 years) and 92.5% at 10 years (95% CI: 90.0-95.0 years); men at 96.1% (95% CI: 94.3-97.9) and 93.8% (95% CI: 91.1-96.5), and women at 88.8% (95% CI: 81.9-95.7) and 85.6% (95% CI: 77.6-93.6), 5 and 10 years, respectively (P = .033). There were 37 revisions to total hips (7%) at a mean time of 3.3 years (SD 2.7). Indications for revision were aseptic loosening of acetabular (n = 12) and femoral component (n = 7) and pseudotumor (n = 6). Radiographic parameters were respectable and consistent, median acetabular inclination angle 41.2° and femoral stem shaft angle 137.7°. Patient-reported outcome measure scores significantly improved and remained stable at 2 and 5 years postoperatively.ConclusionAlthough choice of surgical approach should always be based on surgeon’s technical expertise, this study has shown that HAA is safe and effective for hip resurfacing. Mindful attention to long-term metal ion exposure must still be considered.  相似文献

BackgroundAlthough total hip arthroplasty (THA) has been established as a cost-effective intervention, some patients experience ongoing pain and dissatisfaction. There is interest in predicting postoperative outcomes based on preoperative data, but the relative contribution of different preoperative factors is unclear. The study’s aim was to develop multivariable prediction models for the assessment of patient-reported outcomes.MethodRegistry data on 1412 patients undergoing THA for osteoarthritis at two hospitals between 2013 and 2018 was used. Potential predictors included age, sex, body mass index, spoken language, education level, previous THA, American Society of Anaesthesiologists (ASA) score, lower back pain, depression/anxiety, other lower limb arthritis, number of other comorbidities, the preoperative expectation of pain and function, EuroQol Visual Analogue Scale (EQ-VAS) and preoperative OHS. Radiographic scores were also used: joint space narrowing (JSN), osteophytes, sclerosis, and an overall grade based on the Kellgren-Lawrence (KL) classification. Outcomes assessed were the patient-rated improvement, satisfaction, and OHS at six months.ResultsJSN or overall KL scores were the most important predictors (P < .001) for all outcomes, with better radiographic scores associated with worse outcomes. Other predictors associated with poorer outcomes were lower back pain and lower expectation (predicting poor improvement), lower education and higher ASA (predicting lower satisfaction) and younger age, female sex, non-English speakers, lower preoperative EQ-VAS, lower education, back pain, and anxiety/depression (predicting lower OHS).ConclusionPreoperative radiological scores are an important predictor of patient-reported outcomes at six months postoperatively. Understanding the relative strengths and significance of different factors in predicting outcomes will help the clinician and patient decision-making for THA.  相似文献

BackgroundPoor preoperative mental health has been associated with worse outcomes after total hip (THA) and total knee arthroplasty (TKA). To fully understand these relationships, we assessed post-THA and post-TKA improvements in patient-reported mental and joint health by preoperative mental health groups.MethodsElective cases (367 THA, 462 TKA) were subgrouped by low (<25th percentile), middle (25th-74th), and high (≥75th) preoperative mental health, using Veterans RAND 12-Item Health Survey Mental Component Summary (MCS) scores. In each subgroup, we assessed the relationship between preoperative MCS and 1-year postoperative change in mental and joint health. Pairwise comparisons and multivariable regression models were applied for THA and TKA separately.ResultsMedian postoperative mental health change was +14.0 points for the low-MCS THA group, +11.1 low-TKA, +2.0 middle-THA and TKA, −4.0 high-THA, and −4.9 high-TKA (between-group differences P < .001). All MCS groups had improved median joint health scores, without significant between-group differences. Preoperative mental health was negatively associated with mental health improvements in all groups (B = −0.94 - −0.68, P < .001-P = .01) but with improvements in joint health only in the low-THA group (B = −0.74, P = .02). Improvements in mental and joint health were positively associated for low and middle (B = 0.61-0.87, P < .001), but not for high-MCS groups, with this relationship differing for the low versus high group.ConclusionPatients who have low preoperative mental health experienced greater postoperative mental health improvement and similar joint health improvement compared to patients who have high preoperative mental health. Findings can guide subgroup-targeted surgical decision-making and preoperative counseling.  相似文献

We sought to examine the amount of bone removed during total hip arthroplasty with a resurfacing femoral component, compared to with a conventional, stemmed femoral component, by using 6 male and 4 female cadaveric pelves with attached bilateral proximal femora. Using randomized assignment and order, a total hip arthroplasty with a resurfacing femoral implant was performed on one side, and total hip arthroplasty with a cementless, stemmed femoral implant was performed on the contralateral side. The relationship between native femoral head diameter and the implanted acetabular socket was on average within 2 mm for both procedures. No significant difference was observed in the amount of acetabular bone removed (9.8 g for hip resurfacing vs 8.8 g). However, a resurfacing component resulted in approximated 3 × less bone removal from the femur (25.8 g vs 75.1 g). This study shows that the preservation of femoral bone with a resurfacing femoral component does not result in an increased removal of acetabular bone when compared to the use of a conventional, stemmed femoral component.  相似文献

A prospective, multi-center postmarket approval study has been ongoing since May 2006 to assess safety and efficacy of the first US FDA approved hip resurfacing implant. 265 patients have been enrolled at five study sites. The average age of the patients is 51.3 years. There have been 7 revisions (2.4%) in the cohort to date. K–M survival curves for the cohort are 97.6% at 5 years. There is a trend toward a gender difference in implant survivorship, with 98.6% of men and 94.7% of women free from revision. Metal ion analysis revealed median cobalt and chromium levels of 1.5 ppb and 1.7 ppb at 1 year. In this prospective US study, the Birmingham Hip Resurfacing implant is demonstrating results comparable to those in the literature.  相似文献

Resurfacing arthroplasty according to Wagner was used in 40 hips (31 patients) with coxarthrosis or arthritis. The severe, disabling preoperative pain was eliminated in all patients. There was a considerable increase in range of movement, and the functional results were very good. Of the 24 patients previously in employment but disabled by their hip condition, 22 have returned to their former occupations. Complications occurred in only four cases.  相似文献

BackgroundMany studies have analyzed the outcomes of total hip arthroplasty (THA) after failed intertrochanteric fracture fixation, but not after healed fracture. The objective is to investigate the influence of a prior healed intertrochanteric fracture fixation on the outcomes of a subsequent THA for osteoarthritis.MethodsThis is a matched retrospective cohort study of THA between 43 patients who suffered a prior intertrochanteric fracture successfully managed with internal fixation and 43 patients without prior hip fracture. Mean age was 73.6 vs 74.2 years. A conventional cementless THA was used in both groups. Functional outcome was assessed by the Harris hip score (HHS) and reduced Western Ontario and McMaster Universities Osteoarthritis Index questionnaire. Radiological assessment was also performed.ResultsMean follow-up was 6.6 (range, 5-8) years. The mean operative time and blood transfusion rate were significantly higher in the fracture group (P = .001), but there was no significant difference in the length of stay. HHS significantly improved in both groups. At final follow-up, HHS was significantly higher in nonfracture group (P = .008), but the rate of patients with excellent and good outcomes was similar (P = .616). Western Ontario and McMaster Universities Osteoarthritis Index score at the final follow-up was not different between groups (P = .058). Complication rate was similar between groups. There were no revisions, dislocations, or loose implants in the study group.ConclusionCementless THA provided successful functional outcomes and implant durability at medium term in patients treated for osteoarthritis following healed intertrochanteric fracture fixation, comparable to those without prior fracture who underwent primary THA.
